The Weight Is Over: To Sleeve Or Not To Sleeve? Patients who have weight loss surgery have significant positive metabolic effects as well. Type 2 diabetes, high blood pressure and sleep apnea improve and therefore decrease obesity related morbidity and mortality. Many patients will be able to decrease or completely get off medications for high blood pressure and diabetes. Back and knee pain also improve with weight loss.

W

eight loss surgery has evolved over the last 30 years. Laparoscopic Sleeve Gastrectomy (“Sleeve surgery”) or taking out a portion of the stomach (see diagram 1) has emerged as the most common weight loss procedure in America over the last couple of years. It is estimated that in 2017, 70-80% of the weight loss surgeries done in the US will be the sleeve gastrectomy. This surgery is done through a series of incisions, most of which are the diameter of a dime. The Sleeve surgery basically converts the stomach from a football size to a banana size. The decreased size of the stomach causes a restriction feeling to the patient when they eat and therefore they will eat less. In addition, the surgery removes a part of the stomach that produces a hormone called ghrelin, which is secreted by the oxyntic cells of the fundus in response to fasting. Ghrelin stimulates appetite. The Sleeve decreases the amount of ghrelin produced and therefore patients are able to better control their hunger.

Fortunately, with the sleeve gastrectomy, safety profiles have been proven to be high with few serious complications. Sleeve surgery causes less nutritional deficiencies post-operatively compared to the gastric bypass. Complications after Sleeve surgery in the literature vary from 2.9 to 9.5% which appear to be lower than the complication rate after gastric bypass, varying between 4.6 and 20.5% in the literature.

In a recent study from the Netherlands on 1000 patients undergoing sleeve surgery found on average an excess weight loss of 86.6% in the first postoperative year, 84.2% after two years, and 84.5% after three years. Pre-existent comorbidities appear to improve or even resolve after sleeve surgery. Resolution of type 2 diabetes occurs in 60-96% of the patients. Dr. Sandra Costa is not only a family physician, but a sleeve surgery patient of Dr. Barnett’s herself. Dr. Barnett graduated from Princeton University in 1983 and obtained his medical degree from George Washington University in 1987. He completed his surgical training at Thomas Jefferson University affiliate Lankenau Hospital in Philadelphia in

Many times when I start to write a short publisher’s note, it is with a glass of red wine, stretched out watching the sun go down. Thus it takes a very long time to complete the publisher’s note, and several glasses of red wine over many evenings. Some would say that it is not a good idea to drink and work, but I say it is better to drink and work, than just write a boring publisher’s note. For as you can see, the publisher’s note is still boring but I am quite happy and content! When I had to take over The Kent County Women’s Journal back in February I cried, seriously I cried, because I really did not want to take on the burden of managing another paper at my age. I was just getting the other papers under control, so that I could slip away and pretend I was semi-retired every 3 months. But life has a way of taking side trips, and this was just another side trip that life had in store for me. So I pulled myself up by my bootstraps, and with the help of my dear friend, Barbara Steele, and my daughter Stephanie, we took on Kent County and I am very happy to say that things have been going great. We would like to welcome all of our new contributors on board with us in Kent County. Children’s Vision T

he ‘school years’ are a very important time in a child’s life. All parents want their children to do well in school, but often one important learning tool is overlooked - a child’s vision.

Children with undiagnosed vision conditions face many obstacles academically, socially and athletically. This is why a comprehensive eye examination is so important for children. Early detection and treatment provides the best opportunity for your child to see clearly and learn successfully. As children progress in school, they face increasing demands on their visual abilities. Parents and educators often incorrectly assume that if a child passes a school screening, there is no vision problem. However, many screenings only test visual acuity when the visual skills needed for successful reading and learning are much more complex. The most common vision problem is myopia, also known as nearsightedness. Some children have other forms of refractive error like farsightedness or astigmatism. In addition, eye focusing, eye tracking and eye coordination problems may affect school or sports performance.

Eyeglasses or contact lenses may provide correction for many vision problems. However, a program of vision therapy may also be needed to develop or enhance vision skills.

