Safety And Results Drive Change In Eye Surgery:
Epi-LASIK And Laser Cataract Surgery As Examples

o No Harm. All medical students learn this most central tenet when training to be physicians. Yet in medicine, as in many fields, choices are regularly made in which risks must be weighed against beneficial effects. Whether balancing treatment options with potential side-effects, such as in cancer drugs or life saving
surgeries, or considering purely elective lifestyle enhancing procedures such as cosmetic surgery or laser vision correction, doctors and patients must have a careful and frank discussion of risks, benefits, and alternatives. In ophthalmology, new technologies have opened up a world of possibilities, but also complex decisions, which require careful discussion and a close doctor-patient relationship. New techniques in corneal transplantation, premium intra-ocular lenses and lasers for cataract surgery, and advanced laser vision correction equipment are available to provide better than ever vision results – but these aren’t for everyone. I have enjoyed practicing in this era of unprecedented advancement in technology. I have eagerly embraced evolution in being first to introduce numerous new eyecare modalities to Delaware- including laser vision correction in 1995, near vision CK for presbyopia,
DSAEK partial thickness corneal transplants, and most recently laser cataract surgery just a few years ago. However, I have never lost “sight” of the importance of a careful analysis of new treatments, their efficacy and their safety, and the needs and goals of my individual patient. A prime example of this balance is Excimer Laser PRK, a radically new technology in refractive eye surgery when introduced in the early 1990s, which offered much more accuracy and safety than its predecessor Radial Keratotomy. Involved in the early research leading up to its FDA approval in 1995, I quickly moved to make this technique available to my Delaware patients. In fact, I initially brought these patients to Johns Hopkins’ Wilmer Eye Institute for treatment (my training institution) prior to laser availability in this state. Since that time, gradual evolution of that technique has enhanced convenience,
safety and accuracy. Changes included LASIK (involving the cutting of flaps to hasten recovery and improve comfort), LASEK (using alcohol rather than flaps to prepare the surface of the eye), and epiLASIK (which altogether avoids cutting of flaps, and instead gently and smoothly removes the outer layers, which then quickly regrow); all laser LASIK is another option, allowing faster recovery but somewhat greater risk in flap creation. It is only with careful and thoughtful conversation between surgeon and patient that the best treatment option can be determined. A more recent example is laser cataract surgery, the fantastic next step in the evolution of cataract removal and vision rehabilitation. This technique is far more accurate than standard cataract surgery, and also allows the reduction of astigmatism, thereby reducing dependence on glasses after surgery. Following FDA approval, my partners and I at the Center for Advanced Surgical Arts, obtained the first laser in Delaware (and one of the first 100 nationwide). While industry experts tout this as a revolutionary and important advancement, it definitely has its specific indications and roles. Once again, a careful evaluation by the surgeon well versed in ALL options and a willingness to explain those options to the patient is important in assuring the best outcome. While technology has provided wonderful new methods and tools to improve outcomes in ophthalmology, now more than ever, thorough understanding between patient and physician is vital to ensuring great and satisfying results.

He completed a cornea and refractive surgery fellowship at the renowned Wilmer Eye Institute of Johns Hopkins. Previously, he graduated magna cum-laude from Harvard, earned his medical degree from the University of Massachusetts, completed internship at Harvard’s Mt. Auburn Hospital, and served his ophthalmology residency at Washington University. Dr. Minkovitz participated in the early research on Excimer Laser refractive surgery, and has offered this procedure to Delawareans since FDA approval in 1996, before Delaware had its own laser. These early pioneering individuals travelled to Wilmer Institute with Dr. Minkovitz, where he maintained his teaching and clinical appointment. In addition to introducing laser vision correction to Delaware, he also introduced NearVision CK to eliminate reading glasses, DSAEK (a newer, less invasive type of corneal transplant) and most recently the newest advance: laser cataract surgery. Dr. Minkovitz is a co-founder of the Center for Advanced Surgical Arts and serves on its medical advisory board. He is also on active staff at Christiana Care. While on staff of Wilmer at Johns Hopkins University, Dr. Minkovitz received the Outstanding Teacher award. He was also named Top Doc in ophthalmology by Delaware Today Magazine six consecutive years, and has been an invited instructor at American Academy of Ophthalmology Annual Meetings.

The typical sequence of therapy to address these complex problems requires at least 3 surgeries. Initially the teeth are extracted and the bone and teeth extraction sites are allowed to heal. The implants are then placed and left to heal for 3-4 months. Finally a third surgery is done to expose the implants. Start to finish time is 6-8 months. During this time patients are left with only removable dentures on which to function. aryn Robinson of Delaware had begun showing signs of placenta percreta 24 weeks into her second pregnancy. “Placenta percreta occurs when the placenta penetrates through the entire uterine wall and attaches to another organ” according to the American Pregnancy Association (2006). Placenta percreta is the rarest and most severe form of placenta accreta with only a 5% risk rate. At 29 weeks, Karyn began to bleed and she was placed on hospital bed rest until delivery due to the potential for hemorrhaging. “I needed to be where they could get me into surgery quickly and where there would be blood products available, so they kept me inpatient for five weeks on bed rest,” says Karyn.

After she was prepped, Karyn was moved to a trauma operating room. Karyn gave birth to her beautiful son Wyatt on May 21, 2014. After a quick kiss, Wyatt was moved to the Neonatal Intensive Care Unit (NICU) and the doctors began performing surgery on Karyn. Given the extent of Karyn’s condition, a hysterectomy and repair to her bladder was required. During surgery she received nine units of blood products: four units of red blood cells and four plasma products to replace the blood she was quickly losing, and one platelet product to assist in clotting.
After surgery, while in recovery, she received three more units of red blood cells and ten units of cryoprecipitate, another blood product to assist in clotting. Her son only spent three days in the NICU and Karyn was released five days after surgery. Karyn and her son, Wyatt, are happy and healthy today. To celebrate her son’s first birthday, the family decided to have an intimate party for family and friends and sponsor a blood drive with the Blood Bank of Delmarva, in honor of his birthday. “I thought it was a good way to pay it forward,” says Karyn.
Karyn remained awake throughout the entire procedure with her husband by her side for comfort. “He kept looking at the ground, and I was like, ‘what are you looking at? Are you going to pass out?’ and he was like, ‘no’; afterwards he told me he was just looking at how much blood there was because it was just pouring out of me,” says Karyn. “I could definitely feel it. I remember telling them several times, ‘I feel like I’m going to pass out’ and they were like, ‘we are giving you more blood,’ and I could feel myself coming

