2016 brings new developments, expansion for WellStar By KYLIE McFADDEN WellStar Health System solidified its status as Georgia’s largest healthcare system this year. Between facility purchases from Tenet Healthcare and new construction, WellStar is now one of the country’s largest not-for-profit healthcare systems. WellStar purchased five Tenet Healthcare hospitals and 26 physician clinics for $575 million in April. The five hospitals acquired in the purchase are WellStar Atlanta Medical Center, WellStar Atlanta Medical Center South, WellStar North Fulton Hospital, WellStar Spalding Regional Hospital and WellStar Sylvan Grove Hospital. All are now operated as notfor-profits. In addition to the acquisition of 6 | Medical Answer Book 2016
Tenet Healthcare properties, WellStar partnered with West Georgia Health in LaGrange to form WellStar West Georgia Medical Center. Marietta-based WellStar now operates 11 hospitals and more than 200 medical offices and employs over 20,000 team members, making it the largest healthcare system in Georgia. This year, WellStar has expanded its reach in Georgia beyond the purchase of Tenet Healthcare facilities. In February, WellStar broke ground on a three-story, $77 million outpatient facility in Vinings. WellStar Vinings Health Park is scheduled to open in fall 2017. The facility will provide services including diagnostics and imaging, physical therapy, urgent care, car-
diac and pulmonary rehabilitation, health screenings and community education classes. A new chair and vice chair also joined the WellStar Board of Trustees July 1. Dr. David Hafner, CEO of Vascular Surgical Associates, replaced Greystone Power Corporation CEO Gary Miller as chair of the Board of Trustees. Hafner will serve as chair for two years. Otis Brumby III, president of Times-Journal Inc. was named vice chair, and after serving as vice chair for two years, will succeed Hafner as chair. Putting big chill on chronic pain Emory Johns Creek pioneers non-invasive pain cure By HATCHER HURD Clinical trials by an Emory Johns Creek radiologist have astounding results in eliminating pain at the source – the nerve signal itself. Dr. J. David Prologo is the radiologist who is blazing the trail in what is called cryoablation interventional radiology. Using X-rays to guide a needle-like probe, Prologo can zap nerve endings with a freezing material that will cause the nerve endings to reduce pain signals if not completely at least to more than tolerable levels. Prologo is the director of Interventional Radiology Services at EJCH. He has used to treat phantom limb pain in amputees
and has also had success treating pain in trauma patients and cancer patients – and eliminating the need for risky opioid painkillers. “What we do is a procedure with the image guidance, so that we can reach places in the body no one else can reach,” Prologo said. “Now we are applying the procedure to the pain sources [directly].” The procedure was first applied to phantom limb pain – PLP – for amputees. When a limb is amputated, it necessitates the severing of nerve endings as well and these can be damaged so that these nerve endings can keep firing off pain signals that are perceived as coming from the missing limb. pelvic pain read about the trials and asked Prologo to try the procedure on her. She had spent more than two years of chronic pudendal neuralgia and she wanted to try a new therapy. Prologo said he was reluctant at first, but after consulting with other radiologists agreed to try although it had never been done before. “The pudendal nerve is hypersensitive but it had always been impossible to get to it. With the cryoablation probe we thought we could do that. And we were successful,” Prologo said. “We freeze the pain source. Pa-

tients who could only take opioids before are now free of those with cryoablation,” he said. “Patients with years of pelvic pain are cured with one treatment.” Prologo said he was just in the right place at the right time to begin studying cryoablation and the remediation of pain. “It really was the luckiest thing to happen to me in my career to be the one to get into this,” he said. The doctor first began trials to treat painful cancer lesions that so far been successful in lessening or eliminating the pain. It is one treatment lasting about an hour. Cancer tumors can attach to the
bone and the lesions that attach are extremely painful. “We are not 100 percent sure that pain is permanently relieved but we have patients who are three and four years out now and the pain has not come back,” Prologo said. While it is not cure for cancer, its palliative effects are life-changing for patients. By taking the pain away and taking away the need for opioids, the quality of life soars for these patients. “I mean their other option is to be zonked out on narcotics. With say abdominal cancer, we can freeze the abdominal nerve and