A child may not be able to tell you that he or she has a vision problem because they may think they see the way everyone else sees. Signs that may indicate a child has a vision problem include: •

Frequent eye rubbing or blinking

Short attention span

Avoidance of reading and other close activities

Frequent headaches

Covering one eye

Tilting the head to one side

Holding reading materials close to the face

One eye turning in or out

Childhood Congenital Myopathies Do You Know What It Is? supportive medical and particularly respiratory or breathing therapy, infants with Type I SMA will die from pneumonia or respiratory failure and many do. However, some families opt for a plan of aggressive supportive care for their children. Working with these children and their families is a joyful and meaningful challenge. Meticulous care and monitoring are needed to maintain health and safety in the face of such enormous physical challenges and functional limitations. But looking beyond the doctors, the nurses, the respiratory and physical therapists, the medications, machines and devices that are required to maintain the health and safety of children with SMA, one sees the life and spirit embodied in the hearts and minds of these special children and their families. Watching them learn, grow and develop so much like mobile kids, and yet so differently, makes us remember how much the same we all are and how important it is to realize that it is truly what is on the inside, in the heart, mind and soul that makes us the people we are, and not necessarily our physical strength or capabilities.

every day, most myopathies do not affect the most important aspects of the brain: children with muscle disease can think, hope, imagine and dream of a world in which there are better treatments and even cures for muscle disease. At the present time however, there is no definitive treatment for any of the common childhood myopathies. By Aaron Chidekel, MD

e all take movement for granted: walking from place to place, scratching an uncomfortable itch or shooing a fly. Even the most basic things that happen without us thinking rely on our muscles functioning normally and with sufficient strength: eating, drinking, and most importantly, breathing. For individuals living with muscle diseases, these basic functions can be complicated, impaired or even impossible. Childhood myopathies, diseases that affect the muscles of children, are a diverse and challenging group of genetic diseases that are much more common than expected. Childhood muscle diseases are present from birth or become apparent early in childhood and involve various defects in the structure, function or survival of muscle cells or the nerve cells that control them. This makes these conditions very different from traumatic paralysis, stroke, or even Amyotrophic Lateral Sclerosis (ALS) or Multiple Sclerosis (MS). Yet, despite the severe functional limitations that children with muscle disease live with every moment of

One of the most common congenital myopathies is called Spinal Muscular Atrophy or SMA. SMA affects roughly 1 in 10,000 babies and occurs in all races and both sexes. SMA is caused by a specific mutation in a gene that encodes a protein that is critical for the survival of the nerve cells that control the skeletal muscles; muscles that are responsible for the movements we all take for granted. In SMA, the lack of a functional Survival Motor Neuron Gene (SMN1) causes these motor nerve cells to die and muscle function is lost. There are four types of SMA: I, II, III and IV. Type I SMA is the most severe whereas Type IV is the mildest, based upon the severity of the weakness experienced by the child. SMA is the most common genetic cause of death of infants. Infants with Type I SMA become extremely weak very early in life but develop normally cognitively. Infants with SMA lose the ability to move their limbs, roll over and eventually they lose the ability to swallow and even breathe. Without intensive

enopause is defined as the period in a woman’s life at which menstruation ends. This event occurs 12 months after a woman’s last menstrual cycle due to a loss of sex hormones. Another highly common, underdiagnosed, hormone condition in aging men and women is subclinical hypothyroidism. pring and summer... time to step outside and commune with nature, neighbors, friends and family. For many, it’s the best time of the year. But while we enjoy the great outdoors there’s a large segment of our region’s population who won’t experience those happy times because they’ll find themselves in a situation where they need blood! Blood recipients come in all shapes, sizes, colors and ages and their needs are unique. Some are accident or trauma victims and need that lifesaving gift of blood to replenish what they’ve lost in an emergency situation and THEY NEED IT IMMEDIATELY! Some are afflicted with cancer, particularly leukemia, and may need multiple transfusions of red blood cells and platelets. Sickle cell patients, principally with severe complications, can use up to 15 units of red blood cells to treat their condition. Newborn babies, especially born prematurely, have been known to need blood products to stay alive. And, one of the largest uses of blood occurs due

