It’s been a long, cold winter, BUT spring is HERE! I can’t remember that last time I’ve been so eager to see spring arrive. This edition brings our readership many great educational articles as we embrace and welcome the coatless weather. Preventive Blindness shares the importance of eye protection. It’s not only to be used while at the beach, but also while you’re out running, doing water sports and/or if using tanning beds. “Utlra Violet Rays May Cause Damage to The Eyes Today and Tomorrow” is a must read (Pg 9). If you’ve neglected not only your eyes, but also your skin, you must read “Wisdom & Wrinkles? Do they have to come together?” (pg 25). As the sole owner of Alternative Health & Healing for 18 years, Dr. Louise has over 12 years of professional hypnosis experience. Her goal is to guide people to become the best version of themselves. She began her career as a dental hygienist in 1985, practicing in all areas of dentistry for sixteen years. In 1995, she trained and became licensed with the Board of Nursing to start a second career as a massage therapist, and this was the birth of Alternative Health & Healing. However, she wanted to work with the whole person, mind, body and spirit, so in 2002, she trained and became certified through the National Guild of Hypnotists in her third career as a Hypnotherapist. Understanding Hypnosis What It Is and How It Works?

What is Hypnosis? Before considering what hypnosis is, perhaps it would be appropriate to establish what it is not.

If you both feel that it will be worthwhile to proceed, your hypnotherapist may give you some small tests to determine your type of suggestibility, you ability to relax, your skills at visualization – all which are used to adapt to you as an individual. It is not sleep, nor surrender of control of the mind, nor is it magic. Hypnosis cannot be sleep as the subject is fully aware of communication and is able to respond on request either verbally or by signal. A subject asked to make a specific movement will comply unless it is objectionable, in which case there will be a refusal. There is no surrender of mind or control. A person who does not want to be hypnotized cannot be hypnotized nor be induced to do or say anything which violates personal standards of behavior or integrity. Hypnosis is better described than defined. It is often described as a state of extreme focus of the mind. Where the subject chooses to see only what is relevant, blocking out everything else. Hypnosis involves guided concentration, which can be provided by a qualified practitioner or, in the case of self-hypnosis, by the individual. Hypnosis is a very natural state that everyone has been in almost every day. For instance, when watching a movie that is scary, sad or funny and you feel those emotions, you are in hypnosis. When reading a book or engrossed in a hobby, you are in hypnosis. If you have ever driven to a regular location and realized you did not pay attention to how you got there, that is highway hypnosis. Even staring out the window and daydreaming about something is hypnosis. Where did Hypnosis Originate? The basics of hypnosis go back to ancient times. Prehistoric clans had medicine women who performed healings, revelations and the like.

Wider usage began in the 1700’s in several forms under different names. There have been periods of progress and periods of stagnation in the development of modern hypnosis Medical interest and acceptance came in 1951 with the AMA’s endorsement of Hypnosis as a therapeutic tool.

In essence, hypnosis is a means of communication between the conscious mind and the subconscious mind. Many problems, habits, stresses, attitudes or deficiencies can be traced to interpretations by the subconscious mind which, when understood by the conscious mind, can reduce or resolve specific problems. The subconscious is also the seat of all memory. Traumatic events can be buried there until the mind is able to process it safely. A major benefit of hypnotherapy is its ability to uncover and bring into the light of understanding the information or experience which may be the cause of a troublesome habit or disorder.

Dr. Esther N. Amagoh Esther N. Amagoh, PT, DPT is the newest member of the Women’s Health Physical Therapy team. She earned a Doctorate Degree in Physical Therapy in December 2012 from Howard University, located in Washington DC. After graduating, she relocated to Richmond Virginia where she is completing a Physical Therapy Residency program to become a Board Certified Women’s Clinical Specialist. Women’s Health Physical Therapy is a clinic that specializes in treating women and pelvic floor dysfunctions and is also one of six Women’s Health Residency programs accredited by the American Physical Therapy Association in the United States. Dr. Esther Amagoh was born in Nigeria and moved to the United States with her family in 1995. She was raised in Richmond Virginia where she attended Henrico County Schools before graduating with a Bachelor’s Degree in Health, Physical Education & Exercise Science from Virginia Commonwealth University in 2010. Dr. Esther Amagoh is an active member of American Physical Therapy Association’s Section on Women’s Health and the Virginia Physical Therapy Association. Through her residency experience, with mentorship from Cora T. Huitt, PT, DPT, BCB-PMD and her courses from APTA’s Women’s Health Section for Pelvic Physical Therapy, Dr. Esther Amagoh evaluates and treats conditions including but not limited to male and female pelvic floor dysfunctions, urinary and fecal incontinence, sacroiliac joint dysfunction, back pain, vulvodynia, coccydnia, dyspareunia, prenatal discomfort, and post partum recovery to name a few. She enjoys visiting new restaurants and traveling with her friends and family members.

Rosalinda Rivera Rosalinda was born and raised in Chesterfield County where she resides today. A powerhouse of energy and devotion, noted for her compassion for the broken and hurting, Rosalinda Rivera is passionate about helping others. Her ability to communicate life-altering messages of love, acceptance and forgiveness has impacted people all over the world. Her father was a former gang warlord and heroin addict. After turning his life around he decided to spend his life reaching other young people. Rosalinda serves as Executive Director of New Life for Youth, a 43-year-old charity based in Chesterfield, founded by her family. Here, people who struggle with addiction learn job skills as they learn to live a sober life. The charity is partially funded by New Life Thrift Store and Furniture For Change, a retail store that helps sustain New Life For Youth through social enterprise, an idea fostered by Rosalinda Together with her husband and three children, Rosalinda serves as Pastor of New Life Outreach International, a multi-cultural gem in the heart Chesterfield and more than 38 churches around the world. Rosalinda has been recognized for her impact on today’s youth by the White House and local government, and honored as one of Richmond, Virginia’s 40 under 40 young leaders impacting the city with great change. Internationally, she has led mission trips to more than a dozen countries, and has consulted with government officials in several countries on the issue of youth addiction. Her weekly blog posts are read by fans throughout the world.

