Safety And Results Drive Change In Eye Surgery:

Epi-LASIK And Laser Cataract Surgery As Examples

o No Harm. All medical students learn this most central tenet when training to be physicians. Yet in medicine, as in many fields, choices are regularly made in which risks must be weighed against beneficial effects. Whether balancing treatment options with potential side-effects, such as in cancer drugs or life saving

surgeries, or considering purely elective lifestyle enhancing procedures such as cosmetic surgery or laser vision correction, doctors and patients must have a careful and frank discussion of risks, benefits, and alternatives. In ophthalmology, new technologies have opened up a world of possibilities, but also complex decisions, which require careful discussion and a close doctor-patient relationship. New techniques in corneal transplantation, premium intra-ocular lenses and lasers for cataract surgery, and advanced laser vision correction equipment are available to provide better than ever vision results – but these aren’t for everyone. I have enjoyed practicing in this era of unprecedented advancement in technology. I have eagerly embraced evolution in being first to introduce numerous new eyecare modalities to Delaware- including laser vision correction in 1995, near vision CK for presbyopia,

DSAEK partial thickness corneal transplants, and most recently laser cataract surgery just a few years ago. However, I have never lost “sight” of the importance of a careful analysis of new treatments, their efficacy and their safety, and the needs and goals of my individual patient. A prime example of this balance is Excimer Laser PRK, a radically new technology in refractive eye surgery when introduced in the early 1990s, which offered much more accuracy and safety than its predecessor Radial Keratotomy. Involved in the early research leading up to its FDA approval in 1995, I quickly moved to make this technique available to my Delaware patients. In fact, I initially brought these patients to Johns Hopkins’ Wilmer Eye Institute for treatment (my training institution) prior to laser availability in this state. Since that time, gradual evolution of that technique has enhanced convenience,

safety and accuracy. Changes included LASIK (involving the cutting of flaps to hasten recovery and improve comfort), LASEK (using alcohol rather than flaps to prepare the surface of the eye), and epiLASIK (which altogether avoids cutting of flaps, and instead gently and smoothly removes the outer layers, which then quickly regrow); all laser LASIK is another option, allowing faster recovery but somewhat greater risk in flap creation. It is only with careful and thoughtful conversation between surgeon and patient that the best treatment option can be determined. A more recent example is laser cataract surgery, the fantastic next step in the evolution of cataract removal and vision rehabilitation. This technique is far more accurate than standard cataract surgery, and also allows the reduction of astigmatism, thereby reducing dependence on glasses after surgery. Following FDA approval, my partners and I at the Center for Advanced Surgical Arts, obtained the first laser in Delaware (and one of the first 100 nationwide). While industry experts tout this as a revolutionary and important advancement, it definitely has its specific indications and roles. Once again, a careful evaluation by the surgeon well versed in ALL options and a willingness to explain those options to the patient is important in assuring the best outcome. While technology has provided wonderful new methods and tools to improve outcomes in ophthalmology, now more than ever, thorough understanding between patient and physician is vital to ensuring great and satisfying results.

He completed a cornea and refractive surgery fellowship at the renowned Wilmer Eye Institute of Johns Hopkins. Previously, he graduated magna cum-laude from Harvard, earned his medical degree from the University of Massachusetts, completed internship at Harvard’s Mt. Auburn Hospital, and served his ophthalmology residency at Washington University. Dr. Minkovitz participated in the early research on Excimer Laser refractive surgery, and has offered this procedure to Delawareans since FDA approval in 1996, before Delaware had its own laser. These early pioneering individuals travelled to Wilmer Institute with Dr. Minkovitz, where he maintained his teaching and clinical appointment. In addition to introducing laser vision correction to Delaware, he also introduced NearVision CK to eliminate reading glasses, DSAEK (a newer, less invasive type of corneal transplant) and most recently the newest advance: laser cataract surgery. Dr. Minkovitz is a co-founder of the Center for Advanced Surgical Arts and serves on its medical advisory board. He is also on active staff at Christiana Care. While on staff of Wilmer at Johns Hopkins University, Dr. Minkovitz received the Outstanding Teacher award. He was also named Top Doc in ophthalmology by Delaware Today Magazine six consecutive years, and has been an invited instructor at American Academy of Ophthalmology Annual Meetings.

CK® Testimonial... It finally occurred to me what has been missing during the last 2 days. Since Richard’s surgery, he isn’t complaining about his inability to read something on the fly without searching for his glasses. He wired 6 recessed lights (NO GLASSES), he read the Sunday paper sections A & B and menus (NO GLASSES), and is using the computer (NO GLASSES). I suppose for Richard, needing to look for glasses when he wanted to see something, has been the hardest adjustment of becoming older. His dependence was so gradual that I never related to why he would get so angry over needing them. My house has been quieter. I now have 2 less things to do everyday: find the glasses and make sure they were clean!

This is our 1st quarter issue for 2014, which is our Bridal focused issue. It is always a pleasure to do our front cover photo shoot, and this year was no different. We were extremely honored that we were allowed to do our photo shoot on the grounds of the beautiful Hagley Museum property. Even though we have focused our profiles and cover toward Bridal - this issue is certainly not totally Bridal. February is also Heart Awareness Month, and for many it is the month of love because of Valentine’s Day. In reading the paper you will see that we have many wonderful articles that can help you in finding the right doctor, the best Med Spa, and wonderful gifts of all kinds. Many times we forget that the best gift we can give someone is our love, and also taking care of our health so that we can be with our loved ones for many, many years. On The Cover... I would like to extend a special thank you to the Hagley Properties for graciously allowing us to have our profile photo shoot for this issue at their wonderful properties. We could not have asked for a more beautiful location, and the staff at the Hagley Properties was probably one of the easiest to work with ever. If you are considering different properties for your wedding or reception I would highly recommend this one. I wanted to do an outside shoot, and since we normally do our bridal shoots in the spring or summer I decided to show case the properties in the fall, so that you could see how lovely the weather was and the colors, and I certainly was not disappointed with either. As always we were blessed with a beautiful model for our Bridal Shoot. Jennifer Mumford, RN, CCRN, who works with Bayhealth Medical Center in Dover, DE, was very gracious in volunteering to play our bride. She did admit that since she has never been married that it was a lot of fun, especially when the groom was so entertaining. At the last minute we pulled J. Christian in to our shoot as the handsome groom. Our flower girl was the adorable, and beautiful Ms. Isabella Chrobak, daughter of Laura & Michael Chrobak of West Grove PA. She receives a transfusion every two to three weeks and would not be a vibrant, happy child without them. “I know some people are not sure about giving blood, but I would say ‘just take a

Unfortunately, Ashlynn’s story is not all that uncommon. Every two seconds someone in the United States needs blood. Blood Bank of Delmarva needs 350 blood donors a day to meet the needs of the 16 hospitals on Delmarva. This year, 20,000 patients will have a fighting chance because they received a blood transfusion. About 83,000 units are collected at Blood Bank of Delmarva centers and locations annually. While the numbers tell one story, stories like Ashlynn’s motivate many to give throughout the year. During the winter season, donations typically slow

down because flu and colds are more prevalent and weather issues can hinder donations. The need for blood; however, never takes a break. Blood Bank of Delmarva hopes that individuals will consider taking an hour to give one of the most personal and impactful gifts--the gift of life--to people right here in their own community. Giving blood takes about an hour from start to finish including the review of materials, interview process, blood collection and time for refreshments in the canteen. One pint of blood can be used to help up to three people having surgery, battling serious illness or being treated for trauma.

Also, many times, doctors use O- for blood transfusions for babies because blood typing a baby that weighs six pounds or less is difficult. Therefore, Blood Bank of Delmarva is always in need of type O donors.

With the New Year, Blood Bank of Delmarva and blood centers across the country observe National Blood Donor Month in January. Every day is a chance to thank donors and show appreciation for the lifesaving donation they make, but in January, special efforts are made to let them know how grateful we are for their lifesaving efforts. In January, Blood Bank of Delmarva blood donors will receive special giveaways and tokens of appreciation.

Blood Bank of Delmarva needs all types of blood to make sure the right type is on the shelf at the right time. In most cases, patients are typed before they receive a transfusion so they can receive the same blood type. But there isn’t always time. In trauma situations, medical professionals need to act quickly and often use type O- to save the patients. O- is rare and universal. While only 6 percent of the population has O-, 100 percent of the population

can receive it. Blood shortages usually develop with first O- because it’s rare and is often used as a substitute for other blood types in emergency situations.