In the Delmarva region alone, over 20,000 patients a year are saved by the precious gift of life given by dedicated Blood Bank of Delmarva (BBD) donors. Keeping that supply at a safe level requires the dedication of thousands of donors annually, who selflessly give of their time and of themselves to ensure the blood supply is sufficient for whatever may occur. Here’s another fact you may not know, 65% of the US population cannot donate blood! And, of the 35% who qualify, approximately 5% actually do. It’s a very small percentage of people who actually donate and those numbers are dwindling rapidly as many of our most dedicated donors leave the donor pool due to age or health issues. To complicate matters, younger donors aren’t coming forward to replace those who have been donating for years. There is still no substitute for human blood. The 20,000 local patients we serve each year would have serious issues if it weren’t for the dedicated individuals who answer the call and continue to donate. elaware Palliative, a program of Delaware Hospice, works as a trusted community partner in end-of-life education and support in Delaware. Delaware Palliative has joined the collaborative effort of national, state, and community organizations committed to ensuring that all adults with decision-making capacity have the information and opportunity to communicate and document their healthcare decisions.

Unfortunately, only about 25% of Americans have documented their future healthcare wishes in writing in the form of an advance directive. It is important to understand that the right time to make your wishes known is while you are still in good health. While making healthcare decisions is often difficult in the best of circumstances, having others make the decision for you is even more difficult. Each of us has the right and ability to make our wishes known. By allowing our healthcare providers and our loved ones to know what we want, you’ll ensure the right decisions are made.

Advance directives gives you the ability to document the types of healthcare you do and do not want, and to name an “agent” to speak for you if you cannot speak for yourself. When we put off documenting our wishes through an advance directive, too often the result is family conflict, stress and confusion at a time when you are most vulnerable and cannot speak for yourself. Easter Seals and undergo thorough background checks, are bonded, insured and have current First Aid/CPR training. The PAS program puts you in charge as the employer. You are able to hire family or friends as your attendant. You decide how much they are paid, when they come and how to do things. Easter Seals will assist in any technical aspects of being an employer. You must qualify for Medicaid funding to be eligible for this program.

ometimes the “golden years” do not feel very golden. In fact, dealing with the many ailments can feel overwhelming at times. That is where Easter Seals Senior Services steps in to lend a helping hand to ease your way gracefully through the golden years. Easter Seals has many types of services to help seniors remain independent and safe living at home. The In-Home Services program and Personal Attendant Services (PAS) are available to a broad population including seniors, who could benefit by having a “caring partner” or an assistant help with daily needs. Both services provide assistance in areas, such as: companionship, meal preparation, light housekeeping, performing work tasks and bathing needs. It is easy to get started with In-Home Services. Each person’s needs are different so Easter Seals offers a free, confidential in-home evaluation. This meeting will establish an individual care plan as well as address: routines, health needs, cost of services and transportation requirements. A suitable caring partner will be paired based on the needs of the person. All caring partners are carefully screened employees of

Partially funded through Delaware Division of Services for Aging and Adults with Physical Disabilities, the Caregiver Resource Center is another part of the COP that is available to those caring for a loved one. The Money Management program can also assist seniors in keeping track of their finances. Easter Seals’ Case Manager is on site every day to provide information, resources and support to family caregivers. Along with Senior Services, Easter Seals offers a range of other services, including children’s therapies, assistive technology, recreational camping, day programs for adults with physical or intellectual disabilities and respite services for caregivers. ife doesn’t always go according to plan, but sometimes it can turn out to be the greatest gift in the world. This was the case for Angela and Andy when they gave birth to their daughter, Brinley, and found out she had Down Syndrome. It was a surprising diagnosis, as it was missed in the ultrasounds, but Brinley has been an incredible blessing to their family. Angela and Andy knew it was important to get Brinley into a children’s therapy program as soon as possible. After heart surgery at 10 weeks, Brinley’s parents found Easter Seals Children’s Therapy program and it has made all the difference.

“We wanted to be proactive so she did not fall behind. We want to do all we can to set her up for success later on. Early intervention makes all the difference,” Angela says. “We have been very happy with Easter Seals. I have no idea what we would do without the therapists at Easter Seals. They make such a difference in Brinley. I can’t put into words how much the therapists and services mean to us.”