Dr. Nadeem Khan is honored to be one of the first MD’s in the Richmond/TriCities area to perform the Minimally Invasive Laminectomy Decompression. He serves as a consultant for Boston Scientific - a neoromdulation company. Dr. Khan was the first Physician in the TriCities area to be board certified and fellowship trained in pain medicine. Dr. Khan has expertise in dealing with all Neuro-Musculo-Skeletal problems. His special interests are Interventional Spine and Pain procedures to tend to debilitating and painful conditions.

Non-Surgical Solutions For Debilitating Lower Back Pain

L

Lower back pain is one of the most frequent musculoskeletal complaints. Typically the symptoms of lower back pain resolve within four weeks, depending on the pathology associated with the complaint. However, the pain often returns, leading to a high percentage of the American population with a chronic condition requiring lower back pain management. Pain may arise from damage or irritation to structures of the lower back including the vertebrae (bony spine), facet joints, discs between the vertebrae, vertebral ligaments, muscles of the lower back, spinal cord and peripheral nerves, as well as internal organs of the pelvis and abdomen (spleen, kidney, pancreas, liver). In my personal experience, physicians need to evaluate the patient completely. A thorough history and physical examination are integral parts of patient evaluation. The physical exam should be supported by imaging studies. It is not that a physician should only look at the imaging studies and decide on a plan of care. Without evaluating the patient, one should not rush to expensive diagnostic studies or extensive surgical interventions. Accurate diagnosis and then precise intervention usually leads to a successful or better outcome. The sad part is most of the surgical interventions are done unnecessarily without getting to the precise etiology. I believe in treating each patient as a whole. I offer many options for pain relief, and create an individualized treatment plan for each patient. Patient treatment plans may include many facets from my comprehensive care program,

including conventional pain management modalities.

The primary focus of Interventional Pain and Spine Specialists is to provide comprehensive pain care originating from spine and various musculoskeletal sources while fulfilling a responsibility to enrich the field of interventional spine pain care. As an interventionalist, I use thorough history and physical examinations, supported by various imaging studies, electro diagnostic evaluations, and targeted x-rays guided interventional procedures, to accurately diagnose and effectively treat painful spinal and musculoskeletal conditions.

Cora Huitt has been practicing physical therapy for forty-one years. She has been an adjunct professor and clinical instructor at several universities. She is a certified women’s health specialist in pelvic floor dysfunction. Her clinic Women’s Health Physical Therapy is one of six credentialed by the American Physical Therapy Association in the United States and specializes in treating women and pelvic floor dysfunction. Cora is an affiliate member of the American College of Obstetrics and Gynecology, International Continence Society, National Vulvodynia Association and International Society for the Study of Women’s Sexual Health. She earned her Bachelors of Science at Ohio State University, Masters in College Teaching at University of North Carolina, and her Doctor of Physical Therapy at Alabama State University.

Why Didn’t I Know…

T

…that after having a baby, a women should do her 3 B’s? As soon as most women discover that they are pregnant, they try to learn and do everything they can to ensure a healthy pregnancy and delivery. From the moment the baby is born, women are thrust into an exciting and exhausting lifestyle. Many women take care of everything but themselves. Besides trying to lose the baby weight and get their mojo back, women pay little attention to their bellies, backs, and bottoms. Regaining or improving all of these areas prevents prolapsed conditions (the descent of bladder, rectum, or vagina), incontinence, back pain, pelvic stability, and improves sexuality. Postpartum is a critical time for women to take care of their bodies. It is a common occurrence— you have a baby, so you exercise to get your body back. Unfortunately, women in general do not get checked for abdominal diastasis, or if they have pelvic floor muscles that have too much or too little tone, or are not working functionally. Urinary stress incontinence, which is the involuntary loss of urine after coughing, sneezing, or during exercise, affects up to 50 percent of postpartum women. Stretched, weakened, and separated muscle fiber during pregnancy can

lead to a flabby looking tummy. Abdominal diastasis is when the abdominal muscles shift to the left or right side with fascial stretching or tearing between the halves. Diastasis recti is weakness in trunk support. Back muscles often remain tight and shortened due to increased weight gain, and the body’s center of gravity shifts from increased ligament laxity when a woman is pregnant or nursing. Over 41 percent of women suffer from back pain. Women are more vulnerable than men due to menstruation, pregnancy, childcare, and domestic activities.

By tailoring an exercise regimen to this mindset, women are able to achieve the strong foundation of belly (abdominals) that support the back and spine, to regain normal spinal curves and bottom (pelvic floor muscles), that lift the bladder, vagina, and anal canal. All of these work to synergistically give the pelvic bones stability as well as closure around the urethra to prevent leaking. Physical therapists are skilled in evaluating and providing individualized treatment for musculoskeletal problems. Physical therapists specializing in women’s health have additional expertise in working with women who experience physiological musculoskeletal, psychological, and social changes during the child bearing years.

Hormonal changes from pregnancy can remain in the body for three months after delivery. Resuming normal exercise may be dangerous if abdominal muscles have split. Repetitive strain injuries can occur during pregnancy, from lifting the baby or stroller, and from breast feeding positions. The depth of research over the last twenty years provides clinical evidence:

Hormonal, weight, and body size changes contribute to altered posture, joint pain, poor abdominal, pelvic floor, back extension, and diaphragm breathing. Postpartum needs of women, whether in a developed or underdeveloped country, are often not addressed as much as pregnancy and birth. Unlike in some other developed countries, the United States does not have regularly recommended postpartum education/exercises to return to normal posture and body mechanics.

