Important Questions To Ask When Considering Cataract Surgery In Delaware A
cataract is a clouding of the natural lens in the eye, part of the normal aging process. Typical symptoms include blurry vision, glare, and loss of color and contrast. Causes include aging, smoking, diabetes, UV By light, and certain Jeffrey B. Minkovitz, M.D. medications. The only definitive treatment is removal and replacement with an intraocular lens, though changing spectacles may help early cataracts. Once a patient becomes symptomatic, there are several important questions to ask when considering surgery,
IS SURGERY NECESSARY? The existence of a cataract does not always require surgery. Only the affected individual can decide when vision loss warrants surgery. With modern cataract surgery, not only is restoring vision possible, but oftentimes vision is improved to levels better than before the cataract. The surgeon can help determine whether vision loss is due to cataract, or to another condition (such as macular degeneration or glaucoma). Certain conditions require prompt treatment to avoid irreversible damage, and therefore a thorough, timely evaluation is important. Fortunately, cataracts usually require surgery only when vision loss interferes with activities: when patients can no longer perform necessary or enjoyable tasks. Occasionally, surgery may be “necessary” because vision loss makes certain activities dangerous, such as driving a car.
WHICH SURGEON? Important factors to consider are the surgeon’s training, experience and continuing education. Is she or he board certified and fellowship trained? Also, it is important to have available the latest advances in surgical techniques and options. The surgeon should be the one to examine and make the determination of which procedure is best, and then to have a thoughtful and thorough conversation with the patient explaining indications, options, and “informed consent.”
OPTONS FOR SURGERY: Once surgery has been elected, the next decision is the type of lens implant. Aspheric lenses
may give sharper vision, but they are not suitable for everyone. Advanced Technology intraocular lenses, which can further help reduce the need for glasses, include Toric lenses, which correct astigmatism, and multifocal and accommodating lenses, which may enable patients to see far away as well as near. Each has unique advantages and disadvantages. A surgeon experienced in each can help patients decide which is most appropriate. Finally, laser assisted cataract surgery is now available, and offers a greater level of precision and further options to correct astigmatism. Surgical technique is critical, and experience in refractive surgery (including laser vision correction) is beneficial. Only after careful consultation (including a thorough examination and a thoughtful discussion) with an experienced surgeon should surgery be elected.
WHERE TO HAVE SURGERY Cataract surgery is now most often performed in ambulatory surgery centers. Ideally, these are specialized, sterile facilities licensed by the state and accredited by recognized organizations such as The Joint Commission or the Accreditation Association for Ambulatory Health Care (AAAHC). Having anesthesiology available, whether or not sedation is needed, provides a safer environment. Anesthesia is typically topical (no needles), with IV sedation available to make the experience pleasant and comfortable Under the right circumstances, the results of modern cataract surgery are brilliant.

These early pioneering individuals travelled to Wilmer Institute with Dr. Minkovitz, where he maintained his teaching and clinical appointment. While on staff of Wilmer at Johns Hopkins University, Dr. Minkovitz received the Outstanding Teacher award. He was also named Top Doc in ophthalmology by Delaware Today Magazine six consecutive years, and has been an invited instructor at American Academy of Ophthalmology Annual Meetings.

Jeffrey Minkovitz, MD is an ophthalmologist with Eye Physicians and Surgeons in Wilmington, Delaware. Dr. Minkovitz specializes in cataract and refractive surgery, and is one of only a few Delaware surgeons specially trained in corneal transplantation. He completed a cornea and refractive surgery fellowship at the renowned Wilmer Eye Institute of Johns Hopkins. Previously, he graduated magna cum-laude from Harvard, earned his medical degree from the University of Massachusetts, completed internship at Harvard’s Mt. The year 2015 is a big year for me, because it is my fifteenth year as a publisher with the Women’s Journal. I never dreamed that when I started with the Women’s Journal that it would take me in the direction that it has. When I first met the owner’s of our National Licensing they told me that I was about to begin a life changing experience, and I can now say that I totally agree. They have been 15 of the best years of my life, and even though it has been very hard work at times it has also been very fulfilling, and has given me the ability to do things that I never dreamed I would be able to do. So to all my contributors that have been with me for a majority of that time I hope that the relationship has been a great one for you as well. This issue we also want to bring recognition to the fact that February is Heart awareness month, and with that comes the reminder that women need to take heed of the article that Dr. Kevin Copeland has provided. Coming from a family with a large history of heart disease I am always aware of the importance of taking caution in how I treat my heart and myself. I lost my mother when I was just 20 years old, and have since lost 2 of my siblings to heart failure. So take care of your heart and learn the many factors that can cause a heart attack or stroke. This is also our issue to bring your mind to love and marriage, and all the wonderful vendors that we have within our publication that can help you have the wedding of your dreams. Thanks to all our wonderful contributors for your support of the County Women’s Journal, I hope that the holidays were a wonderful time for everyone and that 2015 is going to be the best year yet. On The Cover... I would like to extend a special thank you to the Delaware Art Museum for graciously allowing us to have our photo shoot for this issue at their wonderful property. We could not have asked for a more beautiful location, and the staff at the Delaware Art Museum was wonderful to work with. If you are considering different properties for your wedding or reception I would highly recommend this one, especially for an outdoor wedding. The sculpture garden would be a wonderful location for photos. Unfortunately, the weather was quite wet for our photo shoot thus, the shoot had to be done completely inside. As always we were blessed with beautiful models, and for this issue not one, but three lovely Brides. Holly Ann Mays honored us as the model for our gown provided by Bridal Chic Boutique. Holly is represented by MMA/Model Management Agency. Our second model is the lovely Dr. Arianne Missimer, owner of Core Fitness Studio in Wilmington. Arianne is modeling a vintage look gown provided by Lady’s Image. Our third beautiful bride is Corey Grabowski, of the Jewelry Exchange of Delaware. Corey is modeling a gown that was custom made for her by Terry Richards. As you can see we have given you three totally different options to consider as a wedding dress choice, yet all are very beautiful in their very own unique way. And last but not least our charming young flower girl wearing a vintage looking ballerina length gown provided by Hansel and Gretel Children’s Boutique on Rt. 202 North. Slighted By The Tooth Fairy A
bout 20% of people don’t get the standard issue of 32 permanent teeth!!! The third most common teeth to be congenitally missing are lateral incisors. Lateral incisors are the teeth to the right and left of the two center teeth. These are by far the most challenging to replace and the focus of this article. The lateral incisors normally erupt at about six years old. Since going through childhood without these teeth has an impact on selfesteem and speech development, we usually provide temporary tooth replacement by placing a plastic tooth on the orthodontic archwire or on a removable retainer. Unfortunately decisions for definite tooth replacement, after growth is complete, must be planned at an early age, usually 8 – 10 years. The dentist, orthodontist and the family need to consider the risk/benefits of the long – range solutions using statistical data, genetics, and clinical judgment. No one can 100% predict the growth pattern of any individual patient. Most dentists agree that the most ideal treatment keeps the natural teeth in the position for which they were designed and replaces the missing lateral incisor(s). While it is possible to close the space left by the missing lateral incisors by orthodontically moving all the permanent teeth forward, it is a compromise. The canine, eye tooth, is moved into the position
next to the central tooth where the lateral incisor should be. The problem is that the shape of the canine tooth and root of the canine is nothing like that of the delicate lateral incisor. The canine can be reshaped and restored to look fairly good but the bulky root remains. More importantly we have lost the function for which the beefy canine was designed. The canine root is important for mouth shape and support of the corner of the mouth. The canine in its proper location guides the jaw in side to side movement, protecting back teeth from unfavorable forces which can contribute a host of pathologic events. The premolar tooth which has been orthodontically moved into the canine position has one or two delicate spindly roots. These roots are not nearly as resistant to functional demands as the canine.
development is complete. The most ideal tooth replacement today is the dental implant. Implants are little more than artificial roots placed into the bone upon which a tooth is built. The implant looks and feels very much like a natural tooth. It preserves the bone and permits all other teeth to function as they were designed. The problem is that maintenance of the orthodontic result is imperative as previously discussed. Even
when maintenance is perfect, there may be unpredicted growth patterns or insufficient bone to place an implant without additional preparation of the site. Obviously there are many considerations when treatment planning for a young person who is missing a lateral incisor. Finances, temperament/cooperation of the patient, genetics all play a role. The tooth Fairy’s slight creates quite a challenge.
Closing the space by orthodontic repositioning is not without its benefits. Permanent tooth replacement is available at an early age, when orthodontic treatment is complete. This eliminates the obligation to maintain the space and precise root position of teeth next to the space for the years between orthodontics and permanent tooth replacement, when growth is complete. The repositioned canine tooth can be modified slightly immediately following orthodontics but definitive restoration of the tooth is best completed when mouth growth and

