Laser Vision Correction: LASIK vs Advanced Surface Ablation; Safety And Results Dictate My Guidance L
aser vision correction, performed to reduce dependence upon glasses, affords patients the opportunity to dramatically improve their lives. As with many ophthalmology procedures, it also affords surgeons the privilege to provide impactful By changes. With Jeffrey B. Minkovitz, M.D. privilege comes responsibility to be prudent about risks, benefits, and patients’ interests and to use surgical skills and medical experience to guide patients appropriately. Neither celebrity endorsements, latest trends, nor advertisements of corporate medical chains should supplant thoughtful patient centered conversation and evaluation by responsible medical professionals. Laser Vision Correction has experienced remarkable advances in technology since approved in 1995. Expansion in treatment parameters allowing stronger corrections (nearsightedness, farsightedness, astigmatism) has made laser vision correction an option for many patients originally deemed ineligible. Technology advances affording greater accuracy, faster recovery, and less risk, have converged into a modern, accessible procedure with dramatic results.
Involved in laser vision correction since my ophthalmology training and an investigator in early studies leading to FDA approval, I participated in many of these advancements. Most have been of obvious benefit to patients. However, some have addressed patients’ desire for rapid results and convenience (admittedly not inconsequential) while also increasing risk. Eager to provide my patients with the latest advances, I oftentimes adopted new technology early. Nevertheless, I have always been guided by my responsibility to evaluate safety and efficacy, to “put to work” my experience and judgment and to determine whether a procedure, especially if new, meets the test of safety. Laser vision correction was first approved in 1995 as PRK (Photo Refractive Keratectomy). Safer and more accurate than its predecessor RK (Radial Keratotomy), it was hampered by discomfort and slow recovery. LASIK (Laser-Assisted in-Situ Keratomileusis) was introduced several years later, combining creation of a corneal flap and laser. Pain was virtually eliminated and great results were achieved more quickly, addressing patients’ main objections. Unfortunately, flap creation also adds an element of risk, and advantages must be weighed against disadvantages. Advanced Surface Ablation is a hybrid of the original PRK procedure and the subsequent LASIK procedure. Using the laser to reshape the cornea, no flap is created, but rather superficial surface cells are removed or loosened (just before laser is applied), matching or exceeding the accuracy of LASIK while avoiding safety concerns of cutting flaps. While surface treatment is necessary for certain cases, such as thin corneas, dry eyes and active lifestyles (military, police, sports) in which LASIK flaps may be contraindicated altogether, Advanced Surface Ablation can be used for all laser vision correction patients to provide a safer, accurate procedure. Surface treatment creates less thinning (therefore stronger corneas with less risk of ectasia or injury in active patients), avoids flap complications including wrinkles, dislocations, debris or inflammation
under the flap, and less risk of dry eye. In fact, the vast majority of LASIK related complications are associated with the corneal flap and can be eliminated by performing Advanced Surface Ablation instead. Furthermore, some recent studies suggest surface treatment may better take advantage of newer wavefront treatments designed to reduce aberrations and optimize results. Having performed RK, PRK and LASIK, I now recommend Advanced Surface Ablation for all laser vision correction patients. I strongly believe this form of laser vision correction provides the best balance between safety and convenience, while providing the absolute best visual results. Jeffrey Minkovitz, MD is an ophthalmologist with Eye Physicians and Surgeons in Wilmington, Delaware. Dr. Minkovitz specializes in cataract and refractive surgery, and is one of only a few Delaware surgeons specially trained in corneal transplantation. He completed a cornea and refractive surgery fellowship at the renowned Wilmer Eye Institute of Johns Hopkins. Previously, he graduated magna cum-laude from Harvard, earned his medical degree from the University of Massachusetts, completed internship at Harvard’s Mt. Auburn Hospital, and served his ophthalmology residency at Washington University. Dr. Minkovitz participated in the early research on Excimer Laser refractive surgery, and has offered this procedure to Delawareans since FDA approval in 1996, before Delaware had its own laser. These early pioneering individuals travelled to Wilmer Institute with Dr. Minkovitz, where he maintained his teaching and clinical appointment. In addition to introducing laser vision correction to Delaware, he also introduced NearVision CK to eliminate reading glasses, DSAEK (a newer, less invasive type of corneal transplant) and most recently the newest advance: laser cataract surgery. Dr. Minkovitz is a co-founder of the Center for Advanced Surgical Arts and serves on its medical advisory board. He is also on active staff at Christiana Care. While on staff of Wilmer at Johns Hopkins University, Dr. Minkovitz received the Outstanding Teacher award. He was also named Top Doc in ophthalmology by Delaware Today Magazine six consecutive years, and has been an invited instructor at American Academy of Ophthalmology Annual Meetings.

Each Quarter as I start to write the publisher’s note my mind will wonder off in a hundred directions. Should it be upbeat, should it be thought provoking, or should it be just cut and dry and make my life simpler. Truth is cut and dry is simple, but not of much interest, but it does fill the space. Thought provoking is good, but always seems to take too much space. So I am going to settle for upbeat. This issue is always our “Celebration of Life” issue. We pull together all these extremely smart, beautiful, strong women who have managed to take one of life’s most devastating illnesses and turn it in to a positive. Not just once, but for some several times. Breast cancer and ovarian cancer does not discriminate, it can happen whether you are young or old. It doesn’t care what ethnic group you are from, or how rich or poor you are. The one thing that I see each time I do my favorite photo shoot is that these are some of the bravest, most beautiful women that I have ever met. Happy to be alive! Appreciating what life has to offer, and always willing to help other women going through the same thing. Each of their profiles are their way of sharing their story, and hopefully giving hope to someone else that may or will someday have to go through what they already have. Breast cancer is the most common cancer among women in the United States (other than skin cancer). But millions of women are surviving the disease thanks in part to early detection and improvements in treatment. Ovarian cancer causes more deaths than any other cancer of the female reproductive system. But when ovarian cancer is found in its early stages, treatment works best. Ovarian cancer often causes signs and symptoms, so it is important to pay attention to your body and know what is normal for you. Symptoms may be caused by something other than cancer, but the only way to know is to see your doctor, nurse, or other health care professional. Thanks to all our wonderful contributors for your support of the County Women’s Journal, I hope that the holidays will be a wonderful time for everyone and that 2015 has been your best year ever. Is Your Old Filling Fine Or A Ticking Time Bomb? C
hances are your old mercury–silver amalgam fillings (commonly referred to as “silver” fillings) are pretty far down on your list of things to worry about. They’re there and don’t hurt, so they must be fine. Right?
This weakness is then complicated by simple physics. When you drink hot or cold liquids, your teeth expand and contract as do your silver fillings. The problem is that your teeth and fillings expand and contract at different rates and that difference can lead to micro cracks and hairline fractures.
Maybe. Maybe not! Some old fillings keep doing their job for years – even decades. Others however, are ticking time bombs that can strike without warning, causing a world of pain and setting you up for infection, a broken tooth, or even tooth loss. That’s something to think about – especially if you have fillings that are 15 years old or more.
When you bite down on these cracks they open, irritating the pulp (the soft inside of the tooth that holds the nerves and blood vessels). Once this pulp is damaged a root canal is the only way to save the tooth. A crown (cap) is needed to restore the tooth to sufficient strength to withstand biting forces. If the crack extends down the root of the tooth, the tooth must be extracted.
When Good Fillings go Bad
Fillings are subject to a lot more stress than you might imagine – from things like chewing hard foods, clenching your teeth when you are angry or upset, even grinding your teeth in your sleep.
Protection and Peace of Mind
It’s important to remember that not all amalgam fillings cause problems. I’ve seen fillings 20 years old that were just fine. The fact is that even without symptoms a serious problem can be brewing.
Over time, all that stress causes pretty significant damage. The scary thing is you probably won’t feel a thing or notice anything wrong until you have a serious problem.
So how do you know? My team and I examine old fillings under high magnification so we can see if there are problems and fix them before they cause significant trouble.
Stress can cause your filling to wear around the edges or pull away from the surrounding tooth enamel. This allows food and bacteria to sneak in under the filling creating new decay. The undermining of the filling with decay can infect the nerve of the tooth leading to pain and abscess. Any infection in the body can have serious consequences for your overall health.

Cracked and Fractured Teeth
If that’s not enough to be concerned about, how about the fact that old amalgam fillings can actually lead to cracked or fractured teeth? The problem actually started back when the original filling was placed. Unlike new tooth colored resin fillings which chemically bond to tooth structure, the amalgam fillings require mechanical retention to stay in place. Healthy tooth structures need to be removed to create adequate retention for the silver filling. This in itself makes the tooth weaker. Now add the decay undermining the filling years later and the tooth is vulnerable to breakage.

