Latest Advances In Eye Surgery To Delaware

T
he field of Ophthalmology over time has provided wonderful opportunities to improve patients’ health and well being. The confluence of medicine and technology in our era has spawned numerous innovations which have revolutionized eye care. These advancements
have dramatically improved outcomes for patients, at a pace never before experienced. While previous standards strived for visual rehabilitation, now vision enhancement, comfort, and rapid recovery are the new benchmarks. Technology has played a key role in the progress of all of medicine, but in few areas are these advances as dramatic as they are in ophthalmology. Several key areas are noteworthy: cataract and corneal surgery, refractive surgery, diagnostic testing, and medical therapies for glaucoma and macular degeneration. Cataract surgery dates to the 5th century BC when early practitioners pushed cloudy lenses aside. It wasn’t until the 1700s that cataracts were actually removed, and not until the 1940s that intra-ocular
lenses were first implanted. Now, high quality implants improve vision to levels often better than ever, and may correct astigmatism and improve reading and distance vision, reducing the need for glasses. Newer, safer machines aid in the removal of cataracts, and now laser cataract surgery is on the horizon. Corneal transplantation has been successfully performed since the 1950s, but recent developments in the last decade have made tremendous improvements both in visual success and transplant survival. DSAEK and DALK are two very new, less invasive procedures, which are now available in Delaware. Refractive surgery, and in particular laser vision correction, is perhaps one of the most “high profile” advancements in
ophthalmology. Combining laser precision, rapid recovery, and desirable results, laser vision correction has allowed millions of patients to reduce their need for glasses. Available since FDA approval in 1995, the procedure has evolved and become safer and more accurate and effective. Finally, new diagnostic instruments, incorporating the latest technology in high resolution scanning and computer analysis have allowed better monitoring and treatment of diseases. New medications have allowed ophthalmologists to arrest the progression of vision stealing glaucoma and macular degeneration, two devastating diseases which are the leading causes of blindness in the United States. This is truly an exciting time to practice medicine, and Ophthalmology, in particular. Patients can now look forward to preserving, and even enhancing, their vision, where previously there may have been limited success. Jeffrey Minkovitz, MD is an ophthalmologist with Eye Physicians and Surgeons in Wilmington, Delaware. Dr. Minkovitz specializes in cataract and refractive surgery, and is one of only a few Delaware surgeons specially trained in corneal transplantation. He completed a cornea and refractive surgery fellowship at the renowned Wilmer Eye Institute of Johns Hopkins. Previously, he graduated magna
cum-laude from Harvard, earned his medical degree from the University of Massachusetts, completed internship at Harvard’s Mt. Auburn Hospital, and served his ophthalmology residency at Washington University. Dr. Minkovitz participated in the early research on Excimer Laser refractive surgery, and has offered this procedure to Delawareans since FDA approval in 1996, before Delaware had its own laser. These early pioneering individuals travelled to Wilmer Institute with Dr. Minkovitz, where he maintained his teaching and clinical appointment. In addition to introducing laser vision correction to Delaware, he also introduced NearVision CK to eliminate reading glasses, DSAEK (a newer, less invasive type of corneal transplant) and most recently the newest advance: laser cataract surgery. Dr. Minkovitz is a cofounder of the Center for Advanced Surgical Arts and serves on its medical advisory board. He is also on active staff at Christiana Care. While on staff of Wilmer at Johns Hopkins University, Dr. Minkovitz received the Outstanding Teacher award. He was also named Top Doc in ophthalmology by Delaware Today Magazine six consecutive years, and has been an invited instructor at American Academy of Ophthalmology Annual Meetings.

Since Richard’s surgery, he isn’t complaining about his inability to read something on the fly without searching for his glasses. He wired 6 recessed lights (NO GLASSES), he read the Sunday paper sections A & B and menus (NO GLASSES), and is using the computer (NO GLASSES). I suppose for Richard, needing to look for glasses when he wanted to see something, has been the hardest adjustment of becoming older. His dependence was so gradual that I never related to why he would get so angry over needing them. My house has been quieter. I now have 2 less things to do everyday: find the glasses and make sure they were clean!

This issue is our Breast Cancer Awareness Issue normally, but this year we decided to bring awareness also to Ovarian Cancer, all of our participants on our cover are either breast cancer or ovarian cancer survivors.

How many women get breast cancer? The American Cancer Society’s estimates for breast cancer in women in the United States for 2014 are: about 232,570 new cases of invasive breast cancer; about 62,570 new cases of carcinoma in situ (CIS) of the breast will be found (CIS is non-invasive and is the earliest form of breast cancer). There are about 40,000 deaths from breast cancer. Breast cancer is the most common cancer among women in the United States, other than skin cancer. It is the second leading cause of cancer death in women, after lung cancer. The chance of a woman having invasive breast cancer during her life is about 1 in 8. The chance of dying from breast cancer is about 1 in 36. Breast cancer death rates have been going down. This is probably the result of finding the cancer earlier and better treatment. Right now there are more than 2.8 million breast cancer survivors in the United States. How many women get ovarian cancer? The American Cancer Society’s estimates for ovarian cancer in the United States for 2014 are: about 21,980 new cases of ovarian cancer, and about 14,270 deaths from ovarian cancer. Ovarian cancer ranks fifth as the cause of cancer death in women. It is more common in white women than African-American women. A woman’s risk of getting invasive ovarian cancer in her lifetime is about 1 in 73. What Causes Unusual Swelling In The Body Following Breast Or Ovarian Cancer? Article by Joanna Mikhail, DPT, ATC, CLT Doctor of Physical Therapy Certified Lymphedema Therapist
L
ymphedema is the accumulation of lymph, the protein rich fluid, in the tissue of body parts, often extremities, due to damage to the lymphatic system that results in unusual swelling of the body. As the lymphatic fluid can no longer travel freely through the lymph system, this causes swelling of the affected limb or limbs, hardening of the skin, and eventually seepage and infection. There are a number of causes of lymphedema, but one primary cause is cancer – both the disease and the treatment for the disease. Cancerous tumors can begin in the lymph system or metastasize to the lymph nodes. When these lymph nodes are surgically removed or damaged by radiation treatments, lymphatic fluid becomes stagnant in affected body parts, resulting in lymphedema. In women, two common cancers that may result in lymphedema are breast cancer and ovarian cancer. Breast cancer can affect the lymph nodes in the armpit, resulting in swelling on the affected side of the body in the upper extremities, chest, side, or back. Similarly, ovarian cancer can impact the lymph nodes in the groin and a common result is swelling in the lower extremities, genitals, buttocks, abdomen, side, or back. While lymphedema does not occur in every instance of these cancers, approximately 20% of individuals whose lymph nodes are impacted will develop this condition. However, there are a number of steps that patients can take to reduce the likelihood of developing lymphedema or to limit the progression of the condition. At the Lymphedema Clinics of America, we can help patients learn lifestyle habits that will mitigate their chances of developing lymphedema as well as keep early lymphedema from developing beyond the initial stages. We will provide you with the Lymphedema Risk Reduction Practices provided by the National Lymphedema Network and help you determine how to best apply www.TheWomensJournal.com
this information to your particular situation. We will also provide therapeutic treatments to address any early symptoms that may be present and thereby, reduce the likelihood of further development. Since lymphedema sometimes occurs months or even years after your initial disease, we will educate you regarding what symptoms to watch for and to continue to monitor you for early signs and symptoms of the condition. We encourage anyone who has or has had cancer that has impacted the lymph nodes to schedule a consultation with one of our therapists and to begin a program that will help reduce the impact of lymphedema as you move forward. Quick, Comfortable and Cost-Effective I
n past articles I have shown many dramatic smile makeovers involving a combination of disciplines and techniques. This issue I wish to highlight what modern dentistry can do for a common problem. The solution is quick, comfortable, and costeffective. Many of us grew up at a time when preventive dental techniques were few and not readily available. We have fillings, lots of fillings. Some are in front teeth. As these fillings age they discolor, chip, and/ or leak. The margins of these fillings microscopically pull away from the tooth allowing staining fluids and bacteria to enter the inner portion of the tooth under the filling. This is how decay occurs under old fillings. Simply removing the old discolored fillings and replacing them with the appropriate toothcolored filling material strengthens the tooth, prevents tooth decay, and improves the smile. In the case shown here, a little tooth reshaping was also done using the same filling/bonding material to enhance the result. Tooth colored fillings used today are either a composite resin, glass ionomer or a combination of the two. There has been a great deal of advancement in the chemistry of these filling materials in the last decade. Dentin bonding is perhaps
the most important advancement. These filling materials now chemically adhere, bond, to not only the outer enamel layer of the tooth but the inner dentin as well. A stronger connection and better seal occurs.
structure to create undercuts or mechanical locks to retain the filling material. Old silver fillings required this type of retention. Composite resins are more conservative of tooth structure.
Most front teeth are filled with a light cured (hardened), BISGMA based resin. Within the base matrix there are small glass particles that greatly impact the properties of the material. The quantity and size of the “filler” particles accounts for differences in strength, polishability, wear resistance, light reflection, as well as handling properties. The introduction of the tiniest particles, nanoparticles, has given dentistry a material which has the luster of natural enamel and much more resistance to surface stains. It is a challenge to keep abreast of advances in this area. This is why it is so important for a dentist to be committed to continuing education.
While composite resin and glass iomoner toothcolored filling materials offer many advantages they are not the best choice in all situations. Crowns, veneers, onlays offer greater strength and superior esthetics. There are situations when simple replacement of defective fillings and bonding is the better choice for the patient as it is convenient and affordable.
Composite resins are a wonderful filling material for both front teeth and the molars. The compressive strength of resins now equals that of the metal silver fillings. The tooth is actually strengthened by the bonding of remaining healthy tooth parts. Silver fillings and the first generations of tooth colored fillings merely filled in the space left from decay removal. Before modern bonded fillings the dentist needed to remove healthy tooth

Christiana Care Health System Helps New Mothers Donate Cord Blood I
n the treasured moments after a mother gives birth, another life-giving opportunity is available inside the hospital delivery room. Mothers delivering at Christiana Care Health System can make their baby’s birth even more special by donating their baby’s umbilical cord blood. By electing to donate their newborn’s cord blood, the new mother can help patients across the nation struggling with chronic illnesses as well as diseases for which no cure exists. In obtaining the sample, no physical contact occurs with either the mother or newborn. Consider the banking of blood from the umbilical cord – the part of the placenta that transports nutrients to the fetus – as a form of recycling that both saves and improves lives. The cord blood stem cells – which are adult stem cells and not embryonic stem cells – can be used to treat diseases, such as certain cancers. Researchers also work with cord blood stem cells, which transform into all the different cell types in our bodies, to find new treatments for disease. It might be reasonable to assume that many mothers would jump at the chance to help the human race by banking their cord blood. But of the 4 million births each year in the United States, cord blood is stored in less than 10 percent.

