The Good Samaritan Health Center of Gwinnet t is a 501(c)3 nonprofit organization that was established and opened in 2005 as a full time Christian charity health center. The mission of GSHCG is to demonstrate the love of Christ by providing quality low-cost health care services to the poor and uninsured. Our purpose is to glorif y God through meeting the general healthcare needs of our patients as well as their spiritual needs through prayer and biblical counseling.

GSHCG made a significant impact in the lives of individuals with over 17,000 patient visits in 2015 alone. In fact, the health center has experienced a 400% increase in patient demand since 2011. In order to increase our ef forts and meet the growing need for a comprehensive approach to charitable healthcare, we will launch Gwinnet t’s first full-time charity dental practice and dispensary in addition to our current low-cost medical and counseling services for the entire family. Our new facility of nearly 13,000 square feet, located at 5949 Buford Hwy, Norcross, GA, will open in June 2016.

As we support Community Healthcare Centers, missions and organized outreach programs, we are struck by the heart of those that give of their time and personal resources to make a difference in the lives of the “underserved.” Once a patient experiences the care given by volunteer dentists, hygienists and assistants, the term “underserved” seems to fade. Not only are the patients receiving excellent care, there is an element that goes much deeper. Many who show up for care are in physical pain, in need of relief. But some are also hurting emotionally or spiritually and struggling with being “under acknowledged” as valuable individuals in a busy and sometimes impersonal world. What we each “give” in terms of materials, time or skilled hands are important.

May we also remember that the care imparted to each patient in terms of personal encouragement and acceptance is priceless. Healing comes in many forms and changes hearts and lives. Thank you for what you are able to share with the world around you.

“Dental Dollar,” “Dental Metrics” and “K.P.I.’s.” All of these catchphrases serve to provide information in a quick, brief and efficient manner by grouping various expenses that are useful and can be used to benchmark your practice against standards as well as against other practices.

Logically categorizing expenses will give “basic financials” a new meaning. By grouping accounts into accepted standard dental groups, the average Profit & Loss Statement offers a wealth of new information and takes on a new meaning to the reader. The basic categories of grouping expenses are: Personnel, Production, Facility, Administration, Advertising, Owner, and Associate (if applicable).

G

eneric financial statements that list “collections minus expenses equals practice profits” could be so much more than just a list of items in alphabetical order culminating in a bottom line profit. Don’t get me wrong, profit is very important, but the detail that falls in between revenue and profit contains a lot of data that can easily be reviewed by the doctor in only a couple of minutes if presented properly.

The goal of a Dental CPA is to provide advice along with a financial tool that allows the dentist to quickly and easily review the practice numbers.

Good dental financial statements will present information by category in a way that shows the doctor a “Snapshot” of the practice. This snapshot would ideally group dental expenses into major dental categories that all dentists will recognize. Most dentists have heard about the

The first two groupings of Personnel and Production comprise almost half of the expenses of a dental office. Personnel Costs would include: Staff wages (hygiene / front office / chairside / manager); payroll taxes; retirement plan contributions; and fringe benefits. Generally, this group will make up about 25% to 30% of overhead expenses. Production Costs would include: Dental Supplies and Laboratory Expenses. Generally, this group will make up about 10% to 15% of overhead expenses. With the increase of in-office CAD-CAM / milling machines, which should lower laboratory expense, it is important to list dental supply and laboratory expenses separately but consider them as one group. The cost of milling blocks will often be categorized as a dental supply expense since they are ordered from the dental supply representative and are delivered along with the practice’s normal supply order. Dental CPA’s prefer to see the milling blocks categorized as a laboratory expense if possible since they are replacing outside lab costs. By grouping these two expenses together, we are able to see the individual expense ratio variances, which may be over or understated but be perfectly acceptable as a group category ratio.

Another key to readable and comparable dental financials is to categorize “Owner” and “Associate” expenses separately. By lumping the owner & associate wages with staff payroll, the personnel group becomes overstated and not easily comparable. Proper grouping allows for comparison between a Sole Proprietor / L.L.C. / S Corporation / C Corporation and Partnerships.

Owner and Associate groupings would typically show: Salary, Retirement Plan Contribution, Health Insurance, Continuing Education, Charitable, Uniform, and Auto Expenses.

Now that your expenses have been re-categorized into meaningful groups, the results can now be compared to dental industry benchmark standards.