Welcome to the 3rd Quarter of The Women’s Journal. This is my second issue since I took over as the publisher of the Chester County Women’s Journal, and we are slowly working to make all of the county papers have their own identity. Keep in mind that Rome was not built in a day, nor were our other papers. The New Castle County, Delaware publication was started 13 years ago, and when we printed our first publication it was made up of 44 pages of great articles from other nearby publications. But within two years, we had managed to grow to 44 pages of great articles from contributors within the New Castle County area. Since then, we have grown to 84 pages, and each year we have stayed consistently at 72-84 pages. Some of our contributors, such as Dr. Jeffrey Minkovitz and Carmella Gibellino-Schultz for example, have been with us for the entire 13 years. Our dream is that within the next two years you will see both Delaware and Chester County, PA papers grow to the same quality as our other papers. Building a paper in any area isn’t easy, but when you live over two hours away from the county that you are working in, it makes life quite challenging, but not impossible. I feel very fortunate that I now have some wonderful sales associates that are making the journey with me, helping to establish the Pennsylvania papers. Our goal is to replace every page in the Journals from another county with a qualified contributor in both Chester and Delaware County within the next two years. On May 15th of this year my granddaughter Kourtney Rose Marie that suffered with this disease finally went home to be with her creator. It was with much sadness that we had to let her go, but we all knew that she was finally at peace. We were very blessed to have had her for as long as we did. If you are not familiar with this disease, please take the time to find out more about SMA, it is the #1 Genetic cause of death for infants, and unfortunately there is no cure. Infection and delayed healing
iabetes is a chronic disease which affects your body’s ability to process sugar resulting in high blood sugar. High blood sugar can cause problems with many parts of the body, can lower your resistance to infection and can slow the healing process.
When a diabetic patient requires oral surgery, the dental surgeon usually consults with the patient’s primary medical doctor or endocrinologist to access the patient’s status. A wellcontrolled diabetic without other serious medical compromises can often have dental procedures including dental implants with the same success as healthy patients. Many times antibiotics are used before and after surgery to assist the patient compromised by diabetes in controlling infection. Blood sugar is monitored as well as other vital signs at the time of surgery. Sometimes the medical team must work together to improve the blood sugar before surgery can be performed. In severe cases, non-elective dental surgery must be done in a hospital setting to better manage the medical complexities that may arise.
Diabetics are at greater risk for developing oral health problems. The most common oral health problems associated with diabetes are: Gum disease Diabetics are at greater risk for developing both types of gum disease: gingivitis (early form of the disease where no bone is lost) and periodontal disease (bone loss present). Both gingivitis and periodontal disease are inflammatory diseases. Disease begins when bacteria in the plaque (sticky colorless film that continuously forms on teeth) causes the gums to become inflamed. Toxins produced by bacteria in the plaque irritate the gums. The toxins stimulate a chronic inflammatory response. The body mounts a defense to rid itself of the disease producing bacteria and their toxic by-products, and in doing so, destroys itself. The bone that supports the teeth is destroyed and plaque is able to migrate further down the root surface harboring more dangerous bacteria. The more the disease progresses, the more aggressive the response. A vicious cycle can take place unless treatment is rendered. Without intervention teeth may be lost. Since diabetes impairs one’s ability to fight infections and to heal, fighting gum disease is more difficult. In fact, a dentist is often the first healthcare professional to suspect that a patient has diabetes when there is an unexplained increase in gum inflammation or a less than satisfactory response to treatment for the disease. Not only does diabetes put an individual at higher risk for gum disease, but conversely, gum disease makes diabetes harder to control. Blood sugar increases with infection. In my career, I have had more than a few diabetic patients who were sent to our practice to have all sources of dental infection diagnosed and treated for the purpose of getting blood sugar under control. After dental treatment, the blood sugar indeed was easier to manage.
Fungal infections Since diabetes compromises the immune system, a diabetic is prone to developing fungal infections. The most common fungal infection is candida or thrush. It often appears as a white film on the tongue, palate, cheeks or throat. Symptoms include painful sores and difficulty swallowing.

istening with a hearing loss can be challenging, tiring, and frustrating. So much so that some people may even “tune out” of important conversations. It is important to realize that even with the best hearing devices, communication can still be a challenge in a variety of environments. In order to communicate effectively with our loved ones, good communication strategies are essential. Here are several effective strategies which enhance communication with, or without, the use of hearing technology.
Attention! Hearing takes concentration, especially for those with hearing loss. Gaining the listeners attention and allowing them to focus on you before beginning a conversation will ensure they are fully prepared to listen to what you have to say. A simple task such as addressing the person by name or making eye contact is a great start to a successful conversation.
Provide Context Providing context allows the listener to focus on the intended topic and will decrease the “guessing game” of what is being said to them. Think of “Wheel of Fortune” as an example. The players guess letters based on a known category. The players are more likely to solve the puzzle if they can narrow their scope to “animals” or “places” instead of an open ended topic. This is similar for folks with hearing loss
In most situations, it is important for the listener to see the lips of the speaker. Lip reading (or speech reading) is a common practice among those with hearing loss. Providing speech reading cues, along with contextual cues, increases the ability to gain the full picture of what is being said. Well lit areas are best in order to utilize speech reading compensation strategies. Long story short, speaking from a different room should be considered out of the question!

Location! Location! Location! Where conversation takes place is just as important as how it takes place! When possible, avoid situations where high background noise is present. Fans, wind, and other extraneous noises can also complicate the listening situation. Although hearing technology has come a long way, the most benefit will be gained in a quiet, well lit location.
Speak Slowly, Speak Clearly Rapid, muffled speech can lead to a discouraging conversation. When communicating, use slow, clear speech. There is no need to shout or over exaggerate lip movements. If there are multiple people in the conversation take turns speaking and make sure to follow the other strategies previously discussed!
Attitude Whether you’re the speaker or the listener, it is important to continue to have a positive attitude. It is understandable that communicating with a hearing loss can be frustrating at times; however, the goal of any conversation is to be able to communicate successfully with each other. Using good communication strategies, in addition to your well fit hearing technology, is bound to improve your level of communication!

Why Are My Teeth Wearing Away? H
ave you noticed that your front teeth seem to be getting shorter or smaller? Do the edges of your lower front teeth appear By worn, with whitishYvonne DeLoache, DDS colored enamel around the outside and an area of yellow to brown color in the middle? Why do you have nicks at the gum line of various teeth, especially the teeth on the sides of your mouth? Or why are your gums receding showing the roots of your teeth and making them look longer and you look older? Why are your grandfather’s teeth in your mouth?!! What is going on? Help! Sadly our teeth do wear away from use as we age but much of the destruction we see in our teeth is self-induced. Our teeth are damaged and wear for several major reasons. First, many people clench or grind their teeth or have a problem in their biting pattern. Second, our teeth are damaged by the way we brush or by using our teeth as tools. Third, our teeth are damaged by chemicals in our food or in our own GI system. Each of these problems has a name and the good news is that they are treatable. So if you have noticed this type of wear on your teeth don’t despair, don’t hide what looks like an aging smile behind your hand when you laugh, and definitely don’t start saving for dentures. There are solutions.
Types of wear and damage ATTRITION: wearing of the biting/chewing surfaces of teeth. The most common causes to this problem are grinding (Bruxism) and clenching, often during sleep. However, clenching and grinding can occur at any time, as it appears to be a response to stress. Another cause for excessive wear is an incorrect biting pattern. This can be the result of missing and shifting teeth that no longer bite together properly and wear unevenly.

PROBLEM: Same as attrition and abrasion combined with the possibility of GI tract disorder. PROBLEM: Teeth can appear long and unattractive, are much more susceptible to decay and gum line fractures, often become sensitive to touch and temperature, making eating certain foods uncomfortable, again may require extensive restoration. EROSION: dissolving of the tooth surface from acids. Many of our foods and drinks today are strongly acidic and filled with sugar. The acid from the chemicals actually dissolves the enamel of our teeth and the sugar results in increased acid production, leading to tooth decay. (Alternative: Drink water - both your teeth and your waistline will thank you.) Another cause of serious damage to our teeth can be acid from the digestive tract. This condition is known as acid reflux. Even without other symptoms, if you notice that your teeth look or feel thinner in the front or that your back teeth look more yellow and are developing pits or depressions, consult your dentist right away. This can be a sign of a systemic problem with your digestive tract that needs to be addressed.
So what can you do? These issues can easily be addressed by your dentist. You don’t have to just tolerate an unattractive smile or uncomfortable teeth. It is not just a part of getting older. Mid-Urethral Slings For Urinary Incontinence A Successful Alternative A
bout half of all women experience stress incontinence -- losing urine when they cough, laugh, sneeze or exercise -- at some point in life. It’s a common problem in women of all ages, starting in their 20s. Many women tell us stress incontinence limits their ability to work out or enjoy an active social life. Fortunately, there are many treatment options to consider. You owe it to yourself to talk to a urogynecologist, a doctor who is specially trained to treat pelvic floor disorders and bladder problems, who can partner with you to explain the various nonsurgical and surgical treatment options to find the right choice for you. Many women find relief by simply adopting a few healthy habits, such giving up caffeine or artificial
sweeteners that can irritate the bladder. Losing weight can help, too. Exercises that strengthen the pelvic floor can alleviate the problem for many women.
Minimally invasive sling
However, behavior modification doesn’t resolve the issue for everyone. For those patients, surgical intervention is a highly successful alternative. For more than 15 years the mid-urethral sling has revolutionized the treatment of stress incontinence. In a minimally invasive procedure a mesh sling is fashioned to support the urethra, (the tube that allows urine to pass out of the body). The mesh sling has an impressive success rate and good long-term outcomes. It is usually performed at an outpatient surgery center.
At the turn of the century, my colleagues and I began performing mid-urethral slings for stress incontinence. We have done many of these the procedures, which are generally highly successful and low risk, depending on the individual patient circumstances. Patients can take a few days or a couple of weeks off depending on the nature of their work. Further, the risk of developing a mesh-related complication is low. Only about 1 or 2 percent of currently used slings have to be removed due to some form of complication. That means 98-99 percent of patients who have slings end up having no significant complications. Up to 95 percent of women get great results from their sling, and they are able to resume sports and exercise