Hospitals must have in place a robust and coordinated collection method in order to properly procure and bank the cord blood. Clearing Up Five Myths About Varicose Veins Effective treatment is available with no down time T
he treatment for varicose veins used to be an invasive process, requiring arduous surgery and up to seven days in a hospital. Nowadays, the condition can be treated quickly and painlessly, allowing patients to promptly resume their regular activities. “It’s a completely outpatient procedure with no down time,” says Dr. Anthony Alfieri, founder of Delaware Advanced Vein Center. “It takes about 15 minutes to do, it’s more or less painless, and you can work the next day.” Dr. Alfieri, who is a cardiologist for 25 years, explains that having varicose veins is “needlessly limiting” for many people. Here are five myths about varicose veins and what the facts really are. Myth: Varicose veins are
Myth: Only women suffer from varicose veins. Fact: Symptoms in men can include discoloration, large bulging veins, and skin ulcers. But males can also be successfully treated. Surgeons formerly used a procedure known as saphenous vein stripping to deal with varicose veins. This method consisted of making an incision at each end of the vein. The surgeon then threaded a flexible wire through the vein and pulled it out. However, that procedure could result in scarring and there was a risk for infection. Today, the treatment for varicose veins is much less invasive, with the gold standard being ultrasound. And the results have been overwhelmingly positive. “I haven’t seen one patient we have done that wouldn’t have done it over,” says Dr. Alfieri. “I would say that’s universally true.”

Broyles, D.O. regained weight after stopping these diets and their popularity ceased. Now, a diet based on an individual’s DNA is available. The good news is our DNA will not change, as do fad diets. The “genome” (the complete set of genetic information) of any individual is unique. In 1990 the Human Genome Project launched. It was an international research project whose goal was to identify and sequence all of the genes of the human genome. The project concluded in 2003 after 92% of the human genome was sequenced. This is a major scientific breakthrough. The hope is that genomic studies will lead to advances in the diagnosis and treatment of diseases, including obesity.
In less than 40 years, the prevalence of obesity in the U.S. has increased by over 50%. Currently, two out of every three American adults are overweight or obese. Obesity and overweight account for nearly one in every ten American deaths, and costs our nation $223 billion dollars per year. Obesity is a complex problem, and now the epidemic has spread to our children. It is estimated that today’s children will be the first generation to live shorter life spans than their parents. In some cases of obesity, genetics seem to be responsible; in others, various combinations of hormonal, metabolic and behavioral factors may play a role. We are now able to identify a variety of genes that influence an individual’s eating behaviors, metabolism, response to diet and exercise, as well as identify an individual’s propensity to develop certain medical conditions. DNA testing is a great catalyst to help people make lifestyle changes, especially in respect to diet. A single saliva sample can identify how a person metabolizes fats, carbohydrates and protein. This information can be very helpful for a person trying to lose weight. For example, if an individual is a slow metabolizer of fats, then it’s recommended he/she be on a low fat diet to achieve weight loss. Clinical studies

However, the physician must submit necessary paperwork which includes the patient’s vital signs and a valid reason (i.e. weight gain, fatigue) for ordering the test. 3. Adopt healthy habits

ow do you know if you or someone you love is simply a bit forgetful or experiencing a serious memory problem? With age, it’s natural to wonder if forgetfulness is an early sign of a memory disorder. 1. Get regular exercise Keeping your body active is one of the best ways to protect against memory disorders. Brisk walking, riding a bike, and swimming or exercising in a pool are all low-impact ways to get your body moving.
2. Maintain a healthy diet A balanced diet of whole grains, lean meats, lots of fruits and vegetables and healthy fats, such as fats found in fish or nuts, is associated with lower rates of memory disorders. Drinking too much alcohol can cause shortterm and long-term issues with memory. Limiting drinking to one glass of wine with dinner can help avoid this damage. 10. Don’t waste money on supplements There are a lot of unproven and unregulated supplements on the market that claim to improve memory. But there is no evidence that they work. Take a vitamin supplement if your doctor has recommended it for a nutritional deficiency, but save your money on supplements that claim to boost your memory - use it towards a gym membership. The radiologists had found a mass in her kidney but did not say what they thought it was. Here, the problem was not the imaging, it was the way the radiologists interpreted the images. Sometimes you can have the best pictures but the radiologists prefer not to say what it might be! I reviewed the pictures for her doctor who took it to two other radiologists as well. I was the only one who said it was cancerous and needed immediate treatment.

Are they also a neuroradiologist? Find out before deciding where to take the prescription your doctor gave you to get an MRI. (Remember, the doctor writes the prescription, the patient decides where to fill it.). MRIs with contrast are particularly good for finding tumors. It does this without radiation and the contrast used in MR (which is neither iodine, nor shellfish-based) is much safer than the contrast used for CT. And, even within the group of five or six MR contrasts, we use only the safest—Multihance, Prohance and Eovist. (See www.safecontrast.com).

Oral Health: The Oral-Body Connnection T
he connection between medical and dental problems has begun to blur over the past few years. It wasn’t long ago when a physician wouldn’t think about referring a patient with heart disease to By Dr. Grace Liu and a dentist for gum Dr. Lewis Yu treatment. The same went for diabetes, pregnancy, headaches, or many other medical conditions. There has been a surge of new studies and evidences that connects the impact of oral health with rest the of the body. The relationship between MD and DMD is changing. We no longer just work in our separate fields. We have begun to collaborate as a team in order to provide our mutual patients a higher level of care. In this article, we will review some of the proven and emerging links between oral and medical health.
Oral Health and Diabetes
There is a flora of bacteria living in our gums and on our teeth. This flora of bacteria strikes a very sensitive balance with our immune system. When this balance is disrupted by an increase of certain bacteria counts, the body’s immune system responds to bring the system back to status quo. Inflammation is part of the response to infection. The inflammation in the mouth weakens our body’s ability to control sugar by interfering with the body’s ability to use insulin. Without insulin, the sugar level in the blood rises. High blood sugar facilitates further gum infection and destruction of teeth foundation.
Oral Health and Heart Disease
Ninety-one percent of patients with heart disease have gum infection as well. It has been found that the same kinds of bacteria causing gum disease are also present in the arterial plaques which lead to heart attack and stroke. There is emerging evidence that suggests snoring is a risk factor for heart attack (A person can snore without being sleep apnic). The current thinking is that the vibration created by snoring induces formations of coronal arterial plaques. The plaques would reduce the blood flow to the heart and cause heart attacks. The plaques may also become dislodged and travel to the brain to cause stroke.

Another pathway deals with production of Nitric Oxide. Nitric Oxide is a naturally produced chemical mainly from cells in our sinus. Its function is to dilate blood vessels. Without a vasodilator, blood has to flow through a smaller space at a higher speed. The overall effect of this scenario is high blood pressure.
Oral Health and Headache
Ninety-five percent of headaches are muscle tension induced. There are large groups of muscles wrapped around our head and attached to our jaw. When the jaw is aligned improperly, the muscles have to overwork to compensate. This compensation over long periods of time causes muscle spasms. Muscle spasms would feel like a headache. This is a tremendously important connection for the chronic headache patient. This means a dental solution to a medical problem. It is an opportunity to address the source of the problem, instead of treating symptoms.
Oral Health and Pregnancy
Babies who are born too early or with low birth weight often have more significant health problems, including lung, heart conditions and learning disorders. There are many risk factors contributing to this condition. Gum infection and inflammation of the body seem to interfere with a fetus’ development in the womb. Researchers are currently working on other oral health connections. Vitamins And AMD • Age-Related
Eye Disease Study 2 • (AREDS2) Results By Paula C. Ko, MD
A
ge-Related Macular Degeneration is the leading cause of blindness in the United States in patients over 60 years of age. There are so many people over age 60 that it is the leading cause of blindness in the United States overall. The original AREDS study (Age-Related Eye Disease Study) was first published in 2001. It was sponsored by the NIH (National Institutes of Health) and showed that the following vitamins were beneficial to the treatment of moderate to severe age-related macular degeneration. The original AREDS formulation included Vitamin A 15 mg (Beta Carotene) Vitamin C 500mg Vitamin E 400 IU Zinc 80mg Cupric Oxide 2mg When taken together these vitamins decreased the progression to advanced AMD from moderate to severe AMD by 25%. The long-term positive affects have lasted up to 5 years and were most notable in patients with the wet form of macular degeneration. Because of this study, ophthalmologists recommend that patients who have moderate to severe AMD take AREDS vitamins whether or not they have the wet or the dry form. Despite this, there have been other vitamin-based studies that make it difficult to recommend these high-dose vitamins in some patients. For example, it has been noted that the risk of lung cancer is increased in smokers who take high doses of vitamin A. Because of this we do not recommend that smokers take the AREDS vitamins. Patients that have AMD are counseled to stop smoking to decrease the risk of progression of AMD. Even if patients stop, they should not take high doses of vitamin A as the risk of lung cancer still remains high even after quitting smoking. We also do not recommend high doses of zinc to men who have problems with Benign Prostatic Enlargement (BPH) as zinc can compound this problem. Lastly, it has been shown that vitamin A may be problematic in patients with osteoporosis, decreasing bone density and increasing the risk of fracture. With all these other studies in mind, we as ophthalmologists must consider many other factors when recommending the AREDS vitamin to patients with moderate to severe AMD. Patients with AMD not uncommonly have other medical issues as these other diseases are also commonly found in patients over the age of 60. When recommending vitamins to a patient, all medical issues must be taken into consideration. The AREDS2 study was designed to test whether adding lutein (10mg), zeaxanthin (2mg), and omega-3 fatty acids (1gm) with or without the AREDS original formulation would be of additional benefit in reducing the development of advanced AMD. It was
thought that perhaps the AREDS2 formula could have more affect as well as be a safer vitamin. Lutein and Zeaxanthin are pigment found in the macula. The macula is part of the retina that allows people to recognize people’s faces, drive and read, in another words, it is the part of the retina that allows for central vision. It was felt that Lutein and zeaxanthin were a safer substitute for vitamin A. Because of the risk of vitamin A and lung cancer in smokers, vitamin A was eliminated from the AREDS2 formula. The dosage of zinc was also reduced because the side affects on BPH as well as the fact that such a high dose of zinc may not be absorbed fully. The findings of the AREDS2 Study were recently published this year and showed the new formulation will also decrease the rate of progression to advanced AMD by 18% and wet AMD by 22%. It did not help the dry form of AMD called geographic atrophy. This new formula does not work any better than the original AREDS formula but it is felt to be a safer supplement. The positive affects of taking omega-3 fatty acid (fish oil) was not recognized in the study. It is still recommended that patients eat fish 2-3 times a week. Fish is beneficial not only for the eyes but for the heart. In addition, a diet rich in green leafy vegetables also can be beneficial in treating AMD. These diet recommendations are highly recommended. It should be noted that taking vitamin supplements do not substitute for a healthy diet.

