O
n October 24th, 2003, while at the ADA convention in San Francisco, I received news that the FBI executed a search and seizure warrant on me and my practice. They battered down the back door to my office, and a team of agents carried out every patient and business record I had. I was shocked and terrified. Then things quickly moved from bad to worse. There has never been much that scares me, but one thing that scares me now is the thought of going back to prison. “Things like this don’t happen to people like me,” I told myself. After graduating from dental school in May 1981, I opened a private solo practice in my grandfather’s hardware store building in the small southwestern Virginia town where I grew up (population 1,800). There was one traffic light, and it was just down the block from my office. My grandfather was superintendent of schools, my father taught in the local high school, and my mother was a nurse in the local hospital. Growing up on a farm, I learned from an early age two important lessons: 1) how to work hard, and 2) that I wanted to do something else. Farming was too hard and paid too little. I chose dentistry and was excited to come back home to “hang my shingle.” I saw my first patient July 27, 1981. I don’t know whether my success was a result of the great people in my hometown or from being part of a family with a long-standing community presence and good reputation, but the practice grew every year. We went from two employees (my wife at the front desk plus

one assistant) to having two business staff people, three chairside assistants, and two hygienists. We worked hard to provide excellent dental care as gently as we could. My practice was located in one of the poorest counties in Virginia. Over 85% of the children in the school system qualified for free or reduced lunches, and most had Medicaid. Oral hygiene was often neglected. Children with rampant decay were the rule not the exception. We were needed and felt appreciated. Eventually, practice management consultants helped us implement business and clinical systems that increased both our productivity and the quality of our services. I was living the dream. Everything I had worked and planned for had come to fruition. Then lightning struck, and it all came crashing in around me. The criminal investigation was not initiated by Medicaid. In fact, Medicaid conducted two audits during the course of the three-year probe and determined that they did not have a problem with me, the care provided, or the billing of the work done. But once a complaint was made, Medicaid had to investigate. My Medicaid billing records were sent to a dental consultant for review (a non-practicing dentist in Kentucky). She determined that there was possible cause for further review and offered her services for the expanding investigation. It didn’t matter that I had never had a complaint to Medicaid or the Virginia Board of Dentistry or that according to Medicaid, “I wasn’t even on their radar.” The investigation expanded. My patients were examined and then my records seized. It was like a snowball had been set into motion and was rolling downhill, gathering speed and size. It was just a matter of time before criminal prosecutors gathered enough information to construct a case that they felt would convince a jury of my guilt. My fate was sealed. The search and seizure occurred the Friday before the yearly town festival, and it was in the newspaper the following day. The news spread like wildfire, and I thought I was ruined regardless of the outcome. Amazingly, I was wrong. In spite of the investigation, patients remained loyal, and the practice continued to grow and flourish, though it was difficult and slow-going. We had to provide the FBI with our daily schedule so