To sum it up, the midurethral mesh sling is minimally invasive and generally highly successful and low risk. That’s why the American Urogynecologic Society has adopted a clear position statement confirming that mid-urethral mesh slings are the procedure of choice for uncomplicated stress urinary incontinence. Unfortunately, fear of those risks has led many women to avoid mesh altogether, including the sling procedure, which can alleviate many of their symptoms. Our advice to women who suffer from incontinence is to discuss all your options with your doctor. RI Consultants installed the first 3T MRI in Delaware in 2007—and we’ve been providing Delaware with the most advanced MR imaging ever since. We recently added a second 3T MRI in Lewes. If you want to know about 3T MRI you should ask Dr. Philip Chao who has 30 years experience. I am always frustrated that doctors and patients don’t take 2 minutes to call me to ask me about 3T MRI and how it might help you. I see examples every day of why 3T MRI or a well equipped 1.5T MRI are important. The average patient or doctor, however, may be completely unaware of the difference 3T can make. When a patient is moving, a scanner that has BLADE installed on it CAN ELIMINATE MOTION ARTIFACTS. Without this, the patient’s movement can render your study useless. Many fail to understand that an MRI specialist can get information from this socalled routine study when other doctors fail. We had two patients yesterday come through the office that only we could have done. The first was a 425 lb patient who did not fit in anywhere else. We found a pituitary tumor and also two small strokes and polyposis of the maxillary sinus. Anywhere else they would have just given up. We used BLADE. The second was a nice gentleman who just could not hold still due to mild claustrophobia. Again, using BLADE we got diagnostic images. At 3T you can use another advantage of 3T – it’s remarkable speed. In that same patient who could not hold still, we used BLADE – the 3T scanner produced

Another advantage of 3T is simply that you can see more clearly with higher resolution and higher contrast resolution. We had a patient who had 3 brain MRI studies elsewhere, none of which showed why she was still having vertigo after several head concussions. Our 32-channel 3T scanner picked up the hemorrhage in the brainstem that was causing the vertigo. An old professor once told me when a technology comes along and can see what others cannot see, it eventually will win. This is true of 3T MRI – it simply can see better. But many people still do not understand how much better the pictures are. I only ask that you question and look it up on the Internet, because 3T MRI is better for breast cancer detection. It is better for brain tumor imaging. It is better for MS imaging. It is better for all musculoskeletal imaging. In the spine it can often find smaller lesions in the cord or on the nerve roots. This has been our motto since day 1. 3T MRI, twice as good! I review many other MRIs done on slower and less capable scanners. It is remarkable how bad some of the studies can be. In this day of increasing costs, does it make sense to do an MRI on a less capable scanner when you have a 3T MRI or a 1.5T scanner with BLADE? I know of hundreds of patients whose diagnosis was missed simply because one of these two elements was absent in their MRI study. In addition, we use the safest MRI contrast in the world. Because we care about the patient we use the lowest dose possible with the more stable agents. This means that none of our patients have ever gotten NSF (nephrogenic systemic sclerosis) in 30 years of MRI practice. We care about the patient –ask about why our scans are safer and better. About Dr. Chao Dr. Philip Chao graduated Phi Beta Kappa from Yale University and was Scholar of the House. He continued his studies at the University of Rochester School of Medicine and Dentistry, receiving his medical degree in 1983. After a transitional internship year at the Mary Imogene Bassett Hospital in Cooperstown, NY, Dr. Chao did his residency in Radiology at Brooklyn’s SUNY Health Science Center (1984-1987), where he became Chief Resident. He stayed on to complete a fellowship in Neuroradiology (1987-1988). Dr. Chao has been interested in MRI technology since its very beginning – the first MRI scan took place in 1981 while he was pursuing his medical studies. The University of Pennsylvania, where Dr. Chao was both a Body MRI Fellow (19881990) and a Neuroradiology Fellow (1989-1990), was at the heart of the development of this technology and Dr. Chao was able to work with pioneers in MRI on the first GE Signa 1.5T (tesla) scanner. His advanced research and specializations complete, Dr. Chao left the University of Pennsylvania for a position as Director of MRI in Wilmington, DE – a position he held for 14 years. left that position to create the best MRI Center in Delaware which uses the very latest technology: 3T MRI. While 3T (tesla) scanners have been used for scanning research patients since 2002, these advanced, more powerful MRI scanners only became available for clinical use in 2004. Advancing Women’s Care with Minimally Invasive Gynecology Surgery For women with abnormal uterine bleeding, a hysteroscopy procedure can visually help diagnose the problem with no incisions
Robotic-assisted surgery is further expanding the reach of MIS
In robotic-assisted surgery, rather than the surgeon holding instruments at the patient’s bedside, the surgeon operates the robotic arms (which in turn hold the instruments). One benefit of robotic surgery is that the robotic instruments have a range of motion and articulation beyond that of hand-held laparoscopic instruments, allowing the doctor to observe from different angles and get into smaller spaces than is usually possible in laparoscopy. Also, the robotic instruments do not wiggle or fatigue, as a person’s hands may, which may come into play during complex or lengthy surgeries.
The hysteroscopy procedure involves inserting a small camera into the vagina and uterus. Saline is then used to distend the inside of the uterus and give the surgeon room to see and, when necessary, operate. Using this diagnostic technique, without making any incisions, a doctor may be able to determine the real cause of abnormal uterine bleeding and may be able to take tissue biopsies or even remove polyps or fibroids seen within the uterus. ood news for women an increasing number of gynecological conditions that once required open surgery and hospitalization now can be treated with minimally invasive techniques that require just a few incisions or no incision at all. To perform a minimally invasive surgical procedure (MIS), a doctor uses specialized tools -- such as miniature cameras, fiber optics and robotic devices -- inserted into the body through small incisions in the abdomen, or through the vagina with no visible incision. As a result, patients usually have an easier time healing, with less pain and potentially fewer complications, because the inflammation and scarring caused by traditional incisions have been minimized.
Laparoscopy requires making small incisions in the abdomen
Used to diagnose and treat other causes of pelvic pain and abnormal bleeding, laparoscopy involves looking into, and possibly operating in, the abdomen. A doctor inserts a camera and surgical instruments to perform procedures such as removal of endometriosis, ovarian cysts, or uterine fibroids. Though the number and size of incisions vary by procedure, most incisions are 1 centimeter or smaller and cause minimal scarring.

Shedding Light on Diabetic Neuropathy and Other Painful Conditions L
aser light has been used therapeutically, and aesthetically, in the treatment of patients for decades. Powerful, pinpoint lasers can By Seth D. Lower power lasers are a mainstay of dermatological treatment and are used for everything from tattoo removal, varicose vein treatment to hair removal and wrinkle reduction. There is another therapeutic area where lasers have, in recent years, begun to have significant impact: pain reduction and tissue repair. Specialized lasers, which emit laser light at wavelengths in the infrared spectrum, and have been used in veterinary medicine for years (to heal damaged tendons in racehorses, for example), are becoming powerful tools to treat pain, repair damaged muscles, tendons and ligaments, and treat a range of other conditions in the human body. These infrared lasers (K-Laser (k-laserusa.com) is a leading company in this field) are considered Class IV lasers, which makes them significantly more powerful than the typical Class III, or “cold,” lasers that a number of practitioners use. K-Laser Class IV lasers are FDA-approved, and they emit laser light at two different wavelengths that are biologically active. K-laser therapy should not be performed over the thyroid gland, in children (as the laser can interrupt the growth plate in the bones of growing children), on or near the abdomen of a pregnant woman, or over an active tumor. Safety glasses must be worn by the patient and operator as the accidental application of laser light to the eye may be damaging to the retina. Treatments may last anywhere from a minute or two (say, for treatment of an injured finger), to 10 or 20 minutes for more chronic, widespread conditions (such as diabetic neuropathy or lumbar stenosis). During a treatment session, the clinician activates the laser, chooses the optimal dose and treatment time based on preprogrammed clinical protocols, and topically “paints” the area of treatment with a handheld probe that emits the laser energy. The patient may feel warmth and/or tingling at the site of treatment, otherwise the treatment is generally very well tolerated. Side effects may include a temporary increase in symptoms, usually lasting 24 hours or so, or some soreness at the site of treatment. The number of sessions required to treat a particular condition depend on the chronicity of the injury, its location, and its severity. A mild, acute ankle sprain may require only a few treatment sessions, while a 10-year history of 15
diabetic neuropathy may require more sessions for an effective treatment. Flashes And Floaters – Symptoms Of Possible Retinal Tear Or Detachment T