Hearing Loss and Dementia

A
decline in hearing has often been considered an inevitable part of the aging process. However, we now have sufficient evidence to know that hearing loss should no longer be considered a benign condition to be passively dealt with. In fact, the consequences of untreated hearing loss can have a far reaching devastating impact on health. Consider the following correlations:
The Heart and Hearing Connection
Studies have shown that a healthy cardiovascular system has a positive effect on hearing. A higher level of physical activity is associated with a lower risk of hearing loss in women as determined by a recent study out of Brigham and Women’s Hospital. In contrast, impaired cardiovascular health negatively impacts hearing since the inner ear is very sensitive to reduced blood flow and blood vessel trauma. A low frequency hearing loss may, in fact, be an early warning sign of cardiovascular disease.
Diabetes and Hearing
Adults with Type 2 diabetes have twice the risk factor for developing hearing loss due to the damage caused by the disease to the inner ear. This correlation highlights the need for regular hearing checks. The earlier a hearing loss is diagnosed, the more effective hearing treatment options are likely to be.
Fall Risk
Hearing loss is also related to an increased risk of falling if left untreated. People who do not hear well may lack good awareness of their overall environment, thus increasing the potential to trip and fall. Reduced sensory input also increases the cognitive demands on the person who is trying to maintain balance and gait. In addition, hearing disorders may
Multiple studies have indicated that untreated hearing loss can affect cognitive brain function and is associated with early onset of dementia in older people. A 2011 study at the University of Pennsylvania concluded that a decline “in hearing abilities may accelerate gray matter atrophy and increase the listening effort necessary to comprehend speech.” Could improved hearing then decrease the chance of developing dementia? The conclusion of the study was that “Hearing aids may not only improve hearing but preserve the brain.” Several recent studies have concluded that early diagnosis and treatment of hearing loss slows the progression of dementia and Alzheimer’s disease.

Hearing in the Workplace
The Better Hearing Institute evaluated the impact of untreated hearing loss on family income. Their study linked untreated hearing loss with “reduced earnings, increased workplace absenteeism, and lower workplace productivity, as well as depression, anxiety, and cognitive decline.”
Cancer Treatments and Hearing
Certain chemotherapy medications or radiation therapy can cause damage to the ears and therefore result in temporary or permanent hearing loss, tinnitus and/or balance disorders. This is something that patient and physician should discuss so that the patient can be aware of the potential risks. A hearingmonitoring program could then be put into place as part of the treatment process and thus relieve potential anxiety and prompt timely treatment.
The Importance of Audiological Evaluation
Hearing and good health go together. If someone suspects hearing loss, an audiological evaluation can help the physician determine other health conditions. Prompt treatment of hearing loss can also help prevent other undesirable complications; such as falling, dementia and depression.

ost people, even if they have never experienced a treatment, have at least some understanding of acupuncture. They know, for example, that acupuncture involves the insertion of thin, sterilized needles into various points around the body for the treatment of pain and other conditions. This form of acupuncture is typically known as classical or “body” acupuncture. Several different “systems” exist within the rubric of body acupuncture, such as TCM or traditional Chinese medicine acupuncture, Five Element acupuncture, and French Energetic acupuncture. Fewer people are aware that there are additional forms of acupuncture that utilize a specific body region to treat similar conditions that the various systems of body acupuncture treat. This form of acupuncture (known as Microsystem acupuncture) primarily utilizes a single area of the body, such as the scalp or hand for treatment. Auriculotherapy is a specialized form of acupuncture, developed by a French physician, Dr. Paul Nogier, over 50 years ago, that utilizes the ear as the primary therapeutic intervention point. Auriculotherapy can be an effective treatment for a wide range of conditions, including pain, anxiety, depression, dry
During an auriculotherapy treatment, various points on the outside of the ear are stimulated via the insertion of extremely thin, sterile needles. Sometimes, other modalities, such as laser light or mild electrical current are used in place of, or in addition to, the use of needles. Like body acupuncture, auricular acupuncture often requires a series of treatments to reach full effectiveness.
How It Works Battlefield acupuncture is a unique system of acupuncture that utilizes specialized needles called ASP needles (for auricular semipermanent) for treatment. ASP needles are extremely small (about the size of the tip of a ballpoint pen) and made from gold.
In recent years, a specialized form of auriculotherapy has emerged called Battlefield acupuncture. Also known as Rapid acupuncture, Battlefield acupuncture was developed by Dr. Richard Niemtzow, a former military doctor and radiation oncologist by training. Dr. Niemtzow currently advises the Surgeon General of the U.S. Air Force on complementary and alternative medicine as well as continuing his research into Battlefield acupuncture and maintaining a clinical practice.
The needles are inserted at specified points on the outside of the ear and are kept in place for two to three days. Because the ear is highly sensitive and innervated, ASP needles are highly effective at treating pain and other conditions. In a typical treatment for low back pain, for example, an ASP needle is inserted into a specified point and the patient is asked to walk for 30 seconds or so. The level of pain is reevaluated and, if the pain level has not dropped a sufficient amount, a second needle is placed in the same point in the other ear. This process is repeated and each time a needle gets inserted in a new point, until the pain level diminishes 80-90 percent. There are up to five points in each ear that can be utilized for the treatment of pain.
Though not actually used on the battlefield, Dr. Niemtzow developed Battlefield acupuncture to be a quick, effective therapy for the treatment of pain in wounded military personnel. Initially, it was designed to be used by physicians and medics in military field hospitals to rapidly treat pain in wounded soldiers when conventional pain medications were not available or not indicated. Since then, Battlefield acupuncture’s reach has expanded to include some of the military’s major medical facilities such as Walter Reed Military Medical Center and Landstuhl Regional Medical Center near Ramstein Air Base in Germany.
In most cases, the patient experiences a dramatic reduction in pain. The needles are then left in for two to three days and the process is repeated additional times if needed until pain is gone or significantly improved.

While the needles are in place the patient can sleep with them in, bathe, and use the telephone. He or she must examine his or her ears twice daily for signs of irritation or infection and remove the needles if these are apparent. Battlefield acupuncture is extremely safe and efficient. Needles can be inserted within a ten to fifteen minute office visit. The main side effects are soreness of the ears. There is a risk of infection but it is very low. In my experience, patients who try Battlefield acupuncture often experience dramatic reductions in pain within a few treatments. Sometimes, even with the first treatment, pain is reduced so much that some patients have a hard time believing it. We have also had very good success treating migraine headache. In addition, Battlefield acupuncture can be used to treat anxiety and mild depression, dry mouth, allergies, and other common conditions.

ery few people realize how much we use our teeth on a daily basis. That is until tooth loss occurs. When teeth begin to fail a patient’s oral health and quality of life suffers. Like the old phrase “You don’t know what you’ve got until it’s gone.” When we lose teeth we begin to understand the scope of impaired oral function and how it affects our daily lives. Patients with missing/failing teeth suffer with malnutrition, reduced self esteem, and confidence. They possess a high risk for developing oral conditions such as bone loss. Patients also suffer from periodontal disease, cavities, root canal infections, and serious bite problems called “Bite Collapse”. Patients with severely failing dentitions can also be more at risk for serious medical conditions. It is well known that these patients have high risk of cardiovascular problems, risk of stroke, diabetes, and complications if pregnant with a high risk of preterm and low birth weight babies. The typical sequence of therapy to address these complex problems requires at least 3 surgeries. Initially the teeth are extracted and the bone and teeth extraction sites are allowed to heal. The implants are then placed and left to heal for 3-4 months. Finally a third surgery is done to expose the implants. Start to finish time is 6-8 months. During this time patients are left with only removable dentures on which to function. New Techniques In MRI

RI is constantly changing and new techniques and knowledge is shared at the annual meeting of the International Society for Magnetic Resonance in Medicine (ISMRM). At the last meeting, there were examples of coronary artery imaging completed within 15 minutes, brain trauma highresolution imaging at 3T, MR neurography at 3T, functional MR imaging of brain trauma and PTSD. Each one of these topics could fill this article, but I will use an example that hits close to home. I recently saw a college student who had sports-related head trauma; he had difficulty in concentration and a decline in grades. The student and physicians knew there was a problem but all the MRI scans were negative. A 3T MRI performed with susceptibility weighted (SWI) revealed the problem. Even though the previous studies were negative, a better study showed microscopic bleeding in the brain. This is only possible using the higher resolution of 3T (or 7T—not easily available). Over the past few years, we have found these microbleeds are related to a host of brain diseases including: Parkinson’s, Alzheimer’s,
head trauma, brain atrophy and stroke. In all of these diseases, the connections in the brain are disorganized and lead to various brain and psychiatric disorders. I mention this because surprisingly few neurologists or neurosurgeons seem to have much interest in studying these microbleeds which are so vital to the understanding of brain diseases. Sadly, it appears one has to go to university hospitals to have a reasonable discussion of this concept. There are so many brain diseases that require SWI at 3T, however, patients tell us that when they ask their doctors about this, they are met with a blank stare. Do your own web search. Inform yourself. Feel free to give Dr. Chao a call to discuss. This is just ONE of literally hundreds of benefits of 3T MRI imaging. These reasons, amongst others, are why MRI Consultants, with two 3T MRI scanners, was created. We have more than eight years of experience and continue to explore the benefits. We have an office in New Castle County and one in Sussex County. MR neurography should be used for most patients who have a peripheral neuropathy which is not explained by MR scans of the spine. To date, I have not had a single request for MR Neurography. Diffusion Tensor Imaging (DTI) shows the connections in the brain and can show evidence of microscopic trauma – only doctors have asked about this very important technique. If you have head trauma and have difficulty thinking, you need a DTI study and a SWI study done at 3T.