A simple approach of asking about the bottom line - Does “this” encourage life? - Will guide you to making better memories. Growing up, when family or friends got together, we partied the right way. We’d stay up late, share stories, laugh, cry - eat comfort food, drink our fill - and love almost every minute of it - until the next day. Every time it was the same thing - I cherished the inside jokes, quirks, hugs, but my body didn’t feel like that. The truth: we weren’t celebrating in the most life-affirming way, and it sent my biological systems into panic mode trying to function normally. I’ve traveled to other parts of the world and country and found the same is true away from home. Family traditions and intoxicated decision making are two staples when it comes to getting friends and family together; both of which are notorious for being misaligned with general good health practices. Specifically, over-indulgence - refined sugar, white flour, alcohol, sitting around, 2nd helpings - is the favorite way to party.

This line of reasoning is not to induce guilt, blame or shame, but instead to challenge you into choosing some of the time to celebrate with your friends and family in a positive way. The health and fitness trend is a national buzz, possibly reaching a saturation point. But, this is about you! You can choose to make better memories, share your inspiration and hope with the people you care for most deeply. I am very blessed to organize private parties and teach yoga with some of those private groups at Kaya Wellness Center. It is so heartwarming and beautiful when a group tries something new like aerial yoga or a group of 12 women on a bachelorette weekend do yoga on the beach in the morning. More and more people are choosing activities that are fun, exciting, but also challenging because they strengthen the bonds of family and friendship in a much more meaningful and lasting way. Nancy Hawkins Rigg (Nonnie) is the founder and owner of Forever Fit Foundation, a personal training business specializing in Pilates, Gyrotonic, post- rehabilitation and sport-specific training. Forever Fit Foundation began seeing their first client in Kent County in 1993. They currently accept clients at three sites; Dover and Lewes, Delaware and Mendenhall Station, Chadds Ford, Pennsylvania. Nonnie, a licensed and certified athletic trainer, earned her bachelor’s degree from the University of Delaware and her master’s degree in exercise science and sports medicine from Miami University of Ohio. Linda first came to Forever Fit Foundation as a client looking for training to compete in competitive bodybuilding. She received great training, education on nutrition and she experienced that everyone at Forever Fit was very professional and well educated. Linda was impressed by the facility, staff, and the owner Nancy Hawkins Rigg. After four years as a client, Linda received her personal trainer certification and began working at Forever Fit Foundation as a manager/trainer. Linda has been in the fitness industry now for 14 years. James Bowen

James Bowen grew up in Wilmington, DE and relocated to Dover, DE in 2011 to attend Wesley College after being recruited for football and receiving an academic scholarship. James graduated from Wesley College earning a bachelor’s degree in Exercise Science. James is an alumni of Alpha Phi Delta Fraternity. James was brought to the personal training world from his involvement in football, wrestling, and lacrosse. James’s extensive sports career and his knowledge with his degree provides an excellent foundation for a new and supportive approach to training his clients. James began his career at Forever Fit Foundation as an intern before moving into a full time staff position. As the newest member to the Forever Fit Foundation family, James is committed to furthering his education and learning different techniques to help achieve the overall goals of each client. James recently received his basic and advance certification in classical Mat Pilates from Power Pilates. James believes that improving the overall quality of life of the client can be achieved through a balance of various exercise training methods such as: Pilates, traditional weight training, a variety of myofascial release techniques, and specialized flexibility routines. His clientele ranges from high school/college athletes, to individuals recovering from injury as well as those working through chronic conditions. Barry Foor

Barry Foor changed careers midlife to pursue a lifelong passion for physical fitness. Returning to school in 2002, he earned two degrees from Delaware Technical & Community College in Exercise Science and as a Physical Therapy Assistant. He began his new career with Forever Fit Foundation as part of a clinical internship. He currently splits his time between working as a Personal Trainer and a Physical Therapy Assistant. Barry’s interest in physical conditioning began in high school, where he competed at the state level as a wrestler and played football. After college, he joined the military and survived the rigors of basic training and continued his fitness goals after the military through many of the same avenues we all do: gym memberships, various fitness trends and equipment, and different nutritional approaches. Ultimately, he decided to pursue a formal education in fitness. Barry has developed his fitness philosophy through these experiences. He believes each individual needs a tailored program that views health and fitness in a holistic manner—through training that incorporates cardiovascular conditioning, resistance training, core strength, flexibility, and nutrition—and that all these elements are critical to a vibrant life. To get there, however, he evaluates each client’s needs based on where they are in the present moment, including their personality, age, body composition and daily activities and interests, as well as any prior injuries, restrictions, or limitations, in order to develop a program that fits each person.