Agents were in the office almost every day. There were days when I left the office and would find agents or prosecutors talking with staff in the parking lot. The staff was shocked and confused; they were interviewed repeatedly. Most ended up leaving because of the stress of living under the microscope and having to deal with agents, prosecutors, and the pressures of working “in it” day-to-day. The investigation branched out into the community and into our personal lives. The FBI subpoenaed a video recording of a church service during which I addressed the issue following the search and seizure. I would get periodic calls from a business or a colleague who informed me that the agents were asking questions or subpoenaing records. Teams of agents questioned my three children appearing at their universities, presenting credentials to university authorities, and stopping my children or removing them from classes. Certain of my innocence, my thought again was that this doesn’t happen in the United States. A few days would pass without any new incidents, and we would hope that they had decided to give up the hunt. Then on October 24, 2006, our worst fear was realized. Multiple agents appeared at my home, arrested me, and confiscated all our vehicles. While being read my rights, the telephone rang. The agent in charge ordered my wife not to answer it. The answering machine picked up, and we heard my youngest daughter, distraught, explaining that agents were in Charlottesville, and they had taken her car. She needed reassurance that we were okay. She said, with a strength and determination in her voice that I can’t explain, “I’m okay. Don’t worry about me, but are you guys okay?” Again, that voice of reason in my head was saying, this can’t be real—but unfortunately it was. Further misfortune revealed that the government’s case against me was partially correct. We had made errors in billing and coding that should have never occurred and about which I was unaware. During our review I recognized the errors and offered to repay the overpayment and any penalty fine applicable. However, the government was not interested in this kind of resolution. They made it clear that they wanted prison
time in addition to any restitution and/or penalty due. I was unwilling to plead guilty and go to prison. The errors were not intentional, and I ignorantly believed this was an adequate defense. I learned ignorance is no excuse in the eyes of the law.
There were 140 acts and 10 counts charged in the indictment. The government used a shotgun approach charging one count of healthcare fraud, one count of racketeering, seven counts of money laundering, and one count of structuring. Everything, with the exception of structuring, hinged on being guilty of healthcare fraud. No healthcare fraud, then no racketeering and no money laundering. Though there were 140 acts outlined in the indictment, I was charged with a single count of healthcare fraud. However, the jury needed only to find guilt associated with a single act of the 140 alleged for a guilty verdict. The 140 acts included: • Upcoding and submitting services that were not provided. For example, during this period there were two codes for extractions performed at the same appointment. D7110 was the code to report the first extraction, and D7120 was the code to report additional extractions done at the same appointment. Instead of using both codes, D7110 was reported for multiple extractions at the same appointment resulting in an overpayment of a little less than $2.00. The Medicaid administrator who processed the claims did not catch the error, and we were paid more than we should have been paid. Additionally, there were instances where sealants were placed, and when investigative exams were conducted the sealant was missing. The examiner said the sealant service was therefore not provided.
• Submitting for services that were incomplete, medically unnecessary and which in many cases were detrimental to the patient. The prosecution alleged that many of the pulpotomies and stainless steel crowns placed were medically unnecessary and detrimental to the patient. The examiner from Kentucky indicated in her review that because caries were not evident on the x-ray or that widespread decay was not evident, the restorations placed were not necessary.
• Submitting billings for services performed in such a manner as to be the medical equivalent of no service having been performed at all. We had some root canals that failed. Although our overall success rate was better than 94%, it was alleged by the prosecution that our failures were due to inadequate treatment. (The prosecution had a dentist testify that he had never had an endodontic failure.) The jury, of course, believed if the treatment failed, it was because of the

dentist’s failure. The prosecution said that if a procedure had failed,payment should have been returned. Therefore, keeping payment for a failed procedure was fraud.
• Submitting inaccurate billings that were not supported by documentation in the patient’s chart. We did make some billing errors, and my documentation was not as complete as was necessary to support treatment decisions I made. I failed to record what diagnostic “test” I used to determine the need for treatment (occlusal stick with explorer, transillumination, x-ray, etc.). I had an intraoral camera but seldom kept copies of the pictures as documentation. An itemized treatment plan was not enough to establish need for treatment or to serve as justification to a jury as to why treatment was provided.
• Submitting billings which reflect abuse of billing for “emergency” treatment. The prosecution alleged that billing for emergency exams to determine the nature of the patient’s complaint was an abuse of the code. Prosecutors said exams should be included in the emergency treatment fee (e.g., D9110 fee), not separate from it. (Editor’s note: contrary to page 82 of the ADA’s CDT Coding Companion 2009/2010.)
• Billing DMAS (Virginia Medicaid) and private health insurance companies for services rendered in connection with a single procedure, collecting for both, and failing to reimburse either payor for fees collected. In essence, collecting double payment for services provided. Although we attached the EOB to the claim form submitted to DMAS and indicated that DMAS was secondary, DMAS processed the payment as if there was no primary insurance. We should have manually calculated the amount DMAS was supposed to pay and entered that information in the “administration only” column next to the procedure in order for DMAS to pay the claim properly. DMAS reimbursement being much less than UCR seldom resulted in a credit balance on the patient’s account. The business person at the front desk stated in her testimony that if she was aware that DMAS paid improperly, she never alerted me. The prosecution said that we should have caught DMAS’ mistake and returned the payment.
The government’s argument was that billing errors were made intentionally or with “blind disregard” or “blind indifference.” The prosecution argued that I was an intelligent, highly trained professional who bore the responsibility to see that systems and procedures were in place to identify and correct mistakes. They argued that I was being overpaid to my advantage and “probably” knew it, but even if I didn’t - I was still responsible. The truth is I was paid more than I was entitled. I am responsible. However, it was never my intent to receive any unjustified payment. I had no scheme to defraud the

