Profits of a Joyful Practice by Dr. Dave Lee

If you imagined your dental practice in its 50th year of existence, what would it be like? Maybe a better question is, would you want to be in the practice of dentistry for 50 years? In a world where corporate health care seems to be controlling the direction of medical and dental care, how long would you like to be involved? If you’re a dentist, or involved in dental care, the future may be brighter than you think.

Dentistry has unique aspects that give us a natural separation from most businesses in the world. Number one, it is an extremely personal service. When my female patients compare being at my office with being at the gynecologist’s office, I know they are telling me that dentistry is an extremely “personal” profession. And women are not alone in the “personal/fearful” category. Some of my most satisfying patient transformations, from fearful to comfortable, have been men! Because we are such a personal profession, it means our most successful businesses are built on trust and relationships. We are in the “people” business! Because we are in the “people” business, it means we are all unique, and one size never fits us all. There have been tremendously successful dental offices based on the personalities of a multitude of dentists. From an office with one dentist and one assistant, to offices with dozens of dentists and employees, successful dental

practices exist in all forms and sizes. It is a wonderful gift that we have the opportunity to create a dental practice that best fits our own personality, and also allows us to attract and serve patients that we most enjoy! For these, and several related reasons, we are envied by many other health care providers.

“Because we are in the ‘people’ business, it means we are all unique, and one size never fits us all. ” My recent season of Thanksgiving and Celebration of Faith was preceded by two wonderful programs sponsored by the Hinman Dental Society here in Atlanta. One program featured Dr. Ronald Goldstein, one of the great founders and innovators of Cosmetic Dentistry. The second program featured Dr. Gordon Christensen, founder of Clinical Research Associates, and dentistry based on clinical and research-proven success. There were a few remarkable similarities in their presentations. First, in 1975, both men were present with a small group of health care providers dedicated to providing the best in comprehensive Esthetic Care for their patients. Both men had been in practice for about 15 years at the time. Secondly, after more than 50 years of practicing, both of them are still practicing dentistry at an elevated level of service, success, and profitability. Thirdly, both men are still excited about the future of dentistry.

What keeps these men motivated for all these years? One of the easiest measures of success is money, and I would say that both men have enjoyed financial success. But we’ve all known financially successful individuals whose personal lives are sad disasters. So what are some of the things that make Dr. Goldstein and Dr. Christensen different? I think both of them apply the simple and profound principals of another wonderful master of dentistry, Dr. L.D. Pankey. Thankfully, I had the blessing of meeting and learning from Dr. Pankey in 1985. I had been practicing dentistry for two years at the time, and my plans were to practice “successfully” for 25-30 years, build my retirement, and move on to greener pastures. But as Dr. Pankey presented his “Cross of Life”, and his “Cross of Dentistry” with his passion for excellence in dentistry and life, a transformation began in my motivation for practicing dentistry.

To complete the cross, a balanced life requires challenging work that is, for the most part enjoyable. All work can feel unexciting from time to time, but the overall feeling should be one of enjoyment and appreciation of its value. Autonomy, complexity, and a connection between effort and reward are, most people agree, the three qualities that work has to have if it is to be satisfying. Work. In fact, in the average lifetime, only sleep occupies more hours of our life than work. So if we’re going to spend a significant percentage of our life at work, how do we create an environment that provides autonomy, complexity and a sufficient connection between effort and reward? To help us find these essential elements in dentistry, Dr. Pankey presented his “Cross Of Dentistry” seventy years ago, and I think you will find his cross at the center of most successful dental practice consulting programs today. It is filled with the principles, ethics, relationships, and challenges that make a dental practice a rewarding source of happiness in our lives.

Dr. Pankey placed Know Your Work at the foundation

of the cross. Excellence in your dentistry sets you apart from the average dentist, and Dr. Pankey was fond of reminding us that there is little competition at the top. Excellent dentistry increases our efficiency, and reduces the tremendous emotional and financial cost of redoing unsuccessful dentistry. Today, more than ever, the excellent and ethical dentist has tremendous opportunity for success.

