Major Advance In Laser Vision Correction Technology – Now Available In Delaware I
nnovations in laser eye surgery have enhanced accuracy and safety, while improving comfort and convenience, and at the same time lowering costs. I have been fortunate to participate in this continued evolution, having been involved in laser vision correction research By and patient care Jeffrey B. Minkovitz, M.D. since my training. As a co-investigator in the FDA clinical trial for the first refractive lasers in the early 1990s, I was thrilled to be part of the transition from the effective but occasionally unpredictable radial keratotomy to fast and precise laser surgery. Since then, new techniques have further improved outcomes and reduced side effects. The range of possible treatments has dramatically expanded, from low levels of nearsightedness to much stronger prescriptions, as well as farsightedness and astigmatism. Lasers have also become more sophisticated, with more innovative beam profiles and treatment patterns, and enhanced ability to “create” the ideal naturally shaped cornea. But while existing lasers have improved, change has been marked more by evolution than revolution. In fact, until just recently, one of the first lasers to achieve FDA approval in 1995 has remained the industry leader (and the only platform available in Delaware) for nearly 20 years, long by any technology standard. Recently, a leading ophthalmic device manufacturer has emerged as the new laser technology leader. The Alcon Wavelight Allegretto offers improved physician and patient ergonomics, faster treatments and more frequent monitoring of eye position and laser beam energy, and study results reveal better outcomes. Wavefront – optimized to “create the most natural corneal shape”, this laser is also less dependent upon pupil size than other laser platforms. Operating at 400 pulses per second, this laser is nearly 10 times faster than previous lasers (most operating at 60 pulses per second). Such speed reduces patient anxiety (treatments lasting a few seconds are much easier for patients to fixate on a target) and reduces environmental variables including surface drying, which affect outcomes. Faster treatments require more accuracy, which the laser provides by verifying eye position before each pulse, and making adjustments within 6 milliseconds, much faster than any other laser.
Each pulse is also analyzed 3 times before delivery to the eye, and energy is automatically adjusted as needed (a technology termed “Perfect Pulse”). Sealed beam pathways, unlike other lasers, reduce environmental factors, allowing broader tolerances of room temperature and humidity. What all this means is more accurate and predictable results and a more pleasant treatment. Numerous studies agree that results are superior (including studies at University of Utah and at Stanford in 2014) with a higher percentage of patients achieving 20/20 and 20/16 vision, and good vision achieved earlier. Results presented for FDA approval also showed superior outcomes, with 92.7% of patients achieving 20/20 vision and 68.8% achieving 20/16, also significantly better than results by the previous industry leader. As with any technology, continued research and innovation provide advances which result in better functionality, often at a lower cost. In medicine, that means a better patient experience and most importantly better outcomes. Just as I was excited to introduce laser vision correction to Delaware in 1995 (those pioneering patients had to travel with me out of state before Delaware had its own laser), I am enthusiastic about ushering in this next phase in vision correction. Laser Vision Delaware will make the Allegretto available to other Delaware eye surgeons as well, in order to provide all patients with the latest technology and best visual outcomes. Jeffrey Minkovitz, MD is an ophthalmologist with Eye Physicians and Surgeons in Wilmington, Delaware. Dr. Minkovitz specializes in cataract and refractive surgery, and is one of only a few Delaware surgeons specially trained in corneal transplantation.
He completed a cornea and refractive surgery fellowship at the renowned Wilmer Eye Institute of Johns Hopkins. Previously, he graduated magna cum-laude from Harvard, earned his medical degree from the University of Massachusetts, completed internship at Harvard’s Mt. Auburn Hospital, and served his ophthalmology residency at Washington University. Dr. Minkovitz participated in the early research on Excimer Laser refractive surgery, and has offered this procedure to Delawareans since FDA approval in 1996, before Delaware had its own laser. These early pioneering individuals travelled to Wilmer Institute with Dr. Minkovitz, where he maintained his teaching and clinical appointment. In addition to introducing laser vision correction to Delaware, he also introduced NearVision CK to eliminate reading glasses, DSAEK (a newer, less invasive type of corneal transplant) and recently the newest advance in cataract surgery: laser assisted cataract surgery. In 2016, Dr. Minkovitz founded Laser Vision Delaware, llc and brought the newest and most advanced refractive laser to Delaware. Dr. Minkovitz is a co-founder of the Center for Advanced Surgical Arts and serves on its medical advisory board. He is also on active staff at Christiana Care. While on staff of Wilmer at Johns Hopkins University, Dr. Minkovitz received the Outstanding Teacher award. He was also named Top Doc in ophthalmology by Delaware Today Magazine six consecutive years, and has been an invited instructor at American Academy of Ophthalmology Annual Meetings.

It seems that the year is running away with us again, and as we are working to finalize the 2nd Quarter Edition of the Women’s Journal I can’t help but pray that 2016 will go a little slower and that God will be much gentler with us. A 20-Minute Solution For Women With Stress Urinary Incontinence A
bout half of all women experience stress incontinence -- losing urine when they cough, laugh, sneeze or exercise -- at some point in life. It’s a common problem in women of all ages, including women in their 20s. Many women tell us stress incontinence limits their ability to work out or enjoy an active social life. Fortunately, there are many treatment options to consider. Women owe it to themselves to talk to a urogynecologist, a doctor who is specially trained to treat pelvic floor disorders and bladder problems, who can partner with them to explain the various nonsurgical and surgical treatment options to find the right choice. Many women find relief by simply adopting a few healthy habits such as
losing weight or performing exercises that strengthen the pelvic floor.
Minimally invasive sling However, behavior modification doesn’t resolve the issue for everyone. For those patients, surgical intervention is a highly successful alternative.
Since the 1990s, small devices known as midurethral slings have been effectively treating women who suffer from stress incontinence. There are studies that have followed patients for 15 years and show more than a 95% success rate. That translates to a significant improvement in lifestyle, as women no longer have to worry about leaking urine when they laugh, sneeze or exercise. In fact, the comment
we hear most often from patients who have had the sling procedure is: “I wish I had done this sooner!” The small sling, made from polypropylene mesh, is inserted through an incision in the vagina – using an advanced, minimally invasive surgical technique. There are no cuts to the abdomen and most women have very little pain. This procedure is the recognized worldwide standard of care for the surgical treatment of stress urinary incontinence, according to a position statement by the American Urogynecologic Society (AUGS) and the Society of Urodynamics, Female Pelvic Medicine and Urogenital Reconstruction (SUFU). Making the decision
We perform the surgery in an outpatient center and the procedure is quick—about 20 minutes, in all. Patients don’t usually require general anesthesia or a breathing tube. Because this surgery is minimally invasive, patients can resume daily activities within 24 hours, such as driving, returning to work or taking a shower. Most patients report very little pain, only a bit of discomfort that is treated with over the counter pain medication. But, the best news is that improvement for incontinence is usually immediate. Many patients

wake up the next morning and find that they no longer leak urine when they cough or sneeze. It’s amazing what a huge impact a simple surgery can have on women’s lives. So who should consider a sling? For most women, that decision can occur as soon as stress incontinence begins interfering with their daily lives. Y
ou take good care of your teeth and they feel fine – except when you eat or drink something cold or hot. But when you start to savor that first cup of coffee or sip a refreshing glass of iced tea, the pain begins. And you find yourself wondering “what’s going on?”

our routine dental exam includes more than looking at teeth and gums. The dentist also checks the mouth, face, throat, and neck glands for signs of cancer. A routine dental x-ray may reveal a bony cancerous lesion. Many times the x-ray is the first and only sign of the disease in its early stages. According to the National Cancer Institute, oral and throat cancer affects an estimated 43,250 Americans each year. Of those an estimated 8,000 will die of the disease. There is about only about a 50% five year survival rate of those diagnosed with this type of cancer largely because it is too often first detected in advanced stages. As with all cancers early detection and treatment are key to a successful outcome. Risk factors include male sex (twice the incidence of female for cancer of mouth and throat), tobacco use, both smoked and chewed, and alcohol consumption. The risk is greatly increased when there are multiple risk factors. Ultraviolet light is a risk for cancers of the lips and face. The above risk factors are well known but there is a growing risk that is seldom discussed, infection with the Human Papilloma Virus, (HPV). HPV, usually associated with genital warts and cervical cancer, seems to play a significant role in the development of mouth and throat cancers. At this writing there is no clear agreement in the medical community if HPV causes mouth and throat cancers but there is clearly a very strong

Of the many species of HPV virus, it is the HPV16 which seems to play a significant role in mouth and throat cancer. Since this HPV16 is targeted by the HPV vaccine it is possible that the vaccine may help prevent these head and neck cancers as well.

aricose Veins are caused by incompetent vein valves in the superficial venous system of the legs which is progressive, often symptomatic and whose treatment is covered by most insurance plans.

veins?
Most people believe that varicose veins are just cosmetic and inflict women more than men. However, there are many factors that can lead to the painful varicose veins. Below are some of those indicators:
Family History: If varicose veins run in your family, you may have up to a 50% chance of having them yourself. Just because they run in your family, does not necessarily mean you are destined to be afflicted. There are other factors that can cause them.
Age: As you get older, the valves may start wearing out over time. Even if you take care of yourself, you still may get varicose veins.
Gender: Varicose veins do impact women more due to the hormonal changes in their body during their lifetime. Obesity: Body Mass Index (BMI) is a weight to height ratio. There are many BMI calculators available. Being overweight (BMI between 25.0 and 29.9) and especially obese (BMI > 30.0) can cause varicose veins due to the added pressure in the vein system of your legs.
Inactivity: Sitting more during
Get Active: Moving around during the day instead of sitting can help your legs. It is recommended that if you have a desk job, you should move for 2 minutes for every 20 minutes. Being athletic does not mean you can’t get varicose veins. There are other factors involved if you are at risk to get them.
Elevate Legs: To help with circulation, try elevating your legs to assist the veins to get the blood to your heart.