Charissa has been in the fitness industry for 25 years. She is a trainer at Forever Fit Foundation at the Dover and Lewes DE sites year round. She works in Dover on Tuesdays and primarily works her clients to the bone because she doesn’t see them the rest of the week! Although she has a degree in business, her first love is fitness. She is certified as a Personal and Group Fitness trainer. She is proud to be a Pilates Practitioner. Charissa is married and has two adult children. Her husband was in the Coast Guard for 25 years. Together they traveled throughout the US and raised their children in many states. Fitness was an obvious “fit” for Charissa as she implemented and managed many health clubs and programs along her travels. Each destination has provided the opportunity for educational advancement. He is also a former bodybuilder and competed in Delaware, Maryland, and Pennsylvania from 1993-2004. More recently, he enjoys power lifting and holds State and National records in the 242 lb weight class with lifts over 700 lbs in deadlift and over 600 lbs in the squat competitions. Tim also has a passion for cooking. He has been working as a Chef or Chef Instructor for over 20 years. He is currently the Executive Chef at the Food Bank of Delaware. “Shasta Shutterbug” Has Arrived!

As the sign says in the camper... GRAB A PROP – STRIKE A POSE! “Shasta Shutterbug” camper photo booth was an idea I saw online from California and Florida. My niece had sent a couple of pictures to me, and I started to research. It’s also something popular in Europe. Most of those are from the back of a VW bus or a similar vehicle. I initially saw this 1969 Shasta Compact Camper for sale about a year ago alongside a local road. Not knowing what I could do with it, I somewhat shelved the thought of buying it. After I had seen these ideas online, I knew I needed that “Shasta.” I was sure it was gone eight months later. I took a 15-minute drive where I saw it previously. There it was still sitting there waiting for me. I immediately jumped on the idea. I purchased the camper and started looking at different ones all over the US. There are not many out there, but I think it will grow. I never realized how big photo booths are. I shoot many weddings throughout the year and starting noticing the appeal. I figured photo booths would be more of a short-term fad and dwindle away. Not true! They are getting more and more popular.

was attending UD while I worked on it and came home to check on it (and my work). I was blessed with a relatively warm fall and winter. I gutted it. You never know what’s underneath until you start pulling it apart. I replaced some rotten walls and rotten floors from 47 years of weathering. Put new ceiling and wall coverings with a hardwood floor. It is decked out with a stereo system inside and outside. The photo booth is built to fit in the area that is left. Courtney just put this up on my facebook page 3 weeks ago. I have made one appearance with it with many raves. It has already been booked for the American Heart Association HeartBall in Dover on April 23rd. We have gotten many bookings already for weddings, gala events and other outdoor events. We also have the standard photo booth, but I think this one has gotten much attention. After all, this is the first one in the First State!

So this was my winter project! Not having any idea what to do, I had my daughter, Courtney, do the designing as I did the engineering and labor. My 21-year-old daughter knew what was “in” more than I did. She www.TheWomensJournal.com

Permanent Cosmetics? What You Need to Know… By Melisa A. Ferriola, CPCP, LE

What is permanent cosmetics? And is it really permanent? Permanent Cosmetics is cosmetic tattooing. Colored pigment is implanted into the upper layer of the dermis. As with any tattoo, fading does occur, requiring periodic maintenance, color refreshing. Just like hair color, furniture that may be located near a window, or even house paint, pigment implanted in the skin will fade with time, but you should think of it as permanent.

How are permanent cosmetic procedures done? Permanent cosmetics procedures are performed using various methods, including the traditional tattoo or coil machines, the pen or rotary machine and the non-machine or hand method. I prefer the manual hand application method for a softer, more natural result.

I love the thought of never applying make-up again! Wow! Really? If it sounds too good to be true, it usually is. Permanent Cosmetics covers eyeliner, eyebrows and lip color. Not foundation, blush, eyeshadow, mascara, lip gloss. In addition, at Lasting Looks, we believe in a conservative approach. Do you really want to look like “a night on the town” first thing in the morning?

