scar winner Matthew McConaughey dropped close to 50 pounds, stayed out the sun and ate only fish and vegetables for four months to prepare for his role as Ron Woodroof, a real-life rodeo cowboy and electrician who after being diagnosed with AIDS in 1985 taught himself about drug treatments and eventually began smuggling in experimental drugs from outside the U.S. He sought these solutions elsewhere because his immune system responded negatively to the FDA pre-approved drug, Azidothymidine

(AZT), which was used to delay the onset of AIDS. If the transformation in McConaughey was dramatic, his success may have been even more dramatic— McConaughey nabbed a Golden Globe Award for best actor in the film and, basically from the moment it first screened, has been considered a front-runner for the best actor Oscar (he received his first Academy Award nomination for the role in January of this year).

The Power of HIV

The First Years

Dr. David Wright from Blackstock Family Health Center has been treating patients with HIV since the beginning. â&#x20AC;&#x153;Fear and desperationâ&#x20AC;&#x201D; thatâ&#x20AC;&#x2122;s what people lived in,â&#x20AC;? he remembers.

Over the past three decades, we have learned a tremendous amount about HIV. But after all these years, are we any closer to a cure?

Have we been able to reduce the spread of HIV? And, have our old misconceptions and biases simply been replaced with new ones?

The term AIDS (Acquired Immunodeficiency Syndrome) was officially introduced in 1982, but it would be a few more years befire the virus was officially named: HIV (Human Immunodeficiency Virus).

The Game Changer

The new century brought us new technologies and drug formulations. Doctors could select drugs and combinations based on the strain of the virus, and the combinations reduced the burden on the patient. Unlike the high doses of AZT, the new lower-dose combination of medications allowed people to live healthier, more active lives.

Of all the viruses and diseases we have had to face over the centuries, perhaps none have baffled us, challenged us and strengthened us as much as HIV and AIDS. When AIDS first appeared as a mysterious illness in a handful of homosexual men in the early 80s, it was feared (people with HIV were shunned) and organizations that tried to help were picketed and harassed. Now, HIV has all but vanished from the public eye as new medications help people diagnosed with HIV live normal, healthy lives.

In 1981, a cluster of homosexual men came down with rare forms of pneumonia and skin cancer. It was soon discovered that these â&#x20AC;&#x153;opportunistic infectionsâ&#x20AC;?â&#x20AC;&#x201D;illnesses that take advantage of a weak immune systemâ&#x20AC;&#x201D;were part of a mysterious underlying disease that affected only certain populations.

As more people contracted HIV, communities took notice and came together. In cities across the nation, health providers, counselors and ZSPYRXIIVW VIEPM^IH XLEX ½KLXMRK ,-: ERH %-(7 VIUYMVIH E HIHMGEXIH effort. The Fighting Chance

As scientists searched for an HIV treatment, interest was renewed in an antiretroviral drug developed in the 1960s called Azidothymidine (AZT), also called zidovudine, which is a drug that is used to delay the development of AIDS in patients who are infected with HIV. After promising trials, the drug was approved for use in record time and the effects were clear: No longer were people dying from opportunistic infections.

However, the cost and side effects of the drug were severe enough that it barely seemed better than the disease. â&#x20AC;&#x153;We saw anemia, neuropathy, wasting away,â&#x20AC;? Dr.Wright explains.â&#x20AC;&#x153;When other people started to see that, especially people who werenâ&#x20AC;&#x2122;t sick yet, they didnâ&#x20AC;&#x2122;t want to take the drug.â&#x20AC;?

Over the next few years, the FDA approved a steady stream of other drugs designed to combat opportunistic infections.They also approved using AZT to prevent transmission from mother to child. But the treatments were limited. â&#x20AC;&#x153;We got pretty good at treating opportunistic infections, but we HMHR´X HS E KSSH NSF EX MQTEGXMRK ,-: MXWIPJ %X ½VWX %>8 [SYPH [SVO FYX after several months, the virus would become resistant,â&#x20AC;? says Dr. Wright.

To many people who study HIV, it seems the epidemic was a perfect storm of different issues colliding. The virus itself has unique properties, making it hard to combat. It hides in our immune systems, lying dormant for years. HIV also replicates quickly and mutates easily, making effective treatments harder XS ½RH 8S XLMW HE] XLIWI TVSTIVXMIW EPWS QEOI HIZIPSTMRK E ZEGGMRI RIEVP] impossible.

In addition, because HIV is spread primarily through sexual contact, it has been an uphill battle to educate people. In many cultures around the world, including the United States, talking about sex can be uncomfortable or taboo.

Since HIV disproportionately affects homosexual men and other already stigmatized populationsâ&#x20AC;&#x201D;such as intravenous drug users or people in the prison systemâ&#x20AC;&#x201D;negative opinions about HIV-positive people continue to exist.

While our current treatments offer hope to millions of people, they can be a double-edged sword. As the visibility of HIV decreases in the public eye, fewer peopleâ&#x20AC;&#x201D;especially in the younger generationsâ&#x20AC;&#x201D;remember the danger it still poses. Over the past few years, nearly 40 percent of new infections occurred in people under the age of 29. And an estimated 60 percent of people infected with HIV do not know their status.

The New Struggle

While older stigmas about how one can contract HIV may have been cleared up, doctors and HIV prevention specialists now face a new set of misconceptions, primarily that HIV can be cured with just a pill. â&#x20AC;&#x153;The biggest issue that we wrestle with now is that people have to be treated for a lifetime,â&#x20AC;? Dr. Wright explains. â&#x20AC;&#x153;Itâ&#x20AC;&#x2122;s understanding that you are going to be on medications for an MRHI½RMXI TIVMSH SJ XMQI ERH MJ ]SY WXST XLI QIHMGEXMSRW EX WSQI TSMRX XLI virus is going to come back.â&#x20AC;?

