Six months into my chairmanship of Avante Partnership I continue to be struck by the constancy of change in the social care business
T
his year, the new legislation Health and Social Care Act 2012 seemed to be missing the point when the Treasury decided to defer any decision about implementing the Dilnot reports recommendations, only for the matter to be raised again by the Prime Minister, apparently with support from the coalition partners. There is no doubt that meeting the costs of care for older people in residential or community care, is becoming a major issue for families as we all live longer, making the prospect of needing care more likely for many of us, that coupled with the huge reductions in public spending required of Local Authorities, puts the onus more firmly on families and relatives to make arrangements for, and to pay for care. That the Liberal Democrats have decided that social care is a bigger election issue than reform of the House of Lords can only be a good thing, and acceptance of Dilnot, or any similar resolution, would take much of the uncertainty away from families in terms of the likely costs of care of their loved ones. For those that choose, or who need to be placed in residential care, we recognise how important it is for them and their families to feel confident about the care available. Avante Partnership is fortunate indeed in having a Board with a group of Trustees, who together, demonstrate a huge range of experience and skills, and are all committed to supporting our CEO, management team and all of our workforce in ensuring that that will be the case. They already do a great job for them and for us.
We believe that together we will be well placed to offer families, individuals and our commissioners the best possible and the safest care, wherever we provide it, and the best possible value for money in all that we do. This is especially the case in our service for children and families provided by Stepahead Support not withstanding the difficulties we have experienced through Local Authorities and commissioning process for these services. I believe that we are well placed to continue to offer and indeed to develop the services that are very effective for young people and their families and I look forward to the opportunity to see this aspect of our work further developed in the future. The next few years will offer all of us in the care and support business a new set of challenges, whether in residential, or in home care, for adults or for children and their families and it is vital that we are in a strong position to respond to them. This Annual Review demonstrates our achievements thus far, and the Boards’ determination to build upon them and take the organisation and its services further forward as we strive to tackle the increasing needs of our Service Users of the future. CHIEF EXECUTIVE’S FOREWORD
Before starting to write my piece for this year’s Annual Review, I thought I’d quickly look back at what I wrote for last year’s Review!
T
he last thing I wanted to do was to repeat what I said last year or worst still contradict what I said. I have to confess having read it again, whilst not repeating it, I can only echo what I said last year, that 2011/12 was a challenging year for the country, our sector and the organisation! Whilst this Annual Review is a positive look back at the last 12 months, I’d like to look a little way into the future. As you read this Annual Review, colleagues from across our organisation are working hard to review our current Business Plan and develop our 2013/18 strategy and Integrated Business Plan. In a social care sector that is changing faster than most people and organisations can comfortably cope with, some might question the benefit or even validity of a plan that looks 5 years ahead, and indeed, we have posed that question to ourselves. We have answered it by focusing our thinking on the question ‘why?’. Simon Sinek in his book ‘Start with Why’ sets out a powerful argument that it doesn’t matter what you do, what matters is why you do it, so we are basing the development of our new 5 year strategy less on what we intend to do over the next 5 years or even how we intend to do it, but we are basing our strategy and plan on a clear understanding about why we do what we do. By focusing on the ‘why?’ question, I believe we can look 5 years ahead with confidence, no matter what changes are ahead of us.

I cannot tell you how proud I am to be the Chief Executive of Avante Partnership. This is a great organisation. We are an honest and transparent organisation trying to do the right things for the right reasons every single day. Our staff have worked amazingly hard to deliver great care and support to the thousands of people we have come in contact with over the last 12 months, and for that I thank them. We have a Board of Trustees who under the leadership of first Brian Munday and more recently Peter Smallridge have made a huge contribution to the organisation with positive support and constructive challenge throughout the year, and for that I thank them. Overall, given the complex and challenging nature of the world and sector that we operate in, we have had a good year. OVERVIEW OF OUR SERVICES Avante Partnership
was established in 1990 and is a not for profit charitable organisation and one of the largest Social Care providers in the South East. Avante Partnership is a large, complex, intergenerational organisation, so in order to simplify our various service offerings we have structured the organisation into separate business units, creating a virtual group structure. Through this structure, we believe we are best placed to respond to the particular needs of each of our customer and Service User groups, commissioners and the communities in which we work.
Avante Community Support
is a long established and ambitious charity that primarily supports people with high quality domiciliary care in Kent, Bexley and Greenwich, but also supports children, young people and families with learning disabilities.
Avante Care and Support
has 14 care homes specialising in dementia care across Kent and South East London and 1 care home in Woolwich specialising in mental healthcare. The team of well trained staff and volunteers across the business exceeds 1500 people, with the quality of our services assured by Investor in People and ISO9001 accreditations.