Age Related Macular Degeneration A

ge-related macular degeneration (AMD) affects millions of Americans and can cause them to lose their central vision. AMD is a degenerative disease of the macula, a part of the retina which controls a person’s central vision. Symptoms can start with distortion of straight lines or missing parts of letters but can lead to a dark spot in the center of the vision causing complete loss of central vision. Patients will never go completely blind from this disease but they can become “legally blind” from loss of central vision. They can lose the ability to do things like drive, recognize friends’ faces, read and write legibly. There are 2 types of AMD, the dry and the wet form. The dry form is characterized by little yellowish deposits in the macula called drusen. These are “aging spots” that accumulate from by-products of retina metabolism. Unfortunately there still is no treatment for dry AMD. In contrast, the wet form of AMD is characterized by abnormal blood vessels that grow underneath the retina. Twenty percent of patients have wet AMD but these patients make-up 80% of the people who have significant vision loss. Patients with wet AMD can experience sudden vision loss when these abnormal blood vessels start to leak or bleed. This type of vision loss occurs over days to weeks and inevitably gets worse if not treated. Intraocular injections are successfully being used to treat wet AMD. These injections not only stabilize but can improve vision. They are given indefinitely to a patient off and on for the rest of a patient’s life. Intra-ocular Avastin (Genentech $60) injections can be given monthly. It has been shown to stabilize or improve vision in about 90-95% of patients. Currently this drug is being used off-label as this drug was FDA approved for colon cancer, not for use in the eye. Intra-ocular Lucentis (Genentech $1,950) is another drug that successfully treats wet AMD. Lucentis is FDA approved for treatment of wet AMD. Both drugs are vasoendothelial growth factor (VEGF) inhibitors and work by causing abnormal blood vessels to regress. A recent study (CATT trial) funded by the NIH compared the efficacy of Avastin and Lucentis. This large prospective randomized study showed that Avastin and Lucentis were equally effective in preventing vision loss and improving vision in patients who were followed monthly. However, there was a trend toward Lucentis drying out the retina better and having a slightly lower post-injection hospitalization rate for systemic complications. In addition there have been a small number of site-threatening infections in patients receiving Avastin. This may have occurred because of possible contamination during the compounding process that occurs when Avastin is used. Clinically, Avastin and Lucentis are equally effective, but they are different drugs and behave differently when given to a patient. It is vastly important for the retina specialist to have the ability to choose which drug (and switch between the two if appropriate) to use on a patient with wet AMD. They are both good tools to use in successfully treating wet AMD. Anti-vascular endothelial growth factor (VEGF) inhibitors Avastin and Lucentis have revolutionized the way we care for patients with age-related macular degeneration. Prior to VEGF inhibitors, treatment was limited to thermal laser and photodynamic therapy (PDT). Unfortunately, even with treatment,

many patients with AMD became legally blind garnering only modest visual improvement with laser or PDT treatment. With the use of Avastin and Lucentis, the diagnosis of AMD no longer means eventual and certain blindness. “VEGF Trap” is now a new weapon in our armamentarium against AMD. This unique molecule may have advantages over Avastin and Lucentis in that it has a higher binding affinity for VEGF and longer half life than the currently used VEGF inhibitors. This means that the time between injections may be longer than what we see in Avastin and Lucentis. Multiple clinical trials have demonstrated VEGF Trap’s equivalence to Lucentis. Adverse events in all treatment groups were also equivalent so it is felt that this new drug is at least equally effective and safe. The critical results of this trial demonstrated that VEGF trap may reduce the treatment burden for patients and families regarding frequency of intraocular injections. VEGF trap has the same ability to deliver outstanding visual results and may require less frequent dosing than Lucentis and Avastin. It is a hopeful time for many AMD patients as new therapies become available. SYMPTOMS OF WET AMD • Acute central vision loss • Acute distortion in vision • Unexplained vision loss in someone with dry AMD Call your eye care provider if you have any of these above symptoms, as they may be the beginning signs of AMD. ery few people are aware of how much they use their teeth on a daily basis. It is not until an individual begins to lose teeth and their oral health and quality of life declines that they begin to understand the impact of impaired oral function on their daily life. Patients with missing and/ or failing teeth suffer from malnutrition, reduced self esteem, and self confidence. They possess a high risk for developing oral conditions such as boney atrophy. Patients also suffer from periodontal disease, cavities, root canal infections, and serious bite problems called “Bite Collapse”. In addition to serious dental conditions, patients with severely failing dentitions can also be more at risk for serious medical conditions such as: cardiovascular problems, stroke and diabetes, complications if pregnant, with a high risk of preterm and low birth weight babies. There are several solutions for individuals with failing dentition. The typical sequence of therapy to address these complex dental conditions could require at least three surgeries. First, the teeth are extracted and the bone is allowed to heal. Patients will temporarily function in a traditional upper or lower denture, which at times impacts the patient’s diet choices. Next, the implants are placed and require a healing time of three to four months. A third surgery is done to expose the implants. The start to finish time is six to eight months. During this time, patients use removable dentures to function.

I
’ll never need it, so why should I give…why should I donate blood?
There are approximately 20,000 patients a year that are treated with the lifesaving products that generous donors give every day in the Delmarva region that could probably easily answer that question. But how does that affect you? Why should you be concerned? You’ve never needed blood. Are you absolutely sure that one of those 20,000 patients won’t be a family member, friend, neighbor or even yourself? Of course not, because none of us can predict what’s going to happen in the next five minutes, hours, days or years. Accidents don’t happen because people want them to, that’s why they’re called accidents and that’s why the Blood Bank of Delmarva works tirelessly every day to be prepared. It’s not my intention to sound alarmist, but this is why it’s critical that Blood Bank of Delmarva continuously collects the blood that supplies all the hospitals in our area, the hospitals you or your family members would be taken to should an emergency occur. We don’t take vacations, we don’t take extended holidays because EVERY DAY blood needs to be collected so that it’s ready on hospital shelves to be transfused when the next serious incident happens. But collecting this lifesaving gift is not simply for the purposes of
disaster or trauma. Blood is used to treat a variety of illnesses and diseases: leukemia, those with blood disorders like sickle cell disease, cancer patients, surgical patients, burn victims and so many others. Blood is also used to treat women who experience severe blood loss during childbirth. In fact, complications connected with childbirth can be, per incident, responsible for some of the highest usages of blood. In a rare case in San Antonio, Texas, a woman was given an astonishing 540 units (35 gallons) of blood after delivery complications. Thankfully, she is alive today and raising her child thanks to the selfless act of hundreds of blood donations from complete strangers. We simply call them heroes. But why is it important for YOU to give? You first need to understand that approximately 65% of the US population cannot donate blood. Think about it, that means if you’re in a room with three people only one of the three has the ability to donate… not great odds. Potential donors are deferred from donation for a number of reasons from travel to certain medications and vaccinations, medical conditions, lifestyle and life events and a host of other causes. Of the population that remains eligible to donate, only a small percentage, much less than 10 percent actually do. What’s even more concerning for those of us in the blood industry is that the average age of a blood donor is increasing every day. If blood centers across the country aren’t successful in recruiting younger donors, there may come a time when there won’t be anyone left to donate. And since modern science has yet to develop a substitute for human blood, the results could be devastating. Because of the criticality of our mission, we must do everything in our power to make sure that never happens.