Dr. Kelly grew up nearby the Wilmington area in Delaware County, Pennsylvania. He attended Garnet Valley High School. His family belonged John J. Kelly III, to the Graylyn Crest Swim Club in M.D., F.A.C.C. Northern Wilmington. Dr. Kelly obtained a Bachelor of Arts from Swarthmore College and attended Jefferson Medical College. His post graduate training began with a residency in Internal Medicine at the University of Illinois in Chicago. He served as Chief Medical Resident there. Dr. Kelly’s recognitions include AOA membership, being named Intern of the Year at University of Illinois and being named Chief Cardiology Fellow at the Texas Heart Institute. hat do I need to do about my diet? This question is one of the most frequently asked by patients after they have had cardiac events. Diet remains the one factor that almost every patient can have an impact on that may lead to good hearthealth. What you eat definitely can change your risk of developing heart disease. A heart-healthy diet can stabilize and perhaps even reverse heart disease. I recently reviewed an article prepared by the Mayo Clinic staff. The article provides eight steps that patients can follow to take control of their diet and help themselves to healthy hearts. The first point to consider is portion size. Many of the things that we have been accustomed to doing are not necessarily good eating habits. Overloading our plates, taking seconds, or eating until we feel full can lead to consuming more calories, fat, and cholesterol than we need. Thinking about portion size can be a first step towards controlling our intake of unhealthy foods. Try to load up the plate with more lowcalorie, nutrient- rich foods. This includes things such as fruits and vegetables. Try to stay away from the high sodium foods that are common in refined, processed, or fast foods. There are some simple rules that can be applied to our portions. A serving of pasta is only about one-half cup. This is about the size of a hockey puck laying on your plate. A serving of meat, fish, or chicken is 2 to 3 ounces. This is about the size and thickness of a deck of cards. Using these rough guidelines, you
can get an idea of what an appropriate portion size is. You can start to make changes that will lead you to healthier habits. It turns out our mothers were right when they told us to eat our fruits and vegetables. They are an excellent source of vitamins and minerals. They are low in calories and provide dietary fiber which has beneficial effects. Fruits and vegetables contain naturally occurring substances that may help prevent cardiovascular disease. Filling up on fruits and vegetables can also steer us away from the higher fat foods such as meat, cheese, and snack foods. We should try to buy fresh-frozen fruit or vegetables, low-sodium canned vegetables, or fruits that are canned and packed in juice or water. We should try to avoid fruits and vegetables that come with coconut, creamy sauces, fried or breaded vegetables, or canned fruit that is packed in heavy syrup. You should be careful with frozen fruits and make sure they do not have a lot of sugar added to them.

Low fat dairy products, egg whites, and egg substitutes are also good sources of low fat protein. We can all make the simple step of removing the skin from chicken before we eat it. Soy beans and soy products are another source of low-fat proteins. When buying ground meat, try to pick the leaner cuts of ground meats. The other sources of protein are certainly enjoyable but we need to limit our intake. This includes limiting our intake of organ meats such as liver, and then other products such as egg yolk, fatty and marbled meats, spare ribs, cold cuts, hot dogs, sausages, bacon, and any fried or breaded meats.

This includes any of us who are 51 and older, African-Americans, those who already have hypertension, those with diabetes or anybody with chronic kidney disease. We also need to realize that there is a lot of salt in canned and processed foods before it reaches our plate. This includes commonly consumed items such as soup and frozen dinners. Eating fresher foods and making homemade soups and stews reduces the amount of sodium we take in. We need to look at the condiments we use on our food as well. We should try to use low sodium items. Think about using herbs, spices and salt substitutes when you are trying to spice up your foods. Many canned soups and
prepared meals come in reduced salt versions and we should try to choose them. Also remember that low salt soy sauce and ketchup are options as well. The Mayo Clinic staff suggest another step in creating a strategy to consume a more heart healthy diet. This is to plan our meals ahead. Using the tips and categories of foods that we have discussed previously can help us make a meal plan. Thinking about things like portion size, lean proteins, avoiding fat and reducing sodium intake provide a framework for planning heart-healthy meals. Never underestimate the need for variety in your diet. This may help you stay on track to a heart-healthy diet.

What Is A Urogynecologist Y
ou may have heard of urologists, doctors who treat disorders of the urinary tract or gynecologists, doctors who focus on the female reproductive system.
these organs are connected and are located in a very close area; problems with one organ or system often create problems with the others.
But what is urogynecology? A urogynecologist is a doctor who completes an obstetrics and gynecology residency and a fellowship in female pelvic medicine and reconstructive surgery.
Approximately onethird of women experience pelvic floor problems and can benefit from a urogynecologist’s care.
The fellowship consists of three years of additional training focusing on advanced treatment options, including both surgical and non-surgical therapies used to treat problems with the pelvic floor. The medical term “pelvic floor” refers to the organs and systems in a woman’s pelvis. These structures include the bladder, uterus, vagina, and rectum as well as the surrounding muscles, tissues and nerves. Many of

Anatomical (body) changes which may occur after having a baby is
one of the most common reasons women become incontinent or have other problems with organs in the pelvic area. Our training in obstetrics gives us tremendous insights and understanding about how a woman’s body changes after childbirth. Combining this understanding with our comprehensive fellowship training gives us the unique ability to treat problems that may affect the systems and organs within the pelvic floor -- even years after giving birth. Another common problem is the need to go to the bathroom often, when infection is ruled out the likely cause is known as overactive bladder. Urogynecologists also treat patients with fecal incontinence – an inability to control bowel movements. Although most of these complications are not life

threatening, they often have a negative impact on a woman’s quality of life. Many patients tell us they hesitate to spend long periods of time away from home. Some have given up exercise and other activities they once enjoyed. In the overwhelming majority of cases, a urogynecologist can help. Women can benefit from a variety of non-surgical treatment options, such as medications, physical therapy and/or changes in diet, daily habits or medications. Stress And Chiropractic
T
he modern world is becoming increasingly complex, and the demands on our time and attention are many. Our jobs are also more complex, By and the business world Dr. Katie Schuler D.C. has become more competitive. With the industrial age came helpful machines and increased productivity, but it also brought us the jackhammer, the loudspeaker, the motorcycle and car, and innumerable noisy machines. Physical stresses such as sound, air and water pollution have worsened over the last century, especially in the United States. We are also, because of electronic media and the Internet, aware of troubles and tragedies around the world that previous generations would never have known about, creating anxiety. Taken together, these assaults on our senses create a physical and emotional reaction. The word for this overwhelming feeling is stress. It’s term borrowed from the field of engineering, in reference to physical forces upon mechanical structures. The term was first used in its modern medical sense around the turn of the 20th century by Walter Cannon, a Harvard physiologist. Cannon also first described the “fight or flight” syndrome, and noted the damaging physical effects of a continuous stress response. Modern healthcare is increasingly recognizing that many illnesses are caused by stress, or worsened by stress. In fact, in every chiropractic office, many patients will show the ravages of chronic stress. While chiropractic can treat the effects of stress on the body, such as headaches, it can also help the body manage and process stress in a healthy way.
How We React to Stress
We perceive stress from three basic sources: our environment, our body and our emotions. Environmental stress includes noise, weather, physical threats, time pressures, and performance standards. Body stress includes disease processes, organ malfunction, poor nutrition, poor sleep, and physical injury. Emotional stress is more difficult to define. It encompasses our reactions, in thought and emotion, to environmental and physical stressors. Criticism, for example, is devastating to some, while others seem unaffected. In many ways, however, we are each in control of how much emotional stress we will feel. Relaxation techniques, meditation and contemplation, hypnosis, and similar approaches can all have a strong mediating affect on stress. These techniques provide tools that allow one to gain greater control over reactions to stress, and therefore reduce the overall effects of a stressful event.

All of this is very useful if we’re running from a prehistoric raptor, or confronting a more modern threat to physical safety. When prolonged, however, the long-term effects of this state can be disastrous to good health. Many studies of people who have been subjected to chronic stress have found evidence of the negative health effects of stress. These effects include high blood pressure, damage to muscle tissue, diabetes, infertility, damage to the immune response, and slowed healing from disease and injury. Stress reactions are also at the root of disorders such as post-traumatic stress disorder, and stress has been linked in human and animal research with cardiovascular disease.
Chiropractic Treatment and Stress
Chiropractors work primarily with the spine, the root of the nervous system through which nerve impulses travel from the brain to the rest of the body. One effect of chronic stress is prolonged muscle tension and contraction. This muscle tension creates uneven pressures on the bony structures of the body, often leading the misalignment of the spinal column. Chronic stress also leads to nerve irritation. The adjustments of a chiropractor release muscle tension, and that helps the body return to a more balanced, relaxed state. Adjustments also reduce spinal nerve irritation, and improve blood circulation. These changes may be enough, in many cases, to convince the brain to turn off the fight or flight response, beginning the process of healing. A healthy and balanced spine is one key to effectively managing stress. Chiropractors are trained in nutritional and other therapies for stress. Several nutritional supplements, including B vitamins, help the body cope with stress. A Doctor of Chiropractic may also recommend relaxation techniques, and discuss posture and environmental changes to help recovery from chronic stress. A chiropractor cannot make a job less stressful, or create a quieter, calmer world. What chiropractic treatment can do is help you develop healthy responses to stress, reducing potential physical damage.
Ways to reduce stress
Wake up your spine – do some simple spinal stretches daily to enhance posture, breathing, and health. Eat the breakfast of champions – breakfast should consist of anti-oxidants (fruits and vegetables), fast-absorbing protein (nondenatured
whey or eggs), and high-quality fat (coconut). Your body should consist of 70% water, but you may measure around 50%, especially in the middle of the afternoon. Optimal hydration, with pure, clean water and functional beverages is often the limiting factor in maintaining your optimal energy and stress levels.

