When it comes to managing expenses in your practice, there are dozens of categories to evaluate: equipment, technology, loan costs and interest rates, sundries, marketing, and on and on they go.

Many practice owners are quick to shop-out what they believe are the most obvious expenses, but few understand the impact of one of the largest expenses and how it can be dramatically reduced to increase profitability. The highest expense for most practices is payroll, followed by real estate. Real estate encompasses your monthly rent or mortgage payments, along with the property’s operating expenses, maintenance fees, utilities, and janitorial costs.

If you consider these top two expenses, payroll and real estate, only one of them is really negotiable. With payroll, you can either pay people their value or they usually find another job that will.

You may decide that you can cut staff, but if you need people you need to pay them what they deserve or they will eventually leave.

Real estate however, is 100% negotiable. You have the choice of leasing or owning, as well as being in an office building, retail center, a stand-alone building, or large medical complex with many other providers. You can choose the size of your space, the design, and the landlord you want to work with—or to be your own landlord. And if you do own, you get to decide whether to buy an existing building, an office condo, or to develop your own building from the ground-up.

When negotiating the economic terms of a lease, you get to have a say in the length of your lease, the desired concessions including build out period, tenant improvement allowance, free rent, lease rates, annual rate increases and many other provisions.

With this many choices to evaluate and understanding that each one affects the final economic outcome, why is it that so many practices fail to capitalize on their real estate opportunities? The short answer is that most practice owners and administrators simply don’t have the knowledge and expertise in commercial real estate to understand how to make the most of these opportunities. They view real estate as a necessary evil instead of an incredible opportunity to improve profitability, reduce expenses and improve the quality of their patients’ experience. When the correct approach is taken, you may actually look forward to it instead of dreading your real estate negotiation.

Featured Article Let’s take a look at three key ideas that will help you make the most of your next real estate transaction.

1. Timing Every type of transaction has an ideal timeframe to start the process. When starting too early or too late, you communicate to the landlord or seller that you don’t really know what you’re doing. When that message is communicated, it hurts your ability to receive the best possible terms. For example, don’t wait for your landlord to approach you on a lease renewal negotiation. Start by consulting with a professional so you can understand the ideal timeframe to start your transaction, come up with a specific game plan for what you want to achieve, and then you be the one to approach your landlord with renewal terms.

2. Representation Landlords and sellers prey on unrepresented tenants who don’t really know the market or what their options are. If the tenant was a Fortune 500 company, the landlord would approach them with a high level of respect, expecting that they either have a real estate broker hired to represent them or have a team of professionals internally that are well equipped to handle the transaction.

In contrast, when a landlord or seller starts speaking with a tenant who isn’t represented, and who they don’t believe knows the market as well as they do, that tenant is not going to get the same level of respect through the process. This is because the landlord senses an opportunity to take advantage of a small tenant who is not an expert, doesn’t have a full complement of real estate knowledge and skills, and who doesn’t have adequate representation.

When you understand that commissions are paid in commercial real estate just like they are in residential real estate—they are set aside in advance for two parties, not just one—then you

understand there aren’t any savings by not having a broker. And if there aren’t any savings by not having a broker, then showing up without one only further detracts from your credibility.

3. Leverage and Posture It is nearly impossible to emerge victorious from a negotiation without leverage and posture which are created by having multiple options in the market. If you limit yourself to one property, you are at the mercy of that owner. Since most landlords and sellers negotiate professionally, it is easy for them to know when you don’t have other viable options.

Simply telling a landlord that you have a proposal from another landlord won’t give you a strong enough posture. Most landlords look at unrepresented tenants and assume they do not know the market, do not understand all their options, and are not really serious about making the landlord compete for their business.

Leverage and posture are created when you have the right timing, professional representation, an understanding of all your available options, and a detailed game plan of what you want to accomplish in order to capitalize on the market.

These three key ideas are the first of many factors that allow healthcare tenants and buyers to reduce their second highest expense which dramatically impacts profitability and cash flow.

Take the First Step: Create a Website for your practice

A first-class practice website should serve as the foundation for all online marketing efforts for your practice, functioning as an informational hub for your current and prospective patients. There’s a wide availability of user-friendly platforms, it’s easy to get a modern website launched and up and running quickly.

For years dentists have largely relied on word-of-mouth and traditional forms of advertising, such as the newspaper, direct mail, or radio ads, to attract new patients to their practice. Today we find those days are gone. The Internet now plays an important role in a patient’s decision-making and ensures that failure to market your practice online will place your practice at a significant disadvantage.

Only 19% rely on those same online reviews to validate their selection prior to making an appointment. It’s clear that dentists have to be where their patients are, and that is on the Internet.

The quickly changing dynamics of the Internet and patient demographics have forced dentists to view their practices, and how they attract patients, differently from the way they have in the past. Dentists are slowly learning to assume marketing strategies that are similar to other industries, in which online marketing efforts are integrated with traditional advertising methods.