In addition to new drugs for treating HIV, government and RSR TVS½X EKIRGMIW WXEVXIH XS JSGYW SR TVIZIRXMSR ERH education strategies. In 2010, the Obama administration released the National HIV/AIDS Strategy, which proposed a new goal: to create an AIDS-free generation. HIV and AIDS organizations are now working on three fronts to ½KLX XLI ITMHIQMG 1) Get people tested to know their status and get into care 2) Keep people in care to reduce their viral load and reduce the chance of spreading it to others 3) Work with both HIV-positive and HIV-negative people to encourage safer behaviors

Now that people are living longer, there are other unanswered questions that current research hopes to address: ¡ Does being positive reduce life expectancy, even if medications have lowered the viral load? ø -W XLIVI E [E] XS žYWL EPP SJ XLI ZMVYW SYX ERH WXST medications? ¡ How can we give people an infusion of medication that is long-acting rather than a daily pill? ¡ How do we make care more accessible, more open and less threatening? ¡ How do we now make medications more affordable?

Just like in the 80s, it will require a concerted effort on the part of healthcare specialists, government agencies, drug manufacturers and community supporters. Dr.Wright is optimistic about the future. â&#x20AC;&#x153;We [MPP TVSFEFP] ½RH E [E] XS GYVI XLMW MPPRIWW ²

oday, most exercises and sports, even those in the Olympic Games, are focused mainly on developing muscular strength and cardiovascular stamina. However, the human body is not only composed of the muscular and cardiovascular systems, but it also includes the immune, endocrine and lymphatic systems, among many others. However, these systems are not addressed by most approaches to exercise and fitness. As a result, the current fitness industry appears to fall short of serving the true health needs of people today.

Furthermore, although regular exercise is thought to be highly effective for the prevention and treatment of many common chronic diseases and improves cardiovascular health, there is increasing evidence suggesting that excessive exercise can have a negative impact on the body and longevity. Studies have shown that acute, high-intensity exercise causes short-term damage to cells’ mitochondria and short-term muscle damage in the body, according to the Clinical Journal of Sports Medicine.

Additionally, according to a review of the literature published in the June 2012 issue of Mayo Clinic Proceedings, long-term excessive endurance exercise may induce pathologic structural remodeling of the heart and large arteries. Emerging data suggest that chronic training for and competing in extreme endurance events, such as marathons, ultra marathons, ironman distance triathlons and long distance bicycle races, may cause structural changes to the heart and large arteries, leading to myocardial injury.

Cancer, heart disease, strokes and respiratory problems are among the leading causes of death in both the U.S. and most of the industrialized world. Our health today has been greatly affected by the chemicals, hormones and pesticides in our food and environment, often resulting in high levels of toxicity within our bodies. These contaminants, paired with more people working behind a desk with more stress and less exercise, have resulted in an increased incidence in modern diseases. Where do we go from here? Fitness needs a revolution!

The Mind Is Key to the Fitness Revolution

The mind is a powerful tool for health. If we know how to use the mind properly, and combine it with effective exercises, I believe we can bring about significant, long-term changes in our health. We can understand more about harnessing the potential of the mind, and our potential to create the health we desire, through the theory of “Intended Evolution.”

This is a new theory that I have been developing for some years now. According to this theory, the evolution of life is driven by two main factors: first, natural selection and outside pressures, and second, "Intention” and internal drive.

For a very simple example, let us use two children who are brought to a swimming coach for lessons. The first child, John, loves to swim and wants to be a swimming champion later on in life. The second child, Ben, dislikes swimming and is only there because his parents brought him. Both John and Ben are exposed to the external influences that will make them good swimmers, but John is much more likely to succeed because of his internal drive. He really wants to become a swimming champion. Only when the external and internal factors are working together can true change happen.

HEALTH In a similar manner, the mind can play a key role in changing our health.

Most of us know that the mind can affect our hormone levels, causing physical and biological changes in the body. For instance, negative thinking can increase stress, giving rise to the adage "don’t worry yourself sick,” while positive thinking can improve our functioning and strengthen the immune system. When we improve functioning, we increase our chances of survival and longevity. By using our minds in focused, positive ways, we are following the principle of natural selection, which is “the survival of organisms best adapted to their environment.”

We can apply the theory of Intended Evolution to improve our health and fitness. For example, if every day we focus on the idea that the natural human lifespan is 120 years, or even 150 years, we can help ourselves immediately feel younger. When we create this feeling, I believe our hormones respond accordingly, allowing us to become happier and healthier. If we believe we can live only 80 years or so, then, when we are 50 or 60 years old, we begin to feel our life is close to its end. However, if we focus on the belief that the natural life span extends to 120 or even 150 years, then by the time we reach age 50, we will still think of ourselves as young.

The more clear, focused and powerful our intentions are, the more our minds can help us balance our endocrine systems and metabolisms . . . they may even allow us to affect our DNA. This revolution in fitness can result in a long and vital life far beyond what we once thought possible!

Immunotherapy

An allergy extract individually prepared for you Special Contribution to Austin MD by Dr. Amin Mery

Immunotherapy (IT) is a series of injections, or shots, in which you are given a small, concentrated dose of a protein mixture, containing extracts that you are allergic to. IT causes changes in the immune system, so that when your body is exposed to the allergen (what you are allergic to), you have few or no symptoms. Your extract is individually prescribed and contains protein from the positive allergens on your skin. Pollen and environmental extracts are diluted in saline. Venom extracts for stinging insects are mixed with small amounts of human albumin, a blood coagulating protein.