1. Pelvic floor muscle strength significantly decreases following vaginal delivery, even years afterwards. 2. Pelvic floor muscle training can help to maintain continence long-term. 3. The deep pelvic floor muscles play a large role in supporting pelvic organs. 4. People who receive training of the pelvic floor muscles had an elevated bladder and rectum with less severe and more infrequent pelvic organ prolapse.

move during contractions) check for a rectus abdominis belly separation. The evaluation allows observation of collective functioning of abdominal, pelvic floor, back muscles, and proper breathing when exercises are performed. Taking care of someone with Alzheimer’s Disease can be a demanding task. It requires time and energy. Even if you are young and in good health, giving care can be difficult. If you want to continue to give care, it is essential that you look after yourself. Many caregivers suffer health problems as a result of the intense demands on them of looking after someone. Your body will let you know when you are working too hard or feeling too much stress. There will be signs. For example, you may notice that you are more easily upset, feel on edge, get angry or cry more easily. You may sometimes feel that you just can’t cope. You may have tense or sore muscles, stomachaches, headaches or problems sleeping. You may be much more tired or get more colds than usual. Realize that you are smoking, eating or drinking more. You are less able to concentrate. Simple tasks take longer. It is important to realize that caring for someone takes time and energy. There will be limits to what you can do. You will have to decide which things are most important to you. Which do you value most? A walk

with the person you are caring for, time by yourself, activities you have always enjoyed with others or a clean and tidy house? There is no “right” answer to this question; only you know what matters most to you at any particular time. Besides making choices, you will have to set limits on what you do in a day. It may be difficult to admit that you can’t do everything. It is not easy to say “no.” To be realistic, you will need to think carefully about how much you can do.

Ultra Violet Rays May Cause Damage To The Eyes Today And Tomorrow

You may remember the lyrics to the 1980s song “The Future’s So Bright, I Gotta Wear Shades.” What you may not know is how true the title actually is. In fact, ultraviolet (UV) rays can cause immediate as well as lasting damage to your precious vision. Ultra Violet (UV)-blocking sunglasses and hats are the ultimate sight-saving outdoor accessories, and should be worn any time you and your loved ones are outside, regardless of the temperature. Prevent Blindness Mid-Atlantic recommends wearing sunglasses that: •

reduce glare

filter out 99-100 percent of UV rays

protect the eyes

are comfortable to wear

do not distort colors

Sunglasses without UV protection may shade the eyes but actually cause the pupils to dilate, allowing in even more harmful rays. Sunglasses, especially for children, should be made of unbreakable polycarbonate for active lifestyles. The delicate skin around the eye and the eyelids is also susceptible to UV damage. According to the Office of Air and Radiation at the Environmental Protection Agency, basal cell carcinoma is the most common type of skin cancer to affect the eyelids and may appear on the lower lid, in the corners of the eye and under eyebrows. When going outside, both adults and children should always wear a wide-brimmed hat or cap and the proper UV-rated sunglasses. Wraparound sunglasses are best as they protect the eyes and the skin around the eyes. Some contact lenses may offer UV protection but they cannot protect the entire eye and the skin around it. While UV-A has lower energy, it penetrates deep into the eye and may injure the macula, the part of

the retina responsible for sight in the center field of vision. UV-B radiation is presumably more dangerous and is mainly absorbed by the cornea and lens of the eye and can damage those tissues. Exposure to any UV rays can cause damage to your eye health and vision, now and in the future.

If at the beach or on the ski slopes, sunglasses should be worn that have a darker tint to block more light. The risk of eye damage from the sun is greater because of reflection off the water and snow. Also, you should always wear eye protection when using a tanning bed. According to the American Academy of Ophthalmology, tanning beds can produce UV levels up to 100 times more than the sun, which can cause serious damage to the external and internal structures of the eye and eyelids.

Photokeratitis, or corneal sunburn, is a result of intense exposure to UV-B. It is most common among individuals who spend long hours on the beach, in the water or on ski slopes without proper eye protection. It can be extremely painful and can result in temporary loss of vision for 1-2 days. Pterygium, a growth of tissue that forms on the white of the eye, also correlates directly to an individual’s UV exposure. Left uncorrected, this condition may need surgical treatment.

UV exposure can cause immediate damage to the eyes, as is the case with a corneal sunburn, or cause damage that surfaces later in life. The key to healthy vision is to make eye protection part of our daily routine. We encourage everyone to make a committed effort to wear UV-blocking sunglasses year-round to help protect the gift of sight. As you enjoy the warmer weather and your increased time outdoors, remember to wear your sunglasses – and make sure your children, family, and friends are wearing theirs as well. This is a big part of protecting your healthy VISION FOR LIFE!

UV damage is also cumulative and has been linked to eye problems later in life, including tumors, cataracts and macular degeneration, an eye disease which currently has no cure. Also, people who have had cataract surgery or other retinal disorders, and people who take certain medicines, such as tetracycline, sulfa drugs, birth control pills, diuretics, and tranquilizers are at special risk. Ask your doctor about prescription medication that may cause increased sensitivity to light. A survey conducted by NL3 Optics found that more than 40 percent of runners and 35 percent of water sports participants do not consistently wear sunglasses during those activities. And, only 50 percent of men and 33 percent of women make it a priority to have the right sunglasses for their sport. Reflective surfaces like snow, water, sand and asphalt can greatly increase the amount of UV radiation

Improving Your Abdominal Appearance:

F

The desire to improve abdominal appearance is ubiquitous. For some people, it’s about improving contour and having that chiseled appearance to look better than normal. For others, it’s about reducing the quantity of skin and / or fat just to get to a point where they once again look normal. For some, achieving these goals may be easy, whereas for others, it may be very difficult. One of the consequences of getting older is that our metabolism slows down and we tend to accumulate fat in areas that we would like to keep thin, namely the hips and abdomen. With the increasing emphasis and importance of maintaining good health, nutrition and exercise, many people are able to achieve these goals without surgery. However for some, losing that excess skin and fat may prove to be more of a challenge because there may be diet and exercise resistant fat. For those individuals, surgical options such as abdominoplasty or liposuction may be considered, but only when they are good candidates. I want to emphasize the term “good candidates” because there are many factors that must be appreciated when considering surgical approaches to abdominal contouring that include patient expectations, general health, body weight, and quantity of skin and fat. Some patients will be good candidates for liposuction, some will be a good candidate for abdominoplasty, and some may not be a candidate for either. This monograph will review the current indications and surgical options for improving abdominal contour as well as to offer some assistance for individuals considering which option may be most appropriate for them. Abdominoplasty (Tummy Tuck) According to statistics compiled by the American Society of Plastic Surgeons (ASPS), over 106,000

to optimally contour the skin. Your plastic surgeon will review this during the consultation and determine which would be most appropriate. Some women may have a localized area of fat excess and be a better candidate for the less invasive procedures such as liposuction rather than a tummytuck. Liposuction will be reviewed later in the monograph.

