Major Advance In Laser Vision Correction Technology – Now Available In Delaware I

nnovations in laser eye surgery have enhanced accuracy and safety, while improving comfort and convenience, and at the same time lowering costs. I have been fortunate to participate in this continued evolution, having been involved in laser vision correction research By and patient care Jeffrey B. Minkovitz, M.D. since my training. As a co-investigator in the FDA clinical trial for the first refractive lasers in the early 1990s, I was thrilled to be part of the transition from the effective but occasionally unpredictable radial keratotomy to fast and precise laser surgery. Since then, new techniques have further improved outcomes and reduced side effects. The range of possible treatments has dramatically expanded, from low levels of nearsightedness to much stronger prescriptions, as well as farsightedness and astigmatism. Lasers have also become more sophisticated, with more innovative beam profiles and treatment patterns, and enhanced ability to “create” the ideal naturally shaped cornea. But while existing lasers have improved, change has been marked more by evolution than revolution. In fact, until just recently, one of the first lasers to achieve FDA approval in 1995 has remained the industry leader (and the only platform available in Delaware) for nearly 20 years, long by any technology standard. Recently, a leading ophthalmic device manufacturer has emerged as the new laser technology leader. The Alcon Wavelight Allegretto offers improved physician and patient ergonomics, faster treatments and more frequent monitoring of eye position and laser beam energy, and study results reveal better outcomes. Wavefront – optimized to “create the most natural corneal shape”, this laser is also less dependent upon pupil size than other laser platforms. Operating at 400 pulses per second, this laser is nearly 10 times faster than previous lasers (most operating at 60 pulses per second). Such speed reduces patient anxiety (treatments lasting a few seconds are much easier for patients to fixate on a target) and reduces environmental variables including surface drying, which affect outcomes. Faster treatments require more accuracy, which the laser provides by verifying eye position before each pulse, and making adjustments within 6 milliseconds, much faster than any other laser.

Each pulse is also analyzed 3 times before delivery to the eye, and energy is automatically adjusted as needed (a technology termed “Perfect Pulse”). Sealed beam pathways, unlike other lasers, reduce environmental factors, allowing broader tolerances of room temperature and humidity. What all this means is more accurate and predictable results and a more pleasant treatment. Numerous studies agree that results are superior (including studies at University of Utah and at Stanford in 2014) with a higher percentage of patients achieving 20/20 and 20/16 vision, and good vision achieved earlier. Results presented for FDA approval also showed superior outcomes, with 92.7% of patients achieving 20/20 vision and 68.8% achieving 20/16, also significantly better than results by the previous industry leader. As with any technology, continued research and innovation provide advances which result in better functionality, often at a lower cost. In medicine, that means a better patient experience and most importantly better outcomes. Just as I was excited to introduce laser vision correction to Delaware in 1995 (those pioneering patients had to travel with me out of state before Delaware had its own laser), I am enthusiastic about ushering in this next phase in vision correction. Laser Vision Delaware will make the Allegretto available to other Delaware eye surgeons as well, in order to provide all patients with the latest technology and best visual outcomes. Jeffrey Minkovitz, MD is an ophthalmologist with Eye Physicians and Surgeons in Wilmington, Delaware. Dr. Minkovitz specializes in cataract and refractive surgery, and is one of only a few Delaware surgeons specially trained in corneal transplantation.

He completed a cornea and refractive surgery fellowship at the renowned Wilmer Eye Institute of Johns Hopkins. Previously, he graduated magna cum-laude from Harvard, earned his medical degree from the University of Massachusetts, completed internship at Harvard’s Mt. Auburn Hospital, and served his ophthalmology residency at Washington University. Dr. Minkovitz participated in the early research on Excimer Laser refractive surgery, and has offered this procedure to Delawareans since FDA approval in 1996, before Delaware had its own laser. These early pioneering individuals travelled to Wilmer Institute with Dr. Minkovitz, where he maintained his teaching and clinical appointment. In addition to introducing laser vision correction to Delaware, he also introduced NearVision CK to eliminate reading glasses, DSAEK (a newer, less invasive type of corneal transplant) and recently the newest advance in cataract surgery: laser assisted cataract surgery. In 2016, Dr. Minkovitz founded Laser Vision Delaware, llc and brought the newest and most advanced refractive laser to Delaware. Dr. Minkovitz is a co-founder of the Center for Advanced Surgical Arts and serves on its medical advisory board. He is also on active staff at Christiana Care. While on staff of Wilmer at Johns Hopkins University, Dr. Minkovitz received the Outstanding Teacher award. He was also named Top Doc in ophthalmology by Delaware Today Magazine six consecutive years, and has been an invited instructor at American Academy of Ophthalmology Annual Meetings.

Welcome to Summer, and the 3rd Quarter of The Women’s Journal Publication. Though it has been raining continuously, and the rabbits have had a field day with my raised vegetable garden, I am very thankful that at least they have left my beautiful lavender alone. Funny part is I keep telling myself at least I don’t have to water my plants, but then I look at most of them and they are actually rotting in the ground, can I say... that just isn’t right! Thus my next prayer will be, can we have just a little less rain and a lot more sunshine through October. Nothing like being greedy, and asking for more than you should. Summer is here, and we should all celebrate just being alive. I hope that everyone had a wonderful 4th of July, and you were able to enjoy your family and friends, celebrating our freedom. Being an election year, we should all take time to review and educate ourselves on our candidates and make the best decision for our country. It is hard to complain about what our politicians are doing if we do not go out and vote, so make sure that you are registered to vote, and be prepared to take a stand for what you feel is right. Our freedom was not gained without sacrifice and certainly will not continue, if we all do not stay involved.

Laryngology: If You Have A Voice Problem, Speak Up! T

he voice is perhaps the most powerful method of human By Dr. Jonathan Romak, MD communication and emotional expression. For most of us, the voice is vital for connection with friends and loved ones, and it is a professional necessity for many. Unfortunately, people often don’t realize the true importance of their voice until it fails them. When voice problems do occur, many people do not know where to turn or what, if anything, can be done to help. Laryngology is a subspecialty of otolaryngology (ear, nose, and throat) that focuses on the structure and function of the larynx (voice box). Laryngologists are experts in the treatment of voice, swallowing and airway disorders. The only voice patients many people are aware of are high profile singers such as Julie Andrews and Adele. However, voice disorders are very common in the general population; particularly among professional voice users, and in people over the age of 65. The majority of professional voice users are not professional singers – they’re teachers, religious leaders, attorneys, salespeople, business executives, servers, actors, telephone operators and many others. In fact, approximately 30% of the working population in the United States are considered professional voice users. The portion of our population made up of those 65 years of age and over is rapidly increasing. Currently, there are nearly 42 million adults in this age group, and that number is expected to nearly double by the year 2030, which will represent 20% of the total population. Several studies have examined the prevalence of voice disorders in older individuals, with estimates ranging from 12% to 47%. Many older individuals do not seek care for voice problems because they assume it is a natural part of the aging process. However, research has shown that nearly all elderly patients with voice disorders have another disease process affecting the voice in addition to age related changes. Voice difficulties have been shown in many studies to have a negative effect on quality of life. Conversely, treating a voice problem has been shown to significantly increase quality of life through improved physical, social and emotional health. Voice production is an extremely complex process involving input and output from multiple organ systems within the body. Therefore, voice problems may have diverse causes including not only the condition of the vocal cords themselves (polyps, nodules, tumors, weakness, inflammation), but also lung disease, medication side effects, neurologic disorders and reduced physical conditioning, among others. Treatment of voice disorders depends on the underlying cause and is therefore equally diverse. www.TheWomensJournal.com

Voice therapy, medical treatment and surgery are all potential options. Surgical intervention is usually recommended only as a last resort. Voice surgery is most often a same-day surgery (no overnight hospital stay), and can sometimes even be performed in the office setting. Laryngology is a relatively new and exciting field of medicine with the opportunity to dramatically impact patients’ quality of life and well being. If you or a loved one are suffering with a voice problem, speak up! The solution may be closer, quicker and easier than you think.

Relief For Overactive Bladder

Without Medications Or Surgery!

Tibial nerve stimulation helps women to reclaim active lives

A

bout half of all women experience stress incontinence -- losing urine when they cough, laugh, sneeze or exercise -- at some point in life. It’s a common problem in women of all ages, starting in their 20s. Many women tell us stress incontinence limits their ability to work out or enjoy an active social life. Many women suffer from overactive bladder, or OAB, in which they need to urinate frequently or have a sudden, urgent need to go to the bathroom. In fact, about 40 percent of women in the United States have OAB, making more than eight trips to the bathroom during their waking hours and often getting up at night. There are several methods to treat the

problem, including medications and behavior changes that can help. One effective option is a nonsurgical treatment called tibial nerve stimulation. This option often works when medications and other treatments have not provided relief.