When you first start IT, you should receive at least one shot a week. You begin at a very low concentration and gradually build up to the highest concentration, or your “maintenance” strength. When you receive the lower concentrations, you may receive an

The length of time on IT varies from patient to patient. Typically, the minimum course is three to five years. Some patients will have to remain on IT longer if they wait too long in between sessions. If you miss your shot period of time (one to two weeks), you will

be given the same dose you received at your previous visit; however, this may lengthen the necessary time to reach your maintenance dose. If you miss an extended amount of time, the dose may be reduced, resulting in additional charges.

Improvements will vary from patient to patient.

As a general rule, you will not begin to notice significant improvements until you have reached your maintenance strength for several months. Most patients will reach their maximum benefit after they have been “maintaining” for a year to a year and a half. You should continue the allergy medications your doctor has prescribed until advised to reduce or discontinue them. This combination of treatments will provide not only the best results, but also a decrease in your symptoms.

The key difference between a person taking steroid shots versus allergy shots is in the allergen protein extracts. Your allergen extract is individually prepared for you in our antigen preparation lab. The steroid shot only temporarily improves allergy symptoms, while IT with allergen extract has long-term benefits.Your extract expires one year from the time it is prepared, so it is best to start your shots as soon as possible. We will contact you when your extract arrives in the office.

You cannot start IT if you are pregnant.

Please notify our office as soon as you know you are pregnant so that we can make necessary accommodations for your concentrated dose.

The immediate side effects can range from local inflammation such as: swelling, redness and itching at the injection site, to the typical allergy symptoms of itchy, watery eyes and running nose.

More severe reactions may include asthma (shortness of breath), difficulty swallowing or hives.

If you are taking beta-blocker medications (usually for high blood pressure, heart conditions, glaucoma or migraine headaches) prescribed by another doctor, you should not receive an allergy shot.

Rarely do patients have a severe reaction with reduced blood pressure.

Studies have shown that patients who have the most severe reactions begin to experience symptoms within 20 to 30 minutes after receiving an injection.

This is why we require you wait 20 minutes after an injection.

Some high-risk patients, including those on higher dosages, will be asked to wait 30 minutes.

If you develop an adverse reaction at the injection site with only increased swelling or redness, you may apply ice and take an antihistamine if the reaction is bothersome.

This should be reported to the nurses prior to your next injection, as your dose may need to be adjusted.

These reactions can occur regardless of how long you have been on IT. If the reaction involves an increase in allergy symptoms, such as hives or shortness of breath, call the office immediately.

If you are experiencing severe shortness of breath, immediately go to the nearest Emergency Department or call 911.

If you are in severe distress, do not attempt to drive yourself to the Emergency Department, instead call 911.

Principle 2: Low IQ is not what we need to look at in terms of efficiency. What we need to concern ourselves with is what I like to call low IfQ, or “interference quotient,” as that is what truly determines learning, behavior and health.

Our IfQ can be blown out of proportion when our reptilian brain (our basic, survival brain) hijacks the higher logical centers of the brain and leaves us “out of control.” If you have minimal roadblocks, your IfQ will be low, which will allow your intelligence and performance to reveal your true potential.

Principle 3: Inherited anatomical patterns, such as the shape and placement of the skull, eye sockets, jaw and eye fascia, as well as trauma to the whole body’s anatomy, can interrupt learning and performance channels.

In my practice, these roadblocks are addressed with gentle, hands-on repeated stimulation, until a new neural signature takes hold and replaces the less efficient system.

Steve’s family was in a dilemma. One parent and one sibling wanted him committed to a mental facility because he seemed schizophrenic and potentially violent. The other parent was searching for other solutions.

Steve was 27 at the time, but since the age of 16, his life had gone downhill. No one knew why.

As it turns out, Steve played soccer for years, and by the age of 15 his father took him to the emergency ward at least five times. When I heard that piece of the puzzle, I agreed to work with Steve; not on the possible psychiatric issues, but simply on the head injuries.

Research verifies that high levels of cortisol (a stress hormone) and lower levels of serotonin (often associated with depression) often follow concussions. If I could help release the trauma from those multiple injuries, it might be possible to normalize the cortisol and serotonin levels and help Steve regain his pre-injury way of being.

That is exactly what happened. Within days of our sessions, Steve’s attitude changed. He began thinking more positively. Within two months, he had a new job and a new outlook on life. His father called me—three times at Thanksgiving and three times at Christmas—to thank me for giving him his son back and for turning his son’s life around.

How did I do it? Over the past 30 years, I have learned a lot about the brain and the nervous system. More importantly, I have learned how to help people become free of the obstacles that result from a tangled up, incoherent nervous system. For many years, I focused on helping children and adults with dyslexia and ADHD, as all learning and behavioral issues have a neurological component. The same principles apply in all areas of life.

luten is a natural protein in several different grains, including wheat, barley, rye, triticale, spelt, kamut, farro and durum. Several foods do not inherently contain gluten, but are often contaminated by gluten during the growing process, including oats.

It is not the gluten that is terrible, rather it is the wheat The current type of wheat that we eat was hybridized into a non-hulled dwarf variety to make it easy to cultivate and grow.