helps young people who are finding life’s problems difficult to overcome. Support is provided for young people living at home, in foster care, leaving care, who are homeless, in council or private accommodation. The services we provide include one-to-one confidential support, advice and help, group workshops, mediation services and special outdoor events for fun and friendship. Our approach is person centred, tailoring support to individual need. We look to engage the whole community around an individual, including their family and friends as appropriate.
My Time
is a short term respite service for people living with dementia, whilst offering support for the carer. The new service can be booked and paid for online at www.avantemytime.org.uk to suit the times and needs of the client and carer.

We understand dementia
Person centred care is at the heart of our work and to ensure we achieve this our Dementia Strategy clearly identifies the objectives to be achieved over a three year period.
We deliver a wealth of support services for elderly people, with the aim of not just providing care and comfort, but also a new, positive and more engaging stage in their lives.
To support the delivery of this work Avante Care and Support has created three new positions within the organisation.
At the heart of our work is our Philosophy of Care, influenced by the Eden Alternative, where we encourage social interaction, engagement and the eradication of boredom. We help develop interests and hobbies within our care homes and provide as many opportunities to meet and make new friends.
Our Quality Manager has been appointed to take lead responsibility for auditing quality assurance in each of our homes; this will be in addition to our continued auditing by our external auditors ISOQAR against our ISO 9001 accreditation. Diet, health & wellbeing of our residents are an important part of our work. This new role, Health & Wellbeing Advisor will work with our care homes to develop and improve nutrition and hydration for all residents, as well as developing a broad range of menus to meet the increasing needs for diverse and specialist resident diets. The health and well being advisor will also work with our catering and care staff by proving specialist support and training and will liaise closely with each home’s local dieticians to ensure best practice.

One objective of this role is to review every residents medication to ensure it continues to be appropriate for each resident, with an emphasis on optimising medicines and where possible reducing the use of antipsychotic medications, whilst introducing other non pharmacological approaches to maintain our philosophy of care and enable residents to live an ordinary life. To support our Philosophy of Care and ensure the good practice of our Philosophy is consistently delivered across our care homes we now have 79 Eden Associates, 7 Eden Champions and 1 Eden Mentor. Two of our care homes already hold the Eden accreditation but were re-assessed (to ensure the good practice of our philosophy remains) in May 2012 and passed. We have been working in partnership with a performing arts company called Bright Shadow, which has helped and supported Service Users living with dementia through music, song and drama. The pilot scheme ran in our care home Bridge Haven and due to its success will now be carried through to our Bexley homes with the aid of a grant from the Worshipful Company of Wax Chandlers. The Not Forgotten Society, a charity to benefit ex service veterans, arranged a very professional entertainment group to give a concert to veterans and others at our care home Court Regis during 2012. The charity also invited 10 veterans from our care homes, with escorts, to attend a Garden Party held at Buckingham Palace in May this year. The Learning Centre Project ran from September 2011 to June 2012 and involved social work students from the University of Kent completing work experience in the care homes, engaging with staff and residents.

A weekend away for Honeyfield care home residents
Sally* Age: 14
Friday 13th July can be an unlucky day for some, but not for a few of the residents at Honeyfield care home in Hextable, as Friday 13th July was the day that eleven residents and ten carers from the home set off for their annual weekend away to Whitstable, Kent. Two mini buses left Hextable at 6pm on the Friday evening heading straight to Seaview Holiday Camp for a three night break at the seaside town.
Sally was referred to Stepahead Support by her school in January. Sally was presented as disruptive in class, was frequently tearful and there was also conflict at home between Sally and her parents, especially her mother. The behaviour Sally was displaying in school suggested she had very low self-esteem.
With the first trip away in 2011 proving to be such a success, Maureen Attaway and Christine Caird decided to make the weekend away an annual event for residents and their families to take part if they choose. Residents and carers shared three caravans between them and settled in well to their temporary accommodation. Team Leader Jacqui Stone said, ‘As soon as we headed off for our weekend away you could see a change in the residents, it was the sense of adventure of a weekend away that was noticeable and the excitement that we all feel when we head off to somewhere new for a holiday!’. The weekend was packed with entertainment including 60’s nights at the holiday camp club house, a bus trip to Herne Bay, a fish and chip supper at The Plough Inn at Chestfield