hink of the eye like a camera: it is a very small organ with many different parts to it. The eye has a lens that focuses the light on the retina. The retina is like the film of a camera – it lines the back wall of the eye similar to wallpaper. When a patient complains of flashes and floaters, as a retina specialist I am always concerned about problems with retinal tears and /or detachments. The retina can tear or detach and the initial symptoms may start as flashes and floaters. If the retina starts to detach, then a patient may notice a dark shadow or shade that descends over their vision. If left untreated, a patient will go blind. Floaters are a common symptom that many patients may experience. Typically floaters are dark, small, and float back and forth. Many times floaters get in the way of vision, but if the eye looks back and forth, the floater can float out of the way. Acute floaters can occasionally be harmless however more commonly acute floaters are a symptom of something more serious. If one has had floaters for many years and they have not gotten worse, then usually it is nothing to worry about. If one experiences floaters acutely, and they have not experienced them before. it usually signifies a problem. The only real way to tell the difference is to get a complete eye examination including dilation, allowing the entire retina to be checked. If a tear develops, many times it is in the periphery of the retina which can not be seen unless the pupil is dilated. If a patient complains of new floaters in one eye and the floaters do not go away s/he should be seen immediately. Flashes are not as common and are typically associated with floaters. Flashes that are caused by a retinal tear or detachment usually occur in the periphery of the vision (side vision) and are noted at night or in the dark, usually lasting for less than a second. They can occur frequently but the actual flash lasts for a very short period of time. Flashes can also be induced when one looks quickly from side to side. The most common cause of flashes and floaters is a posterior vitreous detachment (PVD). This occurs as a normal aging process. The process usually begins by age 50, and by age 70 more than 50% of people have had a PVD. The back inside cavity of the eye is filled with vitreous fluid. When you are born, the fluid is clear and gelatinous, like egg white before it is cooked. As one ages, the vitreous starts to liquefy. As it liquefies, the vitreous starts to contract and pull away from the retina. When it pulls away, the vitreous tugs on the retina and causes flashes – the retina is stimulated by the vitreous tugging on the retina. When a PVD occurs, patients will commonly notice flashes and floaters. It is usually during this 6-8 week period of the vitreous pulling away from the retina that a patient is at risk for retinal tears or detachments. Sometimes when the vitreous jelly is pulling loose, it will cause a tear or a retinal detachment. If a retinal tear goes untreated it commonly will lead to
a retinal detachment. The initial symptoms of a PVD should be checked out immediately. A retinal tear is much easier to treat (requires laser in the office) than a retinal detachment (requires more involved surgery). Hence, catching a problem earlier and before vision loss is always better. While a PVD usually occurs spontaneously, it can also be caused by blunt trauma to the eye or a hard hit to the back of the head that causes secondary injury to the eye. In summary, acute flashes and floaters should always be checked out with a dilated eye exam immediately, usually within 1–2 days from when they start. The most common cause is a PVD, which causes very little damage, but a tear or retinal detachment may be the cause and immediate treatment may be needed. Atrial Fibrillation-On The Rise A
trial fibrillation (AFIB) is characterized by erratic, irregular and rapid heartbeat that originates from the upper chambers of the heart. The likelihood of developing this disorder increases with age. AFIB is usually not a life threatening disorder but it does cause two major problems. It may cause patients to have symptoms of palpitations, “heart fluttering,” chest pain or a “pressure” sensation, shortness of breath, lightheadedness as well as fatigue and poor exercise tolerance. This can be intermittent or persistent. A potentially more serious problem is that AFIB can lead to blood clot formation in the upper chambers of the heart. This blood clot can then travel to the brain and cause a stroke. This is the most feared complication of AFIB and this, unfortunately, is not uncommon. Stroke is more common in the older patient and is seen more commonly in patients who also have other common medical problems such as heart valve disease, thyroid disease, heart failure, diabetes and high blood pressure. It is not uncommon for atrial fibrillation to be discovered as the cause after the first stroke in older adults. AFIB is difficult to treat as most of the medications available to control the rhythm are often not very effective. There are a select few medications that work better at controlling the rhythm than others but the side effects can be problematic in some patients. When patients fail medications or are intolerant to them, radio frequency ablation is considered which involves an invasive procedure placing catheters through the skin into the heart locating the origins of the rhythm disturbance and then applying radio frequency to “ablate” and thus terminate the arrhythmia. While this procedure can rid some patients of AFIB long-term, its ability to do so is
limited and is said to be effective in about 6575% of patients on the first attempt. On the contrary, there are very effective medications to significantly reduce the risk of stroke. These medications, commonly known as “blood thinners,” have been shown to reduce the risk of stroke in patients with atrial fibrillation by as much as 75%. Among the blood thinners available, warfarin (Coumadin) has been used for the longest time but is now being supplanted by the newer generation of blood thinners which in some cases are more efficacious and have a lower serious bleeding risk than warfarin. The main drawback with the newer medications is the cost.

Evaluation And Treatment Of Hypothyroidism Weight gain? Fatigue? Depression? These are just a few symptoms of an underactive thyroid gland. Thyroid hormones By Dr. Melissa P. Broyles, D.O. are needed for every cell in the body. Therefore, a deficiency of thyroid hormones can diffusely affect the body and cause a host of symptoms, including: depression, fatigue, weight gain, forgetfulness, cold sensitivity, high cholesterol, high triglycerides, high blood pressure, swelling, menstrual abnormalities, dry skin, hair loss, brittle nails, muscle weakness, constipation, shortness of breath and impaired kidney function. Hypothyroidism affects 5% of the US population. However, if the diagnosis is based on symptoms and basal body temperature, not solely on laboratory results, it is possible that the prevalence of hypothyroidism is closer to 40% of the US population. This is partly due to the fact that mild cases of hypothyroidism may not be detected by blood tests. Hypothyroidism is more common in females with an 8:2 female to male ratio.
The most common cause of hypothyroidism in the US is Hashimoto’s disease. Hashimoto’s is an autoimmune disease where antibodies bind to the thyroid gland and impair thyroid hormone production. Hashimoto’s is a genetic disease that is triggered by environmental toxins (i.e. perchlorate, iodine in excess, fluoride, and mercury). There is also a correlation between Hashimoto’s disease, intestinal permeability (leaky gut) and food allergy, namely gluten. Laboratory evaluation of hypothyroidism should include: TSH, free T4, free T3 and thyroid antibodies. Thyroid antibodies are used to diagnose Hashimoto’s disease and have a 90% accuracy rate. T3 hormone is 4-8 times more potent than T4 and is critical for cellular function. A subset of people are not able to convert T4 to T3 hormone. The treatment of hypothyroidism consists of a diet rich in iodine, zinc, copper, selenium (i.e. seafood, liver, nuts and seeds) and tyrosine, an amino acid found in protein. These nutrients, in addition to vitamins A, C, and E, are necessary for thyroid hormone production. In most cases supplemental forms of these nutrients are necessary. However, it is not recommended to take supplemental iodine unless a person is iodine deficient. Iodine excess can trigger Hashimoto’s disease in genetically susceptible individuals.