government. Over the course of the six-and-a-half years that were the focus of the investigation, I was paid $17,889.57 more than I was entitled, or 0.5% of $3.5 million billed. I want to be clear that I believe that I should not have received any amount in error— no matter how small. Yet during the investigation we discovered a number of services that I could have billed to DMAS but didn’t. The fact that the total amount of these unbilled services exceeded what I was overpaid by DMAS was not taken into consideration.

n March 6, 2008, I was found guilty on all ten counts after a nine-day trial by a jury. In our motion to set aside the verdict the Judge wrote, “The defendant argues that the evidence showed that he acted in good faith in his dental practice, with no intent to defraud. Similarly, he contends that he acted with a ‘legitimate medical purpose and within the bounds of accepted medical practice.’ I agree that his evidence supported such a defense, but there was substantial evidence to the contrary. A verdict in Dr. Shelburne’s favor certainly would have been plausible and equally supportable. Under our system of justice, however, the jury’s view of the evidence cannot be set aside where, as here, there was substantial evidence to support it.” The seven money laundering counts were dismissed as a result of a Supreme Court decision handed down four days before my sentencing. The racketeering and structuring convictions carried a forfeiture penalty that was determined to be $200,000. In addition to the $17,889.57 restitution, I was given a $75,000 penalty and was sentenced to 24 months in Federal prison with three years of supervised probation to follow release from prison. I voluntarily surrendered my license to the Virginia Board of Dentistry before going to prison. I was advised that a hearing would likely result in revocation. By this point I was completely beaten down and didn’t have the time, money, or energy to mount a fight, so I ran up the white flag. I will reapply for my license in July of 2011 and will throw myself on the mercy of the Board. In the meantime, I’m doing all I can do to remain current to help assure the Board that I am safe to provide dental services. However, my journey is not over. In November of 2009, I was served with a Civil Action while still in prison. The U.S. Government and the State of Virginia have brought a civil suit based on the same course of conduct as in the criminal action. Yes, they can do this, and lightning can strike twice in the same place.
“ ...lightning can strike twice in the same place.” So the looming question is...what “triggered” the investigation? We don’t know for sure. The consensus is that it began as a complaint from either a disgruntled employee or a jealous neighboring dentist—a “whistleblower.” Fortunately for the whistle-blower, his/her identity is protected by federal and state law. It will never be known. Honestly, I’m glad I don’t know who initiated the complaint. It does no good to bear ill will toward anyone. It just poisons your thoughts and feelings to the detriment of your health and attitude. People and situations only have as much power over you as you allow them to have. I’ve already been to prison. I refuse to let anything or anyone affect my mental freedom again. Today I work as a minimum wage dental lab tech while serving the three years of supervised probation. I can
never own a gun again. I can’t vote. I can’t leave the district without written permission from my probation officer. I will always have the stigma of being a convicted felon. I have very little left from a monetary standpoint (I filed for personal bankruptcy this past August), but life is good nonetheless. I am free physically, spiritually, and emotionally. I have many people who love me (family, friends), a roof over my head, and food on the table. Compared to the majority of human beings in this world I am wealthy beyond their wildest dreams. I am grateful to you, my colleagues, for your continued interest and concern for my family and me. We have an amazing profession with an unlimited ability to reach out and help. You didn’t slam the door on me. You opened your arms to me, and I appreciate it. I am determined to move forward, so I now write, speak, and consult in an effort to encourage others to protect and defend themselves and their practices. How You & Your Team Can Help Protect Your Practice. No one ever thinks it will happen to them. Believe me, it happens more often than you think, and often from the person you trusted the most. That’s right – embezzlement! Too often, doctors are shocked because they trusted their teams and did not feel it was necessary to have specific checks and balances in place. It might be argued that dental practices are more susceptible to embezzlement than large businesses. Unlike large business, they do not have the resources to establish sophisticated internal controls. For example, in a dental practice a bookkeeper’s routine duties might require him/her to issue invoices to patients, record the receipt of the patient’s payment and then deposit the amounts in the doctor’s bank account. The lack of other individuals involved in the process provides opportunities for the embezzler. For instance, the bookkeeper/embezzler, without the