Adult Day Care Is The Right Fit For The Whole Family By Natalie Scott
Y
ou’re not alone. According to the National Family Caregivers Association, more than one quarter of the adult population has provided care to a family member or friend during the past year.
After having a heart attack and a stroke, 64-year-old Diane, was no longer able to work. She started to become depressed sitting at home all day, so when she heard about the adult day program at Easter Seals, she told her daughters she wanted to give it a try. Now she happily spends her days among
friends, enjoying her time out in the community and the feeling of independence that it brings. “My mom used to sit at home and cry all day because she was so depressed,” Diane’s daughter, Kim, says. “Easter Seals helps her a lot. It helps keep her involved in her community.” The Easter Seals adult day care program serves adults who have experienced a disabling condition such as Multiple Sclerosis, stroke, brain injury, other physical limitations and conditions of aging. Transportation and meals are provided to the participants. The program provides a break from caregiving for families or those who work and have other responsibilities. Nursing care is provided to all participants. A full time nurse provides medical support including monitoring of blood pressure and blood sugar, coordinating with physician offices and arranging appointments. The nurse acts as a liaison between participant families, case managers and other health care providers. Easter Seals provides physical, occupational and speech therapies on site, which, if needed, can be added to the individual’s personal care plan. In addition, trained staff are able to help all participants with activities of daily living and personal needs “I’d be lost without Easter Seals. It is very nice,” says Diane, who comes to Easter Seals five days a
week. “I enjoy going out shopping, out to eat and to church with my friends from Easter Seals.” Easter Seals provides choices when a loved one needs assistance with daily living, and wants to remain living at home. Participants are encouraged to choose from a variety of activities such as arts and crafts, exercise, community trips, computer access and more. “My mom is always doing something different when I go to Easter Seals. She does a little of everything,” Kim says. “She is sad when she isn’t there.” Like all Easter Seal services, the adult day care services are comprehensive and individualized to meet each client’s needs, familyfocused to meet each family member’s concerns and have the goal of enhanced independence. The adult day care program offers a break for family caregivers responsible for a relative who is unable to function independently, but does not require 24-hour nursing care.

he “medical mistake rate” is estimated to be approximately 10%. This applies to all doctors— even the very best. A colleague from Boston recently developed severe malaise and a low grade fever after staying in a damp old house. She experienced severe weakness, neck stiffness and headaches for two weeks. After “self-treating” for two weeks with little result, she asked specialist colleagues for their opinion. No good ideas from multiple specialists. My colleague thought some bed rest would help, but none of her colleagues could cover her patients. She then experienced double vision. A neurologist was consulted who suggested (only) an MRA of the brain to exclude aneurysm. My colleague’s complaints of headaches and neck stiffness were disregarded. Her daughter (an attorney) did a Google search which actually provided better results than all the doctors consulted. Lyme Disease . . . or something linked to it. This is an important consideration in Delaware—Lyme is endemic. Given the symptoms, the radiologist looked at the history and conditions being evaluated and added an MRI of the brain with and without contrast. This ended up saving the doctor’s life. If the original order had been followed, no meaningful result would have been seen. An MRA does not actually “see” the brain. It looks only at the blood vessels to the brain. It does not evaluate the internal structures. The
A word about contrast: MRI Consultants uses only the safest Gadolinium contrast. It does not build up in the brain or body. Some contrasts are known to build up in the body over time. Typically, these are older forms or less stable forms. We have been a proponent of using the safest contrasts available since 2007. Incidentally, had we failed to use contrast, we would not have been able to observe the change it made in how we “saw” the lining of the brain (the meninges). Again, the error rate in medicine is approximately 10%. One way to help avoid mistakes is to review as much of a patients medical history as possible at every decision point. Dr. Chao does this for free when asked by any physician or his patient. We highly recommend you bring copies of imaging done elsewhere with you when you come to our facility. Dr. Chao is then able to compare and note possible changes. Technology has become so complicated that a medical consult for advice should be done before procedures are ordered. Also, as noted above, the way a test is ordered (whether contrast is used or whether the study, as ordered, can generate the information required) is of critical importance. Ordering a test properly is of great importance, but it is also important that the patient be able to remain still during the test. There is some software that helps us correct for slight blurring due to movement, but we understand that an anxious patient may simply be unable to remain still. For these people—and the very claustrophobic—we offer IV Sedation. It costs extra, but it can make the difference between getting a good image and blurry one. Call our office to inquire. As you know, stroke is one of the leading causes of death. It affects 20% of us. It causes severe prolonged morbidity. There is now a proven technology which can cure 35% of stroke victims! While this is currently available in New Castle County, it is not yet available in Kent or Sussex counties. The delay of even 30 minutes (the time of a helicopter to a different hospital) causes an increase in stroke and mortality. Time is brain. Look up STENT RETRIEVER TRIALS on the internet. Heart disease is the leading cause of death in this country. A cardiac surgeon taught me years ago that the amount of atherosclerosis of the aorta reflects on the smaller arteries. A paper published in 2013 directly found that aortic wall intimal thickness is related to your risk of dying from a cardiac event. Spread the word, a good MRI can give advance warning of heart attacks.