A recent study by the Pew Research Center’s Internet and American Life Project indicated that 72% of Internet users looked online for health related information,

1. Create a dynamic, optimized practice website.

2. Adopt strategies so you can be found by patients online.

3. Take advantage of social media.

4. Pay attention to your online reputation.

Visitors to your site should be able to easily locate basic information, including your name, the address for the office, and most importantly, your phone number. This information should be listed on every page of your website. The hours of operation and a biography page for each dentist is also recommended. Most importantly, you should include a listing of services offered, including a specific section that highlights patients you have treated with before and after photos.

Keep your site simple, keep it succinct, and keep it interesting, so that it will motivate patients to give you a call.

Google released a study in 2015 showing that in 10 countries, including the United States, Internet searches made on a mobile device now surpass those made on a desktop computer. Therefore, it’s imperative that your practice website have a responsive design that accommodates both mobile and desktop devices. Because patients are on the go and often try to find key information in transit via a smart phone or tablet, not having a mobile friendly design will limit the ability to access information quickly and prevent a potential patient from reaching your practice.

A well-designed practice website includes online booking and bill pay, allowing patients to request appointments at their convenience.

However, it’s critical that requests are responded to in a timely manner; the same business day for example. A delay in contacting the prospective new patient may result in them choosing another provider who will meet their needs more quickly.

Featured Article The ability to make appointments online has become an increasingly popular feature and can often be the deciding factor for patients. According to a 2015 Salesforce report, 74% of Millennials based their decision on the ability to pay bills or book appointments online at their convenience.

Another easy way to enhance the patient’s online experience is to offer digital forms, such as new-patient registration forms and health history forms. This will allow the patient to complete the forms prior to their appointment instead of rushing to fill them out in the reception room of your office.

Make sure your website is mobile friendly.

If your website isn’t readable on a mobile device you’ll lose patients. Search engines, especially Google, don’t like websites where readers have to pinch, zoom, and contort their fingers to get to the link they’re after.

Furthermore, given that more than half of all searches are performed on mobile devices, Google has decided that if your website isn’t optimized for mobile use you’ll drop down in rankings.

Let your personality show.

Patients are not going to form a relationship with your theme or choice of font; however, they will connect with a human face.

Include photos of yourself, your staff, and happy smiling patients, to inject personality into your practice. My recommendation is to avoid using stock photos and to only use photos of actual patients.

Don’t forget about your written communication. Make it user friendly as well. Explain what you do in layman’s terms, avoiding any jargon. Always keep your patient in mind with every manner of communication; never forget the site is really for them.

Step Two: Get Found Online Once your site is launched, the next goal is to drive traffic to the site. Before you can

get patients to your site decide what services most often searched for by patients to take and procedures you want to promote. A short-tail keyword represents a broader search term, while a long-tail keyword is more specific. For example, the short-tail keyword “dentist” is a very broad topic that will have significant online competition. It would be difficult to rank highly in search engine results. However, a long-tail keyword like “dentist in Atlanta that specializes in adults with gum disease” will have less competition and increases the odds of ranking highly in search results. It is critical to incorporate behaviors to make it easier to target the long-tail keywords into both PPC campaigns correct individuals. and with the practice’s website content.

Once a dentist has determined their focus, paid local search engine campaigns can be an effective way to reach patients looking for a new dentist. By defining specific parameters, such as keywords, ZIP Codes, or age brackets, Pay-Per-Click (PPC) advertising allows users to target their intended audience.

These parameters can be used to create an ad as well as determine when and where it’ll show up in a search result. You can also set a maximum advertising budget to offer more control over how much this will ultimately cost.

Search engine optimization (SEO) can supplement paid advertising. Most commonly referred to as an organic search, these are the results that are driven by the content on your website. According to a BrightEdge study; it revealed organic searches result in 51% of all website traffic and it’s not surprising because this type of advertising is generally more trusted by online users.

Search engines use an algorithm to determine how results are ranked. They take into consideration things like the usability and quality of the site in addition to the number of directories, like Yelp or Angie’s list, where the practice is listed. The new direction of the Internet is based on content marketing that uses keywords and phrases that are readily used by search engines to find the proper websites. Dentists need to use the terms

SEO can also be improved by listing your practice on relevant directories. For your practice this could include ZocDoc, Healthgrades, or Craig’s List. Online directories tend to rank higher organically.

If a patient conducts a search for a specific dentist, the dentist’s directory profile will likely show up in a key location such as the first page of the searcher’s results. You’ll find that there are dozens of directories and claiming a profile on just a few is sufficient.

A study by Advanced Web Ranking showed that on average 71.3% of searches result in an organic click when it is listed on the first page, while pages two and three received only 5.59% of clicks. Creating directory profiles such as the sites listed above improves SEO, making it easy for the patient to find and research your office, as well as initiate contact.

Regularly updated content, in the form of blogs, pictures, video, and patient education will provide another channel that funnels patients to your site. Consistently adding well written, unique content to the website enhances SEO. Since most patients aren’t necessarily searching for a specific dentist but are looking for an answer to their dental problem, creating blogs or pages that contain information on dental conditions and treatments is an excellent way to increase organic reach.

