Welcome to Spring! I am sure that everyone is happy that our winter is finally over. I have been a resident of Delaware since 1986, and I believe this has been one of the worst that I have experienced. I wrote this last year, and I have to say I was wrong, I think 2014-2015 beat last year. This issue we did our Business People in Profile with a focus on Home and Garden. I am sure that many of our readers already know these great business people if not personally at least professionally. All of these are companies that I personally would highly recommend. Jeffrey Minkovitz, MD is an ophthalmologist with Eye Physicians and Surgeons in Wilmington, Delaware. Dr. Minkovitz specializes in cataract and refractive surgery, and is one of only a few Delaware surgeons specially trained in corneal transplantation. He completed a cornea and refractive surgery fellowship at the renowned Wilmer Eye Institute of Johns Hopkins. Previously, he graduated magna cum-laude from Harvard, earned his medical degree from the University of Massachusetts, completed internship at Harvard’s Mt. Auburn Hospital, and served his ophthalmology residency at Washington University. Dr. Minkovitz participated in the early research on Excimer Laser refractive surgery, and has offered this procedure to Delawareans since FDA approval in 1996, before Delaware had its own laser. These early pioneering individuals travelled to Wilmer Institute with Dr. Minkovitz, where he maintained his teaching and clinical appointment. In addition to introducing laser vision correction to Delaware, he also introduced NearVision CK to eliminate reading glasses, DSAEK (a newer, less invasive type of corneal transplant) and most recently the newest advance: laser cataract surgery. Dr. Minkovitz is a co-founder of the Center for Advanced Surgical Arts and serves on its glasses. He was also named Top Doc in ophthalmology by Delaware Today Magazine six consecutive years, and has been an invited instructor at American Academy of Ophthalmology Annual Meetings.

The Value Of A Smile W
e live in a culture where youth and beauty are celebrated. Case One Only bonding was needed to achieve a beautiful result. No tooth structure was removed, no anesthesia required. This young lady had the procedure completed in just one visit.
Case Two This is a middle-aged gentleman who did not wish to have orthodontic tooth movement. Since health and stability were not compromised by the non-orthodontic option, we elected to place crowns and veneers to achieve a more pleasing smile. Treatment was completed in less than one month.

our routine dental exam includes more than looking at teeth and gums. The dentist also checks the mouth, face, throat, and neck glands for signs of cancer. A routine dental x-ray may reveal a bony cancerous lesion. Many times the x-ray is the first and only sign of the disease in its early stages. According to the National Cancer Institute, oral and throat cancer affects an estimated 43,250 Americans each year. Of those an estimated 8,000 will die of the disease. There is about only about a 50% five year survival rate of those diagnosed with this type of cancer largely because it is too often first detected in advanced stages. As with all cancers early detection and treatment are key to a successful outcome. Risk factors include male sex (twice the incidence of female for cancer of mouth and throat), tobacco use, both smoked and chewed, and alcohol consumption. The risk is greatly increased when there are multiple risk factors. Ultraviolet light is a risk for cancers of the lips and face. The above risk factors are well known but there is a growing risk that is seldom discussed, infection with the Human Papilloma Virus, (HPV). HPV, usually associated with genital warts and cervical cancer, seems to play a significant role in the development of mouth and throat cancers. At this writing there is no clear agreement in the medical community if HPV causes mouth and throat cancers but there is clearly a very strong www.TheWomensJournal.com

Of the many species of HPV virus, it is the HPV16 which seems to play a significant role in mouth and throat cancer. Since this HPV16 is targeted by the HPV vaccine it is possible that the vaccine may help prevent these head and neck cancers as well.

Prevention of mouth and neck cancers is of course most desired. Refrain from tobacco use, both smoked and chewed. Limit alcohol consumption. Wear sunscreens with high SPF values and protective clothing. Practice safer sex ,even when engaging in oral sex. Consider immunization with the HPV vaccine.

After reducing one’s risk of head and neck cancer, early detection and treatment is essential for a favorable outcome. Be aware of the signs and symptoms of these cancers. White patches which do not rub off (leukoplakia), red erosive lesions (erythroplakia), lumps or thickening of oral soft tissues. Difficulty in chewing, swallowing, or moving the jaw, sore throat, a feeling that something is caught in the throat, hoarseness or change in the voice as well as loose teeth could be indicative of a cancer. Unfortunately cancer symptoms are very similar to several other common and less serious conditions. Succinctly, here is potentially lifesaving advice: If any ulcer, lump, sore lasts more than TWO WEEKS see a dentist or medical doctor. If you have any of the signs and symptoms discussed above see your dentist or medical doctor! See your dentist at least twice a year and your medical doctor at least annually so that a professional exam made be 7