As an aside, you are probably aware that most insurance companies now require prior authorization before agreeing to pay for Advanced Imaging. What many people do not know is that in order to prevent self-referral and inappropriate imaging, this request for authorization must come from the doctor who knows why you need the study. Dr. Philip Chao graduated Phi Beta Kappa from Yale University and was Scholar of the House. He continued his studies at the University of Rochester School of Medicine and Dentistry, receiving his medical degree in 1983. After a transitional internship year at the Mary Imogene Bassett Hospital in Cooperstown, NY, Dr. Chao did his residency in Radiology at Brooklyn’s SUNY Health Science Center (19841987), where he became Chief Resident. He stayed on to complete a fellowship in Neuroradiology (19871988). Dr. Chao has been interested in MRI technology since its very beginning – the first MRI scan took place in 1981 while he was pursuing his medical studies. The University of Pennsylvania, where Dr. Chao was both a Body MRI Fellow (19881990) and a Neuroradiology Fellow (1989-1990), was at the heart of the development of this technology and Dr. Chao was able to work with pioneers in MRI on the first GE Signa 1.5T (tesla) scanner. His advanced research and specializations complete, Dr. Chao left the University of Pennsylvania for a position as Director of MRI in Wilmington, DE – a position he held for 14 years. While 3T (tesla) scanners have been used for scanning research patients since 2002, these advanced, more powerful MRI scanners only became available for clinical use in 2004. How does tibial nerve stimulation work?
Essentially, it’s an acupuncture needle inserted into the skin to stimulate the anterior tibial nerve on the outside part of the ankle. Most people say it hurts less than a bug bite. Typically, patients get weekly treatments for 30 minutes for 12 weeks. Some patients might require treatment twice a week—the schedule can vary based on how each individual responds.

Most women see improvement by the fifth week, as their OAB symptoms decline. For people who have been struggling with the discomfort and challenges of OAB, the change marks a big improvement in quality of life. A Three-Tier Approach to Spider Vein Treatment By Nadiv Shapira, MD, First State Vein & Laser Med-Spa
P
atients often come to our office with spider veins, hoping a few injections will cure them. But even though they may look innocuous, spider veins are often indicative of venous insufficiency. Addressing spider veins while ignoring the underlying veins that feed them is unlikely to be successful. Venous Insufficiency: Healthy veins carry blood through a series of one-way valves. These valves allow blood to flow from superficial to deeper veins and then to the heart. Normal valves prevent back flow. Defective valves cause blood to flow backwards and pool inside the veins. Pressure then builds up and the vein’s wall weakens, distends, bulges and twists. Spider Veins: Red, purple, or blue fine veins appear in starburst-like clusters near the surface of the skin, measuring between 0.3 and 1.2 mm in diameter. Most spider veins are caused by venous insufficiency involving deeper veins. Tier I. – Simple Spider Veins Treatment of spider veins not caused by insufficiency of deeper veins is limited to the spider veins alone

Should I Learn CPR? L
et’s start with a few statistics. The number of deaths from breast cancer in the United States is 40,000 per year. The amount of deaths of prostate cancer per year is 30,000. The deaths from sudden cardiac arrest are 350,000 per year. Sudden cardiac arrest is not the same as having a heart attack. Sudden cardiac arrest usually occurs from a sudden abnormal electrical rhythm of the heart. With this abnormal electrical activity, there is not enough blood pressure to support brain activity and within minutes, it is usually fatal. With CPR alone, the survival rate can be increased three-fold. Unfortunately, only one-third of the patients with out of hospital cardiac arrests have bystander CPR performed. 85% of the sudden cardiac arrests happen at home so the CPR would be usually performed by a loved one. Always a troubling statistic is frequently even people who can perform CPR do not always perform immediate CPR on family members because of the inability to perform CPR in a sudden stressful situation. So how can you perform CPR? Traditionally, you would go to the American Heart Association and attend a basic lifesaving class. These classes still exist and I would encourage you to take one. There is, however, a new exciting approach where you (you reading this article) can become a lifesaver as well, even though you may have not ever considered it. The American Heart Association has started a new program to try to get CPR literally into more hands. You can do this whether you are 8 years old or 80 years old. If you simply go on the Internet and type heart. org/cpr, you will be on their website and if you go to the tab that says Hands-Only CPR™ in the upper right corner, you can watch a 1 minute video on how to perform this lifesaving method.
Why did the American Heart Association create this in addition to their other lifesaving classes? It is a matter of increasing the number of lifesavers. It has been discovered that immediately calling 911 and immediately starting good chest compressions is the most important initial treatment in improving the survival of out of hospital cardiac arrests. This is even without giving respiratory breaths to the victim. Another way that we are increasing the amount of lifesavers is the fact that in 2014 the Delaware legislature passed a bill that all high school students will learn CPR prior to their graduation. They will also be instructed in the use of automated external defibrillators. Menopause – Don’t Fear The Change

Menopause is when a woman’s monthly meno (“menses”) pause (“pausis”) for a consecutive year. The average age women reach menopause is 52 years old. However, it can occur anytime between 42 – 56 years of age. The primary complaints about menopause are hot flashes and night sweats. Other common symptoms include: vaginal dryness, mood swings, irritability, insomnia, depression, memory lapses and weight gain. Most cases can be diagnosed on the basis of symptoms and
menstrual history. To confirm menopause, a FSH (follicle stimulating hormone) level can be checked. If the level is greater than 30, then menopause is confirmed. However, during the perimenopausal period (the time leading up to menopause) a woman’s hormones can fluctuate immensely and therefore laboratory testing of hormones is not reliable. The goals of treating menopause are to provide relief of symptoms and to prevent disease (i.e. osteoporosis and cardiovascular disease). An integrative approach to menopause focusing on diet, exercise, stress reduction and hormonal balance is essential. There is no need to fear the “change” as this holistic approach will help you feel better and prevent chronic disease. Nutrition plays a fundamental role in menopause treatment and prevention of chronic disease. There are numerous health benefits of regular exercise during menopause, namely a reduction in cardiovascular disease and breast cancer. Other benefits include increased bone density, improved heart function, reduced blood pressure, decreased cholesterol, increased energy and an improved sense of well-being. It is recommended that menopausal women exercise at least 30 minutes, four times per week to achieve these health benefits. However, the requirement for weight loss during menopause is 60 minutes daily. Perimenopause and menopause can be a very stressful time in a woman’s life. Continued stress weakens the immune system and increases the risk of disease including: autoimmune disease, cancer, diabetes and high blood pressure. Therefore, it is important to practice stress reduction techniques such as meditation, yoga, or breathing exercises. In addition to eating a whole foods diet, exercising and engaging in stress reduction techniques, vitamin and botanical supplements can be very helpful in menopause. Most women should take a good multivitamin (one that provides 70% – 100% of the recommended daily allowances of vitamins and minerals) and fish oil. Fish oils (with EPA and DHA components) do not only aid in cholesterol reduction and mood elevation but also have been shown to decrease hot flashes and night sweats. Black cohosh is also very effective in treating hot flashes. In addition, there are numerous supplements for other individual symptoms of menopause, such as: Valerian or Hops for insomnia, Chaste tree berry for erratic menstrual bleeding, St. John’s Wort for depression, and Kava for anxiety. However, an integrative physician should be consulted regarding the proper dosing, safety, and potential herb-drug interactions. When diet, exercise, stress reduction and botanical treatments are not effective, hormone replacement therapy (HRT) should be considered. Understanding the benefits and risks of HRT is critical to determine if it is right for you. Any HRT should be used in the lowest dose and for the shortest duration. Bioidentical hormones (those that match our body’s hormones) prepared by compounding pharmacies offer several advantages, including: customized dosing, more hormone combinations and delivery options (i.e. creams, capsules, pellets). Again, this should be under the direction of a physician experienced in bioidentical hormone replacement therapy.

Physical Therapist Or A Chiropractor? All Musculoskeletal Pain Is Not Created Equal

hysical therapy and chiropractic care share many of the same qualities, but each has its own benefits and it’s important to know which specialist to see for certain care. They both have been proven to be effective treatments of conditions like lower back pain, neck pain and general muscle & joint injury, but they are not one in the same.
So what’s the difference? Chiropractors primarily focus on the spine and alignments of the joints. Their goal is to make sure the body is balanced. Misalignments and imbalances can lead to disorders that affect the nerves, muscles and organs. Chiropractic is mostly used to treat neuromusculoskeletal complaints. Physical therapists treat the spine AND their muscle attachments. They aim to restore, maintain and improve function & movement to ensure optimal quality of life. Physical therapy patients range from athletes (of all ages) to the working class and the elderly — as they also help address orthopedic problems like osteoporosis or neurological problems due to stroke and Parkinson’s disease.
Choose wisely.
Physical therapists emphasize independence and return to function with activities (i.e. walking, stair climbing, and getting into and out of a car or chair) throughout your life. This is achieved through evaluation of acute and chronic pain conditions then incorporating mobilization and treatment of soft tissue and joints which restore natural motion. They are very effective when your movement and function are affected by aging, injury, disease or environmental factors (i.e. work or auto accidents). This treatment is also ideal for rehabilitative exercise before and after surgery.
Is one better than the other? Physical therapy and chiropractic care have similar end goals: to alleviate tension & pain, and restore mobility without drugs or surgery. However, if you’re looking for a more comprehensive approach to care, physical therapy is a better option. The scope of technique of a physical therapist goes beyond alignments and adjustments of the spine, to include mobilization of the spine and joints, re-education of muscle function, dry needling, endurance training and focus on return to sport or recreational activities. In many cases, with physical therapy, no surgery or long-term medication is needed.