Realistic Resolutions

very year on January 1st, millions of Americans become overachievers by setting New Year’s resolutions which typically involve health and weight loss. The bar is set high, but often by February, those goals have been long forgotten. Is it because we’re weak? Lazy? Unmotivated? No—it’s because we are busy. How can we accommodate goals for healthier behavior into our hectic lifestyles? It’s simple: Be realistic in our intentions. Healthier eating doesn’t need to be achieved by eliminating certain foods from our diet. Take a positive approach - what could you add to your diet each day to make it more balanced? Do you eat five servings of fruits and vegetables daily? Do you eat foods containing omega-3 fatty acids, such as salmon and walnuts, regularly? Do you consume enough calcium? Are you regularly eating 25 grams or more of fiber each day? By analyzing what we could be eating for optimal health and adding the necessities, junk foods can be eliminated without intentionally removing them from the diet—there simply won’t be enough room for junk once you’re meeting your nutritional needs. Have you made a deal with yourself to join a gym and spend one hour there every day? It may seem like a big commitment www.TheWomensJournal.com

because it is. Take the pressure out of the physical activity by making a resolution to try many various types of exercise until you find several that you like. Working out in a gym setting isn’t for everyone. Exercise can be incorporated into the lifestyle easily if it is enjoyable. For example, instead of meeting a friend for breakfast on the weekend, meet at a park and walk instead. Consider trying a yoga class. Browse workout routines on-demand. Don’t find yourself in a gym rut which you may cause you to abandon your fitness goals. Keep exercise fresh by incorporating variety.

Remain active Physical activity reduces stress and gives us more energy. If you know you will indulge a bit, increase your exercise time a bit every day. Remember, if you increase your exercise, that splurge won’t ruin you.

Drink lots of water Adequate water intake keeps your metabolism humming along at a maximum rate and staves off hunger cravings as well. Mild dehydration can mimic hunger.

Being flexible in our expectations can allow us to be creative and enjoy working towards our goals. This year, set realistic resolutions and be prepared to have fun along the way.

3T MRI. Worth It! (*nephrogenic systemic fibrosis—a very rare, and incurable, side effect).

RI Consultants has provided 3T (tesla) state of the art MRI for 7 years now. Most doctors are unaware of 3T’s superior imaging which can make a huge difference to your care. Basically 3T MRI improves all MRI images because the scanner has twice the signal to noise of a 1.5T scanner and 10 times the signal to noise of a 0.3T OPEN MRI. We are the only MRI specialists with two 3T MRI scanners. If a doctor asked me for real world differences between scanning at 1.5T and 3T, I would say, it is diagnostic certainty. All radiology studies show that 3T MRI is better. The extra detail and speed gives us a more definitive picture. It shows more lesions in almost any brain abnormality. It especially helps MRA (angiography) studies. The 32-channel head coil in our Lewes office, combined with 3T magnet strength, generates the finest MRI brain scans in Delaware. Just to compare to a 1.5T scanner for MS—we do a sagittal FLAIR image which has 20 images (with a 50% gap) in 4 min. On our 3T scanner in Newark we do a 128 slice 1mm contiguous study—or 6 times more images in 7 min. On our Lewes scanner we do 448 images (20 times more pictures) with 0.6mm overlapping slices in 4 minutes. It is the remarkable 32 channels (*the best technology currently available) that speeds things up. 3T is crucial for accurate evaluation of MS. And, it does not cost any more. 3T MRA shows vessels about three times more detailed in 1/3 the time

The 32 channels also let us do remarkable studies like susceptibility weighted images (SWI)—which show microscopic hemorrhage (in 6 min) and super high resolution MR angiography in only 4 min. We have learned that MANY brain pathologies involve microscopic hemorrhage. This can completely alter stroke or tumor care. 32 channels allow diffusion tensor imaging in as little as 2 min, which shows the subtle changes in brain trauma or a NEUROQUANT study in 10 min. It was shown (by NYU radiologists) that a quantitative MRI shows the effects of a single blow to the head one year later. An MRA of the head takes only 4 minutes.

Even our “open” MRI is an 18channel 1.5T MRI scanner! (Ask your MRI provider how many channels his closed magnet has). Because of its 100% sensitivity to Breast Cancer, we prefer to do our Breast MRI’s on our 3T MRI, but for someone who either will not fit, or who is extremely claustrophobic, our 18-channel 1.5T OPEN BORE MRI is a good alternative. Getting a bad MRI costs no less then getting the very Best MRI possible … except for the time it will take you to repeat an inferior study. By having a better MRI study you get a more accurate diagnosis. Ask questions. Get the best. We also have 29 years experience doing MRI procedures.

Our facility in Lewes offers proton spectroscopy—a revolutionary chemical biopsy of tissue done with 3T MRI. In older patients, this is a free test for evaluation of Alzheimer’s disease versus multi infarct dementia. Basically, it can tell us if a patient has Alzheimer’s dementia and how much it has progressed. It can be used to monitor the disease over time. It is EXTREMELY useful. This is an important thing for most patients over 65 and in some patients’ families it is a good baseline and monitors for early onset of Alzheimer’s. It costs nothing more when added to a routine contrast MRI exam. It can show the difference between a stroke and a possible tumor or metastasis. Proton spectroscopy is also good for evaluation of prostate and breast cancer. It is free when added to a contrast MRI study. It can guide a biopsy or make a biopsy safer by telling us if the lesion we wish to biopsy is cancerous. With prostate cancer we also do a DTI study of the prostate in addition to the MRSI (two dimensional) proton spectroscopy. The DTI study provides apparent diffusion coefficient values which can be directly correlated to the Gleason stage of the prostate cancer. This additional information helps guide the urologist where to biopsy the prostate gland.

astrointestinal problems are among the most common reasons people seek medical care. Symptoms range from mild: bloating, cramping, gas, indigestion, constipation, and abdominal distention to severe: bleeding, blockage, severe inflammation, or infection. Left untreated, a sick gut can morph into chronic disease affecting the immune system and overall health and well-being. While many of the symptoms of a dysfunctional gut are localized, there are other disease states that are directly related to or affected by gut function. These include heart disease, cancer, auto-immune diseases, mood, depression, allergies, and skin disorders. Our food and the way we eat has changed more in the last 50 years than it has in the last five thousand. High-fructose corn syrup, corn-fed factory-farmed meat, preservatives, geneticallymodified wheat, vegetables, and grains, processed/packaged foods, and hormone-injected dairy are all examples of foods that are being consumed in great quantities like never before. Discovering your toxic triggers is key to healing your gut. Not everyone will have the same toxic triggers but there are patterns. Dairy and gluten are two of the most common. Some others are corn, soy, grains, wheat, alcohol, caffeine, and processed sugar. The best way to uncover the foods that are problematic for you is by testing them. One approach to finding your offenders is to adhere to a “clean” diet for 3 weeks. A clean diet consists of eliminating the questionable foods and ingesting only vegetables, good protein, good

After three weeks, re-introduce the questionable substances one at a time for two to three days and monitor your reaction to each. It is important not to incorporate more than one potential toxic trigger at a time because if you have a negative reaction, you will not be able to discern which food is responsible. Note: It is helpful to keep a journal to record your diet and any undesirable response that develops. Once you’ve determined your toxic triggers, you can decide whether to eliminate them or greatly reduce your intake. This process will be done on an individual basis taking several factors into consideration. First and foremost would be how you feel eating “clean” and how negatively you are affected when the foods are re-introduced. Other determining factors are discipline, convenience, time-management, stress, support, expense, etc. “Old habits die hard.” Making dramatic changes in your diet can be very challenging. Today’s fast-paced, instant-gratification, mass production world does not make eating good, clean, nutritious food easy or easily affordable. Unfortunately, just the opposite is true. However, successfully altering this aspect of your lifestyle might produce benefits that far outweigh the obstacles. If you are suffering from any of the symptoms listed above, it might be worth your while to talk to a nutritionist or healthcare professional knowledgeable about gut function and a healthy diet. Additionally, supplements to aid in digestion, restore gut flora, and address nutritional deficiencies will most likely be recommended.

When Implants Are Not An Option R

eplacing missing teeth when implants are not an option is a challenging reality for many patients. Some individuals may not be candidates for implant retained tooth replacement for a variety of reasons, including poor health or having insufficient bone to support an implant (even with all of our current bone growing techniques). The other non-removable method of tooth replacement, fixed bridges, is only possible if there are healthy teeth on either side of the space and the space is not too long.

Today we will look at tooth replacement using a removable partial denture. Removable partial dentures replace one or more teeth as well as lost/resorbed gum tissue. Replacing the missing teeth facilitates better chewing ability, stability of the bite, and esthetics. The challenge is to make the removable partial denture

strong enough to withstand chewing as well as torqueing forces caused by placement and removal of the denture, and have it still be comfortable. There are many more choices today with the advent of new materials. Each has advantages and shortcomings. The dentist and laboratory technician survey molds of the mouth and photos of the teeth and face to arrive at the best design for the specific patient. Removable partial dentures need some mechanism to stay in place, usually clasps which hug select remaining teeth. Clasps may be metal, tooth color coated metal, clear or invisible. Invisible clasps, known as precision attachments fit within the confines of specially designed crowns secured to the supporting teeth. When possible, this is a very good option. Esthetically it is superior and forces are directed down the long axis of the abutment teeth where the shock absorbing ligament system is most effective. Unfortunately this is the most costly option and requires teeth with a good long-term prognosis. Clear clasps, when mechanically possible, blend very nicely with remaining teeth. Careful design can even minimize the visibility of metal components which exceed all other choices in the strength and longevity category.

Major connectors, the supporting framework of the denture, can be made of a variety of metals or plastics. Plastics generally are thicker and more prone to breakage. The advantage is that teeth may easily be added should additional teeth be lost in the future. Some newer materials afford a very thin and cosmetically pleasing major connector but are flexible and difficult to repair or alter. A new partial denture which has both a clear major connector and clasps is available. This type has limited applications and is fragile. Botox® vs. Dermal Fillers: Which One is For You?