Save Lives In Your Community: Donate Blood! problem posed crucial problems; “scrambling for donors” meant gambling with lives. Discussions with other community leaders led to the creation of what would become the Blood Bank of Delmarva (BBD). By Ellen Jones Marketing & Community Relations Specialist Blood Bank of Delmarva
R
ecently, a bleeding patient rushed to a nearby hospital received a diagnosis of esophageal varices, a life-threatening condition. The patient, needing blood and blood components immediately as part of the emergency treatment to stop the heavy bleeding, was given multiple units of blood, plasma and platelets. Because the blood was readily available, the patient’s condition stabilized. But, blood and blood products were not always so readily available to hospitals in the Delmarva region working to save the lives of their patients. In 1954, a prominent Wilmington businessman’s wife needed 26 blood transfusions during and after surgery to keep her alive. She had two options at the time: pay the $1,000 bill for the blood or provide two volunteer donors to replace each pint of blood used. Her husband secured the 52 donors, but he pressed for a better system. The blood supply
2014 school year, culminating in 8,227 donations from young donors. “Roughly 65% of the population cannot donate blood for one reason or another,” said Michael Waite, BBD Director of Marketing & Community Relations. “Of the 35% that can, less than 10% actually do. With an aging population becoming increasingly unable to donate with each passing year, recruiting young donors is extremely important so that we continue to keep blood on the shelf when it’s needed in the future.”
For more than 60 years, BBD has provided much-needed blood and blood components to area hospitals, helping to avoid blood shortages and saving thousands of lives with assistance from more than 150,000 current donor members. What began as a scheduling agency in a small business office has grown into a best in class blood bank handling not only donor scheduling but also collection, testing and distribution of blood and blood products to hospitals on the Delmarva Peninsula and beyond.
As an added benefit to giving blood, donors receive a mini health screening with each donation. BBD checks and records blood pressure and hemoglobin levels, and offers a glucose screening so donors can check their blood sugar level and seek appropriate treatment if needed.
More than 20,000 patients a year receive transfusions at hospitals BBD serves, from accident victims to cancer patients, surgical patients to premature babies, and more.

Phones & Hearing Aids: No More Static! •

M
odern hearing aids have become highly sophisticated in recent years offering hearing aid users many high tech features that include superior noise reduction, information sharing between a set of binaural hearing aids, and automatic program change in response to the user’s changing sound environment to name a few. So it seems difficult to believe that many hearing aid users report difficulty hearing well on the phone. In his survey of hearing aid users published in 2005, Sergei Kochkin found that hearing aid users reported dissatisfaction with their ability to hear on the phone comparable to their difficulty hearing in large groups. In a more recent study published by the American Journal of Audiology in 2009, the authors reported that over 70% of experienced hearing aid users performed poorly when using the phone. Why the difficulty with phone conversation in light of many high tech hearing aid advancements? For those with at least a moderate degree of hearing loss combined with a significant speech recognition loss, phone conversation presents special challenges: •
Absence of Visual Cues: Those with greater degrees of hearing loss are usually more dependent upon seeing the person they are talking with to fill in some gaps in conversation. They must rely on audition alone using the phone.

Background Noise: Those of us with normal hearing know the challenge of holding a telephone conversation in a noisy or busy environment. Have you ever closed your non-telephone ear because your environment was too loud? Now imagine that you have a bilateral hearing loss with hearing aids that amplify not just the phone but the environment. An impaired auditory system has difficulty handling this scenario without access to visual cues. Acoustic Feedback: This is that squealing sound a hearing aid can make when covering the microphone with your hand or another object. While modern hearing aid technology has virtually eliminated random acoustic feedback, placing a telephone handset next to the microphone of a hearing aid may cause some whistling. Even when the whistling is momentary this is irritating to the hearing aid user as well as their communication partner on the other end of the phone.
Wireless Streaming and Hearing Aids: Modern hearing aids now have the ability to stream audio signals directly from electronic devices, including cell phones. Wireless streaming eliminates background noise, has no potential for acoustic feedback and gives a binaural hearing aid user the advantage of hearing phone conversation in both hearing aids. Placement of the phone is not an issue as the phone does not need to be close to the hearing aids. Research has shown that hearing aid users perform best with wireless streaming. Additionally, smartphones offer hearing aid users the ability to obtain visual cues with use of applications like Skype and FaceTime (iPhone).

Positioning the Phone: The loudness and clarity of the phone signal can vary significantly with very small changes in the position of the telephone handset relative to the hearing aid microphone. While experienced hearing aid users may be able to quickly position the phone for maximum audibility, maintaining this position becomes difficult for long phone conversations.
Telephone Solutions: Telecoils and Hearing Aids: A telecoil is a small magnetic induction coil that is in some hearing aids and also in most landline telephones. Use of the telecoil or T mode of the hearing aid effectively eliminates potential for acoustic feedback and also eliminates amplification of all noise in the environment as the hearing aid amplifies only the phone signal in the T mode. Telecoils are inexpensive and many behind the ear hearing aids come already equipped with this device. Placement of the phone on the hearing aid must be practiced using the telecoil mode. 10

Why You Need To Choose Your MRI Provider improve care and actually save money.

am constantly frustrated when doctors tell me they send their patients to inferior facilities just because “it’s easier”. After 8 years of offering the finest quality 3T MRI in the state, doctors and their staff still seem unaware of how much better 3T MRI is. Ask your doctor one simple question—and this will tell you how much they know. Is 3T better for MRI? I am not sure how they will answer the question, but write it down, call me, and tell me so we can discuss how informed your doctor is. I was at a medical meeting where a very intelligent and excellent doctor asked me if the artifacts at 3T were too hard to deal with. We don’t really have many artifacts at 3T- well, no more than they have at 1.5T or 0.3T (yes, 0.3T—instead of 3.0T does still exist). I was encouraged that he asked—but the question itself spoke volumes. This information is readily available. Many doctors simply do not understand why an MRI trained radiologist with a dual fellowship in body MRI and neuroradiology could help them
MRI provides a road map for your treatment. You want the combination of the best pictures and an experienced radiologist. When you have a pain or radiculopathy high quality MRI reveal the nerve roots, bone marrow and surrounding tissues. X-rays see only bone and calcium and, as a result, their evaluation of soft tissue is poor. Most pain is caused not by calcifications—which are old findings and can take years to develop, but by acute soft tissue tears, muscle sprains, and disc herniations. To reliably see this, you need the highest possible resolution. Most scanners will only scan at very low resolution, whereas a 3T scanner can scan at HDTV resolutions. This is the difference between 3T MRI and 1.5T MRI—the ability to scan with high resolution. And, of course, that there may be things that might not be seen at weaker resolution.

pider veins are red, purple fine lines that look like starburst clusters or web-like mazes, just beneath the skin. They measure between 0.5 - 1 millimeter in diameter. Varicose veins are enlarged, swollen, bulging, often twisted rope-like veins that occur on the surface of the legs and thighs. Both, varicose and spider veins are a manifestation of reverse blood flow due to the failure of delicate valves that direct the blood towards the heart. In tiny veins this will result in spider veins, in larger veins it causes varicose veins. Spider veins are usually just a cosmetic concern, while varicose veins are often a medical problem. Duplex ultrasound examination is an essential tool for a precise diagnosis and the successful treatment of venous disease. Diagnosing & Evaluating Abnormal Uterine Bleeding
any woman whose regular cycle suddenly changes is best advised to ask a doctor about it. Women who have any amount of bleeding after menopause should always seek an evaluation.