Which should I choose? If movement has already been lost and has become painful, chiropractors are effective with restoring function to your spine. Through realigning the vertebrae in your spine, tension is released in the muscle allowing the brain to communicate better with your body, thereby promoting better movement, balance and nerve function. Unfortunately, in some cases, opting for the wrong specialist can worsen an injury or cause a new one. This is particularly true of a chiropractor who focuses more on skeletal manipulation than any other technique, as some patients do not respond well to these techniques. Diagnostic testing is typically key to ensuring the appropriate method of treatment, however, oftentimes physicians will refer a patient to physical therapy for treatment prior to ordering any tests. Many insurance plans allow patients to visit a physical therapist without having to see their physician first. The wrong choice can lead to endless amounts of back adjustments at worst or a lifetime of improved overall well-being at best.

n our last article, it was discussed how to identify the changing needs of our loved ones, as well as the process of searching for an Assisted Living community. This article is meant to discuss what to look for in the Caregiving staff, apartments and activities.
What should I look for in the Caregiving staff?
The Caregiving staff is a group of individuals who will be doing the hands on, day to day care, and supervision of your loved one. Ask what the certification and experience level of the Caregivers is. How many hours do they work per shift? Is there a licensed nurse on staff? How does the staff handle emergencies? Does the community hire and train their own staff or do they use agency staff? If you are out of the area, how will you be notified of changes in your loved ones condition or care? These questions will help give you an idea as to what your loved ones day to day care will be.

What should I look for in an apartment/suite?
This is one of the areas where communities can vary greatly. Some communities offer private apartments, and some communities offer semi-private accommodations. Look at this from two different perspectives. What kind of atmosphere is your loved one used to? Will they be comfortable in a private apartment or will a semi-private room be acceptable? Is there adequate closet space? How much of your loved ones belongings will fit into the space? The other way to evaluate this is, how much space does your loved one need? Some communities have generous public areas and encourage Residents to get out of their apartments as much as possible to socialize and be active. Other communities offer larger suites/apartments, but may not have as many generous or elegantly appointed common areas.

Volunteers Come In All Shapes And Sizes D
uring our 33 years of service to the community, Delaware Hospice has been touched by the tremendous support of thousands of volunteers. These fun-loving, individuals have donated their time and talents throughout the years in a variety of ways. Within the past year alone, volunteers have donated over 15,200 hours! The quality of life for patients and their families are dramatically improved through the services and visits our volunteers provide. Many of our volunteers come to us and offer up their skills for very unusual projects. While others are able to meet our specific daily needs. “I don’t have any skills to be a good volunteer with Delaware Hospice,” replied Lisa to a family member. “Don’t worry, we will find something for you,” commented Lisa’s family member. That was 14 years ago, when Lisa first joined Delaware Hospice as a Festival of Trees volunteer. After some encouragement and asking for a position that coincided with her strengths, she has found her place at Delaware Hospice. Based on skills and wishes, volunteers help in a variety of areas throughout Delaware Hospice. Running errands with or for patients is always a needed service and is a wonderful experience for the volunteer and patient. Visiting with patients in one of our programs, Transitions, Palliative Care, or Hospice Care, is another rewarding volunteer opportunity. Many volunteers choose to talk, read, and play cards with a patient and their family. Other volunteers enjoy helping in the Delaware Hospice office and contribute administratively by updating the database, completing informational packets, or even taking pictures at our events. Volunteers are always
able to see how their dedication positively impacts our patients’ lives. A few of our volunteers have even altered their life’s work based on their volunteer experience.
Each day, requests come into the Volunteer Department for volunteers and each day those needs are met. Here are just some of the ways volunteers have saved the day: •
Michael Buchler, Jason Clarke, and Anthony DiMarzo of Williams Agency, Inc. in Wilmington have helped run the New Castle County Golf Outing for many years.

Staff members from Hopkins & Sons donate their services since 1991 to be the official movers for over 1,000 trees for the New Castle County Festival of Trees.

Cindy Wright has been doing database entry, special projects, and has also played cards and looked at family pictures while visiting with patients for the last three years.

As a professional Christian clown, Bill Robbins would visit nursing homes to entertain the residents. He loved making people laugh. One day, he walked into a room and saw that a patient was dying. She had no family or friends with her and that troubled him. During this experience, he realized that people die alone every day and he wanted to do what he could to not let this happen in his community.

Mr. Robbins learned about the Delaware Hospice Milford Center and wanted to be a part of the mission by becoming a volunteer. Based on his day of shadowing the Center Chaplain, Mr. Robbins decided to become a chaplain. During his six years with Delaware Hospice as a volunteer chaplain, Mr. Robbins has developed a Volunteer Chaplain Training Program. He works with interested chaplain volunteers.

fter considerable research interviewing successful women about their concerns, ideas, and opinions regarding “women and their money,” I thought I’d give you the benefit of some of those “initially” incredibly important points as I continue my research. “Most women are not on sound financial footing, they have too much debt. They are too vulnerable!” One lady mentioned in earnest recently. “In your next article in Women’s Journal, you should stress to them to prepare for contingencies.” So here goes:
Do you….
• have a credit card in your very own name? • have your own checking account? • actually participate in bill paying each month? • know where all the money is and where it’s going? • have an “emergency fund?” • know how much total debt you have and actually monitor it monthly? • have a plan to reduce and eliminate your debt? • have a plan to “live within your means?” (It’s called a budget!) • know what your monthly expenses are? • know what your monthly income is, net of income taxes?
• know how all the accounts are titled? • have the ability to access all the accounts via the web? • you know the passwords and logins? • have a will; power of attorney, trust, and living will? • know if those important to you have copies of or access to those documents (children, trustees, etc.)? • know where your original social security card is? • know if you are the beneficiary of your husband’s life insurance, retirement plan and IRA’s? • know what benefits are due you in the event of your spouse’s divorce, death or disability? • know your credit rating? • know that all your bills are being paid on time, including income taxes? • know if you’ll have health insurance if something happens to your spouse? • always put other’s needs ahead of yours? Why? • have long term care insurance in case there is no one who will take care of you? • Are you prepared to go back to work, if you need to? • Are you prepared to increase your education or job skills if you need to? • Are you taking good care of your own health? • Have you faced reality or are you living in denial? (Which is not a river in Egypt!) • Do you have professional resources to call upon should the need arise (attorney, accountant, wealth advisor, insurance agent)? • Do you know if your beneficiary designations on your company retirement accounts, IRAs, and life insurance are current?

The Top 3 Myths about Having a Will: How a Little Bit of Planning Can Spare Your Loved Ones a Whole Lot of Trouble A
s an estate planning attorney, the one question that I am asked the most frequently is “Do I need a will?” With few exceptions, the answer is “yes.” A will allows you to direct how your assets will be distributed upon your death. It is an important part of your overall estate plan and one that should be carefully considered. Here are the top three myths that end up getting people into trouble:

Unfortunately, the disposition of assets doesn’t work this way. If you die without a will, the law steps in and determines how your assets will be distributed. Depending on whether you have surviving children, children from a previous marriage, parents or siblings, your spouse may not get everything. You cannot assume that just because you’ve orally left instructions for someone that those wishes will be carried out.
Circumstances change. For example, children born after a will is executed may not necessarily inherit the same property in the same shares as children already named in the will. Personal relationships between family and friends evolve and you may want to add or remove someone from your will. The legal climate fluctuates as well, sometimes making it advisable to modify your will to minimize
state and federal taxes. In most cases, it’s important to update your will every 5 years because your will from 10 years ago is likely not consistent with your current wishes. There are several other estate planning tools that you can utilize, depending on your needs. A trust, for example, is a very effective vehicle that can be used in tandem with a will or as a substitute for one altogether. Like a will, a trust allows you to retain control over the exact disposition of your assets. As an added benefit, the assets in the trust will usually not be subject to probate upon your death.
It is a common misconception that unless you have a lot of money, you don’t need a will. The truth is, if you have young children, you should have a will. When there are two parents fighting over an issue pertaining to their child or children they will expect the following: 1. That the parents have talked about the issue before coming to Court; 2. That the parents have tried to come to some agreement by acting reasonably; and 3. That the parents are using the Court as a last resort. Do not expect either a happy Judge or a decision in your favor when you’ve done nothing to help move along the situation before taking it to Court. Number two, Co-Parenting: this is a BIG issue. The Court expects you and the other parent of your child or children to continue to raise this child or children together in a respectful, mature, thoughtful, unresentful, non-spiteful, reasonable manner. However you chose to have your child or children – be it a long term relationship, marriage, or a one night stand (to quote Dr. Phil “When you choose your actions, you choose your consequences” and we all must understand that in every action we take we are making a choice. I’ll continue to have my “Dr. Phil” hat on throughout this article, so be forewarned.) – you and your child or children’s other parent are tied together for life. It doesn’t stop when the Custody order comes down, it doesn’t stop when she, he or they turn 18. It lasts forever, and Ever, and Ever. This means, going back to point one (Expectations of the Court), you www.TheWomensJournal.com
have to communicate with the other parent somehow. So many people I speak with state that they just can’t talk to the other person – they either get yelled at or berated; agreed with so they’ll just shut up; or ignored altogether. Of course you can’t communicate! You’re divorced, broken-up or barely know each other in the first place. But guess what? You will get absolutely nowhere if you don’t continue to try to communicate. You have to make decisions together. You have to agree on deviations from the Custody and Visitation schedule together. You have to sometimes make alternate arrangements for care for the kids together. You have to agree on extracurricular activities, summer camp/care, schooling together. It comes down to one thing – you remain the parents of this child or these children and no matter how much you may hate the other parent or how stupid you think they are or how they are acting, you have to be able to communicate somehow and keep at it until you find a way that works for both of you. Third is considering your actions through the eyes of your child or children. I think we can all agree that a divorce/break-up is a decision made by parents and children get no say or control in the matter. You must constantly consider how your actions against/ about/involving the other parent looks from your child or children’s point of view. Kids are sponges and absorb so much of their

surroundings, much more than they let on. They for sure are taking in how you treat their other parent. This not only puts the child in the middle of your relationship and creates feelings of guilt and sadness, but also affects how they are learning how to be married and have children in the future. Is it hard to be respectful of the other parent when you have a lot of resentment? Of course, but you have to do it anyway for the health and welfare of your child or children.

Are Uber drivers protected? Delaware Workers’ Compensation Law

here’s good news for tech-savvy Delawareans looking for a cheap way to get around: the app-based car service Uber is now officially operating in the first state. Almost like a cross between a dating site and a taxi company, the handy smartphone app allows users who need rides and drivers who are looking to make some fast money to connect, with the drivers using their own cars and the whole transaction conducted through the app. Sounds like a great thing for Delaware, right? Well, probably – as long as the app’s users understand its risks. As a workers’ compensation attorney, when I learned about Uber’s debut in Delaware I immediately thought (like any lawyer), about what could go wrong – particularly for Uber drivers if they get into an accident on the job. Normally within the Delaware workers’ compensation system, if you are injured at work, you can be compensated by your employer’s insurance carrier for your medical bills, lost wages and other costs. These laws are in place as a kind of exchange - you give up your ability to sue your employer for unlimited damages, and in return, they are required to carry insurance that will
cover you if you are injured in the course and scope of employment. If, on the other hand, you are classified as an independent contractor or freelance worker, as is the case for Uber drivers, you cannot expect your employer to pick up any expenses if you are injured in a work accident. That is because, legally speaking, independent contractors are selfemployed. According to the Delaware Workers’ Compensation Act, workers in this category, which includes everyone from Airbnb hosts (who rent out their homes online) to drivers with Uber’s competitor, Lyft, do not qualify as official employees, and therefore are not guaranteed typical employee benefits, like workers’ compensation. While Uber drivers (contractors) deal with many of the same dangers as truck drivers or delivery drivers, if they are injured on the job they can be left with huge medical bills and no way to pay their rent, while employees are protected under the workers’ compensation laws. Some believe that since Uber drivers share these hazards with other workers, they should have access to the same workers’ compensation coverage, whether they are official employees, or contractors. Of course, independent contractors also tend to benefit from greater flexibility in their working lives. Uber drivers set their own hours and can work as little or as much as they would like to throughout the week, a perk that, for some drivers, outweighs the risk of dealing with expensive bills following a work injury. Still, under the current laws, what is important is that anyone who decides to use the app is aware of what they are getting into. Those who are thinking about becoming Uber drivers in Delaware should weigh whether the convenience of providing rides through the app is worth potentially bearing the cost of an accident. social sites provide many with a personal platform to connect with friends and provide personal details of our lives, what many do not realize is that this personal information may adversely affect your personal injury case. Just imagine the distress of being involved in an automobile or work accident that was not your fault, where you suffered pain and physical limitations, and loss of income, forcing you to file a lawsuit for compensation. It may include postings and pictures highlighting the fact that you were “feeling good” and having a “great day” at any point after your accident. Pictures showing you and your family traveling on vacation, your participation in a sporting event, or even you holding your young niece or nephew at their birthday party may be highlighted. Then imagine the attorney for the insurance company showing a jury all of these personal postings and pictures and arguing that you clearly are not as hurt as you claim to be. Once a jury begins to doubt your
posting any information regarding an accident you were involved in. This includes information about the accident itself, your injuries, as well as pictures of you engaging in physical activities. All of that information could be used by the insurance company to distort the truth of the impact of the injuries on your life.
And while you may think your posts on social media are private and should not be shown to the jury – guess again. Regardless of the privacy settings you use on your social media sites, according to recent Delaware case law, “where a proponent seeks to introduce social media evidence, he or she may use any form of verification available under Rule 901 – including witness testimony, corroborative circumstances, distinctive characteristics, or descriptions and explanations of the technical process or system that generated the evidence in question – to authenticate a social media post.” Parker v. State, 85 A.3d 682, 682 (Del. 2014)

I am writing this summary of my breast cancer journey just as a friend lost her brief and courageous battle with another form of this disease. Today so many of us survive cancer, but it remains an ominous diagnosis. I grew up in a generation where a cancer diagnosis was a death notice, and treatment options were limited, brutal, and seem to yield few survivors. I was as afraid of the treatment as I was of the disease. This was my perception and it created an edict in my mind that if I was ever diagnosed with cancer, I would take a pass on treatment, therefore, why bother with medical tests? Such a warped philosophy was borne from fear. In July of 2011 I was suddenly confronted with my worst fear: a diagnosis of a stage 3 infiltrating ductal carcinoma and a treatment plan that included chemotherapy, mastectomy and radiation, oh my! Confronted with my mortality, however, I did not “pass” on treatment; instead, I became willing to do whatever was necessary to save my life.
The message I want to share is that everything I feared in a diagnosis and treatment was unfounded. So run, don’t walk, for your mammogram. My actual experience was that treatment was not easy, but it was humane, there were available remedies to deal with side effects, I was treated with dignity, and I was able to maintain a fairly normal, but limited (and bald!) lifestyle. I also availed myself of all the resources I could to adjust to living with cancer. Those resources included the Delaware Breast Cancer Coalition, the Cancer Support Community and the Helen Graham Cancer Center. I realized my treatment experience was the result of thousands who had come before me and who had contributed to the decades of research. Those people from my childhood, and the years in between, who had no chance to survive, but whose sacrifices contributed to the medical treatment I was receiving now. I was amazed at the vast number of survivors who give hope to others, and what is hope? Facing the unknown and finding out you are not alone.

In October 2014 I went in for a 3D mammogram, I chose the 3D because there is a history of breast cancer in my family. The next day they called and said I needed to come back in for more images. After additional images and an ultrasound it was determined that I needed a biopsy. The biopsy confirmed that at age 45, I had Stage 1 lobular carcinoma and I was told it was 1 centimeter in size. After meeting with my surgeon I decided to have a double mastectomy with reconstruction. I knew this was the best choice for me. A week after my surgery, my surgeon called and said I made the absolute right decision. Not only was my original cancer actually 3.5 centimeters but there were also two other spots that were each 1 centimeter in size, none of the imaging picked this up. Due to the size I was now told I had Stage 2 breast cancer. Lobular breast cancer grows flat. It is not something that is felt in a routine exam. My team of doctors told me numerous times how lucky I was, and I will forever be thankful to the radiologist at the Helen Graham Center for flagging my mammogram.
I am blessed with a wonderful supportive husband who was there with me every step of the way. My children at the time were ages 7, 9 and 11, truly three of the bravest girls on the planet. Telling them I had cancer was probably the hardest part of it all. We had so much amazing support from our family and friends it really helped all of us through this. During one of my doctor’s appointments I picked up a brochure from the Delaware Breast Cancer Coalition explaining their Peer Mentor program. I decided to call and they matched me with someone who went through the exact same type of cancer as me. Words cannot describe how thankful I am for this program. I have been very open about my breast cancer journey in hopes that it helps to save someone else. Hearing you have cancer is not something anyone wants to hear but knowing you survived it makes you realize that anything is possible. This November I will celebrate one year being cancer free!