PRP is an autologous preparation of platelets in concentrated plasma. PRP is being widely applied in various medical fields, including dermatology, for its ability to stimulate wound healing. Further, it has been used clinically in mesotherapy for skin rejuvenation. Although PRP injection is considered one of the minimally invasive procedures, it usually causes skin bruises, which will result in longer downtime when compared with other minimally invasive modalities. As PRP secretes a variety of growth factors in order to regenerate the skin, it may be hypothesized that PRP promotes new collagen synthesis as well as other ECM (extracellular matrix) components through activation of fibroblasts. Up to this point, there is no evidently clear method for the clinical application of PRP. Different methods that are being tested include topical application or direct injection into the skin.

A

s hospitalists at WellMed – physicians who focus on providing care for patients in the hospital – we are all too aware of the challenges facing the healthcare system in curbing readmission rates for patients, particularly Medicare patients. The federal government claims some $26 billion is spent on readmissions for Medicare patients, and more than $17 billion pays for return trips that could be avoided if patients are given the right care the first time around.

The issue is critical for healthcare providers because the Centers for Medicare & Medicaid Services (CMS) is financially penalizing hospitals that fail to get a handle on high rates of hospital readmissions – typically defined as patients returning to the hospital within 30 days of being discharged.

The issue is critical for Medicare patients because of the emotional, physical and financial toll that hospitalization often places on them and their families. In 2010, the 30-day hospital readmission rate for Medicare patients in Texas and Travis County hovered just above 15 percent, according to “The Revolving Door: A Report on U.S. Hospital Readmissions.” Translated, that means a little more than one in seven patients discharged from the hospital were back within 30 days.

The report, released last year by The Robert Wood Johnson Foundation, provides a common-sense (and evidence-based) list of reasons for why this is happening: differences in patient health status; the quality of inpatient care, discharge planning and care coordination; the availability and effectiveness of local primary care; and the threshold for admission in the region.

We are part of the Specialists for Health medical group, supporting patients in the WellMed Network. We have privileges at four hospitals and two nursing homes in Travis County. Like our counterparts at WellMed Medical Group, we specialize in senior healthcare. Our Austin team includes four hospitalists, with a fifth joining our team in April. Before the hospitalist program for WellMed patients was initiated in early 2012, the hospital re-admission rate was close to 20 percent for the year 2011. Since then, we have helped reduce that rate to 14 percent and have just initiated a new protocol that shows the promise to drastically reduce the rate further.

First, here are a couple of things that we do that go beyond the protocol for hospital providers: 1. PCP visits within 72 hours – We coordinate with the primary care physician/clinic to arrange patients discharged from the hospital to be seen within three days.

CMS standards advocate for patients to be seen within two weeks after discharge. By shortening the time before the transition visit, the patient’s medical home care team can immediately address any issues that have popped up postdischarge and ensure patients are following their postdischarge care plan. This is also accompanied by a nurse phone call to the patient within 24 hours of discharge.

2. Regular meeting with PCPs – we meet regularly (usually weekly) with WellMed primary care physicians to discuss discharged patients, their progress, their care plan and more. This interaction is sometimes done over the phone, but often we have face-to-face meetings at the clinic, giving us a level of familiarity with not only the PCP, but also the staff. This translates into more effective communication and coordination of care. These may sound like small steps, but they have a major impact on the way patients receive treatment after leaving the hospital.

Many of our Medicare patients are on fixed incomes and are rightly concerned about any additional medical costs

This year, we initiated a pilot program to help meet a need we recognized was contributing to hospital re-admissions – the failure of discharged patients to pick up or properly take medication prescribed for them at the hospital. When patients do not pick up prescriptions—or wait days to pick up a medication— that should be taken immediately, this can lead to acute problems and a potential visit to the emergency room.

So we arranged to have medications prescribed for postdischarge use to be delivered to a patient’s hospital room. Again, this sounds like a small thing, but it took some work to achieve. First, we initially could not find a pharmacy willing to deliver to the hospital room without incurring additional delivery fees. Many of our Medicare patients are on fixed incomes and are rightly concerned about any additional medical costs.

Ultimately, we partnered with the independent South Austin Pharmacy, who agreed to deliver medications to the hospital room at no additional cost. The delivery staff are not only bike messengers, but they are pharmacy technicians or the pharmacy owner himself.

This is important because, upon delivery of the

prescriptions, patients are counseled at their bedside about what the medication does, possible side effects, when to take it, what to take with it and, perhaps most importantly, what medicines to avoid taking with it. This reinforces the pharmacy education we also provide for patients before discharge, and the education patients receive at their PCP follow-up visit. Most importantly, it puts prescription medication that is necessary for proper rehabilitation and recovery into the hands of patients at a critical time that is most effective for them.

Over a six-month period, only seven percent of the 53 patients who participated in the pilot prescription delivery program were re-admitted to the hospital within 30 days. veryday in the U.S., roughly 100,000 individuals are waiting on some type of organ transplant list. According to The Hastings Center, a bioethics non-profit center in New York, about a dozen people die each day waiting for an organ that will never arrive. Fortunately, with the help of 3-D organ printing, researchers and developers in the biotechnology field are coming closer everyday to solving this problem.

While the ultimate goal for usable biomaterials is one that can be taken off the shelves at any Home Depot, researchers have found that a patients’ own cells and tissues work best.

Bioprinting works by taking cells from an individual’s organ and then transplanting those cells into an ink cartridge, layered as tissues and ultimately organs. The 3-D printer is able to print organs with the cells and tissues. The cells eject as a gel, and the organ maintains its shape in a glue-like fashion. 3-D organ printing, otherwise known as bioprinting, consists of printing human stem cells onto a 3-D printer to produce a human organ. This process includes taking human epithelial cells, which stay actively reproductive throughout most of a persons life, and then transferring them into a gel, where they are rigorously tested for their durability with micro-pump machines. Once these stem cells clear this check point, they are then placed into an ink cartridge in their gel format (think of the gel as being equivalent to ink in a printer cartridge) and printed as a hollow organ that maintains its shape in a glue-like fashion, based on the inputs from the computer. Think of this much in the same way as when we click a button to have our computers print papers of any kind.