(for all 21 residents and carers), a trip to Margate and lots of walks and fresh sea air along the seafront. Jacqui Stone commented, ‘The whole weekend was a wonderful experience for everyone involved. I have vivid memories of one of our residents called Jean who came away with us. Jean can sometimes struggle with her speech and isn’t as chatty as she used to be, but the trip away quite clearly helped Jean. We visited Margate for the day and Jean was quick to recognise the seaside town and was actively pointing out local landmarks and recalling to staff and fellow residents various memories that she had of Margate. When you see a positive difference in someone like Jean it makes you realise how trips away like this are so meaningful, I certainly learnt a lot over the weekend and I have great memories of our weekend away.’

Sally was establishing short-lived ‘friendships’ but would then report that she was being ‘bullied’ by these friends and this had become a pattern in her social engagement within school. Sally would also fabricate powerful stories, including claiming her mother was dying of cancer. The initial session with mum and Sally and a Stepahead keyworker confirmed that a cycle had built up, whereby Sally’s behaviour had reached a level where mum was rejecting her daughter. This made Sally feel she had nothing to lose and her behaviour would then become worse. The first issue we worked on was the relationship between Sally and mum, acknowledging things had gone wrong but encouraging them to focus on solutions. Using person centred tools such as ‘like and admire’ and ‘important to and important for’ encouraged Sally and mum to focus on strengths and positives. Mum and Sally subsequently

created their own ‘Action Plan’ based on what they both wanted to change. The Stepahead keyworker also saw Sally in school for one-to-one sessions and focused on social skills, establishing and sustaining friendships, the importance of self-worth and empathy. The situation has slowly but gradually improved; Mum is now more positive about Sally, who in turn is happier at home and less likely to challenge mum’s ‘boundaries’. At school Sally is engaging better with her peer group and is learning that she can be ‘accepted for herself’ and does not need to make up stories to gain attention and sustain relationships. *name has been changed

CASE STUDY New Outlooks

Betty Age: 66 Betty is a lively young 66 year old who is very sociable and loves her day trips out and has been a client with Avante Community Support, New Outlooks, for over six years. Betty lives in sheltered accommodation and has received support from New Outlooks for areas in her day to day life that she had previously struggled with, such as shopping and budgeting her finances. With the help of New Outlooks Betty now enjoys holiday trips to both Blackpool and Spain. In July 2012 Betty celebrated her 66th birthday with Avante Community Support at the annual Summer Getaway. Betty was over the moon that her birthday fell on the same day as the annual event as she enjoyed showing people her birthday cards and badges on the day, she was also presented a bottle of Bucks Fizz on the day from Chief Executive, Cedric Frederick. Since Betty has been with New Outlooks she has become the life and soul of every event and keeps other Service Users and Keyworkers amused with her sharp wit and humour!

STAFF TRAINING, DEVELOPMENT & AWARDS ‘Avante Partnership remains committed to developing our workforce and our overarching objective is to ensure a competent and capable workforce, fit for purpose and able to deliver the highest quality services’ During the last financial year, we delivered 440 courses to 5,790 people. Staff have also undertaken qualifications in Health and Social Care, following the dementia pathway, at Levels 2 and 3 and in our young people’s services, Level 3 qualifications with modules focusing on children and young people. We have also recruited 6 Apprentices who have joined the organisation to undertake qualifications. We are members of the National Skills Academy and engage with Skills for Care to access the Workforce Development Fund. Our training courses cover approximately 40 different topics and incorporate our organisational values and Philosophy of Care. To augment our training courses, we deliver learning through use of the Aged Care Channel (a satellite TV based learning platform) action learning sets, coaching, secondments and e-learning. We continue to make significant investment into the learning and development of our staff to ensure our staff are developed and qualified to deliver high quality care and support services.

NEW DEVELOPMENT AND INVESTMENTS Avante Partnership has embarked on a 5 year programme to modernise or replace, where necessary, our care homes to ensure they provide the highest quality homes that meet the aspirations of our customers and provide an attractive and efficient working environment for our staff. Bridge Haven Our Care Centre in Bridge, near Canterbury, has been transformed by an extensive refurbishment programme that has seen the construction of larger, more modern en-suite rooms in the new Patrixbourne and Kingston suites, along with many other improvements. A new Architect designed reception and office area adds a light and attractive entrance area, decorated with eye catching modern but

functional furniture. Outside, the newly landscaped areas add to a peaceful environment with a focal point around the new tea room and sculptured water fountain. To celebrate the improvements, the home formerly known as The New Close, has now been re-named Bridge Haven.
Puddingstone Grange