abdominoplasties were performed in 2012 with the vast majority being women (reference: www. plasticsurgery.org). This represents a 70% increase since 2000. The main reason why women choose abdominoplasty is because they want to improve abdominal appearance by simultaneously removing skin and fat. Many will report one or more factors that have led them to this decision that include but are not limited to pregnancy, advancing age, weight fluctuations, heredity, and prior abdominal operations. In order to be considered a good candidate for a tummy tuck, one should be in good physical and mental health, maintain a stable weight, and avoid all tobacco products. It is always emphasized that proceeding with a tummy tuck should not be perceived as a substitute for dieting, exercise, and weight loss. In women, that are overweight, my advice is to attempt conservative measure such as dieting and exercise first in order to stabilize their weight. In women that are able to do this successfully, results have been more predictable and reproducible and women have been generally happier with the results. I also advise women that are considering future pregnancies to postpone having a tummy tuck until having children is complete.

Your surgeon should also review the risks and benefits of having a tummy tuck, take photographs, and discuss cost. As with any operation, there are certain adverse events can occur with an abdominoplasty. These include but are not limited to bleeding, infection, scar, fluid collection, delayed healing and contour irregularities. Fortunately, these are uncommon and the vast majority of women are very pleased with the outcome. The cost of an abdominoplasty will vary based on where you live and surgeon experience. Keep in mind that this is a cosmetic operation and rarely covered as an insurance benefit. In general, you should plan on 30 to 45 minutes for the consultation.

During the initial consultation, your board certified plastic surgeon will determine if you are a good candidate based on the factors reviewed. Some women will be candidates for a miniabdominoplasty that requires less skin and fat removal; whereas, other will require a full-abdominoplasty

The procedure itself is generally straightforward and usually takes about 2 hours. General anesthesia is usually recommended but it can sometimes be performed under twilight anesthesia in conjunction with a local anesthetic. The incision usually extends from hip to hip and is located just above the pubic bone so it’s is nicely concealed within the bikini line. Sometimes a second incision around the belly button is necessary in situations where extensive abdominal contouring is needed (full-abdominoplasty). In some women that have had children, the muscles supporting the abdomen may slightly separate resulting in a condition known as rectus diastasis. This condition can be repaired during an

abdominoplasty by suturing the supportive structures and muscles back together along the midline to eliminate that central midline bulging. Once the excess skin and fat are removed, the incision is closed in layers using dissolvable sutures. A drainage tube is usually placed above the muscle layer to remove any fluid that may accumulate. This is usually removed a few days after surgery. An abdominal binder may also be recommended for support and to reduce the amount of swelling. Abdominoplasty is usually performed as an outpatient procedure but on rare occasions, overnight hospitalizations may be recommended for safety reasons. Following the tummy tuck, your abdomen will be firmer and the excess skin and fat will be gone. Initially, there may be some mild swelling but this diminishes after a few weeks. Your plastic surgeon will prescribe pain medication and antibiotics (when needed) as indicated. Be prepared to walk in a slightly flexed position at the waistline until the incisions heal and the skin elasticity returns (usually a few days). I typically instruct patients that it’s ok to shower and get the incisions wet after 3 days. I also advise patients that it’s ok to drive when they are off their pain medication and they have full range of motion (usually 7 – 10 days). Most patients are able to resume normal activities after about 1 month.

Abdominal Liposuction Liposuction has been one of the most common cosmetic procedures

Exercise, Liposuction or Abdominoplasty performed in the USA. According to ASPS statistics, over 200,000 liposuctions procedures were performed in 2012 (reference: www. plasticsurgery.org) . Abdominal liposuction is ideal for women with a localized area or areas of excess fat. It is an excellent way to improve abdominal contour and proportion in a much less invasive manner. Although liposuction has been shown to be safe and effective for fat removal, it should not be perceived as a substitute for weight loss, good eating habits, and regular exercise or as a treatment for obesity. Individuals considering liposuction should consult a board certified plastic surgeon to determine if you are a good candidate. Ideal candidates for abdominal liposuction are in good physical and mental health and are within 30% of their ideal body weight. Good candidates for abdominal liposuction will typically have firm, elastic skin with good muscle tone. As is the case with all patients seeking cosmetic surgery, avoidance of tobacco products is strongly recommended to reduce the risk of complications. During your consultation, the plastic surgeon will typically assess your expectations and what you hope to achieve, obtain a comprehensive history, perform a physical examination, review the treatments options, discuss complications, and take photographs. As technology has made a big splash in our everyday lives, so have the options for liposuction. Traditional liposuction is still the most common and involves the insertion of a blunt tip cannula into the area of localized fat in order to suction out the fat. This is achieved using a negative pressure apparatus to generate the suction force. Newer technologies will use ultrasound or

radiofrequency waves to dissolve or breakup the localized fat followed by the suction phase. Still newer technologies will utilize external devices that will cool or freeze the localized fat resulting in fat dissolution. Your plastic surgeon should review these options and determine which is preferable for you. Whenever considering liposuction, you should be aware of the potential complications. Fortunately, liposuction is considered a relatively simple procedure and the vast majority of patient do very well and are very happy with the outcome; however, there are situations when things may not go as expected. Adverse events are related to various factors that include but are not limited to the length of the procedure, the amount of fat removed, and whether or not additional procedures are performed. Some of these adverse events include irregular contour, waviness of the skin, fluid collections, infection, and the need for a “touch-up� or additional procedures. More serious adverse events include blood clots (deep venous thrombosis) with dislodgment of these clots to the heart and lungs (pulmonary embolus). Most liposuction procedures are performed as an outpatient; however, in some situations, an overnight stay in a hospital like setting is recommended. Some of the reasons for an overnight hospitalization include removal of more than 5 liters of fat or it the operation requires more than 5 hours. Fluid and electrolyte imbalances may occur in these situations. Performing simultaneous operations such as breast surgery and abdominal contouring procedures may also increase the chances of a complication.