What is involved with a procedure? The initial procedure will generally take approximately 2 to 3 hours. The process includes an initial consultation to select color and to complete a brief medical history. Before and after photos are taken to help both the client and technician maintain the client’s natural features. Topical anesthetics are applied and the pigment application is completed. Each procedure includes a follow up visit for adjusting the shape and color or density of the pigment. At Lasting Looks, this follow up procedure is included in the pricing.

Is it painful? Topical anesthetics are used before and during the procedure. Most people find the experience surprisingly comfortable. This may vary according to each individual’s pain threshold and the skills of the technician performing the service.

What if I don’t like it? No procedures should be done without your design approval. Your technician should also be setting realistic expectations. Permanent Cosmetics has a different appearance IN the skin than conventional makeup has ON the skin. Although the procedure is considered permanent, these procedures do have flexibility in changing color and shape to some extent, depending on the expertise of your technician. There are lightening procedures

available if really necessary. And fading will eventually occur. Talk to your technician about all your concerns at a consultation.

My lips are very thin. Can I have them enlarged by tattooing above and below my natural lip?

As we age, our lips thin and women request lip pigment placement to enlarge the lips. However, it is highly recommended that tattooing of the lips does NOT extend beyond the lip vermillion border. Lips can be shaped – with good artistry – to appear fuller while staying within the boundaries. Beyond the borders, the result may be a sloppy mess that looks like you didn’t know how to apply your lipstick! This is an extreme faux pas in permanent cosmetics! Do not have this done. Want fuller lips? Get lip filler.

Is it safe? If proper sterilization and sanitary guidelines are met, permanent cosmetics should be completely safe. Ask for a current certificate of training for Bloodborne Pathogens. Consultations are highly recommended. Be sure you are comfortable with your technician and the place of business. Ask questions. Get references. Ring Size Matters!

By Ron Sayers Graduate, Gemologist (GIA) Few people realize the importance of having your rings fit correctly. First and foremost, could you remove them quickly in the event of an emergency? Many times rings have to be removed in hospital emergency rooms, which is done as quickly as possible and usually results in damage to the ring. Other times surgery is scheduled and won’t be performed by the hospital until the rings are removed. We have had numerous customers’ say they have never taken their ring off since they were married. That may be very romantic, but, if the finger has grown larger and they can’t remove the ring, it will need to be professionally removed by a qualified jeweler. The process is usually quick and painless if the right tools are used, and at Sayers Jewelers and Gemologists the service is performed at no charge. The other benefit is, by using the correct tools for removal, the ring can then be resized at a lower cost because there is no damage to the shank. If the ring is removed with ordinary cutters and pliers you may have around your home there is often damage to the ring shank. Rings cut off with ordinary tools can usually still be repaired to look like new but it requires much more time and often more metal which makes the process more costly. www.TheWomensJournal.com

Fitting new rings is often not as easy as it seems. The time of year needs to be taken into consideration because in the winter due to higher barometric pressure and lower temperatures your rings will fit more loosely. On the other hand, in the summer your rings will fit tighter due to higher barometric pressure, heat and humidity. Another factor is what we eat in the summer, which includes salty foods such as potato chips, pretzels and other picnic foods, which make us retain fluid. After a great crab party with steamed crabs and beer or any other beverage of your choice, you may wake up finding you can’t get your rings off which normally corrects itself later in the day. Using the correct sizers is also important. A wide wedding band for example will not feel the same as a narrow wedding ring that is the same finger size. Wide sizers should be used for wide rings and narrow for narrow. If you solder two rings that fit together, they will feel tighter on your finger than each one separate. There is also the factor of the contour of the ring; comfort fit or straight makes a difference. Plastic sizers and paper sizers are not as accurate as metal sizers and guessing at the size to surprise someone usually never works.

