Threats, Temptations and Triumphs There are countermeasures to every threat that endangers your practice. We hear of businesses every week, from Facebook, to hospitals, to dental practices, impacted by a Cyber Security breach. Achieving Accuracy and Accountabilit y

Dental schools do a good job teaching the clinical fundamentals to prepare us for our professional careers. Unfortunately, dental schools don’t teach us anything about business! We come out of dental school ready to take on the clinical world, but then reality hits that there is also a financial side to dentistry. For dentists in private practice, the reality is that we are clinicians and small business owners.

While dentists aren’t trained in business, we definitely have an economic interest in maintaining accurate financial records.

Without accurate systems in place, we are going to have a difficult time monitoring practice performance.

While clinical care always comes first, we expect to be financially rewarded for our efforts. Ensuring accuracy in financial reporting provides excellent practice management feedback and allows us to make modifications in our business processes to maintain efficiency. Failure to spot unfavorable trends can lead directly to declining performance.

Additionally, accurate financial records help detect innocent mistakes that can negatively impact our practice management reports. We all make mistakes, and the dental team is no exception. Innocent mistakes by team members can make it difficult to detect negative trends that might affect practice performance. We have all heard the adage “garbage in equals garbage out.” If incorrect data is entered or omitted in our practice management or accounting software, the generated reports are going to be essentially useless.

The only way to ensure dental practices perform at a high level is to ensure that processes are in place to minimize mistakes.

Less common, although an unfortunate reality, is that embezzlement can be involved. Obviously, embezzlers are not making innocent mistakes. In these cases, the data input is intentionally inaccurate in an effort to hide embezzlement. Any report subsequently generated is not only going to be inaccurate, but also intentionally misleading.

Unless the proper business systems are in place to ensure accuracy and accountability, how are we going to detect errors whether innocent or intentional? It is essential that we can rely on our software reports so that we can monitor performance, spot negative trends and ultimately increase practice productivity. While our software programs can generate dozens of reports that can be mind-boggling, there are a few essential reports that will help ensure that our financial records are accurate, especially related to patient payments.

First and foremost, the only way to determine if software reports are accurate, is to compare them to some type of external financial statement. For example, the practice management software can

Feature Article | create a collections report. The way to determine if the report is accurate is by comparing the collections report to a bank or credit card statement. If there is a significant discrepancy (whether innocent or intentional), something is wrong. Without performing a reconciliation, there will be difficulty detecting errors.

I am going to review what I feel are essential reports that should be created by your team and reviewed by you or someone other than a team member such as your spouse, accountant or other advisor. I am by no means suggesting these are the only reports you need.

However, the following reports are highly recommended.

On a daily basis, there should be a report that lists production, collections, adjustments and deleted entries. In some software, these may be listed on one report while others may require multiple reports to gather this information. Most software will generate a “Day Sheet” report which at a minimum provides production and collections for the day. Simply generate what is needed to retrieve adjustments and deleted entries as well.

When reviewing collections, payment types should be reconciled separately. Most reports will sort collections into cash, patient checks, insurance payments, credit cards and third-party payers such as Care Credit. I can’t stress enough the importance of reconciling payment types separately as opposed to simply looking at the total deposits (I can’t go into details in this article but looking only at total deposits can allow some types of embezzlement to hidden).

For example, if collections include cash and paper checks, a deposit slip will be filled out by a team member. Once the deposit is made, the bank will provide a receipt confirming the deposit. This receipt should be returned to the office and attached to the day sheet or collections report. If the amounts do not match, something is wrong. Most likely it is an innocent mistake, but keep in mind that undetected innocent mistakes can get expensive over time.

I should also note that I like the idea of scanning checks from your office to reduce the number of trips to the bank. When checks are scanned, a report is generated which can then be compared to the software report.

For credit card payments, make sure a report is generated from the merchant terminal (or the vendor’s web site) which shows all the credit card payments for the day. Compare the merchant terminal report to the Day Sheet or other collections report and confirm they match. The same process is performed for any thirdparty lender such as Care Credit if applicable.

It is also important to monitor adjusted and deleted entries. I have found many doctors have no idea how much of their production is being adjusted.

Occasionally question your team members in a nonsuspicious way regarding some specific adjustments or deletions. This sends a message that you are checking the reports and should improve accuracy when entering transactions.

At the end of the month, the process is repeated. All collection types should be totaled for the month and compared to the corresponding financial statements.

Some doctors will only perform this reconciliation on a monthly basis, but I recommend both daily and monthly reconciling. Daily reconciling detects errors more quickly and sends constant “accountability” messages to your team members. Likewise, adjustments and deletions should be monitored monthly to detect troublesome trends before they become harder to manage several months later.

| Feature Article Lastly, an Accounts Receivable report should be generated every month. It is vital to understand how much is owed to the practice. An increasing accounts receivable balance is a warning sign that team members may not be collecting payments in a timely manner or that statements are not being sent on a regular basis. Either scenario results in decreased cash flow. Additionally, the older a patient’s balance becomes, the harder it is to collect. Once a balance goes beyond 90 days, it becomes extremely difficult to collect a payment. Uncollectible accounts that are written off has the same effect as any other adjustment—the doctor does not get paid!

