BUSY, BUSY, BUSY The last few months have been busy ones for AVA volunteers and staff. Tonya and I are pleased to announce that Jennifer Finlayson joined the staff as our muchneeded Administrative Assistant in April. Here are a few of the highlights, in case you missed them.

NEW STRATEGIC PLAN The Board of Directors approved a new strategic plan in February. The plan focuses on the member benefits AVA does best: providing the best-quality continuing education to improve clinical practice and workforce entry, featuring the Fundamentals of PIV 3-part course.

New in this updated plan is a focus on building awareness of the value of vascular access specialists with employers, public policy makers, and like-minded organizations. AVA has a renewed focus on advocating for VA clinicians and patients. Advocacy takes many forms, including Vascular Access Specialty Day and demonstrating that AVA is the dominant thought leader in vascular access. The Board also put a special emphasis on diversifying AVA’s membership. We want membership to mirror the multi-disciplinary healthcare teams found in your workplace. Please help us recruit more physicians, physician assistants, nurse practitioners, respiratory therapists, radiologic technologists, and others in order to strengthen AVA’s voice and impact.

CARES BY AVA Working with the over 50 AVA Networks, a joint membership recruitment and retention campaign was launched: CARES by AVA. CARES stands for Competency • Action • Results • Education • Satisfaction. This marks the first time AVA and its networks have had a coordinated effort to recruit new members to both AVA and its Networks. An important aspect of CARES is that AVA can now collect dues for the Network. Being able to pay both Network and national dues is a huge convenience for our future and current members. A big “thank you” to MOKanVAN and COVAN for being the first to sign up! New members to AVA also receive a special, one-time discount of 20 percent off, which makes first-year dues $99. SUPPORTING HARMONY AND RESILIENCE IN THE WORKPLACE

Prior to the COVID-19 pandemic, clinicians suffered from workplace-related stress. They dealt with stressful and emotional situations with sick patients and concerned family members, were exposed to suffering and death, endured physically demanding work, worked long hours and, many times, extra shifts, and risked exposure to hazardous materials and infectious diseases. The COVID-19 pandemic intensified these existing challenges by adding elements such as fatigue, increased workload due to short staffing, and post-traumatic stress and anxiety related to patient care during the pandemic. As individuals, team members, and leaders in our clinician field, it is vital that we understand and recognize the impact of these challenges on mental health and identify ways to mitigate each one. Don’t miss the session titled “Supporting Harmony and Resilience in the Workplace” presented by Dr. Marlene Steinheiser, PhD, RN, CRNI®, Director of Clinical Education, Infusion Nurses Society, and Dr. Adele Webb, PhD, RN, FNAP, FAAN, Executive Dean of Healthcare Initiatives, Strategic Education, Inc., as we discuss the barriers to harmony and resilience in the workplace openly and identify factors that lead to harmony/resilience. SAFE AND RELIABLE VASCULAR ACCESS. PROVE IT! M.L. Hawes, DNP, RN, CRNI, VA-BCTM, Chair | Director-at-Large AVA Foundation

The vision of the AVA Foundation is a world with safe and reliable vascular access. These are nice words to put on a plaque and stick on a wall, or we could consider it the mountain top and continue to take steps to reach the summit. The AVA Foundation is committed to ascending the mountain by promoting and facilitating multidisciplinary research.

HOW CAN THE FOUNDATION PROMOTE RESEARCH? The first step we will be taking is to survey the membership to find out what research topics in vascular access interest you.

Projects may include anything from fundamental quality improvement to randomized control trials.

These ideas can be for topics you are interested in pursuing at your facility or just suggestions you

have because you perceive a gap in evidence to support a practice.

RESEARCH IS NOT AN ACTIVITY THAT ONLY HAPPENS AT LARGE TEACHING HOSPITALS. It is the systematic investigation of an issue to establish facts to reach new conclusions or reinforce the current practice. Research can seem daunting even if you are passionately committed to finding the answer. The AVA Foundation is dedicated to helping new and seasoned clinician-researchers to launch investigations, complete the process and publish the evidence.

Mentorship of clinicians to navigate the research path will be a part of the foundation’s support to increase evidence-based answers to current patient safety issues.

Pairing members’ research plans with financial support through small grants administered by the foundation will further remove barriers to completing the process.

Finally, the effort would be futile if the research outcomes were not shared. The AVA Foundation will help the research team move towards poster submission, podium presentation, and publication of their new evidence as appropriate.

The AVA Foundation believes our members are capable of improving vascular access and patient safety through research. Therefore, the foundation will be sending out a survey to understand the perceived research needs of the AVA members. Once the survey is assessed, the Foundation will follow with information on mentorship and grant opportunities.

AVA FOUNDATION UPDATE: APPLY FOR AVASM22 SCHOLARSHIPS NOW!

AVASM22 SCHOLARSHIPS The AVA Foundation Board of Directors recently approved accepting applications for the AVA Annual Scientific Meeting Scholarship (AVASM).

The scholarship pays for one full registration to the meeting, which promotes clinician hands-on learning, continuing education and improved clinical outcomes and patient safety. To apply for a AVASM22 scholarship, please submit your Resume/Curriculum Vitae, letters of recommendation (minimum of 2 required), and completed application form below. Incomplete applications will automatically be ineligible for further review or consideration.