Dr. Pankey’s philosophy is based on the thought that an

ultimate desire for all mankind is the desire for happiness. With Happiness at the fulcrum of the “cross of life”, he generalized four basic needs in our lives that must exist, and be balanced, for happiness and fulfillment to have a long-term existence in our lives. The cross has Worship, or spiritual life as a foundation. It is a quality that separates humans from the rest of the world, and encourages us to believe in a cause, and a purpose, that is greater than our self. Love resides at the top of the cross. Love is the most powerful creative emotion in our lives. It builds the relationships we establish with our family, friends, and patients, and it guides our decisions to help others based on their emotional, spiritual, and physical needs. Dr. Pankey placed Play directly across from Work. We desperately need time for hobbies, creativity, and physical fun to keep our minds and bodies fresh and healthy for today’s sensory

Yet we’ve all known excellent clinicians who haven’t quite found the rewards to match their skills, so “The Cross of Dentistry” also requires the balance of Know Yourself. We must be willing to honestly evaluate ourselves and create a support team that complements and supplements our strengths and weaknesses. Family, friends, staff, and professionals are available, when we are receptive to guidance, to help us be ethical and competent as professionals.

“Excellence in your denistry sets you apart from the average dentist.” As the leaders of an ethical and competent team we have the opportunity to open the door to a critical ingredient for success of every dental practice; patients who trust us, who desire better dental health, and who are willing to pay us a fair fee for our services. I’ve heard it said that we love to “buy”, but we hate to be “sold”. And we most desire to buy from people who have “heard” our needs. Dr. Pankey labeled this concept Know Your Patient, and the best way to know our patients is to provide the opportunity and skills to listen to them. In our hurry up world it is beneficial to be efficient with people’s time, but listening to our patients gives us great opportunity to once again separate an exceptionally rewarding practice from an average practice.

The last balance for rewarding work is the challenge to Apply Your Knowledge. Here is where we gain the efficiency of our dental career. When we can maximize our personal skills, and the skills of others to know and understand our patients, we have the opportunity to provide our services at an elevated level with a commensurate return of material and spiritual reward.

When I started this discussion I gave Dr. Goldstein and Dr. Christensen as examples of dentists who have practiced over 50 years, and still resonate with the excitement and enthusiasm of dentistry. They’ve incorporated CAD-CAM, cone beam, implants, porcelain, and bonding into their practices. They’ve progressed from the previous “golden age of dentistry” into the current “golden age of dentistry”, and now they are leading the way into the next “golden age”. Twenty-five years ago I thought I wanted to be retired by now, but the timeless principles of Dr. L. D. Pankey created a new vision for me, and now I’m hoping that I’m only about half way through my dental career. I’m thankful for many dentists who have lived the example of a balanced, ethical, and rewarding life. I’ve watched them be blessedly rewarded with family, friends, and financial success that they have graciously shared with others. For dentists who provide exceptional service based on trust and integrity the “golden age” is just beginning, and the “profits of a joyful practice” are just around the corner.

Dr. Dave Lee has been in the practice of general dentistry, with an emphasis on esthetic dentistry, in Fayetteville, Georgia for 28 years. He celebrated his 30th year of marriage to his wife Cheryl, and they have three children; Dr. Lauren Lee -MCG School of Dentistry 2011, Jennifer – PhD in psychology program at UGA, and Taylor – double major in chemistry and food science and a junior at UGA. Dr. Lee is a member of the American Dental Association, Georgia Dental Association, American Academy of Cosmetic Dentistry, Pierre Fauchard Academy, and the International College of Dentists. He is past president of the South Metro Dental Study Club, past president of Fayetteville Rotary Club, a speaker for Gideons International, Sunday school teacher and youth leader for 25 years. He is currently serving as President of the Hinman Dental Society, a member of the Board of Directors for Piedmont Fayette Hospital, and clinical chair for 2011 Georgia Mission of Mercy. When he’s not involved in dentistry and his community, Dr. Lee spends time restoring old cars and furniture, and paddling a canoe at their lake, while his wife Cheryl does the fishing. Practice Growth Opportunities Creating a Professional Office through Key Business Strategies & Resources