Another important item to monitor in the accounts receivable report is the number of patients with credit balances. A credit balance results when a patient or third-party payer makes an overpayment.

Most states now require patients with credit balances be notified of the credit balance. If the patient chooses to apply the credit balance to future treatment, that is fine. The important point is that patients need to know if they have credit balance. Should a patient discover a credit balance that was not brought to their attention, and they file a complaint with the state (this is rare but does occur), a difficult situation can result. Some states are now aggressively issuing fines to businesses who improperly manage credit balances! It is vital to review accounts receivable on a monthly basis to minimize the financial damage that could occur either in fines or having to issue patient refunds.

Another recommendation I strongly encourage is to eliminate insurance payments issued via a credit card number. Numerous third-party payers are trying to reduce paper checks, so they encourage electronic deposits. If a doctor chooses not to accept electronic deposits, the third-party payers will often provide a credit card number with the explanation of benefits statement. The team member then enters the number into the merchant service terminal. While this may seem convenient, there are two problems: One is the temptation of diverting the payment. Again, most team members are honest, but having a credit card number that most doctors never see, can be a tempting target. Second, when the payment is entered in the merchant service terminal, it is treated as a credit card payment. The result is you are charged a merchant service fee just as if a patient had paid by credit card! Also, there is a good chance you have accepted a fee reduction from the third-party payer.

on top of the fee reduction? You have the right to contact the third-party payers and insist they stop paying claims in this manner.

I admit this is a cursory overview of some basic business processes. The overall point I am trying to make is that there are basic reports than can be monitored and reconciled to financial statements. It doesn’t take long to complete this process, and the returns can be significant. It is hard to reclaim collections that are lost due to excessive adjustments, deletions, mistakes or intentional errors. Additionally, verifying reports sends a strong message to your team that accountability and accuracy is important for everyone.

After all, the team members benefit when the doctor benefits!

The rush for HIPAA compliance in the 2010s set the Dental Industry up for failure in IT security moving forward. The “Great HIPAAning” did one of two things to most industry members: It either fatigued them to any discussion of related topics such as IT security, or it caused industry members to go through processes which only provided a false sense of security.

Admit it: You’re sick and tired of the word ‘HIPAA’.

No judgment, so am I. Let’s table the word ‘HIPAA’ and just discuss the subject of IT Security. No laws, no regulations, just what’s best for your business – your livelihood.

The year 2021 has been rife with major security incidents: [another] Facebook Data Leak, the Colonial Pipeline Ransomware Attack, the Kaseya IT Management platform breach, the Parler social media scraping attack, etc. etc. It is easy to think that these incidents only happen to major companies, but these are just the ones we hear about.

Your dental practice is the ideal target for bad actors and hackers. The combination of valuable information in your systems (SSN, DOB, medical history and records, prescription information, family names, good contact information – basically dossiers on each of your patients), and the relative ‘soft’ underbelly of a small business is enough to make the bad guys start salivating.

“But we have insurance.”

Let’s get this notion out of the way right now.

Traditional insurance won’t touch cyber incidents, costs associated with data, or breach notification costs. If you purchase Cyber Security Insurance, you are still responsible for doing what is “reasonably required” to protect your data. Claim denials do happen because of failures to maintain minimum adequate security standards, and if you decide to just ignore known security threats, your insurance company can easily deny a claim.

Material costs of an IT incident rack up quickly: Operational downtime, notification costs, possible ransoms, and the cost to return to normal business in general can take a profitable quarter or year and turn it on its head – even with insurance.

In addition to monetary costs, there’s the issue of your reputation. You’ve worked hard for your 5-star ratings and stellar reviews; don’t put yourself at risk of tarnishing a reputation by being complacent about IT security or continuity.

You don’t need a degree in IT Operations to understand and make sense of the threat landscape present in the world today. However, you and your staff must first acknowledge that you have a role in the IT security of your practice. Each time you touch a computer, pick up the phone, or leave a patient alone with a computer, you create exposure. If you and your staff understand that you have a role in IT security, training is crucial.

Feature Article | User training is the single most important thing you can do because, contrary to what TV and the movies would have us believe, ‘hackers’ don’t usually brute-force their way through a router or firewall. That’s too much work, or requires too much expertise. Instead, a moderately resourceful bad actor prefers to attack the least secured resource in your practice: the humans. Social Engineering is the concept of deceiving a person into divulging information, granting access, installing a piece

Social Engineering is the number one attack vector against small businesses.