NEW STRATEGIC PLAN ✓ The AVA Foundation Board approved the Foundation’s new logo and color scheme. ✓ The AVA Foundation Board is continuing its work to update its strategic plan. Approval is planned for October 2022. Pillars of the strategic plan include: • Promotion of Clinical Scholarships: Enhance education and support the vascular access specialty and its education programs.

• Developing a Research Program: Promote understanding of the research process through journal assessment of current vascular access studies. The Foundation plans to partner with AVA, AVATAR, JAVA and others.

• Collaboration: Enhance collaboration with AVA at AVASM and other fundraising and awareness opportunities • Improve Fundraising: Increase major donations by providing industry partners, grant makers and others robust opportunities for funding research and scholarships.

WHAT A DECADE OF COLD/VIBRATION RESEARCH HAS TAUGHT US ABOUT VASCULAR ACCESS, PAIN, AND FAINTING AMY BAXTER, MD, FAAP, FACEP While we’re back in person this year, the COVID pandemic revealed how much needles matter and vaccine refusal impacts healthcare. At the same time, NIH funding for finding opioid alternatives has discovered how pain itself works in our brain, with an overlapping opportunity to improve pain relief for vascular access.

Over a decade of specific research on vibration and cold for multiple needle procedures has answered some controversies and curiosities. How much does cold help pain relief, and does temperature matter? Why does vibration reduce pain but massage doesn’t? What kind of distraction matters, and why?

These discoveries impact vascular access comfort and success. Do certain procedures hurt more? Why do some people faint and what stops it? What pain management changes lab values and what doesn’t?

VASCULAR ACCESS DEVICE USE IN SKILLED NURSING FACILITIES More than 1.6 million patients are cared for in the 15,000 licensed skilled nursing facilities (SNFs) in the United States (Centers for Disease Control and Prevention [CDC], 2016). Approximately 3% (48,000) of patients in these facilities require vascular access for intravenous therapy (LTC Pharmacy Provider, source). As of date, there are only five studies over the last 15 years, that have been published on this patient population in this care setting.

Comparatively, there are approximately 6,000 hospitals in the United States. A search of articles published since 2007 related to vascular access device (VAD) utilization and outcomes in acute care settings yields hundreds of results.

The limited studies in SNFs demonstrate significantly higher complication rates for VADs compared to acute care settings including CLABSI, catheter migration and dislodgment, catheter occlusion, and deep vein thrombosis, leading to resident transfers to the emergency department and subsequent hospitalizations (Hand, 2016; Harrod et al., 2016; Chopra, Montoya, et al., 2015).

Additional studies found that 35-40% of hospital readmissions from SNFs were related to vascular access device complications; studies concluded residents with VADs in SNFs were significant predictors for hospital readmissions, and unplanned (Huang et al., 2018, Schmidt et al., 2017).

Patients in SNFs are older, have multiple comorbid conditions, and are at an increased risk of developing VAD complications

(Lin et al., 2016). Skilled nursing facilities face staffing challenges due to high staff turnover and nurseto-patient ratios. The majority of the direct care staff in SNFs is unlicensed (over 60%); 23% are licensed practical nurses, and only 12% are registered nurses.

What about all the standards and guidelines we have access to drive evidence-based practice (EBP) in VAD utilization?

For various reasons (staffing, culture, complex care setting), implementation of EBP in SNFs is deficient.

HOW DO WE RESPOND TO THIS CHALLENGING SITUATION? The first thing we need to do – is respond. How can we collaborate with our SNF partners to support improved patient outcomes?

NETWORK NEWS On Friday, November 5th, 2021. AVACNY held its 12th Annual Teaching Day Symposium at the Craftsman Inn and Conference Center in Syracuse NY. After a year off due to the COVID Pandemic, we were excited to be in person again! With over 120 attendees and 20 sponsors, the event was a huge success! Nurses came from all over New York State to attend the event.

Speakers covered a variety of topics, including evidence-based medicine, pediatric vascular access needs, needless connector designs and CRBSIs, the history of the Association for Vascular Access (AVA), home infusion therapies and vascular access changes in practice.

In 2022, our network goals are to continue to offer opportunities for education, expand our network of healthcare providers, encourage certification through Vascular Access Certification Corporation (VACC), and to promote membership in the National Association for Vascular Access (AVA).

We would like to thank Thomas Kuracina with Injectimed for hosting our postsymposium dinner where we were able to discuss new advances in technology with Vascular Access devices that will promote patient safety and provided positive outcomes for our future.

Some of our accomplishments so far this year included a dinner presentation hosted

by Lineus Medical on The D.I.P.P.E.R Study (Dislodgement, Infiltration, Phlebitis, Prevention Eliminating PIV Restarts): A Randomized Controlled Trial Evaluating a Novel Force-Activated Separation Device (FASD) presented by Spencer Jones in March.

2022 brings many new opportunities to discuss new trending technologies, advanced practices for our scope of practice and current infusion therapies for the alternative care setting.

CALL FOR MANUSCRIPTS

We invite you to submit original manuscripts in the field of Vascular Access. We are interested in receiving manuscripts on clinical practice, education and research related to vascular access including articles on vascular access manufacturing and technology, and vascular access care and maintenance issues in hospitals, home settings, hospice, and alternative care facilities.

We also invite submissions to our Patient/Consumer Perspective column where we ask you to share personal stories or “lessons learned” about caring for, living with or having a vascular access device. In about 1000 words and in conversational style, present your story. You can submit on behalf of someone or encourage them to write it themselves.