Botox® and dermal fillers (Belotero®, Radiesse®, Juvederm®, Restylane® etc.) are different products for treatment of skin wrinkles and folds. Skin Wrinkles and Folds which occur naturally as we age, can develop through two mechanisms: through repeated muscle overactivity, or, loss of skin fullness due to depletion of fat, connective tissue and elasticity. Muscle overactivity during facial expressions like frowning or smiling, will form wrinkles over time. It often occurs at the forehead, between the eyes, or at the outer eyes (crow’s-feet). On the other end, loss of skin fullness results in skin sagging and development of folds. It is common at the cheeks resulting in Nasolabial folds or Marionette lines. Loss of skin fullness can also lead to skin depression like the dark

How These Drugs Work? A simple way to visualize the difference between Botox® and fillers is to think of a skin crease as a valley between two mountains. Botulinum toxins prevent the mountains from getting closer to each other; fillers fill-in the valley.

Fillers are used not only to “lift” the skin beneath wrinkles, but also to reshape and enhance facial structures such as filling-in acne scars, enhancing cheekbones, restoring the fullness of lips or filling-in hollows dark circles under the eyes. When to Use what? If creases are • apparent only when we squint, only Botox® is needed.

Botox® contains an agent that in large quantity paralyzes nerves and muscles. The minute quantity that is injected into a facial muscle will just weaken and relax it so that it won’t overwork and will slow formation of wrinkles. It can also be used to treat neck bands and lines and for armpit and hands sweating. Furthermore, as facial muscles relax, the patient gets out of the habit of creasing and overusing those muscles. Fillers usually contain hyaluronic acid, a naturally occurring substance. The difference between fillers has to do with thickness and longevity. Generally,

In some cases, usually after menopause, your doctor may suggest one of a variety of medications to prevent fractures that are available while helping weigh the benefits and risks.

steoporosis is a serious disorder that can keep a woman from enjoying an active lifestyle, especially as she grows older. It is a disease in which the bones lose their normal architecture, become extremely soft and are subject to fracture and slow healing. This can occur especially in women following menopause. Certain medical conditions, such as an overactive thyroid gland, can further contribute to decreased bone strength. Unlike some other disorders, osteoporosis doesn’t begin with obvious symptoms. That’s why it’s especially important for women to develop good habits when they are young to help keep their bones healthy so they can remain vibrant and fit well into their golden years.

Bone density is a medical term referring to the amount of calcium and other mineral matter per square centimeter of bone. Bones should be dense, so they don’t easily break. One common way to measure bone density is with a simple imaging test known as dual photon densitometry or DEXA. If a woman isn’t getting enough calcium, her bones can be weakened. The impact can be devastating, resulting in fractures that can rob a woman of her mobility and even shorten her life expectancy.

Good habits make a tremendous difference in bone health. Daily diet plays an important role. Every woman should know her daily calcium intake and make adjustments to ensure she is getting enough to at least replace her daily loss. The minimum daily requirement for calcium intake varies according to a woman’s age and various specific co-existing medical conditions. In general, women who are pregnant or breastfeeding require 1000-1200 milligrams of calcium per day. Premenopausal women who are 20-50 years old and post menopausal women on estrogen therapy need 1,000 milligrams of calcium per day. Post menopausal women who are not on estrogen therapy should strive for 1200 milligrams per day. (Even men need calcium in the 1000-1200 milligrams range, with the actual amount depending on age, so give your spouse, brothers and dad a friendly nudge, especially if they are seniors.) On the flip side, one should also remember that too much calcium can contribute to other problems.

enjoy yogurt. Choose orange juice fortified with calcium. Always remember, however, that your most ideal source of calcium is from natural dietary intake, not supplements or fortified liquids. Along with calcium other dietary minerals including magnesium, phosphorous, boron and manganese are thought to also contribute to bone health. Vitamin D helps the body absorb calcium and is naturally produced in the skin with exposure to sunlight. As we age, our skin tends to produce less vitamin D. Vitamin D is not found in many dietary sources but is present in certain fish like wild salmon and is sometimes added to milk, juice, or cereal whenever you see labeling like “fortified with Vitamin D.” In winter, when it gets dark early and we don’t spend as much time outdoors, Vitamin D supplementation is especially helpful. General guidelines are for 600 international units daily below the age of 70, and increasing to 800 international units over age 70. Smoking and more than moderate alcohol consumption are associated with lower calcium absorption and potentially lower bone mass or density. Too much caffeine can be

What may occur, however, is the hip bone has become so fragile that it fractures on its own and the woman then actually falls because of the spontaneous fracture. Other particularly vulnerable bones include the vertebrae in the spinal column, the wrist and the upper arm. Regular exercise can help prevent these types of injuries by also helping to maintain muscle mass and tone. Inactivity can cause you to lose as much as 10 percent of your body’s muscle mass with each decade of life after age 40. This loss of muscle mass can result in a condition called sarcopenia. Even if you don’t think of yourself as athletic, you can get on the right track with a simple weight-bearing exercise program because muscle tone as well as muscle strength provide stability to the way we walk or get around. It’s never too late to start! Always remember, whenever other medical conditions are also present, always first seek the advice of your primary care physician. You might begin by taking a brisk 30-minute walk each day. As you grow stronger and more confident, you can ramp up your exercise

routine. Start wearing 3-pound wrist weights when you take your walk. Then work up to 5-pound weights. Make multiple forms of fitness training part of your routine. Ride your stationary bike while you watch the news. You will build strength and endurance -and the muscles you are toning will support and stimulate your bones. You might find that you enjoy exercise so much that you add weight training to your regimen. Women who maintain a consistent exercise program will find over time that they can even regain some of the bone mass they have lost. The History Of Amalgam Fillings Dental Amalgams were first introduced in 1833. Why Is Mercury In A Filling Dangerous? One of the primary concerns is that any time mercury amalgam fillings are stimulated, Mercury gas is released and continues for up to an houran-a-half after the stimulation occurs. That means that when you eat or drink, mercury vapors are released into your system – and there is as much mercury in the average filling as in a thermometer – enough mercury in one large filling to contaminate a 10-square-mile lake and cause restricted fishing. And according to the International Academy of Oral Medicine and Toxicology, just having your teeth cleaned releases 1,000 amounts the EPA allows for the air we breathe. When these vapors are released into your mouth by chewing or other stimulation, the mercury is absorbed into your body fat, liver and spleen. It even crosses the blood brain barrier, depositing mercury in your brain. It all adds up to one thing – mercury fillings may be the leading cause of mercury toxicity in the United States.

What Is The Current Clinical/Political Policy Trend Since the FDA classifies mercury as a poison – a volatile one – it shouldn’t be surprising that the dental use of mercury is a controversial topic. Some dentists and dental organizations maintain that the use of mercury in dental amalgam fillings is perfectly safe, while others, including several European countries, assert that Mercury is too unstable to be placed in anyone’s teeth. But mercury does not only affect human health. Great concern has been expressed over the environmental effects dental mercury can have. Because of this, the use of mercury-based dental amalgams has been banned in Sweden, Norway, and Denmark. All this isn’t new information. A 2003 monograph on toxicity from the World Health Organization concluded that “Studies on humans and animals have demonstrated that dental amalgam contributes significantly to mercury body burden in humans with amalgam fillings.” Yet, many dentists continue to employ mercury amalgam as their primary restoration material, and they do it with the blessing of the ADA!

Vitamins And AMD • Age-Related

Eye Disease Study 2 • (AREDS2) Results By: Paula C. Ko, MD

A

ge-Related Macular Degeneration is the leading cause of blindness in the United States in patients over 60 years of age. There are so many people over age 60 that it is the leading cause of blindness in the United States overall. The original AREDS study (Age-Related Eye Disease Study) was first published in 2001. It was sponsored by the NIH (National Institutes of Health) and showed that the following vitamins were beneficial to the treatment of moderate to severe age-related macular degeneration. The original AREDS formulation included Vitamin A 15 mg (Beta Carotene) Vitamin C 500mg Vitamin E 400 IU Zinc 80mg Cupric Oxide 2mg When taken together these vitamins decreased the progression to advanced AMD from moderate to severe AMD by 25%. The long-term positive affects have lasted up to 5 years and were most notable in patients with the wet form of macular degeneration. Because of this study, ophthalmologists recommend that patients who have moderate to severe AMD take AREDS vitamins whether or not they have the wet or the dry form. Despite this, there have been other vitamin-based studies that make it difficult to recommend these high-dose vitamins in some patients. For example, it has been noted that the risk of lung cancer is increased in smokers who take high doses of vitamin A. Because of this we do not recommend that smokers take the AREDS vitamins. Patients that have AMD are counseled to stop smoking to decrease the risk of progression of AMD. Even if patients stop, they should not take high doses of vitamin A as the risk of lung cancer still remains high even after quitting smoking. We also do not recommend high doses of zinc to men who have problems with Benign Prostatic Enlargement (BPH) as zinc can compound this problem. Lastly, it has been shown that vitamin A may be problematic in patients with osteoporosis, decreasing bone density and increasing the risk of fracture. With all these other studies in mind, we as ophthalmologists must consider many other factors when recommending the AREDS vitamin to patients with moderate to severe AMD. Patients with AMD not uncommonly have other medical issues as these other diseases are also commonly found in patients over the age of 60. When recommending vitamins to a patient, all medical issues must be taken into consideration. The AREDS2 study was designed to test whether adding lutein (10mg), zeaxanthin (2mg), and omega-3 fatty acids (1gm) with or without the AREDS original formulation would be of additional benefit in reducing the development of advanced AMD. It was

