Standards of Care or Caring about the Standards

Standards of care for the dental hygiene profession were adopted by the American Dental Hygiene Association (ADHA) years ago and have been updated and revised as late as 2016.1 These standards provide a framework for the dental hygiene professional to assume personal responsibility for care based on current scientific evidence. All dental hygiene students are introduced to these standards in their first semester of school and throughout the remainder of the program. After years of teaching dental hygiene and introducing these standards to students, it still surprises me that there are hygienists who seem confused when asked about the standards of their profession; particularly when it comes to ethics and responsible patient care. Through my interactions with hygienists and students across the country, I’m constantly taken aback by stories and questions concerning the lack of infection control measures, negligent treatment, unethical behavior and unsafe working conditions. Dental Hygienists are highly educated. However, they feel at the mercy of their employer when it comes to implementing safe practices and standards. It seems that many hygienists are often put into uncertain situations and reach out to social media to answer their questions or to comment on their concerns. Whenever I read these blogs and posts, I can’t help but refer back to the standards provided by our professional organization. Truly, all of the answers can be found within this framework. I believe hygienists already know what to do in most of these cases. They are simply looking for reinforcement. For this reason, I thought it might be helpful to review a few of these

standards, along with real life situations, to bring them into perspective and give hygienists confidence in their decisions.

Within the ADHA Standards of Care, there is a whole section devoted to Professional Responsibilities and Considerations. Below are a few bullet points that can serve as a brief self-assessment list for hygienists.

Understand and adhere to the ADHA Code of Ethics The ADHA Code of Ethics is a lengthy but important document found at the end of the ADHA Bylaws document.2 It outlines the purpose, goals, beliefs, core values, principles and ethics of the dental hygiene profession. It defines our objectives as dental hygienists and basically sets the parameters for our behavior and ethical consciousness.

Every day we encounter patients with special needs and health conditions. We are responsible for documenting health histories and treatment records. All documentation must be complete and accurate. These are legal documents to be protected and maintained.

Sharing photos and patient information on social media or in casual conversations violates the patient’s privacy and is definitely considered inappropriate and unprofessional conduct. Offices must comply and be up-to-date on the Health Insurance Portability and Accountability Act (HIPAA). Patients must be made aware of their rights regarding patient records and electronic transferring of that information. Having lived through the introduction of computers and the internet into the dental practice, I can appreciate the technology and what it does to increase our efficiency and effectiveness.

At the same time, social media has become so mainstream that hygienists often forget that what they encounter in patient care doesn’t necessarily belong on a Facebook post.

Be conscious of this and respect your patient’s right to privacy.

Demonstrate professional behavior.

This is a broad statement and can be interpreted differently by everyone. Professionalism can encompass behavior in the form of integrity, attire, punctuality, and adherence to infection control measures. Skills in the form of communication and clinical expertise are critical. Knowledge of trends, products and procedures keeps us current and relevant. Ultimately, it is our level of knowledge and skill that determine our behavior. As a Registered Dental Hygienist, you have been credentialed and hold a professional license. Your conduct inside and outside the practice is influential and reflective of your level of professionalism. For those of us in smaller communities, we may come in direct contact with our patients at church, sporting or local events.

Regardless of not being “at work”, your conduct can influence how others perceive your clinical behavior and overall integrity. Your social media posts can also show another dimension of your professionalism or lack

thereof. Unfortunately, big brother is always watching.

Maintain compliance with established infection control standards following the most current guidelines to reduce the risks of healthcare-associated infections in patients, and illnesses and injuries in health care personnel

•

Prevent situations where patient safety and well-being could potentially be compromised

These two bullet points are easy to combine and are probably the most commonly mentioned. Patient welfare should be placed above profit at all times.