onathan of Elkton, MD has been a regular platelet donor for several years. It is something he has always believed is the right thing to do. In October 2011, the need for donors like himself truly hit home. Jonathan’s four-year-old son Gage was diagnosed with portal hypertension, a serious and potentially fatal condition caused by impaired blood flow to his liver. He ended up needing regular platelet transfusions prior to diagnostic and reparative procedures since his platelet counts were dangerously low and left him at risk for uncontrolled bleeding.

Platelets are needed for local patients battling various conditions including cancer or who are undergoing surgery. The automated process for giving platelets takes a couple of hours. The machine separates the blood into platelets and red blood cells, keeps the platelets and returns the red blood cells to the donor. Platelets only have a shelf life of five days, two of which are spent in testing, so the demand for the product is always high. Giving whole blood takes about an hour from start to finish including the review of materials, interview process, blood collection and time for cookies and juice in the canteen. One pint of blood can be used to help up to three people having surgery, battling serious illness or being treated for trauma.
“It was shocking that my own son would be a patient in need of platelets. You never think that your own child will need any type of transfusion,” says Jonathan. “It really made me think about the other children and adults who are able to live because of dedicated donors who give of themselves every day. You just never know when it is going to affect your life so deeply.”
In addition to whole blood, donors who meet the requirements can be twice as nice and save twice the lives by being a double red cell donor. The automated process takes a bit longer and donors give two pints of red cells instead of one. During the automated process, the double red cell donor receives their platelets and a saline solution so there is no loss of fluid volume. To be a double red cell donor, men must be at least 5’3” and 130 pounds and women at least 5’5” and 150 pounds.

3T MRI Is The Best For Imaging The Shoulder you lose the use of your shoulder. This is why imaging with 3T MRI earlier is ALWAYS better.

octors continue to learn after they graduate. Dr. Chao worked with Mike Zlatkin who developed MRI imaging of the shoulder. A good radiologist makes a huge difference. The added detail we see using 3T MRI matters for imaging of the labrum. We had a 60 year old patient with shoulder pain and a negative x-ray. The routine x-ray is a very poor test for imaging the shoulder. It is always done but it misses almost EVERYTHING that might cause shoulder pain. The doctor wanted to inject steroid without MRI imaging. This is a mistake. If you use steroids without knowing if the patient has a tear – you may make matters worse. An MRI of the shoulder in all patients older than 50 is necessary because of the high incidence of rotator cuff tears and disease. Tears smaller than 1 cm can be repaired with an arthroscopic acromioplasty (day surgery). If you wait till the tear is larger it will often require extensive surgery. If the tear is retracted most surgeons will not operate and
radiation. CT scans use a lot of radiation. This x-ray radiation causes cancer. We strongly recommend you ASK your MRI facility how strong their MRI scanner is. Radiologist experience and training matters as well. Dr. Chao is both a neuroradiologist, musculoskeletal and body MRI doctor (double fellowship). Find out before deciding where to take the prescription your doctor gave you to get an MRI. (Remember, the doctor writes the prescription, the patient decides where to fill it.). Contrast MRIs are particularly good for finding tumors, infection and inflammation. It does this without radiation and the contrast used in MR (which is neither iodine, nor shellfish-based) is much safer. And, even within the group of five or six MR contrasts, we use only the safest—Multihance, Prohance and Eovist. www.safecontrast.com
3T MRI scanners had more detail and are used for most sports teams. It costs the same as the slower inferior 1.5T MRI which most doctors think is adequate. At 1.5T the scanner cannot see a labral tear without an expensive and painful procedure called an MR arthrogram. At 3T we can see the tears without any contrast. So we save up to 1000 dollars with our extra detail. If there is any question we can use IV contrast and see the labral tear of your hip or shoulder joint or knee without an invasive procedure. This is called an indirect arthrogram. Our OPEN MRI scanner is the widest and most spacious, and least claustrophobic MRI in the world. We also offer IV sedation ($50) in case you can’t tolerate the study. BLADE software actually compensates for movement (of your shoulder or hip) and allows us to get clearer pictures. Most doctors don’t know this can make the difference in seeing a tear. Some patients who come to our OPEN MRI have been to 4 other scanners before finding out that ours is the only one that they can fit into.

Sharing The Precious Gift Of Mother’s Milk absolutely devastating. Breast milk is best right? Not for George. It took me weeks to wean from pumping because my supply was so strong, and I had no idea what I would do with the almost eight gallons stored in our freezer.

s an obstetrician, I encourage moms to breastfeed their babies because mother’s milk is the best nutrition, but I also know that some moms, even with good intentions, may not be able to nurse their babies. Medications, certain illnesses, challenges that come with twins, and other obstacles may limit breast-feeding success. Mothers who are adopting a baby or having a child via surrogate also need access to breast milk. As a mother who has been lucky enough to nurse two babies, I understand how meaningful it is for moms to provide the perfect food to help their babies grow healthy and strong.
It look me a while to come to the decision to part with my breast milk. Initially my pediatrician suggested storing it and retrying when George was older and potentially could tolerate the milk. I had put so much effort into pumping and storing more than a 1,000 ounces of milk. To pour it down the drain would have broken my heart. Approximately six weeks after George’s initiation into soy based formula use, he was a happy and thriving baby. My husband and I decided we would not give him the breast milk again. Now, what were we going to do with all of the breast milk in our freezer?