A
bnormal uterine bleeding is a common reason women see their doctors. Abnormal bleeding can occur at any age of life and has many causes – some are easily treated, while others are more serious. Women should consult with their health care providers if ever they have bleeding concerns. The first step in diagnosing abnormal bleeding is to recognize normal and abnormal menstrual patterns for women in their reproductive years. Normal menstrual cycles usually start every 21 to 35 days; that is, a woman gets one period every three to five weeks. Bleeding that is more or less frequent than this is abnormal. In addition, bleeding between a woman’s regular cycles is not normal. Other important considerations include how much and how long a woman bleeds. Periods which cause women to bleed excessive amounts (technically, more than 80cc of blood per cycle) or that last more than seven days, are too heavy. Spotting-only or missing a cycle altogether is too little. There are times in women’s lives when they may be more likely to have abnormal uterine bleeding. In the years immediately after a woman starts menstruating and again in the years before she enters menopause (defined as 12 months without a period), women may have irregular cycles due to the changes in hormonal regulation in the body. In addition, as women age, their chance of having a medical condition, structural lesion or other cause of irregular bleeding may increase. Regardless of age,
There are many causes of abnormal uterine bleeding, most of them benign. Pregnancy, changes in the hormonal cycle and structural abnormalities are common causes. Abnormal lesions in and around the uterus (for example, endometrial polyps or uterine fibroids) can interfere with the body’s natural way of slowing or stopping a woman’s period, resulting in excessive bleeding. They can also cause women to bleed between periods and produce other disruptive symptoms. Numerous medical problems which affect the body’s hormones or overall health also cause changes in menstrual bleeding. Specific conditions which result in abnormal clotting may predispose women to heavy periods and medications that thin the blood may have similar effects. Certain infections in the uterus and cervix can trigger bleeding. Rarely, abnormal bleeding may be a sign of cancer.
Because there are so many causes for abnormal uterine bleeding, the evaluation of this problem starts with a thorough history by your care provider, including questions which address the issues above. He or she will ask specifically what changes are occurring, how long the changes have taken place, and what other symptoms or signs a woman may have experienced. Other information which a doctor may ask a woman include her medical and surgical history, family history, pregnancy history, sexual history and her use of medications, including tobacco and alcohol. Overall, abnormal uterine bleeding in women of reproductive age is usually benign, but it often can, and should be, treated. For these reasons, women are encouraged to seek care if they are having bleeding concerns. Any woman who has bleeding in menopause should see her doctor for an evaluation. The Healing Touch Of Osteopathic Medicine: A Gentle, Natural Approach To Healing Acute And Chronic Pain And Many Other Conditions