Daily we encounter many obstacles that prevent us from adhering to these standards. Insurance benefit limitations and basic cost of doing business expenses frequently top the list of barriers to providing quality care. I can’t tell you how many times I hear about offices that skimp and/or totally skip on infection control practices because “it costs too much” or “it takes too long”. Allowing only one mask per clinician per day, reusing disposable products, procrastinating about water line treatment measures as well as purchasing products from gray or black-market platforms are just a few of the “cost saving tricks” that are alive and well. But, that’s exactly what they are, tricks. These behaviors don’t save money in the long run and only serve to endanger the safety and welfare of both the clinician and patient.

One particular recommendation that has received a lot of attention, and pushback, is the statement from the Centers for Disease Control (CDC) in April 2018, that reiterates the importance of handpiece sterilization.3 CDC guidelines for infection prevention and control state that, between patients, dental health care personnel (DHCP) should clean and heat-sterilize handpieces and other intraoral instruments that can be removed from the air and waterlines of dental. 4 This recommendation is based on studies that have shown that the internal components of air-driven dental handpieces (both low-speed and high-speed devices) can become contaminated with patient material during

Featured Article use, and this material can then be expelled into the mouth of other patients during subsequent uses.5-8 A good rule of thumb is to have three handpieces per operatory. One in use, one sterilized and ready to go and another in the sterilization process. Yes, there is a cost associated with purchasing new handpieces. But, based on the evidence, not following this recommendation compromises the patient’s well being. Standards require us to maintain compliance with established infection control practices. Once we are aware of the research and the recommendation, we can no longer plead ignorance. Manufacturers are aware of this CDC guideline and have responded to the marketplace by offering numerous specials and options to make handpieces and motors more affordable.

Commit to lifelong learning to maintain competence in an evolving health care system.

Every state has different criteria for the number of continuing education units required per year as well as how those units are acquired. I recently moved to a state that allows all credits to be awarded via online or through webinars. While I’m not suggesting that those aren’t convenient and reputable sources, I do wonder if we are losing sight of the importance of networking to enhance the educational experience. Unfortunately, too many hygienists attend the same meetings year after year and become bored because they are listening to the some of the same speakers discussing the same topics. Attending a live course allows you to maximize learning opportunities through hands-on experiences, relationship building with key opinion leaders, and exposure to techniques and technology that may not be accessible through online offerings. Get outside of your comfort zone and attend out-of-state and national meetings. Make an effort to grow your professional relationships. Our healthcare system is ever-changing so staying abreast of what is going on nationwide can help you bring innovative products and techniques to your practice.

The Dental Hygiene Oath that we all recited at pinning or graduation is relevant no matter how many years we practice. It is our pledge to promote high standards of quality care as well as to uphold the highest standards of professional competence and personal conduct in the interest of the dental hygiene profession and the public it serves. 9 As licensed clinicians, we have the

responsibility of making critical health care decisions.

It is up to each dental hygienist to assume responsibility for the care they give. Through reasoning and self-assessment, hygienists can begin to think on their own rather than rely on word of mouth and opinions for critical information. Standards of Care are simply guidelines, not substitutes for sound clinical judgement.

After earning her BS in Dental Hygiene from the Medical College of Georgia, Cathy practiced clinical dental hygiene for several years before joining MCG as an educator and clinical instructor. Upon receiving her MBA from the University of Tennessee- Chattanooga, she served as Office Administrator for a dental practice management consulting firm. She also contributed significantly to the development of educational and marketing materials as the SE Regional Manager for a dental loupe manufacturer. As clinic coordinator and instructor, she was able to draw from her unique career experiences to provide practical and relative experiences for her students. Along with serving as the Student American Dental Hygienists’ Association (SADHA) advisor, Cathy taught pre-clinical didactic courses and labs as well as periodontology. Her credits include presentations at several State Dental Hygiene and Component meetings as well as publications in the journal of Contemporary Oral Hygiene and RDH Magazine. Cathy is a member of the American Dental Hygienists’ Association and the Florida Dental Hygienists’ Association. As clinical specialist for Simply Hygiene™, she now works with practicing hygienists and dental hygiene programs to promote education through the introduction of new technology, products and ergonomic practices.