Financials should also show Current Month and Current Year-to-Date compared to Prior Year to determine if the practice is growing and meeting goals. Month and Year-to-Date Profit and Loss Statements with %’s of collections is crucial to seeing how your practice compares to dental benchmarks. Variances from standards are, at a minimum, a good starting point for asking a few questions. A few questions and answers with some follow up analysis can improve your dental office’s performance and profit.

Keeping your practice productive, profitable and vital is a necessity for every private practice. To achieve your goals, you will need to make good strategic decisions, like knowing when the time is right to equip an empty operatory, hire an associate, enhance your marketing plan, or even build a new office. Like every good leader, to make good strategic decisions you will need good information. As previously mentioned, a properly 6 Dental Explorer | Second Quar ter 2016

As a practice owner, you already know that practice management statistics provide very important non-financial information you need to fully understand the health of your practice. In fact, your practice management software could probably generate more statistical reports than you could read in a month. With the time demands of patient care and a busy modern life, you need to focus on a select few that really matter. My top two are: New Patients per Month, and Units of Crown and Bridge per Month. New patient stats have the power to tell you whether your patient base is growing or shrinking and by extension whether your marketing plan is working. C&B stats give you important information on whether your practice is producing what it should. Here’s how to evaluate each of them.

New patients are the life blood of every healthy dental practice. In today’s competitive market, new patients are more important to your success than ever. But how many do you need? Most practices experience patient attrition of around 10% each year as patients move away or pass away – more in some communities. This means that you must replace 10% of your active patients every year just to keep from shrinking! A practice with 2000 active patients will need 200 new patients per year or 17 per month to maintain its current patient base. If our hypothetical practice plans to grow 5%, it would need another 100 new patients or 25 per month to reach its growth target. If your practice is shrinking or not meeting your growth targets, that indicates you need to focus on improving your marketing plan.

New patient numbers are fairly easy to get and the math for calculating your monthly need is very simple if you have done a recent active patient count. If you haven’t counted recently – and active patient count is something to keep tabs on – there is an easy way to estimate your active patients that doesn’t require counting charts until

oday’s Private Dental Practice must be more business-focused and strategic than ever, especially when designing a new office or making improvements to an existing one. Two priorities for consideration are, “Designing for Health & Productivity” and “Establishing an Efficient Project Process.”

Design for Health and Productivity In today’s busy private practice, it is important for patients to experience a noticeable difference in the way your office and team functions. This Experience begins by creating a healthy, productive environment for both your team and your patients, thus producing a comfortable atmosphere for building relationships and relaxed presentations of needed treatment.

Treatment Area Ergonomics The most important 40 sq. ft. in your office is the area surrounding the head of the patient’s dental chair. The way you and your team function and move in this area will impact your productivity.

Other areas of the office are important to the design process but should be secondary to the “Treatment Zone” because the treatment zone is where dental services are delivered to the patient and revenue for the practice is generated.

Here are points to consider: • Learn the principles of good posture and minimize unhealthy movements for you and your team. Ergonomic instruction of the operatory seating you use will add to your practice satisfaction and longevity. The way the doctor and assistant position themselves in relationship to the patient is the first step. Placement of work surfaces, equipment, and materials should be adjusted to the ideal seating positions. After this is accomplished, the dental team is able to encourage each other to practice good posture and movement to form healthy habits.

After a procedure begins, there are many movements that deliver the needed items to the point of treatment. All items should be easy to reach and easy for the assistant to deliver to the hands of the Doctor. A floating work surface that is over the lap of the assistant, along with organized storage systems are two important aspects of this concept. Equipment Delivery, Dental Lighting, and Technology also need proper placement, and can be incorporated into your office design plans.

Organizational Principles Making clinical organization and efficiency a priority is essential to increasing productivity.

Creating the right habits and perhaps changing bad ones is the key. Below are several principles to consider: • Incorporate a system of instrument cassettes and material tubs to organize needed items in the

Personalized Professional Image Evaluating the esthetics of your dental office is an important conversation for the dental team. One consideration is looking at the way people interact with the environment. From the doctor and staff, to patients and other visitors, the flow of the office has an impact on the way the practice is perceived esthetically. Proper zoning, having adequate space for each area, and paying attention to lines-of-sight are all elements of good esthetic design. These are obviously easier to address when designing a new office, but there are often improvements that can be made to an existing office. Another objective is to incorporate visual points of interest that highlight your office’s unique personality and display how your private practice is different from other options.