their pain is gone,” he said. “The nerve signals are not dead but they are arrested. Clinically for now we can only say pain is absent for two to three years.” Initially, Prologo had trouble getting permission to use the procedure to treat cancer pain. But his father-in-law came down with kidney cancer, so he got permission to treat him. “He agreed to it, and he did awesome. He was able to get up and do things again. It was really something to watch him play catch with his grandson again,” Prologo said. After that first case, the “floodgates opened,” he said. Forsyth ranked healthiest county in Georgia By KATHLEEN STURGEON The overall health of Forsyth County residents is looking pretty good. According to the annual County Health Rankings & Roadmaps, Forsyth takes the first-place spot for the healthiest county in the state. The rankings were compiled using a variety of data sources and include categories such as overall health outcomes, length and quality of life, health behaviors — such as smoking, obesity and physical activity — and physical environment. Some other metro Atlanta counties ranked in the top 25 (out of 159), including Gwinnett in fifth-

place, Cherokee in sixth, Cobb seventh, Dawson 13th, Hall 14th and Fulton in the 25th spot. Forsyth came in first for health outcomes in overall rank, and length and quality of life. For health factors, Forsyth came in third for overall rank, but first in health behaviors, seventh for clinical care, second for social and economic factors and 146th for physical environment, which measures air and water quality as well as housing and transit. A more in-depth look at the county finds there are 12 percent of residents who consider themselves in poor or fair health, which is lower than the overall state percentage of 17. As for health behaviors, 13 percent smoke, 26 percent are obese and 19 percent report physical inactivity — all lower than the state percentages. Lynn Jackson, administrator for Northside Hospital-Forsyth, said the health and wellness of the community is a top priority for Northside and they are proud to play a part in keeping Forsyth County the healthiest county in Georgia. However, given the double-digit percentages of Forsyth County residents who smoke, are physically inactive and suffer from obesity, Jackson said, “Those are never good numbers, no matter where we place on the list.” “As health care providers, we are constantly challenged with how to reach patients and encourage
them to make changes that support healthy living,” Jackson said. “We continually evaluate our services to ensure that they meet the growing needs of our community. We offer smoking cessation classes and screenings for cancer, heart disease and stroke at the hospital throughout the year. Our mobile mammography van, corporate health fairs and other community partnerships increase the convenience of health care, allowing us to take our services into the community.” Simple changes can make a diet healthier, Jackson said, and there are many options in Forsyth to be physically active, including various parks and recreation centers, the YMCA and the greenway. “Our health is something we always have to work at. We can’t get comfortable,” she said. “There’s always room for improvement.” Other components were included in the rankings aside from health. For social and economic factors, 90 percent of Forsyth residents have graduated high school with 75 percent having some college education. Forty-eight percent of commuters drive alone, higher than the Georgia percentage of 39. Forsyth’s crime numbers were far better than Georgia’s, however, with 141 violent crimes and 38 injury deaths reported, compared to the state average of 385 violent crimes and 58 injury deaths. Chuck Smith brings training system to players across the country By MILES GARRETT When you play defensive line in the National Football League for eight seasons, you’ll most likely pick up on a few tips on what makes a player great. Chuck Smith, a former All-Pro defensive end who played seven seasons with the Atlanta Falcons and an additional year with the Carolina Panthers, is now passing along his knowledge to athletes and teams across the country. “I am the founder and inventor of the system that I teach,” he said. “We use it all around America and we have been very successful with it at the high school, collegiate and pro level.” Chuck Smith Training Systems is based on a four-step acronym that he calls VGHH, Vision Get off Hands and Hips — a system that helps maximize results from pass rushers in football. Smith often teaches at GATA Training in Duluth, where he calls his home gym. “I teach the art of pass rush,” he said. “We specialize in defensive lineman, outside and inside linebackers and we focus on blitzing and pass rushers. It’s a system I’ve had since 2001.” A native of Athens, Smith has been around the Atlanta area since his retirement from professional football in 2000. He was also a part of the Atlanta Falcons’ sole trip to the Super Bowl in 1998, a year after he was named All-Pro. Forsyth is second mostinsured county in state By KATHLEEN STURGEON Last month, the annual County Health Rankings & Roadmaps study was released, naming Forsyth County the healthiest county in the state. Now, another component of that study is offering a possible explanation: Forsyth has the second-highest number of residents with health insurance in Georgia. Out of roughly 10,214,860 residents in Georgia’s 159 counties, 21 percent are uninsured. In Forsyth County, according to the County Health Rankings & Roadmaps study 86 percent of the population under age 65 have health insurance. Fayette was the only Georgia county with more insured residents. Dave Palmer, information officer for Georgia Department of Public 14 | Medical Answer Book 2016