Guidelines for donating
Mothers who want to donate milk must be willing to commit to strict guidelines in order to ensure the milk is safe. Here’s a checklist: • The donor mom must be in good health. • She must have at least 100 ounces of milk to donate. • She must take a blood test to rule out diseases such as HIV, hepatitis B and syphilis. (The milk bank pays for the test).
At this time, a patient of our practice had delivered her baby prematurely and for a variety of reason could not produce milk. One Saturday morning, she revealed they had signed up for the Milk Bank. A wave of relief flooded over me … why did I not consider donation before?
• Pass a drug-screening test. • Submit a form from your pediatrician reporting that your baby is in good health. Breast milk is absolutely the best food for babies. The American Academy of Pediatricians recommends breast milk exclusively, if possible, until six months of age. Mothers who find themselves in a similar situation to mine or are blessed with a bountiful supply can help other families in the best way, by donating.

istening is one of the most important skills we have. Webster’s dictionary defines listening as ‘paying attention to sound; to hear something with thoughtful attention; to be alert to catch an expected sound’. So based on this definition, we know that listening requires conscious attention to speech and sounds. Whether it’s face-to-face in quiet, seated in a noisy restaurant or holding a telephone to the ear, being able to listen requires that we accurately receive and interpret spoken messages. It is not surprising then that great listening skills depend on being able to hear and differentiate sounds in speech and in our environment.
Listening is Not the Same as Hearing Our ears detect sound and alert us to things and people. Hearing is one of the five senses, the one that gives us the ability to perceive sound waves, some barely audible, others so loud they cause discomfort. But listening is so much more. Listening is how the brain analyzes and interprets the sounds and speech in our environment. Because hearing and listening are so complex, we must rely on verbal (speech and tone of voice) and non-verbal (gestures and facial expressions) information in order to interpret messages in more challenging listening environments. How well we listen has a major impact on the quality of our personal and professional relationships with others. When hearing diminishes, whether suddenly or gradually, listening becomes more challenging. It is a struggle to understand conversation with family, friends and colleagues. We frequently ask people to repeat themselves and background noise makes speech very difficult to hear. As hearing loss progresses, many report that people seem to mumble, and speech is unclear; background noise is especially annoying; TV volume is turned up too loud; high-pitched sounds, such as birds chirping, bells and whistles are no longer audible.

TMD (Temporomandibular Joint Dysfunction) – The Great Impostor A Case Study of TMD Disguised as Various Medical Conditions

aw pain is the most common symptom people associate with TMD. However, it is not the only one. Due to the joint’s close proximity to the brain and pathological mechanism, it induces a wide range of symptoms throughout the entire body. When these symptoms surface at locations other than the jaw joints, they tend to mimic other medical conditions that may share some common pathways. Because of this sharing of common pathways, the true etiology is often masked. These symptoms may be reduced to a tolerable state, but they will always return without treating the true cause of the problem. To appreciate the uniqueness of TMD, we need to have a basic understand of the anatomy. Our human skull made of several bone plates stitched together by sutures. This flexible design allows our head to absorb blunt force without damaging the brain. TMJ sits under the Temporal bone. Within and around this particular bone, there are several major nerves and muscles passing through. These nerves control a wide range of functions such as facial expression, hearing, swallowing processes and other involuntary muscle movements. As we open and close our mouth, TMJ normally just glides over temporal bone with the help of several major muscles. In a pathological situation, these muscles are often in spasm. The direct consequence is bracing of TMJ against the temporal bone. Compression of temporal bone would in turn compress the nerves nearby. A group of symptoms is the direct consequence of such compression. Below is a partial list of symptoms: Headaches Ear Congestion Vertigo (Dizziness)
Tinnitus (Ringing in Ears) Dysphasia (Difficulty Swallowing) Facial Pain Paresthesia of fingertips Thermal Sensitivity (Hot & Cold) Trigeminal Neuralgia Bells Palsy Nervousness/Insomnia Cervical Pain Individually, each of these symptoms listed in the above table does not show a clear connection to TMD. Patients present these symptoms to their medical doctors. Based on their medical training, the doctors see and treat these symptoms differently. The result is partial improvement at best since the true cause of the problem has not yet been addressed. As time goes by, the side effects produced by chronic use of medication may further mask the true origin of the problem or create other health issues. Although non- life threatening, quality of life would suffer from this chronic condition. CASE REPORT Heather is a young lady who came to our office for consultation regarding her chronic headaches. She had many headache episodes each week. The pain mostly comes from the left side of her face and around the eyes. In addition to chronic headaches, she also has hypothyroid condition. Her medical doctor diagnosed her headache as vestibular migraine. Medication given has proven successful in reducing the numbers of episodes. Clinically, Heather has sore chewing muscles. Her TMJ clicks and is sometimes painful. She admits to clenching. There is no other significant issue with her dentition and supporting tissue. Based on the size her upper front teeth, we estimated her teeth are over closed by 5 mm. This is a significant number in dental terms. Analysis of her major chewing muscles by scanning muscle fiber firing frequency shows a significant spasm. Notice the first four lines of the graph below. The left side of each line has three letters designated and a number. That number represents what normal muscle should read. One the right side of each line, the number listed is the actual reading.

Impression: By definition, vestibular migraine is a kind of migraine that is preceded by vertigo (dizziness). Other symptoms may include: nausea, fainting, abnormal eye movement, hearing loss, weakness in arms and legs. The cause of the vestibular migraine is unclear. However, it is generally believed to be a neurological abnormality in the back of the brain. There are no drugs to treat vestibular migraine specifically. Patients are given drugs to treat vertigo and migraine separately. The most common age group is late teens /early 20’s. Heather certainly fits the profile of a vestibular migraine patient; however, the clinical picture changes once we add the TMJ clicking, over closing, clenching, and muscle spasm. The list of vestibular migraine symptoms is very similar to TMD symptoms. Over closing by 5mm could certainly compress the nerves to produce the symptoms. The question becomes which is the real etiology? Is it a neurological abnormality or TMD?
Treatment: We treated Heather based on the assumption of TMD. Using neuromuscular techniques, we constructed an oral appliance for her. The goal of this appliance is to decompress TMJ and relax the chewing muscles. Three months after she began to wear the orthotic, we took another look at her muscle spasms. Compare the first four lines to the pre-op graphs. We see a dramatic improvement even with in three months. Although there are other muscles with spasms that have not been resolved, this early result is still very remarkable. Subjectively, Heather is reporting fewer episodes of headaches and her sense of general well being is improving. This case is far from over. There are still issues that we may or may not be able to solve by TMD treatment alone. Ultimately, we suspect it would take the combined efforts of both the medical and dental teams to resolve Heather’s issues. This case does demonstrate how a fresh

