NINE SIGNS It’s Time for In-Home Care If you have an aging parent, friend, or neighbor, you understand the value of stopping by their home from time to time, bringing conversation; a smile; and, on occasion, groceries! Your visits likely help in even more ways than you think. Seemingly small favors, like resetting clocks for daylight saving time, standing on a stepstool to change a FALL 2023–WINTER 2024 SECOND ACT
smoke-detector battery, checking the refrigerator for expired foods, and taking out the trash, help keep people comfortable in their home, where they want to be. Over time, especially if your loved one has signs of Alzheimer’s disease or dementia, their need for small favors will grow. They may need someone to sort medications, refill prescriptions, and drive them to medical appointments. They may need
help around the house more often and more immediately. It’s not just a matter of comfort, but of safety. Eventually, there’s a tipping point. Family and friends simply cannot be there as often as help may be needed, and they may lack the ability or comfort level to help with personal care, medication assistance, and meal prep. They worry about their loved one and
AGING & CAREGIVER RESOURCES | 7 feel guilty that they can’t do more, and the person who needs more help may find it tough to stay in control of their everyday life while relying on favors. If you or the person you love wants to stay safe and independent at home, hiring a trusted home care organization may be a good next step. Here are the nine signs that you or someone you love may be ready for nonmedical, in-home support. • It has become difficult or exhausting to keep up with household chores, preparing meals, and cleaning. • Loss of mobility, vision, hearing, or memory has made it hard to care for one’s self. • Concern regarding safety when using the shower or bathtub.
If these signs sound familiar, help is available. A professional in-home caregiver can help with daily needs, such as: • Personal care: showering, shaving, and dressing • Medication reminders • Exercise support and diet monitoring • Housekeeping, laundry, and meal prep • Grocery shopping and other errands • Companionship With the right kind of help, you or your loved one can continue living at home safely and independently.

How You Can End Up in Medicare’s Donut Hole and How to Get Out of It by Cindy Kiley-Craig
Medicare prescription drug (Part D) plans can have a coverage gap, called a donut hole, which limits how much Medicare will pay for your prescriptions until you reach a specified annual out-of-pocket amount. Although the gap has gotten much smaller since Medicare Part D was introduced, 2006, FALL 2023–WINTER 2024 SECOND ACT
there may still be a difference in what you pay during your initial coverage compared to what you pay in the coverage gap. Part D Deductible and Co-payment When you first sign up for a Medicare prescription drug plan, you pay a
deductible. In 2023, this amount cannot exceed $505. Once you’ve paid the deductible, you may still be required to pay a co-insurance amount, also called co-payment. Medicare will pay the rest. Co-insurance is usually a percentage of the drug cost. If you pay co-insurance, the amount may vary throughout

the year due to changes in a drug’s total cost. Entering the Donut Hole Once you and your plan have paid the specified annual amount—$4,660 in 2023—you enter the coverage gap, or donut hole. Previously, coverage stopped completely at this point until total out-of-pocket spending reached a certain amount, but the Affordable Care Act mostly did away with it. In 2023, until your total out-of-pocket spending reaches $7,400, you’ll pay 25 percent for brand-name and generic drugs. Once your total covered drug spending exceeds $7,400—the catastrophic coverage threshold for 2023—you’re out of the coverage gap and will pay only a small co-
insurance amount. Learn more about co-insurance drug payments on the medicare.gov website. Catastrophic Coverage Once you are in the coverage gap, your yearly deductible and co-insurance payments count toward the amount you need to pay to reach catastrophic coverage. The amount of out-ofpocket costs that you have to pay to reach catastrophic coverage will vary depending on the type of drugs you take. In the case of brand-name drugs, you’ll pay only a certain percentage of the price; however, the entire price will count toward the amount you need to qualify for catastrophic coverage.
Bear in mind that only payments for drugs that are covered by your plan count toward the out-of-pocket threshold. Your premium and the portion of the drug cost that Medicare pays does not count toward reaching catastrophic coverage. Also, any help with paying for Medicare Part D costs that you receive from an employer health plan or other insurance does not count toward this limit. Connect With the Experts The rules around Medicare can be very complicated. If you’re looking for guidance, consider consulting with a local Medicare expert in your area. You also may seek answers to your questions by calling 1 (800) MEDICARE (1 (800)-633-4227), where you can access free support 24 hours a day, 7 days a week, except for certain federal holidays. Also at your disposal is your local State Health Insurance Assistance Program (SHIP). This, too, is a free service and offers unbiased advice from trained professionals.