Forest Institute for Regenerative Medicine, gave a TED Talk on bioprinting. In his speech, he displayed a kidney that was printed earlier in the day for the audience to see. What might have been lost on the audience members was that this kidney was not a fully functional organ, rather a kidney model. This displays one major hurdle for researchers in this innovative field to overcome: how to print fully functional, complex organs that can be placed into human beings so they can function with them. This brings up yet another issue, which is: how do researchers accomplish this goal without the patient’s own cells rejecting the organ that is not their own?

Vascularization presents the largest problem, but one that bioprinting has the largest capacity to solve. The entire body system is highly vascularized, and it is the white blood cells in our immune system that either accept or reject whatever we ingest. If our white blood cells—specifically cytotoxic T cells— reject foreign antigens, then the potential for cell apoptosis (cell death) is high. One solution to this problem is seen in infants, where artificial heart valves are surgically replaced.

Workable solutions have been in progress, such as taking individual muscle cells and working them in the lab, so that they can be engineered to make the necessary tissues that make up the complex organs of the heart, kidneys and liver. The problem is not the organs themselves, rather the highly complex system of blood vessels, capillaries and different tissue types surrounding the organs that poses the biggest problem. Regenerative medicine’s main goal is “to do no harm,” with the promise of “making our patients better.”

Researchers at the Gladstone Institutes and University of California, San Francisco, recently released an innovative type of cell programming that may make epithelium cells functional as liver cells. Sheng Ding, the senior investigator at Gladstone Institutes, says, “We thought that, rather than taking these skin cells all the way to a stem cell-like state, perhaps we could take them to an intermediate phase.” Rather than starting over, they were able to create endoderm (embryonic) cells. While growing these liver cells in the lab, they have made significant progress and seen great results.

“The power of regenerative medicine already allows scientists to transform skin cells into cells that closely resemble heart cells, pancreas cells and even neurons,” concludes a study from Nature, a scholarly journal. Hopefully before the end of 2014, we as a population can see a 3-D human liver fully functional as a transplant, ultimately capable of saving hundreds of thousands of lives.

ccess to care. It is a buzz-phrase that has been tossed about for years, but certainly not more prominently so than throughout this past decade and, most notably, over the past six years. It is a phrase that often creates controversy and raises the question of both those on the left and the right, politically speaking, for various reasons.

With the passing of The Affordable Care Act, whether or not you agree with the legitimacy of the affordable aspect of its title, a spotlight has once again been thrust upon the topic of access to care.

The average American’s access to medical care is growing across the board.

Whether or not you believe access to care to be a basic human right, versus a simple privilege extended as a gift of American citizenship, the average American’s access to medical care is growing across the board. I believe the expansion of that access represents the greatest medical innovation for the masses that we will see coming down the road for a long, long time.

Today, as Americans, we have more options and choices than ever before when it comes to seeking medical care. Urgent Care centers exist in almost every city and town in America, while major retail outlets such as Target and Walmart offer an array of ‘clinical’ services in a menu-like format, to treat a variety of common, non-emergent ailments.

Another major innovation where access to care is concerned comes in the form of the proliferation of stand-alone or freestanding emergency centers.

An American Medical News article from April 29, 2013, states, “Centers are opened by hospitals and physicians, sometimes separately, sometimes in alliance,” and they also mention that “the centers’

numbers have doubled during the past decade, up to 284 in 45 states.” The number of centers has continued to increase since that article was first published.

Freestanding emergency centers have the capacity to treat more severe medical conditions than retail clinics or urgent care facilities. They often maintain transfer agreements with larger local medical facilities in order to transfer patients requiring specialty medical or surgical care, or hospital admission.

Although there is no substitute for an ongoing relationship with one’s pediatrician, specialist or primary care physician, there are times when, for whatever reason, you may not be able to make a timely appointment.

Regardless, at least at this juncture in time, I believe that as Americans, we are already reaping the benefits of expansion in our access to care… to an extent well beyond the scope of any other developed nation of the world.

Our expanded access to care can be attributed not only to innovative existing hospital systems that have recognized, at times, the need to expand their services out into their communities, but also to innovative physicians and entrepreneurs willing to develop something ‘outside the box’ of the traditional hospital-based model, in an effort to make good quality and efficient medical care more accessible to us all… 24/7.

The alternative option that urgent care and freestanding emergency centers provide, as an access to minor and emergency care, simply did not exist for most people two decades ago.

Not only have these options created an expansion of choices for all citizens who have the occasional need to seek medical attention outside of their primary providers, but most of these alternatives accommodate for a variety of payment options.

For example, an uninsured individual with a tight work schedule who contracts the flu may be able to receive adequate treatment, paying outof-pocket at any number of the aforementioned centers, without breaking their bank.

A variety of private insurance plans are generally recognized by these innovative alternative treatment centers and many accept Medicaid and Medicare as sources to pay for service.

It remains to be seen just how affordable the business of medicine will become as we witness the full implementation of The Affordable Care Act over the years. Anyone can speculate as to whether the growing role of government in the delivery of healthcare is a positive or negative development, and many of us will shape our view through the lens of political affiliation.