How does tibial nerve stimulation work?

Essentially, it’s an acupuncture needle inserted into the skin to stimulate the anterior tibial nerve on the outside part of the ankle. Most people say it hurts less than a bug bite. Typically, patients get weekly treatments for 30 minutes for 12 weeks. Some patients might require treatment twice a week—the schedule can vary based on how each individual responds.

Improvement in five weeks

Most women see improvement by the fifth week, as their OAB symptoms decline. For people who have been struggling with the discomfort and challenges of OAB, the change marks a big improvement in quality of life. Benefits of the treatment include using the bathroom less frequently especially in the middle of the night, getting a better night’s sleep and feeling more rested. In terms of urgency, patients say they have more time to get to the bathroom, which reduces their anxiety about leaking urine. Further, people who respond well to tibial nerve stimulation can be gradually weaned off of

Safe and effective

Although many people aren’t familiar with it, tibial nerve stimulation is not an experimental treatment. We’ve been doing the procedure for six years, and it’s been around for more than 10 years, starting in Europe – and the success rate to cure or improve OAB is about 75 percent. Even though it is effective, tibial nerve stimulation is not permanent. The positive impact lasts nine months to 18 months. Happily, the neurological system has a good memory, so the response is much quicker the second time around, likely in four weeks.

It is Never Too Late

A more beautiful and healthy smile could be just months away M any people still feel orthodontic treatment, (traditional braces and clear aligners), are for adolescents. Not true. Many adults benefit from having their teeth moved to a more ideal position.

Straighter teeth don’t just look better, they work better too. Poorly aligned teeth can interfere with bite function, wear out quicker, and are more prone to cavities and gum disease. them, each aligner moving your teeth just a little bit at a time. No wires, no brackets, just clear, convenient comfort.

The most common procedure I perform in adult patients is eliminating crowding of the lower front teeth. The natural movement of these teeth is to the front of the mouth. It is thought that this is to compensate for the natural wear between teeth throughout life. When teeth are ideally aligned the movement is not perceptible. If a tooth is out of position, however, the forward movement allows teeth to slip past each other, progressively becoming more crowded. Many adults with crowded front teeth had braces in their youth and “relapsed”. We now know much more about the forces that will cause relapse and how to protect against them.

The aligner shown here is nearly invisible and so thin it is easy to wear. Some of the patients who have worn these retainers to straighten their teeth are teachers, lawyers, lecturers, and even speech pathologists. Wearing the aligners did not interfere with their everyday activities. Their spouse, co-workers and friends did not even know they were having orthodontic treatment. We are happy to offer financing and submit to insurance to make this type of treatment possible for our patients.

Another relatively simple correction is to upright tipped teeth. The uprighting allows better cleaning of the formerly tipped tooth and delivers the forces of biting down the long axis of the tooth which is how the natural shock absorbing system of ligaments works best. Many times teeth are tipped because the tooth next to them was removed or congenitally missing. The tooth next to the space tips into the void. Uprighting allows us to place an appropriate replacement for the missing tooth.

We use orthodontic tooth movement to enhance our cosmetic and functional outcomes. When teeth are too narrow for the arch they occupy, spaces occur. Childhood Congenital Myopathies Do You Know What It Is?

every day, most myopathies do not affect the most important aspects of the brain: children with muscle disease can think, hope, imagine and dream of a world in which there are better treatments and even cures for muscle disease. At the present time however, there is no definitive treatment for any of the common childhood myopathies. By Aaron Chidekel, MD

e all take movement for granted: walking from place to place, scratching an uncomfortable itch or shooing a fly. Even the most basic things that happen without us thinking rely on our muscles functioning normally and with sufficient strength: eating, drinking, and most importantly, breathing. For individuals living with muscle diseases, these basic functions can be complicated, impaired or even impossible. Childhood myopathies, diseases that affect the muscles of children, are a diverse and challenging group of genetic diseases that are much more common than expected. Childhood muscle diseases are present from birth or become apparent early in childhood and involve various defects in the structure, function or survival of muscle cells or the nerve cells that control them. This makes these conditions very different from traumatic paralysis, stroke, or even Amyotrophic Lateral Sclerosis (ALS) or Multiple Sclerosis (MS). Yet, despite the severe functional limitations that children with muscle disease live with every moment of

One of the most common congenital myopathies is called Spinal Muscular Atrophy or SMA. SMA affects roughly 1 in 10,000 babies and occurs in all races and both sexes. SMA is caused by a specific mutation in a gene that encodes a protein that is critical for the survival of the nerve cells that control the skeletal muscles; muscles that are responsible for the movements we all take for granted. In SMA, the lack of a functional Survival Motor Neuron Gene (SMN1) causes these motor nerve cells to die and muscle function is lost. There are four types of SMA: I, II, III and IV. Type I SMA is the most severe whereas Type IV is the mildest, based upon the severity of the weakness experienced by the child. SMA is the most common genetic cause of death of infants. Infants with Type I SMA become extremely weak very early in life but develop normally cognitively. Infants with SMA lose the ability to move their limbs, roll over and eventually they lose the ability to swallow and even breathe. Without intensive

supportive medical and particularly respiratory or breathing therapy, infants with Type I SMA will die from pneumonia or respiratory failure and many do. However, some families opt for a plan of aggressive supportive care for their children. Working with these children and their families is a joyful and meaningful challenge. Meticulous care and monitoring are needed to maintain health and safety in the face of such enormous physical challenges and functional limitations. But looking beyond the doctors, the nurses, the respiratory and physical therapists, the medications, machines and devices that are required to maintain the health and safety of children with SMA, one sees the life and spirit embodied in the hearts and minds of these special children and their families. Watching them learn, grow and develop so much like mobile kids, and yet so differently, makes us remember how much the same we all are and how important it is to realize that it is truly what is on the inside, in the heart, mind and soul that makes us the people we are, and not necessarily our physical strength or capabilities. All children with muscle diseases are special people, no matter what the cause of the underlying myopathy and we all can share in their hopes and dreams of a world with better treatments and cures for all of their muscle diseases. 11

ow that we have officially welcomed summer, make sure you’re taking the right steps to be an active participant in the activities that come with it! Activities such as walks along the beach, road trips, and backyard barbecues should be fun events in the summer. However, for people with hearing loss, these events could be stressful because of these challenging listening environments. Did you know that new hearing aid technology offers features which allow people with hearing loss the ability to enjoy the sounds of summer without compromising their ability to communicate? The new technology also focuses on reducing background sounds such as wind and party noise that could be bothersome.

Background Noise

It is normal to hear all the sounds around us. Hearing technology is designed to focus on audibility of speech to give you the best opportunity to communicate normally. Not all background sounds can be completely filtered out, nor should they be. New noise reduction features allow hearing aid users the ability to experience comfort while experiencing an improved speech signal from the person of interest. So enjoy that backyard barbecue or intimate conversation at your favorite restaurants. How is this possible? Digital hearing aid technology can control background noise in a variety of different ways while adapting to each new environment. Many new, more sophisticated digital hearing devices can do all this in the startup automatic program leaving you free to enjoy your day without the need to make manual adjustments. Automatic features can adjust the hearing aid microphones to enhance speech audibility while reducing background noise differently for different listening environments.

For example, at a party, the microphones may be directed toward the person you are listening to while decreasing the volume of the conversations beside and behind you. Additional “behind the scenes” noise reduction features also work in conjunction with the automatic program. These features specifically look at very soft noise (i.e. hum of the air conditioner) and impulse sounds (i.e. dishes clanging). All of the noise reduction features in the hearing aids can be applied accordingly to optimize the speech signal while also improving listening comfort in most noisy situations.

Wind Noise

Hearing in the presence of wind noise can be particularly challenging, especially for hearing aid users. New hearing aid technology is more accommodating for people who are in the presence of wind noise. Wind noise reduction features are designed to handle wind from all angles. The hearing aids automatically analyze where the wind is coming from and then significantly reduces it. This allows for improved understanding, and better sound quality while in windy conditions. More importantly, it reduces the chance that hearing aid users will need to remove their hearing aids when they are outdoors!