AGING BETTER
In a city routinely rated one of the top places to retire, individuals and organizations focused on supporting healthy and active aging deserve celebration. On May 18, 2023, AgeBetter, Inc.’s first Aging Advocacy Award—presented to the community member who advocates for older adults and devotes significant volunteer and professional time to promoting healthy aging and preventing ageism—was presented to Dr. Alexis Eastman, clinical geriatrician and faculty member in the Division of Geriatrics and Gerontology at the Department of Medicine, University of Wisconsin—Madison. “We recognized Dr. Eastman’s impact in the Madison area community to dispel ageism, provide education to older adults, and promote the profession of geriatrics,” says Ann Albert, executive FALL 2023–WINTER 2024 SECOND ACT
director of AgeBetter, Inc. “Alexis is a super person who has also been involved with our SAIL (Sharing Active Independent Lives) program for over five years, enthusiastically providing AgeBetter and our SAILing into the future! attendees with educational workshops, clinical expertise, and related recommendations—especially as we navigated the pandemic.” When asked about her involvement with SAIL and her contributions to the lives of older adults throughout the community, Dr. Eastman shared, “I don’t view it as volunteer work; I view it as a necessary part of community care—it’s a part of my job to make sure that the information I have reaches the most people possible.” As a medical educator, Dr. Eastman provides classroom and clinical
instruction to residents and fellows. Her educational efforts include teaching EMS providers and other senior health professionals about DNR/DNI status, dementia diagnosis and evaluation, and dementia care. As a clinical geriatrician, Dr. Eastman provides medical care for older adults in inpatient and outpatient settings, both in primary and memory care. She serves as director of clinical operations for primary and specialty care at the Division of Geriatrics and Gerontology and as medical director at Badger Prairie Health Center in Verona. Aging well is always on the forefront of Dr. Eastman’s mind, and she has spent a lot of time examining the things seniors can do to keep up their physical and psychological well-being. First is exercise, “not only for physical health and to aid in falls prevention and other
GENERAL | 27 “I don’t view it as volunteer work; I view it as a necessary part of community care—it’s a part of my job to make sure that the information I have reaches the most people possible.” ailments, but for mental health and to reduce the risk of dementia by up to 50 percent,” says Dr. Eastman. “As we get older, things get stiffer and saggier, but shouldn’t start hurting. “In terms of memory, it’s normal to take longer to process information or learn something new, but you should still be able to do it and not forget important things. When in doubt, get checked out. It never hurts to ask questions about changes. Safely do aerobic and strength-training exercise for about a half hour most days of the week, keep your medical health under control, don’t smoke, invest in supportive shoes, give the Mediterranean diet a try, get good rest, and find a doctor that is best for you. “Anyone over 65 can see a geriatrician, but the people who really need us are those with complex health and social situations. Socialization is very beneficial. People think they’re fine alone or they aren’t lonely, but Covid was incredibly isolating; we saw a lot dementia and depression get worse, and some died that didn’t need to. A silver lining was video visits with providers. Get together, play cards with friends, make that call, and if you’re considering a move to a retirement or assisted-living community, eat lunch there first; it’s the most common meal, and you can learn a lot about the environment and people.” Dr. Eastman also sees a lot of benefits for herself in working in the field of geriatrics. “I love helping people,” she says. “First, the sheer nerdiness of it. Aging physiology is a very unique biochemical process, and it takes you outside the textbook. You have to think
creatively, and I find that intriguing. Bigger yet, I’ve always preferred older people—even when I was little, I’d talk to the older people on the city bus. During clinical rotations, I was paired with a particularly cool person in the Senior Partners Program and thought it would be fascinating to work with aging patients. I fall in love with these incredible individuals, their stories, and their wonderfully rich lives. To help be a part of that and use my knowledge and apply it to make their lives better inspires me.” Aging is something to be celebrated. It’s possibly the most important part of being human. As we grow, we learn, and we age. Thanks to Dr. Eastman and similar geriatricians, older adults can age in the best mental and physical health their lives have afforded them.