steopathic medicine is a gentle, handson approach to healing the body. Developed at the end of 19th century by Dr. Andrew Taylor Still, a frontier physician frustrated by an over-emphasis on By Seth D. Torregiani, DO the use of medicines and treating only symptoms (sound familiar?), Dr. Still believed the human body had most of what it needed to heal itself, and he set out to develop a form of hands-on medicine designed to unleash the body’s inherent power to heal. He undertook an extensive, in-depth study of the human body, and, over time, developed a healing approach that corrects anatomical dysfunction in the body resulting from trauma or injury, emotional or physical stress, infection, poor posture or lifestyle or other insults. The result of this kind of treatment is a substantial improvement in the functioning of the bodily systems critical for health. While a great deal of modern medical practice seeks to “fix” a condition by suppressing its symptoms with drugs, Osteopathic medicine seeks to get to the root of a health problem and, whenever possible, correct the underlying issue to actually heal the problem. The applications of this approach are endless. Osteopathic medicine can treat a wide range of conditions, either curing them or reducing or eliminating the need for medications, surgery or other invasive procedures. Osteopathic treatment can be of great benefit in the following conditions (and many more): Acute and chronic musculoskeletal injury – Osteopathic treatment helps to release acute and chronically strained muscles and muscle spasms that can be a source of ongoing pain and dysfunction. This special healing work decreases swelling and improves circulation in tissues hurt from sprains, strains, pulled muscles, neck and back injuries, as well at tendon and ligament injuries (such as tennis or golfer’s elbow). Once swelling is reduced and circulation improved, inflammation is diminished and motion is restored to the injured area resulting in dramatic improvement. Old Trauma – A great deal of chronic pain is due to old trauma that the body had not been able to release. After a significant trauma to the body, say from a motor vehicle accident, concussion, fall or other major impact, the tissues of the body may chronically stiffen and the body itself may be stuck in a “shock” state – it still thinks it is in an injury fighting mode. The result is that many patients, even months or years after the original injury, often still experience pain and dysfunction, even though there is no “evidence” of a problem
on imaging studies such as x-rays or MRIs. Utilizing gentle techniques that focus on releasing the chronically injured tissue, an osteopathic physician can often achieve significant pain reduction and improvement in function in patients with trauma dating back months or even years. Headaches, Neck Pain, Back Pain – Osteopathic treatment can often provide significant relief to these common conditions. The osteopathic physician evaluates the entire body, seeking areas of dysfunction that may be contributing to symptoms of pain. Because the body is interconnected via muscles, tendons and ligaments, nerves and vessels, it is common, for example, for back pain to start with dysfunction in the ankle, knee or hip, or headaches to have their origin with problems in the low back, pelvis or tailbone. Correcting areas where there is dysfunction anywhere throughout the body, as well as treating the area of complaint, often results in significant improvements in these and other common conditions. Pediatric problems and pregnancy-related pain – Because of the gentle nature of osteopathic techniques, and the focus on correcting anatomical dysfunction, osteopathic medicine can be very effective in treating a wide range of pediatric problems, including GERD, colic, otitis media, plagio-cephaly or moulding of the head, scoliosis, difficulty sucking or latching, torticollis, etc. It can be a very effective treatment for children with physical, neurological and developmental disabilities as well, and parents often notice significant cognitive improvement and better physical functioning in children undergoing osteopathic treatment. Pregnant women experiencing back, hip or pelvic pain, lower extremity swelling, GERD or other symptoms also respond very well to osteopathic treatment. Other conditions that often improve with osteopathic medicine include: vertigo, acute and chronic sinus infections, asthma, digestive problems, post-surgical pain, and many more. Regular osteopathic treatment in athletes or very active individuals can also help maintain proper form and prevent injury. Osteopathic physicians (D.O.s or doctors of osteopathy) are fully licensed physicians who have attended four years of medical school as well as a minimum of three years of residency training. Like MDs, they can practice in any specialty they wish and can perform surgery and prescribe medications. While most D.O.s practice medicine in a fashion similar to medical doctors, a small number, such as myself, focus their practices on the healing, hands-on work they were taught during their training. D.O.s differ from chiropractors generally by the fact that they are fully-licensed physicians, equivalent to MDs, and the manual medicine techniques that D.O.s utilize tend to de-emphasize aggressive spinal manipulation or 16
adjustments in favor of gentler, muscle and fascia (connective tissue)-based techniques. Macular Hole T
he following article is is the second part of a three part series about macular diseases. Macular Pucker, Macular Hole and Macular Degeneration. These are 3 separate clinical diseases that all affect the macula but have different causes and different treatments. Patients commonly confuse the three diseases because they all affect the macula. This article will be about Macular Hole. Macular Hole is a condition of the retina that can cause distortion, blurred central vision and usually also causes a dark spot in the center of the vision. This condition is similar to macular pucker as many times the two conditions can occur at the same time. For example, sometimes a macular pucker can progress to the point where there is so much distortion of the macula that a hole develops in the center of the macula. Not only will the patient have distortion, they will also have a scotoma (missing area) in the center of the vision. Not all macular holes will have a macular pucker. If one looks an OCT (Optical Coherent Tomography) picture of someone who has a macular hole, retinal tissue the shape of a round hole is missing in the center part of the macula. Like macular pucker, a macular hole affects the macula but it also has nothing to do with macular degeneration. This is also a completely separate disease with different etiologies and treatments. Macular hole is also most commonly idiopathic (etiology is unclear) but can be caused by trauma. It is more common in women and as one gets older. Macular holes in young patients are usually caused by blunt trauma to the eye. Common examples of blunt trauma are a punch to the eye or a sports injury from a soccer or tennis ball. The treatment for macular hole requires outpatient surgery. In addition to the sutureless surgery called pars plana vitrectomy that is done to remove the vitreous, gas is injected into the entire posterior cavity of the eye. Post-operative facedown positioning is required for about 10 days after surgery in order for gas to pneumatically tamponade the hole closed. Once the hole is closed and the gas reabsorbs, the central vision is restored. When gas is in the eye a patient cannot fly in an airplane, go into high altitudes or receive Nitrous Oxide anesthesia. Many patients are worried about facedown positioning for 10 days. Surprisingly most all patients are very successful at maintaining the correct position. Patient age ranges from their 20’s to 90’s that all have this procedure done. When a patient knows that the success of vision return relies on their positioning, they tend to do it! The main complication of macular hole surgery is cataract formation. Most all elderly people will develop a cataract (if it has not already been removed) within a year of the surgery. Retinal detachment is another complication but much less common. In summary, Macular Holes can be successfully repaired surgically in symptomatic patients. This surgery is done on an outpatient basis and will require post-operative positioning for up to 10 days. www.TheWomensJournal.com

ransradial access (TRA) through the left or right radial artery is rapidly growing in popularity for the performance of diagnostic coronary angiography (heart or cardiac catheterization) to visualize the heart arteries and for percutaneous coronary intervention (PCI) used to treat or fix the blockages identified by angiography. In many areas of the United States, including our Christiana Health Care system, TRA accounts for up to 30% of the procedures compared to the remaining done through the more traditional transfemoral approach (TFA) accessing either of the femoral arteries. Outside the United States the rates of TRA exceed 50%. Patient comfort, safety, lower costs, improved outcomes and shorter hospital stays are all driving the “Radial Shift.” A 2013 study from Michigan State University estimates a cost savings of $1,375 per patient for PCI performed with TRA vs. TFA driven mainly by same day discharge and decreased procedural costs. The RIVAL (RadIal Vs femorAL access for coronary intervention) trial of 7021 acute coronary syndrome patients in 32 countries is the largest radial vs femoral access trial to date. There was no difference in the combined endpoint of death, myocardial infarction, stroke or bleeding at 30 days. When the ST elevation myocardial infarction (STEMI) patients in the RIVAL are analyzed, primary outcome was 3.1 vs. 5.2 % (P=0.025) in favor of TRA. Among the STEMI patients, vascular complications (1.4 vs. 3.7%) were less in the TRA patients and most importantly there was 54% reduction in mortality once again favoring the TRA group. Education for Women Creates Ripples of Positive Changes Fresh Start Scholarship Foundation Changes the Lives of Entire Families Like the ever-expanding ripples created when one small pebble is dropped into a pond, Fresh Start Scholarship Foundation, Inc. has had a far-reaching impact on many women and their families for the past 18 years. Established in 1996 as the charitable arm of Wilmington Women in Business, Inc., Fresh Start’s mission is to provide incentive to adult women who face (or have faced) personal and economic challenges and are seeking to improve opportunities for themselves and their families through education. Annual scholarships are based on need and provide mentoring support through WWB members.