Effects of Stress on Health
Stress, whether physical or perceived, triggers a fight or flight response. This is a systemic physical reaction, affecting almost every part of the body. The hypothalamus, a part of the brain, stimulates the sympathetic nervous system (SNS). The heart rate increases, blood volume and blood pressure increase, blood is directed away from digestion and the extremities. Vision becomes more focused, hearing more acute. In response to the 20

A Hospice Nurse Forever! Meet Delaware Hospice’s Admissions Nurse, Amanda Jamieson. the transitional process. We always focus on the care, wherever the patient and family want it, and do whatever we can to facilitate that. Coincidentally, Amanda was born in Christiana Hospital. She lived in Newark until a recent move to Middletown with her husband and two children, aged 3 and 1 year old.

manda Jamieson probably became a nurse because her aunt was a nurse for 20 years. “She influenced me tremendously and encouraged me to try nursing. I did, and I loved it! ” When Amanda’s grandmother became ill, the family asked Delaware Hospice to help care for her. Amanda witnessed firsthand the type of care offered. She said, “They really do handson patient-focused care. When you are nursing in other facilities, you must do your required tasks, then hurry up to the next patient. You don’t have time to spend with one patient to really understand his or her needs, or who they are. It was quite beneficial to be in a position to compare specialties, and I became convinced that my future would be as a hospice nurse. Delaware Hospice hired Amanda as a registered nurse in the Admissions Department and trained her to do home visits as well as to assess individuals for admissions to the program. Amanda was ecstatic about the opportunity and five years later remains equally as enthusiastic. Amanda described a typical visit. “We are the first team member to make a visit, so we never know what to expect and it’s often a bit chaotic. We spend time listening to the patient and caregivers, trying to figure out what’s going on. There’s always a reason to call. No one just wakes up one day and decides to call Delaware Hospice. We then educate the family about our services, and we work with the patient’s physician to review and organize medications.” For the past two and a half years, Amanda has held the position of hospital liaison at Christiana Hospital. “I love it here, she said. “I am all over the place, visiting patients, seeing new referrals. Staff knows to call me if their patients have questions. Doctors and social workers call me directly about new referrals. Everyone has my cell phone number. A physician can pick up the phone and say, “Hey, Amanda, can you come up real quick?” “Sometimes a decision is made to stop a patient’s treatment and the family is in a crisis situation. They are on their way home and don’t know how to care for their loved one. I’ll come in to talk with them and work with them through

Amanda graduated with an Associates Degree as a Registered Nurse from Del Tech, then earned her Bachelors Degree from Wilmington University, and has continued her education in the Nurse Practitioner Program. She expects to graduate next year. She said, “My future plans definitely include staying on at Delaware Hospice, eventually as a Nurse Practitioner. I can’t see myself doing any other type of work!” Asked how hospice work has impacted her, Amanda replied, “It’s changed my whole outlook. This type of care has such a positive effect. It’s wonderful to be able to provide care to a family, to give them a sense of relief as you take some of the pressure from them; and to encourage them that they can do this.” She said, “Initially, the family might not even want to talk to us. However, once they do and they realize how much we offer, it’s a great relief to them. There’s such a misperception in the public that we simply come in with meds and it’s the end. It’s just not true. We come in, we manage symptoms, and we support family members including children. We help the patient and family live life’s special moments to the fullest. Once we break that barrier of misperception and overcome those obstacles, they feel such relief and appreciation for our help.” Amanda talked about families’ fears. “Their greatest fear is that they’ll be home and unable to care for their loved one--unable to take care of basic needs. With the support of Delaware Hospice’s team, they no longer need to worry about this. We take it one day at a time, working with the best plan of care for each family. Each family varies on the needs. It might be as basic as medication, or it might be where to get more care.” Others fear the cost. How much? Will my insurance pay for it? When they hear that we’re a nonprofit hospice; and that even if you lose your insurance, we’re still going to be there with you. You’re not going to see a bill—it lights up their eyes!” Alan Henderson, Delaware Hospice’s business development manager said, “Amanda Jamieson is the essence of what a hospice nurse should be when it comes to patient care. She’s all about delivering the care necessary to elevate a patient and family’s quality of life. Amanda is an excellent nurse and has been recognized as such

by the physicians and clinicians in the hospital.” More than nursing skills, an admissions nurse must be able to educate the patient and family. This may involve teaching how to turn patients, or simply reassuring them or getting them started in a caregiver’s role. Amanda remembered one family ready to transition to their home to care for a loved one. At the last minute the home care aide didn’t work out. She said, “The caregiver was in tears, frantic. I said, ‘Give me the phone. We’re going to call health agencies together to find someone satisfactory.’ So we went through it together and you could just see the relief in her face.” Why is hospice nursing such a rewarding experience? Amanda said, “Our families don’t realize how much they help me more than I help them. They give me such a different perspective on life. They hug me and thank me, but they have no idea how much they have impacted my life in a positive way.” “I wasn’t sure before, but now that I’m here, I have no desire to be anywhere else. I’d rather be here doing this. I want to talk to the families, learn what they really need, be able to help them work with the doctors, and figure out how they can get home or wherever they want to go.” “I want to be a hospice nurse forever!”

Diabetes is a common disease: • Approximately 75,000 adults, or 8.10% of Delaware’s adult population, have diagnosed diabetes. • Adults without diabetes who are overweight and not physically active are at risk of developing diabetes • 64% of all adults in Delaware are overweight or obese
Susan’s career is reflective of her commitment to the health and wellbeing of our community. Since 2001 she has locally, and across the Delmarva Peninsula, implemented education and awareness programs, educated corporate and community citizens, and facilitated signature fund raising events. Working collaboratively with community leaders, corporate executives, volunteers, countless businesses, and local organizations, she has raised in excess of $10 million. Ms. Polikoff earned her Master’s Degree in Organizational Leadership from Wilmington University, and is also a University of Delaware alumnus. She is an active member of the Rotary Club of Wilmington and a volunteer for the Delaware Symphony Orchestra. She lives in Newark with her husband Jim, and their twin felines. Susan chose to further her professional journey by joining the American Diabetes Association in 2013. The prevalence of the disease and the astounding rate of growth coupled with limited awareness and underfunded research was the challenge she needed to join the organization and facilitate the growth of the Delaware office that will meet the needs of our community. Diabetes is a serious and costly epidemic that poses a major public health problem. The American Diabetes Association believes that if we are to truly make an impact against this devastating disease we must engage our communities on every level, in every facet of our mission.

Diabetes is a costly disease: • People diagnosed with diabetes have medical expenses that are approximately 2.3 times higher than people who do not have diabetes. • Individuals with diabetes spent approximately $13,700 on medical costs in 2012. • In 2012, diabetic expenses in Delaware totaled $860 million, and indirect expenses, such as lost productivity and premature mortality, totaled over $260 million.

It was a beautiful, sunny day in May of 2006 when I went for my annual mammogram. By the end of my appointment it was pouring “tears”, my tears. I was told I had breast cancer. My life was now headed down a different path. A path that was totally foreign to me. I could not believe this was happening to me. No one in my family had cancer. That just did not happen. My tears continued to flow until one day I decided that either the cancer was going to get me or I was going to get it! Those who know me, also know I am a fighter and my fighting spirit resurfaced. Once I became proactive and took a stand against this disease, things started to fall into place. My life became a whirlwind of tests, biopsies, two lumpectomies and finally a mastectomy. Follow up treatment ensued and today I am living a happy, full and productive life cancer free!
I give a lot of credit to my doctors, friends and family that were my core support and were by my side every step of the way. However, there was another group of people just as important and supportive that became my new friends. These ladies were cancer survivors too and were more than willing to share their experiences, encouragement and support. The saying “been there, done that” truly applied and it was comforting to know these ladies made it and so could I. To any woman struggling with a recent diagnosis or undergoing treatment, please reach out to a fellow survivor. You don’t have to travel this road alone. Today I am employed full time as a Community Relations Liaison with MeadowWood Hospital. Working in the mental health field, helping others has opened many doors for me. My after hours are spent volunteering with organizations that help and support those who have experienced a traumatic event in their lives. To me, it is all about being grateful, giving back and helping others... just as my wonderful husband, two daughters, grandson and many others gave back to me.

When asked to give my testimony after my first diagnosis, I said I always knew God would heal me one of 3 ways: by a miracule, by medical treatment, or by death! Many congratulate me for surviving breast cancer three times. Even Dr. Fox, head of UPenn’s Rena Rowan Breast Center, said he’s only seen 3 people in 30 years survive breast cancer in 3 sites. I really am a walking, talking miracle! My grandmother died of breast cancer when I was 12, so I went early for a baseline mammogram resulting in a first diagnosis at age 37. With children ages 12, 9 and 7, my prayer was to see my grandchildren grown and married. Despite a “cyst” on ultrasound, my radiologist said “Just do your routine mammogram next year.” I had an uneasy feeling and two months later I found another lump. I asked the surgeon to remove both the lump and cyst. While too deep to remove in his office, surgery could “remove all fibrocystic tissue.” I woke up to the news of breast cancer under the cyst—not visible on mammogram or ultrasound.
Since my breasts were too dense for technology back then, I requested a full mastectomy. My plastic surgeon specialized in breast reconstruction and said tram flap reconstruction was my best option. Surgery was difficult and chemo chemically exhausted me for months. I thought my ordeal was over, but 10 years later (2007) I needed a hysterectomy for metastatic breast cancer in my ovary. Incorrect pathology started a year of chemo for a new type of cancer I didn’t actually have. And a follow-up CAT scan found malignant breast cancer in my lung also. The oncologist suggested chemo for the rest of my life. I said “I don’t think God would want me to feel like a dishrag forever!” A second opinion at MD Anderson in Houston determined I couldn’t have that type of cancer or I would have been dead. They re-ran pathology to find it was the original cancer that spread to my ovary and lung. The next week they removed the as yet untreated lung lesion. “When should I come back?” The specialist answered, “I hope never!” Now I’m on a drug to suppress estrogen and stop new tumors from forming, and I thank God for my health. Whether I’m helping clients with financial situations or helping breast cancer survivors, I know every day I live and breathe, God has a plan for me!

I am a 5-year breast cancer survivor as of Memorial Day 2014! My goal is to inspire others with my story of advanced Stage 3 cancer and give them hope.
I was 47 yrs. old when I noticed my right breast nipple and areola was darker than the left side. I did a breast exam and felt many cysts from my having fibrocystic breast disease. I called my doctor and after an exam, he felt a large cyst and ordered an ultrasound. The ultrasound showed a mass and the looks on the faces of the radiologist and the technicians made me worry. I had seen those looks before, when my daughter was three she was diagnosed with liver cancer. I knew a biopsy was my first step. Being a nurse, a surgeon I used to work with was wonderful about getting me in. After the biopsy, he took my hand and said, “It’s cancer but this type of cancer responds very well to chemotherapy and you will be fine.” Wow-- the two dreaded “C” words- cancer and chemo! I smiled and thanked him, then walked to the dark parking lot alone on Thanksgiving eve. I called my husband, close friends, and family and the tears started. Thinking about how much I loved being a mom to my 14 year-old daughter (my cancer survivor) and my 16 year-old son; fear consumed me for weeks. I knew I must stay positive but the survival stories I had heard were all early stage cancer. I prayed that I would be filled with the hope and promise that Jesus gives. Then, the darkness began to lift- my faith, my loving family, and my friends kept me strong. I had 7 months of chemotherapy at Helen Graham to shrink the 5cm tumor. The tumor didn’t seem to be shrinking, but my surgeon told me that it was probably dying from the inside. I stayed hopeful because I knew I had the best possible care. I decided to use Hopkins for the surgery because they are amazing, they did my daughter’s liver resection 18 years ago that saved her life. I had a right mastectomy and then radiation. After this treatment, a mammogram on my left breast showed a possible mass. I couldn’t believe it... I was just getting my hair back and they tell me I need another biopsy! I decided that I wanted a mastectomy no matter what the results. 13 lymph nodes were removed, 9 were positive. Then had bilateral reconstruction and tattooing a few months after that. I’m very happy with the results and it’s so nice not to have to buy bras anymore! It was fun to experiment with the wigs and scarves. I had more compliments on my wig than I ever had on my own hair! A nice side effect- the chemotherapy helped my complexion! I’m now on hormone blocking therapy for a few more years. Though the treatments were challenging, I wake up knowing my life is so blessed and has been enriched by my having gone through this cancer experience. The wonderful staff and patients I met through my journey, my husband, and friends who kept me positive and cheered me on over the years. Each day is a gift that I cherish! - Jodi Sheats 52