Another valuable development from digital technology is the intraoral camera. I can show a patient an enlarged picture of a decayed or broken down tooth on the computer screen in front of them. This allows the patient to have a clear understanding of their dental needs and facilitates a very healthy discussion of treatment options. I consider the use of the intraoral camera to be the most effective patient education tool available.

Digital technology is also used to improve many dental treatment procedures. Computer aided design-computer aided manufacturing, or (CAD-CAM) technology, is EHLQJ XVHG PRUH IUHTXHQWO\ LQ GHQWDO RIĂ&#x20AC;FHV HYHU\ GD\ This technology involves digitally scanning a tooth needing a restoration, and then designing the restoration chairside. arrytown Pharmacy has been serving the Austin area for over 70 years. Over these years, innovations in the pharmaceutical industry and the prevalence of big name pharmacies have changed the industry, but Tarrytown Pharmacy has stayed true to its mission: dependability and quality service.

Mark and Leslie Newberry are the third generation of Newberrys to own Tarrytown Pharmacy, starting with Mark’s grandfather, Jack Newberry.

Mark’s father, Brian Newberry, purchased the store and continued the traditions of service, community and dependability until Mark decided that he, too, would purchase the store and carry on the family business.

Mark explains that he was never asked to take on the family business.

One thing pharmacies do very well is treat people who are sick. An area we can improve is to help our patients lead healthier lifestyles to prevent them from getting ill in the first place.

“My father told me, ‘Son, you need to find out what you love doing and what makes you happy. You are not going to be good at it if you don’t love doing it,’” Mark explains.

Mark loves being a pharmacist and says that going into work at Tarrytown Pharmacy each day is like social hour because he knows all of his customers so well. His patients and customers are his friends, and he takes pride in helping those he cares about as well as offering a pharmacy with great service and convenience.

Tarrytown Pharmacy offers conveniences that are uncommon to the times. Customers unable to come into the pharmacy to pick up their prescriptions or other necessities are able to have their orders delivered.

For the mother with a sick child or a disabled senior citizen, for example, this service is helpful and appreciated.

The most unique of the conveniences available at Tarrytown Pharmacy is their ability to produce compounded prescriptions. Ironically, pharmacies used to produce compounded prescriptions quite often because there were not any large pharmaceutical companies to massproduce the medications. As these companies started becoming more prevalent, compounding became less common, but its use is on the rise again. Compounding gives the pharmacist the capability of personalizing a prescription to fill the individual needs of each customer, including pets.

Tragedy struck the Newberry family when Mark lost his father to cancer, but through this tragedy, Mark found the inspiration to lead a healthier lifestyle.

“Tragedy can strike at any time, and we can’t control a lot of it. I can control my own risk factors through lowering my cholesterol, controlling my blood sugar, eating healthy and exercising regularly,” Mark explains.

The birth of his twin daughters gave him a new perspective on how, but more importantly why, to live a healthier lifestyle. Mark is participating in his first marathon in two weeks and will be competing in the Hawaii Half Iron Man. He is excited for these competitions, because they play into his competitive nature while keeping him in shape, and this provides a way to stay active with friends!

“Healthy living is key,” Mark says. “In order to instruct my customers to live healthy, I have to walk the walk myself. That is why my wife and I have taken an active approach to refining our diet, exercising regularly and being healthy.”

CT scans are used in a variety of situations, but they are most commonly used after head or other body trauma, or if a patient is experiencing acute abdominal pain, chest pain or shortness of breath. A CT scan uses an X-ray source just like a "plain" X-ray does. However, it uses a narrowly focused source and a rotating detector that can be thought of like the sensor in a digital camera.

Using this focused source, images taken from many angles create a cross-sectional image of the bodyâ&#x20AC;&#x2122;s bones and soft tissues. This tool is very effective in diagnosing injuries, trauma and acute illness throughout the body including the head, chest, abdomen and pelvis.

The process of getting a CT scan begins with visiting a physician to see if the scan is appropriate or not. If a physician believes the best way to diagnose any problems is a CT scan, the patient will be sent to a radiologist to have the scan done. Patients are sometimes given a contrast material, or dye, to see certain areas better. If a contrast material is needed, it may be administered orally with a drink, with an injection in the arm or in the rectum depending on what area of the body needs to be viewed. Next, the patient will lie on a table that is attached to the donut-shaped CT scanner. The table will move slowly into the â&#x20AC;&#x153;holeâ&#x20AC;?, or gantry, of the donut, and images will then be taken as the gantry rotates around you. The whole process takes only a few minutes. The technician will be close by in a separate room and can be spoken to through an intercom.

CT scans are safe, though abdominal scans on pregnant women are not normally done, with certain exceptions.

Radiologists also take care to avoid medical radiation for the pediatric population, and lower dose whenever possible. Nevertheless, there are many cases when CT is necessary and should not be delayed in children, including trauma. When CT is needed for children, using the lowest possible dose is essential since children are more sensitive to radiation exposure, and have the longest lifetime ahead.

The SafeCT scan is innovative to the radiology field. The new SafeCT technology provides a comparable image to traditional CT scans, while decreasing the risks of medical radiation exposure. So when it comes to avoiding heart disease, early diagnosis is critical as it is the key to avoiding trouble before arteries become clogged or a heart attack strikes. We have two materials that cover all the bases: first, a monolithic porcelain called e.max, which has excellent aesthetics and is much stronger than porcelains of the past. This e.max porcelain in most cases can be used anywhere in the mouth. The second material is Zirconium, which is metal-free and extremely strongâ&#x20AC;&#x201D;so much that it can be hit with a sledgehammer and not break. Although stronger, this material is not quite as aesthetic as the e.max so we reserve this for restorations in the back of the mouth for utmost strength. Zirconium is ideal for people who tend to clench or grind their teeth. The latest innovation that dentistry has to offer for a missing tooth in the front of the mouth, where aesthetics is of the utmost importance, would be placing a Zirconium abutment over an implant and cementing an e.max all-porcelain crown over the abutment. Do not be afraid to have the discussion with your dentist about what materials are being used and why.