Macular Pucker T
he following article is the first part of a three part series about macular diseases: Macular Pucker, Macular Hole and Macular Degeneration. These are 3 separate clinical diseases that all affect the macula but have different causes and different treatments. Patients commonly confuse the three diseases because they all affect the macula. This article will be about Macular Pucker.
Macular Pucker is a condition of the retina that can cause distortion and blurred central vision. Other names for this idiopathic disease include epiretinal membrane or pre-retinal membrane. The macula is the center part of the retina that allows one to read, drive, and recognize people’s faces. It controls the central vision. Even though the name of this disease is “macular pucker”, it not the same thing as “macular degeneration.” They are two completely separate diseases with different etiologies. Macular pucker is not as common as macular degeneration but it is not uncommon. It is more prevalent in patients as they age, but can also be found in younger patients. Many times the disease is idiopathic (don’t know why one gets it) but sometimes it can be related to trauma, eye inflammation, and sometimes diabetes. Patients may have a macular pucker and not even be aware, if it is mild. The disease is not always progressive so if there are no symptoms then no treatment is recommended. It is caused by very fine scar tissue (membrane) that lines the inner (top) layer of the macula. Usually the macular anatomy is nice and smooth with a gentle dip in the middle called the foveal dip. This is where the light focuses on the retina when a patient looks directly at something, giving them very fine precise vision. The anatomy is altered by the fine scar tissue growing on the retina, causing the area to wrinkle. Instead of a nice smooth surface with a gentle dip, the surface can be corrugated and elevated by the scar. As a result the patient’s fine vision is altered. They may complain of distortion, which means that straight lines look crooked or wavy. There also be a blur in the center of the vision. The peripheral (or side vision) is usually no affected. This anatomic change can occur in one or both eyes. If it is only in one eye, the best way to determine if a patient is symptomatic is to check vision one eye at a time. If both eyes are open, sometimes one is not aware of distortion, central blurring or decreased vision because the good eye takes over. Treatment for macular pucker involves outpatient surgery in the operating room. A patient would need a pars plana vitrectomy procedure that requires 3 small sutureless incisions in the sclera (the white part of the eye), which gently removes the vitreous jelly in the eye as well as the fine scar tissue that lines the inner layer of the retina. Sometimes the membrane is so fine we need to stain the tissue in

surgery with a special dye to see it as we operate. Preoperatively, a special test called OCT (Optical Coherent Tomography) can measure the extent of the membrane. The OCT is used to gauge the extent of the disease and to look at the detailed anatomy before and after surgery. Most times a patient sees better after the surgery. The visual acuity improves and the anatomy changes to a smoother surface allowing the patient to read much easier. If a patient is bothered with his or her vision, especially if they are having problems reading or driving then surgical intervention is recommended. If a patient is not bothered by the distortion and is functioning fine with the vision that they have, then the macular pucker should just be monitored. The patient can do this monitoring at home with an Amsler grid, which will help objectively map out their distortion. If there are any significant changes on the Amsler grid, the patient is counseled to call their doctor for an examination. Macular puckers do not tend to progress quickly so this is not an emergent situation if more distortion is noted. In summary, Macular Pucker can be successfully repaired surgically in symptomatic patients. This surgery is done on an outpatient basis and vision usually improves slowly over a 6 week to 6-month period. This disease is not related to macular degeneration and does not increase the risk of this disease. Dr. Glazer-Hockstein graduated Cum Laude from Jefferson Medical College. She was a member of the Hobart Armory Hare Honor Medical Society and was elected to the Alpha Omega Alpha Honor Society. She also received the Carol R. Mullen prize in ophthalmology. After residency, Dr. Glazer-Hockstein completed a two years medical retina fellowship at the Scheie Eye Institute, University of Pennsylvania. show that individuals who follow a genetically appropriate diet lose weight more easily. Most insurance companies pay for a portion of the DNA testing. However, the physician must submit necessary paperwork which includes the patient’s vital signs and a valid reason (i.e. weight gain, fatigue) for ordering the test. It takes 2-3 weeks for the lab to process the DNA sample. The patient is then provided a personalized genetic report and an individualized diet plan according to their DNA.

Can Lymphedema Cause Cellulitis? Article by Joanna Mikhail, DPT, ATC, CLT Doctor of Physical Therapy Certified Lymphedema Therapist
W
e’ve all had accidental falls, bumps and impacts that resulted in swelling and tenderness. This is our body’s protective reaction at the site of an injury. However, when that swelling does not resolve in a few days, it’s time to suspect something more serious – cellulitis. Cellulitis is a bacterial infection in the deeper layers of the skin that can spread throughout the skin very rapidly. The skin begins to look red and swollen and possibly have a rash over the reddened area. When the skin is felt it is warm and tender. A fever is a later sign of an infection. When you suspect a cellulitis infection in your body, you need to go see your primary physician Image of cellulitis preferably that day. courtesy of LCOA If you cannot get an appointment that day then it may be best to go to the emergency room. After being diagnosed with cellulitis, you will need to be treated with antibiotics to help your body fight the infection. One common cause of cellulitis is lymphedema. This condition requires special treatment beyond antibiotics. The lymphatic system is part of our blood circulatory system to remove lymphatic fluid from the body. L ymphatic fluid consists of white blood cells, protein rich fluid, waste products, and any bacteria, virus, or fungus that may have entered our bodies. The lymphatic vessels collect the fluid and take it to the lymph nodes in our body. The purpose of the lymph nodes is to clean the blood and fight the infections that have ensued, providing essential immune system functions. When the blood leaves the lymph nodes, it returns back to the heart to recirculate. When part of this transport system fails, lymphatic fluid accumulates in the affected area. This condition is referred to as lymphedema. www.TheWomensJournal.com
Lymphatic fluid is rich in protein. When these proteins pool in any part of the body, they begin to layer in the skin. The skin can darken, harden, and eventually crack, allowing the fluid to seep out of the underlying tissues and allowing bacteria to enter our skin. Protein is an excellent medium for bacterial and fungal growth, so infections are frequently problematic for a patient with lymphedema. When bacteria enters the skin it is taken up by the lymphatic vessels and transported to the lymph nodes where infections are fought. If the lymphatic transport system has failed then the lymphatic fluid with the bacteria in it is not transported out of the skin and tissues to the lymph nodes. The bacteria then multiply in the skin and can cause a cellulitis infection. If you or your doctor suspects that the chronic swelling of your limbs is the result of lymphedema, you will need to be seen at a lymphedema clinic for treatment. However seeing a doctor first is important because the lymphedema therapist cannot begin treatment until the patient has been taking antibiotics for several days. Proper compression is key to removing fluid and bacteria from the affected area to a lymph node, where, with the correct antibiotics, it can be eliminated from the body. Antibiotics alone are not effective in patients with chronic cellulitis resulting from lymphedema. The infection tends to return time and time again because the swelling continues to stretch and open the skin and allow more bacteria to enter this medium (lymph) that is rich in protein and an excellent breading ground for infection. We encourage anyone who has chronic swelling and infections to schedule a consultation with one of our therapists and to begin a program that will help reduce the risk of another cellulitis infection. Make A Commitment: Plan To Reach Fitness, Health Goals In 2015! realistic, and maybe even conquer and divide. For example, if your goal is to run a 5K with your friends, but you have never run a mile, know what you will do and when. Start with mini milestones, and give yourself deadlines, such as: I will walk a 5K by March 15. I will be able to run a mile by March 30, and so on. Don’t forget to reward your successes, but not with unhealthy food. You need to set aside time to work on your goal. ho hasn’t said “I need to lose 20 pounds before...” or “I need to get in shape for...”? Usually by the time we speak these words out loud, we’re looking at an impossible dream, a wish that sets us up for frustration. We know that despite the claims of a product/ drink/ supplement that will help us drop significant body weight or regain a teen-age shape, that’s probably not going to happen. Here’s why: being in “good shape,” fit and having a normal, healthy ideal body weight takes continuing time, focus and commitment. Eyes on the prize, and for a lifetime. Depending on the person, some people have difficulty staying away from sweets, alcohol or processed food while others have never had a good experience working out. So, where do you start? You can look at your challenge to get in shape, lose weight, train for a 5K just like any other part of your life. Identify your goals. Make them

You can’t get in shape while you’re shopping or watching TV or driving. You need to schedule workout time that works for you and write it down, not just once, but at least three times a week. If something comes up, exercise is important. Reschedule yourself.