knowledge of the doctor, could simply establish a separate bank account, deposit a patient’s check in the account and then cover up the misappropriation by crediting the patient’s account using one or more bogus transactions. If the bookkeeper has access to incoming mail along with the general ledger, it would be easy to perpetuate the fraud with little risk of detection. Given the lack of resources to develop and maintain sophisticated internal controls, how might a dental practice prevent embezzlement? Well, to be honest, it’s virtually impossible to prevent embezzlement. As they say, if there’s a will, there’s a way. On the other hand, every doctor could make it more difficult for the would-be embezzler by the implementation of policies and procedures designed to reduce the opportunities to steal.

“Given the lack of resources to develop and maintain sophisticated internal controls, how might a dental practice prevent embezzlement?”. The answer to overcoming fraud can be simply stated. First, be aware of behavior trends of embezzlers. Second, exercise prevention through daily attention, controlled authority, and systematic access. Patterns of behaviors of fraudulent employees are a challenge to detect as they often disguise themselves as a dedicated, hard-working, ideal employee. They tend to spend time alone in the practice ---- arriving early in the morning, working lunch hours alone, and staying late. They rarely if ever take vacations as they are afraid something will come up while they are away that may raise red flags. They tend to be extremely territorial about their roles in the finances of the practice and offer to take on as many aspects as allowed.

Most common signs to be on the lookout for to keep you from being embezzled.
Employees who may be: • Deteriorating in financial condition.
Unusual Behavior
• Not taking vacation, working after hours & bringing work home. • Excessive spending, large sums of cash or the reverse, chronic borrowing of money. • Openly resenting the doctor’s income or lifestyle, and/or the fees of the practice. The causes • One employee handles all financial transactions meaning no or few checks and balances in place; or the same employee makes the deposits, posts the insurance, pays the bills & balances the checking account. • The doctor isn’t the first one to open and review the bank statement or doesn’t check the account online. • New employees are hired without reference checks. • The doctor has little understanding about the business side of the practice; he or she doesn’t read the front desk reports and doesn’t check the day sheets routinely. How they do it: • Taking cash payments without posting charges or taking the cash payment and deleting the payment while posting some of the treatment. • Changing the deposit slip on the way to the bank and pocketing the cash. • Writing off the account as non-collectable. • The employee doesn’t bill the patient whose payment hasn’t been posted. • Making out refund checks to a fictitious patient. • Giving the doctor a stack of checks to sign & including an extra one or giving the employee the ability to sign checks. • Billing insurance fraudulently—the check is intercepted and deposited in a private account & the doctor’s signature is forged.