Car Road trips can be a fun event during the summer! However, if you have a difficult time understanding speech in the car this seemingly fun event can become an isolating experience. Why Spider Vein Therapy Doesn’t Always Work treatments depending on how bad her spider veins are. Mary decides to have this painful and expensive procedure done, only to notice months later her spider veins have returned. Unfortunately, this is a more common scenario than one might realize.

s a vein center, we see many cases each year of people who are disappointed in the treatment they received for their spider veins. The problem is that most people do not understand what spider veins are or why they get them. Here is an example we have seen working with patients with spider veins: Mary Smith is a 43 year old female who notices she has several spider veins on her legs. She finds them uncomfortable and cosmetically unattractive, so she goes to a center which specializes in treating cosmetic spider veins. She is told that for $300, the spider veins can be injected and they will resolve. This treatment method is called sclerotherapy. Mary may need to have multiple sclerotherapy

At the Delaware Advanced Vein Center, we see several patients a week who present us with this scenario. The optimal evaluation of a patient with spider veins is a thorough history and physical examination, for venous insufficiency. To diagnose whether Mary has venous insufficiency, she would schedule a vascular ultrasound study done on her legs to measure the vein size and determine if her veins are adequately circulating blood from her lower extremities to her heart. The legs provide the heart 70% of the blood that it receives so the direction of the blood always flows towards the heart.

Thus, spider veins may be one of the earliest signs of venous insufficiency. Saving Vision - The Importance Of Early Care By A Retina Specialist A

ge-related Macular Degeneration (AMD) and diabetic retinopathy are the most common causes of vision loss and blindness in the United States and our goal is to save vision in these patients. In 2012 the American Society of Retina Specialists launched Retina 20/20. This is a program to raise awareness about potentially blinding eye diseases that can be prevented if caught early and treated. AMD and diabetic retinopathy are two such eye diseases that need greater recognition by our communities. AMD An estimated 15 million Americans have AgeRelated Macular Degeneration and it is the leading cause of blindness among adults over the age of 65. The risk of developing AMD over the age of 75 is 30%. Left untreated, AMD would account for 50% of the blindness in the United States. As people age, some develop yellow deposits beneath the retina called drusen. As the drusen increase in size and number, they can interfere with the normal functioning of the retina, destroying the vision cells. This form of macular degeneration is known as dry AMD. 80% of people with AMD have dry disease. Wet macular degeneration occurs when new blood vessels grow beneath the retina. These vessels bleed and leak fluid damaging vital retinal nerve cells. While most people have dry AMD, wet AMD accounts for 80-90% of legal blindness associated with this disease. Symptoms of Dry AMD • Need for brighter light when reading • Difficulty adapting to low light levels • Increased blurriness of printed words • Decrease in brightness of colors • Blurred or blank spot in the center of field of vision Symptoms of Wet AMD • Abrupt decline in central vision • Visual distortions (straight lines appearing wavy, objects appearing larger or smaller than they are) • Well defined blind spot in the center of vision Risk factors for age-related macular degeneration: age, family history of AMD, female gender, people of European descent, cigarette smoking, obesity, hypertension, excessive sun exposure, diet deficient in fruits and vegetables. Diabetic Retinopathy Diabetic retinopathy is a growing public health concern. If detected early in combination with good control of the underlying medical disease, most vision impairment can be prevented. Unfortunately 37% of patients who require screening for diabetic retinopathy do not get it. There is a tremendous lost opportunity at preventing vision loss in this patient population. Diabetic retinopathy occurs in more than half of the people who develop diabetes. The longer someone has diabetes or poor control of blood sugar levels increases the risk of developing diabetic retinopathy. There are two general causes of vision loss from diabetic retinopathy: diabetic macular edema and proliferative diabetic retinopathy. Diabetic macular edema is swelling or leakage in the center of the retina (macula). Vision loss occurs when the retina tissue dies from poor blood supply and impaired delivery of oxygen through the swollen retina. Proliferative diabetic retinopathy occurs when new blood vessels grow in the eye that bleed and cause scar tissue formation. The eye can fill with blood (vitreous hemorrhage) or the scar tissue can cause a

retinal detachment leading to vision loss. It is possible to have diabetic retinopathy for a long time without noticing symptoms until substantial damage has occurred. Diabetic retinopathy typically affects both eyes. Symptoms of Diabetic Retinopathy • Blurred vision • Difficulty reading • Spots or floaters in vision • Partial or total loss of vision Risk factors for developing diabetic retinopathy: disease duration, poor control of blood sugar levels, high blood pressure, high cholesterol, pregnancy in someone with diabetes. Today patients with AMD or diabetic retinopathy have more treatment options than ever before at preventing progression of the disease. But first, we have to find the patients and intervene early in the disease before there is irreversible damage done to retinal nerve cells. Regular dilated examinations and prompt referral to a retina specialist if appropriate is critical in our goal of preventing blindness. Patients need to be evaluated and identified before they have significant visual symptoms as it may be too late to restore vision in advanced disease. ery few people are aware of how much they use their teeth on a daily basis. It is not until an individual begins to lose teeth and their oral health and quality of life declines that they begin to understand the impact of impaired oral function on their daily life. Patients with missing and/ or failing teeth suffer from malnutrition, reduced self esteem, and self confidence. They possess a high risk for developing oral conditions such as boney atrophy. Patients also suffer from periodontal disease, cavities, root canal infections, and serious bite problems called “Bite Collapse”. In addition to serious dental conditions, patients with severely failing dentitions can also be more at risk for serious medical conditions such as: cardiovascular problems, stroke and diabetes, complications if pregnant, with a high risk of preterm and low birth weight babies. There are several solutions for individuals with failing dentition. The typical sequence of therapy to address these complex dental conditions could require at least three surgeries. First, the teeth are extracted and the bone is allowed to heal. Patients will temporarily function in a traditional upper or lower denture, which at times impacts the patient’s diet choices. Next, the implants are placed and require a healing time of three to four months. A third surgery is done to expose the implants. The start to finish time is six to eight months. 7 Things To Consider When Preparing For Your Birth By Diana Spalding

W

e hear the term birth plan a lot. Giving birth is one of the most intense, intimate things we do, so the desire to make a birth plan is natural; plans make us feel safe. But the problem with birth plans is that they are, well, plans. Our bodies (and babies) are really good at birth, but just like any force in nature, birth is unpredictable. Instead of writing a birth plan, write your birth preferences. A plan details the step-by-step itinerary of your birth while your birth preferences lets your caregiver know what’s important to you, and allows you to be an active participant in your care as your unique birth story unfolds. Here are some things to consider when writing your birth preferences:

Where do you want to give birth?

A hospital? A birth-center? Your home? Consider this early in your pregnancy as it will help you determine who your caregiver should be. Do you have a partner, and 2. will s/he be there with you? What

about family members? Will you hire a doula? Think about who will have a calming, supportive presence. Remember that birthing places have different policies about the number of guests allowed so do be sure to ask.

How do you want to care for your baby?

Do you have traditional and cultural considerations?

Do you want the baby placed on your belly immediately after birth? Delayed cord-clamping? Baby rooming in with you? Circumcision? Breastfeeding? It’s overwhelming to think about, I know. But it’s even more overwhelming right after you’ve given birth. Consider taking a newborn care class to help you learn about your options. Or, pick 1 topic per week to research and think about what feels right. Then let your provider know.

Who is going to cut the umbilical cord? Do you want to take your placenta home? Are you banking your cord blood? Are there religious traditions the team should be aware of? It’s helpful to communicate these preferences before the baby is born and you are too flooded with emotion to think about them.

Is there anything else we need to know?