Then know when you need professional expertise and assistance. If you feel as though you’ve tried and failed, it’s probably worth the financial investment to hire a personal trainer who has experience in helping guide others. Get it in your budget by swapping out entertainment or non-essential clothing purchases. An hour’s appointment a couple of times a week can help you stay the course, build physical strength, avoid injuries and grow your confidence. While working on specific exercises, you will also have time to discuss with your trainer concerns about nutrition, fitness apparel, a training schedule and other things you need to know to reach your goal. Here’s another key point: once you’ve achieved the goal of weight loss or increased fitness, you can’t quit. It’s time to raise the bar. Do more. That doesn’t mean you need to lose more weight or run farther or faster, unless you want to. You may have learned that running is not for you, for example. You may want to try swimming or cycling or tennis or Pilates. 25

Soft blush, pink, gray, and cream colors are combined to create this bouquet. Flowers used include cream peonies, pink lisianthus, gray brunia berries, blush roses, pink hydrangeas. The bouquet stems can be wrapped in dark gray ribbon, to coordinate with the grooms suit. Many bouquets can also do double duty as tabletop decorations. The peonies are normally available in spring only, but their season is growing along with their popularity. Silk flowers were used to create the cascade bouquet, light and dark pink flowers, pop against a cream dress. If the flowers are quality made, it can be very difficult to know if the flowers are fresh or artificial. With the amount of destination weddings, and brides wanting to take their bouquets with them, or if the weather is going to be hot and the wedding is an outside event, silk flowers are a really good choice. Wedding Day Themes

edding themes and styles seem to occur in cycles. What once may have been considered a “country” theme is now called rustic. What was once thought of as an “old fashioned” wedding is now called vintage. And no matter what style a bride chooses, there are multitudes of flowers of every description - color, shape and size - that can complete any look and carry out any theme. Since Pinterest became a “household word” it is a valuable resource for brides as a means to factor in the many components of the wedding planning process. In the floral category alone, a bride
may have already perused the endless possibilities on Pinterest and narrowed her choices down to the top three which most successfully fulfill her vision. When she ultimately sits down with her florist some of the basic decisions have been made and need only to be fine-tuned.
bouquets are pieces of jewelry such as a special locket or a favorite rosary that can be interlaced among the flower stems. Each of these choices helps to fulfill the well known wedding day saying - Something old, something new, something borrowed, something blue.
Flower choices that have made a huge comeback in recent years are football mums, carnations and babies breath. All of these make good financial sense in that they are available all year around. As for carnations, they come in an extensive array of colors, hold up in all temperatures and are affordably priced. Football mums are a great choice for their size as they can take up as much space in a bouquet or arrangement as four roses. Babies breath is a yearround crop, imported from South America, that is hardy and versatile and again, popular!
When a bride is confronted with the enormous selection of containers and vessels for centerpieces, serving table arrangements and accessory pieces, a decision may seem daunting. Again, Pinterest categorizes the many possibilities making it easier for a bride to visualize the finished product before her wedding day. There is no longer such a thing as a standard choice. Vases, vessels and containers that fit a rustic or vintage themed wedding reception are often wooden or galvanized. One of the most popular “comeback” items is the Mason Jar which can be found in a variety of colors, as well as clear, to compliment any floral composition. Milk glass, carnival glass and crockery items found in thrift shops and at yard sales by today’s savvy, thrifty brides ensure that the atmosphere at the reception is oneof-a-kind.
On every girl’s wedding day there is really just a brief moment when all eyes are on the bride; that is when she is walking down the aisle and for that moment her bouquet is one of the accessories that are in the spotlight. What she carries needs to have some amount of visual impact, yet not detract from the bride herself, and it is likely that it will include some quality of sentimentality. Brides often accomplish this by having the flower stems wrapped in a piece of fabric that was once part of her mother’s or her grandmother’s wedding attire and might be a sleeve or a section of the hem or part of the veil. A lace or linen handkerchief is another sentimental item from a grandfather or god-parent that can be used to wrap the flower stems. In an oldfashioned [pardon, I mean, vintage] wedding a degree of nostalgia is always part of the theme. Other items that brides have chosen to include with their

When a bride has chosen to adorn the pew aisles with pomander balls, these can easily be brought to the reception and placed on glass cubes to become centerpieces. If the deal seems to be “too good to be true”, well chances are your instinct is correct. I’ve been conned before, and it really stinks. It’s the same thing when buying a diamond and finding out what you bought was not what it was represented to be. It’s wrong and its corrupt and it infuriates me. We buy diamonds frequently and many times people come in with certificates of authenticity or appraisals of the diamond and the paper says one thing, but the diamond grade is something completely different. When buying a diamond from a jeweler the one question that people always ask is, “Why does it cost one price here and another price somewhere else?” I mean if we are comparing apples to apples why not the same price? The problem is that one place has an apple and another place has a rotten tomato with a certificate that says it’s an apple. Recently there was an article written about a jewelry store that was selling diamonds with EGL International certificates. It said that the diamond was G Color and VS2 Clarity. The customer paid a price for a G color VS2 clarity that was cheaper than the other retail store selling the same stone. When this customer
took the diamond to have the ring appraised by the G.I.A they found out the diamond was in fact a J color SI2 clarity. That discovery would amount to a HUGE difference in price and quality. Diamonds are priced based on the 4 C’s (see our grading chart SKjewelers.com). The problem is that some retail stores are getting bad certificates so they can say to you, “Hey look it has a certificate” to lure you in to believe, it must be what the certificate says!” So now the question is, how do you protect yourself from making a bad investment and actually know what you are buying is a true quality diamond? Many times stores’ will have certificates that go along with the diamond from an independent Certified Gemologist vs. other stores that offer their own certificates, which is a conflict of interest or in layman’s terms, a huge no-no! You might see legitimate looking certifications like EGL or IGI or AGL, however the only certification company that is consistently recognized around the world is the GIA. No I don’t work for GIA and no the GIA is not paying me to write this article. If you do any research or understand the diamond trade business at all, GIA will be the one certifying body that most, if not all experts agree is the most qualified to grade stones.

received a question recently from a woman asking me to help her understand what she was doing wrong with her bra? In her fitting, I had explained the concept of the smaller band/ larger cup bra versus the larger band/smaller cup sizing. These two bra sizes are considered “sisters”. A 34DDD and a 36DD bra would both fit the customer correctly depending on her comfort level. However, if the bra is being worn in the middle or tightest hook, this would be a clue that the bra band is too