The perpetrator will often display unusual behavior that when taken as a whole is a strong indicator of fraud. The fraudster may never take a vacation or call in sick for fear of being caught. He or she may not assign out work even when overloaded. The employee may become very defensive, unusually irritable and suspicious.
Complaints
Frequently, tips or complaints will be received which indicate that a fraudulent action is going on. Complaints have been known to be some of the best sources of fraud and should be taken seriously. Although all too often the motives of the complainant may be suspect, the allegations usually have merit that warrant further investigation.
Stale Items in Reconciliations
This is a big indicator in dental practices! In bank reconciliations, deposits or checks not included in the reconciliation could be indicative of theft. Missing deposits could mean the perpetrator absconded with the funds; missing checks could indicate one made out to a bogus payee. Bounced checks could indicate that funds are being siphoned out of your bank account.
Excessive Voids
Voided patient receipts could mean that the patient paid, the payment diverted to the use of the perpetrator, and the internal copies of the receipt subsequently voided to cover the theft.
Missing Documents
Documents that cannot be located can be a red flag of fraud. Although it is expected that some documents will be misplaced, your CPA should look for explanations as to why the documents are missing, and what steps were taken to locate the requested items.
Excessive Credits
Similar to excessive voids, this technique can be used to cover the theft of cash. A credit to a phony customer is processed, and the cash is taken to make total cash balance.
Common Names and Addresses for Refunds
Insurance refunds are received in error and the perpetrator will make the refund out to a bogus name and the address shown for the refund is then made to the employee’s address, post office box the perpetrator maintains, or to the address of a friend, relative or co-worker.
Increasing Reconciling Items
Stolen deposits, or bogus checks written, are frequently not removed, or covered from the reconciliation. Hence, over a period of time, the reconciling items tend to increase.
General Ledger Out-of-Balance
When funds, inventory, or assets are stolen and not covered by a fictitious entry, the general ledger will be out of balance. An accounting of the inventory or cash is needed to confirm the existence of the missing assets.
Adjustment to Receivables or Payables
Ghost Employees
Ghost employee schemes are frequently uncovered when an individual other than the perpetrator distributes the paychecks. Missing or otherwise unaccounted for employees could indicate the existence of a ghost employee scheme.
Duplicate Payments
In cases where patient payments are misappropriated, adjustments to receivables can be made to cover the shortages. Where payables are adjusted, the perpetrator can use a phony billing scheme to convert cash to his or her own use.
Duplicate payments are sometimes converted to the use of an employee. The employee may notice the duplicate payment; then he or she may prepare a phony endorsement of the check or turn it into cash at the bank.
Write-off of Receivables
Large Payments to Individuals
Slow Collections
Employees being paid for overtime hours not worked by altering time sheets before or after your approval, if required.
Comparing the write-off of receivables by patients or insurance companies may lead to information indicating that the employee has absconded with payments. Only the doctor should have the authority to write off an account. Pay close attention to your receivables. If a patient or insurance company that has always paid within guidelines has a balance/claim that is starting to age, you should inquire.
Excess Purchases
Excess purchases can be used to cover fraud in two ways: A. Fictitious payees are used to convert funds and B. Excessive purchases may indicate a possible payoff to the perpetrator.
Phantom Vendors
Perpetrator establishes one or more phantom vendors to pay for goods or services never ordered. The payment goes to an address controlled by the employee.
Excessive large payments to individuals may indicate instances of fraudulent disbursements.
Employee Overtime
Inventory and Supplies Shortages
Normal shrinkage over a period of time can be computed through historical analysis. Excessive shrinkage could explain a host of fraudulent activity, from embezzlement to theft of inventory or phantom inventory.
Charge Accounts and Credit Cards
The perpetrator makes charges for his or her own benefit to charge accounts and credit cards. When the bill comes in, it is paid in the normal course and the merchandise is used by the employee.

Oral Health
The association between oral health and cardiovascular disease remains inconclusive. The close association between oral health & systemic health raises the question of just how connected the oral cavity is to the body’s most important muscle— the heart. Considering that cardiovascular disease (CVD) is the nation’s No. 1 killer, dental hygienists may play an important role in reducing the risks of this serious health threat. (1)Research has demonstrated a link between periodontal diseases and atherosclerotic disease, heart disease and stroke. (2) The fact that CVD often causes death underscores the need to reduce all risks—including any that might relate to the oral cavity. “Any oral infection can theoretically become a risk indicator for CVD,” observes Michael P. Rethman, DDS, MS, a board certified periodontist, past president of the American Academy of Periodontology and former chair of the American Dental Association’s Council on Scientific Affairs. “Oral microbes and the systemic effects of biochemicals released into the circulation by inflammation have been implicated in aggravating CVD.” Rethman characterizes the connection between CVD and periodontal diseases as an “association” rather than a causal relationship. Oral infections, he says, do not cause CVD for the majority of patients; conversely, eliminating oral infection does not cure CVD. There may be more to this relationship than what research has thus far revealed, however. One study found test subjects with chronic periodontitis exhibited up to a 1.59-fold greater risk of developing coronary heart disease than subjects who did not have periodontal disease. (3)Risk factors attached to these test subjects included diabetes, obesity, alcohol consumption, high blood pressure and smoking.
Heart of the Matter IDENTIFYING PATIENTS AT RISK Scientific study has failed to identify a specific age cohort for patients with periodontitis who are most at risk for CVD. Rethman points out, however, that chronic periodontitis and CVD both tend to manifest during middle age. “Both are diseases of aging. Many individuals with periodontitis were or are smokers, and smoking has been implicated in both periodontitis and CVD, albeit separately,” he notes. The presence of C-reactive proteins (CRPs), which are found in blood and increase in the presence of inflammation anywhere in the body, may also prove helpful in risk assessment. Some research indicates CRPs—associated with chronic periodontitis—may be independent biomarkers capable of predicting cardiovascular events. (4)The literature shows that as baseline levels of high-sensitivity CRPs rise, so does an individual’s risk of a first heart attack or ischemic stroke. While there is research to support an association between periodontal diseases and CVD, a determination of cause-and-effect has yet to be made. Frieda Atherton Pickett, RDH, MS, adjunct associate professor in the graduate division of the Department of Dental Hygiene at Idaho State University, Pocatello, and a prolific oral health researcher, notes that if a causal relationship is established, the implications will be significant. “CVD and periodontal diseases affect large numbers of people, and if maintaining oral health reduces CVD—even if only by a small percentage— this could still benefit a significant number of patients. However, the relationship may be purely coincidental and the associations spurious,” she cautions.