Sometimes parts of our histories can have a profound effect on how we approach intense experiences like birth. For example, women that have experienced violence

often report that vaginal exams feel invasive and upsetting. Please don’t feel like you have to tell your provider anything. But do think about the important stuff that could have a true impact on your experience. This is your birth. Your midwife or doctor will serve as your guardian of safety, but you have every right to let them know about what is important to you. Give Blood & Win Big!

pring and summer... time to step outside and commune with nature, neighbors, friends and family. For many, it’s the best time of the year. But while we enjoy the great outdoors there’s a large segment of our region’s population who won’t experience those happy times because they’ll find themselves in a situation where they need blood! Blood recipients come in all shapes, sizes, colors and ages and their needs are unique. Some are accident or trauma victims and need that lifesaving gift of blood to replenish what they’ve lost in an emergency situation and THEY NEED IT IMMEDIATELY! Some are afflicted with cancer, particularly leukemia, and may need multiple transfusions of red blood cells and platelets. Sickle cell patients, principally with severe complications, can use up to 15 units of red blood cells to treat their condition. These few examples illustrate that there are hundreds of local residents on a weekly basis who need blood. In the Delmarva region alone, over 20,000 patients a year are saved by the precious gift of life given by dedicated Blood Bank of Delmarva (BBD) donors. Keeping that supply at a safe level requires the dedication of thousands of donors annually, who selflessly give of their time and of themselves to ensure the blood supply is sufficient for whatever may occur. Here’s another fact you may not know, 65% of the US population cannot donate blood! And, of the 35% who qualify, approximately 5% actually do. It’s a very small percentage of people who actually donate and those numbers are dwindling rapidly as many of our most dedicated donors leave the donor pool due to age or health issues. To complicate matters, younger donors aren’t coming forward to replace those who have been donating for years. There is still no substitute for human blood. The 20,000 local patients we serve each year would have serious issues if it weren’t for the dedicated individuals who answer the call and continue to donate. Easter Seals and undergo thorough background checks, are bonded, insured and have current First Aid/CPR training. The PAS program puts you in charge as the employer. You are able to hire family or friends as your attendant. You decide how much they are paid, when they come and how to do things. Easter Seals will assist in any technical aspects of being an employer. You must qualify for Medicaid funding to be eligible for this program.

ometimes the “golden years” do not feel very golden. In fact, dealing with the many ailments can feel overwhelming at times. That is where Easter Seals Senior Services steps in to lend a helping hand to ease your way gracefully through the golden years. Easter Seals has many types of services to help seniors remain independent and safe living at home. The In-Home Services program and Personal Attendant Services (PAS) are available to a broad population including seniors, who could benefit by having a “caring partner” or an assistant help with daily needs. Both services provide assistance in areas, such as: companionship, meal preparation, light housekeeping, performing work tasks and bathing needs. It is easy to get started with In-Home Services. Each person’s needs are different so Easter Seals offers a free, confidential in-home evaluation. This meeting will establish an individual care plan as well as address: routines, health needs, cost of services and transportation requirements. What You Should Know About Skin Cancer

S

kin cancer may initially appear as a bump, nodule or irregular patch on the surface of the skin. As the cancer grows, the shape of the skin mass can change. As the changes are occurring the cancer is likely expanding and moving into the lower layer of the skin. Most suspected skin cancers are often first noticed by the patient or a family member before they are brought to the attention of a doctor.

There are 3 basic types of skin cancer and some are more serious than others: Squamous Cell Carcinoma: These cancers account for approximately 20 percent of nonmelanoma skin cancers. They develop from squamous cells that make up the outermost portion of the skin, the epidermis. This cancer tends to occur on areas that have been exposed to the sun such as the neck, face, ears, back and hands. It can, however, occur in other areas, such as in scars or skin ulcers or in the genital area. These cancers usually grow slowly and it is not uncommon for them to spread or metastasize. They are more likely than basal cell carcinomas to invade fatty tissue beneath the skin or spread even further. To diagnose skin cancer the doctor will take a complete medical history and ask the patient about past exposure to the sun and other known causes of skin cancer, and whether he or she has a personal or family history of skin conditions. It is important to ascertain when the mark appeared and whether it has changed in size or appearance. He or she will perform a skin www.TheWomensJournal.com

There are some who continue to be sun worshipers and there are some who continue to seek the “golden tan” despite the fact that we know that sun damage can lead to skin cancer. It is not just sun worshipers that can get skin cancer but also those who have had over-exposure from their younger years or those who work outside over long periods of time. There are 3.5 million cases of skin cancer diagnosed every year in this country. examination and may check for enlarged lymph nodes. Treatment: Most squamous cell carcinomas can be cured if detected and treated early. But because this type of skin cancer can grow quickly, delaying treatment could make it more difficult to cure. There are numerous treatment options that are available, and most are easily performed in the doctor’s office. The treatment decision is based on whether the squamous cell is high or low-risk, which is based on the risk of recurrence. Other considerations include the patient’s age, general health, medical history, and cosmetic preferences.

Basal cell Carcinoma: It tends to occur in areas of skin that receive the most exposure to the sun, like the head and neck. They usually grow slowly and it is rare for them to spread, or metastasize to lymph nodes. However, this can occur if the cancer is left untreated so early detection is key. If the two above are caught early they can be treated with very noninvasive techniques such as a cream. The goal is to keep these types of areas under control prior to it becoming a problem. Basal cell is very treatable by a medical professional and requires a medical diagnosis. Lab tests or imaging may be required.

If left untreated skin cancer will grow down to the dermis and the subcutaneous tissues. In the most advanced stages, skin cancer may spread into nearby muscle tissue, cartilage or bone. Once cancer spreads into the blood or lymph fluids, it may reach other areas in the body such as the liver or lungs. If your doctor thinks that a particular patch of skin may be a melanoma, you will have a biopsy, in which a small piece of tissue is removed. A pathologist will examine the tissue under a microscope to determine whether melanoma cells are present.

Ways to prevent Skin Cancer: Avoid sun exposure. Most damage is typically done at a very early age in early childhood; however, the risk accumulates over time so at any age you can take steps to address overexposure. Apply sunscreen when you are outside as frequently as possible.

atients who have coronary artery disease need to have a lower cholesterol. Many studies have been done that show the benefits of lowering bad cholesterol. The drugs most commonly used for lowering cholesterol are called statins. These include drugs such as: Pravachol, Mevacor, Crestor, Zocor, Lipitor and Livalo. These drugs have been the mainstay of therapy for lowering cholesterol for many years. They have proven to be of great benefit in lowering the risk of recurrent cardiovascular disease. There are also some patients who benefit from using these drugs to prevent cardiac events primarily. These drugs do have their limitations. There are potential side effects involving muscle breakdown and liver inflammation. There is some concern that some of these drugs, such as Lipitor (Atorvastatin) might increase the risk of diabetes. In addition, sometimes statin drugs interact with other medications that patients are on. There are also some patients who still have cardiac events despite being on statin drugs. The goal in treating patients is to lower the bad cholesterol to a level around 70. Many patients fall short of this goal. There is a new class of medications that have become available for treating patients in lowering their cholesterol. These drugs are a new class of medication. They are called protein convertase subtilisin/kexin type 9 serine protease drugs. For short, they are called PCSK9 drugs. These drugs act in an area that is extremely important in cholesterol metabolism. They regulate the receptors for your bad cholesterol (your LDL). These drugs have been studied for a number of years now and are now finally becoming available for clinical use. They have proven to be highly effective in lowering your bad cholesterol. They are especially helpful in patients who have a type of high cholesterol

that runs in families. This is called familial hypercholesterolemia. The PCSK9 drugs act to increase the number of LDL receptors available on liver cell surfaces. This accelerates the clearance of a circulating LDL cholesterol. The data so far suggests that these medicines are safe and highly effective. There are two drugs that have been released with the approval of the U.S. Food and Drug Administration. These drugs are actually administered by injection. Patients are taught how to inject themselves and they are treated usually every two weeks. These potent new drugs will likely prove to be a very effective addition to the treatment choices patients have for hypercholesterolemia. They will be very useful for the patients who have familial hypercholesterolemia. enopause is defined as the period in a woman’s life at which menstruation ends. This event occurs 12 months after a woman’s last menstrual cycle due to a loss of sex hormones. Symptoms include decreased energy, weight gain, low sex drive, vaginal dryness, hot flashes, poor sleep, and irritability. Andropause is a gradual, variable decline in male testosterone levels. Similar to menopause, this event occurs in middle-aged men causing low energy levels, problems with erectile dysfunction, decreased muscle, increased fat, and decreased sense of well being. Does either scenario sound familiar? Men often view women going through menopause as moody psychopaths, while women look at men as selfish lunatics going through a mid-life crisis. Bioidentical hormone replacement therapy (BHRT) is a form of hormone replacement that involves replenishing lost hormones with molecules that are biologically identical to those your body makes on its own. Acid Reflux Treatment – A Bitter Pill To Swallow