I am 50 years old, and was born in Costa Rica. I have lived in the United States half of my life and am a proud citizen of the USA. I am a divorced, single mother of a 20 year old amazing daughter, a Dean’s List Student at UD.
I was diagnosed with Breast Cancer in October of 2010. That day was probably the saddest day of my life - for about 3 hours - it didn’t last long because I decided with the help of God my life would be positive and healthy. I had a series of biopsies and then a week before Christmas my chemotherapy began. It lasted for 16 weeks. Christmas day of 2010 I began loosing my hair (which was one of my best attributes and still is). I decided to call friend of mine, a hair dresser, she came to my home and cut it short- instead of waiting for it all to fall out. Surprisingly, it didn’t fall out completely, but I had to wear wigs to work. I actually had fun with the wigs, and why not, some women wear them for fun. Another surprise was that I didn’t get sick with my chemo sessions, instead, I got insomnia. So, the days that I had my 5-hour chemo sessions (always on a Friday), I went out with my friends and danced all night long. I also went to the gym 5 times a week and did pretty hard cardio, my belief - the stronger I made the blood running through my veins the faster the Cancer would be eliminated (my theory only, but one my Oncologist loved). In April of 2011, I had my Lumpectomy, which came back clear, no Cancer, God is amazing! Lymph Nodes were also clear and “clean” ;). About a week later my radiation sessions began, I was told that the skin around my breast would burn, well, it didn’t happen, another plus. But, I sure got tan! I wanted to tan the other breast but the radiologist didn’t allow me. My experience with Cancer was “a blessing in disguise”, it made me see how precious life is, that people that do really care, and it made me appreciate my family and friends much more. I began telling everyone how much I love them, we shouldn’t waste time not saying those beautiful words. I felt God with me even more, how He is always with us, at ALL times- I pray more now. Cancer touches us deeply in a way that no one else can understand. God has more for me to do; including things for others. I have a boyfriend who understands how life with me works and how we all can love with such pure love. Now that, is just plain amazing. We should thank the Lord every day to be alive - I believe this regardless of having had Cancer, though maybe I understand it better because I had Cancer. Life is wonderful, three years cancer free; living each day to the fullest – helping others and loving everyone.

My journey began eight years ago. I was 59 years old. I remember the phone call to my cell phone. I was in Monterey California visiting my daughter and her family. It was the nurse from the Breast Center. She said I needed to come back in and discuss my recent mammogram with the doctor. Her voice sounded urgent- I knew this call would be a defining moment in my life. I returned home to doctor’s visits, biopsies, and then the phone call from the breast surgeon requesting that I bring my husband to my upcoming appointment. Wow, this was for real. Terror was probably the best way to describe my feelings at that time, what would I look like without hair? Could it spread? The diagnosis was DCIS, ductal carcinoma in situ- no I would not need chemo or radiation- no it had not spread- yes I would need a total mastectomy because it was in most of the milk ducts of my left breast. I remember the night before the surgery. I knew part of me would be gone the next day, and I would never look the same. I also knew God would take care of me. I had total faith and let go and let God take control. I had decided to have reconstruction via a tram flap, and this was not an easy operation. I’m not sure I would recommend it but I would encourage having some sort of reconstruction. Well fast forward eight years and I remember going through all of the above, it was an important part of my life, but it did not stop me from living a full life. I have been blessed with a wonderful husband. We have been married for 45 years. We have four beautiful daughters, four terrific son in laws and eight fabulous grandchildren. I have recently retired from my part time job of 32 years as a Museum Guide at Winterthur Museum. I enjoy playing duplicate bridge, tennis, golf and squash. Breast cancer was a small blip in my journey of life. I try to focus on my blessings and not the blips along the way.

I consider myself a very lucky woman. Diagnosed with breast cancer in 1990 at the age of 46, I fortunately had discovered it early. As ductal carcinoma in situ, my great surgeon, Dr. Balin, was able to remove all traces of the cancer. No radiation treatment or chemotherapy was necessary and wonderful care was provided to me by the Women’s Center at St. Francis Hospital. I did have a bit of a scare the following year when another lump was detected, but it was benign. Now approaching my 70th birthday, I continue to have an annual mammogram and check up with my doctor. An annual visit to my dermatologist for a scan for possible skin cancer is also done. As a result of the latter, a basal cell cancer on my nose was detected which was surgically removed. With this history, and discovering the mission of the Delaware Breast Cancer Coalition, I was happy to get involved by volunteering at the Great Stuff Home Store. I have also been lucky in my private and professional life. I was married to a wonderful man for 32 years who unfortunately died at the age of 56. We did not have children of our own, but teaching in elementary schools for 31 years, provided me with the opportunity to work with many wonderful children! After retiring from active teaching, I continued to work as a coordinator in environmental education and as a grant coordinator for the Delaware Department of Education.
Having now fully retired, I enjoy attending classes at the Osher Lifelong Learning Institute, volunteer for Meals on Wheels, Fish (driving seniors to medical appointments), Arasapha Garden Club, and serve on the board of the New Castle Library Friends. Also, I have met another wonderful man and we spend time biking, hiking, traveling and enjoying our friends and relatives. Life has been and is good!

Dorianne Short
For over a decade, Dorianne Short has dedicated her time and efforts to raising awareness of ovarian cancer with an emphasis on raising funds for research to discover a reliable diagnostic test. An 11 year survivor herself, Dorianne is the founder and President of the Delaware Ovarian Cancer Foundation (DOCF), founded in 2009 as the premier ovarian cancer organization serving Delaware and the surrounding vicinity. Dorianne formerly served as Delaware Chapter President of the National Ovarian Cancer Coalition (NOCC) from 2005-2009. Under her leadership, the DOCF has raised over $250,000 and she has dedicated more than 10,000 hours of volunteer time to raise awareness and help women with ovarian cancer. In addition, she was instrumental in raising well over $100,000 for NOCC during her tenure. Through her efforts of donating funds raised to various research grants, DOCF has become ‘a Partner in Science’ with the Ovarian Cancer Research Fund. In 2013, Dorianne was the recipient of the prestigious Governor’s Outstanding Volunteer Award.
Dorianne oversees two annual DOCF fundraisers: the Teal Ribbon 5K to Fight Ovarian Cancer in May and the ‘On the Wings of Hope’ Teal Ribbon Luncheon and Silent Auction in September. With a small group of dedicated volunteers, other awareness activities and projects are organized throughout the year with an emphasis on September, Ovarian Cancer Awareness Month. Some of these activities include: exhibiting at various local health fairs and women’s events, mailings to ob/gyn offices throughout Delaware, a Salon Awareness Program (distributing information and awareness baskets to hair salons), Turn the Towns Teal (a national initiative placing teal bows in various locations to promote awareness in September) and a new upcoming project of providing ‘new patient bags’ to newly diagnosed women. Dorianne truly lives her passion, which is ‘Hope for the Present, a Cure for the Future’. This passion has transformed her personal illness of ovarian cancer into a healthy, vital, supportive and expanding all-volunteer organization. She urges all women to learn the symptoms and risk factors for ovarian cancer because, currently, the only prevention is early detection. I am an Ovarian Cancer Survivor and I wear this label now as part of my life, part of my Journey and it is part of the new me.
I started this phase of my life very unsuspectingly in 2012. My Hereditary did not warn me nor did any of my life’s circumstances or personal habits. Ovarian cancer is a quiet disease that once you tally the symptoms it is usually in a later stage. I have been married for 56 years, a mother of three children, one is deceased, and I have four wonderful grandchildren. I was a nurse during my career and even though I have found technology has advanced beyond my working days, I have a keen understanding on how one should feel. When you look at the symptoms of early ovarian cancer, even collectively, it is not enough to cause alarm. My symptoms persisted and I went to the doctor with digestive issues, bloating and some mild pain similar to menstrual cramps. Some blood values fluctuated which did not fit a pattern either. It wasn’t until I went for a virtual colonoscopy, in the evaluation process, that the doctor saw shadows in areas around the right ovary area. A virtual colonoscopy can only diagnose colon issues but the blurs and surrounding area can be observed in the background as a warning to seek further investigation. Since there is no test that can definitively diagnose ovarian cancer other than a CAT scan, my family doctor ordered one, which confirmed my diagnosis. He, in turn sent me to Dr. Rashid, GYN who put me “on the fast track” to surgery, chemo and remission: all done at the Helen Graham Center under Dr. Browosky’s care. I could hardly face the uphill climb of recovery from such devastating surgery and then tear down my strength again in chemotherapy. This persistent disease needed eight months of problematic chemotherapy. My Journey began with little sunshine and definitely no rainbows. But what kept me strong was a reconnection with friends, survivors and family. It was a time of renewal of life, a new beginning and a fresh start. All that was important suddenly isn’t. Your goals and zest for life changes. You finally understand the philosophy of day to day and the importance of celebration. Fresh out of chemo, I joined the first Delaware Ovarian Cancer Foundation 3k walk, to begin my newfound life, in remission. As luck would have it, I also reconnected with Dorianne Short, who I knew years ago, under different circumstances, who started the foundation, and has become “my sister survivor!