The steps of the traditional liposuction operation are relatively straightforward. Liposuction is sometimes performed under general anesthesia (completely asleep) or twilight anesthesia (light sleep) with the addition of a local anesthetic agent. Your plastic surgeon may recommend one or the other. Small incisions will be made at specific locations on your abdomen followed by the instillation of a tumescent fluid into the fat compartment. This fluid is comprised of saline, lidocaine, and epinephrine and is used to constrict the blood vessels in the area, to minimize intraoperative and postoperative bleeding, and to expand the fat compartment. About 20 minutes later, a thin hollow tube or cannula is inserted and attached to a suction device to remove the fat. This is performed using back-and–forth movements either manually or with a powerassisted device. The suctioning is continued until the desired contour is achieved. The small incisions may be closed or left open and bandages are applied. Recovery following liposuction of the abdomen is usually easier than abdominoplasty. Patients should refrain for strenuous activities or exercise for about 1 month. Compression garments are

essential. I explain to all patients considering liposuction that removal of the fat completes about 60% of the task and that adequate postoperative compression completes the rest. For this reason, compression garments are recommended and should be worn continuously for several weeks. Failure to do so will result in prolonged swelling and a less desirable outcome. Showering is permitted 3 days following liposuction. Pain medication and antibiotics are prescribed as indicated. Most patients are able to resume normal activities after 4-6 weeks.

In summary, both abdominoplasty and liposuction are safe and effective methods to achieve improvement in abdominal appearance. That said, dieting and exercise are highly recommended both before and after these procedures. When considering these options, it is recommended to consult with a board certified plastic surgeon to determine which may be the most appropriate.

Many women are afraid of mammograms but having one is definitely worthwhile. In fact, it can even save a woman’s life. If the test is normal, you will feel better knowing that you are OK. If however; there is a problem, it is better to know early. Regular mammograms can detect breast cancer BEFORE it spreads. Treatment works best when the cancer is found early. When should I have a mammogram?

Starting at age 40 women should have a mammogram every 1 – 2 years. If you feel a lump, see or have questions about anything unusual in your breast contact your health care provider.

How do I get ready for my mammogram? • Make the appointment for 1 week after your period • When the breasts are less sensitive. • Wear a shirt with shorts, pants or skirt. • This makes it easier to undress from the waist up. • Do not wear deodorant, perfume, lotion or powder under arms or on your breasts on the day of your appointment. These products make shadows on the mammogram.

Your Mindset Could Be Keeping You Fat

Have you been constantly on a diet yet you always stay around the same weight? Have you been trying to lose weight, but it keeps finding you again a couple months after you have stopped your diet? Have you tried every weight loss program in the world and have decided that none of them work? If you are reading this and nodding your head up and down or you know someone that falls into this category, then I have a new concept for you to consider. The problem with your weight loss may not be your diet program, but instead, your thoughts and feelings that are sabotaging you. Stressful situations or stressful decisions about your weight loss can derail your weight loss goals and success. In my 10 years of practice I have learned that if you don’t address the emotional side of weight loss, you will not be successful at losing weight. As humans, we are emotional beings and it doesn’t matter how much you are an expert about a particular subject, your decisions will often be affected by your emotional state of mind. So you could know every fact about healthy eating and exercise routines but the minute you get upset with a family member about something, the next thing you know you are on your second piece of chocolate cake. If you don’t recognize when your emotions are controlling your eating decisions, you will not be successful at losing weight or keeping it off. According to a survey conducted by the Consumer Reports National Research Center, over 1,300 licensed psychologists agree that any successful weight loss plan must take into consideration that “understanding and managing the behaviors and emotions related to

weight management is essential to weight loss.” So what are examples of these emotional behaviors they have identified? •

Emotions about making healthy food choices

Emotions related to maintaining a regular workout

Gaining self control over behaviors related to weight loss

Feeling guilty about poor behavioral choices that cause you to fall off your diet regimen

Emotions related to other stressful situations in your life

ability to make effective decisions regarding your weight loss program can become compromised. When you create strategies for how you will deal with common weight loss problems, before they happen, you are more likely to be successful with your weight loss goals. Having a plan allows you to execute your plan before the stressful situation arises. In my practice, I have created a protocol that includes strategies focused on education about proper eating behaviors, combined with education to deal with stressful situations related to your dietary plan as well as your life. Here are the key components that have created success for my patients.

How in the world can you manage your emotional behaviors to create successful and sustainable weight loss? Simply put, you have to start with the right emotional attitude about your weight loss goals. In order to have sustainable weight loss (weight loss that you can keep off for a long period of time), you have to have realistic expectations for the amount of weight that you want to lose. Realistic expectation for weight loss is 1 – 2 pounds per week. Many of us want to lose too much weight too fast so that when we can’t lose 10 pounds in 10 days, we get emotionally frustrated and we give up.

Goal-setting before we start the program

Record-keeping of successes and setbacks during the plan

Therapies to identify and address negative emotions that trigger unhealthy eating and avoiding those triggers

Detailed balanced whole food plan

What are the best times and amounts of food to eat daily

What is a realistic exercise regiment for your lifestyle

Finding solutions to setbacks and obstacles (before they arrive)

Mindfulness training to reduce judgment and self-punishment for bad behaviors

Protecting Your Rights And Your Children, When Facing Divorce

They say experience is the best teacher. I have been through a divorce, and now I am an attorney helping couples who are going through a divorce. So I understand both the emotional and the legal aspects of the painful process of divorce.