A modern quality elastomeric floor coating is resistant to automotive chemicals like gas, oil, hydraulic fluid, and anti-freeze. Just wipe them up and they won’t leave a trace. Here are a few things to look for when you are shopping for a floor coating. The floor coating should have a dry film thickness of 40 – 50 mils. This is about 5x thicker than the homeowner applied coatings. As such, the coating is able to cover minor imperfections in the floor and withstand dropped wrenches without damage. Surface preparation is the key to any successful coating. The professional will use a diamond grinder or shot blaster to put a profile or “tooth” on the floor to help the epoxy coating adhere. Surface preparation also opens up the pores of the concrete allowing the epoxy coating to soak into the concrete and create an even stronger bond. An epoxy formulation for the base coat and a poly aspartic top coat is the ideal combination. The epoxy is able to absorb into the prepared floor surface creating a strong mechanical bond. The urea-based poly aspartic top coat formulation resists ambering from the sun and provide a fast curing, extremely tough and clear topcoat. The texture of the “full broadcast” flake floor provides some degree of slip resistance especially compared to a solid color smooth floor. If higher slip resistance an aggregate material can be added to the top coat to increase the traction.

O

ne of the least understood divisions in the Department of Insurance is the Bureau of Captive and Financial Insurance Products. Let me see if I can explain exactly what captive insurance companies do and why they are so important to Delaware’s economic vitality. Captive insurance companies are owned by the entities they insure and are generally formed by businesses who wish to better manage the cost and administration of their insurance coverage. A captive insurer, like any other corporation, may have legal residence (or “domicile”) in any state and many captives choose Delaware because of our favorable laws and business climate. Additionally, the great majority of revenues generated from this program comes from out of state companies who are domiciled in Delaware. Those companies domiciled in Delaware pay the state both registration fees and taxes on the premiums they collect. Last year, for example, the Department of Insurance’s captive operations generated several million dollars that went into the General Fund for the State of Delaware. When I started serving as Insurance Commissioner in 2009, Delaware already had laws that allowed for the formation of captives, but I created a separate captive bureau and made a conscious effort to attract captive insurance companies to our state. At that time, Delaware had licensed 38 captive insurance www.TheWomensJournal.com

to insure the risks of its parent company, American Water, a publicly traded company headquartered in New Jersey. American Water provides water and wastewater services to an estimated 15 million customers in over 40 states and Canada.

Captive insurance is a mid20th century product, but here in Delaware its roots were established long ago. Delaware permitted the formation of holding companies that allowed larger companies to set up subsidiaries in Delaware. Using these Delaware-based companies, businesses could hold certain assets, thus gaining the benefits of Delaware’s tax laws and Delaware’s internationally recognized business court, the Court of Chancery. The captive insurance program worked like a holding company. However, the benefits shift from tax incentives to insurance incentives.

The Captive Bureau’s efforts have also been noted by its peers. In 2013 we were recognized as the top captive domicile in the country by Captive Review magazine, who also regularly cites Captive Bureau Director Steve Kinion as one of the most influential figures in the world of captive insurance. And I was recently named to Captive Review’s first “Enterprise Risk Captive Pioneers” list, which recognized 20 of the “key influencers” in the United States’ smaller captive industry.

Delaware has long been known as the favored state of incorporation for U.S. businesses. Delaware offers many benefits to traditional businesses. Over a million business entities are incorporated here, including 65% of the Fortune 500. These benefits extend to captive entities as well, creating the ideal domicile for captive business. Delaware is the third largest captive domicile in the United States and the fifth largest in the world. Delaware captives can be organized in a variety of legal forms: as limited liability companies, partnerships, limited partnerships or statutory trusts. Our highly respected Court of Chancery enhances our reputation as a stable and predictable place for captives to do business. One of Delaware’s local captive insurance companies is the Delaware Surety Insurance Company in Dover. Delaware Surety was licensed in 2009 to reinsure surety bond business. A surety bond is a promise by a surety or guarantor to pay one party a certain amount if a second party fails to meet some obligation, such as fulfilling the terms of a contract. The surety bond protects the first party against losses resulting from the second party’s failure to meet the obligation.

Reach out to local farmers and visit local farmers’ markets. It is important to only feed your dog pasture-raised, antibiotic-free and hormone-free animals. The drugs that factoryfarmed animals are fed can transfer to your dog, affecting health, growth and they even stay in their DNA!

I feed Ardesia, a 90 pound mastiff puppy for about $4-5 a day. If i fed her high quality dry or freeze-dried food, it would cost me $15-20 a day! Hearts can be fed as muscle meat and are only about $2-3 per pound. Chicken necks are great, $1-1.50 per pound, the cartilage in them is an excellent source of glucosamine and chondroitin.