The Tooth Fairy (Just For Fun) S
pring is here, the flowers are blooming, and the warm breeze is blowing. There is new growth everywhere, and what new growth is more fun than a child’s new tooth. That is where the tooth fairy comes in. We all remember By fondly putting Yvonne DeLoache, DDS our newly lost tooth under our pillow and dreaming of the tooth fairy leaving some coins or a special gift, to be found in the morning. This lovely tradition has been enjoyed in many English speaking countries like the US, Canada, and England for many years. But what you may not know is that many countries and cultures have traditions surrounding the rite of passage of children’s tooth loss. I thought that it would be fun to share some of these traditions with you. Though the Tooth Fairy may have its origins in a book called, “The Tooth Fairy”, published in England in 1949 by Lee Rogow, the actual origins of a small tooth collecting character seem to date back centuries. Traditions surrounding the loss of baby teeth seem to fall in one of three categories; a character that collects the teeth and leaves a present, the child throwing the teeth in various locations, and the placement of teeth in special places. Other traditions similar to our tooth fairy are found in France and several Spanish speaking countries. The character in these countries is a small mouse. In France he is known as La Petite Souris while
the Spanish version is a tiny rodent called Raton Perez, Ratoncito Perez, or Raton de los Dientes. This tradition predates our tooth fairy by several centuries. The little mouse also takes the child’s tooth left under their pillow or in a glass of water beside their bed and leaves a coin or small present. Another interesting tradition is that in many countries children throw their newly lost tooth onto the roof of their home and ask that it is replaced by a new healthy tooth. Some make this request of the sun, some of a little mouse who will take it and leave a strong new tooth, and some hope that a bird will take it and leave another. There is also a tradition of throwing the lower teeth on the roof and the upper teeth on the ground hoping that the new teeth will grow in the direction of where the baby tooth is thrown. Lastly, there are traditions surrounding various placements of the teeth. In some Russian families, the baby tooth is placed in a mouse hole in hopes that the mouse will bring the child a healthy new tooth. In other cultures the tradition is to bury the lost tooth by a straight tree so that the new tooth will grow straight and strong. In some parts of Turkey, folklore states that the baby tooth holds the key to the child’s future. Thus, whatever the parents want a child to be they bury the baby tooth in or by that area. An example would be that if the parent wanted their child to be a soccer player they would bury the tooth in a soccer field. Though there are many variations and customs for celebrating children’s lost teeth, it is clear that the loss of a child’s tooth is an important rite of passage. In many cultures, this is a time to be celebrated and marked by a special ceremony, event, or ritual. So whether it is a fairy or a mouse or burying the tooth in a soccer field, it can be fun to celebrate with your child when their teeth are lost. It will make a memory that you can share forever. Left Atrial Appendage Closure Device A
trial fibrillation is the most common arrhythmia encountered in practice today and affects over 33 million people worldwide. One of the most devastating complications of atrial fibrillation is stroke. During atrial fibrillation, the atria (top chambers of the heart) do not contract normally and the blood pools inside the atria increasing the risk of clot development. If these clots migrate from the heart to the brain, a stroke develops. A patient with atrial fibrillation is five times more likely to develop a stroke over their lifetime. In light of this risk, most patients with atrial fibrillation are prescribed anticoagulants (blood thinner). Currently there are two major types of anticoagulants—Warfarin (Coumadin) and the Novel Oral Anticoagulants (NOAC). The three most common NOACs prescribed in the United States are Pradaxa, Eliquis and Xarelto. Both Warfarin and the NOACs provide good protection against strokes due to clots but have increased risks of bleeding and other side effects. While these medications reduce the risk of strokes due to clots, they can increase the risk of a bleeding stroke. Furthermore, many patients find anticoagulants to be inconvenient and costly. Randomized trials of Warfarin and the NOACs demonstrate that roughly 20-28% of patients end up discontinuing their anticoagulant therapy. The risk of major bleeds on these medications ranges from 2.1-3.6% per year. Based on these limitations, significant research has been dedicated to develop a non-pharmacological approach to stroke prevention in patients with atrial fibrillation. Several studies have demonstrated that in patients with atrial fibrillation not due to valvular heart disease (vast majority of patients) over 90% of the clots that develop come from the left atrial appendage. The left atrial appendage is a sack-like projection attached to the top left chamber (figure). The Watchman Left Atrial Appendage Closure device (figure) is the first FDA approved alternative to longterm anticoagulation therapy for stroke prevention in patients with non-valvular atrial fibrillation. This device is delivered percutaneously through the femoral veins (in the leg) and implanted directly into the left atrial appendage effectively occluding this structure and preventing clot formation. The procedure takes approximately 90 minutes and requires that the patient stay overnight in the hospital. At the end of the procedure, the patient
has only a small bandage over the puncture wound in the femoral vein. The risk of complication from the procedure is 3-4%. The Watchman device was evaluated in five clinical studies involving more than 2,400 patients and nearly 6,000 patient years of follow-up. This device was compared to warfarin therapy and demonstrated comparable stroke risk reduction. Importantly, when compared with warfarin, the Watchman device resulted in an 85% reduction in hemorrhagic (bleeding) stroke, 63% reduction in disabling stroke, and 56% reduction in cardiovascular death over longterm follow-up. After 45 days, most patients with the Watchman device were able to stop taking warfarin and over 99% were off warfarin at 1 year. Are You At Risk?
The ideal candidate for consideration of Watchman if you have increased risk implantation is a patient with non-valvular atrial fibrillation felt to be at high risk for thromboembolic (clotting) strokes, who has had significant bleeding complications from anticoagulation therapy or is deemed high risk for future bleeds. Pre-procedure Diabetes assessment requires a screening transesophageal echocardiogram (TEE) to assess the left atrial appendage size and shape. Patients will need to be Smoking on warfarin prior to the procedure and then at least for 45 days after the procedure. At 45 days, a repeat Family history of Vascular Disease TEE is performed and if this demonstrates good occlusion of the left atrial appendage, warfarin can be Family History of Aortic Aneurysm discontinued. Menopause – Don’t Fear The Change
By Dr. Melissa P. Broyles, D.O. Menopause is when a woman’s monthly menses (“meno”) pause (“pausis”) for a consecutive year. The average age women reach menopause is 52 years old. However, it can occur anytime between 42-56 years of age. The primary complaints about menopause are hot flashes and night sweats. Other common symptoms include:
vaginal dryness, mood swings, irritability, insomnia, depression, memory lapses and weight gain. Most cases can be diagnosed on the basis of symptoms and menstrual history. To confirm menopause, a FSH (follicle stimulating hormone) level can be checked. If the level is greater than 30, then menopause is confirmed. However, during the perimenopausal period (the time leading up to menopause) a woman’s hormones can fluctuate immensely and therefore laboratory testing of hormones is not reliable. The goals of treating menopause are to provide relief of symptoms and to prevent disease (i.e. osteoporosis and cardiovascular disease). An integrative approach to menopause focusing on diet, exercise, stress reduction and hormonal balance is essential. There is no need to fear the “change” as this holistic approach will help you feel better and prevent chronic disease. Nutrition plays a fundamental role in menopause treatment and prevention of chronic disease. Adopting a whole foods (unprocessed) diet consisting of vegetables, whole grains, beans, seeds, nuts, fruits, lean protein and healthy fats will decrease your risk for cardiovascular disease, diabetes and osteoporosis. Fermented soy (i.e. miso, tempeh) can be effective in relieving hot flashes; and new studies have found that a moderate consumption of soy (less than 11g/day) has a protective effect against breast cancer. the number of hot flashes by half within six weeks. So try flaxseed on cereal, yogurt or in a smoothie. There are numerous health benefits of regular exercise during menopause, namely a reduction in cardiovascular disease and breast cancer. Other benefits include increased bone density, improved heart function, reduced blood pressure, decreased cholesterol, increased energy and an improved sense of well-being. It is recommended that menopausal women exercise at least thirty minutes four times per week to achieve these health benefits. However, the requirement for weight loss during menopause is sixty minutes daily. Perimenopause and menopause can be a very stressful time in a woman’s life. Continued stress weakens the immune system and increases the risk of disease, including: autoimmune disease, cancer, diabetes and high blood pressure. Therefore, it is important to practice stress reduction techniques such as meditation, yoga, or breathing exercises. In addition to eating a whole foods diet, exercising and engaging in stress reduction techniques, vitamin and botanical supplements can be very helpful in menopause. Most women should take a good multivitamin (one that provides 70%-100% of the recommended daily allowances of vitamins and minerals) and fish oil. Fish oils (with EPA and DHA components) do not only aid in cholesterol reduction and mood elevation but also have been shown to decrease hot flashes and night sweats. Black cohosh is also very effective in treating hot flashes. In addition, there are numerous supplements for other individual symptoms of menopause, such as: Valerian or Hops for insomnia, Chaste tree berry for erratic menstrual bleeding, St. John’s Wort for depression, and Kava for anxiety. However, an integrative physician should be consulted regarding the proper dosing, safety, and potential herb-drug interactions. When diet, exercise, stress reduction and botanical treatments are not effective, hormone replacement therapy (HRT) should be considered. Understanding the benefits and risks of HRT is critical to determine if it is right for you. Any HRT should be used in the lowest dose and for the shortest duration. Bioidentical hormones (those that match our body’s hormones) prepared by compounding pharmacies offer several advantages, including: customized dosing, more hormone combinations and delivery options (i.e. creams, capsules, pellets). Again, this should be under the direction of a physician experienced in bioidentical hormone replacement therapy. So, don’t fear the change of menopause. any Delaware Hospice patients receive care at home, but sometimes there are circumstances that require 24-hour, short-term care to help manage a patient’s symptoms. Protecting Your Property Before, During and After a Flood

hile fire may be the more common concern among homeowners, your home is as much as ten times more likely to be damaged by water than by fire.* According to the Federal Emergency Management Agency (FEMA), floods including inland flooding and flash floods caused by seasonal storms and heavy rains, occur in every region of the United States. In fact, 90% of all natural disasters in the United States involve some type of flooding, estimating nearly $50 billion in economic losses each year.** Living in a high-risk flood zone can increase the likelihood of experiencing a flood, but being outside a high-risk zone does not mean homeowners are safe; flooding is always a possibility. Even if you do not live in a high risk area, it is worth looking into flood insurance because more than 20% of all flood insurance claims are filed in low to medium moderate flood risk areas.** It is important to recognize that standard Homeowners and Renters insurance policies do not cover flood damage; only a flood insurance policy will cover floodrelated losses. Flood insurance is affordable and easy to purchase. Federal flood insurance policies can be purchased directly from an independent insurance agent and the annual premium for a residential NFIP (National Flood Insurance Program) starts at just $112 per year and increases according to the level of flood risk and amount of coverage needed.** A growing number of private insurers also offer excess flood policies, intended to provide water damage protection to homeowners over and above the coverage offered by national programs. lue Hen Utility Services, Inc. was a business concept that was formulated in March of 2014 when Michele Reynolds’ current business, Shelly Sons Electrical Contracting, was contracted to perform high-medium voltage work. From its start, Shelly Sons subcontracted its customers’ highmedium voltage electrical work to other firms, firms with the technical training and equipment to handle that highly-specialized work. Through that high-medium voltage sub-contracting experience, Michele learned a single fact that would change everything for her business- there were NO Delaware based utility service firms geared to self-perform high-medium voltage work. When storms or other issues breached power lines in the First State, all of the repair work was contracted to out-of-state firms. A Delaware-based utility service company would have a faster response time to address power outages than those more remote out-of-state firms, keeping First State communities and businesses up-and-running when power crisis strikes. Throughout this process of working for several clients to complete their distribution projects, they had the opportunity to collaborate with numerous vendors, supply houses, and business owners that rendered one common theme. There is an urgent need for additional utility service contractors to meet the demand of increased electrical service reliability. To bring her high-medium voltage plans to fruition, Michele needed a sharp team that was up to the challenge. Her husband and longtime business partner Michael Reynolds, co-owner of Shelly Sons

Spiders who live in homes help keep unwanted insects out of your house. So the next time you see one, instead of killing it you might want to give this some thought? Would you rather have a bunch of insects coming into your home and flying around everywhere or have the spiders catch them and eat them them for dinner? With a little education, I got over my hatred of spiders and so can you! But I still get freaked out getting a face-full of spider web.
If we got rid of all the spiders in the world, there would be an excessive amount of insects like flies. Each spider, on average, eats about 2,000 insects a year. If all the spiders are gone, then many of these other insects will destroy of our crops. Spiders also are an important part of the diet for birds and lizards. So in a way, if you’re a bird-watcher...you should also be a “spider-thanker!”