While hardly an exhaustive catalogue of divorce do’s and don’ts, this list presents 7 key steps you must take when going through a divorce. If you follow these rules, the entire process will be easier. After writing this list, an eighth point occurred to me that is actually the most important consideration of all. I certainly found this insight truthful when going through my own divorce. Here it is, in short: Be Kind and Be Civil Divorce is a raw and painful process, and there is a tendency to grow angry, bitter, and vindictive. Avoid those toxic traps wherever possible. Do not inflict needless emotional pain on your spouse. Don’t become greedy and try to get more than what you’d award yourself as an impartial judge— since you may very well end up in front of one. If you have children, you and your spouse will continue to be partners in the most important project of all: raising wonderful kids that you hope will become productive, contributing, emotionally healthy members of society. Don’t mess up their future with the emotions of your present. The painful emotions associated with your divorce will eventually pass. You do not want these transient problems to wreak permanent damage on your precious children, or your relationship with them, do you? Of course not! With this consideration in mind, here are 7 Key Steps You Must

both parties’ finances will make this process easier and more fair. 4. Know your debt and your spouse’s debt Cancel all joint credit card accounts or lines of credit. You will probably be jointly and severally liable for any charges made on these accounts. You do not want to be on the hook any debts racked up by your spouse. For similar reasons, you should also cancel all joint checking accounts. 5. Change your will If your spouse is a beneficiary in your will, this status will not necessarily change when you divorce. Consult a competent wills, trusts, and estates attorney to ensure that your will is revised to reflect your post-divorce wishes. You will also want to ensure that other important documents, such as life insurance policies, are appropriately amended.

Take to Protect Your Rights and Children: 1. Know the law and your rights as a divorcing spouse While you should not be greedy, there is no reason to give in to your spouse’s demands without knowing your rights. For example, do not move out of your house simply because your spouse demands it, or agree to a seemingly unfair settlement proposal without knowing if you have better options. Obtaining wise legal counsel to guide you through the divorce minefield is absolutely necessary to ensure that all of your rights are preserved. 2. Know your rights as a parent As a parent, you have rights with respect to custody, visitation, and child support. Your relationship with your children and the material well-being of your children should not suffer because you failed to exercise your rights, and theirs. Obtaining competent legal counsel is essential to ensure that your relationship with your children is fully protected. 3. Know your assets and your spouse’s assets In particular, keep a very careful budget. Divorce can be costly and complicated, and you want to ensure that when everything is over, you will be financially stable. Also, you and your spouse’s available assets will be key to determining child support and alimony. A clear and well-maintained picture of

can raise suspicions of adulterous behavior and create unnecessary prejudice in the eyes of the judge. This is particularly true if you are cohabitating with someone other than your spouse. Even worse, it can make an already emotionally complicated situation even more difficult and confusing, particularly for children. 7. Decide what you want in an attorney Do you want someone who takes an impersonal, hands-off approach, or someone who is available for texts and calls at all hours of the day? Do you want a technocratic practitioner who has no direct experience with divorce, or someone who has been through the process, understands your heartache, and can represent a source of emotional support as well as legal counsel? For many people, having an attorney who understands the heartache, stress, and frustration of divorce is an invaluable asset when going through the process.

Many unforeseen stresses and challenges can come with a cancer diagnosis. LINC is helping cancer patients in the community address these issues – patients like Keith Chantree. Keith was diagnosed with sarcoma and contacted the Legal Information Network for Cancer (LINC) for help with several legal issues. Client Services attorney, Cris Gantz drafted his life planning documents and advised him on employment and family law issues to ensure his affairs were in order. After receiving an update from his doctor about treatment options, Mr. Chantree made the brave decision to have his lower leg amputated to remove his cancer. Prior to being diagnosed, Mr. Chantree worked a physically demanding job and needed a special type of prosthetic to return to work which would cost him personally over $30,000. Mr. Chantree’s insurance denied coverage of his prosthetic telling him that he could use crutches for the rest of his. He was distraught and needed some guidance to appeal the decision. He called LINC for help, and Cris referred him out to a volunteer professional who was able to advise him on the process. Mr. Chantree completed his appeal and successfully got his insurance company to cover 100% of the cost. LINC has been assisting cancer patients like Keith with their legal and financial problems in the Greater Richmond area since 1995. Many of these services are provided by dedicated volunteers and to honor them, LINC will be holding its annual awards luncheon on May 16th at the Virginia Historical Society. The awards and honorees are: the Ronne Jacobs Leadership Award to Debbie Johnston; the Allison Held Volunteer Award Sally Watkins Bobbitt and Karen Fitzsimmons Miller; the Krista

Latshaw Pro Bono Award to Joley Eason; and the Outstanding Law Firm Award to Sands Anderson PC. Special Guest Speaker is Garth Callaghan, known as the “Napkin Notes Dad.” Debbie Johnston is the founder and president of Care Advantage. She has been recognized as the Entrepreneur of Richmond, Philanthropist of the Year, and Woman on the Move. Care Advantage, Inc. was born out of a need to help people once discharged home. This has naturally evolved from helping patients leave the hospital to holding their hand and making their last few hours the best possible. What Is Acupuncture?

Chinese Acupuncture is 4,000 – 5,000 years old. It is the insertion of needles just under the skin. The needles are only as thick as three or four pieces of hair. Acupuncture is treating the whole person to a deeper level so that not only do the symptoms go away but also the subtle causes of theses symptoms. Most patients who have acupuncture treatments steadily get better with no ill side effects. Acupuncture is also a great treatment for people with negative emotions, problems with anger and grief. There are certain treatments for many emotional states including fears and phobias. Many people claim that their stress level drops and they sleep better after only a few treatments. How does it work? Let’s use an example to explain the process. A person with Asthma may experience shortness of breath, lung mucus, inability to breathe and chest tightness. They may take Western medicine and get better. Acupuncture does not interfere with medicines. However, the patient has a sense that something is wrong with their lungs and that their lungs are weak. To an acupuncturist each organ has a unique pulse found on the wrist, usually with Asthma the lung pulse is quite weak. Treatment will

involve acupuncture points, which open the lungs and strengthen the life force of the lungs. This process should be done weekly until the lung pulse again becomes strong. One of their most popular programs is LIVESTRONG® at the YMCA, a 12-week program designed to help adult cancer survivors achieve their holistic health goals. The small group offers cancer survivors a safe, supportive environment to participate in physical and social activities. Survivors work with YMCA certified instructors to build muscle mass and strength, and increase flexibility and endurance while improving confidence and self-esteem. In addition to physical benefits, LIVESTRONG® at the YMCA focuses on the emotional well-being of survivors and their families by providing a supportive community for people impacted by cancer. By focusing on the whole person and not the disease, LIVESTRONG® at the YMCA helps people rebuild spirit, mind and body. The program is proving to be a vital asset. According to Cancer.org, 68 percent of people diagnosed with cancer survive. LIVESTRONG® at the YMCA fulfills the important need of supporting the increasing number of cancer survivors who find themselves in the transitional period between completing their cancer treatment and the shift to feeling physically and emotionally strong enough to attempt to return to their normal life or their “new normal.” Classes take place not at medical facilities, but at the Y, to emphasize that the program is about health, not disease. The class size is generally 8 to 10 participants with a maximum of 12. Local Ys -the Atlee Station Family YMCA, Petersburg Family YMCA, Tuckahoe Family YMCA,