“Many of our scholars are first in their families to receive a college education,” said Rita Wilkins, president of Fresh Start Scholarship Foundation. “The one thing they all have in common is the enormous sense of pride and accomplishment when they finally reach their goal – a college degree!” Thanks to the generous support of friends of Fresh Start, businesses, corporations, foundations and our dedicated volunteers, we have awarded more than $650,000 in scholarships for 159 women, many of whom have not only fulfilled their own dreams of higher education, but who are also inspiring others to do the same. “I love the fact that it wasn’t just about giving money to us for a scholarship,” said Bonita Penn, BSN, RN, a Fresh Start scholarship recipient who is now in pursuit of a master’s degree, and who sits on the Fresh Start board of directors. “Fresh Start was truly interested in seeing me succeed and graduate.” To receive the scholarship, candidates my be enrolled in a two– or four– year degree undergraduate program at an accredited Delaware college or university. “People can help by sharing information about the Fresh Start scholarship program with other, encouraging candidates to apply, attending fundraising events, and making contributions to the scholarship fund on our website,” added Wilkins. Mathematics is not just the ability to compute numbers, but is the application of complex concepts that intertwine, and build on one another. When a child is struggling with reading and/ or math skills it affects much more than the ability to achieve good grades in school. Reading and mathematics permeates every facet of life, impact all subject areas, and can have a major effect on a student’s academic confidence. Multiple academic disciplines are affected when a student is struggling in reading and/or math. Areas of concern may include: • Math: Students are expected to read and interpret mathematical word problems, follow complex directions, and apply critical thinking skills to mathematical scenarios. Before beginning any mathematical calculation a struggling reader will likely face difficulty comprehending the problem and determining where to begin. Math computation skills are required to apply a student’s ability in each concept. • Science: Courses, such as chemistry, biology, physics, and earth science, require students to read and understand textbooks, follow laboratory directions, and interpret scientific data. Mathematical concepts are applied in all areas of Science especially with Chemistry and Physics. Struggling readers face additional difficulties with complex vocabulary and academic terms that impact overall understanding. • History and social studies: Solid reading skills are essential for success in social studies and history courses. Students must be able to read and understand textbooks, academic articles, biographies, and primary documents. Many of these texts include specialized vocabulary, require students to identify historical
connections, and are best understood within the context of previous reading and learning. • Foreign languages: Learning a foreign language requires a student to understand and be able to apply phonological (blending sounds together and segmenting sounds from words) and orthographic (the sound-symbol connection) processes. Struggling readers may face difficulties with these basic processes which impact both their reading ability and the comprehension required to learn a foreign language. • Career preparation: Your child’s success in school will have a direct impact on career preparation and the ability to succeed in future work. A struggling reader is less likely to successfully access professional or technical texts, understand and process complex directions, and apply new learning. Poor reading skills can hamper a student and will limit future educational and vocational options. The reading programs for struggling readers at Huntington Learning Centers are designed with the individual child in mind. Our professional tutors begin with an academic evaluation that covers all areas of reading to determine a student’s weak skills and areas for potential growth. hen it comes to estate planning, women have unique concerns. The fact is that women live an average of 4.8 years longer than men.*
What is an estate plan? An estate plan is a map that reflects the way you want your personal and financial affairs to be handled in case of your incapacity or death.
Planning for incapacity Incapacity can happen to anyone at any time. Health-care directives can help others make sound decisions about your health when you are unable to.These might include: • Living will • Durable power of attorney for health care (health-care proxy) • Do not resuscitate (DNR) order • Joint ownership • Durable power of attorney • Living trust
Wills and probate A will is quite often the cornerstone of an estate plan. It is a legal document that directs how your property is to be distributed when you die. You can also create a trust in your will. Most wills have to be probated. The will is filed with the probate court. The executor collects assets, pays debts and taxes owed, and distributes any remaining property to the rightful heirs.
Trust basics A trust is a versatile estate planning tool that can protect against incapacity; avoid probate; minimize taxes; allow professional management of assets; provide safeguards for minor children, elderly parents, and other beneficiaries; and protect assets from future creditors. Most importantly, trusts can provide a means to administer property on an ongoing basis according to your wishes, even after your death. You can create a trust while you are alive (a living or inter vivos trust) or at your death (a testamentary trust).