thought that perhaps the AREDS2 formula could have more affect as well as be a safer vitamin. Lutein and Zeaxanthin are pigment found in the macula. The macula is part of the retina that allows people to recognize people’s faces, drive and read, in another words, it is the part of the retina that allows for central vision. It was felt that Lutein and zeaxanthin were a safer substitute for vitamin A. Because of the risk of vitamin A and lung cancer in smokers, vitamin A was eliminated from the AREDS2 formula. The dosage of zinc was also reduced because the side affects on BPH as well as the fact that such a high dose of zinc may not be absorbed fully. The findings of the AREDS2 Study were recently published this year and showed the new formulation will also decrease the rate of progression to advanced AMD by 18% and wet AMD by 22%. It did not help the dry form of AMD called geographic atrophy. This new formula does not work any better than the original AREDS formula but it is felt to be a safer supplement. The positive affects of taking omega-3 fatty acid (fish oil) was not recognized in the study. It is still recommended that patients eat fish 2-3 times a week. Fish is beneficial not only for the eyes but for the heart. In addition, a diet rich in green leafy vegetables also can be beneficial in treating AMD. These diet recommendations are highly recommended. It should be noted that taking vitamin supplements do not substitute for a healthy diet.

Not Only Your Big Veins Matter, The Little Ones Too! I

often reflect back on my experience at First State Vein and Laser Center, thinking about what matters most to my patients. Obviously, patients that have “big” unsightly, bulging, winding painful veins are concerned about them. But I realize that it’s the “small” things that really matter to the majority of the patients. “Small” things like reducing downtime, lessening discomfort and the possibility of scarring - matter. To most, it is also the small veins that frequently matter. Even after ablation (closure) of an incompetent great saphenous vein, the source of their initial complaint, I am often asked what can be done about the remaining bulging vein, or about the small stubborn blue-purple spider veins. Obviously, we are concerned about life threatening D.V.T. (deep vein thrombosis) or leg ulcers. But for most patients, it is irrelevant; they don’t have them. Most are bothered by non-critical vein problems. As ‘vein doctors’ we should address the entire spectrum of vein problems, should not ignore patient’s strong wishes and treat what matters to them - the big and the small veins.

What is Venous Insufficiency?

Uni-directional valves are essential to insure the flow of blood from superficial to deep veins and from the legs to the heart. These valves are present in ALL veins, the tiniest and the largest alike. When these valves leak or become incompetent, the blood flows backwards, pools within the veins and weakens their walls. This condition, called venous insufficiency, may occur in veins of all sizes. In large ‘trunks’ like the great saphenous vein, this will result in what is commonly known as varicose veins, while in smaller (2-3 mm) veins under the skin (reticular veins) it will produce spider veins.

Spider Veins - Often Part of a Bigger Problem As mentioned above, leaky valves in reticular veins result in spider veins. These can be isolated phenomena. But spider veins can also be a manifestation of a deeper, larger vein’s insufficiency that over the years has gradually affected more superficial veins and eventually, reaches the reticular veins. A patient who comes in for the treatment of spider veins, should be first assessed to determine whether these are an

isolated issue, or part of a broader and deeper, problem.

Isolated Spider Veins Can Be Treated By:

Sclerotherapy – closure of the veins by injection of a chemical solution called sclerosant, ideal for spider veins 0.8 - 1.2mm in width. Ohmic Thermolysis - works by a heat generated by radiofrequency device called “Vein Gogh”, best for spider veins smaller than 0.8mm in width. L.H.E. Therapy – combined Light and Heat Energy Technology – effective for very tiny veins. When spider veins are a manifestation of an underlying large veins venous insufficiency, which can happen even without the presence of large varicose veins, the underlying problem should be addressed first.

ongestive heart failure (CHF) is a major public health problem and is the number one reason for hospital readmission in patients aged 65 and over. Nearly five million Americans are affected by congestive heart failure with 550,000 new cases diagnosed per year. Congestive heart failure can be classified into two groups, systolic (inadequate pumping function) and diastolic (inadequate relaxation). In all, about 70% of patients have heart failure related to coronary artery disease (ischemic cardiomyopathy) with

the remaining 30% being nonischemic with responsible factors such as hypertension, viral infections, pregnancy, thyroid disorders, arrhythmias, and sleep apnea.

The Cardiology Physicians’ Heart Failure Clinic is designed to meet the challenges of caring for this growing population of patients. Furey, Bachelor of Science D.O., F.A.C.C. Degree in Biology. During his four years at Lafayette, he lettered in Varsity Basketball and was Captain of the 1983 squad. Learn The Facts About Bladder Slings

A

lot of women needlessly endure stress urinary incontinence (SUI), a condition in which they experience an involuntary loss of urine when they laugh, sneeze or exercise. In fact, SUI is the most common form of incontinence in women, impacting 28 percent of women age 30-39 and about half of women over 50, according to a recent University of Washington survey. There are many treatment options for this condition, both surgical and nonsurgical. One of the surgical options is a sling operation. This is an effective procedure involving the placement of support material through small incisions in the vagina.

There are many types of slings, most of which have been used successfully for more than 15 years. However, concerns have been raised recently about complications associated with synthetic slings. In 2011, the U.S. Food and Drug Administration issued an alert on the use of some types of slings for incontinence surgery. Consequently, we often see patients in our office who are frightened at the prospect of getting any type of sling to resolve their issues with leaking. However, there are a number of slings that are highly effective and have very low risk.

Learn the facts

When considering any kind of surgery, you

shouldn’t be shy about asking all the questions you would like answered. To get the information you need to make an educated decision about slings, talk to a urogynecologist, a specialist who is trained in both female urology and gynecology. Urogynecologists are focused on finding solutions for women who suffer from leaking and other problems involving the bladder, uterus and rectum. A vaginal sling is a sort of hammock. It provides support for the urethra, the tube that carries urine from the bladder. That support keeps the urethra from opening and spilling urine when you do jumping jacks, have a coughing fit or do other activities that result in leaking.

The mesh is made from polypropylene, a form of plastic. It has been used safely for more than 30 years in many surgical procedures, such as hernia repairs and vascular surgery.

Know your options

According to the FDA, the use of slings to treat SUI is less invasive than repairs that don’t involve slings, which require a larger incision in the abdominal wall. Because there is less cutting of tissues, patients can expect less pain and a quicker recovery. Sling procedures involve tiny cuts about the size of an eraser head. So there is very little scarring. The outpatient procedure for the slings takes about 20 minutes and is performed under anesthesia. Within 36 hours, most women are back on their feet, feeling as

if they have not had surgery. The time for a full recovery varies with the individual, from a couple of days to a few weeks. After they recover, about 85 percent of women will be able to jog, sneeze or laugh at good joke without leaking. So is a sling right for you? he modern world is becoming increasingly complex, and the demands on our time and attention are many. Our jobs are also more complex, By: and the business world Dr. Katie Schuler D.C. has become more competitive. With the industrial age came helpful machines and increased productivity, but it also brought us the jackhammer, the loudspeaker, the motorcycle and car, and innumerable noisy machines. Physical stresses such as sound, air and water pollution have worsened over the last century, especially in the United States. We are also, because of electronic media and the Internet, aware of troubles and tragedies around the world that previous generations would never have known about, creating anxiety. Taken together, these assaults on our senses create a physical and emotional reaction. The word for this overwhelming feeling is stress. It’s term borrowed from the field of engineering, in reference to physical forces upon mechanical structures. The term was first used in its modern medical sense around the turn of the 20th century by Walter Cannon, a Harvard physiologist. Cannon also first described the “fight or flight” syndrome, and noted the damaging physical effects of a continuous stress response. Modern healthcare is increasingly recognizing that many illnesses are caused by stress, or worsened by stress. In fact, in every chiropractic office, many patients will show the ravages of chronic stress. While chiropractic can treat the effects of stress on the body, such as headaches, it can also help the body manage and process stress in a healthy way.

How We React to Stress

We perceive stress from three basic sources: our environment, our body and our emotions. Environmental stress includes noise, weather, physical threats, time pressures, and performance standards. Body stress includes disease processes, organ malfunction, poor nutrition, poor sleep, and physical injury. Emotional stress is more difficult to define. It encompasses our reactions, in thought and emotion, to environmental and physical stressors. Criticism, for example, is devastating to some, while others seem unaffected. In many ways, however, we are each in control of how much emotional stress we will feel. Relaxation techniques, meditation and contemplation, hypnosis, and similar approaches can all have a strong mediating affect on stress. These techniques provide tools that allow one to gain greater control over reactions to stress, and therefore reduce the overall effects of a stressful event.