Many women wear their bras too large in the band causing the bra to ride up in the back. If your band is too big it will make your straps fall and create “back fat” as it is pulled up the back. A proper fitting bra should be even all around the torso from front to back. It should feel like a “hug”. When I founded my shop and began working with bra companies and designers who taught me the art of bra fitting, I was told that 70-85% of women wear the wrong size bra. I saw firsthand that this statistic is correct. You know why? Anyone can find some fault with the way a bra fits if they try hard enough. Each customer has a different view of the tightness of a bra or strap. However, there are certain aspects of the fit that are crucially important. Only you know your comfort level, however, you may not get the look or the proper fit you want if the bra band is too
large. As mentioned, certain key aspects of a good fitting bra are taught to my fitting team and I believe are important to a woman’s shape and appearance. FORGET ABOUT THE NUMBERS AND THE LETTERS. Just see how good you look in the mirror.

s Permanent Cosmetics is a fast growing industry, most women have at least heard of the glorious notion of “never having to apply makeup again.” Many women have considered the possibilities of permanent makeup, for themselves or for someone else, knowing that such procedures could make a tremendous difference, even a life change. However, in our busy worlds not all have found the time to explore even the most basic aspects of the procedures, putting it “on the list” as we say, hoping to eventually find the time to learn more. We are here to clarify. So, we are fully aware that we can permanently enhance our natural beauty with permanent eyeliner, brows, and or lips. Permanent cosmetic pigment is made of all natural minerals, and it does fade over time, especially with prolonged sun exposure. Certain products and medications, such as glycolic or Retin-A can also cause premature fading of the tattoos. It is best to avoid these products around your makeup area, and ALWAYS wear SPF (even in the winter!) to protect your skin and your investment in yourself. Color boosting your make up every 1 to 7 years is the best way to keep your fresh, youthful color. Every person’s skin is unique, as will be your makeup. The good news is you don’t have to keep up with the maintenance if you feel you no longer care for the enhanced look. It will not turn blue or black. It will simply fade over time, eventually leaving no trace. Honestly though, it will be hard to return to the drudgery of the daily makeup grind. One gets a bit spoiled you know, and it’s hard to go back to the old way. Yes, you can wake-up with makeup, you can exercise, swim, sweat and play and you’ll have no smears or smudges.

(It is important that you do not pick or peel at the dried ink, it has to absorb into the dermal layer!!) It is advisable not to make any major social plans during this time. It will take 4 weeks for the skin to heal and the pigment to fully resurface. Often after the first round there will be spots or areas where the pigment completely absorbed and did not resurface. How Parents Can Help Children Prepare For Assessment Tests T

Two consortiums are currently rolling out the next generation of assessments to measure students’ progress toward college and career readiness (the Partnership for Assessment of Readiness for College and Career (PARCC) and the Smarter Balanced Assessment Consortium (SBAC)). That means that the standardized tests that your child has taken in the past will look very different. In addition, school curriculums and expectations of students are significantly different than under the previous state standards. How can you help your child do his or her best on assessment tests? Here are several trusted tips to share with your child to insure the best chance for success on these or any important exams: • Read all questions carefully and follow the directions. On any test, it is critical that you identify each question being asked before rushing into solving or answering the problem. • Narrow down choices. If you are unsure of the answer to a question, eliminate choices that do not make sense or are obviously incorrect (for example, answers that include words such as always and never are red flags). • When working on math problems, eliminate extra information provided that has nothing to do with the question being asked. • If you get stuck on problems, mark them and move on. You can return to them later if you have time at the end of the test, but don’t waste valuable test time spinning your wheels. • Pace yourself. At the outset of a test, do a quick calculation of how many minutes you have for each question—and give a little extra time for essay questions. Try to leave a few minutes
at the end of every test to revisit skipped or troublesome problems. • Draw out problems to help yourself visualize them better. • Minimize test anxiety by keeping things in perspective. Remind yourself that tests are simply how teachers can evaluate what you know. The best form of test preparation is to keep up with homework and in-class work. What can you do to prepare your student for Common Core-aligned assessment tests? Here are a few suggestions: • Talk with your child’s teacher about how you can ease your child’s transition to school work and tests under the Common Core State Standards. • Continue to emphasize the importance of thinking critically—an area of elevated importance under Common Core. Ask questions about what your child reads and learns. Encourage your child to talk about how he or she feels about stories, current events and more. Have your child explain his or her thinking — in both math and reading. • Embrace technology in your household. The new standards focus heavily on the skills needed by today’s 21st-century digital learners and incorporate research and media skills into all content areas. As always, one of the best ways to help your child do his or her best on any exam is to equip them with good study skills and habits. It is also important that you stay apprised of the curriculum changes that are happening in your child’s classroom and understand how those may impact the tests that your child takes.

This principle prompted the instructors in the University of Delaware’s Social Media Marketing Strategy Certificate to include a real life consulting project for an area nonprofit into the program. The first time the certificate was offered, participants teamed up with Big Brothers Big Sisters of Delaware to fine-tune the organization’s social media communication strategy. Since then, consulting projects have also been completed with Bellevue Community Center in Wilmington, and the dropout prevention program Communities In Schools of Delaware. With its focus on how to develop a social media marketing campaign and improve an organization’s social media presence, the program attracts a variety of professionals from the region. “For the students, the opportunity to work with a client is always a highlight,” said Meredith Chapman, UD’s director of digital communications, who serves as the program director and primary instructor. “Working hands-on with Big Brothers Big Sisters of Delaware was a really great experience,” commented Christine E. Serio, one of the first graduates of the UD certificate program and an external communications specialist for Blood Bank of Delmarva. “Not only did we get a chance to delve more deeply into such an

We received hours of professional consultation services that will really benefit our organization – what a valuable experience!” added Fox.