Health, District 2, said the high insurance rate could be due to higher incomes and education levels in Forsyth. “What we’ve seen from research is that in areas where people have higher incomes and more education, the insurance rates tend to be higher,” Palmer said. “The health status for that county is generally higher because people who have insurance usually go for care earlier because they do have insurance to pay for doctor’s visits. Georgia puts hold on new addiction treatment centers Legislature sends mixed signal as drug-related deaths continue to climb By HATCHER HURD Georgia has a heroin addiction epidemic and North Fulton is in the middle of it. So what does it mean when the state initiates a moratorium on the licensing of new private treatment centers for one year? On the one hand, it seems wrong-handed to stop licensing addiction clinics when the rate of heroin addiction is rising quickly. The Legislature’s bill, signed by Gov. Nathan Deal, puts a one-year moratorium on accepting licensing applications to open a new opioid treatment center. The suspension is through June 30, 2017. Meanwhile, the Legislature is convening a study committee to look at licensure requirements for such clinics and other related issues. The moratorium does not apply to applications currently under review. While it may seem like the General Assembly wants to inventory the lifeboats after the state has hit an iceberg, it may not be as contrary as it first sounds. These treatment centers are springing up around Georgia. There are 67 currently licensed clinics that dispense methadone and Suboxone, two approved drugs used to treat heroin addicts. The problem of course is that these are both addictive opioids themselves. So it is like treating a 16 | Medical Answer Book 2016
rattlesnake bite with anti-venom, which is itself snake venom. The danger is that in dispensing these drugs which do inhibit the worst effects of heroin and OxyContin addiction, they are themselves highly addictive opioids. Legislators want a year to study how best to regulate the state’s addiction clinics lest Georgia find itself in the same “pill mill” situation. That was where “pain clinics” shoveled out prescriptions for OxyContin, Percocet, hydrocodone and similar opioids by the bushel all over the state. That is well and good, but the General Assembly should be prepared to beef up the agency that now regulates the states’ clinics. It has an anemic staff of three to monitor those 67 existing clinics. One Johns Creek addiction clinic, DecisionPoint does not use opioid treatment at all. Mike Reis is the CEO and owner of DecisionPoint and he shrinks from fighting the fire of heroin and pain-killer addiction with the gasoline of Suboxone and methadone. “We’re the only abstinence-based clinic in North Fulton. If (a clinic) treats addiction with opioids, it must use medically managed procedures,” Reis said. “Pills are not the silver bullet to cure addiction. It’s how most of us got started to begin with.”
Reis, who is himself a recovering addict, said 75 percent of all heroin addicts began their addiction using pain medication such as OxyContin or its clones. “The state wants to get a handle on how opioid clinics are run before they are licensed,” Reis said. “Methadone and Suboxone are highly addictive and the withdrawal is worse than heroin.” Clinics that do not medically manage their patients have ruined thousands of lives. Yet thousands of people are alive today because of well-run programs using methadone and Suboxone. “So it is important that those who do use opioids to treat addiction do so properly,” he said. Remco Brommet is the executive director of The Hub Family Resource Center, a Johns Creek nonprofit organization that helps families find the right resources for the entire spectrum of social services including drug addiction, mental health, incarceration and family dysfunction. Brommet agrees that the key to any methadone-type clinic is proper clinical supervision of the patient and the dosages administered. “The risk is to become dependent on the drug without sufficient wraparound programs to treat more than just the addiction,” Brommet said. “There are worlds of associated problems such as anxiety, psychological issues and traumatic experiences.