All of this is very useful if we’re running from a prehistoric raptor, or confronting a more modern threat to physical safety. When prolonged, however, the long-term effects of this state can be disastrous to good health. Many studies of people who have been subjected to chronic stress have found evidence of the negative health effects of stress. These effects include high blood pressure, damage to muscle tissue, diabetes, infertility, damage to the immune response, and slowed healing from disease and injury. Stress reactions are also at the root of disorders such as post-traumatic stress disorder, and stress has been linked in human and animal research with cardiovascular disease.

Chiropractic Treatment and Stress

Chiropractors work primarily with the spine, the root of the nervous system through which nerve impulses travel from the brain to the rest of the body. One effect of chronic stress is prolonged muscle tension and contraction. This muscle tension creates uneven pressures on the bony structures of the body, often leading the misalignment of the spinal column. Chronic stress also leads to nerve irritation. The adjustments of a chiropractor release muscle tension, and that helps the body return to a more balanced, relaxed state. Adjustments also reduce spinal nerve irritation, and improve blood circulation. These changes may be enough, in many cases, to convince the brain to turn off the fight or flight response, beginning the process of healing. A healthy and balanced spine is one key to effectively managing stress. Chiropractors are trained in nutritional and other therapies for stress. Several nutritional supplements, including B vitamins, help the body cope with stress. A Doctor of Chiropractic may also recommend relaxation techniques, and discuss posture and environmental changes to help recovery from chronic stress. A chiropractor cannot make a job less stressful, or create a quieter, calmer world. What chiropractic treatment can do is help you develop healthy responses to stress, reducing potential physical damage.

Focus On Your Waist, Not Your Weight

our waist circumference is a better health indicator than your weight or BMI (body mass index). It can be considered the fifth vital sign, after By: Dr. Melissa P. Abdominal obesity, defined as a waist circumference greater than or equal to 40 inches in men and 35 inches in women, is linked to Type 2 diabetes, cardiovascular disease, breast and colon cancer, and fatty liver. Therefore, it is imperative to focus on your waist, not your weight. One way to maintain your waistline is to stay away from processed foods, especially those containing high fructose corn syrup. High fructose corn syrup has similar effects on the body as alcohol. Just as you can develop a “beer belly” from alcohol, you can develop belly fat from high fructose corn syrup and other processed sugars. In addition, high fructose corn syrup, like alcohol, causes your liver to become fatty. When food shopping, stick to the perimeter of the grocery store. That is where you will find less processed and more whole foods. Choose food both for personal satisfaction and for nutritional value. Another way to a healthy waistline is exercise. After menopause, a woman’s testosterone level declines. Testosterone helps maintain muscle mass. The lack of testosterone and a diet of processed foods are the main reasons for the “menopause middle.” Therefore, exercise is not only critical to burn calories, but builds abdominal muscle mass. Yoga, Pilates, running and swimming are great choices to slim your waistline. Many studies have linked obesity to insomnia. Research shows that inadequate sleep (less than six hours per night) can lead to fat deposits in the belly. So, adequate sleep can improve your waistline. In addition to good sleep hygiene, consider chamomile tea or other calming herbs before bedtime. It is also important to maintain a healthy waistline to prevent sleep apnea (periods of not breathing while sleeping). Sleep apnea is associated with high blood pressure, cardiovascular disease, heart failure, insulin resistance and systemic inflammation. One of the most important things to focus on for a healthy waistline is stress reduction. Stress stimulates sugar production. The more stress the body experiences, the more sugar gets excreted from the liver. As a result, your waistline increases. Also, many

people overeat when stressed as it is their primary way of coping. To avoid overeating, learn to distinguish between physical and psychological hunger. Stress reduction techniques like breath work, biofeedback and meditation are beneficial modalities as well. In summary, by eating whole foods, exercising, sleeping well, and reducing stress, you will not only reduce your waistline, but will also decrease your risk for Type 2 diabetes, cardiovascular disease, cancer and fatty liver. So remember, focus on your waist, not your weight.

I wanted to explain this to you, because many folks do not realize they have this PIP coverage and can obtain therapy for their injuries. Most folks live in fear that their insurance premiums will increase and do not obtain therapy for their car accident injuries. They continue to live with their aches and pains. The Auto Industry has been making safer cars with stronger cages and more flexibility to absorb the shock and protect the drivers and the passengers. However, drivers and passengers still have “unseen injuries” known as “soft tissue” damage. The impact of the collision still goes through the bodies and impacts the muscles and soft tissues. From a Massage Therapy viewpoint, the muscles can be massaged to help release stored trauma and tension from this impact. I believe the Motor Vehicle Accident (MVA) clients have endured enough pain. Many techniques are very aggressive and go deep quickly before the upper layers have been released. This may cause unnecessary pain and damage to the tissues. The real source of the problem may “hide” due to “fight and flight” syndrome. The tissue may have a harder time to heal itself. I

n every yoga class that I teach I emphasize the importance of breath to my students. “Disease cannot live where there is oxygen,” I always attest as I encourage students to flood the body with fresh oxygen in full yogic breaths. Far from rote catch phrases, this principal could be the most important, life changing action. Consider the fact that we can go without food for weeks, and without water for days, but cannot go for a few minutes without oxygen. As the important yogic text Science of Breath simplifies, “To breathe is to live, and without breath there is no life…Life is but a series of breaths.” (Ramacharaka 2008) But we all know how to breathe. It’s innate, right? True, we are born with the ability to breathe, but how to breathe fully, completely is something that most Westerners forget how to do. Shallow breathing that is commonplace from stress and fastpaced lives poses a serious threat to health. Yoga in itself is beneficial because the physical movement creates oxygenation. Many styles of yoga, including Kripalu-based and VaihAyasa Aerial Yoga, are breath-based practices, that not only teach you how to breath properly, but that also offer techniques to more fully permeate the body with fresh oxygen. Dr. W. Spencer Way writes in the Journal of the American Association of Physicians,

The significance of oxygen to health is not a new discovery. In 1931 Dr. Otto Heinrich Warburg received the Nobel Prize for proving that cancer cells cannot live in the presence of oxygen. The cancer cell is the “arch criminal” of cancer. It is a common cell, nothing very special until it begins to change from lack of oxygen. Dr. Warburg showed that the “cause of cancer is the replacement of the respiration of oxygen in the normal body cells by a fermentation of sugar”.

In addition to the health connection of the physical act of breathing properly, along with the physical movement of asanas (yoga postures), breath is importantly linked into many schools of yoga as a source of mental clarity, stillness, and connection to a higher source. In the yogic text Hatha Yoga Padipika, it is said that “When the breath wanders the mind also is unsteady. But when the breath is calmed the mind too will be still, and the yogi achieves long life. Therefore, one should learn to control the breath.” There are many schools of yoga that offer an interconnection of breath to postures, and philosophy. Predators, Prey & Peace

I

write these articles knowing that the people reading it will probably have little to no experience with horses. I write these articles because of this assumption - in the hope that I can prevent the cycle of violence that is so harmful to horses and humans, that seems to occur way too often, causing harm to one or both parties. So if you’re new and wondering what you can do to help prevent the cycle of violence I would recommend three things: 1. Realize that when people are interacting with horses in a situation that is physically and/or mentally challenging, just the shear size of a horse can make this experience overwhelming to a human. When a human feels overwhelmed by a horse they tend to have an emotional response which basically means that they will respond in predatorial manner. When a predator interacts with a prey animal, the prey animal tends to react usually at first by attempting flight. If flight fails, the next reaction tends to be fight and the final response is what I refer to as freeze. Freeze simply refers to the catatonic mental state a horse will achieve as last ditch effort to accept the unacceptable. As you can see this usually creates a cycle. This cycle escalates and becomes violent. To be honest, this cycle of

violence is the norm in the equine world. It’s sad, it’s wrong; it’s far from what most people expected when they entered the horse world. 2. You’re doing it right now! Read, learn, and understand what you’re stepping into. Horses are powerful, dangerous, expensive, beautiful, peaceful creatures who can offer you more pain and pleasure then you ever dreamed. I can guarantee you that if you have a hole in your life a horse can fill it, but you owe it yourself and the horse to get educated before you become part of the norm. 3. Observe yourself and the horse closely. If something seems not quite right, I am here to tell you it’s not. Discontinue and find a way to fix the relationship. Riding horses is fun and fulfilling and should be for the most part, always that way for the horse and rider. Remember life is short, so have fun!

