Avante Care & Support offers over 800 specialist dementia and nursing placements within its care home portfolio, as well as day care and short term respite breaks. Avante Care & Support provides over 4500 weekly hours of care to individuals living at home, to enable them to continue to live full, independent and active lives.

We have over 20 years’ experience in providing home care and support across Kent and South East London. All support is tailored to meet individual needs. Whether home care is required as a result of illness, recovery from a hospital stay or simply because individuals are finding some tasks too difficult to manage.

DAY CARE Our day care centres are safe and secure environments located within our care homes, providing organised and spontaneous activities encouraging companionship and care. The services are suitable for older people with varying levels of care needs, including those who have dementia. Attending day care services helps to relieve loneliness and boredom for people living on their own, and assists people in maintaining independence.

Avante Care & Support knows that caring for someone living with dementia can sometimes mean putting your own life on hold. Our ‘My Time’ service at Bridge Haven care home is a service to help to ensure this doesn’t happen.
‘My Time’ short term respite care is there to support you and your loved one. For RESPITE AND more information on ‘My Time’ and how to book please call SHORT STAY CARE We provide respite care breaks for older people and provide all the care required in a high quality and safe environment. The respite care can be for as little as one hour, for a weekend or week long breaks, delivered by trained, qualified and caring staff.

CARE HOME CHECKLIST Use our checklist to help you make an informed choice about the care homes you visit. You can also visit www.cqc.org.uk to view public CQC regulation reports. Where is the home? Is there good parking? Are there transport links near by? Is there wheelchair access? Are facilities such as shops, pubs, parks and places of worship within easy reach? Is there a lift? Is the home clean, fresh and inviting? Do the rooms feel hot and stuffy or cold and draughty? Are there quiet areas to go to when visiting? Are the rooms a good size? Do the rooms have en-suite facilities or basins? Will you be able to bring your own possessions such as pictures, plants and furniture? Does the home allow pets? How does the home make you feel? What was your overall impression of the home?

HOME CARE & SUPPORT Avante Care & Support provides domicilliary care and support into peoples homes. Offering assistance with daily living, enabling individuals to maintain independent and active lives. Our compassionate and fully trained staff provide support from 30 minute visits through to live-in care to achieve this outcome. Whether someone is coming home from hospital or in need of help due to mobility or health related issues, services can be tailored to meet each individual’s needs. We also provide another care and support service called New Outlooks. New Outlooks offers high quality support to adults with learning difficulties, physical disabilities, sensory impairment and mental health related issues. We support individuals across Kent and South East London to live as independently as possible.

Funding for home care can be through assessment by social services, including the option of direct budgets controlled by the recipient of services. Individuals can also purchase privately.

OUR MISSION
In general, staff and volunteers act in a person-centred way, treating people as individuals with respect and dignity.

NUTRITION AND HYDRATION Avante Care & Support understands how important diet is for a person’s wellbeing and takes a special interest in nutrition and the impact on mental wellbeing. We regularly review the way nutrition and food services are delivered and perceived. The eating habits of people with dementia can have a direct effect on their health and the progression of the condition. It is very important that people living with dementia maintain a good nutritional intake as well as adequate fluids. Being fully hydrated helps many avoid or, at least lessen, some of the symptoms such as agitation, confusion and restlessness that can strike during the early evening hours.
“Dad’s appetite has always been a worry for me, but since he’s moved into the home we have seen a remarkable difference in his eating habits. I think it is due to the variety on offer. There really is something for everyone and Dad’s appetite has increased a great deal.”

A PROF ESSIONAL’S VIEW Parkview, Bexleyheath “I a m writing with regards to the above home, which I visit frequently both as a Para medic for the London Ambulance Service and as a Clinical Care Practitioner, working in partnership with Bexley & Greenwich Healthcare. The first impression when you walk in to Avante Care & Support’s care home, Parkview, is a relaxed home full of laughter. There’s always music playing and residents all seem relaxed and happy. The staff are always interacting with the residents, taking time to talk to them, hold their hand and interact in every way possible. Over the past few weeks, my role as a Clinical Care Practitioner has brought me into the home at least once a week to assess residents. Parkview staff are highly aware of the risks of dehydration, urine infections and lethargy and are of the opinion prevention is better than cure and therefore call us in to assess residents if there is even the slightest change to the residents ‘normal’ behaviour. A lot of homes could learn so much from Parkview, it’s not all mod cons and fancy furniture, it is a place filled with love, laughter and genuine happiness for people in their twilight years who have to live with dementia, I for one would be more than happy for my loved ones to go into Parkview if the need arises.”

PAYING FOR CARE When choosing a care home for a relative or yourself, it is important that you choose a home that can meet all your needs. Once you choose a home, you will usually be responsible for paying the care fees (unless these are covered, or partially covered by Social Services). If you are concerned about your ability to pay the fees, you can approach the local Social Services Department for an assessment of care needs. At the same time they will undertake a financial assessment and determine if you qualify for assistance with the payment. Whether funding is provided is determined by several factors. These include a person’s wealth, their entitlement to state benefits and their ability to pay. The financial thresholds are reviewed annually by the Government. As a provider we recognise that whilst some individuals will wish to privately fund their care, others will look to Social Services for assistance. You can obtain free independent advice on care fees from Paying for Care – www.payingforcare.org.uk