Nutritional Needs:

I would do some online research to see what feels best for you to feed, based on your dog’s needs. The Calcium-Phosphorus ratio is important. Large breed puppies need more bone in their diets, than full-grown dogs for example. You want their bones to growth healthy and strong, not overgrow too fast with too much protein going to the muscles. I feed about 50% raw meaty bones, 30% muscle meat and 20% veggies. Liver should

only be fed a couple times a week. Other than that, as many different parts, of as many different animals you can feed is best. This fits well with what is available at given times of the year. My friends that hunt give me deer legs for her. Also, I try to mostly feed animals she could ‘take-down’, so less beef, more lamb and pork and I don’t feed beef load-bearing bones. The nice thing is, that dogs thrive on many of the animal parts that humans don’t eat regularly, which means these are less expensive to purchase. For veggies, I use kale, sweet potato, celery, cucumber, beets, zucchini, pumpkin, a little ginger and apples. Other fruits digest too quickly and can ferment before the rest of the meal is digested, so bananas, pineapple, papaya, blueberries, strawberries are better for snacks. I pulverize the veggies in the Cuisinart, then freeze them for 24 hours, so they are very broken down. Your dog is really not getting the nutrients so much as using the fiber to move everything through. A good fiber diet of veggies and ground flax seeds help scrub the intestines and prevent parasites from being able to settle. 90% of a dog’s immunity is in their intestines, so it is very important to keep healthy! 32

We know the opportunity to take those afternoon and early evening strolls has been eagerly anticipated by so many of you and with the continued revitalization of Downtown Smyrna, there’s never been a better time to come through and check out what our historic town has to offer; from the landmark buildings to the many new businesses and everything in between. We here at The Drunk’n Baker have loved seeing so many new faces, as well as our many regulars who come to see us every day. Perhaps you are new to town or haven’t driven through recently and you are unfamiliar with The Drunk’n Baker. Well, we’re your one-stop shop for all of your pastry and light meal needs; whether it’s breakfast, lunch, that much needed (and well deserved) mid-day snack or who could forget – dessert! We offer a wide range of treats that include both sweet and savory, so you’re sure to find something that hits the spot, be it an old classic or maybe a new favorite. Our bakers use the freshest ingredients without the preservatives, which means you and anyone you may be spoiling get to indulge without all of that guilt. Wait, lunch? That’s right! The Drunk’n Baker is pleased to offer daily lunch specials. Our aim is to be an alternative to the barrage of fast-food choices, yet still offer items that are quick and easy for those of you on the go. We have really been pleased with the positive response we have received over the last couple of months and we encourage you to stop in and see what all the buzz is about. As one of our loyal customers put it, “it’s the kind of items I would put in my brown bag lunch” and we always strive to keep our lunches in the $5 - $7 price range. We offer wraps, sandwiches, sliders and more, all made in-house; and all of our lunch specials can be purchased as a combo which comes with your choice of a beverage and one of our many varieties of cookies. Recently, we’ve expanded our lunch menu, for those of you looking for an even lighter fare. We are now pleased to offer pasta salad, potato salad, cucumber salad and more. Of course, as always, we also love to see all of our many customers who aren’t “on the go”. Whether it’s sitting and enjoying a cup of coffee (we have 6 roasts available daily), iced coffee, hot or iced tea; reading the newspaper (yes, they still make those) or taking a break and playing one of the board games or puzzles we have in stock, we truly enjoy all of you who come in and hang out with us! So, where does the name The Drunk’n Baker come from? Believe me, it’s a question we hear all of the time and are always more than happy to answer. Our bakery specializes in, get ready for it . . . alcohol-infused treats!! Spirits, cocktails, wines and beers; you name it and our bakers have worked it into a sinfully delicious concoction at one point or another. One of our most popular creations is the Time Warp Truffle, which uses the Time Warp Espresso Vodka available from The Painted Stave, another one of Smyrna’s thriving small-businesses. We have also been very proud and delighted to partner with Delaware-based businesses: Blue Earl Brewing Company, Harvest Ridge Winery and Fordham and Dominion Brewing Company for some wonderful events and to incorporate their products into our treats. We really look forward to more pairings as both we and these other great local businesses continue to grow.