ew Year’s resolutions, sneakers, carrot and celery sticks, protein shakes, exercise equipment, and all things pointing to the move to improve that clutter our minds, refrigerators, cars and homes. Whether our primary concern is our health, appearance, or both, losing just 10-15 pounds and getting 30 minutes of exercise five days a week can prevent or delay diabetes in people with prediabetes. Twenty five percent of adults 20+ years of age or older have diagnosed or undiagnosed pre-diabetes. Diabetes is a serious and costly epidemic that poses a major public health problem. The American Diabetes Association believes that if we are to truly make an impact against this devastating disease we must engage our communities on every level, in every facet of our mission. One of ADA’s strategic partners locally is Blood Bank of Delmarva (BBD). BBD supplies life saving blood and blood products to the hospitals in the Delmarva region as well as some in adjacent areas. Being one of the first blood centers in the country to do so, BBD over the last ten years has offered its donors the opportunity to have their glucose levels tested when they donate blood. In that span of time, almost 100,000 donors have taken advantage of this option. More importantly, over 19,000 of those tested were advised to contact their primary care physician to share their results, and potentially begin treatment. ADA and BBD believe services such as this are making a significant difference in the lives of people in our community and aids in working toward a healthier and better informed population. “It’s Just A Sign, It Doesn’t Have To Be Professional.” ? y l l a e R , h O
rom the business card in your pocket By Carol Mack Warner to the digital billboard President you see on the highway while going to work, signs are so much a part of your everyday life you take them for granted. Outside to inside, lobby to office, in the home, everywhere, signs are there to announce, inform, direct, warn and guide. Signs are a visual, non-verbal way to communicate and not just by the words alone. The type of sign, location, choice of font or letter style, sizing, layout and design all play a part in getting the message and your best image across. It’s not just a sign – it’s tells others who you are and if designed correctly, will attract the type of client you want. I have a friend who decides where she will eat if she likes the sign outside. If it’s neglected, she wonders what else is being neglected. This applies to all the signage you have in your business. Does your print material reflect the type of business you do? A comic book store will likely suffer using signage that a funeral home would use and vice versa. A comic book letter style and super hero are as out of place as somber tones and lettering are for the comic book store. Where do you go and what do you look for when searching for a graphics professional? Often word of mouth is a good way to start but don’t stop there unless they make you feel welcome and comfortable to discuss your needs. A full service sign company can save time and money if they fit. They can design and produce the final product without having to search out a company to produce your sign. A good relationship should form and they can provide other types of signs that follow your look without having to repeat the introduction process. If you want a logo designed, determine the purchase price of the design package so you can own the artwork. This is standard in the industry and should be discussed prior to work being done. This will allow you to take and use your artwork freely. Remember it is your business and you are the driver, so don’t be intimidated if it doesn’t work for you. If you work with the designer, you should be asked a variety of questions including the type of client you want to attract. Yes, that is what a sign can do by the way it is designed. All you have to do is watch TV advertisements to see how that works. Other questions include a discussion of the following topics: Type of sign, where will it be located and mounted, what are you trying to say and conveying the message using the visual clues of font style, colors and images.
Type of Sign: The type of sign is determined by the purpose and what you want to say: Large format printing, posters, banners, A.D.A. interior signage, real estate site signs, traffic signs, business cards, engraving, raised lettering, cast bronze plaques, flags, specialty items, vehicle lettering, car wraps, window lettering, labels, decals, magnets, directories, trade show, floor graphics and custom items all have different properties and uses. It is amazing to think that many weren’t in existence even 20 years ago. New products are coming out all the time. Location: What distance away from the sign will viewer stand? How will it be mounted? Please check with your township for the appropriate permits and regulations for exterior signs. A.D.A.(American Disabilities Act) signage with raised lettering and Braille II translation also have specifications for production, installation and use. Layout - Image This is where your message will work or fail. The layout of your message is influenced by many considerations including purpose, fonts or letter style, graphics or icons, color and size. If your message is to announce your place of business, then it is important to be visible from
the street quickly – so cars passing by can see the sign easily within the few seconds they pass by. Remember, this applies to vehicle lettering as well. The back door is a “billboard” and gets the greatest length of viewing time from the people traveling or stopping behind the vehicle. Drivers pass by the sides quickly, their eyes on traffic ahead. This valuable “real estate” needs to be optimized for quick viewing. Window lettering, banners, interior placards, business cards and brochures are saved for more detailed information. Traffic, parking and warning signs are designed to impact immediately and are simple and direct. Choice of font or letter style, graphics and materials express more information than the written message. The prospective client gets a feeling about the business from the sign: “It looks like it’s a good, clean and friendly place to eat.” “Too cheap looking – the furniture will probably fall apart.” “Outdated and bad maintenance – probably cutting back on quality.” “New sign – they must be doing well –let’s try it.” Handwritten signs are pretty much a thing of the past except in “back in 5 minutes” type of situations. Do not confuse this with professional hand lettering, almost a lost art in the industry and highly prized. The office printer is common place and can be forgiven to be used as temporary signage. After a short period of time, the sign should be removed or replaced if the message is a permanent one. Temporary signage gets tattered, worn and generally lacks the authority of your company image. There is more to it than just written words. It’s an opportunity for prospective and current customers to identify with your business and image. An experienced, professional graphic designer and sign maker will make your signs work for you.

Pre-Nup? I Don’t Need No Stinkin’ Pre-Nup! Okay, before you become completely appalled that I would even write about such a topic in the annual Bridal Issue let’s take this one step at a time. We’ll start with a quiz: 1) What do you know about prenup agreements?

2) Do you think they are just for the rich and famous? a. Yes. b. No. c. Of course, nothing I have is even worth protecting … although I did work for about 10 years prior to getting married and scrimped and saved to buy the house we’re about to buy with my money and none of his/her funds are going towards the down payment …
3) What do you know about ante-nup agreements? a. Ante-what agreements? b. A little (I work at a law firm). c. Enough to know that I should have had one during my marriage to my second husband/wife. In this article I attempt to dispel the myths of pre-nuptial agreements, explain their advantages, and how such an agreement is beneficial. I also explain what an ante-nuptial agreement is and their benefits as well. As discussed in previous articles (found on my website www.pinterlawonline.com), property acquired prior to marriage is generally considered pre-marital. However, a pre-nup puts the non-marital nature of such property in black and white and excludes it from division therefore saving you legal fees and time. Also, due to space constraints, this article is only an elementary discussion of pre-nups and ante-nups. For further and more in depth discussion, please visit my blog at www.pinterlawonline.com. A pre-nuptial, or “pre-nup” as it’s commonly referred to, is a legally binding contract between two expectant spouses which is executed prior to marriage. It describes what property is owned by whom, that each person has full knowledge of the assets of the other person, and how that property should be divided (i.e., excluded) in the event of a divorce.
By entering into a pre-nup that lists those properties as non-marital and therefore excluded from any property division in the event of a divorce when Mr. or Ms. Right turns out to be Mr. or Ms. Right-for- someone-else-but-not-for-me, you are protecting them from having to be divided and therefore protecting yourself and your family by keeping the gains you all worked so hard for.
Myth Two: Only really rich people need a pre-nup. FALSE. While “rich” people may be more likely to get a pre-nup and understand (and appreciate) its importance, many other people can benefit. Think about your artsy friend who has worked for 10 years on her art/fashion line/music and is finally breaking into the “scene” and getting paid for her inspired works. Or think about your friend who has been single (on purpose!), just recently started a business which is struggling, but has a lot of potential, and now, at 42, decides to get married. Both of these women are perfect examples of who should be protecting all that they’ve worked for – which right now isn’t a lot but both have the potential to grow exponentially. And I don’t mean just in money terms. I also mean the actual rights to that art/fashion line/ music/business-with-lots-of-potential. By entering into a pre-nup these women are protecting their passion, in all honesty, from having to be separated into pieces and distributed during property division. Just briefly on ante-nuptial agreements – these are agreements that are entered into by spouses after marriage and protect assets that are the result
of the work of one of the spouses so that those specific assets are excluded from division during a divorce. Think of your friend who started a law firm after she was married for several years to her husband and she worked alone on that law firm with virtually no input from her husband. Under property division laws in Delaware, this business is marital and subject to division in divorce. However, if your friend enters into an ante-nup agreement with her husband excluding her law firm from marital assets, the success culled from all of her hard work, tears, and perseverance will be protected in the event she gets divorced (note to self – draft ante-nup agreement). In conclusion, I hope this article sheds some light on the issues of pre-nup agreements and introduced you to the concept of ante-nup agreements – and puts them in a better light. What Happens When The Caregiver Becomes The Patient? Delaware Workers’ Compensation Law