Downtown YMCA and the Midlothian Family YMCA - are excited to bring a health-based solution to people in the community to give them the tools they need to get back on the road to good health after cancer. To date, approximately 6,000 cancer survivors have participated in this program nationwide. Mary Martin of Richmond, Va. is one of them. As a participant in LIVESTRONG® at the YMCA, she admits that exercise was never high on her priority list, until she was diagnosed with breast cancer in November 2011. Mary underwent surgery, but the process left her body weak. That’s when her cancer support group leader suggested she join LIVESTRONG® at the YMCA. Mary said “My support group leader thought that I should go because it would help with my strength, endurance and overall well-being. When I first went, I wasn’t excited because I’ve never been into group exercise. But then, I started to enjoy it.”

Smith is the Regional Hospital Accompaniment Response Team Coordinator for this collaborative initiative where she trains volunteers, nurses, and police to respond to survivors of domestic and sexual violence at Central Virginia’s emergency rooms. Fatima also provides educational workshops on domestic and sexual violence, healthy relationships, teen dating violence, and stalking. Fatima received her undergraduate degree from George Mason University and her Masters of Social Work from VCU.

Sexual Assault Awareness, Start A Conversation

T

The month of April is Sexual Assault Awareness Month, designated as a call to action across the country to speak out against sexual assault and connect to survivors. Teal ribbons are worn during Sexual Assault Awareness Month to raise awareness and show support for the cause. Since November 2009 a special team of volunteers has been on call 24 hours a day, 7 days per week, 365 days per year. If someone requires medical treatment because of sexual assault or intimate partner violence, R-HART (the Regional Hospital Accompaniment Response Team) is there. R-HART volunteers provide advocacy and emotional support to survivors of intimate partner violence & sexual violence during forensic or medical examinations. R-HART is a collaboration among Hanover Safe Place, Safe Harbor, the YWCA of Richmond, Bon Secours Richmond Health System, and the VCU Medical Center/MCV, and this team of volunteers responds to patients in the emergency rooms at 6 of their locations. The data collected from R-HART coupled with national research indicates that sexual assault is an epidemic that is negatively impacting our communities. The national research suggests that one in six boys and one in four girls will experience sexual assault before the age of 18. Between July 2012 and June 2013, seventy-three Chesterfield residents reported sexual assault. Of those seventythree, thirty-six received hospital accompaniment services through R-HART. Between July 2013 and February 2014, 51% of R-HART calls involved sexual assault and twenty-eight Chesterfield residents received hospital accompaniment service. The YWCA of Richmond provides individual counseling, support group, and

case management services to Chesterfield residents. These services are offered at zero cost. Sexual assault is defined as “any type of sexual contact or behavior that occurs without the explicit consent of the recipient,” and may include forced kissing, groping, attempted rape, and rape. These acts are a crime in all 50 states and can be experienced by children, teens, men, women, lesbian, gay, transgender, and the elderly. Every sexual assault is unique and each survivor will respond differently to the trauma of the assault. It is important to remember that the survivor’s experience, feelings, and reaction are valid and to be supportive of the survivor. It is not unusual for a survivor to never speak of the assault or wait several years before disclosing. It takes an enormous amount of courage and strength to discuss their story. If a survivor shares their story, recognize their resilience, listen, believe their story, and respect their wishes by allowing him/her to have control. An individual’s ability to create a space where a survivor feels safe and comfortable enough to share their experience reflects support of the survivor.

Sexual assault may seem like an overwhelming and daunting issue to tackle; however, there are opportunities for individuals to be a part of the solution and affect change. Individuals can help by challenging cultural norms regarding sexual violence, speaking against victim-blaming, and sharing resources. The opposite of sexual violence is consensual expressions of healthy sexuality. In order to shift cultural norms there needs to be a discussion about the behaviors we want to see. Everyone has the right to make consensual and informed choices, which are free of violence and control. Consent is established when all parties involved agree on what is going to happen. Obtaining

Karen’s story is one of many. Her journey with Safe Harbor started on the hotline, as she painfully spoke of the injuries she received at the hand of her husband. She was also suffering from drug addiction, and the complexities of having no family support. Karen had a protective order against her husband, which he continually violated. It was at that point that Karen was admitted to Safe Harbor’s emergency shelter to keep her safe.

Everyone hopes to live a violencefree life, but the reality of that is often hard to comprehend. Karen’s dream included returning to school and re-establishing relationships

with her family. While at Safe Harbor, Karen’s goals became the focus of her counseling. She participated in support groups and learned that she was not alone. Karen found employment, and once stabilized, she moved to a transitional home and resumed her studies. She excelled in an honors program at a local community college and obtained a scholarship at a local university. Her daughter proudly accompanied her to the honors ceremony. Unfortunately, Karen’s story is all too common. The need for sexual and domestic violence intervention services is compelling. According to the Virginia Behavioral Risk Factor Surveillance System, one in four women and one in eight men will experience intimate partner violence (IPV) at some point during their lifetime. In 2012, Virginia’s Office of the Chief Medical Examiner reported more than one out of every three homicides was family or intimate partner related. In Virginia, the majority of firsttime sexual assaults occur when victims are under the age of 18. Safe Harbor is a resource center for people who have experienced sexual and domestic violence. Their mission is to empower survivors of sexual and domestic violence to transform their lives, and promote healthy relationships for all. Services are provided to anyone who has experienced sexual and domestic violence regardless of race, age, gender identity, faith, sexual orientation, and geographic location. Their 24/7 services are available in both English and Spanish, and are offered free of charge to anyone seeking help. More than one thousand new victims are helped each year, and the staff interacts with an average of 42 clients daily. Families can