Two weeks before Christmas of 2007 my life changed. I was 20 years old and the healthiest I’ve ever been in my life. I was competing in fitness competitions and dieting was my lifestyle. It was also the time I’d be diagnosed with Ovarian Cancer. I begun experiencing what started out as little cramps and bloating in my lower abdomen. I went to see my primary care doctor who assured me I was fine and to call back if the feeling continued. Just a few days later I returned to that doctor when the pain became very severe and was preventing me from being able to stand. From that appointment I saw two more doctors that day. I was sent to have an ultrasound right away and then to see my gynecologist. My gynecologist said she would call me as soon as the results from all the tests she performed came back. The following day I was called in to her office to discuss my results. My mother came along with me and I remember being so terrified not knowing what could possibly be wrong, and the car ride to the hospital was silent. My doctor then proceeded to inform us there was a cyst/tumor that was the size of a grapefruit on my right ovary and that it needed to come out immediately. I don’t even know if at that point I could actually comprehend the news.
Friday, December 21, 2007, I showed up at Christiana Hospital to have my ovary removed. I was so worried about the scar that would be across my lower abdomen, little did I know that should have been the least of my worries. A week after the surgery, I was called back in to see my gynecologist who had performed my surgery. Awful news, the tumor that was removed was cancerous and was called an ovarian teratoma. A bizarre tumor that grows hair, teeth and nails and only about 2% are malignant (cancerous) and I was in that 2%. I just remember thinking “why me,” I was so healthy and young. I had to have 5-6 different chemo medicines a week, go through a blood transfusion, and lose my hair. The year after my chemo I went on to win The Delaware State Figure Competition and have now been in remission for eight years. I never let the cancer beat me. I had enormous support from family and friends. I don’t know if I would have made it without them, especially my mother. I now volunteer as much as I can with the American Cancer Society and this past year was on the committee for their Wilmington Relay for Life. That night was the first time I publicly told my story, this article will be the second. But as soon as I was contacted to participate in telling my story I immediately wrote back saying that I would love to. It doesn’t matter how old you are or how healthy you are, cancer can strike and I think it is very important to raise awareness whenever you can.

In April 2004, breast cancer was the furthest thing from my mind. I was 27 years old with no family history of the disease. I found a lump on the side of my breast when I scratched my underarm. I made an appointment with my family doctor and she could not locate the lump without my assistance. Since I had no risk factors, she wanted to monitor me for the next six months. I indicated that my husband Michael and I were ready to start a family, and I wanted to be sure it truly was nothing before getting pregnant. Four days after my mammogram, my breast surgeon showed me the mass was almost cut off the films because of its location. A fine needle aspiration confirmed it was cancer. Two days after my 28th birthday, I underwent a lumpectomy. That summer I started chemotherapy, followed by radiation and one year of Herceptin. I was unable to take Tamoxifen, as I am in the very small population of those allergic to it. I was very up front with my doctors about wanting a family, and how important children were to us. My oncologist agreed it would be okay to stop the Toremifene (the Tamoxifen substitute I was on) after two years to make my dream a reality. By the grace of God, I was able to get pregnant twice and have two beautiful daughters who are our miracles.
I am very lucky to have found my lump by chance. Eleven years later, my scars remain, but they serve as a reminder to have faith, to have hope and to have strength even when it seems impossible. Looking back on my journey, through the tears and the laughter (it truly was the best medicine) what could have been the worst year of my life, turned out to be possibly the most valuable. I learned to be my own advocate. I learned to be a fighter and a survivor. I learned what really matters in my life, and to celebrate and appreciate it all.

When I was first asked to provide a biography, my first thought was: “where do I start?” How can I possibly sum up battling breast cancer two times in three years into a few words? I am forty-three years old and I have been married to the love of my life for eleven years. We have two adopted children: Michael is five and Halie is eight. I do not have a family history of breast cancer and I am BRCA negative. My journey started in October 2011 when my doctor found a lump in my right breast. Luckily, this tumor was benign. In November 2012, my mammogram revealed more questionable areas and I was diagnosed with Ductal Carcinoma In Situ (DCIS) Breast Cancer, Stage 0. The next step was a lumpectomy to remove the cancerous tumor and the surrounding tissue.
I was not prepared for what came next. At my post-surgery appointment, the surgeon explained the pathology report showed a very aggressive, Stage II, Grade III, Triple Negative Breast Cancer. She also said I would need another surgery to clear margins, a lymph node dissection, chemotherapy and radiation. After surgery, I completed the eight, dose dense ATC chemotherapy treatments every other week for four months and then thirty-six radiation treatments. It took me a little over a year to recover from chemotherapy and I am still left with side effects. They are small battle wounds that remind me I won that war. My next challenge came in April 2014, when questionable areas were found on my ovaries, cervix and in my uterus. After many discussions with my doctors, I decided to have a total hysterectomy in an effort to eliminate any chances of this occurring again. My surgery was completed in July 2014. In April 2015, a breast MRI again revealed more questionable areas in my right breast. The diagnosis was DCIS, Stage 0. My surgeon explained I was going to lose my breast and I had to make the decision to reconstruct or not reconstruct. I chose immediate reconstruction. Since my journey began I have maintained my normal activities whether working full time, exercising, participating in my children’s activities, or volunteering as a fire fighter. I refuse to let this disease disrupt my life and it is very important to me for my children’s lives to stay as normal as possible. They are and always will be my inspiration. I also could not have made it this far without the support of my husband, children, family, friends, Pink Sisters and co-workers as well as everyone at the Helen F. Graham Cancer Center, Specialty Rehabilitation and help from the Delaware Breast Cancer Coalition.

I was 45 when I was first diagnosed with breast cancer. A career woman and single mother, I ate healthy, exercised regularly, and there was no history of cancer in my family. It began with a fibrous cystic breast that my GYN was concerned about. The mammogram and ultrasound came back clear yet she urged me to see a breast surgeon anyway. The surgeon found two cancerous tumors in my right breast, which meant I would need a mastectomy. I was confused and paralyzed by fear. Everything had been going exactly right, this was not part of the plan. I knew I wanted to be there to see my daughter grow up so in 2000 I had my first mastectomy followed by reconstruction, physical therapy, five years of Tamoxifen and routine oncology appointments. I learned to view every day as a gift and every visit to my oncologist as a chance to count my blessings as I sat among other women fighting their own battle with this disease.
Eventually life returned to a new normal. I regained my strength, confidence and a new perspective on life. All was good again. Then, 15 years later, a routine mammogram showed calcifications in my left breast, which turned out to be cancerous. The second diagnosis was as stunning as the first. I had taken excellent care of myself, how could this happen twice? This time, there was no hesitation; I mobilized immediately, knowing full well that I would need a bilateral mastectomy. Bi-lateral breast cancer and bi-lateral mastectomy may not have been part of my plan, but living to see my daughter grow into her future is part of my plan. There are no words to describe how blessed I feel to have had the level of medical care and treatment from my surgical and nursing teams; even more grateful for the love and support of my friends and family and for the constant source of inspiration provided by my beloved daughter. There is more to this journey still in front of me but my resolve to fight on has never been stronger. I am indebted to the wise and courageous women who so bravely fight their own battles along this journey; especially one in particular who said to me 15 years ago, “It may sound odd to you now, but you will receive many unexpected gifts from this.” She was right.

As a mother of three children my life was quite full. My schedule seemed impossible. I worked full time at Amazon, managed to take care of my amazing children and my 19 year old nephew, all of whom had hectic school and social schedules and managed to run my household. I was stretched thin, but it was my life, and I loved it. I was diagnosed on April 9th, 2014 at the age of 36 with stage 1A IDC HER2+ breast cancer. I have no family history of breast cancer, so it was a bit of a shock when I found out I had it. I found the lump while performing a self-exam in the shower after having mild pain in my right breast, then noticing an indentation. I underwent a unilateral mastectomy of the right breast May 6th, 2014. It was decided from there that I needed to have chemo and radiation. I went through six rounds of TCH, then an additional 12 rounds of Herceptin, totaling 18 rounds over the span of one year. I also went through 28 rounds of radiation. Reconstruction took place July 15th, 2015!
During this time I realized just how much my life had changed. I didn’t recognize myself in the mirror anymore. I was a mere shadow of the woman I was before breast cancer. It was hard coming to grips with what things are as opposed to what they were. I began to shut down. I began to feel lost and hopeless. Then I went to my first support group meeting and everything seemed clearer. Brighter. From there I got in contact with the ladies from the DBCC and became a peer mentor in June of 2015. I became a peer mentor because I wanted to give a little something back. I wanted to be able to give a woman who feels hopeless hope. I want to have another woman say that because of me she KNEW she would be okay! I want a woman to look at me and say that she didn’t give up because I inspired her, just like the DBCC ladies did for me. I want people to understand that a breast cancer diagnosis is not the end of me, and it is not the definition of me. I am more than my cancer. I am a mother, wife, daughter, sister, aunt, cousin, friend, artist, music lover, and just like every other woman on Earth, yet still uniquely me. I am blessed beyond reason and am grateful for every single moment I am given.

