A New Year always brings to mind the things we need or desire to improve in our personal and professional lives. Our company abides in the belief that all things can be done better and that being complacent is a danger to any business. There is another side that needs to be remembered as we think through the opportunities and possibilities of a New Year.

t PPM, our clients rely on us daily to give them good tax advice. They expect us to help them arrange their affairs in a way that helps them minimize their taxes while keeping them within the law. This is a responsibility we take extremely seriously for one very important reason: our reputations depend on it. If we were asked to pick only one tax planning strategy to recommend to a dentist, whether young or old, it would have to be one that is extremely powerful. We would need it to put the most money back into their pockets with a minimum of risk. It would have to be supported by a great deal of evidence, so that we had confidence that our advice actually worked to our clients’ benefit over the long haul. If we could only choose one, it would be this…start and fund a tax deductible plan. Whether you are just starting out in your practice or you are approaching the finish line, this one piece of tax planning advice has the power to make a big difference in your tax burden this year and to move you closer to achieving your financial goals. For those of you who are just starting out, making the right choices now will provide you with immediate benefits and will open up possibilities that won’t come again later. For experienced dentists reaching your peak earning years, and who perhaps have some catching up to do, there are options designed just for your situation. Being in the wrong plan late in your career can be financially disastrous, so having a trustworthy adviser to help you navigate this terrain is critical. Now, what is it about retirement plans that make them such a powerful tax planning tool? Retirement plans do two vital things that make them an indispensable tool for any business owner: 1) They harness the awesome power of time to your advantage, and 2) they reduce the total taxes high earners pay over their entire lives. The Unbelievable Power of Time With retirement so far in the future, and with so many demands on your money, why would a young dentist think about saving now? Consider this: a 21 year old who saves $5,000 at the beginning of each year - earning 7 percent - can actually stop saving completely after 10 years, and still accumulate over $1M by age 70. However, if that same person waits until age 31 to get started it will take saving 4 Dental Explorer | First Quar ter 2016
$5,000 every year until age 70 to clear the $1M mark! This fact seems like it can’t possibly be true and yet it is. Compound interest over time is the unbelievable force that makes it work, and you want it working for you not against you. The above example illustrates how powerful an early start can be and how costly it can be to catch up if you delay. The younger dentist who initiates a pension plan early as part of their tax strategy, can open up opportunities that would have been unavailable without it. Getting off to a fast start can create the flexibility for an earlier retirement date to travel, having a better standard of living when older, or being able to spend more of the money you are making now. Remember, the person who waited 10 years had to put away 4 times as much as the early bird in order to reach the same point!
Featured Article Here is a basic rule of thumb to help you understand the need to save. By age 35, you need one year of salary saved. By age 40, you need two years of salary saved. By age 50, you need five years of salary saved. By retirement, you should have saved a minimum of ten times your annual pay. This should be a balanced portfolio of cash, stocks, bonds, investments and, last but not least, the sale of your dental practice when you are ready to retire. The practice value alone will not be enough for most dentists to retire on, but it can account for a slice of the pie to help you reach your financial independence. If possible, start on day one of your first job after graduation, since the first few years of your working professional life will be the lowest tax bracket you will ever know as a dental professional. I would suggest contribution to a Roth IRA until your income exceeds the contribution limit. As of today, you can contribute up to $5,500 annually, and since you will be in a relatively low overall tax bracket, this is the time to make after-tax contributions to your future. Most dentists make the investment in their education, not to become a career employee, but with the goal of opening their own practice. As soon as possible after opening your practice we recom-
mend setting up a SIMPLE IRA for the office. As the owner, you will be able to set aside $12,500 + a small company matching contribution. If your spouse works for the office you will be able to set them up as well and they will be able to defer $12,500 from their wages. One of the best attributes of a SIMPLE IRA is that it is simple to administer and you do not need a “Profit Sharing Plan Administrator” to run the plan as is required for more complex savings plans. Once you are in the top tier of tax brackets it is a good time to set even more aside, which will lower your income tax burden. The traditional 401(k) is probably the most widely known retirement product out there. It generally enables a business owner and its employees to make tax-deferred contributions during the length of their employment. 401(k) plans offer a lot more versatility by offering higher contribution limits, and also offer more choices to help you manage and attain your personal financial goals. You can choose to match or not, provide a vesting schedule, or enable penalty-free access to funds via a loan if an emergency arises. 401(k) plans also allow for “catch-up” contributions after reaching the age of 50. Now, if you didn’t get off to a fast start, don’t lose heart. Some plans take age into account, giving older participants an advantage since they have less time to save. And for dentists with very high earnings, there is an option that can allow you to double, or more, the size of your contribution compared to a traditional plan while still claiming a tax deduction. These plans are tailor-made for the empty nester with high income, no mortgage, and a need to put money away very quickly. It is still better to save earlier than later, but this option can give those who need it an opportunity to catch up. A well-designed retirement plan also reduces the amount of taxes high earners pay over their lifetimes. This is of primary concern for dentists because your income level tends to put you into the top income tax brackets while you are working. Retirement plans minimize income taxes by allowing savers to change the timing of their tax payments. Some plans like 401(k)s and profit sharing plans shift payment of taxes into the future. Others such as Roth IRAs or Roth 401(k)s shift taxes into the present. The trick is to shift income out of high tax periods into low tax periods. Whether it is better to shift your income into the present or the future is a complex question to answer. This is because the tax law is complex and ever changing, many variables influence the outcome, and it is not practical to wait 80 years to see how things turned out. “So, ” you might ask, “how do you know saving with deductible money works?” In a 2008 study for the National Bureau of Economic Research, Boston University economist Lawrence Kotlikoff and his co-authors Dental Explorer | First Quar ter 2016