Thyroid hormone replacement therapy is indicated in most cases of hypothyroidism. Synthetic treatment is available in either T4 or T3 forms. There is also a natural desiccated thyroid medication which contains a combination of T4 and T3 hormones. Movement Is Freedom: Relieving Muscle and Joint Pain By Dr. Aisha Lord Ryan MSPT, DPT, CMTPT
T
he ability to stand upright and to move body limbs without pain is an important part of your overall wellbeing. Physical pain and limited movement, when left untreated, can affect confidence and emotional health. Physical therapists are experts in movement and function, with a significant focus aimed at preventing injury or loss of movement. If injury or disability in mobility has already occurred, physical therapists work to restore the loss of movement and to alleviate pain. Some causes for loss of movement include injury, arthritis, sports or dance activities; and improper stretching. Many people with arthritis think they must live with the pain. However, with conditioning, the muscles can be strengthened to support the bone and the patient can learn joint protection strategies. Common issues stemming from sports and dance activities are muscle strains and joint pains. Ninety-percent of young athletes have been injured while playing a sport, and 37% of those injuries were sprains and strains, according to a 2014 Safe Kids Worldwide report. Dancing is also a highimpact activity, with up to 90% of dancers experiencing some form of injury. Dance movements naturally put great stress on bones, muscles, tendons and ligaments. The risk of dance injury is greatest for young dancers during growth spurts when a youth’s bones are weakest – typically age 8-12 for girls and age 10-14 for boys. During the growth spurt period bones grow faster than soft tissues, and as a result, young dancers and athletes often experience tight ligaments and more limited muscle strength. of flexibility and coordination. If a player experiences significant pain, he or she should never try to “work through” the pain of the injury as it may cause more damage. If a dancer or athlete is injured, early medical intervention can drastically reduce rehabilitation time and return them to performance or sport quicker.
When should you see a physical therapist? • If muscle pain does not show improvement within 48-72 hours. • If your body needs to be conditioned for any seasonal or strenuous activity, a physical therapist can help you train properly and address most pre-existing issues (surrounding weakness, decreased mobility, speed and balance). • If you experience any decline in your ability to do activities once done with ease. • If a joint or limb does not feel normal or stable. If your movements are limited, there are a range of treatment techniques that can be implemented to help achieve a greater range of movement and activity tolerance, including: passive range of motion (PROM); joint mobilization; cardiovascular conditioning and dry needling, to name a few.
Trigger point dry needling is one method used to address chronic muscle pain. It involves placing a thin needle into the muscle at the trigger point – typically an area where the muscle is tight and tender – causing the muscle to contract and then release. This improves the flexibility of the muscle and therefore decreases the pain symptoms. Dry needling allows therapists to address pain hidden layers beneath the skin where it originates while diminishing the referred pain patterns often accompanying these trigger points. Dry needling is used often as an intervention with chronic pain conditions, and patients have reported significant results after trying the technique to address various forms of physical pain. The technique may cause some muscle soreness initially as it loosens tight muscles, but in many cases, improvement is noticeable after the first session. How To Know When It’s Time: What Are Some Signs My Loved One May Need Assisted Living?

s our loved ones grow older, their needs change. Some changes are influenced by medical and physical needs. It is difficult for anyone to see a decline in a loved one’s ability to care for themselves independently. This article is intended to help answer basic questions in regards to what is Assisted Living and when it may be time to consider a move to an Assisted Living Community.
What is Assisted Living and how is it different from ‘skilled nursing care’?
Assisted Living is for individuals who need limited physical assistance from trained professional caregivers in a supportive social environment. Assisted Living communities, such as Ivy Gables, provide Residents with limited physical assistance with Activities of Daily Living (ADLs, bathing, dressing, ambulating, and eating), as well as an environment to foster and encourage independence, and an active lifestyle of arts, entertainment and spiritual activities. Ivy Gables provides three restaurant style meals a day, snacks, and assistance with medications and management of medical appointments. Residents live in either studio style apartments, or some communities have apartments with one and two bedrooms. Skilled nursing care is for individuals who may be completely physically dependent on someone else to provide their ADLs as well as complete dependence for medical management.
If you notice your loved one is not as well groomed, their clothing or personal hygiene is not as immaculate as what it once was. They may not be taking their medications properly, or may have a change in their medical condition; a condition may be worsening and require more monitoring or more medical appointments. If you notice a loved one being more forgetful, not eating a well balanced meal, or able to take care of their home or their finances as well as they once used to. These are all signs that a loved one may need assistance with every day living.
Where Do I Start?
Recognizing that a loved one has a need for assistance is a huge step, and probably one of the more difficult steps in this process. Knowing your loved one’s financial situation is the key to where to begin. Assisted Living is paid for privately, with personal funds, coverage from a Long Term Care Insurance Plan and assistance can also be received if one is eligible to receive the Veteran’s Aid and Attendance Pension. Analyze your loved ones financial health, how much income do they receive monthly, where does the income come from, what are their investments and other assets. You’ll need to know what your loved one can afford.

When you tour a community, use all of your senses to take in the surroundings. Here are a few general thoughts to keep in mind: Is the community appealing to the eye, is it clean, uncluttered and decorated in a style that will be comfortable to your loved one? Does the community smell clean and fresh? What sounds do you hear? Are there any loud noises, staff or Residents with loud voices or alarms going off? Are you able to arrange for a tour that will also
include lunch or dinner? Food is such an integral part of our daily lives, you want to be sure the community provides food appealing to you and your loved one. New Hope Celebrates 25 Years!
New Hope is truly a gem of a program that Delaware Hospice offers free of charge to our youngest community members by supporting children and teens (ages 6-17) who are coping with a loss. Delaware Hospice family members, as well as youth referred by schools or community agencies, find help in individual and group settings or at Camp New Hope. The capstone of New Hope is Camp New Hope which is a four day summer camp where children come together to talk about their loss, to remember special times, to meet new friends, and, most importantly, to have fun. Most 17 year old girls look forward to their summer vacation to spend time with their friends at the beach or mall. Adrianna of Lake Forest is looking forward to those activities, but she is more excited about the four days she will spend at Camp New Hope as a camp volunteer. Just before her 15th birthday, Adrianna lost her father to colon cancer. Her family came to Delaware Hospice and Adrianna was able to meet one-on-one with a New Hope Counselor. During those meetings, Adrianna was able to talk through the vast feelings she was experiencing and learn positive strategies to help cope. The summer after Adrianna lost her father, she attended Camp New Hope. As a camper, Adrianna was able to see she “wasn’t alone, all the kids around me had experienced a loss and we each were able to share our memories.” The next summer, Adrianna decided to attend camp as a camp volunteer. She wanted to go back to camp and help other children who had experienced a loss and to be a positive memory in their lives. “It www.TheWomensJournal.com
just breaks your heart to see young children have to deal with such a loss.” As a volunteer, Adrianna was able to work through her own grief while helping others work through their own. She had the opportunity to share her loss and to encourage the campers that they will be okay. The moment that made a lasting impact on Adrianna was when a young camper wanted to know if Adrianna knew how to paint toenails. Adrianna, said “yes” and wondered why this was such an important question to ask. After a few more questions, Adrianna understood that the little girl’s late mother was the one who always painted her toes. And she didn’t know how to paint toes. After talking to the girl’s father, Adrianna took the time and care to paint the young camper’s toes just like her mother used to. “The smile and delight she had on her face was a moment I will never forget,” said Adrianna. “A simple, small task of painting toes made that camper’s whole week and allowed her to realize that even though her mother is no longer here, it is okay to still do the things they used to do together, and think of it as a way to remember all the good times they had together.”

A Tale Of Two Sues. Or, Seven Reasons To Visit The Spa This Summer! S
elf-reliant Sue has been inside all winter. But summer is finally here, so Sue is putting on her shortie-shorts and tank to head to the dog park with Polly, her pooch. As the summer breeze whips her leg hair into a frenzy, Sue realizes she’s as furry as her four legged By Joanne Graves, friend-Yikes! The Obstacle Is YOU!