Lowery Atlanta Design Associates Whether you are considering a small improvement to your current facility or designing and building a new office, there has never been a better time to invest in your practice. The right facility improvements will result in practice growth as well as in your comfort and image as your career moves forward. There are key resources and business strategies to consider when planning a facility improvement project. Below are several resources you may want to consider in assisting with a variety of needs that may exist in your project: Traditional Design Build Contractors: This can make it difficult to

hold a contractor accountable in reducing the change orders that are typical on most projects. These contractors usually provide a less detailed design process that can compromise the needs of the project. A strong project management presence is needed if using this type of contractor. Strategic Design Build Contractors: Provides some design services

which are usually coordinated with the services of others that are involved in the project. This approach can be helpful as long as clear coordination is provided to all the members of the design team through “Cost Based Design”. Traditional Architectural Firms: May compromise the long-term

investment of facility improvements if they have limited experience with dental offices. The opportunity to improve the flow of the office can be missed if a traditional design approach is taken instead of taking a method that is focused on the needs of the individual practice. Construction Managers (Project Managers): Can be an asset to a

complex project if they understand the big picture of how the facility affects the dental practices long-term growth. Their involvement creates another layer of expense that can sometimes be justified if they have “Cost Based Design” training and extensive experience in dental projects.

Dental Supply Drafting Services: Provides basic space planning along with utility requirements and specifications primarily focused on supporting dental equipment sales. The plans provided should be incorporated into complete architectural and engineering documents that are typically provided by architects, engineers or a Design Build Contractor that the dental supply company recommends. The checks and balances that most dentists require when making a complicated expenditure is important to consider. The total value within all aspects of the project can be compromised without progressive project management expertise. Dental Consultants: Professionals that rely on their

particular backgrounds (interior design, accounting, practice management, etc.) to provide expertise in the design and construction process and may not always fully understand the details that are outside of their training. There are also nationally recognized lecturers that can provide excellent expert information and concepts, however, these resources can be limited in providing the personal details needed for a particular project.

This is a progressive process that begins with project design, budgeting, and possibly financing coordinated with acquisition or lease negotiations. The process then moves to contractor negotiations and construction management which produces control of the desired results, while encouraging practical cost solutions from everyone involved. Cost information, along with data from previous dental projects should be blended with extensive experience in evaluating and developing a custom Project Design Strategy. This strategy is tailored and focused on the dentist’s budget and practice marketing objectives. One of the primary goals is to take the concepts that are available in today’s dental industry and evaluate them against the needs and personality of the individual practice.

In addition to considering these specialty resources important strategies need to be considered. One key strategy that should be a vital part of this process is Practice Focused Investments. This concept evaluates today’s technologies and advancements, while making recommendations based on budget priorities, and the doctor’s preference of style and practice. This approach determines a long-term investment strategy. Among the topics that are included in this evaluation and discussion are:

There is not a “cookie-cuter” solution that works best for all practices. Each dentist has preferences, ideas, and a style of dentistry that should be incorporated into their design process. The idea of individual preferences being integrated into a new design is important, but it can also lead to bad habits or designs being ignored and recreated. All of these issues and potential problems should be considered and discussed during the “Cost Based Design” process. Working with a company that uses the “Cost Based Design” strategy can address these concerns.

Soft Tissue Applications of Lasers

for the General Dentist by Dr. David M. Roshkind The use of lasers for soft tissue applications in general dentistry is now a widely accepted modality of treatment. To some extent patients have begun to expect the availability of laser use in general dental practice. The use of a laser can either be as an adjunct to other procedures or the main form of treatment itself. For a number of procedures lasers have now become the treatment of choice by both clinicians and patients, and in some cases, it has risen to become the standard of care. Though there are many uses for lasers in general practice1,2, this article will address only those indications for soft tissue use that have received marketing clearance by the U.S. Food and Drug Administration (FDA). The categories of lasers that are presently used in dental soft tissue applications are shown in Table 1. This includes laser wavelengths from the visible range through the near, middle and far infrared ranges.

SOFT TISSUE LESIONS Laser use is often commonly thought of and seen in the removal benign lesions, a few of the more commonly treated lesions will be discussed here. Fibrous Tissue Removal (Fig. 1)

Biopsy A variety of lesions may be encountered in a general practice over the years, and some may not be very common. With increasing skill and training these conditions may be treated initially in the general practice. However, careful assessment of the difficulty of the intended procedure, as well as the scope of training, skill and knowledge of the surgeon must be made before it can be determined whether the patient would be better served by a referral or treated in the general practice.3 Fibromas A very commonly seen lesion in general practice is that of fibromas. Fibromas are found in a variety of locations, appearances and sizes, and are often associated with some irritating factor. They are often found to be located on the lips, cheeks, and tongue. These lesions are usually best treated by excisional biopsy using traction and subsequently sent to a laboratory for histological examination to rule out any pathology. (Figs. 1-6) These surgical sites heal best by secondary intention without sutures. Scarring is found to be minimal.4,5