“From Xanax to OxyContin, 80 percent of addiction begins with self-medication. If all you do is medicate with painkillers, you are only creating another dependency.” Vivatrol acts as an inhibitor that blocks the pleasure sensors from receiving signals. It can take away the physical craving for opioids in a month. But without working on the underlying psychological problems associated with addiction, the addict will relapse. “For many, it is not the high of the opioid the patient is seeking. It is desensitizing oneself to mask emotional pain,” Brommet said. “You’re just changing the bandage
without treating the wound.” If done right, methadone and its clones are part of a multifaceted program to wean addicts off drugs. “I can see the need for the regulation of treatment centers because of the overall tendency of the medical community to overmedicate,” Brommet said. “A moratorium [on new clinics] is OK if it’s just a pause to develop a continuum of care. That is what’s needed most.” Fulton County District Attorney Paul Howard said we are facing a heroin crisis and it is only getting worse. In 2010 there were four opioid-related overdose deaths. In 2014 there were 77. And 18 percent of
opioid deaths occur in North Fulton. It is a mostly white, middleclass addiction in Fulton County. Ninety percent of heroin users begin abusing the drug between the ages of 18 to 25. But that is not where they start. Many drug users in North Fulton turn to heroin after first becoming addicted to painkillers. Howard said 83 percent of those painkillers come from family or friends and most of that is from family medicine cabinets. Once the painkillers run out, heroin is a substitute. “Heroin is easier to get, it’s cheaper and it’s more potent,” Howard said. Medical Answer Book 2016 | 17
Many roads to recovery North Fulton and Forsyth home to addiction treatment centers By KYLIE McFADDEN Across North Fulton and Forsyth counties, addiction treatment centers attempt to help people out of the influence of addiction. These treatment centers deal with drug and alcohol addictions and the mental illnesses associated with them. Carter Treatment Center – Cumming Staff at the Carter Treatment Center use a holistic treatment method designed to treat not only addiction, but incorporate the mind and spirit in the process. ADHD:
What parents should know Attention-deficit hyperactivity disorder is the name of a group of behaviors found in many children and adults. People who have ADHD have trouble paying attention in school, at home or at work. They may be much more active and/or impulsive than what is usual for their age. These behaviors contribute to significant problems in relationships, learning and behavior. For this reason, children who have ADHD are sometimes seen as being “difficult” or as having behavior problems. ADHD is common, affecting 4 to 12 percent of school-age children. It’s more common in boys than in girls. You may be more familiar with the term attention deficit disorder. This disorder was renamed in 1994 by the American Psychiatric Association.
What are the symptoms of ADHD? The child with ADHD who is inattentive will have six or more of the following symptoms: • Has difficulty following instructions • Has difficulty keeping attention on work or play activities at school and at home • Loses things needed for activities at school and at home • Appears not to listen • Doesn’t pay close attention to details • Seems disorganized • Has trouble with tasks that require planning ahead 26 | Medical Answer Book 2016
• Forgets things • Is easily distracted • The child with ADHD who is hyperactive/impulsive will have at least six of the following symptoms: • Fidgety • Runs or climbs inappropriately • Can’t play quietly • Blurts out answers • Interrupts people • Can’t stay in seat • Talks too much • Is always on the go • Has trouble waiting his or her turn
What can I do to help my child? A team effort, with parents, teachers and doctors working together, is the best way to help your child. Children who have ADHD tend to need more structure and clearer expectations. Some children benefit from counseling or from structured therapy. Families may benefit from talking with a specialist in managing ADHD-related behavior and learning problems. Medicine also helps many children.
What medicines are used to treat ADHD? Some of the medicines for ADHD are methylphenidate (some brand names: Ritalin, Concerta), dextroamphetamine (brand name: Dexedrine), pemoline (brand name: Cylert), atomoxetine (Strattera), and a combination drug called Adderall. These medicines improve attention
and concentration, and decrease impulsive and overactive behaviors. Other medicines can also be used to treat ADHD.