However, remember that if you want something badly enough, you’ll find a way to make it happen. For instance, getting up 45 minutes earlier to make time for a workout is ensuring that you DO YOU first!
Taking the initiative to prepare for obstacles is a great way for you to overcome and conquer. For example, if you’re going to dinner, take a look at the menu beforehand and determine a healthy option. Or if you know you have a day filled with meetings, pack meals or snacks accordingly.

e are confronted with obstacles every day. Sometimes they can be predicted, other times they cannot. Being confronted with an obstacle is very different than being confronted with a failure. While avoiding failure prevents action, avoiding obstacles may still bring success. In the world of fitness, many so-called obstacles exist. Things like “not enough time” or “I don’t like working out.” Are they obstacles or excuses? Hmmmmmmm… The only real obstacle that prevents you from success is your lack of belief in your ability to succeed. Every other obstacle that manifests itself can easily be hurdled if you believe in yourself, and is often an opportunity for growth. If you don’t believe it, there is NO way you are going to achieve it. So you must have faith in yourself when confronting an obstacle. It all starts with you my friend! Work on a way to

racticing mindfulness of your breath is perhaps the simplest way to improve your quality of life. Your entire life, you breathe. It’s automatic, every moment of every day. It’s crucial to life. Counting your breath is the simplest way to bring mindfulness to your breath. Simply count to 3 with each inhalation and again to 3 on the exhalation. As you practice, you can increase your count, starting with the exhale. The exhales should always be the same or longer as the inhale, and you should never feel like you are gasping for breath. The focus is not on how high you can count, but on the evenness and continuity of breath.

Pilates For Golf Conditioning

A
re you looking to improve your golf swing and game? Pilates training will help you hit the ball farther, straighter and more accurately with less chance of injury. Golf requires muscular strength, joint flexibility, neuromuscular training and the perfect balance between mobility and stability. The golf swing is a coordinated movement that on the moment of impact applies compressive forces of approximately eight times one’s body weight. Spinal alignment is vital to proper swing mechanics, since the spine coils and uncoils 100-130 times in one direction over an average 4-hour game.
Is Your Body Prepared For the Game of Golf?
Golf is a game of asymmetry and multidirectional stresses. To improve in golf, players need a golf training program that will produce the greatest results in the most efficient amount of time. Golf injuries are caused by the instability of the golfer to maintain proper spine angle in the swing putting more stress on other muscle groups besides the back. The knees, hips, forearms and shoulders are often subjected to extra stress to compensate for lack of core abilities.
So Why Pilates?
For the most part, golf and Pilates are mind-body activities that share the same basic principles requiring flexibility, rotation and core strength. Pilates movements are based from the center of the body, as are most shots in golf. Pilates spinal rotation exercises de-rotate the spinal imbalance
and realign core muscles. Clients start to think about the body and its functions differently when they take Pilates. They think about the core (trunk, shoulder girdles and pelvis). Strengthening and creating flexibility through specific exercises in these areas will help balance the muscles around a joint and balance pairs of muscles that support the joints. Pilates training will allow the transference of power to the legs, trunk and arms. Addressing physical limitations through a series of Pilates exercises can help create symmetry and coordination, improve muscular endurance and increase range of motion. Everything that is learned in the studio is transferred directly onto the course. Postural alignment is key. This will help the golfer perform the swing more efficiently and reduce the amount of unnecessary strain on the muscles and joints.

Phil Ortiz, Golf Pro at the Peninsula Club in Long Neck DE, says the most common thing he sees on the course is lack of hip rotation. He trains his clients to create “power” from knees to hips and “timing” from the upper body. This connection needs to be supple. Correcting the golf swing at the time of the swing will not improve the physical cause. He explains golf like “life” . You have to prepare, create the action and, most important, follow through. One must complete all for success.

The company offers programs for salon owners, distributors and manufacturers. At the Icons of Hair awards event in El Segundo, Calif., J Christian was saluted for his 38-year professional career. “Together with his wife Marcy, J Christian started their studio salon in Hockessin, Del., back in 2007 with a dual mission,” the awards program noted. “They created a top quality salon that was primarily focused on creating a successful business backed by advanced education, as well as to develop a very strong philanthropic plan that would help them and their staff give back to their community. How To Prepare Your Student To Go Back To School I

Call up friends. If your child hasn’t seen classmates from last year all summer, now is a great time to arrange a few activities with school friends. If you know students who will be in your child’s class, encourage your child to reach out to them, too.
Make a school visit. Stop by the school at least once to let your child walk around and check out his or her new classroom, if possible. If your child has an opportunity to meet staff members and his or her new teacher (at back-to-school night or in another way), take advantage. Go over the routine. If your summer routine has been relaxed, now is the time to remind your child how a typical school day will go. If needed, start enforcing a reasonable bedtime a few weeks before school starts. Hang a family schedule and/ or calendar in a visible place.
Get the home organized. Keep hooks near the entry door to your home where your child can keep the backpack, jacket and other school gear. Have him or her organize his or her desk or homework area. Take your child shopping to restock school and organizational supplies for the home and the classroom. Don’t forget to buy a new planner for him or her to use to stay on top of all obligations and assignments.
Do some refresher work. The final few weeks of summer, incorporate a little school work into your child’s daily schedule, using workbooks or worksheets from last year to brush up on math, reading and other basics. To practice writing, have your child keep a daily journal on the things he or she did this summer. As you bump up bedtime, include reading in the nightly routine-letting your child choose books from the library.
Sit down to talk about goals. Goal setting can be a powerful tool. Talk with your child about the things that he or she would like to accomplish or change this school year-academic or otherwise. If your child had any difficulties last year, let him or her know that you are there to help and want to maintain open communication about school. When it comes to preparing for back to school, a little can go a long way. “Back to school preparation is largely about getting in the right mindset,” says Huntington. “Help your child prepare by talking positively about this coming school year and the good things to come. Remind your child of the best parts of school, and also let him or her know that you’re always there to help problem solve when issues arise. Selecting Your Best Attorney T
his article continues from last quarter with preparing for your meeting with an attorney and selecting the best attorney for you. Our focus is choosing the best fit for you — not the best fit for your sister/brother, cousin, or neighbor. What follows are my top five tips on how to By Shana A. Pinter, Esq. evaluate a potential attorney, to find the person who is going to not only represent you in the best light but also make you feel at ease.
First
do you want an attorney who is going to argue at every step of the way or do you want an attorney who takes a more collaborative approach? There’s nothing wrong with either of these approaches. Remember it’s all about your comfort level with the person. There are some attorneys out there who make their own clients cry and other attorneys who bend over backwards to make them feel comfortable and at ease.

If you’ve been the unintended breadwinner in your marriage while your spouse hasn’t been able to hold a job, you’re probably not going to be looking at a 50/50 split. We attorneys are professional colleagues and frequent the same professional associations, meetings, Continuing Legal Education seminars and might even be neighbors. We will most likely
talk to each other in courteous ways when in your presence at mediations, settlement negotiations, etc. This does NOT mean that we scheme behind closed doors to your disadvantage. We are still representing our own client’s interest and only those interests and doing the best we can do to preserve your rights. Serving and Protecting Those Who Serve and Protect Us Delaware Workers’ Compensation Law Also in 2010, the first female State Police Pilot was added to the ranks. Today, 11.83% of the State Police personnel are women. Over the last 40 years, the number of women working in these types of public servant jobs has dramatically increased- and so has the need for their protection following work injuries. Thomas J. Lawyer Bashing? By Susan D’Alonzo Ament, Esquire Susan D’Alonzo Ament is a partner with Morris James LLP and has practiced for 32 years representing personal injury victims. Over the years, Mrs. Ament has found that some women don’t understand the crucial need to have adequate automobile insurance coverage in the event of a serious accident. O
ver the past 32 years, I have represented many people who have been seriously hurt in automobile accidents or accidents at work. They come to me at a time when they are facing many difficult challenges - legally, emotionally and medically. I like being able to help them sort out all of the issues associated with their case and help relieve their stress. I am always amazed by lawyer bashing - nasty lawyer jokes, name calling such as ambulance chaser, and general disrespect or fear of lawyers. Certain folks have a view that some lawyers are greedy. I have found that couldn’t be farther from the truth. I think lawyers are the MOST caring and charitable people I know… always ready to help. And doesn’t that make sense? The majority of attorneys will tell you they became lawyers to help people. Here are a few facts that support my opinion
that lawyers are good and giving people. Recently Sojourners’ Place, one of the nonprofit boards I am on, sponsored a dinner and auction. When I reached out to my partners and asked if they would support Sojourners’ Place (whose mission is to assist homeless men and women on their journey to selfsufficiency through casemanaged, residential programs), twenty-seven of my partners quickly responded “yes”. Wow! I was grateful, but not surprised. I also called another lawyer friend, Mike Kelly, Esquire, who responded with a very quick “yes!” to volunteer as Auctioneer at the event. Several of my attorney friends either handmade or donated beautiful gifts for the auction. Here at Morris James, many of the attorneys donate their time regularly to non-profit organizations including Girl Scouts, Children’s Beach House, The St. Thomas More Society, Prevent Child Abuse, American Diabetes
Association, Habitat for Humanity, Catholic Diocese Foundation, Philippine-American Association, and the Delaware Adolescent Program, Inc., among others. In our Personal Injury Group, Mike Owen is active in the Big Brothers Big Sisters of Delaware program, while Pete Jones and Kevin Healy are two of several Morris James attorneys who volunteer many hours to coach high school students to compete in the Delaware High School Mock Trial program. The goal to serve is not unique to Morris James but is prevalent throughout the entire Delaware Bar. One of the Inns of Court I am involved with is the Pro Bono Inn of Court. Inns of Court are designed to improve the skills, professionalism and ethics of the bench and bar. The Delaware Bar Association assists attorneys and coordinates the pro bono efforts between pro bono agencies.
One group I have been privileged to be on the Board of for many years is the St. Thomas More Society. The Saint Thomas More Society is a collection of attorneys of various faiths committed to follow the examples of Saint Thomas More in their professional and personal conduct and to promote and foster high ethical principles in the legal profession and in the general community.