I remember when I was a young girl, I was in my basement and a spider was crawling all over my arm without me knowing, oh but when I found out... I freaked out! Even the tiniest of spiders scare me. I hate the fact that they can drop down on me, crawl on me with their creepy legs, and make webs wherever they please. Growing up I always hated them. I’m still not crazy about them now but I have a better understanding of why they are on this planet.

or students with Attention Deficit Hyperactivity Disorder (ADHD), simple tasks such as getting ready for school and finishing a homework assignment can be a stressful battle. Jim Power of the Newark, DE Huntington Learning Center reminds parents of children with ADHD that approaching these issues with patience and consistency can make a tremendous difference. “Your primary goal is to help your child be successful in spite of some of the challenges he or she faces with time management and attention,” says Power. He offers a few strategies for parents to keep ADHD students focused and on task:
Focus on the positive Negative attention and threats can exacerbate a problematic situation with an ADHD child. Instead, point out the things that your child is doing right and reward effort and positive behavior rather than solely focusing on your child’s failures.
Use checklists for daily tasks and routines Relying on checklists for multistep tasks can make a big difference for ADHD children who tend to lose focus even during simple daily routines. For example, an after school checklist might include steps like: empty backpack folders, hand Mom and Dad important papers, bring homework folder and pencils
to desk and grab snack. Put tasks in order or even assign lengths of time. Use checklists for other routines as well, such as homework sessions and chores.

Use timers It’s common for ADHD children to lose track of time or take a long time to complete uncomplicated tasks. Give your child a timer to use along with his or her daily checklist. Break down big tasks into smaller ones and assign time limits for each. For example, if your child can comfortably complete homework in 45 minutes, break down each subject even further: 15 minutes for math, 10 minutes for writing and 20 minutes for reading.

Use focus-enhancing tools Some settings and tools can promote concentration and keep children relaxed and focused. Does your child need a little background noise or music to concentrate? Does he or she prefer sitting on an exercise ball instead of a desk chair? Some ADHD students think best when fidgeting or playing with silly putty or another fidget/sensory toy or tool. Others are most successful when their workspace is clear and uncluttered. Pay attention to how your child works and talk about what could help.
Boost your child’s confidence Children who struggle to get things done and stay focused often struggle with diminished confidence. Help your child recognize what makes him or her special and always compliment his or her efforts and progress, no matter how small.

istant learning or correspondence courses and schools have been around since the early 1900’s. However the new buzz words are cyber, cyber school, cyber education and online learning. So what does this actually mean? Many higher education colleges and universities offer courses or certificate based programs online, for its students. This provides a flexible option for students to attend school on their time and not have to worry about losing days off from work and sometimes
assignments, as well as holding interactive live class sessions.

Cyber schooled students are required to log attendance every day, submit assignments, attend online live lessons and complete the state mandatory testing. Using different types of programs, Moodle, Blackboard, etc students receive their education through an electronic device and the internet. Many cyber schools offer socialization opportunities including field trips, international travel and local community gatherings. While cyber school may not be the right fit for all families, it does afford families the safety of schooling in a home environment. Many students choose cyber school due to its flexible but structured approach to learning. Not all students learn the same, so it’s important to find a model of education that works for your family.

ime and time again we hear parents tell us how frustrated they were because they could not help their child find success in school. It was this exact frustration with her own son’s education that drove Pam Draper, the founder of Gateway Lab Charter School, to establish a school where students with disabilities could be successful. One day in 2006, Pam was watching television when she saw a feature on Sally Smith, and the Lab School of Washington® in Washington, DC. Smith started The Lab School in 1967 when she could not find a suitable school for her son with learning disabilities. After seeing that Smith was helping others to establish schools using Lab’s methodology, Pam reached out to her about starting a similar program in Delaware. While the D.C. Lab School is private, Pam felt strongly that there needed to be an alternative for parents of students with disabilities that could not afford to pay $30,000 per year for their child’s education. After meeting with Sally Smith and Lab School staff and parents, Pam returned to Delaware to assemble a group of individuals with a passion for helping struggling students. For the past eight years, Mrs. Draper and the Gateway Board of Directors have worked successfully to open and develop Gateway into a model school where kids with learning disabilities and differences can succeed. The Gateway Lab Charter School is a state approved, public charter school centrally located in New Castle County. The school is unique, being only one of two of its kind in the state of Delaware.

Don’t Judge a Book by its Cover By Ara Atkinson-Skinner
How many times have you heard the phrase, “Don’t Judge a Book by its Cover”? When wearing one of my humanitarian or artistic hats, that phrase has been invaluable to me on so many levels. Some of our judgment calls are so subtle and engrained in our subconscious from birth due to the environment in which we were raised. All of the exposure or not to religion, spirituality, arts, education, politics, culture and list goes on to develop prejudges and beliefs that will influence our decision making system. You are probably wondering, “What the heck does this have to do with marketing and selling my books?” and the answer is everything!

So, if your name isn’t Stephen King or James Patterson, chances are you need to think about your cover and ask yourself hard questions, starting with, is my cover going to speak to my target audience? Do I even know who my audience is and even more important, will my audience be able to immediately resonate with my cover? Stephen and James were picked up early in their careers by a publisher with marketing experts looking out for them and the cover is a very important part of the launch of any book and usually decided heavily by the publisher based on years of experience of knowing how to appeal to their target audience (publishing house protecting their investment). You may not have that luxury of being picked up by a publishing house going right out of the gate, however it doesn’t have to be that way forever. So for now, you need to protect yourself and make the effort and the investment in your cover that will attract your intended
Authors, especially new authors self-publishing, need to be aware of the judgment and the competition out there marketing to the same audience as you and maybe sharing the same shelf space. Nothing is more frustrating than getting your amazing works overlooked by maybe a lesser quality works, because the other book had a more artfully designed cover capturing the attention of the buyer. The only covers that might escape this kind of scrutiny, might be your religious books, education textbooks and your proven authors who can just write their name on a paper bag cover and they will sell like gold! However,

Who Do You Trust?

y continuing chronicle of interviewing women has produced some extraordinary points of view, opinions, feelings, and ideas about the financial well being of women. Delaware Commission for Women Hall of Fame Inductee Beverly Stewart, President of Back to Basics says “more than ever before, women are not only the bread winners, but are the decision makers; consumers and buyers. They command more respect and are very capable people.” She goes on to say “they do need help with the planning piece and looking at the long term.” Lani Nelson-Zlupko, PhD of LNZ Consulting who is a therapist, coach and consultant says “A husband is NOT a plan! Women need exit cash and walking shoes to maintain their independence.” At age 65, women can expect to live, on average, an additional 20 years.* The U.S. Census Bureau indicates that women own almost 30% of nonfarm businesses. Moreover, according to recent statistics, 42% of older women are widowed, 11% are divorced, and approximately half of all women age 75 and older live alone.** Additional points mentioned by these ladies, were that many times women don’t have the time, inclination, or interest to take care of the important financial details. So what’s a gal
to do? I would submit to the ladies of the jury that consideration should be given to hiring a qualified wealth manager. He or she should focus on saving you taxes, preserving your wealth, helping you pass your wealth along to your heirs, protecting your assets from being taken away unjustly, and helping you make meaningful gifts to charities that you support. Only a small percentage of so called “financial planners” actually do all those things. Most only do one of those things well.
Finding that all important person “of trust” should begin by asking about his or her qualifications. A Certified Financial Planner (CFP®) or Chartered Financial Consultant (ChFC) is a good place to start. Each of those designations requires much study, passing tough exams and above all holds that person to a high degree of ethics. Even so, it does not guarantee you that you’ve got “the best” but it’s a great start. Do you feel comfortable with that person? Do they listen well? Do they answer your questions with respect and in a language that you can understand? Don’t be shy about asking how they are compensated as they should have no problem explaining those important details too.

A court order only permits an ex-spouse to sue the obligated spouse for nonpayment. The Court has no authority to remove a spouse’s liability with the bank. The only way to remove a spouse’s name from a mortgage is for the other spouse to assume the mortgage entirely, refinance, or sell the home. Dividing Marital Equity All marital assets and debts need to be considered in property division. In many instances, it is unlikely one spouse would have cash to “buy” the other spouse out of the home. One spouse may keep the home and related mortgage and the other may get more in investment and retirement accounts. The spouse keeping the home could also take on more marital debt. In property division, total marital assets and debt are split, but that’s not necessarily half of each asset and debt. There are several options available in dividing total marital equity.
Keeping the Home If you are contemplating keeping the home, be sure to remove the emotional attachment to the home, and truly consider whether it makes financial sense. Can you afford all the costs to stay in and maintain the home? Will you be able to pay for any major repairs? Keeping a home you cannot afford will only lead to financial troubles later. If cash flow is a concern, it may be unwise to give up more liquid assets, such as investment accounts, just to keep the home. Remember, your home does not provide you with cash, and is typically not an investment for retirement. Giving up assets such as retirement accounts to keep the home may not make long-term sense. Letting go of a retirement account, which will provide you with future income, is risky. You must look at your long-term goals and needs, as well as short
term major expenses that may be looming. Tax Implications There are tax implications to consider as well. The tax implications relating to the gain on the sale of a home are much different than the tax implications of selling investments or drawing from a retirement account. As a single individual, you typically can exclude $250,000 of gain on the sale of your home. However, selling your home will generally cost you 10% in selling expenses. Drawing from a pension account will result in taxes at ordinary
rates, which vary depending on your income. You could also pay an additional 10% penalty on pension funds you withdraw if you’re under the age of 59 ½. Gains on investments are taxed at lower capital gains rates. However, depending on your level of income, you could end up paying additional investment income taxes. Accordingly, you should seek the advice of a tax advisor to understand the tax costs associated with the assets you receive and their impact on the net value of the asset you receive. Evelyn M. Don’t Do It For Yourself, Do It For Them