After my yearly mammogram, I got a phone call from my doctor. The year before I had been clear but that year, they saw my entire breastbone was black. Stage 2 breast cancer, they said. Due to the depth of my faith, I did not even cry. I own and run my own television and film agency, I’ve booked Broadway - I thought, I don’t have time for cancer! When I was diagnosed, the doctors insisted I would need full chemotherapy and full radiation but I refused. I would prefer to die healthy than poison myself and waste the life I have left suffering as I’ve seen so many of my friends do. I believe in holistic medicine, not the sort the doctors insisted I needed. I did take the Aromasin chemo pill and although it made me a little tired, I didn’t let that slow me down. We first tried a lumpectomy which is done without anesthesia and was the most painful procedure I’ve ever experienced. Yet seven months later, the cancer still came back. At that point, I realized I needed a double mastectomy. Naturally, they sent me to talk to a plastic surgeon. The reconstruction process he described sounded barbaric and the women I spoke to who went through it insisted it was the most painful experience of their lives and that they regretted it. What’s the big deal about not having breasts? I didn’t want to go through that and I didn’t want people’s pity. I just went to the hospital and had the double mastectomy done. It was my faith and the blessed mother that gave me the strength to get through this. The difference in my story is that I fought the system; I didn’t do what they said. I questioned their recommendations and stuck to my faith and my values. I feel that the cancer industry is a business and patients are viewed as price tags. In a lot of ways they try to take advantage of the fear to make the most money. At least I’m alive and healthy, who should care if I don’t have breasts? Now, five years later, I’m cancer free and my experiences have made my faith in our lady that much stronger. My diagnosis was not a surprise to me. Despite feeling that my body had completely betrayed me, I was not surprised that a routine mammogram, along with an ultrasound came back with suspicious lumps on it. I am the third child of eight children, six girls and two boys. When I first learned of breast cancer, I was in fifth grade when my best friend’s mother was diagnosed at the young age of 37. She lost her battle five years later, but not before my own mother was diagnosed with breast cancer also. My mother had eight children at home, ranging in age from 2 years to 19 years old; and her doctors were not giving her a good prognosis. However, my mother opted to have a complete hysterectomy which may have been what saved her life for another 22 years. Cancer had struck our house again, seven years after my mother’s diagnosis, with my younger brother, Tommy. He was diagnosed with a rare cancer called Ewing’s sarcoma, at the age of 20. My brother lost his battle two years later at the age of 22.
My mother’s cancer had returned 17 years later, after all her children were grown, and she lost her battle two years later. A year following the loss of our mother, my youngest sibling, Amanda, had just been married and was not feeling well, after a visit with the doctor, she was told she had a rare sarcoma also, called Leiomyosarcoma, she was only 26. She lost her battle a year and a half later. My own diagnosis would come shortly before losing my little sister to her cancer. At the time of my diagnosis, my son was only eight and my daughter was five. I had a bilateral mastectomy, followed by chemotherapy, bilateral oophorectomy and a year of Herceptin. Despite being very concerned for my future, I try to stay very active and keep women aware of the importance of self-exams and yearly checkups! I have an amazing husband and friends who kept me moving and staying positive. I coach youth lacrosse and field hockey. I run in the Susan G. Komen 5k every year, I run for those people who I lost, and never take my health for granted.

My breast cancer journey began in 2011 when there was, yet another, mammogram that required follow up films. From the first one I had, it never went smoothly. When this one landed me in “that room” with the couch and a box of tissues awaiting the radiologist, I knew it wasn’t good news. Yet another biopsy would be needed. Having had previous biopsies I was prepared for that procedure but still not prepared to hear the news. It was DCIS, ductal carcinoma in-situ. Being adopted, I had no idea about my family history of breast cancer yet, somehow, with years of ongoing issues with mammograms, I always felt this day would arrive but still I wasn’t ready to hear it.
Being a nurse I immediately began dutifully reading everything I could find to learn about my diagnosis and treatment options. I also worried . . . a lot . . . I am good at that. Meeting with my surgeons helped me realize that they had this under control and dealt with this regularly, they were the experts and that is why I chose them. Once a plan was in place I finally felt a bit more at ease. I was still overwhelmed knowing my life would never be the same and somewhat stunned and angry that it was actually happening to me. Having a bilateral mastectomy with reconstruction was the choice I made after hearing my options. I felt this would give me the most peace of mind going forward. I am thankful for the excellent care I received and realized quickly that I made a better nurse than a patient. At the time of my diagnosis I was working as a registered nurse, teaching a program to high school students about self-breast exams. Getting this diagnosis at this time seemed a cruel irony. Fortunately I was also volunteering my time at the Delaware Breast Cancer Coalitions GreatStuff Savvy Resale store. My support system was right there, unbeknownst to me until I needed them. This sisterhood of fellow volunteers, many of whom are breast cancer survivors themselves, was some of the best medicine I could have found. I continue to volunteer there and truly enjoy helping the DBCC help other cancer survivors. My time is also spent managing my Silpada Designs jewelry business and helping empower other women to do the same. My husband John and I seem to be traveling more, golfing more and enjoying our children and grandchildren as much as possible. I am celebrating my fourth cancerfree year and look forward to many more.

Five-Time Cancer Survivor Gives Back! My name is Jackki Kennedy and I am the founder and president of the MDP Cancer Fund, which hosts “The Kennedy Black & White Ball”. As a five-time breast cancer survivor, and one who has lost two family members to this dreaded disease, I have a strong desire for a cure to be found and to help others who are dealing with cancer. Several years ago I decided to move forward with a vision to raise funds for cancer research and programs. It was at that time that the KBWB was launched. The Ball is an annual black tie fundraiser event given each year at the beginning of November. The proceeds from the event are donated to the Helen F. Graham Cancer Center at Christiana Care, Cancer Special Needs Fund. The Center is one of the leading institutions in the nation in research and care of cancer. Did You Know… Even if your ovaries have been removed, it is possible to develop Primary Peritoneal Cancer (PPC), a rare form of cancer, which is closely related to ovarian cancer. The peritoneum is a thin sheet of tissue that protects and supports the abdominal organs. When certain cells transform into cancerous cells, the result is PPC. The cause is not known. There is no screening for PPC. Symptoms are more gastrointestinal in nature and may include: abdominal bloating, changes in bowel habits, an early feeling of fullness after eating. Nausea and vomiting may occur when bloating is severe. Medical evaluation and treatment is the same as for ovarian cancer and recurrences are common.

Low Interest Loans Offer Help For Independent Living A
s Carol looks around her newly renovated room she can’t help but be thankful that she found Easter Seals loan program that helped create a welcoming, safe and accessible room for her brother who uses a wheelchair. “The room has been everything we hoped for, a nice warm or cool haven with lots of windows and a comfy sofa to entertain guests or sleep someone overnight,” Carol says. “The room is close to the bathroom with no steps to access the room and plenty of space for someone in a wheelchair to navigate around.”

The FYI program is part of Easter Seals Community Outreach Program (COP). With the help of low-interest financing, Carol was able to renovate a previously inaccessible area to make a room built for someone using a wheelchair. It includes lowered light switches and window frames, accessible outlets, floors with no carpeting and a level entry from the house allowing easy access without assistance. “Steve is alone while I am at work and having this room for him makes the house more comfortable, plus with the lowered windows, it is like bringing the outdoors in to him,” Carol says. “This modification gives him more space to enjoy.” The loan program is made possible through lending partners Del-One Federal Credit Union and DEXSTA

New UD Program Focuses On Fitness Training For Over 55 By Nora Riehl Zelluk The youngest baby boomers turned 50 last year. Photo by Getty Images Committed to healthy aging like never before, older adults have become a fastgrowing segment of the fitness population. Most health clubs and fitness trainers welcome the trend with open arms, but many do not know how to handle their changing clientele. “The baby boomer generation has a completely different motivation than the mainstream fitness industry recognizes,” says fitness expert Jim Menz, faculty member and program director for a new program training fitness specialists to work with older adults. “Rather than cosmetic results, folks over 55 are thinking more about their health, living longer and enjoying a great quality of life.” The University of Delaware’s new Senior Fitness Training Specialist Certificate was created with a variety of fitness and health care professionals in mind, preparing them to work as personal fitness trainers to clients over

ith the College Board’s impending debut of a completely redesigned SAT, college-bound students and parents everywhere are concerned about how to prepare for college entrance exams.The best study tip for students is to know one’s own specific needs. Effective exam prep is based on individual student needs. Huntington exam prep programs are built on the understanding that every student approaches exams differently and has different skills. A goal of ours is to get students familiar with an exam, but we also tell our students the same thing today that we always have: the skills they need to perform well on these exams are the same skills they need to succeed in college.
Test prep tips for students taking college entrance exams: • Start with baseline information. To understand what and how to study, students must know exactly where they have gaps in knowledge and skills. A diagnostic assessment is the best way to measure a student’s areas of strength and weakness. Any test prep study plan should be built around the areas that need the most attention.