What can I do at home to help my child? Children who have ADHD may be difficult to parent. They may have trouble understanding directions. Children who are in a constant state of activity can be challenging for adults. You may need to change your home life a bit to help your child. Here are some things you can do to help: Make a schedule. Set specific times for waking up, eating, playing, doing homework, doing chores, watching TV or playing video games and going to bed. Post the schedule where your child will always see it. Explain any changes to the routine in advance. Make simple house rules. It’s important to explain what will happen when the rules are obeyed and when they are broken. Write down the rules and the results of not following them. Make sure your directions are understood. Get your child’s attention and look directly into his or her eyes. Then tell your child in a clear, calm voice specifically what you want. Keep directions simple and short. Ask your child to repeat the directions back to you. Reward good behavior. Congratulate your child when he or she completes each step of a task. CONTINUED to PAGE 28

ADHD: What parents should know (continued from page 26) Make sure your child is supervised all the time. Because they are impulsive, children who have ADHD may need more adult supervision than other children their age. Watch your child around his or her friends. It’s sometimes hard for children who have ADHD to learn social skills. Reward good play behaviors. Set a homework routine. Pick a regular place for doing homework, away from distractions such as other people, TV and video games. Break homework time into small parts and have breaks. Focus on effort, not grades. Reward your child when he or
she tries to finish school work, not just for good grades. You can give extra rewards for earning better grades. Talk with your child’s teachers. Find out how your child is doing at school--in class, at playtime, at lunchtime. Ask for daily or weekly progress notes from the teacher.
Will my child outgrow ADHD? We used to think children would “grow out” of ADHD. We now know that is not true for most children. Symptoms of ADHD often get better as children grow older and learn to adjust. Hyperactivity usually stops in the late teenage years. But about half of children who have ADHD continue to be easily distracted, have mood swings, hot tempers and are unable to complete tasks.

What should I do if I think my child has ADHD? Talk with your child’s doctor. A diagnosis of ADHD can be made only by getting information about your child’s behavior from several people who know your child. Georgia medical legislation By KYLIE McFADDEN The beginning of July brought on a series of bills and amendments affecting patients and doctors across Georgia. During the 2016 Georgia Legislative Session, which adjourned in March, dozens of bills affecting the medical system of Georgia were signed or vetoed by Gov. Nathan Deal. The governor issued veto statements for 16 bills following the legislative session this year. Deal vetoed HB 216, which was written to expand the eligibility for workers’ compensation benefits to cancer-diagnosed firefighters who could prove a direct link between work-related risk factors and the 30 | Medical Answer Book 2016
disease. Deal stated the bill was a broad solution for a rare problem in the state. “Since I took office, I am unaware of any firefighter that has filed a workers’ compensation claim for a cancer diagnosis,” Deal said in a statement. The state’s current medical marijuana law remained unchanged in the 2016 legislative period as the Georgia Senate rejected HB 722, which presented a regulatory and registry system for medical marijuana oil, as well as expanding the list of disorders eligible for medical marijuana treatments. While medical marijuana is available to some in Georgia, the
waiting list is long and growth and manufacturing of marijuana is illegal within the state. Not all medical legislation in the 2016 session was rejected, however. HB 965, or “The Honorable Jimmy Carter Cancer Treatment Access Act” will prevent insurance companies from limiting drug coverage for Stage 4 cancer patients. The bill came about as a result of former President Jimmy Carters’ successful brain tumor treatment, and it will only apply for patients with advanced metastatic cancers. This bill carries the same theme as another act passed in this legislative session, the “Georgia Right to Try” act, which provides

According to the National Institutes of Health, the basic standards for qualifying for weight loss surgery are: • For men at least 100 pounds overweight and women at least 80 pounds overweight, with a BMI of 40 or more. • The presence of health problems related to obesity, such as diabetes, high blood pressure, sleep apnea or other conditions. • An ability to participate in treatment and long-term follow-up. • An understanding of the operation and the lifestyle changes you will need to make.