The hoardings have gone up at Plumstead Lodge in Greenwich and our contractor, Castleoak, are preparing the site ready to commence demolition of the old home and begin construction of a stunning new 62 bed home called Puddingstone Grange, that is due to open in October 2013. A fresh approach to marketing of the new home will help ensure Puddingstone Grange is a great success in a highly competitive market.
Behind the scenes activity is taking place to prepare Barton Court for the redevelopment of a new 75 bed home, that will offer both dementia and nursing care. Construction will commence later in the year with a new design approach providing a larger central core for dining areas and offices. The new construction will be in two phases, to ensure that existing residents can safely remain in the old home until the first phase of the new development is completed. The new home will be fully completed in October 2014.
We aim to offer value for money and high quality services in our homes that can offer a choice of accommodation at a range of price points.

COMMUNITY SUPPORT SERVICES The past year has been an exciting but challenging year for Avante Community Support. We have restructured our teams in Bexley and Faversham to offer more streamlined support to both our internal and external customers. Our main focus this past year has been on Quality Assurance. Some of our challenges were to ensure that over 300 active carers were up-to-date on all their mandatory and regulatory training courses, as well as the new requirements of the Essential Standards of Quality and Safety. This has now been achieved. We have been able to offer over 100 different courses between November 2011 and April 2012 to ensure that all new starters and existing staff are trained and tested for competency.
In order to accommodate the restructure, the Bexley team moved offices from Crayford to a modern open plan facility in Erith in April 2012. The new location was opened with a Balloon Launch, inviting health professionals, local press, Social Services, our partners in the care industry, Service Users, Trustees and staff to join in celebrating the opening of the new offices. Overall, the targets and vision for the past year have been achieved. We are pressing ahead with opportunities and ideas that will continue to make Avante Community Support competitive in the local market place and ensure that we are always offering a value for money service with a competitive edge.
Our overall customer satisfaction ratings have risen from 85% in 2011 to 92% in 2012 and this is reflected in the very positive comments we have received about the care staff “The support and care given by the staff at home could not be better” “The girls always give me a happy greeting and a smile” “Staff well trained” “I am impressed that the carers always ring me if they have a problem” “My carers go the extra mile!”

The new team ensure that unannounced spot checks, field-based supervisions, probation reviews, and induction training and shadowing are all completed on a regular basis for new and existing care staff. We currently have over 200 carers working in Community Support in the Bexley Branch and over 100 carers working in the Faversham Branch.

STEPAHEAD SUPPORT This has probably been the most challenging 12 months that Stepahead Support has experienced in our 17 year history. The main challenges have been in terms of dealing with instability of contracts and financial uncertainty, whilst maintaining staff morale. These have, in the main, been created by the fundamental changes in commissioning arrangements in Kent County Council (KCC), the largest source of funding for Stepahead Support. One particular downturn is the impending cessation of the targeted family support service funded by KCC. Stepahead Keyworkers have through their skills, strengths and abilities, provided life changing support services to thousands of children, young people and their families over the past 13 years. It is impossible to demonstrate or do justice to the real impact the service has had over the years, as the benefit of Stepahead’s support leaves a legacy that spans generations. Whilst Stepahead Support is facing difficult and uncertain times with KCC funding streams, there are also great opportunities, to develop the services and grow. The release of tenders on the KCC framework have brought new opportunities both in terms of winning new work and working in greater partnership with other organisations. Stepahead, as part of the Kent Children and Young People consortium, was awarded a contract to deliver Emotional Health and Wellbeing services in Kent. The consortium includes key agencies delivering services in Kent such as CXK, KCFN and Family Action.
In spite of the challenges we have faced, we have remained optimistic and driven to continue delivering and developing quality services to make a positive difference to the lives of children, young people and their families.
Here are just a few of our achievements over the past year: • In November 2011, we launched a new private counselling service which has been crucial in enabling young people and parents/carer to access timely therapeutic support. • We have launched a training arm to the service delivering bespoke training to foster carers in the south east. • We have undertaken a very successful pilot in partnership with Canterbury City Council to provide support to young people living independently, with the pilot now being extended. • One of our housing provider partners has carried out a fantastic refurbishment of an accommodation unit called Maddison House in Northfleet, where we support 7 young people. • Our Community Impact project in Thanet, funded through the Big Lottery fund continues to exceed targets every year.