By Dr. Melissa P. Broyles, D.O. Gastroesophageal Reflux Disease, or GERD, affects greater than 50 percent of the US population. The symptoms of GERD can be heartburn, bad taste in mouth, burping or coughing. Most commonly GERD is due to diet and obesity. Other causes are a hiatal hernia, gastroparesis (the slow movement of food through the stomach), a defective lower esophageal sphincter (the ring of

muscle between the esophagus and stomach) or a weakening in the diaphragm. Lifestyle modification, including weight loss, avoiding spicy foods, caffeine and alcohol, and not eating three hours before bedtime can be a very effective treatment for GERD. However, if further treatment is needed, one must consider the risks versus the benefits of medication. The most common pharmaceuticals prescribed or bought over the counter are a class of medications called Proton Pump Inhibitors (PPIs). While these medications certainly relieve GERD symptoms and can aid in the healing of inflammation of the esophagus and stomach, they have the potential for serious complications. Prolonged use of PPIs can cause malabsorption of several critical nutrients (i.e. calcium, magnesium, zinc, vitamin B12 and iron). A lack of calcium and magnesium can lead to osteoporosis; deficiencies in B12 and iron can cause anemia; and zinc deficiency can cause hair loss. By decreasing stomach acid, bacteria can enter into the lungs and gastrointestinal tract. For this reason, PPIs can increase one’s risk for community acquired pneumonia and bacterial overgrowth in the stomach and small intestine. If that is not enough, a new observational study showed that patients on PPIs for 1-2 years had a threefold higher risk of kidney failure than those who used the drugs for a month or less. Another reason PPIs may be a bitter pill to swallow is their “rebound effect.” Blocking stomach acid production causes stomach cells to secrete a hormone called gastrin. This hormone signals the body that more acid is needed. When the PPI is stopped, the elevated gastrin level causes a rebound increase in acid secretion. This makes it very difficult for a person to get off of the medication. Therefore, it is imperative to talk to your doctor about a weaning schedule to prevent this phenomenon. If dietary changes and weight loss do not alleviate a person’s acid reflux, one may consider alternative treatments, such as aloe vera, slippery elm, or DGL (deglycyrrhizinated licorice). However, it is important to consult with an integrative practitioner for proper dosing and scheduling of these treatments.

At Christiana Care Health System, we offer moms-to-be the option of a unique, hands-on group approach to prenatal care called CenteringPregnancy®. CenteringPregnancy® is different from a traditional prenatal program because it combines routine checkups with a lot of extras, including education, games and social time with other moms-to-be, who share experiences during this exciting time in their lives, in a comfortable setting. Research demonstrates that the CenteringPregnancy® model increases patient satisfaction, promotes healthy behaviors during pregnancy, decreases rates of preterm and low-birth-weight infants, and increases rates of breastfeeding and immunizations. Additional benefits include no waiting time for appointments, the opportunity for women to engage with and receive support from their peers, and more time for education and discussion than typical office visits might allow. Our CenteringPregnancy® team includes heath care providers who are focused on caring for pregnant women. They are dedicated to helping expectant mothers to understand the dramatic changes in their bodies that occur during pregnancy. In addition to obstetricians, the team includes nurse practitioners and a certified nurse midwife. There are usually 10 momsto-be in each group. The groups meet at Wilmington Hospital and at Christiana Hospital. Each session lasts from 90 minutes to two hours.

Each mom-to-be receives a journal, which she will use to log her weight and vital signs during her exam before each group session. She also can use it like a pregnancy diary, recording special moments or need-to-know information.

The moms-to-be range in age, and they include first-time moms and moms with lots of experience. How will I know when I am in labor? What about food cravings? Does breastfeeding hurt?

There are several different scientific assessment tests backed by research that can be used to identify Intrinsic (internal) and Extrinsic (external) strengths. I use these in my work with clients. In addition, strengths can begin to be self-identified by answering basic questions like:

f you are like most people you are fully aware of what is hard for you – the things you just do not do well and the impact that these perceived weaknesses have on your personal or professional life. This perception or awareness may come from self-assessment, or from what others have told you, and it may or may not be accurate. In either case, it draws focus away from an equally important perception: Awareness of your strengths. You, like most people, are not as aware and perceptive of your strengths as you are your weaknesses. Due to basic evolution we are less inclined and often reluctant to look at our strengths, the brain is hardwired to look for risks and problems and ‘the squeaky wheel’ commands our attention. For that reason, we often target weaknesses as areas for growth when in reality – and supported by research studies – focusing on strengths not only diminishes weaknesses, it provides our greatest opportunities for growth. That’s great news and great science! Each of us has natural strengths and a wide range of innate skills. These traits are so much a part of us we may not really notice them. Our strengths are freely available resources. Once we accurately assess them we can develop and grow them in an intentional way. Empirical evidence has shown that use of our strengths increases our energy, is associated with higher levels of happiness and lower incidences of depression, and even helps us recover from illness. Living from our strengths allows us to open up new possibilities in our daily lives.

Thinking back to my childhood, what did I do well and what did I enjoy doing? Your answers to these questions will shed light on your strengths – what they are and when you are using them. Another method of assessment is to use a checklist of words describing strengths. (See my blog for a list of strengths www.bewelllifecoaching.com) A part of my initial session with clients, we begin to identify and label strengths so they can be used to reach goals. Throughout the coaching process we discover and develop primary strengths - and uncover and develop additional strengths – and find ways to utilize them to overcome obstacles.

Those suffering from ADHD are often acutely aware of how the disorder has negatively impacted their performance at school, work, and home. It is even more important therefore for these individuals to identify and use their strengths to help overcome some of the struggles symptomatic of ADHD. For example, some of the common strengths for those with ADHD are being energetic, creative, curious, persistent and having a quick sense of humor. Harnessing these and other individual strengths can be the first step in creating strategies to manage some of the more challenging ADHD symptoms. What is Natural Bio-Identical Hormone Replacement Therapy?

any women as well as most doctors, are confused as to what Natural Bio-Identical Hormone Replacement Therapy (BHRT) is and whether it is good for you? How is it different from traditional Hormone Replacement Therapy (HRT)? The confusion comes because when studies are done and articles are written about traditional hormone replacement therapy they often don’t make the distinction between traditional synthetic HRT and natural BHRT. Studies merely talk about hormone replacement therapy and continually meld HRT and BHRT together. To compare synthetically made products and natural products is like comparing apples to oranges. It is no wonder everyone, doctors included, are confused. When you look at studies using natural BHRT, you will find it to be safe and helpful in maintaining your body’s normal functions. What are Synthetic Hormones? Traditional HRT refers to giving hormones made from conjugated equine estrogens (CEE) from the urine of pregnant mares called Premarin and/or Premarin combined with progestin medroxyprogesterone acetate called Prempro. Synthetic estrogen is referred to as CEEs. Synthetic progesterone is referred to as progestins. All synthetic drugs contain fillers and coatings that may or may not cause side effects. The synthetic estrogen Premarin contains many different kinds of estrogens that are not capable of fitting into human Estrogen receptors sites. There are no studies to date that show what happens to these circulating estrogens as they cannot be detected when testing for estrogens in saliva, urine or blood. Although early studies show Premarin may increase dementia, in the French E3N study 2004, it was also found it may be anticancerous for breast cancer. (French E3N Study 2004-Int.J.Cancer 114, 448-454(2005); (http://www.ncbi.nlm. nih.gov/pmc/articles/PMC2211383) What are Natural Bio-Identical Hormones?

Natural BHRT has only been offered in the United States as an alternative to synthetic hormone replacement in the last few decades. However, it was first used in this country by the American Indians for menopausal symptoms in the form of wild yams. Wild yams, soybeans, and peanut oil are used today to make natural estrogen and progesterone compounds. The bio-identical hormones known as 17 beta-estradiol and progesterone have been FDA approved. Peanut oil is used to make an FDA approved progesterone called Prometrium. The word “Natural” when referring to hormones means “the exact same chemical structure as what your body makes.” This does not mean all bioidentical hormones are made from plants. Hormones produced in a laboratory having the same chemical structure as your body’s are natural and bio-identical. Everything else is synthetic. HRT vs. BHRT What Estrogens to Use and Why. Before the Women’s Health Initiative Study Results became known, HRT was known by the treatment of menopause symptoms with synthetic hormones. Now HRT is often used to describe BHRT. Traditional HRT and BHRT are totally different ways of treating lack of hormones, the similarities and differences of which has created much confusion. To add to the confusion, the FDA

has authorized the same precautions for HRT be put on drug side effects inserts of BHRT. To be clear, HRT and BHRT are not the same drug; they do not have the same side effects. Bio-identical hormones are often made from wild yam. The hormones found in wild yam are biologically identical to the chemical structure your body makes. Your body makes three different kinds of estrogens called Estrone or E1, Estradiol or E2, and Estriol or E3. Synthetic estrogen comes in many forms. The most common synthetic estrogen prescribed is a pill called Premarin. It comes from horse urine and is a mixture of E1, sodium equilin sulfate, 17 alphadihydroequilin, 17 alpha-estraddiok, and 17 betadihydroequilin. The horse estrogens are equilin and equilenin. Additives and the coating of the pill are added which may cause a burning sensation when urinating, during allergic reactions, and joint aches and pains. The estrogens found in premarin and other synthetic estrogens contain many forms of estrogen that do not fit into the estrogen receptor sites in the body. What happens because of this is not completely known, however, it is known that the synthetic estrogens stay in your body for 13 weeks because we do not have the capability of breaking down equine estrogens. Estradiol your body makes is eliminated from your body via the urine in one day.