What are the options for bariatric surgery? Gastric Bypass Roux-en-Y The surgeon divides the stomach in two parts; the large portion of the stomach is closed off with staples to create a smaller pouch. The smaller stomach, your new pouch, restricts the amount of food you can eat at one time. The small intestine is cut below the duodenum and reattached to the new stomach pouch, leaving a shortened path for food to travel through so less food is absorbed. Laparoscopic adjustable gastric banding An adjustable gastric band is placed around the top part of the stomach dividing it into two parts: a small upper pouch and a lower stomach. The size of the band is adjusted using a port placed under your skin. Adjusting the band changes how much you can eat and how quickly food leaves the new pouch so

To mentally prepare yourself: • Understand the surgical process and what to expect afterward. • Talk to people who have had weight loss surgery. • Write a letter to yourself and your surgeon explaining your reasons for having the surgery and outlining your plans to maintain your weight loss after surgery. • Start a journal about your experience. Record how you feel now, the obstacles you encounter, the things you hope to be able to do after surgery. • Get support from friends and family. It helps to know you have people behind you.
What is life like after bariatric surgery? The modifications made to the gastrointestinal tract will require permanent changes in eating habits that must be followed for the rest of one’s life to keep the weight from returning. According to statistics, about 80 percent of patients have long-term success following bariatric surgery. It is important to note that every surgeon does not perform the exact same weight loss surgery procedure and that the dietary guidelines will be different for each surgeon and each type of procedure. Each of his four daughters have also worked at the business. They often help Jordan with his PowerPoint presentations of before-andafter photos of different types of treatments. This is a new way for patients
to get the full picture of how braces can improve their smiles. New and potential patients can come into the office and Jordan will do an analysis of the smile to see what the person might require. From there, they can decide to continue with treatment. “For every type of treatment, we can show somebody 25 beforeand-after examples of something that they pretty much exactly have,” Jordan said. “It helps them visualize it.”
Another technological advance are heat-activated memory wires. These warm up in the patient’s mouth and activate to make the treatment more comfortable and more efficient. “The teeth move a little bit every day or every hour,” he said. “In the old days, the patient’s wires were tightened and the teeth healed for a month. Now, it doesn’t really move when they’re in the chair. But when they leave, it starts warming up and the teeth move a
little bit every day. It’s below the threshold of pain.” No matter which method the patient takes in updating their smile, Jordan’s favorite experience is witnessing their first smile post-braces. “They go check it out in the mirror and look at themselves,” he said. “It’s so cool to watch them even though they don’t know we’re watching. The list of sports injuries can go on and on. Question: If you have a sports injury, what can you do about it? Answer: The best thing you can do immediately is R.I.C.E. Question: What are common shoulder injuries? Answer: There are many activities such as tennis and heavy gardening that Angelo DiFelice can damage the rotator cuff. The rotator cuff tendon is vulnerable to injury in individuals over the age of 40 years due to diminished elasticity of the tendon. Minor trauma such as a ground level fall or lifting a heavy box can damage the shoulder. Repetitive muscle contractions from overhead activity can lead to inflammation and micro-tears of the tendon which can ultimately cause significant structural damage. The initial treatment is to relieve the inflammation and irritation of the rotator cuff tendons and bursa. Rest takes the form of activity modification which means stopping the offending activity such as sports. As the pain subsides with everyday activity, then one must
begin a strengthening program to rehabilitate the rotator cuff. These exercises are performed below shoulder level usually with rubber tubing. Once strength has returned and pain has resolved with normal daily activities, then you can resume sporting activities. There needs to be a gradual resumption of sports to prevent recurrence of symptoms due to the weakness that has occurred. -Angelo DiFelice, M.D. Question: What are common knee injuries? There are many proactive ways to prevent knee problems. Maintaining normal body weight decreases stress across the knee and decreases risk of injury. Maintaining a strength training program that focuses on quadriceps and hamstring strengthening also protects the knee. Knee braces during athletic activities also act like an added “insurance policy” against injury. Question: Why do I need to get screened for colon cancer? Answer: We know having a colonoscopy isn’t something people get excited about. But preventing colon cancer before it starts should be. Colon cancer is ranked as the second leading cause of cancer death in the U.S., and one in 20 people will develop it in their lifetime. But, when detected in its early stages during a colonoscopy, colon cancer is one of the most highly treatable and preventable cancers. In fact, research has shown that 80 percent of colon

Answer: Because the risk of getting the disease increases with age, screening colonoscopies are recommended for every adult beginning at age 50 and at age 45 if you are African-American.* For those with a family history of the disease, or other risk factors, screening could start even earlier. While there is evidence that more people are getting screened than in previous years, one in three adults ages 50 or older are not being tested.