Papillary Hyperplasia Papillary hyperplasia, especially the type that is often seen on the palate associated with a poorly fitting full or partial denture, can be treated by using a laser. The laser is used in a non-contact mode (away from the surface of the tissue) and ablating (vaporizing) the unwanted surface tissues. The remaining tissues, after healing, are left with a healthy and more easily impressed tissue surface whether for a new denture or implant placement with restorations.6 Hemangiomas and Venus Lake Lesions Hemangiomas and Venus Lake lesions, often seen in the older populations on the lips, cheeks, or mucosa, are sometimes unsightly and commonly remain untreated for many years because patients and practitioners alike are unfamiliar with how a laser could easily be utilized for this procedure.7,8 A dental laser is used to puncture into the lesion and coagulate the blood volume. Simply stated, the lesion collapses like a balloon, shrinks, and the bleeding ceases; and, without sutures, healing proceeds uneventfully with little evidence of a previous growth. (Figs. 7-9)

CLINICAL APPLICATIONS Laser Assisted Gingival Retraction (Troughing) Another daily application for a laser in general practice is during crown, bridge, and veneer applications. The results are a beautifully visualized margin with a dry and blood free sulcus which helps to yield a clean and crisp impression. Laser assisted gingival retraction is an excellent alternative to conventional retraction cord.9 This is accomplished by using a laser in small arcs around the tooth sulcus until the entire

circumference is treated. With proper settings and technique, the tissue and margins will predictably maintain their height from the time of impression until the time of seating of the crown, bridge or veneers. When using CO2 and Erbium family lasers (Er:YAG and Er,Cr:YSGG), care must be taken to avoid contact with the tooth surface, as this could cause unwanted damage. This is not as much of a concern when using Nd:YAG or Diode lasers, as their interaction with the dental hard tissue is negligible. (Fig. 10) Cosmetic Gingival Recontouring Gingival height or shape discrepancies can be easily corrected with lasers. As stated previously, the gingival contours are maintained and the field can remain dry and clean ready for impressions.10 The healing shows no significant change in tissue height from visit to visit as contrasted with electrocautery or even a scalpel.11 If cosmetic or gingival recontouring of the soft tissue is desired or indicated prior to tooth preparation or impression, then a laser is the instrument of choice to accomplish this procedure. Soft Tissue Crown Lengthening Similarly, when soft tissue crown lengthening is needed due to excessive tissue height, laser use is an excellent choice. As with any surgical procedure, the proper diagnosis and determination of adequate zone of attached gingiva are necessary for fine clinical results and tissue health. An example of a cosmetic indication is recontouring a “gummy” smile due to altered passive eruption. The biological width must be determined and a minimum acceptable width maintained.12,13 Subgingival Decay When presented with a carious lesion that extends subgingivally, the removal of a small amount of tissue assists in restoring the tooth with a clean, dry, properly restored and finished margin. Gingival Hyperplasia Removal Gingival hyperplasia can be from a multitude of causes and is often seen in general practice. It can be isolated to one area of one tooth or involved large numbers of teeth throughout the mouth. Some of the causes of gingival hyperplasia can be orthodontically related, drug induced, periodontally induced, or iatrogenically produced due to poor restorative margins. Though these could be treated by using various methods, here laser removal is the treatment of choice due to the reduced amount of bleeding, decreased postoperative discomfort, and eliminated need for periodontal packing, dressing or sutures. An additional benefit is the reduction of the bacterial levels at the laser treated sites.14 (Fig. 11-13)