Oral health, as it relates to overall body health, is not necessarily a new concept, but it is slow to be adapted by all dentists. Typically, dentists in the past would just focus on teeth, but now our focus is on how your oral health affects your entire body. basis for Acupuncture is in traditional Chinese medicine while TDN takes a more Western, evidence-based and integrated approach. Even though we do not know the exact mechanism that makes TDN ‘work’, we do have research that demonstrates it is effective for relieving pain and improving function.

Many conditions may respond to TDN treatment, including acute and chronic pain, and myofascial pain syndrome. It can be helpful when used as an aid during rehabilitation and for injury or surgery. Typically, patients see an immediate change in symptoms. Over the course of your care, TDN may be used one to three times as there is no predetermined number of treatments.

Like any other medical procedure, side effects are always a concern. Trigger point dry needling has very low risk and the most common side effect is muscle soreness.

Will it hurt? Because a very thin solid needle is used to penetrate the skin and stimulate the trigger points, you may feel a slight pinch or fleeting sharp prick. In addition, you may feel a muscle cramp or a muscle twitch often referred to as a “twitch response”. Most patients actually report feeling the muscle cramp and being bothered by it more than the sensation of the needle on the skin.

Imagine going through life and only being able to hear out of one ear.

This condition is known as single-sided deafness. In the past, doctors claimed that there was no option or treatment aside from performing major invasive surgery, which involved placing a titanium plate into the skull. Fortunately, there is now a non-invasive and innovative product that allows those with single-sided deafness to hear in both ears.

As the parts manager for Town North Nissan, Keith LeBlanc was used to living in a loud work environment. However, this made life more challenging because he could only hear out of one ear.

"Having someone sitting right on my left side—my good ear—and not even being able to hear them was a problem,” said Keith LeBlanc, a ]IEV SPH [MXL WMRKPI WMHIH HIEJRIWW

LeBlanc dealt with this condition for 31 years of his life.

“All I hear in that aspect is a lot of noise,” he said.

Single-sided deafness was equally challenging for musician Stuart Hampton. “If I was at a party and there was a band playing, I was useless,” said Hampton. “It would all come into one big funnel, and the brain had a hard time sorting out the separate sound streams.”

Both men’s lives changed for the better a few months ago, when Dr. Russell Briggs of the ENT Center of Austin introduced them to the SoundBite Hearing system. It uses the natural principle of bone conduction, where sound vibrations are transmitted through your bones to your inner ears. Sound from your good ear is sent to a small, computerized device worn on your teeth and is then transmitted to a small hearing device behind your impaired ear.

The total cost is about $6,000. Briggs says that most insurance companies pay about half of the cost. The SoundBite device does not restore hearing in your bad ear; however, it enables what you hear in your good ear to be heard in both ears.

1. How is the current real estate market here in Austin, TX? The market is on fire right now, with many more buyers than sellers in the marketplace. Austin area homes sales in December hit a new record high while inventory reaches hit an all time low. The statistics show 2013 ending with increased sales volume, home prices and demand.

2. What statistics can you share with local Austinites thinking of buying or selling in today’s market?

We can only go off of prior sales facts, and while every neighborhood is different, this is why a comparable market analysis is crucial when thinking of selling or making an offer on your property. As a whole, in December 2013 Austin had sold 2,080 single family homes, up from 14 percent in December 2012. Relatively speaking, the Austin real estate market offered two months of inventory in December 2013, which was 21 days less than in December of 2012. This gave us a historical low for the Austin housing inventory according to the Austin Board of Realtors.

3. What is the median sale price of homes for someone looking to purchase in Austin?

The median price is currently at $226,000, which is 8 percent more than this time last year, with homes spending an average of 52 days on the market, down from December 2012. Last year 27,298 A u s t i n area single family homes sold, which is up 19 percent from the previous year. We are definitely on the upswing, with folks spending lots of cash on the growing Austin market.

If you are starting a family and trying to choose a neighborhood, depending on price range, school district, amenities and the area of town you want to live in with, there are so many great areas to choose from, depending on your lifestyle. There are plenty of options to keep in mind, such as resale. The Austin area in general is a great city to live in. Almost anywhere is a great environment to raise a family in, buy your first home or even invest in a rental.

5. What would you say has the best chance of increasing in value? A) downtown condo, B) east Austin bungalow near the river or C) home on Lake Travis?

Well, I would first have to roll with an East Austin Bungalow right now, and then a downtown Condo, only because of current affordability and the sheer amount of appreciation that is taking place in East Austin. I have heard of people who make large profit margins on tear-downs and new builds where the dirt is all the rage. A couple of years ago, downtown condos were starting to depreciate fast, and it seems that now they are selling very quickly with multiple offers along with the rest of the Austin area. People are becoming desperate for a place to live, and pretty soon you will have very little inventory to choose from, which will make sellers very happy when they make 5 to 10 percent more on average the next year.

Tyson Cole has such an enthusiasm for his craft, taking the aesthetic of Japanese cuisine and bringing it to the next level. Evolving a dish from one idea to something different is the whole concept behind innovative cuisine.