Featured Article sought to clarify this very question. They wanted to know whether it was better to save with deductible or non-deductible money. To find out, they modelled a lifetime of spending and savings choices – including tax effects – under a variety of assumptions about household composition, income level, saving strategy, and future tax policy. They used proprietary software that could handle large amounts of data over very long periods of time, and included a hypothetical tax increase in retirement in several of their scenarios. The latter assumption was important, because it is at least possible that the tax “pain” in the future might outweigh the tax benefit in the present if future tax rates rise. The results showed that middle income and single earner families often benefit more from savings taxed in the present like a Roth 401(k) (non-deductible), while earners in the upper income ranges paid significantly lower taxes over their lifetimes when they saved with tax deductible money. This was true even if tax rates go up in the future. I have been advising my clients to sponsor a tax deductible retirement plan ever since.

Since it can be costly to be in the wrong plan, we recommend you periodically review your plan design with your tax advisor to be sure it is meeting your current needs. Plan reviews and amendments will require a fee, but in our experience these are far less costly than being in a plan that you have outgrown. If we could only give you one piece of tax advice, one that would save you money, expand your career options, and set you up for success, it would be to pay into a tax deductible pension or profit sharing plan and to do it as early in your career as you can justify. This tax planning strategy can save tens of thousands in tax payments for many doctors, and it has the support of scholars who say that high earners who save in this way pay less lifetime tax than those who don’t.
All Plans are not created Equal Having a pension plan is definitely preferable to not having one, but all plans are not created equal. There are some fairly simple, low cost plans that can be very economical to maintain, but they have low contribution limits. One of them does not even require a retirement plan tax return. Plans like these are ideal early on when the business owner’s income may be lower and a lot of the family budget is already spoken for. There are some plans with very high contribution limits best suited for the highly compensated empty nester, who needs to build assets quickly, and of course many points in between. A good deal of professional judgment is necessary to know which plan is right for a given Doctor and especially to know when it is time for each. It is very common for a practice to cycle

W
hat does it mean when a patient accepts your treatment? Does it mean that you can communicate treatment effectively or know how to write the best treatment plan? Or better yet, does it really mean you know your patients well enough so they can trust you enough to accept the care you recommend? Presenting treatment to a patient can actually be very relaxing when you have a clear understanding of the patient’s goals, know your patients, have a written plan, and include the patient in the process. When most dentists think of a consultation appointment, they think of it as a time when they tell patients what they need based on the results of their findings during an examination, and the patient either accepts or rejects the plan. Although this is common, I want to provide you with a new perspective. When you have taken an interest in the patient to determine what is important to them, you are able to help them make better choices and customize a treatment plan that will be appropriate for the patient.
There are several factors that must be taken into consideration that have nothing to do with the conditions present in the patient’s mouth before you can begin to write a treatment plan. Whether we like it or not, we know these factors have enormous influence on the patient’s ability to accept treatment: • • • • • •
patients must sense that the dentist “knows” them the doctor’s understanding of the patient’s defini- tion of health a patient’s opportunity to provide their input combined understanding of prior decisions by patient and doctor the patient’s experience with dentistry mutual understanding of what the patient wants
Just about every dentist I have ever met has expressed to me the despair they feel when having to present treatment to patients, and most of that despair is because they just never seem to know where to start. The frustration starts because they know there should be a specific sequence to delivering treatment and they confuse that sequence with a way to present treatment to patients. A dentist always likes to start with the gums, move on to decay, restore broken or missing teeth, and finally addressing cosmetic concerns. While this may be an effective way to treat conditions, it’s not very effective when consulting with a patient. The best way to consult with the patient is by learning where they would like to start. Ms. Jones, the last time you were here we talked about a lot of different things and I know I gave you a lot of information to think about. You wanted to speak with your spouse about some of the things we discussed. Before we get in to my treatment recommendations, I wanted to know if you had any other thoughts or questions that may have come up since we spoke last. Dental Explorer | First Quar ter 2016