Movement MATTERS by Sarah M. Nelson, DC The first law of Newtonian physics states that an object in motion stays in motion, and an old Latin proverb says moss doesn’t grow on a rolling stone. And over the years, several of my wise patients proclaimed, “If ya don’t move it, ya lose it,” and, “Motion is lotion!” I declare from atop my office soapbox with fist in air, “Stasis equals death!” Just kidding, that would be weird…even if it was true. It’s simple enough, though. I find that if a patient understands the why, it helps with compliance. So just what makes active people stay active? Biomechanically speaking, joints that have full range of motion circulate synovial fluid into all the little nooks and crannies of the cartilage on the joint surfaces. Nicely lubricated joint surfaces have less friction, thereby reducing wear and tear and helping to keep the dreaded Arthur (arthritis) away. Osteoarthritis (OA) is the result of thinning, dried-out cartilage, and is FALL 2023–WINTER 2024 SECOND ACT
progressive in nature. Eventually, it can result in bone spurs and joint degeneration. OA is also a positive feedback loop; the stiffer your joints become, the less areas are lubricated and the more cartilage dries out…and on and on. Once Arthur comes to visit, he doesn’t leave, so keep that pesky guest contained or, better yet, avoid it altogether by keeping your joints moving through their full ranges. Action Items - see your chiropractor regularly, not just when you can’t walk or have a migraine - use an inversion table several times a week Joints aren’t the only body parts that need to move. Supple muscles and nonfibrotic fascia are crucial in flexibility, pain management, injury prevention, and venous and lymphatic drainage. Next to yoga, the best way I’ve found to address softtissue elasticity is regular massage therapy. Too often we think of massage
therapy as a luxury or treat, but the therapeutic benefits are innumerable. If it’s not in your budget, get books to learn simple techniques at home. Even a bad massage is better than no massage, except perhaps for those who think they’ll get an award for clenching their teeth in silence through a deep-tissue session. Always communicate your body’s needs. Action Items - preschedule regular massages with a pro or partner - practice yoga often…it’s not just for hippies! - make stretching a routine: coffee + stretches in morning, teeth brushing + stretches at bedtime Other important facets of movement include the ebb and flow of electricity, chi, energy, life force, and prana in your body. We’re bioelectrical beings; every neuron that fires, myofibril that slides, gland that secretes, and receptor that responds relies on the flow of ions. While seemingly etheric because we
HEALTH | 37 don’t always feel it, it boils down to boring old science. The Body Electric, by Robert O. Becker, MD, and Gary Selden, is a book I recommend for more understanding. We’re inundated daily with microwaves, radio waves, x-rays, gamma waves, and UV rays, which are imperceptible, but create a response in our electrical bodies. The earth emanates a frequency called the Schumann resonances, which help balance our body’s electricity when waves get us out of sync. Using rubber-soled shoes, being indoors, and literally just not being in contact with the ground insulates us from this harmonizing frequency. Think about lightning storms and static. Better yet, walk barefoot in the grass or garden and you’ll see that the practice is legitimate. Bonus: the earthing concept is easy and free! Sound also has a healing resonance that can affect the ions in our bodies. It can break glass and levitate mass. The effect on our biology is sound— ha ha, get it? Thousands of years ago, traditional Chinese practitioners recognized the need for the movement of energy for balance, and today, acupuncture remains one of the oldest healing arts in the world. Action Items - regular acupuncture treatments - visit a Reiki healer - use a pulsed electromagnetic field (PEMF) therapy (see drpawluk.com for educational videos) - practice breathwork on your own and take classes - use music and sound as medicine (sound baths, music therapy, etc.) - practice grounding (walk barefoot on grass or use a grounding mat) - block Wi-Fi routers, avoid Bluetooth devices in ears, don’t sleep next to electrical appliances, and don’t move next to a 5G tower (see emfacademy.com)
Lastly, mechanoreception and proprioception improve kinesthetic awareness and modulate pain perception. Want that in English? Keep firing the nerve fibers that go from your body to your brain with information of stimulus and movement in space and time because it will improve your balance and body awareness and reduce pain. The movement nerve fibers are faster and more robust, so they’re able to override the pain fibers. A great example is when you stub your toe and there’s that millisecond delay of, “Oh, that’s gonna…OUCH!” You feel the stub (the more efficient pathway) before the pain, and then you squeeze and rub your toe (stimulating the movement nerves) to reduce the pain. This simple concept also explains why things feel better when you walk them and why chronic pains are worse in the morning or when you’ve been bedridden. Improving the pathway of nerves responsible for awareness of surroundings equates to better balance and reduced risk of falls and injury. I hope you incorporate some of these to improve your quality of life now and in the future. Now, put this magazine down and get moving!