oof! You’re gone! Just like that – you’re gone. No cloud of smoke, no nothing. Gone from the face of the earth. No, you weren’t abducted by aliens. You didn’t get swallowed up by a giant sinkhole on a dark, deserted road. You weren’t spirited away by some invisible force. No, nothing as mysterious as that. You’re gone because you died. Suddenly – without any warning. No signs, no symptoms. No notice. No nothing. You were healthy, or reasonably healthy, one moment and dead the next. Too often, that’s how quickly it happens. A massive heart attack. An accident. An aneurysm. There are any number of reasons you could be here one moment and gone the next. And when death happens like that – suddenly, unexpectedly, a trail of disaster can be the biggest most difficult legacy you leave for your loved ones. A legacy that gets dumped right at their feet – crippling them and rendering them unable to move at a time when they will be expected to stand tall and make sense of the legal and financial morass you have bequeathed to them. Is that really what you want for those you love? Their world as they knew it has been forever altered because you will no longer be a part of it. It’s come to a screeching halt; they have to keep moving at a time when it might take all of the inner strength they can muster to just wake up, put one foot in front of the other, and navigate the choppy waters of their day without you. Why place an additional burden on them by asking them or expecting them to take care of your final business matters when you can do most of it yourself before your death? Even when you have been forewarned and are facing the eventuality of death from a terminal illness, you can take this burden from them. Our lives have become ever so complex and complicated. Technology was supposed to make our lives simpler, and in many respects it has. When was the last time any of us saw a paper paycheck? When was the last time you were actually inside the lobby of a bank and not just in the anteroom to go to the Automated Teller Machine (ATM)? We can conduct just about all of our banking without ever setting foot in a bank building. We can order our groceries and have them delivered. We can shop at any of our favorite stores without going to the mall while we’re still in our pajamas. We can email our physician in the wee hours of the morning when something just doesn’t feel right, and have a phone call response when they arrive at their office in the morning. If we can’t sleep at night there is a staggering array of cable channels to entertain and relax us.

But all of this technology that has so vastly simplified our lives comes with a cost. It has also made our lives more complex and intricate. Our lives are a maze of highways and byways that we crisscross and travel daily while on autopilot until something causes it all to come screeching to a sudden stop. In this case – death is that determining factor. Benjamin Franklin wrote, “In this world nothing can be said to be certain, except death and taxes.” They are both inevitable; however, the intricacies of taxes, financial issues, and other final business matters can all be mitigated by carefully planning ahead of time. Another important phrase is widely attributed to Alan Lakein, author of self-help books on time management. He said, “Failing to plan is planning to fail.” So take matters into your own hands. Be proactive, plan now and take control of your affairs. It’s not just about a last will and testament. Sometimes it’s the smallest, most minute detail that can wind up being so problematic. Does your family know all the people you might want notified of your death? Do they know who you want to have your heirloom collection? Who gets your father’s military medals? Simplify things for them now. Don’t leave them guessing, wondering, and missing things that can be costly mistakes. Women Workers: Bargaining For Yourself A
lthough women make up nearly half of the American workforce, they still earn nearly a quarter less on every dollar than their male counterparts (according to the latest government statistics, women comprise 47% of the workforce, but earn only 78 cents on every dollar earned by a man). Seriously? The reason for this earnings gap is complicated, of course, but setting aside the damage wreaked by millennia of political ignorance, much of this enduring gap may be caused by subtle differences in how men and women communicate and how they view their roles in the workplace. 1. Everything Is Negotiable:

In the marketplace, competition rather than consensus usually wins out and these differences can result in women missing out on, or botching, opportunities for better pay and benefits. Even so, creating enduring relationships and getting people to agree are still strengths, rather than weaknesses. As time passes, and society becomes used to seeing women in positions of corporate power, the job inequalities between the sexes will hopefully diminish and may even disappear. In the meantime, however, women can improve their chances for higher wages and better perks by shifting their view from what is acceptable, to what is possible. Three basic ideas to keep in mind in the context of negotiating for a better workplace experience (1) everything is negotiable, (2) be prepared, and (3) be ready to walk away.

At least, it is in the workplace. Workers often assume – wrongly – that salaries and benefits and job opportunities follow a specific formula or are set in stone. Another adage to keep in mind is “Nothing Ventured, Nothing Gained.” After all, the worst that can happen is rejection (which is never final). And, to borrow yet another catch-phrase, failure is just the chance to begin again, more intelligently.

For women, the challenge is to overcome their inner fear that self-promotion is unattractive. Indeed, the opposite is true - a successful woman is very attractive! Regardless of gender, the two most important rules to successful negotiating is organization and objectivity. Put together a list of
An essential element to every negotiation is remaining strong, focused and removing emotion from the mix – facts strengthen a position, while emotions weaken it. When ready, start negotiations from a position of grace – asking, rather than demanding. Similarly, be sure to listen – showing respect and consideration for the other side will help speed up the process, as well as increase the likelihood of success. Take time and be prepared to compromise: each side has a stake in the deal, as well as the desire to avoid chaos and to maintain balance. Lastly – and most importantly - be ready to walk away if the deal is bad. Women have a lot of trouble saying “no” because they want to keep everyone happy (even at their own cost). When selfadvocating, it is necessary to get over this tendency as quickly as possible! Again, maintaining grace

and professionalism throughout the process - even when walking away – increases the likelihood of success the next time around. Throughout this process, try to maintain balance – not giving up more than you get, and not settling for less than your bottom line. And if you begin to lose confidence, focus on what you would do and how you would act if you were negotiating for someone else rather than yourself. Keep in mind that a strong start will make the rest of the race easier, both in work and in life.

Delaware Workers’ Compensation Rights Protect Undocumented Workers, Too!

ith the approaching presidential election, the issue of immigration has been a hot topic. So let’s take a look at the rights of undocumented workers injured while employed in Delaware. Employees, both documented and undocumented, are susceptible to getting injured on the job. Workers who are injured on the job are generally eligible for workers’ compensation benefits in Delaware; this includes undocumented workers. Recently, the temperature towards undocumented workers has taken a favorable turn in Delaware decisions from the Department of Labor. In order for an employer to prove its case that an injured worker is capable of returning to work, the employer must present evidence showing that there are regular employment opportunities available within the employee’s capabilities. This is often done by way of a labor market survey which is a report prepared by an expert showing local jobs currently open that the employee is capable of performing or securing. Recently, one employer tried to terminate an employee’s disability wage benefits by arguing that the employee was capable of returning to work. The employer offered a labor market survey showing that there were jobs available within the employee’s capabilities. However, the employee was an undocumented worker. The jobs that the employer offered to the Board as “within the employee’s capabilities” failed to consider whether the employee was required to be documented as a condition of employment. The employer went on to argue that because they hired the www.TheWomensJournal.com
employee without looking into their immigration status, other employers likely would as well, so immigration status should not matter. The Department of Labor did not agree. Rather, the Board decided that immigration status is a relevant and appropriate factor to consider when determining whether an employee is capable of performing or securing a job. Thus, in order for the employer to prove there were jobs available within the employee’s capabilities, the employer must offer available jobs that consider the immigration status of the employee. In other words, while it may make it more difficult for the employer to prove that there are available jobs depending on the immigration status of the employee, that is the employer’s problem and not something that should fall onto the employee’s shoulders. Undocumented workers often don’t know where to turn or what effect their immigration status has on their workers compensation rights. An attorney skilled in these types of cases can explain all of the benefits that are due to you and will fight to make sure you receive those rights. But when a rainy spring Saturday rolls around and your weekend tasks are temporarily postponed, I’d like to suggest another, very important activity to add to your seasonal “To Do” list: set aside the time to review your automobile insurance policy to be sure it is adequate and up to date. I can hear a chorus of groans at the above advice, and I can even visualize eyeballs rolling as I write this. A Bouquet Is More Than... are “the stars of the cast” and the sweetheart roses, stephanotis, ranunculus and perhaps Queen Anne’s lace take on the roll of a supporting cast! The greens and filler flowers, such as heather, limonium, wax flower or babies breath carry out the part of the “extras”. Yet all are integral to the entire production or composition or bouquet. A
bouquet is more than ~ ...an assortment of flowers tied with a ribbon or plunked in a vase. The creation of a bouquet combines the art of composition and the principles of engineering. The finished product has to appeal to the eye as well as remain intact over several hours [or in the case of a bridge, for many decades!]. If a bride decides to have her wedding flowers preserved the matter of having every bloom at its peak and every adornment firmly attached and every ribbon securely wrapped is doubly important. The bride and her florist are an artistic team. Most often a bride has a vision in her mind accompanied by a picture from a magazine or a photo from a friend’s wedding or an image from an online site of a particular style of bouquet, and she may have a very specific list of flowers that she loves and are meaningful to her. Enter the floral designer who knows what flowers on the bride’s list are available in the season when the wedding will take place. Lilies of the valley, tulips, peonies, & dahlias are not available year round making it necessary to suggest an alternative for a bride who has her heart set on any of these. Just as an artist selects various objects for a stilllife painting, a floral designer must select flower forms which compliment each other when the bouquet is assembled. One would not want flowers of all one size like football mums and peonies and sunflowers in a single bouquet! Nor would daisies, alstromeria and minicarnations create an eye-popping composition. If we think of a bouquet as a theatrical production, flowers such as peonies, hydrangeas and dahlias