He took such care to get my horse physically comfortable in the first three weeks by trimming her feet, giving her massages. Anything he could think that would help, he did. He could have physically pushed my horse, took my money for a month’s worth of training and sent us on our way, but he didn’t! This man cares about the horses and he cares about the people who own these horses that come through his gates. He has a plethora of knowledge and he loves to share it. He spent hours talking to me and sharing the lessons he has learned in his career…and he does it with passion! He is teaching me how to speak horse and to translate the information my horse gives me and to have a relationship with Onyx. It’s been a beautiful experience to have Dean train me and teach my horse and I’m excited to say my journey isn’t over. I’m continuing lessons with Dean and he is also continuing to trim Onyx’s feet to keep her sound. ~ Donna Austin, Clayton, DE

n 2010, the Council of Chief State School Officers and the National Governors Association Center for Best Practices introduced the Common Core State Standards, which provide teachers and parents with benchmarks for what their students should learn grade by grade from kindergarten through 12th grade. The goal of these common standards is to make sure all students across the country are prepared for college and careers when they graduate high school. To date, 45 states, the District of Columbia and four U.S. territories have adopted the Common Core State Standards. By now, you have probably heard about these standards and know that they will impact your child’s education. What can you expect? Here are several things you should know about the Common Core State Standards: The Common Core State Standards raise the bar for student learning. The standards were created based upon the best state standards in the country and will take all states’ standards to the next level. The highest international standards (of top performing nations) were also

The states that have adopted the Common Core State Standards will soon have computerized common assessment tests (aligned to the standards) that will replace other state standardized tests. The new tests will be administered to students in the 2014-2015 school year. Math standards consist of counting, operations and algebraic thinking, numbers and operations, measurement and data, and geometry for K-5; ratios and proportional relationships, the number system, expressions and equations, functions, geometry, probability and statistics for middle school; and number and quantity, algebra, functions, modeling, geometry, probability and statistics for high school.

The Common Core State Standards promote consistency and collaboration. The standards encourage fairness in education by ensuring all students – no matter where they live – are well prepared

to compete with their peers across the country and around the world. They also allow states to collaborate on things such as teaching tools and strategies for different types of students, textbooks and other teaching materials, assessment systems and more. In the wake of this move toward more rigorous academic standards, Huntington Learning Center is committed to helping our students succeed in school. The answer is false. Although it is true that the right hemisphere controls the left side of the body and the left hemisphere controls the right side of the body, the overall functions may seem similar but are different. For example, the left hemisphere controls the fine motor skills while the right hemisphere controls the gross motor skills. Both hemispheres control the muscles but each targets specific characteristics. The left hemisphere sees the small picture while the right hemisphere sees the big picture. The left side handles math calculations while the right side handles math reasoning. Below is a list of some of the traits associated with each hemisphere.

The two hemispheres must work together to accomplish a seemingly simple task. Being able to add and subtract (math calculations – left hemisphere) is great but being able to apply and interpret the calculations (reasoning – right hemisphere) is more meaningful. The ability for the two hemispheres to work together is crucial.

speech/articulation, poor memory for details/facts, task avoidance, weak immune system, misses small details, poor math operations, poor self-esteem, and poor motivation. Individuals with these traits are often diagnosed with dyslexia, processing disorders, or learning disabilities.

So what happens when there is an imbalance and the two hemispheres do not work together in an optimal manner? This can happen when one hemisphere has developed typically while the other hemisphere is under developed. When this happens, the characteristics of the stronger developed hemisphere are exaggerated. A person with a strong left hemisphere pays more attention to details, is very orderly and organized but will be more hyperactive, unable to focus, and clumsy. When there is a significant gap in development between the two hemispheres, we will see traits often associated with ADD/ADHD, dyslexia, learning disabilities, ASD, OCD and other neurological conditions.

Conversely, someone with a right hemisphere delay may exhibit hyperactivity, poor gross motor skills, lack of self-awareness, poor attention and focus, increased impulsivity or distractibility, inability to control emotions, poor non-verbal communication, inappropriate social behaviors, poor reading comprehension, OCD/ stimulatory behaviors, autoimmunity, allergies, poor spatial orientation, missing the big picture, poor math reasoning, poor eye contact, and anxiety. Individuals with these traits are often diagnosed with ADD/ADHD, OCD/Anxiety, or ASD.

Talking in terms of hemispheric weakness, someone with a left hemisphere delay may have poor reading skills, problems with fine motor skills, poor spelling, poor object identification, poor auditory processing, poor verbal communication, delayed

Children with a hemispheric weakness will struggle academically and/or socially. To address the problem (not just the symptoms), the weaker hemisphere needs to become stronger which will ultimately balance the two sides and increase the communication. re you considering taking a few college courses or completing a degree? Today there are a variety of choices available and understanding and investigating each option can be daunting. So how do you decide what kind of educational program is right for you? • Identify your goal. Are you aiming to transition into a new career, update your skill set for professional development, or pursue courses for personal fulfillment? Articulating your specific goal is the first step in deciding what type of education will be the best fit. • Understand the types of education available. Most education is offered in one of three formats: credit courses, noncredit courses or technical training. If you are interested in taking courses toward the completion of an undergraduate or graduate degree or for college credit, you want to consider educational programs which offer courses for academic credit. If you are more interested in updating your skills for professional development within a specific area, consider noncredit coursework or certificate programs. Want to move into a technical field such as radiologic technology or practical nursing? Depending on the field, you may need training within a certificate or diploma program. • Establish practical criteria. Think about issues of timeframe, schedule and finances. • Do you have a timeframe in which you want to reach your goal? Realistically identifying the amount to time you have can help you to eliminate educational options you won’t have time to complete.

• When are you able to take courses? Depending on the educational program, courses may be available during the day, evening, weekend or online. Online courses often afford the greatest flexibility, as most programs allow students to access courses when they are able during a given week. Knowing what option fits best with your schedule will help you to narrow the choices. • What finances do you have available for education? Will you need financial assistance? Knowing the cost and financial assistance available for each educational program can help in the decision process. • Research educational programs which match your criteria. Searching the Internet for programs that fit your criteria is a great place to begin. For instance, the Delaware Higher Education Office website (www.doe. k12.de.us/infosuites/ students_family/dheo/) lists all the colleges and universities in the state and offers information on financial aid.

Ovarian Cancer... What All Women

(And Those Who Love Them) Need to Know!

E

ach year more than 21,000 new cases of ovarian cancer are diagnosed in the United States, resulting in over 15,500 deaths. It is the deadliest of the gynecologic cancers, affecting approximately 1 in 65 women. Because there is no reliable diagnostic screening, it often goes undetected or is misdiagnosed until it has advanced to the later stages. Less than 20% of cases are discovered in the early stage. Due to late diagnoses and a very high recurrence rate (70-90%), less than 50% of women make it to the 5 year milestone. Ovarian cancer does not discriminate and can affect females of any age and ethnic background. It continues to be one of the most underfunded cancers and the lack of public awareness and education is of critical concern. Although the symptoms of ovarian cancer often are not intense in the early stages, they are NOT silent. Once thought to be a ‘silent disease’, research indicates that 95% of women DO have vague but persistent symptoms. These symptoms are not always useful for diagnosis since they may be indicative of other less serious conditions. If any of the following single or combination of symptoms persist for more than 2 weeks, consult your physician and insist that a manual exam and transvaginal ultrasound be performed. If any abnormalities are detected, a referral to a gynecologic oncologist may be in order. • Bloating • Pelvic or abdominal pain • Difficulty eating or feeling full quickly • Urinary symptoms (frequency or urgency) Other symptoms may include: • Extreme fatigue • Indigestion, heartburn, or upset stomach • Lower back and/or leg pain

7 Things A Woman Should Do After The Loss Of A Spouse

he loss of a spouse can be a devastating, life-changing event. Sadly, for many women, becoming a widow is a first step toward economic hardship. That’s why it’s critical for you to organize your finances after your spouse’s death and to take ongoing steps to secure your financial future and that of your family. Locate important documents and financial records. These include your spouse’s will and other estate planning documents (e.g., trust), insurance policies, bank and brokerage statements, stock and bond certificates, deeds, social security number, birth and marriage certificates, and certified copies of the death certificate. Set up a communications tracking and filing system. Set up a system to record incoming and outgoing calls and mail. For phone calls, keep a notebook handy where you can write down the caller’s name, date, and subject of the call. For mail, keep track of what you receive and whether a response is required by a certain date. Finally, create a filing system for important documents and correspondence with separate folders for different topics. Seek professional advice to settle the estate and file tax returns. Get expert help from an attorney, accountant, and/ or financial and tax professional. An attorney can tell you what procedures to follow. A tax

professional can help you file certain federal and state tax returns that may be due. A financial professional can help you by conducting a comprehensive review of your financial situation and identifying any retirement and survivor’s benefits that may be available to you. Apply for benefits. You’ll need to contact several institutions for information on how you can file for benefits, namely life insurance company, Social Security Administration and your spouse’s employers. If your husband was a federal, state or local employee or in the military, you may be eligible for government-sponsored survivor’s benefits. Update account names. You may need to contact financial institutions to change account names and/or update contact information. Evaluate short-term expenses. You may have immediate expenses to take care of, such as funeral costs or outstanding debts your husband may have incurred. You can use credit cards for certain expenses or you can try to negotiate with creditors to allow you to postpone payment for 30 days or more, if necessary. Make sure you have one or more credit cards in your name, and when you can, order a free copy of your credit report and review it for accuracy. Avoid hasty decisions. Go at your own pace, not someone else’s. For example, don’t commit to move from your current home until you can make a decision based on reason instead of emotion. Don’t spend money impulsively. Don’t cave in to pressure to sell or give away your spouse’s possessions. Find out where you stand financially before you make any large purchases, sell property, or loan money to others. That The “Fear of Failing” Really Means You Are Afraid of….