Making The Most Of Your Legal Consultation And Attorney Selection I get it – meeting with an attorney is an intimidating experience especially if you’ve never met one before. You don’t know what to expect, you don’t know if she is going to be caring and empathetic, and you don’t know what kind of news you are going to receive. Plus you probably have to pay to have this nail-biting experience.

his quarter’s article is about being better prepared for that first meeting and how to evaluate if you and By Shana A. Pinter, Esq. the attorney you are meeting with would be a good fit – i.e., should you hire him or her or not. We’ll first address how to prepare for your first meeting and what to expect and end with determining if you and the attorney are a good fit for one another. My goal is to empower you to be a better – by way of understanding, expectations, and information – legal consumer. First, if you are seeing an attorney for the first time about divorce you can expect to be asked basic information such as your name, address, contact information and preferences (phone v. email v. snail mail), date of birth, date of marriage, if you’re currently separated and when that occurred, if you have children, their ages, dates of birth and names, etc. In my practice, consultations generally last 45 minutes to an hour and the bulk of that time is spent collecting information from my potential client and explaining the process of divorce, custody (if children are involved), child support (ditto), and its timeline.
This includes assets owned in each of your individual names as well as property owned together. Briefly, assets include any homes (“real property”), vehicles, bank accounts, retirement accounts, and household furnishings. For high net worth clients, assets can also include airline miles and hotel rewards, specialty license plates, and country club memberships. Debts include mortgages, car loans, and credit cards. Other important financial information to provide to the attorney is how all of your assets and debts are “titled,” that is: whose name is listed on the document. For instance, is the deed to your house in your or your spouse’s individual name or in both of your names? How about the mortgage? If the deed is in both names but your mortgage is only in one name, why? Was it for credit rating reasons? Was it so you could qualify for a firsthome-buyer program? Another set of financial information that’s important are your expenses and your current and historical income and job history as well as your spouse’s current and historical income and job history. Have you not worked for a few years
because you were obtaining your college degree? Did you or your spouse have a family business that was recently closed? How to choose the right healthcare provider for your workers’ compensation injury (Lessons learned from Jordyn)

ecently, I found myself looking for a pediatric endocrinologist for my daughter, Jordyn. In my line of work, I don’t typically get to interact with endocrinologists, pediatric or otherwise. So, we went with a referral from a local children’s hospital, and lets just say that I was less than impressed with the doctor that they paired us up with. I joined an online support group and through that group, found a fantastic doctor that recently moved to the area from Boston. We met with her, and knew right away that we finally found the right doctor for Jordyn. Every time we interact with this new doctor, I am so glad that I didn’t settle for what I considered to be sub-par service for my now thriving, two-year old little girl!
Injured workers go through the same process of having to find a doctor or other healthcare worker to fix what ails them. It is unusual for an injured worker to already have an established relationship with a physical medicine, orthopedic doctor or other necessary specialist, therefore the worker finds themselves in somewhat of a “fish out of water” situation in that they need to seek treatment right away but are not sure where to start. www.TheWomensJournal.com
In Delaware, injured workers must see a Certified Healthcare Provider in order for the provider’s bills to be paid by the workers’ compensation insurance company. These doctors and therapists take an exam, conducted by the Delaware Department of Labor, Office of Workers’ Compensation, after which, they are issued a certification. The Certification must be renewed by the doctor or therapist every 2 years, and the Department of Labor keeps track of these dates. A list of all certified healthcare providers is available online, and can be found on the Department of Labor website at http://dia.delawareworks.com/ workers-comp/hcps/providercertification.php. The Department of Labor updates this list on a weekly basis, so it is important to check this list prior to seeing any doctor for a work injury. Of course, any emergency visit or treatment is excluded from the requirement that the doctors are certified providers. One of the benefits to hiring a workers’ compensation attorney, is that you get a “friend in the business.” What this means, is that as a workers’ compensation attorney, I have become familiar with the certified doctors and therapists that treat injured workers. Based on where you live, the type of doctor you need, and any other special considerations, I can help you select the one that best suits your particular case. I sure wish that I had someone like that to help me find a pediatric endocrinologist for Jordyn! If you are injured at work, it can be confusing to try and figure out what doctor to see. Additionally, that doctor must be certified by the Department of Labor. ith spring just around the corner, we’re reminded to turn our clocks ahead, and to check the batteries in our smoke detectors. Many of us take advantage of milder weather to clean out the garage or to attack the overgrown hedges in the back yard with that new chain saw we received in December. But when a rainy spring Saturday rolls around and your weekend tasks are temporarily postponed, I’d like to suggest another, very important activity to add to your seasonal “To Do” list: set aside the time to review your automobile insurance policy to be sure it is adequate and up to date. I can hear a chorus of groans at the above advice, and I can even visualize eyeballs rolling as I write this. But – as a personal injury attorney with decades of experience dealing with insurance companies in court and around the settlement table, and as someone

Many events can occur in a person’s life in the course of a year, and some of them can even translate into saving you money. Here are a few items to keep in mind as you examine your automobile insurance policy to determine if your coverage is adequate:
It is important to realize these minimum requirements keep you legal to drive, but are not designed to fully protect the average individual driver. By carrying only the minimum state- required insurance, in the event of an accident, you will be legally responsible for all other uncovered costs. You will have to pay out-ofpocket for any hospital or medical bills, and funeral expenses not covered by insurance. Any assets you have, including your home and your savings, may be at risk.
Is the Minimum Enough? Like most states, Delaware has established minimum liability coverage requirements to pay costs related to injury or damages in an accident. By law, Delaware drivers must also maintain minimum limit requirements ($15,000 per person; $30,000 per accident) for personal injury

Steps 1. Combine water and salt in a 2 quart saucepan. Bring to boil over mediumhigh heat. Stir in Quinoa and follow cooking directions. Place in medium bowl once done cooking, set aside and let stand. Combine orange juice, vinegar, oil and peel in 2 cup measures. Blend well with whisk. 2. Add half of juice mixture to Quinoa. Mix well. Cover and chill at least 1 hour. 3. Add remaining juice mixture and remaining ingredients to Quinoa mixture. 4. Toss gently to combine. Line serving bowl with lettuce, if desired. 5. Substitute any oil or vinegar with your favorite Fusions Tasterâ&#x20AC;&#x2122;s Choice fused/ infused flavors. Serve as a healthy breakfast alternative or for a desert. Nice with the fresh fruits of spring ripening. How To Know When It’s Time: What Are Some Signs My Loved One May Need Assisted Living?