Reach out to local farmers and visit local farmers’ markets. It is important to only feed your dog pasture-raised, antibiotic-free and hormone-free animals. The drugs that factoryfarmed animals are fed can transfer to your dog, affecting health, growth and they even stay in their DNA!

I feed Ardesia, a 90 pound mastiff puppy for about $4-5 a day. If i fed her high quality dry or freeze-dried food, it would cost me $15-20 a day! Hearts can be fed as muscle meat and are only about $2-3 per pound. Chicken necks are great, $1-1.50 per pound, the cartilage in them is an excellent source of glucosamine and chondroitin.

Nutritional Needs:

I would do some online research to see what feels best for you to feed, based on your dog’s needs. The Calcium-Phosphorus ratio is important. Large breed puppies need more bone in their diets, than full-grown dogs for example. You want their bones to growth healthy and strong, not overgrow too fast with too much protein going to the muscles. I feed about 50% raw meaty bones, 30% muscle meat and 20% veggies. Liver should

only be fed a couple times a week. Other than that, as many different parts, of as many different animals you can feed is best. This fits well with what is available at given times of the year. My friends that hunt give me deer legs for her. Also, I try to mostly feed animals she could ‘take-down’, so less beef, more lamb and pork and I don’t feed beef load-bearing bones. The nice thing is, that dogs thrive on many of the animal parts that humans don’t eat regularly, which means these are less expensive to purchase. For veggies, I use kale, sweet potato, celery, cucumber, beets, zucchini, pumpkin, a little ginger and apples. Other fruits digest too quickly and can ferment before the rest of the meal is digested, so bananas, pineapple, papaya, blueberries, strawberries are better for snacks. I pulverize the veggies in the Cuisinart, then freeze them for 24 hours, so they are very broken down. Your dog is really not getting the nutrients so much as using the fiber to move everything through. A good fiber diet of veggies and ground flax seeds help scrub the intestines and prevent parasites from being able to settle. 90% of a dog’s immunity is in their intestines, so it is very important to keep healthy! 31

Lisa Mackenzie has been an audiologist for the past 27 years. She received a Bachelor’s degree in Speech and Audiology and a Master’s degree in Audiology from Ithaca College in Ithaca NY. After completing her education in upstate New York, she returned to Delaware County to pursue a career in Audiology. Her work experience has included diagnostic hearing and balance testing, aural rehabilitation, newborn hearing screenings, occupational testing and hearing aid fittings. Lisa is certified by the American Speech Language Hearing Association and is licensed in the State of Pennsylvania. For the past 20 years she has been a clinical Audiologist at Riddle Hospital in Media, PA. Lisa has enjoyed helping the many patients that she has met over the years. It is especially rewarding for her to witness patients improve the quality of their lives when they pursue amplification via hearing aids or assistive listening devices. Equally important are the patients who benefit from medical or surgical intervention and referrals. Lisa finds her work interesting because she recognizes that every patient has a unique set of traits, symptoms and concerns.

Jessica M. Dr. Bell decided to pursue a career in Audiology following many “job shadowing” opportunities in a local hospital, thanks to her mother. Denise E. Stewart is a Pennsylvania licensed audiologist in practice for 32 years. She received her Master’s Degree in Communication Disorders from Pennsylvania State University and her Bachelor’s Degree in Communications and Secondary Education from West Chester University. Denise is a fellow of the American Academy of Audiology and certified by the American Speech-Language-Hearing Association. Special areas of interest include aural rehabilitation and tinnitus management, hearing aid dispensing, and balance assessment. Denise has worked as an audiologist in private practice, hospital and rehabilitation settings during her career. She has been a staff audiologist at Riddle Hospital for the past 16 years. Balanced Sunshine Is Blissful!

Happy Summer Friends! This is the time to be mindful of the ultraviolet light damage this time of year can do to our hair and skin. We know skin cancer is the most prevalent form of cancer and your scalp is a very important part of your body that is often forgotten. I see countless cases of clients with sunburn here, most of them completely unaware of what is happening. We love Skin Ceuticals, UV Defense sunscreen. This product is my favorite, a fountain of youth that with broad-spectrum protection and a blend of titanium dioxide and zinc oxide!

I put it all over my neck, face, ears, hairline and if I don’t have a hat on my scalp, where the hair parts as well. Too much sun leads to dry, frizzy hair that breaks and too much chlorine and salt water can also make our manes lose their bouncy summer shimmer. Does summer hair frizz make you sad? The greens and filler flowers, such as heather, limonium, wax flower or babies breath carry out the part of the “extras”. Yet all are integral to the entire production or composition or bouquet.

bouquet is more than ~ ...an assortment of flowers tied with a ribbon or plunked in a vase. The creation of a bouquet combines the art of composition and the principles of engineering. The finished product has to appeal to the eye as well as remain intact over several hours [or in the case of a bridge, for many decades!]. If a bride decides to have her wedding flowers preserved the matter of having every bloom at its peak and every adornment firmly attached and every ribbon securely wrapped is doubly important. The bride and her florist are an artistic team. Most often a bride has a vision in her mind accompanied by a picture from a magazine or a photo from a friend’s wedding or an image from an online site of a particular style of bouquet, and she may have a very specific list of flowers that she loves and are meaningful to her. Enter the floral designer who knows what flowers on the bride’s list are available in the season when the wedding will take place. Lilies of the valley, tulips, peonies, & dahlias are not available year round making it necessary to suggest an alternative for a bride who has her heart set on any of these. Just as an artist selects various objects for a stilllife painting, a floral designer must select flower forms which compliment each other when the bouquet is assembled. One would not want flowers of all one size like football mums and peonies and sunflowers in a single bouquet! Nor would daisies, alstromeria and minicarnations create an eye-popping composition. If we think of a bouquet as a theatrical production, flowers such as peonies, hydrangeas and dahlias

The creation of a bouquet when the floral designer sits down to put it together is much like manufacturing a car on an “assembly line”! First, all of the flowers have to be prepped. Having arrived at the flower shop early in the week, all stems have to be cut and hydrated and the flowers of each variety have to be counted and set aside in the cooler. At assembly time it is critical that the designer does not discover that she is 5 stems short of the roses or hydrangeas or whatever flowers are “the Stars” in the wedding party’s bouquets!

The creation of a bouquet, like the main course at a gourmet dinner, is a combination of art and science. The entree is equivalent to the bouquet; each individual flower in the bouquet is an ingredient in the dish. It includes a featured item, a number of secondary ingredients, an assortment of spices and herbs and perhaps a garnish. When the floral designer meets with a bride there some essential questions that have to be answered:

The number of attendants, the number of guests and whether arrangements will be needed at a ceremony site as well as a reception site will dictate how much of the overall budget is available for flowers.

The creation of a bouquet is the product of a bride’s vision and the floral designer’s expertise. Once the bride and her florist have decided on the specific flowers to be used, the bride steps out of the picture. Even though the wedding may be scheduled for a year in the future a floral designer makes her “shopping list” of the flowers that will need to be ordered 3 - 4 weeks before the actual delivery date. etter is one of those ear-marked words for a judgmental person. Plurality and political correctness have made it difficult to determine if and when something is actually better. Right? Says who? How you celebrate the milestones and special occasions is one area that we can all improve upon. Better in this instance is about being more true to who you are at a very fundamental level - alive. A simple approach of asking about the bottom line - Does “this” encourage life? - Will guide you to making better memories. Growing up, when family or friends got together, we partied the right way. We’d stay up late, share stories, laugh, cry - eat comfort food, drink our fill - and love almost every minute of it - until the next day. Every time it was the same thing - I cherished the inside jokes, quirks, hugs, but my body didn’t feel like that. The truth: we weren’t celebrating in the most life-affirming way, and it sent my biological systems into panic mode trying to function normally. I’ve traveled to other parts of the world and country and found the same is true away from home. Family traditions and intoxicated decision making are two staples when it comes to getting friends and family together; both of which are notorious for being misaligned with general good health practices. Specifically, over-indulgence - refined sugar, white flour, alcohol, sitting around, 2nd helpings - is the favorite way to party.