N
urses, therapists, medical technicians and other healthcare workers put their own health on the line every day as they care for their patients. In 2012, registered nurses and nursing assistants in Delaware had a combined total of 270 nonfatal work injuries that caused them to miss time from work. During that same time, and measuring the same nonfatal work injuries, construction workers had only 70 work injuries in comparison. Normally, we don’t think of healthcare workers as being employed in a “dangerous occupation” but the Delaware Department of Labor statistics certainly suggest otherwise. Some of the more common healthcare worker injuries are sprain and strain of the neck and back, rotator cuff injuries, needles sticks, assault by combative patients, concussions and slip and fall type injuries. These injuries can be brought about by virtually any aspect of patient care, and just about every healthcare worker has sustained at least one, if not several, of these types of work injuries. Healthcare workers are used to being the caregivers. They often experience hesitation, anxiety and reluctance for allowing others to take care of them. For that reason, many healthcare injuries go unreported, and the workers attempt to nurse themselves back to good health. Sometimes that works, but oftentimes it does not. As with every other aspect of healthcare, documentation is key. Therefore, if you are injured www.TheWomensJournal.com
(particularly as a healthcare worker) it is necessary to inform your supervisor and document the injury. Ask your supervisor to fill out a “First Report of Injury” and give you a copy. Also, make a note of any witnesses to your injury, and take pictures of any obvious bruising, cuts, scrapes or other outward signs of injury. This is exactly the type of situation where an ounce of prevention can equal a pound of cure, because as all healthcare workers know- if it isn’t documented, it didn’t happen! Once your workers’ compensation injury has been accepted by the insurance company, you may be entitled to benefits including medical expenses, wage replacement, mileage reimbursement, permanent impairment and disfigurement compensation. If you are injured at work, particularly if you work in the healthcare field, immediate and accurate documentation regarding the injury you sustained and how it occurred is of utmost importance. In order to make sure that your injury is properly documented, I highly recommend: 1. That you inform your supervisor in writing about your injury; and 2. Court Costs Awarded To Injured Victim
For Demonstrative Visual Aids Used At Trial

ecently the Morris James Personal Injury team tried a personal injury case in Delaware’s Superior Court and were successful in getting a very good verdict for the client, in large part due to their utilization of cutting edge visual trial graphics. A good trial attorney is a good story teller. Unfolding the story to a jury is much more effective today with the support and use of visual aids. Jurors not only want to hear about the incident, they want to see what is at issue. In this particular case the jury heard evidence of significant spinal cord injuries from three doctors and one dentist. At times, the information was very technical and intricate. To assist the jurors in understanding the medical evidence, the Morris James Personal Injury trial team used visual graphic illustrations and computer animations to assist the jurors by allowing them to visually see and better understand the extent of the injuries suffered by the client victim as a result of a serious motor vehicle accident. We believe, when compared to other jury verdicts for similar injuries in this case, the use of the visual aids and animations increased

the overall value of the victim’s injuries in the jurors’ minds. While visual graphics and animations are commonly seen in courtrooms on television and in the movies, the use of these graphics is very costly and not as commonly used in the “real” courtrooms of our community as often as one would think. One of the reasons these visual graphics are not used is because Delaware Judges have routinely denied successful plaintiffs reimbursement of visual graphics and illustrations costs used at trial. However, it is our understanding that this case was the first Delaware case in which a Superior Court judge ordered that the defendant’s insurance company pay fees incurred by the injured victim for costs associated in presenting demonstrative aids.
The Judge noted that other federal courts have long recognized, “Computers, computer graphics, digitized documents, and other technological advancements have become important tools of the modern-day trial lawyer.” DiBella v. Hopkins, 407 F. Supp. 2d 537, 540 (S.D.N.Y. 2005). Courts recognize trial graphics and visuals aids help the jury by focusing attention on salient documents and concepts. Courts recognize these demonstrative aids are useful at trial because it is an educational process.

pening Fresh Thymes our main goal was providing real whole food to our community and source from local chemical free farmers as much as possible. We wanted to create a place for all people to feel welcome so we designed a menu for vegans, vegetarians, and omnivores. Working with small local farmers we know where our food is coming from, how it is being produced, and supports www.TheWomensJournal.com
their livelihood. If we lose our small farmers, we lose our food supply. Sourcing food locally reduces the carbon footprint making Mother Earth happier and keeps the income flowing within our local community. The community sustains our business. We have met so many amazing people. As the community supports us through our ups and downs, we are happy to support our community. It feels good to help others. We created a happy little corner and thanks to social media attract tourists. It is so much fun meeting people from other areas, hearing how beautiful our city is, and making recommendations of things to do. It’s also really neat to hear people say, “It’s nice your community has great places like this.� It makes me realize we created the
type of place we would search for on our travels. As a small business owner my thought processes have changed. I rarely shop in national chain stores anymore. Joan Wuertz Receives Award For 30 Years Volunteer Service For Delaware Hospice J
oan Wuertz joined Delaware Hospice as a volunteer in 1984 with a passion for hospice care that was unusual in those early years. Joan was a nurse in the U.K., Canada and Washington DC before retiring in Delaware. She volunteered for a cancer ward in Wilmington, where she became quite discouraged with the treatment of terminal patients. “Visiting hours were very strict, which left patients alone when they needed to be with their loved ones. Patients were hauled away for radiation and treatments when they should have been resting. Pain medications were rigidly administered, regardless of the comfort of the patient.” Then, while vacationing in England, Joan had the incredible opportunity to visit an uncle who happened to be a patient at the world’s first hospice— St. Christopher’s, founded by Dame Cicily Saunders. “I was amazed at what I discovered there. People could be happy. There was a tremendous amount of pain control. Dogs and children visited at all hours. There was an overwhelming sense of peace and comfort. I witnessed not only acceptance of their condition by patients, but also dignity and respect surrounding them. It was so different than what I had experienced in oncology departments in hospitals in the U.S.” After her return to her home in Wilmington, a friend, Gloria Thornley, called, who had just become the first volunteer coordinator for the newly-founded Delaware Hospice. She asked Joan if she might be interested in volunteering, offering assistance to patients and families in the program. Joan replied, “Absolutely! Where do I sign up?” Thirty years later, she has never tired of supporting these families during their time of need!

“Things were naturally different in 1984, when Delaware Hospice cared for a total of 10-15 patients per week.” Joan said, “Volunteers provided a great deal more care for the patients in the early years. “We often performed services which are handled by nursing assistants today, such as bathing the patient or changing bed linens. There were only a couple of nurses on staff, and they would take turns doing rotations at night. They were happy to have somebody go out with them at 2 a.m., so they would come by and pick me up to ride along with them.”

Modern technology has alleviated a few challenges faced in those first years of Delaware Hospice’s existence. Joan remembered, “We always carried change for telephone calls since there were no cell phones. Everything was documented by paper. Nurses were not permitted to make pronouncements, so often we sat with families several hours through the night until a physician could come to the house.” Joan’s warm personality suits her volunteer role as patient/ family support, and she has clearly enjoyed the relationships with family members. “I’ve met and become close to so many memorable people over the years through listening and sharing stories. One patient was compulsive about bathing every day. No one came on weekends, so I would go to give her a bed bath. One family’s mother was ill, so I took her two teenagers shopping for school clothes. That was an experience I’ll never forget!”