grandmother’s wedding dress is to be used to wrap the bouquet, or if a sentimental item such as a locket is to be intertwined with the flowers, these items must be brought to the designer in advance of the wedding week. The creation of a bouquet when the floral designer sits down to put it together is much like manufacturing a car on an “assembly line”! First, all of the flowers have to be prepped. Having arrived at the flower shop early in the week, all stems have to be cut and hydrated and the flowers of each variety have to be counted and set aside in the cooler. At assembly time it is critical that the designer does not discover that she is 5 stems short of the roses or hydrangeas or whatever flowers are “the Stars” in the wedding party’s bouquets!
The creation of a bouquet, like the main course at a gourmet dinner, is a combination of art and science. The entree is equivalent to the bouquet; each individual flower in the bouquet is an ingredient in the dish. It includes a featured item, a number of secondary ingredients, an assortment of spices and herbs and perhaps a garnish. When the floral designer meets with a bride there some essential questions that have to be answered:

The color of the attendants’ dresses will partially dictate the variety of flowers to choose from. The date of the wedding will dictate what seasonal flowers will be available. The number of attendants, the number of guests and whether arrangements will be needed at a ceremony site as well as a reception site will dictate how much of the overall budget is available for flowers.

The creation of a bouquet is the product of a bride’s vision and the floral designer’s expertise. Once the bride and her florist have decided on the specific flowers to be used, the bride steps out of the picture. Even though the wedding may be scheduled for a year in the future a floral designer makes her “shopping list” of the flowers that will need to be ordered 3 - 4 weeks before the actual delivery date. He received his Bachelor’s and Master’s degrees at Eastman School of Music in Rochester, NY. He has been shooting semi-professionally since 2005 but didn’t officially open his own business (Adelson Media, Inc.) until 2008. In addition, to photography he has a background managing marketing and social media for an online retailer. When an opportunity to shoot music events presented itself, he decided to follow his passion and to go full time with Adelson Media, Inc. Once he made the decision to pursue professional photography full-time, He enhanced his arsenal by joining the National Association of Photoshop Professionals. Shannon said that the process of taking pictures was always been fun; but recently he discovered the joy and wonder of making portraits. Using creative compositional elements like light, color, framing, gesture, expression, and depth of field, every image can tell its own story.

Sheronda Wesley has more than 27 years’ experience in the income tax services, banking and accounting fields. Sheronda obtained her BS in Business Administration from Widener University and a dual Master’s in Accounting and Financial Management from the University of Maryland. She has a federal license to represent taxpayers before the IRS as well as a secret security clearance. Sheronda has owned her own tax preparation firm, TAXcellent Financial Services, since 2007. She also practices tax litigation and representation as an Enrolled Agent. Sheronda is passionate about using her knowledge and experience to make a difference in the lives of others providing free tax preparation and training services to non-profit organizations.
Working with clients in such a highly confidential capacity, Sheronda often found herself in a position where families would ask her for assistance with the business matters of a deceased family member. Being able to provide this assistance was a natural transition to her new business venture, Final Business Matters. Michele Reynolds was born and raised in Wilmington Delaware. She is President of Blue Hen Utility Services Inc. and also primary owner of Shellysons Electrical Contracting along with her husband and business partner Mike Reynolds which was founded in 1998.Michele’s drive and passion for entrepreneurship did not stop there. She was determined to make a positive impact in the State of Delaware that would fill a void and create job opportunities. Mark J. Aitken, a native Delawarean, is the Co-owner and Senior Vice President of Horizon Services. Katy Wolfington
Katy Wolfington has been working in design for nearly 30 years. Her love of art, culture and architecture led her down a winding path, which eventually blossomed, into Waterbury Kitchen and Bath. She purchased the business in 2004 when it was a home building firm. Katy, wanting nothing to do with building homes from the ground up, turned the business into a kitchen and bathroom design center. She created a showroom that is unlike any other. It really is worth going to see. Katy’s background in interior design, which included working for Ralph Lauren, has contributed to her successful business in the heart of Kennett Square. Katy has a diverse client base and can embody individual clients desires and tastes. She says the most important part of her designs is that each one is unique and something you can’t get anywhere else. Waterbury Kitchen and Bath specializes in everything from full bathroom and kitchen remodels to smaller scale projects such as replacing a backsplash and counter tops. All designs, whether contemporary or traditional, are a collaboration of the customer’s vision and Waterbury’s design team’s expertise. I’m Donna Richardson. I am a native Delawarean living here all but 1½ years of my life. My husband, Roy and I started Royal Pest in 1976. Royal Pest started as a family business. I was an Assistant Manager of a Newark clothing store. I’d come home, take care of my family, then take care of my Royal “office duties”. We have built Royal Pest into a company that operates throughout the eastern United States. We are proud to say that Royal Pest is the #39th largest pest control business in the country according to Pest Control Technology Magazine. (Over 20,000 such companies) Roy and I have two children, Wayne born in 1969 and April in 1972. I worked at Royal until 2000. After our son’s death in 1999, we became legal guardians of his four children and eventually I stayed home to raise the kids. I am an avid nature photographer. My photos have won awards, contests and have been published. In 2004, one of my photos was in the Delaware 24-7, which was an extension of the America 24/7 project, an unprecedented digital photography venture that featured the work of more than 25,000 local photographers in all 50 states. Last month, one of my photos was awarded a Top Ten in the Pest Control Technology 14th Annual Photo Contest. Roy and I are true environmentalists. We run Royal Pest as a planet-friendly company, have a monarch butterfly in our logo, and always consider the environment when controlling pests. This year we are celebrating Royal Pest’s 40-Year Anniversary. Dr. Heidi Gough
Dr. Heidi Gough is originally from England, but moved here in 2001 as part of a work exchange placement. This placement was originally for 18 months, but was extended several times with different position opportunities. As time went by Dr Gough fell in love with the USA, in 2015 she gained her American Citizenship. Heidi, as she prefers to be known, currently works for PA Leadership Charter School as the Director of Marketing and Communications. Her passion for education is clear; she started her career in nursing, taking care of children with profound needs in a correctional facility. Her role became more of a teaching position than nursing, so she returned to school and became a certified special education teacher. Her schooling did not end there; she began pursuing a Doctor of Medicine degree (MD) but switched over to a Ph.D. in Organic Chemistry as she felt it fit her career path better. Since arriving in the USA, Heidi has been in both nursing and educator roles; she has continued to school herself in the areas of education and marketing, this lead to her current position.
As the Director of Marketing and Communications, Heidi manages a team of five across PA. She believes that collaboration, communication and a sense of community are important factors in the workplace. Her team is very knowledgeable and work closely together to support the needs of the families they serve. When you talk to Heidi about education and schooling, you can see that it is the very fire that burns within her. She wants families to feel empowered and educated about their child’s schooling. Some parents may not realize there are several school options available to them, and while PA Leadership Charter School may not be the option for every family or child, they leave feeling satisfied, educated and at ease. Heidi believes in the words spoke by Maya Angelou - “People will forget what you said, people will forget what you did, but people will never forget how you made them feel”.
Michael Hughes
Michael Hughes is the Vice President of Servpro of Kennett Square/Oxford. Servpro is a franchise that provides clean up and restoration services to both commercial and residential properties. Their focus is to make it “like it never even happened” by providing highly trained professionals that respond quickly to emergencies and to minimize business interruption. Michael has been with Servpro for twenty years; starting out as an employee then quickly becoming an owner. He has been trained in residential and commercial drying, fire and smoke clean up and mold remediation. He also manages and runs the commercial duct cleaning division of the company. Servpro of Kennett Square/Oxford is also a part of the Servpro Corporate Storm Team division. When a hurricane, flood or winter storm does damage to an area, the Storm Team moves in to assist the inundated local Servpro franchises. Michael and his team have been involved in events such as Hurricane Katrina, Irene and Sandy. They have traveled to the Dakotas and Chicago for flood cleanup and have traveled as far as San Diego for cleanup of the wild fires. Although they are always willing to help other franchises, Servpro of Kennett Square/Oxford’s top priority is taking care of their local customers in the Southern Chester County and surrounding areas.