This line of reasoning is not to induce guilt, blame or shame, but instead to challenge you into choosing some of the time to celebrate with your friends and family in a positive way. The health and fitness trend is a national buzz, possibly reaching a saturation point. But, this is about you! You can choose to make better memories, share your inspiration and hope with the people you care for most deeply. I am very blessed to organize private parties and teach yoga with some of those private groups at Kaya Wellness Center. It is so heartwarming and beautiful when a group tries something new like aerial yoga or a group of 12 women on a bachelorette weekend do yoga on the beach in the morning. More and more people are choosing activities that are fun, exciting, but also challenging because they strengthen the bonds of family and friendship in a much more meaningful and lasting way. Being An Event Photographer As an event photographer, there’s nothing worse than delivering the photos to find that they’re just not what the client was looking for. Having these conversations beforehand will clarify the shoot and reduce the chances of that happening.

Step Two: The Type of Event By Shannon Adelson Adelson Media, Inc

he title of Event Photographer might not sound particularly glamorous or exciting, but in this role I often tap into my multi-media knowledge and resources to help my client tell the most important story ever captured...their story. This visual narrative includes their vision coupled with my unique perspective and keen eye for storytelling. It’s my job to thoroughly document both planned and unplanned moments to make the story count, to meet their goal, whatever that may be. Each event holds different challenges. Some clients may want a series of reportage style photos of the people and interactions, whereas others will want to focus on the overall setting. In brief, here are some of the steps I move through when considering your event. The list is different based on each photographer’s experience.

Step One: Build the Relationship

In depth, detailed conversation with the client about their expectation is critical. Some have very clear thoughts on the types of shots they want. Others might prefer a more hands off approach.

The style required for a brand launch or wedding varies greatly from that required for a gallery opening or cocktail hour. The type of event determines the style in which I shoot and how to best represent the event’s atmosphere.

Step Four: Don’t Overthink Things

Once an event is over, it’s up to me to convert and post-process responsibly. The majority of people will not want to see vast amounts of creative editing, filters and photo shopping. They’d much prefer a set of clear and crisp shots that are true to life, so I bear that in mind when working in Photoshop and don’t get carried away. Too many photographers forget that they are working

for the client. What one sees as a great photo, others may not like. In the end, you have to deliver what the client expects. I’ve already stamped my mark on the shots through the creative decisions made in camera, the exposure, composition and framing, so I don’t get frustrated if a client rejects my favorite shot.

Step Five: Delivery

There are many methods of file delivery, such as: online galleries, a physical

flash drive, or meeting in person to discuss the images. It’s important that I meet the client’s needs and I’ll go that extra mile to confirm their satisfaction. It’s also vital to understand who are the most important people at the event. At a wedding, it’s obvious, but at a corporate event, it might not be so easy. To ensure that I capture VIPs correctly, I arrange for my clients to point out who should be prioritized.

Step Three: Get the Needed Shots First

Before I start getting too creative, I nail down a good bank of shots that capture the overall event. This usually includes some wideangle shots to establish the venue, both inside and out, and some detail shots of the more interesting aspects of the event and the key people involved. Once I’ve got those in the bag, I can start thinking about creative angles and vantage points. I always find that I get more confident and creative as an event progresses, as I’m more comfortable with the surroundings and the guests are more comfortable with me. 50

Know Your Rights – Delaware Workers’ Compensation Laws is also certified under the Workers’ Compensation Statute. (If you see an out-of-state caregiver, they do not have to be certified under the Workers’ Compensation Statute.) A list of certified providers can be found on the Department of Labor’s website. The worker’s compensation insurance company must pay at least one medical bill every five years in order to keep this benefit active!

By Heather A. Long, Esq. “Knowledge is power” is an old adage that is especially significant if you are involved in a workplace injury. All too often, injured employees attempt to navigate the murky waters of their work injury claim on their own, without the critical knowledge necessary to protect their rights. It goes without saying, that your health is the most important thing to protect. Employees who are injured on the job don’t always understand the rights to which they are entitled. Hopefully, this article will help you by giving you a very broad overview of those rights. Keep in mind, you are generally entitled to these rights no matter how you were injured; workers’ compensation is generally a “no fault” benefit, because it doesn’t matter who was at fault for your injury, only that you are injured while working. What are injured workers entitled to under the Delaware Workers’ Compensation Act? 1. Disability Benefits Most of your rights under the workers’ compensation laws in Delaware are based on your average weekly wage (AWW for short). Workers’ compensation wage benefits can pay up to 2/3 of your AWW, up to a limit of $679.63. This benefit is designed to replace your wages when you can’t work for more than three days due to injury. This benefit is also available on a partial basis if your doctor restricts your work hours. 2. Medical Benefits If you are injured in the course and scope of your employment, you are entitled to medical benefits for the rest of your life. This is as long as the treatment is reasonable, necessary and related to the work injury, and your Delaware caregiver

3. Mileage You are entitled to mileage reimbursement of forty cents per mile, roundtrip for your doctors appointments. 4. Scarring/ Disfigurement If you were unfortunate enough to have suffered a scar or other physical disfigurement as a result of your work injury, you are generally entitled to compensation. Any scar and/or disfigurement that is visible when properly clothed can be awarded a monetary settlement. The amount of the settlement is determined by the location, size and visibility of the scar.

Grandparents’ Custody Rights In Pennsylvania

randparents (and great grandparents) play an important, and often unrecognized, role in providing for the happiness and well-being of their grandchildren. Research has shown that a high level of grandparental involvement improves the welfare of children, and children who enjoy a high level of grandparental involvement (especially children from divorced or separated families) have fewer emotional and behavioral problems.

Over the last 50 years, with the growth of dual worker households, grandparents have assumed greater responsibility over child care and rearing, especially for preschool children (i.e. babies and toddlers). Grandparents also help out with older, school-age children by picking them up from school, taking them to and from their various activities and caring for them during summer vacation. Despite the importance of this precious relationship in family life, it is often pushed aside when divorce, support and custody matters arise. Animosity can develop among the warring parents and the respective grandparents which often results in the child being held back or completely alienated from the grandparents. Whatever the situation,the well-being of the child suffers and legal intervention may be required. Recognizing the value of this relationship, Pennsylvania’s Child Custody Act grants special status to grandparents/ great grandparents. For instance, a grandparent who stands in “loco parentis” (visitation of the child in the presence of one or both parents) to a dependent child can file for partial, supervised or

full custody, provided that: (a) the grandparent’s relationship with the child began either with the parent’s consent or by court order; (b) the grandparent has assumed or is ready to assume responsibility for the child AND (c) the child is substantially at risk due to parental abuse, neglect, drug or alcohol abuse, or incapacity; OR (d) for a period of at least 12 consecutive months (excluding brief, temporary absences) the child has resided with the grandparent. An award granting full legal and/or physical custody to the grandparents effectively removes parental authority and empowers the grandparents to make binding legal decisions for the child with respect to such things as medical care and education. As a result, it is harder to obtain and less frequently granted. In contrast, a partial or supervised visitation or custody award is more common and more limited, as it typically allows grandparents to visit with the child

on a periodic basis (with or without the parents in attendance) or to take the child away from the parents for specified times such as weekends, special events or holidays. Grandparents seeking a partial or supervised custody award must first establish one of the following factors: (a) the child’s parents are unmarried, divorced, in the process of divorce, or separated for a period of at least 6 months, (b) the child’s parent has passed away, or (c) the child has lived with the grandparents for a period of at least 12 consecutive months (excluding brief, temporary absences) and now has been removed from the home by the parents. In the latter case, a suit must be filed within 6 months of the child being removed from the grandparents’ home. The grandparents must further submit evidence that (1) visitation with the grandparents would be in the best interest of the child and (2) visitation would not interfere with the parent-child relationship.

As is clear from this brief outline of the law, an award of custody or visitation for grandparents is neither automatic nor guaranteed, even after a determination has been made as to the child’s “best interests.” It is also important to keep in mind that a custody order is never permanent and any person granted standing under the law may file a petition to change or modify a custody order. Only adoption is permanent, and none of the factors outlined above have application if the child is adopted by a person other than a stepparent or grandparent. Instead, any visitation rights granted prior to the adoption of the child are automatically terminated upon such adoption, and the process must start anew.