Perhaps they are guided by Isaiah 1:17 - Learn to do good, seek justice, correct oppression; bring justice to the fatherless, plead the widow’s cause. The next time you hear someone speak negatively about a lawyer or tell a bad lawyer joke, I hope you will think of this article and set the person straight. Lawyers are good and caring people whose goals are to help others.

BED BUGS INFO TIPS FOR WHEN YOU TRAVEL
Due to increased domestic and international travel, people are at an increased risk for bringing home a bed bug problem. Bed bugs are great hitchhikers and travelers can easily bring them back to their homes in their clothes or luggage. Once inside a persons’ home, bedbugs can rapidly become an infestation problem.

Bed bug eggs are whitish, and resemble tiny grains of rice, but smaller. Check these four areas for bed bug activity: Headboard: Inspect the headboard closely, checking behind it if possible. Check night stands, tables and baseboards directly next to the bed. Pay close attention to cracks and crevices. Mattress: Inspect the sheets first, then slowly pull back to reveal mattress paying close attention to the tuffs, seams and folds.

Keep all zippers closed and do not place or store luggage on or next to beds, upholstered furniture, in closets or in dresser drawers. Keep items that can be laundered in sealed plastic bags. Put your soiled laundry in a tightly zipped laundry bag. Returning Home: This is important if you want to reduce the likelihood of introducing bed bugs into your home. Bag and seal luggage before placing in your vehicle. All luggage should be placed in the heavy duty plastic bags that you packed, and sealed tightly. At home, remove luggage from bags outside, take the bag that the luggage was removed from and discard it in an outdoor trash receptacle. Do not take your luggage inside your home. Unpack your luggage in an area that is well lit and away from any furniture and sleeping areas (outdoors, garage, basement.) Unpack one suitcase at a time, separating clothing that can be

A Bouquet Is More Than... are “the stars of the cast” and the sweetheart roses, stephanotis, ranunculus and perhaps Queen Anne’s lace take on the roll of a supporting cast! The greens and filler flowers, such as heather, limonium, wax flower or babies breath carry out the part of the “extras”. Yet all are integral to the entire production or composition or bouquet.

bouquet is more than ~ ...an assortment of flowers tied with a ribbon or plunked in a vase. The creation of a bouquet combines the art of composition and the principles of engineering. The finished product has to appeal to the eye as well as remain intact over several hours [or in the case of a bridge, for many decades!]. If a bride decides to have her wedding flowers preserved the matter of having every bloom at its peak and every adornment firmly attached and every ribbon securely wrapped is doubly important. The bride and her florist are an artistic team. Most often a bride has a vision in her mind accompanied by a picture from a magazine or a photo from a friend’s wedding or an image from an online site of a particular style of bouquet, and she may have a very specific list of flowers that she loves and are meaningful to her. Enter the floral designer who knows what flowers on the bride’s list are available in the season when the wedding will take place. Lilies of the valley, tulips, peonies, & dahlias are not available year round making it necessary to suggest an alternative for a bride who has her heart set on any of these. Just as an artist selects various objects for a stilllife painting, a floral designer must select flower forms which compliment each other when the bouquet is assembled. One would not want flowers of all one size like football mums and peonies and sunflowers in a single bouquet! Nor would daisies, alstromeria and minicarnations create an eye-popping composition. If we think of a bouquet as a theatrical production, flowers such as peonies, hydrangeas and dahlias
grandmother’s wedding dress is to be used to wrap the bouquet, or if a sentimental item such as a locket is to be intertwined with the flowers, these items must be brought to the designer in advance of the wedding week. The creation of a bouquet when the floral designer sits down to put it together is much like manufacturing a car on an “assembly line”! First, all of the flowers have to be prepped. Having arrived at the flower shop early in the week, all stems have to be cut and hydrated and the flowers of each variety have to be counted and set aside in the cooler. At assembly time it is critical that the designer does not discover that she is 5 stems short of the roses or hydrangeas or whatever flowers are “the Stars” in the wedding party’s bouquets!
The creation of a bouquet, like the main course at a gourmet dinner, is a combination of art and science. The entree is equivalent to the bouquet; each individual flower in the bouquet is an ingredient in the dish. It includes a featured item, a number of secondary ingredients, an assortment of spices and herbs and perhaps a garnish. When the floral designer meets with a bride there some essential questions that have to be answered:

The number of attendants, the number of guests and whether arrangements will be needed at a ceremony site as well as a reception site will dictate how much of the overall budget is available for flowers.