Even with all of their amazing talent at Uchi, one of the biggest mistakes that many chefs make is failing to keep food simple by putting too much food on the plate rather than letting it speak for itself, Tyson says. “Sometimes our chefs come in and will start with 12 ingredients, and XLIR [I [MPP VYPI XLI HMWL HS[R XS EFSYX XLVII XS ½ZI SV sometimes completely change the concept. Other times a chef will make something once and it will be perfect.”

It’s very Japanese and textural, it’s delicious and it has a little acidity; I just love the simplicity of it.”

Sushi is a delicacy that has been super-sized. It is supposed to be bite-sized, and it is supposed to be fresh. As Tyson states in his book, Uchi:The Cookbook, “Our whole ‘bigger is better’ mentality is ridiculous. We should not be looking for a value meal, in which $10 gets you a giant roll that you GERRSX IZIR ½X MR ]SYV QSYXL 2SX SRP] MW MX HMWKYWXMRK FYX ]SY EVI TVMQEVMP] TE]MRK JSV E PSX SJ VMGI ERH E PMXXPI FMX SJ ½WL that may not even be fresh.” Saving money to make money is RSX EP[E]W E KSSH XLMRK [LIR MX GSQIW XS LERHPMRK JVIWL ½WL Food is respected more in Japan. “When people eat, they ½RMWL IZIV] WMRKPI XLMRK SR XLIMV TPEXIW ² 8]WSR WE]W *SSH is very seasonal and plays a large part in people’s lives, but not to a point where it is exaggerated or over-consumed.

Tyson says that the main difference between how Japanese and American cultures view food is that, “Americans can get food anywhere, anytime of the year. So it’s not that Americans don’t appreciate food; it’s just too easy here.”

Parents tell us not to play with our food, but for personal chef Eric Daniel, there are no limits to what can be brought to the table. He has always stood by the statement: We got to where we are by playing with our food.

Much like our attention spans, people seem to be turning to small plates. “We live our lives on social media, our phones, the internet; we have so many options of everything everyday,” says Tyson. “This idea to make all plates sharable and to give people chopsticks and silly things they want to try is the baseline for our concept, which has just blossomed from there.”

Uchi is all about being very accessible. Being comfortable, inviting and not pretentious—a place where people can go to try many different things. Rather than going out to eat a steak and a baked potato, people instead want to try many things and have many options. “Sushi, quite literally, means vinegar rice. It has nothing to do with raw fish; it is about combining rice with various ingredients, small plates and Japanese cuisine as a whole,” Tyson says.

People tend to have such odd and different views of sushi. And as for those who are still skeptical, Tyson advises to simply come in and see what everything is about. He admits that the first time he tried sushi, he thought it was disgusting, “but 99 percent of the time, people really like it and get addicted, which I did.”

Whether it be at Uchi, cooking different meals for his wife and three little girls—all of whom have different tastes—or thoughts and dreams about food, Tyson Cole’s entire life revolves around food. Food is a necessity in our lives and tends to have a way of bringing people together. His love for cooking and coming up with innovative ways of preparing all the components of the perfect bite, with a hint of unexpected creativity, “is almost too real to be a part of and is quite cool,” Tyson says. Finding a balance in his life through controlled chaos and continuing to give talented people opportunities to learn and grow through doing something they love is at the forefront of what Tyson Cole does. “We play with our food,” he says, because why settle for safe and ordinary?

Uchi looks forward to opening their new location in Dallas at the end of the year.

The next time you are having an event catered, think of personal chefs, too. “We take the pretentiousness out of your upscale event,” says Eric—it is a whole new way of dining. We go to restaurants, because that is what we are told to do when we want to go out and eat.

Nobody says, “Oh, you have options.” For the most part, our options are limited to either fast food or a restaurant. Well, why can’t it be in your own home, or a rented vacation home? There is no major market telling you that you have to go to a restaurant to enjoy your birthday.

The thing is, personal chefs specialize in cooking for less. The beauty of it is that Eric does it all in your home, so the cost will be less. Now, you can put that extra money toward your honeymoon; what would you rather do? )VMG´W GSRGITX FILMRH 'YPMREV] 1MW½XW MW to create menus that are based around his clients. He has a diverse clientele, providing services such as private dinners, cocktail parties, rehearsal dinners, wedding receptions, wine tastings and wine-paired dinners with the most versatile cooking requests. “I have people asking me to do anything from sous vid style to traditional outdoor grilling. And then I have some people who just want me to come to their house to make fresh, amazing burgers for two. It’s all over the board,” says Eric.

These chefs, Eric views as artists. They are where he gets his inspiration. He ½RHW QYGL SJ LMW MRWTMVEXMSR JVSQ SPH school chefs, like Julia Child. He gets a lot of his creative ideas about how to put foods together that you might not think would go together, like his crispy pig skin that he puts on top of his pork chops.

“I don’t know why I ever did it; it’s just awesome, “ Eric says.

One of his favorite sunset-inspired dishes that he likes to make is his southwest-blackened scallops paired with a roasted honey tomato relish and green onion vinaigrette. He wanted to get this vibrant red/orange sunset-sky color that he saw when he was out in east Texas one June, when the grass was so green, and duly decided to incorporate the green onion vinaigrette. The scallops are placed in the center on a bed of micro greens, literally like a sunset. Eric says that it was the most beautiful thing he has done yet.

Whether Eric immerses himself in the country or gets ideas for new dishes JVSQ E WYRWIX LI ½RHW MRWTMVEXMSR EPP around him. Eric takes a very holistic approach to his dishes; he takes in all the environments around him. Like an artist, he explains, sometimes it just has to come to you. “There are times when I’m doing a dish and it literally just isn’t coming together. I’ll step back, stop what I’m doing, and approach it again in a few days.”