Providing Powerful Connections Blue Hen Utility Services

lue Hen Utility Services, Inc. was a business concept that was formulated in March of 2014 when Michele Reynolds’ current business, Shelly Sons Electrical Contracting, was contracted to perform high-medium voltage work. From its start, Shelly Sons subcontracted its customers’ highmedium voltage electrical work to other firms, firms with the technical training and equipment to handle that highly-specialized work. Through that high-medium voltage sub-contracting experience, Michele learned a single fact that would change everything for her business- there were NO Delaware based utility service firms geared to self-perform high-medium voltage work. When storms or other issues breached power lines in the First State, all of the repair work was contracted to out-of-state firms. A Delaware-based utility service company would have a faster response time to address power outages than those more remote out-of-state firms, keeping First State communities and businesses up-and-running when power crisis strikes. Throughout this process of working for several clients to complete their distribution projects, they had the opportunity to collaborate with numerous vendors, supply houses, and business owners that rendered one common theme. There is an urgent need for additional utility service contractors to meet the demand of increased electrical service reliability. To bring her high-medium voltage plans to fruition, Michele needed a sharp team that was up to the challenge. here are few things in this life as physically and emotionally devastating to a family or business as a fire. Knowing what to do after a fire can help reduce the damage and increase the chances of a successful restoration. Even if the entire structure wasn’t destroyed the water that was used to extinguish the flames might damage things that were not burned up. There might be holes in the walls from the firefighters looking for hidden flames and they may have even cut holes in the roof to let out smoke and heat. Restoration can be difficult, dangerous and in most cases require professional expertise and equipment. There are risks to your health and safety that specialists are trained to deal with and can be life threatening if not handled properly.

ests are an important part of a child’s education, but for many students, the preparation process induces feelings of fear and anxiety. If test time always seems to overwhelm your child, Jim Power from the Huntington Learning Center of Newark, DE says the first thing to do is remind the child to keep things in perspective. “Parents should make clear to their children that tests are simply a measurement of their skills and knowledge,” says Mr. Power. “When students approach their study sessions with this simple fact in mind, it decreases their stress level, and puts them in the mindset to perform better.” In addition, Mr. Power says, there are a number of ways parents can help their children learn to study more effectively for exams. Here are six tips for students to make the most of study time and increase their chances for test success:

First, do the work. It goes without saying that all the study time in the world won’t be effective if your child doesn’t pay attention in class, do his or her homework and put in the effort required in school to learn. Stay in tune with your child and his or her teacher, and if academic struggles arise, make sure to address them immediately.

Use learning aids to help.

It’s always a good idea to study material as the teacher introduces it, so that study sessions prior to a test are more of a refresher of previously learned material than a cram session. In the weeks before a test, your child should look over class notes or other materials to re-familiarize him or herself with concepts covered.

Confirm what will be tested. Teachers often share with students in advance of an exam what will be tested: sections of a book, a specific topic or a series of notes. This should be your child’s guide as he or she plans out all study sessions. If your child isn’t certain about the contents of an upcoming exam or missed a class and needs help understanding some material, encourage him or her to visit the teacher outside of class.

Certain aids such as flashcards, charts, timelines and photos can help students memorize and understand information. Encourage your child to incorporate helpful tools into the study regimen, but remind them not to spend too much time creating them. In the end, the most efficient test prep approach is regular review of test topics followed by frequent checks for understanding. PA School Achievement Gaps By Dr. Heidi Gough, Director of Marketing and Communications, PA Leadership Charter School

O

ne of the most common concerns parents have about their school, is its performance academically. All public schools (Charter, Cyber Charter, school districts) must show yearly improvements in student growth, achievement (graduation rates) and close the achievement gap (www.paschoolperformance.org).

“Students who live in homes where parents are rarely available for highquality interactions, where televisionwatching is ubiquitous, or where parents do not communicate high expectations for academic achievement are unlikely to have the support they need to do their best in school. One study found that, on average, four-year-old children from “professional” families have already been exposed to 32 million more spoken words than children from families receiving public assistance”

The Pennsylvania Department of Education (PDE) has invested time and a considerable amount of resources to close the achievement gap in PA. PDE has created several tools, grants and information to help educational leaders, teachers, aides and school resource centers to assist with teaching the PA Academic Standards.

According to PSEA (The Pennsylvania State Education Association) Pennsylvania has one of the largest achievement gaps in the country. Anytime a school or a group of students begins to fail, an achievement gaps is created. These gaps have been observed in specific demographic groups – Caucasian/middle class, Latino, english language learners, African American, children with disabilities and low income families.

Both of these statements evoke the emotion of “well what about the school, it’s their responsibility to educate my child.” An additional answer to the question above is what is happening inside our schools. For several years studies and research have shown that the quality of school leadership, teacher experience and preparation, class size, safety and school funding all play a part in creating an achievement gap.

Whose responsibility is it to close these achievement gaps? The answer is varied and very diverse. Factors that take place in communities and the home can play a big part. PSEA reports; “Students in poverty commonly live in communities with poor job markets, inadequate human resources and few educational resources such as museums and libraries. Students who live in poverty also have a substantially higher incidence of child health problems that cause learning problems. Finally, students in poverty have higher-thanaverage rates of mobility across schools and districts. All of these characteristics associated with poverty can impact student achievement”. www.TheWomensJournal.com

Schools are responsible for creating a collaborative learning environment for their students; this includes strong leadership and qualified teachers (in their field/grade). Teacher engagement is a must, if a teacher is disengaged it has a direct negative correlation with student achievement. 67

So in order to close the achievement gap in PA everyone must take responsibility and work together. Parents, Teacher and Leaders need to take a cohesive approach to a student’s education, everyone must be engaged. PDE must train its teaching professionals to a higher level and encourage creativity within its schools. To close, the students in school today are tomorrow’s leaders, what we teach them now will resonate in the world that they will grow up in.

eredith Chapman is one of the instructors in the University of Delaware’s Social Media Marketing Strategy Certificate program and is asked often about how businesses can best improve their strategies and reach more customers. The program, which is a 2016 recipient of a National Outstanding Program Award from the University Professional and Continuing Education Association, helps newcomers learn how to make use of social media platforms and helps experienced practitioners learn new techniques.

1. Know Your Audience:

If a business is trying to reach 17-to-25-year olds, it must use Snapchat. Young parents age 35 to 50 spend their time on Facebook. You need to pick the media where the target audience is.

4. Encourage Peer-to-Peer Recommendations:

It’s great for a business to share its story, but it is better for others to recommend the product or service. That way, they can create experiences that others want to share.

5. Listen, Listen and Listen:

Social media is about having a conversation and that means listening to others. A business must be able to take feedback from customers and turn that into social media strategies that help shape big decisions. “Social media is all about telling a compelling story,” Chapman says. “We’re all human, and tapping into our emotions and making us part of the conversation is vital. Soapstone Soapstone has a fabulous natural grey color that ranges in tones from light to dark with subtle veining and a smooth matte feel. It is resistant to etching from acids, highly stain and bacteria resistant and it is not as harmful to the environment as other quarried stones. Soapstone does need regular maintenance, may crack, chip and scratch, however scratches can be easily sanded or oiled away. Unlike other natural stones, it does not require yearly sealing but regular applications of mineral oil will help to disguise any surface scratches, add sheen and deepen the stone’s color over time.

Marble Marbles come in a variety of colors and patterns and is a choice of many for their countertops. Marble has a smooth surface that won’t chip or dent and is heat-resistant. The downside of marble is that it can be scratched or etched by acids (citrus, coffee, alcohol, some cleaning products) even if you seal it. But if you are vigilant about spills and seal your counters a few times a year (you can do this yourself with a simple spray product) marble is a good option.

Polished Granite More durable than marble, granite won’t scratch, is resistant to stains, heat and water if sealed. Granite is low-maintenance, has

Honed granite is just as resistant to scratching, chipping, cracking and heat as polished granite, making it one of the most durable kitchen countertop options around. Like polished granite, it does require resealing about once a year.

Zinc Zinc is a reactive metal with a living finish that will acquire a patina and change with age and use. This patina will continue to evolve throughout the lifetime of the product. Imperfections, including slight scratches, pinholes, and soldering marks can be expected on your zinc countertop. They help create the charm of this rustic metal. Windex or mild soap and water are recommended for daily cleaning and wipe up all liquids with paper towels. Many owners choose to let a patina develop on zinc. This enhances the “industrial” or “country” flavor of the countertop.

Engineered Stones (CaesarStone, Silestone, Cambria, etc.) Engineered stones have become a very popular choice of countertop material. They are nonporous, resist scratches and stains, easy to maintain, and no sealant is required. They have the looks of natural stone (it’s at least 90% quartz) but can be mixed with pigments for custom color.