Even if you engage with a company to provide IT services, there is a limit to how much protection they can put in place to protect you and your staff from Social Engineering. To prevent users from being able to fall victim to these attacks completely, your IT department would need to lock down your computer systems to the point of being uncomfortable. (As an IT person, I would suggest this though, as your first duty is to staff being able to do their job, and to being profitable, not to making your staff ‘comfortable’ through access to social media and the like. (See: “This is a business”, below.)

So, what do these threats look like, and what can be done to mitigate them?

Phishing: Phishing is the act of sending emails that are urgent in appearance, seem legitimate based on appearance, are just-specific-enough, and [most importantly] have a call-to-action. This call-to-action is typically a link of some sort – likely one that would require a sign-in by the user. The ‘link’ however is to a webpage hosted by the bad actor. If a staff member receives one of these and clicks the link, they’ll see a login page, and may enter good credentials – that go right to the bad guy. The bad guy now has a username and password (presumably for the email account), that they can use to compromise and view the contents of that mailbox, and send messages from… to your patients, whose emails and information is likely in the sent items and inbox of that account!

Phishing Training: Train users to trust nothing, avoid clicking links (especially in emails), and only interact with known parties on known subjects (these attacks can come from known contacts whose accounts have been compromised.)

Mitigation/Prevention: Strong security on your email (not just local antivirus)- this is usually part of any professional business-grade email. This topic deserves a whole article, but you can take two crucial, immediate steps: First, stop using free email services to run your business. They’re not professional in appearance, they’re insecure, and using free email accounts for business purposes is against the terms of service of most free email service providers. Second, turn two factor authentication ‘on’ on any service which will let you – not just email.

Whaling: Whaling is basically phishing on steroids.

Instead of just blasting out generic calls-to-action, whaling goes after specific members of your organization by name and job role. For instance, if the bad actor finds out the name of your accountant or someone with finance access (through sites like LinkedIn), they may focus their attacks on that person.

Tech Support Scams: We’ve all had it happen. You’re browsing the internet, click one bad link, and suddenly your screen is filled with pop ups. These days, these pop ups are typically a very forceful call-to-action: “There’s a problem with your system and you need to call Microsoft at this number right now.” (How nice of them to provide the number for you, right?)

You know where this one is going: a staff member calls the number, and ‘Microsoft’ (in reality, a call center operated by bad actors) picks up. The bad actor walks the staff member through getting the bad actor remotely connected to the machine, and the rest is … a breach.

Many offices lack this crucial segmentation between a ‘Private’ and ‘Guest’ Wi-Fi network, and Wi-Fi keys are rarely if ever changed on most environments.

Mitigation/Prevention: Be vigilant about who and what you allow to connect to your network. Always inform the person managing your IT and security if anything in your environment is changing or being added by 3rd parties and provide first right of refusal for the person responsible for your IT and security to engineer the solution.

Training: The only person a staff member should ever call for a computer problem is your designated IT support person, and there should be very strict methods by which they can gain remote access.

Mitigation/Prevention: A good IT vendor will monitor systems for unknown or third-party remote access tools, but by the time they get an alert, it may be too late.

These bad actors use industry standard remote tools!

The best solution is to prevent your users from being able to execute this action on their own. (See: “This is a business”, below.)

LAN Access: This is a generalized category of threat that has more to do with the way your network and computers are set up. Unknown parties having physical or Wi-Fi access to your sensitive network segment is dangerous. With physical access to the network or computer hardware, bad actors can at best disrupt network functionality or at worst gain access to your data. This threat is sometimes innocent as well – a 3rd party comes in to install a piece of hardware and unintentionally causes issues with your systems or provides unintended public access via Wi-Fi. Wi-Fi presents a silent but significant threat as well. Staff often connect personal devices to the office’s private Wi-Fi. For purposes of Wi-Fi security, all personal devices

Be diligent in understanding what Wi-Fi networks are present in your physical space, what network segments they are making available, who has access to the security keys, and who those keys are given out to. Regularly change the wireless security keys (annually is a good idea). Exclude staff members from the ‘private Wi-Fi’ – their phones will work just fine on the ‘guest’.

This is an incomplete list of threats. A complete list could fill a book, and it would be out of date quickly.

These are just a few of the things that are easily solved with some training and action. A regular (genuine) risk assessment is a good way to figure out what your organization can solve next.

This is a business: You can do a lot of good simply by treating yourself and your IT systems like a business.

We have a saying at our firm: “What would Coca-Cola Do?” It’s a reminder for us to always be delivering enterprise level IT. Big businesses may have a lot

A few examples include: Don’t grant too many rights to users in Practice Management and IT Systems, and audit those rights regularly.

Actively restrict user access to portions of the internet that are not necessary for your business such as personal email and social media. Don’t make your workstation special by allowing access on it either.

(These should not be seen as punitive, but as preemptive ‘safety nets’ for your business against innocent mistakes.)

Speak to your software vendor about this issue.