Question: What is a colonoscopy? Answer: A colonoscopy is considered the single best method for detecting and preventing colon cancer. During the procedure, a trained gastroenterologist will check for any signs of cancer and remove existing polyps that may turn into cancer over time. Cancer treatment for the 21st century By Jonathan Stegall, MD The Center for Advanced Medicine Question: How is cancer treatment different today than it has been in the past? Answer: The field of oncology is changing rapidly. We now have more tools in our tool chest than ever before, ranging from chemotherapy and radiation to nutrition and supplementation. The art of cancer treatment today is to choose which treatments are best for each patient on a case-by-case basis, in order to maximize treatment effectiveness and minimize side effects while enhancing overall health. This field is known as integrative oncology. Question: How do you choose which cancer treatment(s) to recommend for each patient? Answer: I view each cancer case like a fingerprint – no two are exactly alike. Each patient’s

internal environment is a unique culmination of his or her past medical history, lifetime environmental exposures, and stressors. For this reason, I perform advanced lab testing on each patient, which guides me as to which specific treatments have the best chance of being successful. This is contrary to the one-size-fits-all approach many doctors today are still using. Question: What kind of advanced lab testing do you perform? Answer: I send a sample of each patient’s blood to an innovative lab in Greece which provides a genetic blueprint for each patient’s cancer cells. This blueprint not only measures the presence and activity of a variety of cancer genes, but it also tells us which treatments have been shown on lab testing to be most effective against that patient’s cancer cells. This allows me to recommend the chemotherapy agents and nutri-

Cancer patient spreads awareness of platelet donations By MILES GARRETT Fewer than 5 percent of the U.S. population who are eligible to donate platelets to cancer patients actually end up donating. Rich Colvin, along with a few of his friends, are working to reverse this statistic in spreading awareness of donating platelets to local clinics around the country. Colvin was diagnosed with T-cell, lymphoblastic non-Hodgkin’s leukemia in late April. For this particular type of cancer, blood transfusions and platelets are required for patients after chemotherapy is concluded. Many people are unaware of what platelets are, and specifically how platelet donations help those who are in need of them. “Platelets are the portion of your blood that helps your blood clot,” said Nancy Herring, donor administrator at Atlanta Blood Services. “Chemotherapy attacks the bone marrow’s ability to produce enough healthy cells.” A low platelet count can cause uncontrollable bleeding for those who sustain it, and donations are often vital to helping combat it and help the blood clot. Colvin and his friends have been responsible for helping spread awareness on this issue. Social media has been a medium that he has used frequently to consistently remind people of the issue. Getting people to local clinics has been the focus of the campaign. “I have a blog that I’ve been 66 | Medical Answer Book 2016
keeping up with to keep the word out,” he said. “I’ve been encouraging everyone I can to get to their local clinics.” When asked why many people don’t typically donate, Colvin said the process can tax the body of donors. The process can often run close to five hours. Other than that, many people are simply unaware of the issue itself. One friend, in particular, Joey Kurek, has been a frequent donor at the clinic and has helped Colvin get more people involved with donations via social media. “I usually talk to my friends and I use Facebook to share it,” Kurek said. “I still go to the clinic; I don’t mind giving up a little bit of time for this.” The response on social media around the country has been astounding, Colvin said. He has interacted with many people who have decided to go to local clinics and donate platelets to those who need it. Colvin said he recently spoke to people as far away as Las Vegas and Maryland who have donated as a response to his call to action. “He was one of those amazing patients,” Herring said. “He wrapped his arms around it and started educating people about it on social media. He took this on as a personal mission.” Kurek’s donation even helped a woman at the clinic who responded to his transfusion. As a result, doctors were able to get her platelet count to a sustainable level again. The woman’s response to

Kurek’s platelets proved to be vital in advancing her treatment for the future. “By doing that and getting her platelets, we were able to get her counts up high enough and her body was able to make its own platelets,” Herring said. “It’s a really amazing thing. It was key to her treatment.” As Colvin continues his own treatment, he says that the process has been long and trying but one that has been going according to plan. He said he hopes to continue spreading awareness on platelet donations and has faith that his, and Kurek’s story helps more people around the country. “I’m very blessed to have a group of friends do this,” Colvin said. “Treatment has been going well for me, everything is working as expected.” For information on platelet donations, contact Nancy Herring at nherring@bmtga.com.