Featured Article Blogs also have the ability to establish you as a thought leader and enhance your credibility. I recall a client that I worked with in Texas who developed a significant following simply by writing a series of blogs about implants. This dentist then experienced an increase in new patients seeking care for implants. Blogging is a great way to inform existing and potential patients about the key services you offer or to provide a venue to educate patients on overall dental care.

Website Check-up?

If you don’t have any concrete evidence that your site is getting any attention, it’s time to do a checkup to determine where it’s not performing and what should be done to increase your traffic and potential to attract new patients.

Patients and search engines want to know where you are.

If your practice’s name, hours, address, or phone numbers are listed differently on different sites, your SEO power will become diluted with this conflicting information. It also doesn’t make it easy for potential patients to determine where you’re located, if your practice is still open, or your phone number even still works.

The best way to determine if there is any discrepancy regarding your contact information would be to utilize a site that collects data from public records. Here is a list of sites that can help you tidy up your information to be consistent. You’ll do yourself a favor with patients and help your search rankings too.

This will benefit both the search engine and the reader. The search engine wants to see all of the descriptive keywords that establish you as an expert in your field, which will in turn give you a better search ranking. The reader wants to find out more about what you do, how you do it, and what to expect. Make sure your service descriptions are listed on separate pages.

This will help the search engine know exactly what the page is about. If a page describes four different procedures it’ll make you look like a jack of all trades as far as the search engine is concerned. A quick tip to describing your services is to use a “question and answer” format. Both the search engine and readers prefer this, as it’s the most natural form of searching, i. e. “How much does a root canal cost?”

8 Tips for GREAT Website Design 1. Prominently display happy, smiling patients on the home page.

2. Determine keywords to optimize within your site.

3. Focus on specific services you want to provide.

4. Blog, Blog, Blog.

5. Turn off music, sound effects.

6. Make forms easy to use and short as possible.

7. Optimize for mobile devices.

8. Refresh every 18-24 months.

Along with checking discrepancies in contact information you’ll also want to claim your Google Business listing. It’s free to do this, easy to set up and will provide a handy list of your contact details, description of your services, business hours, and even a map to find your office.

Search engines can’t see your pictures.

Be sure your images have keyword related titles, captions, and tags. Tags are like embedded descriptions; they aren’t visible on your site, but they help the search engine understand what the picture contains.

What is the return on investment or how do I justify this amount of capital?

When you purchase a cone beam, I suggest that you look at the ability of the machine to generate images in different formats. You need to ask yourself, besides taking a threedimensional image, can this machine create panoramic images? If it can take panoramic images, ask yourself, “How many panoramic images do I take in a year?” Then ask yourself, “What do I charge for those images?”

For example, if I charge $100 and I take 400 panoramic images a year, then I just billed out $40,000. If this machine cost you $120,000, dollar for dollar, you recoup the expenditure in three years. Realistically, you pay for it over four years.

Remember, it’s just not about the X-rays. It’s about the amount of undiagnosed dentistry and implant planning we can do with this technology. It generates better comprehensive treatment planning and ultimately better dentistry for the patient.

How has communication improved with your patients using CBCT? Patients are blown away when they see a three-dimensional image of their jaw. When I can show patients where I can place an implant in their jaw using their own anatomy and a chairside model, patients can visualize and comprehend the procedure more clearly.

I would encourage the use of videos that all the major dental implant companies produce. Check with their marketing department and incorporate a video into your website. While the patient is in the chair, you or your team can pull it up on your website for the patient to view. This serves two purposes.

First, patients can visualize it more clearly. Second, when they go home to their significant other, they can pull up your website and explain the procedure to them.

The more we review and educate ourselves on the normal anatomy, the better we become at identifying the obscure. I also encourage taking classes that are continually being offered throughout the country on how to review a cone beam.

Do I need an intraoral scanner and CBCT? Having an intraoral scanner and a cone beam is the perfect marriage.

Both are completely independent of each other. The scanner focuses on teeth and tissue and the cone beam

addresses the bony architecture.

When the STL files are joined in the lab, there is complete harmony. Like a great marriage, they make each other better.

From my understanding and speaking with different vendors, one can take a DICOM file obtained from the CBCT, convert it, make a virtual model, 3D printed model and fabricate a crown. So, in theory, you could prep a tooth and have an edentulous area. Plan the implant and make the crown or partial all at the same time. And then there are times we just need to take an intraoral scan and make a crown inlay or onlay.

What’s your opinion on the obsolescence of a machine you purchase today?

The most important question to ask the equipment sale rep is, “Is the hardware upgradeable?” Also, is there a fee for upgrading to a newer version of the software?

Many companies have software version updates so you should ask what is included with your purchase.

Like any other technology, there are advances in how it’s delivered. Going with a company that’s one and done is not the way to go. I want to do business with a company (and there are many out there) that want a longterm relationship with the buyer.