Oral Health measure to improve morbidity and mortality rates for CVD, noting, “The costs of improved oral health seem fairly trivial in the context of even the small likelihood such interventions will lessen or delay the costs associated with worsened CVD.” STAY AHEAD OF THE CURVE In the United States, CVD is responsible for 1 death every 39 seconds. (5)One way dental hygienists can help patients is simply to position good oral hygiene as a benefit to systemic health. “It’s OK to suggest that more aggressive periodontal therapy may improve cardiovascular health,” says Rethman, “but it’s important to explain that improved periodontal health should be sought for its own benefits, and not because optimal oral health is likely to noticeably improve or repair cardiovascular health—despite the possibility that it might.”
The role of the dental professional could shift considerably if a causal link between CVD and oral health is established. Pickett points to a 2008 study that concluded randomized intervention trials would help answer the question of what this new role might look like. That answer may be slow in coming, however, because at present only one pilot study is actively exploring the effect of dental interventions on CVD. “If a group could find funding to study oral care procedures in individuals who have CVD and follow them for many years, the results might give the dental profession guidance to answer this clinical question,” Pickett observes. Although oral infections and inflammation have not been implicated as a cause of CVD, Rethman says there is still good reason to improve oral health as a

This human-derived, mineralized cancellous bone has been manufactured to the highest standards in an FDA registered tissue bank with AATB accreditation. Extraction Socket Preservation: The Time is Now

s dentists, we hear a lot about implants and bone grafting. We hear a lot about sinus lifts and ridge grafting. Extraction socket preservation has not been given the attention it deserves. Unfortunately for our patients, most of us are not grafting extraction sites to preserve the alveolar bone. The following discussion should help to answer some basic questions about socket preservation and hopefully help us all to consider providing this as a service to our patients.
There are 7 important questions to this discussion: 1. What is extraction socket preservation? 2. What is the benefit to the patient? 3. What are the consequences of not grafting? 4. Is it a difficult procedure? 5. Who should be performing this procedure? 6. What is the benefit to the dentist? 7. Is socket preservation the standard of care? In consideration of these questions we should all decide whether or not grafting extraction sites is something we want to make part of our practices. I believe that anyone who is extracting teeth is capable and should be doing extraction socket preservation. It is good for our patients and good for the bottom line in our practices. 52