The creation of a bouquet is the product of a bride’s vision and the floral designer’s expertise. Once the bride and her florist have decided on the specific flowers to be used, the bride steps out of the picture. Even though the wedding may be scheduled for a year in the future a floral designer makes her “shopping list” of the flowers that will need to be ordered 3 - 4 weeks before the actual delivery date. Carolyn Schofield started her career with Loch Narin and quickly moved up to the position of manager of the upscale Farmhouse Restaurant in Avondale Pennsylvania. After a period of time she rose further up and added to her responsibilities, becoming manager of The Greathouse restaurant as well, in addition to The Tavern and Terrace facilities that are part of The Greathouse and adjoining Round Room.
After years of managing these extensive properties she decided to add to her skills and experience by accepting corporate management positions in two major international hotel groups. Carolyn specialized in management, sales and logistics of large corporate functions including weddings, conventions, seminars, and other large venues, adding to her skills and knowledge over a number of years. Barbara Barnes
Barbara Barnes was born to work in the hospitality business! From a very young age, she had a passion and desire to help folks have a wonderful and memorable experience even if only for an afternoon tea or a cold beverage while traveling to an exotic location. Following graduation from Goldey Beacom College, Barbara was hired by the DuPont Company and settled into the corporate life for a short time until she was accepted as a flight attendant for TWA. She loved flying and imagined she had landed the job of a lifetime until she met her future husband Robert. It became very clear to both of them that flying across country and the long separation was not going to work for this couple! She turned in her wings, came home, married her wonderful husband and went back to work for DuPont. Soon after returning to DuPont, an opportunity presented itself for Barbara to join the illustrious and awarding-winning team at the Green Room in the Hotel du Pont as a hostess/group leader and then as the first female Captain. In the early 70’s, restaurants of this caliber only hired male employees to fill this role. She became well known in the community and served dining patrons around the world who were always pleased to see her winning smile and enjoy the gracious hospitality of the elegant Green Room.
A next step in her progression was to join the Hotel du Pont sales and marketing team as a sales manager responsible for selling, organizing and planning events at the property. Her attention to detail and organization skills led to her becoming the Director of Sales for the Catering Department. This is where Barbara really excelled and planned the most extravagant and magnificent parties, galas, weddings, debutante balls, corporate events and all manner of high level social events. Barbara was grateful for the opportunity this position afforded her as she met many past Presidents of the United States, royalty, diplomats from around the world, famous stars of stage and screen, and local business leaders with whom many became friends. After 40 years of carefully handling the most important events in a person’s life, Barbara chose to retire and planned to spend many hours enjoying travelling and walking along the beach with her devoted husband. This short lived dream of a lady of leisure did not actually happen. As Barbara was saying goodbye to her friends at the Hotel du Pont, she received a call from Gini Smedley, owner/proprietor of Loch Nairn Golf Club. Gini had heard Barbara was leaving Hotel du Pont and thought what a fabulous opportunity to hire Barbara in her sales and marketing department to become a part of the Loch Nairn team. Knowing the quality and the standards and being a regular patron of Lock Nairn herself, Barbara had no hesitation in becoming a part of this beautiful facility. She has never regretted that decision and continues to help create the most magical moments for the guests of this fabulous facility known as “A Jewel in the Country” located in the heart of Chester County. She has managed to still carve out quality time for those walks along the beach with her husband, her son Chris and his lovely wife Katie. Originally from Ohio, Dr. Yvonne DeLoache received her dental degree from the University of Michigan, School of Dentistry. After graduation Dr. DeLoache went on to serve in the United States Public Health Service where she completed a General Practice Residency at the USPHS Hospital in Baltimore, Maryland. After her Residency, she came to Chester, Pennsylvania, to work with the National Health Service Corp, a division of USPHS. Upon finishing her work in Chester, Dr. DeLoache opened her dental practice in West Chester and has been a “hometown, family dentist” ever since. Dr. DeLoache married R. Blake Edmonds, DMD in 1986. The loves of their lives are their two sons. After their second son completed his undergraduate education, she was blessed to receive a teaching position at the University of Pennsylvania, School of Dental Medicine. She has been there four years and if you check with any of her students, friends, or family you will hear the same refrain, “She loves it.” Dr. DeLoache says, “Working with my students is a true calling and a great joy for me. The students become like family.”
With family as a top priority, Dr. DeLoache truly enjoys being a small town dentist where her patients are also “like family.” She invests the time to get to know them well, answering their questions, allaying their fears, mourning with them during sad times, and celebrating their successes. Laughter is always on the menu at her office as she and her staff work to restore patients to good dental health. Committed to patient education, Dr. DeLoache believes that patients need clear, concise, and accurate information to make the best decisions about their dental care. Thais da Silva Viggue, the founder and baker of Dia Doce, never in her wildest dreams imagined she’d launch an award winning gourmet cupcake company. L
och Nairn is a special place offering its guests all the amenities of a private club without the expense. I believe this has brought a new audience to Loch Nairn that might not otherwise visit a public golf course. Our grounds are breathtaking and we lend ourselves to beautiful outdoor weddings under the 17th Century William Penn Maple Tree. We have lovely Terrace dining at the Farmhouse, which overlooks the country gardens as well as patio dining at the Greathouse and Tavern. But perhaps the most incredible aspect of Loch Nairn is the story of how it came to be. It all started in 1969. I was a housewife, mother of two wonderful athletic boys whose games I followed all over the country, and in my spare time, an avid golfer. In the summers my niece and I were on the horse show circuit with the boys in tow with their mini bikes. We had just designed and built our new home situated on 70 acres with a barn and large pond. It was the good life. Then one day everything was turned upside down. My husband Hank wanted to start designing and building golf courses and unbeknown to me, he and his partner purchased The Red Fox Inn, a large restaurant with a motel and a 9-hole golf course. Hank was only interested in the golf aspect of the business. The restaurant and motel just happened to be sitting on his land and just like that, I went from a happy housewife to running a restaurant! I knew nothing about restaurants, which was obvious to the seasoned staff of older women who resented my being there. I worked from 7 am to midnight (and that was an easy day) they soon learned I wasn’t afraid of hard www.TheWomensJournal.com
work. I finally gained their respect. My husband was my greatest supporter, encouraging my many changes to the old Inn such as turning the old barn into a banquet room, dinner theater, golfers grill and locker rooms. We only had one piano so I dragged that poor old thing from building to building. My staff, I am sure had never worked with anyone like me. If it needed doing I got it done with the help of willing or unwilling staff. I opened for breakfast and closed. The buildings were haunted, and everyone knew it. One evening my headwaiter announced that he would never go to the liquor room ever again. He didn’t like the gentleman he met downstairs! I closed many a night by myself with my hair standing on end. Weekends I would host breakfast. I usually had one chef and several waiters. On one particular day only one waiter with a very thick accent came to work. I cooked while he was the lone waiter and host. The motel was full, and might I say it was a disaster? At one point the waiter stood in the middle of the floor clapped his hands and announced, “I suggest scrambled”. The guests were entertained & forgiving. The economy was getting very bad. We were paying interest rates higher than 20%. Times were very difficult. I remember one day driving in to work very tired and saw smoke coming out of the roof and I sat there thinking how easy my life would be if it burned down. Should I let it burn? After not too much thought I decided to tell my husband. What a scene the three of us putting that fire out. To make life tougher, for three years while all of this was going on Hank was away in Jamaica building a wonderful world-class golf course, Cinnamon Hill.
not to share & I loved creating our unique ambience. My concept of only serving healthy, fresh food, no chemicals, preservatives or additives was very well received. With everything to be made on property, no frozen foods, no freezers no microwave ovens. I started cooking simple items for lunch later hired a French chef that showed up half the time. Fortunately I had a young bartender follow me from the Fox. She was a very good cook and jumped in. I quickly hired a very talented young man who gave us his wonderful crab cake recipe. Presently we have Chef Roland as our executive chef. Chef Roland works & coordinates two buildings and two kitchens. He is a gem. The Old Farmhouse Restaurant became so busy that I had to convince my son Chris who was running the golf end of the business to let me use his golf shop for a dining room on weekends. Every Friday and Saturday night I somehow managed to transform the golf shop into a lovely dining room with candlelit, linen covered tables, oriental rugs, paintings etc. Our next project was born from my husband’s love of wine. We transformed a creepy old underground room in The Farmhouse (that I avoided at all costs) into a spectacular wine room with chefs table that creates a most memorable experience. Be sure and visit when next here. Again in 1989 we realized we needed even more room to accommodate golfers who weren’t happy sharing their space with ladies and gentlemen dressed to the nines. I didn’t want just another building it had to be special. So we hired our very talented friend Howard Curtis, to help us design
and build the building of my dreams. The Greathouse. I soon began booking weddings and golf outings in The Greathouse. The tavern enjoyed by the golfers is my favorite room. Adorned with a wood burning fireplace, gabled ceilings & huge beams from which I have hung a family old town canoe, porch swing, three wheel tricycle, life size flying goose, the garden gate, wine barrels and much more. It is truly unique. Still we needed more room so the patio off the great room was the logical place to go. Hank came up with the plans for a Round Room. Again Howard was called in to build the round room, a different concept that is a beautiful addition to the properties. Another patio was added off the round room and tavern. I am still looking for ways to expand. That’s downsizing. Loch Nairn came from an unexpected and long journey that I am so fortunate to have traveled all the while hearing my father’s voice. You started it. You will finish it. I’m trying! I have worked every aspect of the business but the position I loved the most was hosting. I got to meet, greet and know my guests. I saw that they were well taken care of and said good night to satisfied smiling faces. I love it when guests tell me how much they enjoy the ambience I created, the food Roland created and the service of rare excellence. It’s very rewarding. That was the hardest 10 years of my life but without those years of experience, Loch Nairn wouldn’t be what it is today. The adventure continued when we sold the Red Fox Inn in 1984 and moved to the adjoining property now known as Loch Nairn. Hank with our sons rebuilt the original 9 holes of golf and added 9 more. There was a small lovely farmhouse, which became a golf shop and a bar with a small 10 table dining area. My intention was to downsize and offer our guests golf, a hot dog and a glass of beer but before long I felt the need to fix up the old farmhouse and offer lunch and dinner. It was too nice 69