As dental professionals, we aim to please our patients, comply with regulations, and promote a safe, productive dental team while maximizing profitability. In a dental practice, there are multiple areas to assess when analyzing successful practice management.

One aspect involves regulatory compliance. Compliance includes adherence to the Bloodborne Pathogens Standard published by the Occupational Safety and Health Administration (OSHA), combined with the latest infection control guidelines published by the Centers for Disease Control (CDC).

As I travel throughout the country conducting assessments, I find it interesting to compare safety and flow in offices that use an instrument management system versus those that use traditional trays.

The problems I identify in practices not using the cassette system are (1) wasted time, (2) potential for injuries and cross-contamination, and (3) not eliciting the “wow” perception from patients.

Picture this: A dental assistant or hygienist, wearing contaminated exam gloves, walks a tray of contaminated instruments into the sterilization area, grabs the dirty instruments, and dumps them into the ultrasonic tank. Then, while wearing the same contaminated gloves, he or she grabs a disinfectant wipe, wipes the tray, and immediately places the tray on the “clean” side of the sterilization center.

What’s wrong with this picture?

Let’s identify key issues: SAFETY: Loose, contaminated instruments place the assistant and hygienist at risk for an injury. Never be deceived into thinking that your dental practice is immune to sharps injuries. As a matter of fact, years ago, the American Dental Association (ADA) revealed that accidental exposures to blood and body fluids did occur while cleaning instruments.1 Note that the auxiliary involved in this incident was wearing patient exam gloves to handle the loose, contaminated instruments. According to the CDC, heavy-duty puncture- and chemical-resistant utility gloves should be worn for the instrument cleaning and decontamination process.2

CROSS-CONTAMINATION: Dirty gloves were used to apply the disinfectant wipe. Oftentimes, the auxiliary fails to apply the two-step process of precleaning the surface prior to application of the hospital-level disinfectant.

TIME: Sorting instruments into procedural setups is time-consuming. Time is valuable and better spent serving the patient directly while adhering to exceptional infection control practices.

PRODUCTS AND INSTRUMENTS: Instrument tips are susceptible to damage (e.g., breakage), increasing costs.

Additionally, the plastic trays may be eliminated if cassettes are used. If a patient is staring at a plastic tray with stains from disinfectants and cements, we lose the “wow” experience. They may perceive a lack of cleanliness.

Now let’s create a new picture: The dental assistant and hygienist utilize cassettes with instruments organized into setups using a color-coded silicone rail system to make visual identification easy. Minutes are saved with every patient, equaling more patient care time. He or she initiates dialogue with the patient as the cassette is unwrapped: “In our dental office, we do everything possible to assure a safe dental visit.”

Patients want and deserve optimum infection control.

Let them truly understand the efforts invested in achieving compliance and how this benefits them as a patient.

When I was conducting a recent walk-through assessment, the receptionist shared a story from a potential new patient she had spoken to on the phone. This caller had gotten up from the dental chair in another office and dismissed herself due to fear of improper sterilization and infection control. The caller explained that a friend referred this particular dental office because “they are clean.” Does your office project this image of cleanliness?

CASSETTE SYSTEM SAFETY FEATURES In a cassette system, the instruments are locked into place, thus minimizing the risk of an injury during transport from the patient operatory to the sterilization area. According to OSHA, following a report of an exposure incident, the

Featured Article employer shall make immediately available to the exposed employee a confidential medical evaluation and follow up.3 Minimizing the risk of an injury promotes a safer workplace . . . and saves time and money.

Time is also money. The instruments are already organized into setups; following a completed procedure, the cassette is closed, locked, and transported. The entire cassette is placed into the ultrasonic cleaner or instrument washer Sorting and unnecessarily handling loose and contaminated instruments is eliminated, and the lifetime of the instruments is extended by protecting the working ends.

Following the ultrasonic cleaner, the cassette is rinsed, dried, and wrapped or placed into a sterilization pouch.

Following sterilization, the cassette is stored, awaiting selection.

My preference is the stainless steel cassettes for their durability and professional appearance.

CONCLUSION Implementing an instrument management system achieves positive patient perception, employee safety, compliance with regulations, and organization. The added benefit is saving time and money.