1 What is extraction socket preservation?
The process begins with atraumatic tooth extraction. Every attempt is made to preserve the surrounding bone and soft tissue, with an emphasis on being careful not to fracture the delicate buccal plate. There are a number of techniques and instruments that aid in this process. In general, one never wants to elevate so that force is directed toward the buccal plate. Once the tooth is extracted, all the granulation tissue is removed from the socket. It is important that good bleeding is established in the socket. Next, a bone graft material is placed into the socket. There are a number of different types of graft materials. I prefer an allograft (human) bone graft over a xenograft (other species) or a synthetic bone graft. There are different types of allografts. There are mineralized and demineralized bone, cortical and cancellous bone, bone putties and bone sponges. Generally speaking, mineralized cancellous bone is appropriate for most socket preservation cases. There are some new bone sponges and putties that have unique applications and will facilitate difficult clinical situations. The ideal bone graft maintains the space of the extraction socket during healing while acting as a scaffold for new bone formation. Allografts have been shown to minimize ridge width and height loss compared to not grafting. (1)Allografts
Regenerative Dentistry will remodel more completely than xenografts over time. (2, 3, 4)I have used all three types of graft materials over the years. In my experience, the allografts have the most complete turnover and for me, are most predictable. After the graft material is placed in the socket, it is then covered with a resorbable or non-resorbable membrane and sutured. Primary flap closure is not ideal in these cases. The graft will support the soft tissue during healing. (5)If one advances the flap to achieve primary closure, the blood supply to the buccal bone is disrupted and will result in more buccal remodeling and subsequent bone loss. (6)In addition, the soft tissue architecture, the amount of keratinized tissue, and the vestibule are adversely affected. There is also more chance of post-operative swelling and potential hematoma during healing. (7)For these reasons we use a membrane or covering without advancing the flap or attempting primary closure. The healing is generally uneventful, and the results are predictably excellent. 2 What is the benefit to the patient?
Below is an example of an esthetic dilemma that is the result of the failure to place a bone graft at the time of tooth removal.
3 What can be the consequences of not
grafting?
The loss of ridge width and height following tooth removal is well documented in the literature. (7, 8, 9, 10) Schropp showed a ridge width loss of 50% over a 12-month period with two thirds of the loss occurring in the first three months following tooth removal. The corresponding dimensional change was between 5 to 7 mm. (11)Therefore, without grafting, there is a higher likelihood that a ridge lap type restoration or a “long pontic” would be necessary to fill in the space of the resorbed socket and ridge. We do not have to accept these compromised esthetic results. We have all dealt, prosthetically, with the effects of a ridge collapse. The surgery to correct these defects is many times more difficult, expensive and less predictable than grafting the socket initially. These defects are preventable for the most part.(7,8,9)
Most importantly, socket preservation helps to maintain the alveolar architecture. Socket preservation significantly reduces the loss of ridge width and height following tooth removal. (1, 5, 7, 8, 9) The result is that more costly secondary augmentation procedures are generally not necessary. (7)When implants are placed, the site will be more ideal. If bone augmentation is required during the implant placement, then it will be a simpler procedure. Furthermore, minimizing The following is an example of a socket graft procedure: post extraction bone loss will also result in better esthetics whether the area will be restored with an implant or a traditional fixed bridge. Socket preservation will allow for a pontic with a more esthetic emergence profile or an implant crown that will mimic natural dentition.

Absolutely not! It is much more difficult to do a good composite restoration than it is to graft a socket. 5 Who should be performing this procedure?
Three-month reentry photo demonstrates excellent bone formation. A new cortical plate has formed with the absence of residual graft particles. Note the thick band of keratinized tissue, important for implant esthetics and function that has formed with the accellular dermis membrane.
Every practicing dentist is capable of socket preservation procedures. If you are comfortable extracting a tooth, you 100% can and should graft the socket. Every dentist owes it to their patients to offer this service. A bone graft, mineralized, particulate bone allograft has been placed. The dermis membrane has been placed over the graft and the just under the buccal and lingual flaps. The flaps are not advanced (no primary closure). Excellent osseous and soft tissue healing was achieved.

6 What is the benefit to the dentist?
You are providing the best possible care to your patients. The result will be fewer restorative challenges and better functional and esthetic results. It is what we all strive to achieve. You are learning a new and interesting procedure, which is something we all like to do. Socket grafting will also add significantly to your income. Â
7 Is socket preservation the standard of care?
As of now, each practitioner must answer this question for him or herself. Based on the points in section one, If it were you, would you want a ridge preservation procedure performed at the time of tooth removal? In my practice, it is the standard of care to offer socket preservation and to explain the potential consequences of not grafting. If someone chooses not to graft an extraction site, I am careful to clearly document it in his chart. It is only a matter time that it will become the standard of care. In the current Dental Economics, Gordon Christensen stated that he felt it should be the standard of care. He also stated that it would add about $500 to the procedure.(13)
At five weeks post-operatively, the site has healed uneventfully. The graft material has supported the soft tissue growth over the site. Note how the mucogingival position has been maintained