My name is Jackki Kennedy and I am the founder and president of the MDP Cancer Fund, which hosts “The Kennedy Black & White Ball”. As a 5x breast cancer survivor, and one who has lost 2 family members to this dreaded disease, I have a strong desire for a cure to be found and to help others who are dealing with cancer. Several years ago I decided to move forward with a vision to raise funds for cancer research and programs. It was at that time that the KBWB was launched. The Ball is an annual black tie fundraiser event given each year at the beginning of November. The proceeds from the event are donated to the Helen F. Graham Cancer Center at Christiana Care, Cancer Special Needs Fund. The Center is one of the leading institutions in the nation in research and cancer care. SAT vs. ACT Which Exam to Take? Presented By Maggie Lage Article By: Dr. Raymond J Huntington, Co-Founder of Huntington Learning Center
T
he time has come for your 11th grade student to begin the college search and application process and to register for the SAT or ACT. You may have heard that the SAT is changing. The redesigned SAT won’t be administered until March 2016 so existing 11th and 12th grade students should continue to focus on the current SAT. Let’s get familiar with the current SAT and ACT in order to help your student decide on which exam to take. Here a few points to understand the differences between these two exams: Exam focus – The SAT is an aptitude test that measures student reasoning and verbal abilities. The ACT is an achievement test that measures what students have learned throughout their education. Exam sections and scoring – The SAT’s sections include Critical Reading, Mathematics and Writing. The total SAT score is out of 2400 based on the sum of the three sections. The ACT’s sections include English, Mathematics, Reading, Science and an optional Writing section. ACT’s total composite score is out of 36 and is based on the average of the English, Mathematics, Reading and Science sections. The ACT Writing section is an additional score.
Both widely used – In 2013, 1.8 million students took the ACT and 1.7 million to the SAT. According to a 2013 New York Times assessment of students taking both exams, the SAT has more test-takers in all Northeastern, Atlantic coastal states (except Florida), Nevada, Texas, Indiana and Alaska. The ACT dominates in the West, Midwest, North Central and South. Colleges across the U.S. require these exam scores as part of a student’s application package. Wrong answers – The SAT currently penalizes students for wrong answers. The ACT is scored based on the number of correct answers and test-takers are not penalized for incorrect answers. Individual students may perform better on one exam over the other. So, which exam should your student take? Here are a few considerations as you decide:

esearch on aging offers a striking model of aging as a potential time of brain health and growth. Mental stimulation and creative outlets can provide the opportunity to lead better quality lives filled with more knowledge and wisdom — and an increased capacity to contribute to society. In one study, participants in creative programs like poetry writing, painting, or choral singing classes reported better physical health than those in the control group, and reported learning “new skills and new ways of looking at and expressing things.” “Getting creative in your later years is a self-fueling engine,” is the message of Dr. Gene Cohen, founder of George Washington University’s Center on Aging, Health & Humanities. “The more you do, the more you can do.” The University of Delaware has been a leader in lifelong learning programs that offer mental stimulation, creative outlets and
social connections that positively impact quality of life. The programs feature an academic cooperative model which encourages age 50 plus members to share their expertise and talents as volunteer instructors and through committees. I still get this feeling of euphoria when I become grounded after an inversion.

xcept now my inversions come in the form of yoga postures. Perhaps it was the “new perspective” I was shown as a child that drew me to Aerial Yoga. As a young adult I thought it was all about the perspective, but as I came to study Aerial Yoga, I realized that the benefits of being turned upside down extend far beyond just having a new perspective. Inversions are a time-honored yoga practice that increase blood flow to the brain and pituitary gland, said to relieve stress and elevate the mood... something everyone can use during this season of cooler temperatures and shorter days. The lack of natural daylight can easily lead to Fall Fatigue. Beyond the esoteric, inversions are credited with alleviating Seasonal Affective Disorder for many significant reasons: Fight the Effects of Gravity: Gravity continually pulls down on our vital organs. Inverting gives your vital organs a break by reversing the effects of gravity, and removing stagnation from the limbs of the body. Reversing the pull of gravity on the body nourishes the organs and glands above the heart. Moreover, when inverted, your body is in line with gravity, allowing your spine to go into its proper alignment. Regular inversions encourage good posture. Dopamine deficiency is associated with Parkinson’s disease, depression, attention deficient disorder and substance abuse. Serotonin helps regulate communication between cells and controls appetite, sleep, memory, learning, mood and behavior. A lack of serotonin is associated with depression, anxiety and insomnia. (The Dr. Oz Show: Two Alternative Health Secrets Feb. 2010). The improved circulation of blood flow and oxygenation to the brain can increase concentration and mental clarity.

In today’s Pilates studios, instructors still use equipment such as the Cadillac and the Reformer among others, and his original training techniques are also used on the mat or with small equipment. While detained as a prisoner of war in England, he continued to develop, what he called Contrology, a holistic system of exercise that he used to help other inmates improve their health. In about 1925, Pilates migrated to the United States. He and his wife, Clara, founded a studio in New York City and directly taught and supervised their students well into the 1960s. The Pilates system encourages students to use their mind and mental focus to control their muscles; and the physical exercises focus on the core postural muscles. Pilates exercise teaches breath awareness, spinal alignment, and strengthening the internal core and abdominal muscles. In his early years in the United States, Pilates built a following in the dance community because his exercises developed the strength, flexibility and stamina needed for performance. Those exercises are still practiced today. People who practice Pilates 43

o hell with certainty!! Too often we live our lives addicted to certainty. We stay in what is familiar and what is comfortable. We love what we know, even if it costs us what we desire the most.
Somewhere in life we are taught that we need certainty to be happy. We spend our lives waiting for that perfect moment to take action, for the perfect moment to live our lives. That certainty will protect us from getting hurt and keep us from feeling like we are not enough. We thought by gaining a degree, it will give us the certainty of a well paying job. Working hard will give us the certainty of job security. That money will give us certainty of maintaining the lifestyle we want. Listening to the doctors will give us the certainty of health and taking a vow will give us the certainty of fidelity. As
many of us learned through life experiences, none of this is true. Anything and everything can be lost or taken away from us. We have been sold a pack of lies and it is all B.S. The ONLY certainty that truly exists is the certainty of what is inside of you. There is no perfect moment. The moment is now. You can’t wait to feel certain you won’t get hurt before you fall in love, you will spend your life alone. Go out and love, be willing to get hurt and realize that you will find someone who is worthy of the gift of your love. You don’t need ANY more certainly to live the life you want.

Why wouldn’t we want to clarify or deliver a message faster? Another very close phrase published in 1911, "Use a picture. It's worth a thousand words." quoted by the newspaper editor Arthur Brisbane discussing journalism and publicity. The Russian writer Ivan Turgenev who wrote Fathers and Sons in 1862, said, "A picture shows me at a glance what it takes dozens of pages of a book to expound." The psychologist Jerome Bruner of New York University has described studies that show that people only remember 10% of what they hear and 20% of what they read, but about 80% of what they see and do.
So now that you know the importance of visual aids, the next step is using them. You can always use the freebie (clip art) that comes with most of the software today, but when you really want to stand out, you need original art! Original art can depict your message or tell your story which will be best provided by an Illustrator. An illustration can bring to life a logo, article, story, poem, event poster or piece of textual information by providing a visual representation of something described in the text. Illustration comes from the Latin word illustra'tio, illu'stro meaning enlighten, irradiate. An illustration is a “PICTURE” a depiction made by an artist, such as a drawing, sketch, painting, photograph, or other kinds graphic means.

“It’s Just A Sign, It Doesn’t Have To Be Professional.” ? y l l a e R , h O
rom the business card in your pocket By Carol Mack Warner to the digital billboard President you see on the highway while going to work, signs are so much a part of your everyday life you take them for granted. Outside to inside, lobby to office, in the home, everywhere, signs are there to announce, inform, direct, warn and guide. Signs are a visual, non-verbal way to communicate and not just by the words alone. The type of sign, location, choice of font or letter style, sizing, layout and design all play a part in getting the message and your best image across. It’s not just a sign – it’s tells others who you are and if designed correctly, will attract the type of client you want. I have a friend who decides where she will eat if she likes the sign outside. If it’s neglected, she wonders what else is being neglected. This applies to all the signage you have in your business. Does your print material reflect the type of business you do? A comic book store will likely suffer using signage that a funeral home would use and vice versa. A comic book letter style and super hero are as out of place as somber tones and lettering are for the comic book store. Where do you go and what do you look for when searching for a graphics professional? Often word of mouth is a good way to start but don’t stop there unless they make you feel welcome and comfortable to discuss your needs. A full service sign company can save time and money if they fit. They can design and produce the final product without having to search out a company to produce your sign. A good relationship should form and they can provide other types of signs that follow your look without having to repeat the introduction process. If you want a logo designed, determine the purchase price of the design package so you can own the artwork. This is standard in the industry and should be discussed prior to work being done. This will allow you to take and use your artwork freely. Remember it is your business and you are the driver, so don’t be intimidated if it doesn’t work for you. If you work with the designer, you should be asked a variety of questions including the type of client you want to attract. Yes, that is what a sign can do by the way it is designed. All you have to do is watch TV advertisements to see how that works. Other questions include a discussion of the following topics: Type of sign, where will it be located and mounted, what are you trying to say and conveying the message using the visual clues of font style, colors and images. www.TheWomensJournal.com
Type of Sign: The type of sign is determined by the purpose and what you want to say: Large format printing, posters, banners, A.D.A. interior signage, real estate site signs, traffic signs, business cards, engraving, raised lettering, cast bronze plaques, flags, specialty items, vehicle lettering, car wraps, window lettering, labels, decals, magnets, directories, trade show, floor graphics and custom items all have different properties and uses. It is amazing to think that many weren’t in existence even 20 years ago. New products are coming out all the time. Location: What distance away from the sign will viewer stand? How will it be mounted? Please check with your township for the appropriate permits and regulations for exterior signs. A.D.A.(American Disabilities Act) signage with raised lettering and Braille II translation also have specifications for production, installation and use. Layout - Image This is where your message will work or fail. The layout of your message is influenced by many considerations including purpose, fonts or letter style, graphics or icons, color and size. If your message is to announce your place of business, then it is important to be visible from
the street quickly – so cars passing by can see the sign easily within the few seconds they pass by. Remember, this applies to vehicle lettering as well. The back door is a “billboard” and gets the greatest length of viewing time from the people traveling or stopping behind the vehicle. Drivers pass by the sides quickly, their eyes on traffic ahead. This valuable “real estate” needs to be optimized for quick viewing. Window lettering, banners, interior placards, business cards and brochures are saved for more detailed information. Traffic, parking and warning signs are designed to impact immediately and are simple and direct. Choice of font or letter style, graphics and materials express more information than the written message. The prospective client gets a feeling about the business from the sign: “It looks like it’s a good, clean and friendly place to eat.” “Too cheap looking – the furniture will probably fall apart.” “Outdated and bad maintenance – probably cutting back on quality.” “New sign – they must be doing well –let’s try it.” Handwritten signs are pretty much a thing of the past except in “back in 5 minutes” type of situations. Do not confuse this with professional hand lettering, almost a lost art in the industry and highly prized. The office printer is common place and can be forgiven to be used as temporary signage. After a short period of time, the sign should be removed or replaced if the message is a permanent one. Temporary signage gets tattered, worn and generally lacks the authority of your company image. There is more to it than just written words. It’s an opportunity for prospective and current customers to identify with your business and image. An experienced, professional graphic designer and sign maker will make your signs work for you.