Q: Is it painful? A: It is an annoying discomfort initially but it is doable and we do use topical numbing agents. We are understanding of the anxiety and discomfort during the procedure so we do what is necessary to keep you comfortable.

Q: What can patients expect after their procedure? A: There may be slight swelling the day after. A cool compress is all you need, use as needed. During the consultation we go over home care and the healing process. After the treatment we give you products to take home to aid in the healing process. We also send you home with a hard copy of post treatment instructions. All of this is discussed before we perform the procedure. Also, it is very important to know the first week treatments will be much darker than what the end result will be. The pigment adheres to the surface cells and that flakes off and will typically take 5-7 days for everything to settle down.

Consult your Artist on how long it will last. A guideline is eyeliner lasts the longest, 3-15 years, brows the least, 1-3 years, and lips, 3-5 years. Dark colors lasts the longest. Q: How long do procedures take? A: It varies per area, but you need to know that it takes a minimum of two hours for a new procedure, but timing will vary. This is your face, it’s not to be rushed. Q: How do I choose the right Artist? A: Our consultations are longer than the actual procedure. We draw on what we’re going to do, we discuss pigment, post treatment and the healing process, it all has to be perfect before we perform the procedure.

Q: How can I identify a bad Artist? A: If the atmosphere looks unkept. Cleanliness is number one. Are there other clients there? Also, pay attention to the attitude of the Secretary. How you are treated as a client from the beginning is an extremely important indicator of how your entire treatment will be handled, including post treatment. Ask if they are certified and if their certifications are not prominently displayed in the office, ask to see their certifications. Also, ask if they have referring doctors. The most important thing clients need to understand is Permanent Cosmetics is a self-regulated field which is why there is potential for issues, therefor buyer beware. Your Artist should not only be a member of the Society of Permanent Cosmetic Professionals (SPCP) but they should also have apprenticed under an established, credible professional.

Delco Chamber Celebrates Technology And Innovation Happening In The County
T
he Delaware County Chamber of Commerce is proud to partner with Philly Tech Week (PTW), a week-long celebration of technology and innovation in the Greater Philadelphia area. Last year, over 25,000 attended 150 events throughout the city – and one of those events was created by the Delaware County Chamber held right in the county seat - Media, PA.

If you missed our 2015 event, join us this year, as the Chamber will be hosting a series of events with our members who have experience and expertise to offer, showcasing all we do here in the STEM fields (Science, Technology, Engineering, and Math). If you are a fan of tech – or just curious to network with this industry group - we invite you to join us at an upcoming PTW16 event programmed within the county. Many of the events are free and open to the public, offering services that are helpful to business people and local citizens alike. One initiative we are particularly proud of highlighting this year is being held by our member Delaware County Technical High Schools (DCTS). DCTS is the home of Delco Girls CODE. For 12 weeks Delaware County Technical High Schools is running a free coding workshop implementing the global technology entrepreneurship program for girls, Technovation. During the Delco Girls CODE – Technovation Challenge teams of girls in grades 6th – 9th work together to develop an app that addresses a problem in the community. They will join a network of over 3000 girls from 28 countries around the world who have entered the 3-month competition in the past. During PTW, teams from Delco Girls Code will pitch the apps they develop to a panel of local women leaders working in business or technology for feedback. 58

I
t’s simple, if ever there was a time in the history of our country that we need to feel connected to each other, it’s now. Never before have so many felt so isolated as we learn to live in the digital age where we navigate ALONE. But we have found a place that actually brings people together – it’s the Center for the Creative Arts in Hockessin, Delaware. Last fall they started Yorklyn Live – Open Mic night and it has showcased the local talent of musicians, comedians and even puppeteers and brought a community of diverse people together. By fostering an environment of acceptance and trust, the Center for the Creative Arts has created something special where a performer can take the stage and take a risk to put it all out there without fear of judgement. In fact J Christian was one of the first performers to take the stage and help this event grow. While getting to know the Center for the Creative Arts, he learned that they feel passionate about the arts and want to make this an on-going community event that brings people together. Making it a source of pride for residents (participants and non-participants alike) in this community and increasing their sense of connection and trust. We know that the arts have been proven to improve health, mental well-being, cognitive functioning, creative ability and academic performance. What may be more important today is that the arts increase the scope of individuals’ social networks as we have seen at the Open Mic night. And although training in the arts is important, for us – the sponsors of this campaign, it’s about PRIDE, CONNECTIVITY, and THE HEALTH AND WELLBEING of our community.

There is a buzz in town about their latest program which truly shows their creativity and passion for the arts. It’s called Yorklyn Live – an Open Mic Night. Held the third Thursday of the month, this event has featured a variety of local talent including soloists, musical groups, comedians and even puppeteers. Thanks to the generosity of a wonderful donor, CCArts now has an amazing sound and light system and transforms their auditorium into a stage and a café setting with drinks and food and candlelight. CCArts has kept the program free to allow the community to come out and enjoy it and just last month aver 95 people attended!
Ours is A Phoenix Rising from the Fire In December 2014, the staff discovered a major case of embezzlement by their trusted Office Administrator. The amount of the theft was staggering with over 6 figures in losses. When most organizations would have been reeling and forced to close their doors, CCArts dug in and stepped it up. Board, staff and teachers doubled their commitment and worked even harder to keep things running and the organization was fortunate to have two generous donors provide emergency funds.

Ovarian Cancer...What All Women (and Those Who Love Them) Need to Know! Each year more than 21,000 new cases of ovarian cancer are diagnosed in the United States, resulting in over 15,500 deaths. It is the deadliest of the gynecologic cancers, affecting approximately 1 in 70 women. Because there is no reliable diagnostic screening, it often goes undetected or is misdiagnosed until it has advanced to the later stages. Less than 20% of cases are discovered in the early stage. Due to late diagnoses and a very high recurrence rate (70-90%), less than 50% of women make it to the 5 year milestone.
Ovarian cancer does not discriminate and can affect females of any age and ethnic background. It continues to be one of the most underfunded cancers and the lack of public awareness and education is of critical concern. Although the symptoms of ovarian cancer often are not intense in the early stages, they are NOT silent. Once thought to be a ‘silent disease’, research indicates that 95% of women DO have vague but persistent symptoms. These symptoms are not always useful for diagnosis since they may be indicative of other less serious conditions. If any of the following single or combination of symptoms persist for more than 2 weeks, consult your physician and insist that a manual exam and transvaginal ultrasound be performed. If any abnormalities are detected, a referral to a gynecologic oncologist may be in order. • Bloating • Pelvic or abdominal pain • Difficulty eating or feeling full quickly • Urinary symptoms (frequency or urgency) Other symptoms may include: • Extreme fatigue • Indigestion, heartburn, or upset stomach • Shortness of breath • Lower back and/or leg pain • Change in bowel habits (constipation, diarrhea) • Unexplained weight gain or loss • Menstrual irregularities (including post menopausal bleeding) • Pain during intercourse

Did You Know… Even if your ovaries have been removed, it is possible to develop Primary Peritoneal Cancer (PPC), a rare form of cancer, which is closely related to ovarian cancer. The peritoneum is a thin sheet of tissue that protects and supports the abdominal organs. When certain cells transform into cancerous cells, the result is PPC. The cause is not known. Symptoms are more gastrointestinal in nature and may include: · abdominal bloating · changes in bowel habits · an early feeling of fullness after eating Nausea and vomiting may occur when bloating is severe. Medical evaluation and treatment is the same as for ovarian cancer and recurrences are common. There is no screening for PPC.

Factory farming is the norm in this country. These animals, seen only as property and profit are made to live horrible lives and frequently mistreated. With supermarket-bought, bigbrand meats, the animals have to be fed antibiotics daily so they don’t get sick living in those conditions. The factory owners also feed them hormones to make them grown beyond what their legs will bear. Whether it is cabinets that are open or some really cool brackets with old wood shelves, keeping open storage on your wall space can add a lot of interest. The shelves themselves could add the interest, or what you decide to put on them. Lighting – A chandelier or pendants over an island can make or break a kitchen design. It is important to take your time in selecting any decorative light fixture in regards to the style, finish and size. It can make a statement and help with the overall look and feel of your new space.
Backsplash Tile – These days most kitchens have cherry stained cabinetry or a neutral paint color and the counter tops tend to go in either a lighter or darker direction, but remain neutral as well. If you feel brave enough your backsplash is a great place to add color. There are so many options in forms of color, sizes and even decorative tiles with patterns can make the backsplash a focal point in your kitchen. Hardware – Keep an eye out for unique hardware for your cabinets. The finish does not have to match your stainless appliances or the finish of your faucets – think outside the box. There are so many amazing finishes and designs out there for