Operculum Exposure of Unerupted Teeth We see patients that present with retained cuspids or other teeth that are completely covered with overlying tissues that is causing delayed eruption. This tissue needs to be removed to allow these teeth to be orthodontically moved into position or to naturally erupt. The operculae or overlying tissues can be readily excised with the careful use of a laser in order to expose the clinical crown in a dry bloodless field. This will allow the immediate bonding of a bracket or button to the impacted tooth, saving chair time and eliminating extra visits.15 Implant Second Stage Recovery Many general practitioners are now placing implants or restoring implants. For those general practitioners, second stage recovery to allow for the placement of a healing cap or abutment is easily accomplished with laser use. In situations where tissue has grown over the margins of an abutment, the use of a laser to trim the excess tissue allows for proper seating of the impression coping or the final prosthesis. A clean, dry, and bloodless exposure site allows immediate impressions which are crisp and accurate. Certain common sense precautions

should be taken such as not to hold a laser tip in one location too long to avoid any heat buildup to the implant fixture. If the tip is moved in a normal manner heating of the implant body should not be of any concern.16 (Figs. 14-16)

Tissue Pigmentation Reduction A unique laser procedure that is little known which could be helpful to some patients is reduction of dark tissue pigmentation. These darkly colored gingival tissues may be perceived as detracting from an otherwise attractive smile. Depigmentation can lighten the tissues and leave a pinker gingival tone, which can be especially important to those that have a high smile line. The results can be quite remarkable. The technique involves removing tissue to the level of the melanin producing cells. The tissue then heals uneventfully and with a new lighter color.18-24 (Figs. 17-19) Frenum Interference Correction An excessively high frenum attachment, often found in labial mandibular or maxillary frenae, could cause a pull and be associated with recession or loss of attached gingiva. Additionally, a short mandibular lingual frenum can cause speech or suckling problems. These are easily treated by a laser frenectomy. Again the advantages of reduced bleeding, decreased post-operative pain and elimination of the need for sutures, makes the use of a laser ideal for these surgical procedures. During surgery, it is necessary to ascertain that all of the muscle fibers are ablated or cut through to assure that reattachment does not occur.25, 26 Pericoronitis Partially erupted third molars can become cyclically infected with Pericoronitis. The excessive tissue that usually causes this problem can be easily removed and the pain relief is quick and effective. Aphthous Ulcers An aphthous ulcer is one of the most common oral lesions seen in a general practice. This condition can be very annoying to the patient, though not generally a serious health threat. There are palliative topical applications available that can alleviate and reduce the symptoms caused by these ulcers, however, these lesions are ideally suited for laser treatment which is simple, quick, and very effective. 27,28 The lesion can be treated at any point from the prodromal stage to the healing phase and takes little more than a few minutes. The objective is to exchange a painful wound caused by viral and secondary bacterial infections with a painless wound from the controlled application of laser energy. This greatly reduces the viral and bacterial levels and completely desensitizes the wound. Additionally, there have been many clinical reports that the lesions do not recur in the same location on future outbreaks. The technique involves use a laser with no anesthesia in order to use patient feedback as an indicator of the progress of treatment. Any laser can be used for this procedure, but the settings will vary by wavelength and delivery system. The laser energy is applied from a distance of about 1 cm at the lowest energy setting in a gated or pulsed mode of operation and the laser tip is constantly moved in a circular or back and forth motion, covering the entire surface of the lesion. The clinician then slowly moves the laser toward the ulcer. If there is discomfort, the tip is moved away and then a slower rate of advancement is attempted until the tip is just out of contact with the tissue. The clinician

then stops and raises the energy level, repeating this technique several times until the surface changes color to a light or medium yellow brown and the patientâ&#x20AC;&#x2122;s tongue can comfortably touch the area. This is a very valuable treatment in terms of its dramatic and immediate relief for the patient as well as the confidence that the patient gains in the doctor and the office. Herpetic Lesions A closely related treatment is used for herpetic lesions. The protocol described above is followed; however, the practitioner should be more careful by being sure to use high volume evacuation so as to remove as much of the plume (vapors and its contents) as possible in order to avoid contamination from the aerosolized viruses to the operator. Additionally, laser masks with a very fine 0.1Âľ particulate filtration size are recommended. If treated early in its formative stage, the time of healing is reduced. As with the aphthous ulcers, the herpetic lesions heal comfortably and uneventfully when treated at any point along their development.29 SUMMARY The use of lasers in general dentistry is now an accepted, and to some extent, expected modality of treatment. Laser use can be either an adjunct to other procedures or the main form of treatment itself. The advantages of laser use, reduced bleeding, reduced need for anesthetics, improved visibility, reduced bacterial counts, and reduced post-operative pain have all been demonstrated. Therefore for many procedures lasers are now becoming the treatment of choice by both clinicians and patients, and in some cases, the standard of care.