That is a lot of uncertainty, so many areas that you could fail, and be exposed. Now, tell me, which one creates more uncertainty? The one that gives you the devil that you know or the one that gives you the devil that you do not know?

The Solution 1. First step is awareness. You must

become aware of what is driving you. Find out what is truly preventing you from success.

n my near 15 years of coaching, I can say that the number one reason my clients share with me why they are not successful is, “I am afraid of failure.” On first glance it sure seems to make sense. You set a goal. There is a desire that you want. You even create some sort of action plan. Then you realize you are not following your action plan and you are not getting closer to your goal. This happens again and again. Finally you draw the conclusion that you must be afraid of failing. Yes, that explains it all. It explains why you don’t give it your all. Why you don’t follow up and why it seems you are self-sabotaging your own success. So yes, your hypothesis must be correct. Right? Perhaps…or perhaps the fear of failure is really code for...fear of success. So wait a minute Joe that makes no sense. Perhaps, but lets look at this a bit closer because I think you will begin to see the truth behind this myth. Part of human psychology is that we dislike change or at least we find it uncomfortable. Change can be awkward, challenging and even humbling and because of that we tend to really like what is comfortable and familiar. Even if it is not good for us. In other words, we will stay in comfort even if it negatively impacts our life.

2. Once you become aware, you must ask yourself what is that worst that can happen? I mean really, what is the worst that can happen? Is it that you will become embarrassed? Broken hearted? Lose your investment? Be rejected? It is imperative that you understand the absolute worst that can happen to you. 3. Ask yourself, “Is what I want to achieve worth risking me getting the worst that could happen to me. If you answered yes, then take massive action and now focus on what you want. If you answer no, then congratulations. You need to negotiate with yourself and dig to discover what is preventing you because you are not yet on board emotionally. By identifying your true fear, you can align with it and solve it. When you are blind to it, you keep running into a wall because you are looking the wrong way.

Custody – It’s What’s Best for the Kids, Pt. 2 This article is the second in a two-part series on child custody in Delaware. The first article, which can be found on our websites, www.pinterlaw.com and www.pinterlawonline.com, covered the definition of custody and how it is decided in Delaware. This issue explains when to file for custody, what to expect, and when you should settle or have a hearing.

T

o begin, I usually recommend to my clients that they should always file for custody By: Shana A. Pinter, Esq. because having a written custody order protects everyone when the inevitable disagreement arises. If there is no custody order in place, you and your ex are left to your own devices to try to resolve the problem(s). So let’s assume that you’ve filed for custody with the Family Court. Unless there is a PFA (Order of Protection from Abuse) in place the Court will schedule you for a Family Court Mediation held at the Family Court of the county in which you file. This Mediation is led by a Family Court Mediator specially trained and experienced in dealing with custody and all other family law issues. The Mediation is for you and your child(ren)’s other parent to reach a decision – you must attend the Mediation but actually reaching a decision is voluntary. You don’t have to agree to anything at Mediation BUT you will most likely leave with at least a “Temporary Custody and Visitation Order” which adheres to the contact guidelines, which can be found on our website under “Resources” and also at the website of the Family Court of Delaware (http://courts. delaware.gov/Help/Visitation/ visitation.stm).

by settling is better than or the same as what you would be awarded if you went to Court. For instance, if the child(ren) is over the age of five, there’s no substance abuse, no domestic violence, and both parents provide a stable and nurturing environment to the child(ren), among other factors, it’s better to agree to joint legal custody and shared residence (see “It’s What’s Best for the Kids – Pt. 1” for an explanation of custody and each parent’s rights and obligations) because that would most likely be awarded by the Judge if you had to go to Court. On the flipside, it’s better to go to a hearing if the other side is insisting on primary residence of the child(ren) and there are no reasons for that to be awarded – reasons like the child(ren) gets upset when they have to switch houses, you eat with the kids at a fastfood restaurant each week, or you’re living with your parents while you get back on your feet and the living quarters are tight, you are most likely better off pursuing your case

If you can’t agree at Mediation, a hearing before a Judge of the Family Court is scheduled. At a hearing, both parents will testify about what they want and why they think that is best for the children. You should view this as your one and only time before the Judge and your only chance to tell the Judge everything. After your hearing the Judge will issue an opinion which generally can’t be changed, or “modified,” for two years. The next question is: “When should I settle?” In short, and this applies to all areas of law, you want to settle if what you can get www.TheWomensJournal.com

in Court because these objections are not significant enough, in and of themselves, to award primary residence to one parent. So if you were to agree at Mediation to allow the other side to get primary residence you would be getting less than if you have your case heard and decided by a Judge.

Course and Scope of Employment: Cupid’s Revenge!

alentine’s Day is right around the corner, and love is in the air! Cupid, that cherubic little archer is said by the ancient Greeks to be the God of Love. Cupid’s modus operandi is to sneak up on unsuspecting people and shoot them with his tiny bow and golden arrows. Although the process sounds similar to an assault, those people who are struck with his golden arrows are said to fall madly and deeply in love, forever and ever, with the next person they see. At times, one has to wonder about all the hazards created by true love and this little mischievous winged child. After all, I barely trust my little boy with a light-up sword on Halloween, much less a bow and arrow! Some of Cupid’s matches have turned out to be love stories for the ages, while others… well, let’s just say, they have fallen short of their mark. One such story that many people have been buzzing about recently involves a government employee from Australia that was injured during some time “between the sheets.” In the Australian lovergirl case, the female employee was observing a budget review at an office that was some distance away from her usual office. Due to the distance, she stayed overnight in a hotel room paid for by her employer, the Aussie government. Cupid must have hit his target with this lovergirl because later that evening she was in bed, doing the horizontal mambo with loverboy when part of a light fixture crashed down on her face, injuring her nose and mouth (as well as her pride, I would imagine). Lovergirl filed a claim with the Aussie government’s workers’ compensation carrier, and sought payment for her injured face.

Originally, she was granted benefits, but that was short-lived and was overturned by their federal court. So, why did her claim get overturned? Under Aussie law, a worker must be induced by their employer to perform a particular activity which brings about their injury. Here in Delaware, there is an entire section of workers’ compensation law devoted to this same type of situation (no, not the horizontal mambo part) called “course and scope.” What it means is that you must be engaged in an activity in furtherance of your employer, or which is intended to benefit your employer at the time of your injury in order to qualify for benefits.

Normally, as long as you are on your employer’s premises, and performing your job duties when you become injured, you will qualify for workers’ compensation benefits. There are, however, situations when this rule has become broadened so as to afford additional protection to injured workers. For instance, an employee who is assaulted by a co-worker in the parking lot while leaving their job may qualify for benefits, especially if they did not encourage the fight, and the disagreement originated in the workplace. Another example of an exception to the general rule is a case where a worker was injured while loading food that she cooked for an office party into her car. She was scheduled to work at 8 am and was loading the food into her car after that time. She was found to be the equivalent of working from home, as she loaded her car full of food to bring to her office.

Those two examples are just some of the ways that the general rule on “course and scope” of employment have been broadened to afford benefits to an injured worker. If you were injured while performing an activity that was not in your job description but would have benefited your employer, you may also qualify for workers’ compensation benefits. Say “I Do” To Safe Transportation Y

ou’ve planned all of the details from the church and reception down to the mementos you’ll give to your guests, and now your special day is here. Why not plan the details of what could be one of the most important events of the day…. transporting your bridal party and close family members to the wedding and reception! For some people, their wedding might be the only time they treat themselves to a limousine ride, so why not be sure your limo or bus service’s insurance coverage is as good as the appearance of their vehicles? To protect yourself you should ask the limousine or bus company to provide you proof of their insurance limits. You should also ask about background checks and driving records of their drivers. On a day when most celebrations will include partaking in alcoholic beverages, you want to make sure that the people in charge of your safety are fully trained, qualified, safe drivers and are fully insured. Asking for this information could avoid undue risk on your most important day.

Don’t think these accidents never happen. They do! When my son was recently married in South Carolina, the limousine was full of guests on the return from the reception to the hotel. As the limo driver turned into the hotel parking lot, he struck another parked vehicle. Fortunately, nobody was injured. What if they were? Think about 25 passengers with potentially competing bodily injury insurance claims. My advice to the bride and groom is to be prepared and do your research in advance so you can relax and enjoy your day. Before the wedding, there are usually the bachelor and bachelorette parties. If you have a large party going out to celebrate your upcoming nuptials, consider renting a limousine and be sure to do the same research regarding the liability limits and types of insurance coverage and driver background checks, so that your family and friends can safely enjoy your evening together. Never drink and drive, or allow your guests to do so. We see the devastating consequences of that conduct all too often.

I have found over the years that people don’t realize if convicted of driving under the influence of alcohol and they cause an accident, the victim injured in that accident can bring a claim against them not only for pain and suffering/ compensatory damages, but also for punitive damages. Punitive damages serve three purposes: 1) to punish the wrongdoer, 2) to deter the wrongdoer from engaging in similar conduct in the future, and 3) to deter others from similar conduct. Punitive damage awards can be very significant. So, don’t set yourself up for risk or disappointment on a day you want to be perfect. Protect yourself and your guests. Do your homework. I have been representing injured victims of automobile accidents for many years. It is always devastating when I have to explain there is inadequate insurance coverage available for the injuries involved. Certain risks can be avoided. Never allow drinking and driving.