In the fall, a wide variety of pests begin invading homes because of environmental cues such as shorter and cooler days. In addition, less food becomes available outside. Pests start by seeking shelter in cracks and crevices, gradually following these openings inside until they often appear in our living and storage areas. Although a few of these pests may become immediately noticeable; many lie dormant until awakened by a warmer period from fall to early spring. Interior lights, which are mistaken for a bright spring day, draw pests deeper into your home. One of the newer pests to invade homes in the fall is the stink bug. Others that continue to be serious fall invaders include fleas, ladybugs, cluster flies, yellow jackets and clover mites. Some pests invade year-round, but pressure increases in the fall. These include rats, mice, crickets, earwigs, ants, millipedes, paper wasps, and many spiders. Sac spiders are fall invaders that can deliver a painful bite. (The pain feels like a bee sting, and lasts for 1-2 hours.) Wild animals such as raccoons and squirrels can gain entry into crawl spaces, basements, and attics seeking a protected place to spend the winter. This is a busy time of the year for these invading pests. It is important that we stay ahead of them to prevent serious pest problems. t’s probably the toughest six letter word to ever hear... Cancer. It is something that is diagnosed and something that we have to deal with. How you and your loved ones deal with it is the burning question. It can take a mental and physical toll on you and your family. While we are all hoping that research can quickly find a cure for Cancer, there are some steps that you can take to help the fight now. Research indicates that physical activity after a diagnosis of breast cancer may be beneficial in improving quality of life, reducing fatigue and assisting with energy balance. Both reduced physical activity and the side effects of treatment have been linked to weight gain after a breast cancer diagnosis. One study found that women who exercised moderately (the equivalent of walking 3 to 5 hours per week at an average
pace) after a diagnosis of breast cancer had improved survival rates compared with more sedentary women. The benefit was particularly pronounced in women with hormone responsive tumors. Another study found that a homebased physical activity program had a beneficial effect on the fitness and psychological well-being of previously sedentary women who had completed treatment for early-stage through stage II breast cancer. That’s some straight science for you, but I know that every person is built differently. I can tell you that in my 27 years of personal training, I have met and trained many people with breast cancer and/or ovarian cancer diagnosis and one common theme came to light.
Being fitter and feeling stronger gave them more confidence about themselves. Try weight bearing exercises to firm your muscles and cardio activity to keep your heart strong. A strong soldier is a fighting soldier! Everything starts with the mind. Get in the mindset to fight and not to succumb to this beast of Cancer. It takes a fit entourage to help you in this journey so not only should the person diagnosed get fit but family members and friends should as well. Make your team a strong team! n our body image obsessed culture, America has largely turned yoga into a fitness craze with cursory namastes and the requisite buddha statue. Every day I see students who pursue a vigorous practice that is not serving their greatest good. It’s time to reevaluate your ideal life and see how your current yoga practice fits into that image, making adjustments as needed. 1. Check Your Asana Ego
Complex twists, balances and inversions are beautiful, but they are not right for everyone. The ultimate goal of yoga is to understand the body and the mind, using your practice as a tool to explore consciousness. Ask yourself why you are pursuing those difficult poses and for whom you are doing them. The benefit of a pose does not have a direct relationship with its skill or experience level.
2. Balance = Health
If you life a face-paced life, juggling career, relationship, children and a slew of other obligations, you may be better suited to a Yin or Restorative style class. Adding a fastpaced, high energy practice to an already stressed life risks burnout and dis-ease. No one is immune to the dynamic laws of spirit that seek to create a state of equilibrium.
3. Yoga Without Spirit Is Just Calisthenics
Chanting OM, setting intentions and adding meditation are vital to a beneficial yoga practice. The most difficult yoga class I ever took was a series of very basic asana,

Survivors face many physical challenges after undergoing breast cancer surgery and treatment, which are often compounded by the emotional stress that results from living with a life-threatening condition. The Pilates method is a gentle form of exercise that engages the mind, body and spirit. The various Pilates exercises help to strengthen muscles, increase flexibility while increasing metabolism and promoting lymphatic, respiratory,

Women undergoing breast cancer treatment often find themselves facing overwhelming fatigue, especially when undergoing chemotherapy or radiation. To perform Pilates exercises correctly you have to concentrate and have mental focus, but this intense concentration will pay off. Pilates offers a gentle low impact reintroduction or first time introduction to regular exercise that can help one regain strength and endurance.

Lymphedema
Many women who have had lymph nodes removed as part of their breast cancer treatment have a high risk of developing lymphedema. Women at risk for lymphedema can exercise, but it is important to strengthen the arms gradually. Because many Pilates exercises are abdominal, they are a natural fit for women concerned about lymphedema. In addition, the muscular contractions from the abdominal exercises performed in a specific sequence help propel lymphatic fluid into an area where it can more easily drain. The emphasis on deep rib cage breathing and the multidimensional breathing patterns helps ease tension, encourage lymphatic drainage, and stretch tight areas affected by scars.

6. Getting double use of bouquets and arrangements can significantly control your costs. Bridesmaid’s bouquets can be used to decorate either the head table, the seating card table or a mantel. Ceremony flowers transported to the reception site can be placed on the food service tables, on the bar or to frame the band.
I found Barb and Belak by reading other reviews and recommendations and I have to say, all those other ladies were right! Barb was beyond awesome! 2. Be sure that your florist knows your budget. If you have daisies in mind and she is considering orchids, or vice versa, you’ll need to communicate what blooms fit your budget. 3. Using readily available, durable flowers in season helps to keep the bouquet cost down. If you choose fragile flowers it often means that the florist has to order a surplus in order to have enough to fulfill the wedding order. 4. Not needing bouquets that are 100% blooms helps to keep costs down. Foliage and some “filler” flowers take up room in a bouquet while still providing the color impact that you desire. 5. Flowers with large blooms mean you will need less overall and thus keep the cost down.
8. Consider how much space is actually available in the center of each reception table: most often, once they are set there is less room than you may have envisioned for the centerpieces. 9. If you have a favorite flower that is more expensive, reserve it for you and your groom. It does not have to be in every bouquet and arrangement to be remembered. 10. Use smaller containers that you may already have for cocktail hour arrangements. Single vases, your Mother’s tea cups, or even flea market finds can reduce your budget.

Many women dream of having a larger bust size. They yearn to turn heads while they walk down the street or comfortably fill out a lacy bra or bustier, like the models from “Vickies”. Perhaps this is the reason that breast augmentation is one of the most popular cosmetic surgical procedures year after year. But, as any “busty” woman will tell you, having a larger bust has its downsides and can even be painful. What’s a woman to do? Let’s take a look at some of the more common problems and solutions.
Bouncing
During exercise, breasts do bounce, sometimes a lot. In order to contain the “girls”, women resort to wearing multiple bras. Layered sports bras don’t give constructive support and they can be uncomfortable. Not to mention all that compression often leads to the dreaded uni-boob.

My Nana was a fabulous woman known for her vivacious style and magnificent hairdos. She was a pillar of strength, raised three boys on a shoestring budget, and was always dressed to the nines. I watched as she slowly lost her hair, her bubbling personality, and her sense of self. The less pretty she felt the less she would leave the house and eventually the couch became her permanent bed. It’s easy to underestimate how important it is to feel beautiful unless you see first hand what happens when someone loses their self confidence. Nowadays, breast cancer treatments are evolving to fight back better and so are beauty treatments.

Understanding the cost: Most people have no idea what is involved with a bathroom remodel. There are many trades involved in the process including: demolition, carpentry, electric, plumbing, tile installation, countertop fabrication, glass fabrication, painting and if needed, a heating and air conditioning installer. A simple faucet can range from $50 to $3000 (and no one ever chooses the $50 faucet). Most average hall bathrooms with a tub/shower combination and double sinks cost between $20,000 - $27,000. This is with mid-priced materials.

Purchasing products from the internet: This rarely ends well. Homeowners often try to reduce costs by supplying products themselves purchased on the internet which typically are lower end brands, fixtures made overseas with plastic parts and odd sizes. Many name brand products purchased online aren’t the same as buying from a dealer. Many times the products are returns or they don’t pass quality control for whatever reason. Parts can be missing or the products come damaged. All of these things can hold up a job for weeks and can create unnecessary expense. It simply is not worth the risk.

Reusing existing materials: The idea of saving money on remodeling a bathroom is a great concept. Reusing materials is NOT the way to do it. There are some exceptions like soaking tubs and mirrors that don’t have working parts to fail, however, you can and should always replace the drain of a soaking tub being reused. Reusing toilets, faucets or whirlpool tubs is like playing with fire. There is no way to guarantee that they will not leak or work properly after being disconnected and reconnected. Whirlpools should never be reused, period. Unfortunately, you won’t know until it’s too late if there are problems. There are no warranties on used materials either. This usually

Trying to design and run the job yourself: There is a reason why we all have different skills and talents (for example I will never cut my hair again). Construction is very involved and if you don’t know what you are doing, it will be problematic and could delay your project for months. Using a full service bathroom remodeling company will eliminate many issues. We understand how to design properly for the space limitations in your home to eliminate common mistakes such as: cabinetry and/or tubs not fitting correctly, tile quantities miscalculated and incorrectly ordering plumbing valves. Trying to organize six different tradesmen to do all of the installation work is not as easy as it sounds. A full service company ensures proper materials are ordered and that all of the tradesmen are scheduled and properly prepared, insured, etc. We give you a start date and a completion date at the beginning of the project.