Timeline of: MEDICAL APPOINTMENTS Newborn: Within just days of birth and discharge from a hospital, a child should visit a pediatrician. This type of physician specializes in the care of children, whose bodies are different from adults. A pediatrician will tell you if a newborn has any medical problems that cause concern, what their initial weight is and what you can expect for their growth, development and eating habits. Toddler Years: During this time period, a child often sees a doctor more frequently than not, simply because of their developing immune systems and the exposure to childhood germs. However, in the absence of germs or injuries, it is a good idea to have a well visit once per year until the age of 10. Doctors will usually recommend the standard continued course of immunizations, including the chicken pox vaccine, which is not given until the child is at least 1 year old. 5 Years Old: A child that is ready for Kindergarten will need to go to the pediatrician to be screened and immunized, in most cases. The screenings consist of hearing, vision and sometimes blood pressure or cholesterol tests. Also, the doctor’s office can give you the necessary forms for enrollment in a public school. Not crazy about vaccines, but still want to enroll your child in school? Schools now accept waivers (available at your school office) that allow parents who do not immunize to request enrollment for their child in school without having such vaccinations. Early Childhood: From the ages of 6 to 10, annual well visits to 68 | Medical Answer Book 2016
the pediatrician are sufficient. 10 Years Old: Children usually go for another round of vaccinations. 11-18 Years Old: Keep in mind that at any point in the teen years when a child is participating in sports, most schools will require the student to undergo a routine physical to make certain that the student is healthy enough to participate in athletics. 18 Years Old: Many colleges require a physical examination upon acceptance to their school, and the paperwork involved with those tests may be required in order to attend classes. Along with proof of a physical exam, colleges will want to verify that a college-aged student has had all the required childhood immunizations. Young women at this age may also begin to schedule visits to a gynecologist. Women should begin breast self-examinations as well. At this stage of early adulthood until senior care, most adults will begin to fill out questionnaires that detail their health history. Questionnaires help physicians to pinpoint risk factors, and evaluate family history issues and other general health concerns. 20s to 40s: Adults should undergo cholesterol and blood pressure checks every 5 years, along with Tetanus and diphtheria boosters every 10 years. There are really no other recommended tests, unless a patient has an increased risk factor or family history of a specific disease or disorder. 40s for Men: The American Urological Association recommends a first PSA (prostate specific
antigen) test at the age of 40 for most men. In a man’s mid-40s, doctors recommend a screen for Type-2 Diabetes as well. 40s for Women: Guidelines for mammograms change often, but as of now, doctors recommend a mammogram every year or two for women who are 40 and older. Beginning at age 45, doctors also recommend a cholesterol check for women and a blood pressure check every other year. A bone density scan may be performed if the patient is concerned about their specific risk of osteoporosis. 50s for Men: By the age of 55, doctors begin to recommend testing for glaucoma, an eye disease that damages the optic nerve. Both men and women at this stage will most likely be checked for thyroid imbalances, eyesight deficiencies and blood sugar issues. 50s for Women: Once women reach the age of 50, doctors recommend that in addition to the battery of screens used in their 40s, women also begin colon cancer screenings. 60s: Doctors are beginning to recommend the Shingles vaccine around the age of 60 for both men and women. This is a once-in-alifetime shot that provides protection against the emergence of Shingles. Hearing evaluations are also important at this age, since nearly 33 percent of the population age 65 and older experiences hearing loss. 65 and Older – Men: Men are now encouraged to undergo testing for abdominal aortic aneurysms. This screening only occurs once, and is typically done after the age