A fall is a threat to the safety and independence of older adults; after an initial fall, chances double that there will be another. Each year, falls of people 65 or older result in more than 3 million injuries treated in emergency departments and are linked to over 800,000 hospitalizations. Fall prevention is an important aspect of promoting health and well-being, and there are several strategies and interventions that can help reduce the risk. FALL 2023–WINTER 2024 SECOND ACT
Regular Exercise Engaging in regular physical activity and exercise can improve strength, balance, and flexibility, reducing the risk of falls. If you avoid physical activity because you’re afraid of falling, tell your healthcare provider. Physical therapists are movement specialists who can create an individualized program for you consisting of customized exercises aimed at improving your balance, flexibility, and muscle strength.
Wear Sensible Shoes Avoid high heels, flip flops, and shoes with slick soles. It’s best to wear properly fitting, sturdy, flat shoes with nonskid soles. Know Your Medications Certain medications can increase the risk of falls due to side effects, such as dizziness or drowsiness. Review all medications with a healthcare professional to identify any potential risks.
SAFE & HEALTHY AT HOME | 55 Regular Vison and Hearing Checks Poor vision and hearing can contribute to falls, as issues with these sensory systems can affect balance and coordination. Home Safety Assessment Remove clutter, ensure good lighting, secure or remove rugs, install handrails and grab bars in bathrooms and staircases, and make sure pathways are clear. Use nonslip mats in your bathtub or shower and consider using a shower chair for extra safety while bathing. Store clothing, dishes, food, and other necessities within easy reach and avoid placing them on high or low shelves. Adequate Nutrition Maintaining a healthy diet and adequate hydration can help prevent falls by ensuring overall health and strength. Fall Detection Systems Consider the use of a fall detection system that can automatically alert caregivers or emergency services if a fall occurs. Report falls to your healthcare provider. Regular Health Checkups Regular checkups with healthcare professionals can help identify and
address underlying health conditions or risk factors for falls.

Speight, CR Savvy homeowners considering a renovation know the value of good design, thoughtful planning, and accurate pricing. If you’re thinking about remodeling, there are dozens of variables to be considered, selected, and determined. The habitual approach of seeking free estimates and competing bids is fraught with unforeseen consequences and openended questions. What exactly is the best plan? What fixtures and features are included? What challenges and opportunities will be discovered? What impact will these have on the final cost? torn apart and put back together? There’s a better way. You can sort out all of these details prior to the project by working with a professional design-build remodeling company. To understand the long-term value of the design-build approach, I like to compare buying a car to remodeling your home. If you want a car, you probably have a budget since it’s a major purchase. Either you have identified a particular car and budget accordingly, or you have a set budget to start looking for a car you can afford. Remodeling is similar. If you want to remodel a bathroom, you can have a detailed plan created and then determine the cost to build it,
or you can work to design and specify a renovation plan that is compatible with your budget. If you have $15,000, you’ll be buying a used car or planning a partial bathroom renovation. If you have $50,000, you have several options for new vehicles or a better bathroom renovation keeping in mind that certain high-end car models or bathroom details/finishes will cost even more. Unlike vehicles, your desired home renovations are not built in a factory. The options for altering and improving your house are almost infinite. Professional advice and guidance will help you steer clear of bad design, poor functionality, and faulty products.

Then they learn that their bathroom renovation may cost $60,000 to $75,000 to build. However, since architects and designers are not contractors, they often do not provide accurate estimates of the true costs. A design-build company helps you avoid this pitfall, guiding your project toward a target budget. If the first concept is too expensive, then a design-build team can suggest alterations to the design and your selections, delivering a plan which fits your needs, wants, and budget. Some homeowners make the mistake of paying for design and then getting competing bids to select a contractor. One contractor notices that the electrical system needs to be updated, but the others don’t notice this. The bids were free, so the amount of information you get varies significantly. These types of quotes are hard to compare. You hire the lower-bid contractor only to learn that the building code will require the electrical updates that were anticipated by the higher-bid contractor. Did you save money? Multiply this oversight by every detail involved in improving your home, and you start to realize the value of paying a trusted professional to accurately price your renovation from design through construction. You could hire a design professional to create a plan and then hire a professional contractor to provide accurate pricing, but what if a problem arises during the construction? Who’s to blame? A design-build firm is accountable for the entire design-build process and project outcome. This avoids those nightmare scenarios where the designer blames the contractor (or vice versa) for oversights or cost overruns. The design-build company should provide a construction contract with a clear scope of work at a fixed price. You, the homeowner, get