ontemplating divorce or in the process of a divorce? Many overlook, or simply do not understand, the financial elements of a divorce. A Certified Divorce Financial Analyst® (CDFA™) can explain the financial aspects of divorce, help you evaluate all pending financial decisions, and empower you to make educated decisions during the highly emotional divorce process. Financial decisions are complicated, and often irreversible. Uninformed decisions can be detrimental to your financial future, as well as time consuming and expensive. A CDFA™ provides you with an understanding of each proposed financial option so that you are ultimately placed in the best financial position. A CDFA™ will also give you a clear view of your short- and long-term financial future. A CDFA™ works with you and your attorney to forecast the long-term effects of a divorce settlement, including details of tax liabilities and benefits. A proper divorce settlement should not only take care of your immediate family needs, but your retirement needs, as well. Many have false expectations about what their lifestyle will be after divorce. A CDFA™ can help ensure a stable economic future, and limit regret about financial decisions made during the divorce process. The Institute of Divorce Financial Analysts (www.institutedfa.com) provides this example of how a CDFA helped one couple:
John and Jane are 40 years old and have two children. They own a home worth $165,000 with net equity of $77,500. Their IRAs and 401(k) retirement plan total $165,500 in value. John earns $90,000 a year and has takehome pay of $68,760 a year. Jane has never worked outside the home and has no job skills, but she hopes to get a part time job for $8 an hour with takehome pay of $8,900 a year. The following settlement has been suggested. After the divorce, Jane and the children will live in the house, which will be deeded to her. She will also receive $44,000 of the retirement monies and John $121,500, thus dividing the assets equally. John will pay Jane alimony of $600 per month for 5 years and child support of $225 per month per child. He will also pay college costs which start in 4 years. John’s expenses include his normal living expenses, child support, alimony and college costs. Jane’s expenses include support of the children and are reduced when each child leaves home. This appears to be a reasonably fair settlement. However, an analysis shows Jane’s assets will be completely depleted within seven years while John’s investments will grow dramatically. To improve Jane’s financial future, the settlement could provide her with increased alimony of $1,500 per month for 10 years. This would actually cost John $1,005 per month in after-tax dollars. The correct child support according to the Child Support Guidelines is $1,125 per month for two children for a couple with their income. Jane also could be awarded an additional $24,300 from the retirement plans. She also may need to cut her expenses by 10%. An analysis with these changes show John will still have a surplus which he can add to his investments. If John stays within his budget and invests all of his extra income, his investments have the capacity to grow to $2.5 million by the time he is age 60.
Imagine where Jane and her children would be financially if she did not consult with a CDFA! This example illustrates how valuable financial planning is in reaching a more equitable economic resolution.

Begin saving now To maximize your chances of achieving a financially confident retirement, start with a realistic assessment of how much you’ll need to save. The sooner you start, the more time your investments have to potentially grow.
Save as much as you can – you have many options If your employer offers a retirement savings plan join it as soon as possible and contribute as much as you can. If your employer offers a pension plan, find out how many years you’ll need to work for the company before you’re vested in, or own, your pension benefits.
Save for retirement – no matter what Even if you’re staying at home to raise your family, you can and should – continue to save for retirement.
Plan for income in retirement At age 65, women can expect to live, on average, an additional
20.3 years. 1 According to recent statistics, 37% of older women are widowed, 14% are divorced, and almost half of all women age 75 and older live alone. 2 So what can you do to ensure you’ll have enough income to last throughout retirement? Divorce 101 – A Basic Overview Divorce. Without a doubt one of the most difficult and life-altering transitions that can occur during your life. And if there are children involved that transition has long-lasting unforeseen effects. Making such a significant decision requires knowledge – knowledge about your rights, obligations, and the law.

he goal of this issue’s article is to provide you with a basic understanding of the process and requirements of By Shana A. Pinter, Esq. divorce in Delaware without focusing on the many issues and complications that can arise. Future articles will provide detailed information on one area at a time. The requirements for getting a divorce in Delaware are fairly simple – at least one of the spouses needs to be a Delaware resident, you must be separated from your spouse for 6 months and you must file all the proper paperwork. When you have children, a parenting class must also be taken. Most divorces are finalized without the need to go to court. After the divorce is finalized, property division starts. In general, all property obtained while you are married is considered “marital property” and is subject to division. Marital property includes assets (i.e., a home, car, bank accounts), debts (i.e., mortgage, credit cards), and retirement accounts. The court will begin the division at an even split of 50/50 and move from that point depending upon the contribution and dissipation to the marriage by both spouses.
alimony is awarded for half of the amount of years. If you were married 20 years or more, alimony is “permanent.” If you and your spouse have children you will also face custody issues. In general, custody is awarded jointly to parents with each parent having equal decision making authority and equal time with the children. Equal time with each parent is usually accomplished by 1 week with Mom and then 1 week with Dad and so on and so forth. Any combination of overnights and days can be used, as long as the result is 7 overnights with each parent over a 14 days period. For example, some people choose to have a “2/2/3” arrangement –2 nights with Mom, 2 nights with Dad and then the weekend with Mom. The next week would alternate that pattern, starting with Dad. Along with custody often comes child support. Both parents have an equal responsibility to provide financial support to their children. Child support lasts until the child turns 18 or 19 years old, depending if
he or she is still in high school. Child support is calculated using each parent’s income and the amount of time the child(ren) spend with each parent. Also considered are medical insurance and expenses related to the child(ren) such as daycare, tuition, and the like. Since it is at the forefront of most mothers contemplating divorce, our next article will delve into the issue of custody and answer the most common questions we hear, questions such as: I’ve been a stay-athome Mother and have been our children’s primary caregiver. How is custody decided? Can I get sole custody? Can I have a say in who my ex introduces to our kids? Shana A. Also considered during the property division phase of divorce is alimony. In general, in order to be eligible for alimony, you must be able to prove that you are dependent on your ex-spouse – meaning that you aren’t able to cover your expenses based on your income – and you also must be employed. Alimony is calculated by taking your income and subtracting your expenses, doing the same for your exspouse and then comparing the numbers. If you were married less than 20 years, 52

Workers’ Compensation Benefits
K
nowledge is power” is an old adage that is especially significant if you are involved in a workplace injury. All too often, injured employees attempt to navigate the murky waters of their work injury claim on their own, without the critical knowledge necessary to protect their rights. It goes without saying that your health is the most important thing to protect. Employees who are injured on the job and don’t understand the rights that they are entitled to can feel pressured to return to work before it is safe. This increases the risk that they will either become re-injured or will not heal properly from the initial injury. The most important thing to do if you become injured on the job is to get the necessary medical attention that you need. If you are not in an emergency situation, notify your employer about your injury. Make sure that your injury is documented and that a “First Report of Injury” is filed with the Industrial Accident Board, the administrative agency that oversees work related injury claims. The Statute of Limitations for filing a workers’ compensation claim here in Delaware is two years. Therefore, as a general rule, the First Report of Injury and Agreement (of the injury) forms must be filed within two years from the date of your injury. Otherwise you will loose your rights under the Workers’ Compensation Act FOREVER. I recommend asking your employer to file these forms immediately following your i njury while the facts surrounding the accident are fresh in both of your minds. This will minimize any dispute about what part of your body was injured, and the facts surrounding how you were injured.
What are injured workers entitled to under the Delaware Workers’ Compensation Act? 1. Disability Benefits Knowing and protecting your rights if you or a loved one is injured on the job just makes good, old-fashioned financial sense. Most of your rights under the workers’ compensation laws in Delaware are based on your average weekly wage (AWW for short) earned during the 6 month period prior to your injury. If you have only been at your job a short time, don’t worry- your rights will still be based on your average earnings for whatever time you have been on the job. Of all the rights available to an injured worker, the one that clients are most concerned about is the disability benefit to compensate for their lost wages. Depending on how severe you are injured, your doctor may restrict your work hours, restrict your work duties, or take you out of work completely for a certain amount of time. If that is the case, then you are entitled to disability benefits.
2. Medical Benefits If you are injured in the course and scope of your employment, you are entitled to medical benefits for the rest of your life as long as the medical treatment is reasonable, necessary and related to the work injury, and your caregiver is a Delaware Certified Workers’ Compensation Provider. A list of the certified providers may be found on the Department of Labor’s website, and is updated weekly. Worker’s compensation insurance must pay at least one medical bill every 5 years in order to keep this benefit active.

3. Mileage You are entitled to mileage reimbursement of forty cents (.40) per mile, roundtrip for your doctors appointments. I suggest keeping a notebook in your glove box so you can write down the dates of your appointments and your mileage both to and from these visits. 4. Scarring / Disfigurement If you were unfortunate enough to have suffered a scar or other physical disfigurement as a result of your work injury, you are generally entitled to compensation. Any scar and/or disfigurement that is visible when properly clothed can be awarded a monetary settlement. The amount of the settlement is determined by the location, size and visibility of the scar.
5. Permanent Partial Impairment If your doctor says that the injury which you received in the course and scope of your employment is permanent, you may be entitled to a lump sum payment. This lump sum payment is not meant to compensate you for pain and suffering, rather, it represents compensation for the permanent impairment to your body as the result of your work injury. The amount of this recovery is based on a complex formula which takes into account the part of your body that was injured, your percentage of impairment and your workers’ compensation rate based on your average weekly wage. As every work-related accident is unique, all of the rights listed above will not necessarily apply to every case. If you are injured at work, the best thing you can do is become informed about your rights under the Delaware Workers’ Compensation Act. What still amazes us to this day is how so many of our clients struggled with the thought of calling a lawyer after being injured. Despite the fact they were injured as a result of someone else’s negligence or reckless behavior, they still struggled with making the call. Thankful for the opportunity to have guided many through the challenges of being injured (medical bills, wage loss, vehicle damage, struggling with the activities of daily living, impact on relationships with family and friends), we are sure they would advise any “potential client” that contacting a lawyer is the right call. Let us share with you some of our observations as to the reasons behind the hesitation, in hopes of making the decision to contact an attorney a “no-brainer”. One of the reasons attorneys never get the call from the injured potential client is that the insurance company for the at-fault driver has pounced on them to settle immediately. The sometimes-irresistible offer of a cash settlement is often made within hours or days of the motor vehicle accident. The truth is that unless you have been in a motor vehicle accident before, you may not be aware that it can take not just hours, but days and weeks before the full extent of one’s injuries may be known. For the person struggling financially to make ends meet the insurance company’s offer of a cash settlement can be the answer to a prayer. The minor aches and pains they may be experiencing at the time are viewed as temporary, and certainly no reason to pass up the
In exchange for the cash the injured person must sign a document known as a release. By signing this document the injured person gives up any and all claims they may have against the at-fault party, now or in the future. Even if the injuries become serious and disabling over time, the injured party is typically stuck with the short-sighted deal they made with the insurance company. Since this person never contacts an attorney it is difficult to estimate how many individuals succumb to this “deal”.
We have had this very conversation with so many clients over the years that we jokingly remark there must be a book out there telling clients what to say to their personal injury attorney. These clients need to be assured that an ethical attorney would not take a case that is frivolous (or lacks merit). A legitimate case is one where the other party is at fault for the accident and the injured party is truly injured. Injuries can run the spectrum from bruises to those injuries causing death.
In fairness, clients over the years have shared stories of upstanding insurance adjusters advising injured persons to consult with an attorney before settling their claims. Whether or not such advice is offered, the injured person must take personal responsibility for the choices they make. Choosing to contact an experienced personal injury lawyer before settling one’s claim is always the right call to make. Such a call is always free to a client.
The ethical lawyer is also going to assure the client that this is not about “winning the lottery” for the client or the lawyer. As we explain to clients, our role as your lawyer is to even the playing field. The insurance company is looking to resolve every claim for as little as possible. Your lawyer is here to protect you from being taken advantage of, and to make sure that you are treated fairly and reasonably in the process. There is no down side to reaching out for an attorney sooner rather than later in the process. There is no need to be embarrassed. Your Rights Under The Fair Debt Collection Practices Act M
any of us have consumer debt: credit cards, a car loan, a mortgage, or other personal bills, and we may know what it is like to fall behind. We By Mary Higgins, Esq. may not know, however, that we have rights under the Fair Debt Collection Practices Act (FDCPA) that can protect us from harassing, threatening, and illegal behavior by debt collectors. (Business debts are not subject to the FDCPA.) Before discussing the basic protections provided by the FDCPA, it is important to have a clear understanding of what debt collectors are. Debt Collectors are hired on commission to collect accounts. They may also purchase debts outright from creditors. All contact between a debt collector and a debtor must include what is known as a “miniMiranda” disclosure (alluding to the rights told to suspects when arrested, known as Miranda rights). Any oral or written communication must include language that the “communication is an attempt to collect a debt and any information obtained will be used for that purpose.”
If the debt collector does not provide the original creditor’s identity, the collection must cease.