Slipcovers Slipcovers are a not an inexpensive panacea for covering up that ugly sofa. They’re designed to change the look of a piece of furniture seasonally or occasionally, but not permanently. Plus if you have pets or children, a slipcover will be a bad investment as they will show wear faster and need constant adjustment. Let’s move on…

When considering a new sofa, one must know how to spot the quality from the duds, and this starts on the inside. The frame of the sofa tells you everything about the quality. A kiln-dried hardwood without knots (which cause squeaks later) is the top choice for a frame. Some low-quality frames use cardboard. Cardboard! The legs should be continuations of the corner joints, and not detachable. An eight-way hand-tied seat is the ultimate in comfort because it allows for uniform movement and can last for decades without wearing out. Foam cushions won’t last, so be sure to go for a spring

The fabric plays an important role too, as it is the only visible part of the sofa. Always look for pattern matching in all four directions, which is a sign of hand-cut and hand-sewn upholstery. All fabrics will wear eventually, but some are more durable than others. Never get suckered into a fabric that has “super-powers” as part of a sale! Most manufacturers test the durability of their fabrics, so ask to see the test results on every fabric you’re considering.

I was lucky enough to grow up in a well educated, affluent family that had the ability to afford me every opportunity parents could want for their child. I went to a prestigious private school, participated in sports, and extracurricular activates. I had great friends and managed to stick to the principles my parents had brought me up to believe in, such as; hard work, honesty, integrity, and self sufficiency - effectively keeping me out of harm’s way. When it was time to go to college, I was happy to go. I had been accepted into a prestigious school in Boston that I was excited about, and felt that it was time to stand on my own two feet and start carving my own way in the world. College accomplished for me what any realistic expectations could hope for: The ability to think analytically and express myself appropriately. However, the reason I chose my school, and the reason most people I know went to college, was not for these rather humble aspirations. I was career focused and wanted college to be my ticket into a world of opportunity. What I found was my analytical mind saying, “You have got a $120,000 education. What now, hot shot?â€? Although I had a good experience at college, I was not an academic at heart and knew that spending another $100,000 for post grad was not for me unless absolutely necessary. Of course it was time to get a job, but where? I started contacting all the advertising companies I had been studying over the past four years and managed to land a handful of interviews. It became quite clear that nobody was hiring, and that the whole industry was slow enough to grant interviews as a way of distracting themselves from the fact that they didn’t’t have enough work to do either. The offers I did get would have given “education investmentâ€? a return in about 6 years as long as I didn’t’t need to pay rent or eat. A Wine Rivalry: California Vs. Oregon

R

Rivalries are a guarantee. There always have been and always will be bitter rivalries throughout any situation when one product declares itself “The Best.” Our free market system thrives on the competition created through demand. Every good product has its rival. Whether it be with your favorite sports team’s arch rival or Pepsi vs. Coke, the competition continually gets better and better. It grows into its own and becomes itself. The rivalry becomes bigger and more important than the product. The Duke vs. UNC game is important, but the overall history of the events and moments are what is remembered. California and Oregon have created a rivalry of their own over the topic of Pinot Noir. A once super-power of all American wine-making, California has never been doubted for possessing the best of the best. That was until Oregon started taking some of the spotlight. What is the difference? Is it the taste? Should I always buy an Oregon or a California Pinot Noir?

more earthy aromas and subtle fruit flavors. Oregon contains one of the most famous Pinot Noir growing areas in the United States. Willamette Valley was found to have the perfect climate for the production of this fickle grape. The Valley is sub-divided, based on climate, soil and elevation, into six different appellations. They are the Ribbon Ridge, Eola-Amity Hills, McMinnville, Yamhill-Carlton District, Chehalem Mountains and the Dundee Hills. The main difference between this area and its counterpart in California is that it is significantly cooler for a longer part of each year. The soil and climate most similar to those is Burgundy, which is where the grape is originally from and has been cultivated for more than a thousand years.

California Pinot Noir California’s Pinot Noirs are going to be more full-bodied and fruit forward than Oregon’s. The flavors and aromas of juicy red fruits will definitely stand out much more boldly. This is in reaction to its warmer climate. Originally, back in the 1940’s, most Pinot Noir grapes were planted in California’s Napa Valley. It took about 30 more years of education about what the grape actually needs to thrive for Cali to move most of its Pinot Noir production to the coast. The Sonoma Coast, for example, enjoy a much more favorable climate of warm afternoons and cool evenings with the moderating factor of cooling winds off of the Pacific Ocean. The farther you move inland, the grapes get more sheltered from the breeze, resulting

in a hotter and more continental climate. The best results for Pinot Noir in California have been in the Russian River Valley and Carneros region. I believe it is like all wines — it comes down to preference. Those who prefer California wines, by and large, like bigger fruit wines. They want those slap-you-in-the-facewith-a-bunch-of-grapes reds. Those that prefer Oregon wines want more complexity and less in-yourface fruit. One isn’t better than the other, just different, but a difference that makes for a fun rivalry. For me, it depends on my mood and what I may be eating. I will usually lean toward the Oregon Pinot Noir if I had to choose. Saying all of this, nothing will stop me from getting a Napa Cabernet. Why that is, I will leave it for another day. As with all good rivalries, good cases can be made for both. It depends on a variety of different factors. Pinot Noir is a very temperamental grape. There are many subtle variations in the climate, how the wine is fermented, and what kind of influence the winemaker creates. I believe they both produce great wines. Choosing between the two as the matter-offact “better” region for producing Pinot Noir would be unwise and almost childish. It eventually comes down to an individual’s personal preference. Saying that, there are some general differences between the two. Take these guidelines to heart if you can’t decide which to purchase. Oregon Pinot Noir Oregon’s Pinot Noirs are typically lighter and more complex than California’s. They tend to obtain