When had a consult to come in with our model so we could determine the hairstyle that we should have to best show case the dress. J. Is a very hip stylist, but when it comes to bridal up dos he is totally about show casing the brides features to her best advantage. Our model had long straight hair, which was wonderful to work with, but that could also have been extremely difficult to style needing to withstand a day of photo shooting. Which comes first, the mounting (ring) or the diamond? We have a lot of brides to be, coming in with a picture of a ring they like, and the photos are often very pretty. This is often helpful in determining a style they lean toward but to be totally sure the style works, a ring needs to be seen from all angles, touched and tried on for comfort and appearance. We find a better way to start is with the diamond or other gem that will be used as the focal point. The diamond is the major part of the investment, and the part that will most likely not be replaced. The mountings will wear out over many years and styles change. Although the center diamond can be upgraded to a larger or different shape diamond at any time, we find that most brides are very sentimental about their original engagement diamond. nytime of the year can be wedding season, however, no matter when you tie the knot, it will be the most important day of your life. One of your favorite things, of course, will be the purchase of a bridal gown. Bare Essentials has some very helpful suggestions that may eliminate a lot of headaches. Before you make the final selection of your gown, know what types of undergarments are needed to help you be the most beautiful bride you can be. *If you are a curvy gal, strapless and/or backless gowns can be problematic as these types of undergarments accommodate less of a size range. Choose a figure flattering style that accentuates your assets. We carry a vast selection of strapless options, however, bras or bustiers without straps create support challenges. he first step in realizing the best skin care regimen for your upcoming wedding is setting aside ample time to prepare! You cannot wait a week before the wedding and expect glowing, healthy skin.

Are you five or six months away from your wedding? While you’re addressing invitations and picking out bridesmaids dresses, take some time to work on your skin care regimen. Making an investment now will pay off big on your wedding day. A daily routine conditions your skin, maintains healthy hydration levels and reduces excess oil production. All these factors can reduce the occurrence of spots and open pores as well as flaky, dry and dull skin. Moisturizing daily will also protect your skin from the environmental elements which cause premature ageing (pollution, smoke, UV rays, air conditioning and stress).

Your skin care regimen should consist of a good cleansing program. Try using a multi-step system including: • a good cleanser appropriate for your skin type in the morning and evening • daily moisturizer with SPF • an exfoliant once to three times a week

Group Lesson Make Learning Fun-and a ‘Family Affair’

Dance Lessons will make this tradition so much easier and, maybe even downright fun. The very best part of my job, and I mean this, is meeting people. This past year, I had a chance to work with some very wonderful ‘Dads’ for the ever important ‘Father-Daughter’ Dance. They were a bit nervous, proud of their lovely daughters, and always a little bashful regarding this upcoming public display of affection and of their dance skills. My goal is always to put them at ease, and build their confidence. Within one lesson everyone is more self-assured and relaxed. When a family makes the time to get together and includes the mother of the bride and the others who are in the wedding party-it offers everyone a chance to brush up on dance skills. This pays off in many ways. The mother of the bride gets to practice with Dad—and the groom gets a chance to dance with mom-everyone is prepared.

Music Choices & Choice Moves

Several of the brides chose songs to dance to that their fathers had sung to them as children. Watching one dad dance and sing, ‘Goodnight Sweetheart Goodnight Well I have to Go’….Priceless! So lovely, so sweet—all I did was give them the steps and techniques that made it all look elegant and effortless. If you don’t have a favorite song memory, then make one. Choice of music can be serious and touching, lighthearted and fun, classic or contemporary. One father-daughter duo danced to ‘Blue Suede Shoes’ and another to ‘Rainbow Connection’. ‘I loved Her First’ and ‘My Girl’ are also great choices I have seen, but there are so many more—A great instructor can help you choose music and the moves.

Take away the worry of that First Dance so that it becomes one of the highlights of your day.

How To Have Beautiful Hair On Your Special Day wedding date to make certain that he/she can accommodate your time frame. Talk to your stylist for their recommendation for your pre-wedding hair care. No matter what type of hair you have, do a deep conditioning treatment once a week to restore moisture. If your hair is oily, be careful to avoid your roots and condition only the length of your hair. Try to avoid heat as much as possible on your hair. And of course, trim, trim, trim -- dry heat will only worsen the dull appearance of damaged or split ends.

ost young girls dream of their wedding day from the time they start grade school. Then they grow up, fall in love, and the plannaing starts. The one item that many times gets forgotten is the hair, yes they often have a bridal hairstyle in mind, but let’s face it -- if it doesn’t work with your wedding day weather, you may be in for a bad hair day. A better bet is to come up with a plan for your hair that will work with the weather that Mother Nature decides to bless you with. Trust me you don’t want to be crying the blues because your hair looks like a limp noodle, nor do you want to have a wild style from the 80’s looking back at you in the photos that will be with you for the rest of your life, and don’t forget you want the hair to fit with your wedding style. It may sound crazy, but check the weather history for the month that you are planning to tie the knot. The weather will always be a factor on your wedding day style. If you are planning to have your stylist come to the site where you are having everyone dress for the wedding, then you have to be certain that your hair stylist can work you in to his/her schedule. Or if you are going to his/her salon with your entire wedding party, this also needs to be discussed in the very early stages. You should visit your hair stylist even as early as a year before your

If the weather is damp and humid, no matter what your natural hair texture is, you’re going to need to work with what you’ve got. This is not the time or the place to try to make a curly mane stay straight -- chances are you’ll wind up with a total mess. Prep your locks by regularly using a frizz-fighting serum. Comb a few drops through soaking wet hair to be sure it gets distributed evenly. On your wedding day: The last time you wash your hair before the wedding don’t rub -you’ll rough up any broken or damaged hairs, increasing the chance of frizz. Your stylist should apply a de-frizzing serum. And finish your dry style with a second application of frizz-fighter (pomade or gel works best). You might want to keep a little on hand throughout the wedding day for smoothing down any hair that may curl up.

Day of - Receive your hair and make-up services. It is recommended that these be the only services you receive on your actual wedding day. You will find the trends for 2013 versatile, interchangeable, and easily mixed and there is something for all hair types.

Flower Girl Shopping Made Easy By: Carol Harvey

t Hansel & Gretel we pride ourselves on our beautiful dresses for all occasions and this dress is one of our favorites. All flower girls are adorable just like all brides are beautiful and we want them to feel special. That’s why we carry a large selection of dresses to choose from. This dress is by Sweetie Pie and it’s in a lovely shade of ivory. It is true there are many shades of ivory from cream to off white but they should all blend pretty well. Often a bride’s gown is in what we call “diamond white”. Not really white but not as cream colored as ivory. In that case we suggest ivory for the flower girl because diamond white does not look good with a flower girl in a stark white dress.

What makes this dress so special is the puckered over skirt that can be accented with a colored sash and snap on roses or can be worn all one color. It comes in white and ivory and the sash and rosettes come in 12 colors – white, ivory, black, brown, café, blue, lilac, red, mint, pink, royal, turquoise, and eggplant. Delaware Hospice’s Care Of Mother Eases Burden Of Caregiving Daughter In the doctor’s office one day, Maureen met someone from Delaware Hospice who recommended she make a call. Her mother was admitted as a patient immediately, and Maureen found the help she needed. “Most important,” remembered Maureen, “was their help in putting a ‘do not hospitalize’ order in effect. They became advocates for my mother, closely monitoring her medications and working through her particularly challenging situation with smart solutions.”

ife’s journey can sometimes take a rough path and you just need someone special to help you along—or in Maureen Reiss’ case, a team of special Delaware Hospice professionals. It was Maureen’s turn to care for her mother. Always an active and healthy woman, June Keenan had suffered a stroke while vacationing in Florida. Along with other symptoms, she lost her ability to communicate her wishes and needs. Maureen’s home near Milford was too small to take her in, so she found an assisted living facility 30 minutes away. Maureen said, “My mother was so frustrated that she was not able to express herself, and she became very challenging. She would grind her teeth constantly, spit out her medications, and throw things on the floor. With her physical disabilities from the stroke, she could only eat with her fingers, so no one would sit with her at meals. From being a lovely, sociable person she had suddenly become someone that few could understand or communicate with, and she was furious about it.” One of the few things that worked well to calm her was Maureen’s therapy dog. She said, “My mother loved the dog. She would make soft sounds and pretend to kiss the dog.” With a few hospitalizations for minor health issues, she suffered additional setbacks in her physical rehabilitation. Simultaneously maintaining a family household and caring for her mother grew to become a tremendous burden for Maureen.

A lot of caregiver stress comes from wondering what’s happening when they are not at the patient’s side. Knowing that Delaware Hospice’s team members would be visiting on a certain day, gave Maureen relief. She said, “I would receive messages on the answering machine just letting me know someone had been to visit my Mom and everything was okay. It was wonderful to realize I wasn’t in it alone anymore. I could take a few minutes to do my own laundry and get there a bit later because I knew she was okay. It was really great to feel that somebody else cared. I still have those messages on my answering machine!”