After initial contact, the FDCPA continues to protect the debtor from overreaching, harassing, and illegal activities.

Threatening violence Debts are private matters; not only is revelation to third parties a violation of the FDCPA, but communication that identifies the sender/caller as a debt collector or allows the contents to be read (e.g. postcards) is also
Within 5 days of initial contact, a debt collector must provide in writing: (1) the amount of the debt; (2) the name of the creditor; (3) a notice that unless the consumer disputes the validity of the debt within 30 days, the debt will be considered valid; and (4) that the consumer can request verification. All collection activities must stop during the validation process.

Cosmetology Deregulation is a Dangerous Trend
Responding to loss of revenue and increasing regulatory costs, an alarming number of states are seeking to deregulate the beauty industry. Their hope is to see cost-savings on administration of exams, licensing, and inspection of salons and barber shops. The problem, eliminating these requirements will allow anyone to open up their own salon. ANYONE!! For consumers, this means when you seek beauty services, you’re doing so at your own risk. Thirty-seven years in the business I can assure you a level of expertise is required in order to protect consumers. Training and licensing ensure core knowledge and competency in areas such as bacteriology, sanitation, anatomy, and chemistry. Why do you think the industry was regulated in the first place? Have you seen the horrid pictures of finger nail and toe nail infections, scalp burns, bacterial infections how about MERSA? How about herpes transference or chemical scarring? You may think these are fanatical statements, but I assure you, they are not. A quick Google search will confirm your worst fears. As an industry we must pause and reflect. Applying salon beauty treatments is more complex now than ever, how is it that politicians are comfortable discussing deregulation?
No wonder politicians look at regulation as a waste of funds or at a minimum access to easy cash. This could not happen without voluntary participation by industry professionals. Tangentially, this should send a powerful message to all of us that our actions are having a seriously detrimental effect on the profession overall.
Realize that things get better when you get better. Engage in and commit to improving your craft. Seek opportunities to grow personally and professionally. Realize personal integrity is all you have. Following through on commitments, be honest, authentic, and never violate the trust that has been extended to you. These things are a reflection of your character. Master your emotions; do not be enslaved by them. Learn to deal diplomatically with difficult people, rise above the fray, and remain objective in emotional situations.
Catalyst for Change
Make it your mission, as a professional to service as a catalyst for change! Contribute to the re-imaging and preservation of the industry. Resist supporting manufacturers that undermine the profitability of salons through unfair pricing practices or who participate in bipolar marketing strategies and engage in diversion. Inspire new talent to expand their way of thinking and to take conscious steps toward restoring power, pride, and prestige to the industry. Let’s take back our profession, after all no manufacturer, point of sales system or distributor would be in business without us.

Genuine hairdressers and salon owners are passionate, deeply passionate about all things beauty. Some wrongly consider the “rigors” of professionalism a threat to the creative aspirations. The most successful professionals in this industry are both passionate and professional. One of my pet peeves is when I hear professionals describe their work by saying “I’m just a hairdresser”. You are not just anything. Don’t discount yourself or your work by qualifying it with the word “just.” The work you do is valuable and important! Elevate the value of your work and your own self-image by approaching your job with these professional mindsets. You will be more satisfied in your work, perform better, and build higher levels of trust with others

Are You Feeling Too Stressed To Spa? I
love the spa industry. I know it sounds like a pretty obvious statement but, I didn’t purposely go into this business; I sort of ‘fell into it”. As a matter of fact, until I got into the business back in the ‘90s, I knew virtually nothing about what a spa was, By Joanne Graves, or what they even Owner of Sanctuary Spa did. I had a friend who convinced me that I’d be ‘great!’ at selling cosmetics at home parties. As it turns out, she was dead wrong, and I ended up owning more products than everyone I knew at the time could use in their lifetimes. Around that same time, a new esthetics’ school opened in Delaware, and I thought if I got a formal education about the products stockpiled in my living room, people would inevitably line up at my front door to buy their cosmetics and skin care from this newly trained Esthetician who was eager to share her new-found wealth of knowledge! As I’m sure you’ve already guessed, I never did get the big pink car, but what I did get, along with a great education, was a passion for the treatments, and how they made me look and feel. I couldn’t believe there was an entire industry in which I would be able to make a living helping other women and men feel this good. It’s one of the reason’s I love this industry. Most of the people I’ve had the privilege to work with got into the spa business because they love to help people feel and look better about themselves. I can’t imagine working in a more rewarding field. As a spa owner, who loves a spa treatment myself, nothing is more disheartening to me than having a woman say to me, “I’d love to have a massage. I just need to lose 10 pounds”, or, “I’ve always wanted to go to a spa, but I’d be too embarrassed”. As a seasoned spa professional, please let me alleviate those concerns. I know the value of making time for the spa. There are more benefits of having a spa treatment than I can list here. Most of us got into this profession because we love to make others feel good. Sure, I’ve been to
places where the modelesque, negative body-fat, incredibly young therapist can make even the most confident of us go weak in the knees at the thought of them seeing any part of my average looking body. Frankly, many of us in the industry have the same insecurities about our less than ‘perfect’ figures. The spa is, or should be, a ‘no judgment’ zone. Spa professionals are trained specifically to treat each guest with respect and care. We properly drape our clients to preserve your modesty and ensure that you’re comfortable, so that you receive the most benefit from your treatment. One of the things women most look forward to after breast augmentation surgery is getting brand new, stylish bras to show off their enhanced figure. Bra shopping after breast augmentation is an exciting time and we want to help make your experience a success. Wear the Right Bra at the Right Time - While fancy lace undergarments might start calling your name soon after the bandages are removed; wait until you have your doctor’s approval before you start wearing a regular

Get professionally sized and fitted, here, at Bare Essentials. You may have discussed a desired bra size with your surgeon, but that doesn’t mean this is the exact size you’ll actually end up with after surgery. Getting measured will help you buy the right size bra. You may need to be professionally fitted a couple of times in the months following surgery as your implants settle. Don’t Buy All Your Bras at Once - It can be tempting to go out and pick up a whole new lingerie wardrobe at once after breast augmentation, but it is wise to wait. yelash Extensions! They are THE accessory for so many women. No fuss, no smudge, waterproof, natural or dramatic, affordable and safe. Eyelash Extensions have become so popular that they are rivaling the monthly nail treatments at the best salons around. Take a close look at celebrities Kelly Rippa, Kim Kardashian, Drew Barrymore, Jennifer Lopez, Jennifer LoveHewitt, and Gwen Stefani to name only a few. They have been wearing extensions since the early 2000’s. Lucky for us, this “trend” is not just a trend anymore and not just for celebrities. lash extensions, when properly applied, will last as long as your natural lash is attached to you. Some will pop off, but with a good adhesive and technique you’ll be able to enjoy your lashes for a few weeks. To keep your lashes looking fresh, Lasting Looks recommends a refining fill every 2 to 4 weeks.
How long does it take to apply lashes? – Extensions are applied one lash at a time with care and precision. This takes time and depends on the set size that you choose. At Lasting Looks, we like to keep eyelash extensions affordable for all budgets and therefore, offer different two size sets: Demi and Full. Set sizes are not universal and can be independently defined by a salon. Be sure to ask about the number of lashes you are getting when you are booking your appointment.

Studies show that olive oil may play a part in reducing rates or risk of some types of cancer, particularly colon, breast, ovarian, and prostate cancers. A study published in the January 2005 issue of Annals of Oncology has identified oleic acid (the main component of olive oil) as having the ability to reduce the effect of an oncogene (a gene that will turn a host cell into a cancer cell). This particular oncogene
is associated with the rapid growth of breast cancer tumors. The conclusion of the researchers was that oleic acid, when combined with drug therapy, encouraged the self-destruction of aggressive, treatmentresistant cancer cells, thus helping to combat the cancer. Olive oil has been positively indicated in studies on prostate and endometrial cancers as well. One way to take charge of your life is to know your Body Mass Index (BMI); based on weight and height, it is an indicator of how healthy your body is, also, changing eating habits and using EVOO can play a part! As quoted from the book called, The Pink Ribbon Diet, A Revolutionary New Weight Loss Plan a “certainty is the more body fat you have, the higher your cancer risk is.� The book uses a plant based olive oil diet and has 150 easy recipes that use extra virgin olive oil because of the health benefits. Too Count Calories Or Not temporarily cure that sweet tooth, but an hour later feeling lethargic, cranky, and hungry. We should probably be more concerned about counting the chemicals in our food than calories.

e all have counted calories that pesky unit of energy. We know junk food contains high calories and no nutritional value. Whole foods such as whole eggs, grass-fed meats, whole milk, and real butter are also considered high calorie but are very nutrient dense. I will gladly indulge in a 160 calorie sprouted grain English muffin filled with six different grains and legumes. It not only is a complex carbohydrate but a complete protein too. The 100-calorie cookie snack pack may

Nutritional labels are required on all processed foods by the FDA. Interestingly, the calorie count is allowed a 20% window of error, which means the 100-calorie snack pack could actually be more calories. The myth that low fat products are lower in calories and better for you is simply not true. The removed fat is often replaced with sugar. Fatsoluble vitamins (A, E, K, & D) are stored in fat. When the fat is removed from the product so are the vitamins. Often these vitamins are added back to the product synthetically. These chemicals are not assimilated by our bodies, stored as fat, and can lead to disease.
Calorie counters tend to focus more on counting than the actual art of eating, enjoying the food, and listening to the body’s intuition. The stress of calorie counting leads to extra cortisol production, which is the stress hormone. The overproduction of cortisol taxes the adrenal system and is immediately stored as fat defeating the purpose of calorie counting. Eating whole foods eliminates the stress of calorie counting and nourishes the body, mind, and soul. The ingredients are the actual whole foods prepared. There is no need to worry about extra scary chemicals or reading labels. Instead of counting calories make your calories count by eating real food.