s our loved ones grow older, their needs change. Some changes are influenced by medical and physical needs. It is difficult for anyone to see a decline in a loved one’s ability to care for themselves independently. This article is intended to help answer basic questions in regards to what is Assisted Living and when it may be time to consider a move to an Assisted Living Community.
What is Assisted Living and how is it different from ‘skilled nursing care’?
Assisted Living is for individuals who need limited physical assistance from trained professional caregivers in a supportive social environment. Assisted Living communities, such as Ivy Gables, provide Residents with limited physical assistance with Activities of Daily Living (ADLs, bathing, dressing, ambulating, and eating), as well as an environment to foster and encourage independence, and an active lifestyle of arts, entertainment and spiritual activities. Ivy Gables provides three restaurant style meals a day, snacks, and assistance with medications and management of medical appointments. Residents live in either studio style apartments, or some communities have apartments with one and two bedrooms. Skilled nursing care is for individuals who may be completely physically dependent on someone else to provide their ADLs as well as complete dependence for medical management.
If you notice your loved one is not as well groomed, their clothing or personal hygiene is not as immaculate as what it once was. They may not be taking their medications properly, or may have a change in their medical condition; a condition may be worsening and require more monitoring or more medical appointments. If you notice a loved one being more forgetful, not eating a well balanced meal, or able to take care of their home or their finances as well as they once used to. These are all signs that a loved one may need assistance with every day living.
Where Do I Start?
Recognizing that a loved one has a need for assistance is a huge step, and probably one of the more difficult steps in this process. Knowing your loved one’s financial situation is the key to where to begin. Assisted Living is paid for privately, with personal funds, coverage from a Long Term Care Insurance Plan and assistance can also be received if one is eligible to receive the Veteran’s Aid and Attendance Pension. Analyze your loved ones financial health, how much income do they receive monthly, where does the income come from, what are their investments and other assets. You’ll need to know what your loved one can afford.
Making Difficult Decisions for Aging Loved Ones

include lunch or dinner? Food is such an integral part of our daily lives, you want to be sure the community provides food appealing to you and your loved one. he first word most people think of when they hear Keratin treatment is FORMALDEHYDE. Formaldehyde is DANGEROUS, won’t that harm my health? Many of you battle your hair every day, some of you devote hours and hours, and experience high levels of stress and frustration over taming and beautifying your fluffy, fat, curly, frizzy, dull hair. If you are that person, read this article and reconsider how this treatment could detangle both your nerves and your hair.
Early Technology
Early Keratin formulations were developed without ingredient limitations. Some contained dangerous levels of formaldehyde causing troubling physical symptoms. These products ended up in the media with severe warnings to avoid Keratin treatments at all cost. Overtime, technology improved and manufacturers have been able to reformulate with better ingredients using less harmful versions of “aldehydes”. Every Keratin product contains some form of “aldehyde”. But, ponder this: Morning breath is formaldehyde; an apple sitting on your counter rotting releases formaldehyde, the thing is the levels are not strong enough to be dangerous. Today, there are several Keratin formulations on the market that are not dangerous when used as directed by the manufacturer. The preferred product in my salon is Artego’s New Hair System.
Today’s Keratin treatments can be customized to meet the preference of the client. After care products enhance the longevity of the treatment by assisting in styling, and maintaining the shine and softness.
When To Consider A Treatment
Early spring through early fall is best. During these seasons humidity is high leading to frizz and unpredictable curl patterns. Done correctly and well maintained, one Keratin treatment can last from 12-18 weeks. Aftercare products range in cost from $100-$150 and generally last up to eight weeks of shampooing daily.

The greens and filler flowers, such as heather, limonium, wax flower or babies breath carry out the part of the “extras”. Yet all are integral to the entire production or composition or bouquet. bouquet is more than ~ ...an assortment of flowers tied with a ribbon or plunked in a vase. The creation of a bouquet combines the art of composition and the principles of engineering. The finished product has to appeal to the eye as well as remain intact over several hours [or in the case of a bridge, for many decades!]. If a bride decides to have her wedding flowers preserved the matter of having every bloom at its peak and every adornment firmly attached and every ribbon securely wrapped is doubly important. The bride and her florist are an artistic team. Most often a bride has a vision in her mind accompanied by a picture from a magazine or a photo from a friend’s wedding or an image from an online site of a particular style of bouquet, and she may have a very specific list of flowers that she loves and are meaningful to her. Enter the floral designer who knows what flowers on the bride’s list are available in the season when the wedding will take place. Lilies of the valley, tulips, peonies, & dahlias are not available year round making it necessary to suggest an alternative for a bride who has her heart set on any of these. Just as an artist selects various objects for a stilllife painting, a floral designer must select flower forms which compliment each other when the bouquet is assembled. One would not want flowers of all one size like football mums and peonies and sunflowers in a single bouquet! Nor would daisies, alstromeria and minicarnations create an eye-popping composition. If we think of a bouquet as a theatrical production, flowers such as peonies, hydrangeas and dahlias

grandmother’s wedding dress is to be used to wrap the bouquet, or if a sentimental item such as a locket is to be intertwined with the flowers, these items must be brought to the designer in advance of the wedding week. The creation of a bouquet when the floral designer sits down to put it together is much like manufacturing a car on an “assembly line”! First, all of the flowers have to be prepped. Having arrived at the flower shop early in the week, all stems have to be cut and hydrated and the flowers of each variety have to be counted and set aside in the cooler. At assembly time it is critical that the designer does not discover that she is 5 stems short of the roses or hydrangeas or whatever flowers are “the Stars” in the wedding party’s bouquets!
The creation of a bouquet, like the main course at a gourmet dinner, is a combination of art and science. The entree is equivalent to the bouquet; each individual flower in the bouquet is an ingredient in the dish. It includes a featured item, a number of secondary ingredients, an assortment of spices and herbs and perhaps a garnish. When the floral designer meets with a bride there some essential questions that have to be answered:

The color of the attendants’ dresses will partially dictate the variety of flowers to choose from. The date of the wedding will dictate what seasonal flowers will be available. The number of attendants, the number of guests and whether arrangements will be needed at a ceremony site as well as a reception site will dictate how much of the overall budget is available for flowers.