Featured Article What you’re doing here is providing a simple summary on how your treatment recommendations will help her achieve her goals. It’s important to carefully listen to the answer. It wouldn’t make sense to go into fees or other details if you are not in agreement on the general direction you’ll take. Don’t worry if the patient has more questions, even if they are about money or insurance benefits. This is all a part of the process of helping the patient work through the details of your treatment plan in their own mind. She has to see it for herself. The more input you allow the patient, the easier you’ll make it for her to make If she hasn’t changed her mind about her goals since a decision. the examination, it would be appropriate to start with them. Otherwise, start with the patient’s chief concern and use your recommendations to explain how you can solve her problem. This is one of the moments of truth in your relationship with this patient. Listen very carefully to determine if there’s been any change in her goals for her oral health. Is she bringing up things that she did not discuss before? Does it seem she is changing direction? Is she asking about something in particular? Pay close attention to be sure you understand before proceeding. It’s okay to let the patient determine where you should start. It’ll be easier for you to gain their trust when you’re not following any hard and fast rules.
Don’t rush into procedures too fast. Verify that you understand the patient’s goal for treatment before you start. As I looked at my notes from our last visit, I got the sense that you wanted to be able to keep your teeth for the rest of your life and keep more of them from breaking unexpectedly. Ms. Jones, did I get that right? You also wanted to maximize your insurance benefits and I know we can work with you on this. I’d like to talk to you about a treatment plan that will address the wear on your teeth, as well as prevent any future wear and protect your teeth from breaking in the future. Again, listen very carefully, because you must have agreement before you can go into any details. Think of it this way, the details will not make any sense to the patient until she senses that you understand her goals. Ms. Jones, I know that we can even out your bite to keep your teeth from wearing down any further and restore the broken teeth with crowns to make them stronger. Even after the general direction of the treatment has been established, it’s still not the time to go into treatment details. Now is the time to provide your patient with an idea of the cost involved. Don’t back away from this. Make it your responsibility to present the entire cost of treatment. Ms. Jones, to treat the wear on your teeth and restore the ones that are broken in a way that will prevent them from wearing and breaking in the future, when I’ve done this for other patients they spent as much as (give them a number) $15,000, how does this sound?
This is where most dentists feel the most despair. I would too if I presented a fee without knowing if my patient agrees that my recommendations would solve their problem. Now, depending on who you’ve been listening to, you may have been told you’re off the hook because when the conversation changes to fees you should leave the room. I want to make a distinction here because this discussion about fees is different from making financial arrangements and is very important in the process of establishing a treatment plan. This part shouldn’t be left to someone else. Take a moment to put yourself in the patient’s shoes. How would you feel if the doctor never told you how much it would cost? Would you think the dentist was confident? Most dentists are either embarrassed, fear rejection, over anticipate how the patient will respond, or are just not comfortable openly talking about the cost of treatment. In reality, patients expect the dentist to talk about fees. The more open you can be about the financial aspect of the cost of treatment, the less of an obstacle it becomes, and the stronger your relationship with the patient becomes. It will also be more likely that the patient will engage with you about other alternatives if necessary. Even though it’s hard to accept, we know money is a big part of a patient’s ability to accept a treatment plan. A former dental client of mine, Don Deems, says, “When you get comfortable talking about money, so will your team, and both you and your team will quit accepting the excuse that money is the reason why patients don’t proceed with treatment. You can begin to take a real look at why patients are not accepting your treatment recommendations. You’ll be able to talk more openly about money, put the cost of care in perspective, and have a healthy, positive attitude about the patient not only receiving treatment, but also about payment for your services. It is true that money is a concern. It is with everybody.” Working with patients on all aspects of their care, including a fee discussion, creates a better working relationship between you and your patient.