The creation of a bouquet is the product of a bride’s vision and the floral designer’s expertise. Once the bride and her florist have decided on the specific flowers to be used, the bride steps out of the picture. Even though the wedding may be scheduled for a year in the future a floral designer makes her “shopping list” of the flowers that will need to be ordered 3 - 4 weeks before the actual delivery date. very month has a special birthstone dedicated to it, but none is more spectacular than diamond, the birthstone for April. We all look forward to spring for the bright sunshine and rain drops that glisten in the sun in April to form beautiful shimmering rainbows of color. This combination of sunshine and rain brings May flowers. The birthstone for May is emerald like the brilliant green but fragile new grass. With Spring we also have thoughts of love, romance and devotion for which Diamond is the ultimate symbol. It began over 500 years ago when Maximillian of Austria presented Mary of Burgundy a diamond ring to seal their wedding vows. That tradition has grown and with the help of modern technology in mathematics and diamond cutting equipment you are able to wear a diamond far more brilliant than Mary’s. Whether it’s your birthstone, wedding band, engagement ring or just for fun, April is diamond month. How to select the right diamond for you. Understanding the four Cs of diamonds. Carat weight: Diamonds and all gemstones are weighed in carats. One carat is 1/142 of an avoirdupois ounce or 1/5 th of a gram. That carat is
divided by 100 parts called points. An example is 150 points = 01.50 cts., or one and one half carats. A good point to remember is that diamonds that weigh .98 or .99 points sell for far less than a full 01.00 carat of the same color and clarity. Color: The color scale for diamonds created by Gemological Institute of America (GIA) starts with D color which is colorless and goes to Z which is a brownish yellow. Clarity: The absence or presence of imperfections in a diamond determines the clarity grade which ranges from Flawless which is blemish free inside and out to the grade of I3 that would have large imperfections that have and effect on the appearance of the diamond and are usually found in outlets that advertise low price. With low price in most cases comes unacceptable quality. Cut: The angles and proportions of diamond determine the cut grade. The finest and most brilliant round diamonds are cut to ideal proportions created by Marcel Tolkowsky (uncle to Lazare Kaplan). Diamonds of this cut are unsurpassed in brilliance by the most advanced light measuring equipment. Diamonds of lesser cut can sound very appealing in price but don’t even come close in beauty. you talk to a Graduate Gemologist trained at GIA. We are able to show you how the other 4 C s interact with each other to give you the maximum brilliance and beauty and size to fit your budget. Some diamonds also have GIA or AGS grading reports. Although there are many other companies that advertise grading reports, most others are not as consistent or accurate. It is the job of a good Gemologist to compare the diamond to the grading report for accuracy and guard against cutting details not listed on reports. Shape is another consideration in selecting a diamond. About 75% of all diamonds cut are round. Round is the most brilliant of all cuts and the most versatile because it can be set in any type of lady’s or men’s jewelry and compliments any age group from infants to seniors. You may prefer a fancy shape such as princess, cushion, asscher, oval or many others but bear in mind that these shapes like clothing fashions come and go in popularity. You can always remount a round diamond into the latest style ring.

Retiring from 30 years of teaching and 25 years behind the scenes in theatre, I made a list of what I wanted in my next career. Number one: continue making a difference. Serendipitously, I met a man at a party who told me his story about a divorce, a new job, and his new home that he didn’t have time to put together. My response regarding his home was “Hire me.” When his house was finally in order, he informed me that “this” was my new career. Concluding that “this” might be organizing, I discovered that The National Association of Professional Organizers was having its annual conference in Chicago in three weeks. I bought the plane ticket, crossed a lot of palms with silver, began the courses and dedicated myself to starting a business. That was twelve years ago. However, most helpful were those backgrounds in teaching and theatre. Every client, like every student, was an individual. Each story and each situation was different and demanded different solutions. Everyone needed compassion, guidance, and a bit of humor to get to the next stage. Every home was a set determining how the characters worked or moved, how they could move better and what was in their way,.
I developed workshops on getting and staying organized, helping people learn the stages of working through the clutter on their own. I became a writer for the industry, winning numerous awards from the Delaware Press Association and a First Place National Writing Award from the National Federation of Press Women. Teaching, theatre and travel got me to a side career: a speaker on historical fashion. Close to the Vest and Under the Skirt features the dresses and the dressing of the Victorian lady and gentleman; Bustles to Bras, connects 200 years of women’s history with the most outrageous silhouettes in women’s fashion. Don Adams
Don Adams started out as an electrician/instrumentation technician at the DuPont Experimental Station. After working there for fifteen years, Don needed someone to service his old pre-owned wooden hot tub. But he couldn’t find anyone. So, Don acquired his Certified Pool/Spa Operators credentials and founded Bubble Works Hot Tub and Spa Co in 1985. In 1991 Bubble Works acquired Clearwater Spas established in Newark, Delaware, in 1985. That’s when he left DuPont went into business full time. The company soon enjoyed an excellent reputation for service. Industry awards followed, including two Aqua 100 awards. As Adams says, “Those were hard to come by, encompassing the top 100 Pool and Spa Retailers in the country.” Don realized a simple business principle was at work here, which was to do the right thing for your customers.
