Gynecomastia is extremely common. My approach takes into account that men generally don’t like the stigma of having plastic surgery. It’s important that we not overlook this when approaching consideration of surgical improvement. That’s why I spent a great deal of time talking about the process, expectation and procedure itself. Gynecomastia is even more prevalent among adult men. The same NIH study estimated that roughly 65% of men between the ages of 27 and 92 years experienced some degree of gynecomastia. Although the symptoms may be similar, the causes of gynecomastia in adolescents and adults are very different. In addition to hormone imbalances that occur over time, male breast enlargement in adults can be caused by certain medical conditions including thyroid problems or cancer of the pituitary gland. Gynecomastia can also be caused by consumption of alcohol and recreational drugs like marijuana.

Innovation in the Time of COVID-19; Cardiologist Adds Video Consults Through Telemedicine IT IS OFTEN TIMES OF CHALLENGE THAT BRING FORTH INNOVATION. MICRA PACEMAKERS
As the nation continues to navigate the impact of COVID-19, the healthcare industry looks toward telemedicine as an innovative method of care. The call for social distancing has accelerated the arrival of telehealth, and providers are utilizing video technology to treat patients. These virtual appointments create a convenient and accessible avenue for delivering medical care where there may have been limited access before. For Don Steely, M.D., an interventional cardiologist with the Conway Regional Cardiovascular Clinic, telemedicine has provided the opportunity for regular visits with patients from the comfort of their homes. “It’s cool. I learn about their dogs, cats and grandkids while I’m finding out about my patients’ health,” he says. The next step, Steely believes, will be extending video visits to patients at hospitals in smaller communities such as Clinton, Dardanelle, and other areas of north-central Arkansas. “In the future, a nurse at one of these hospitals could take vital signs (heart rate, blood pressure, pulse oxygen), and I could look at the results on my phone and ask the patient questions,” he says. “Patients in hospitals without a cardiologist on their staff could still have access to one via telemedicine.” This new level of access, Steely says, will allow him to see more inpatients. “It’s going to open up spots for more first-time patients,” he says. “Overall, more people will be able to access a cardiologist.” Since the COVID-19 pandemic began, Steely has been visiting with about one-third of his patients via telemedicine appointments. His interventional cardiology partners, Deepali Tukaye, M.D., and Lensey Scott, M.D., are also scheduling telemedicine appointments through the Conway Regional Cardiovascular Clinic. Alexa Shipp, an Advanced Practice Registered Nurse with the clinic, held video appointments with 14 of the 15 patients she saw during a recent clinic day. Steely’s weekly onsite visit to Ozark Health in Clinton traditionally includes a full-day of in-person visits with new or follow-up cardiology patients. With the rise of telemedicine, he is also using his breaks to provide video follow-up consults with his patients in other areas of the state.

An interventional cardiologist with more than 20 years in practice, Steely has embraced technological changes. In 2018, he was one of the first interventional cardiologists in Arkansas to begin implanting Micra VR pacemakers for patients with bradycardia (slow heart rhythm). Micra pacemakers are one-tenth the size of traditional pacemakers. The Micra pacemakers are much less invasive and last longer than traditional pacemakers. Because they do not use cardiac leads, the pacemakers are barely visible. On March 11, Steely became the first interventional cardiologist in central Arkansas to insert the new, upgraded model of the miniature pacemaker. Micra™ AV, is the world’s smallest pacemaker with atrioventricular (AV) synchrony, according to Medtronic, the manufacturer. Micra AV expands the number of people who can receive leadless pacemaker therapy. This new device, indicated for the treatment of patients with AV block, extends the most advanced pacing technology — at one-tenth the size of a traditional pacemaker — to additional patients. AV block is a type of heart block in which the electrical signals between the chambers of the heart (the atria and the ventricles) are impaired. A pacemaker is the most common way to treat AV block. Pacemakers help restore the heart’s normal rhythm and relieve symptoms by coordinating the electrical activity of the atria and the ventricles. When this process — known as AV synchrony — is achieved, patients are healthier and have improved quality of life due to increased blood flow from the left ventricle of the heart. Comparable in size to a large vitamin, Steely opted to use Medtronic’s Micra AV based on its ability to deliver therapy via a minimally invasive approach. During the implant procedure, the device is inserted through a catheter and implanted directly into the heart with small tines. Because Micra AV does not require leads or a surgical “pocket” under the skin, potential sources of complications related to leads and pockets are eliminated — as are any visible signs of the device. The updated Micra AV was approved by the U.S. Food and Drug Administration in January 2020. Micra Pacemakers available through the Conway Regional Cardiac Catheterization Laboratory: MICRA AV expands the number of people who can receive leadless pacemaker therapy. Unlike, MICRA VR, MICRA AV can treat patients with AV Block, a condition in which the electrical signals between the chambers of the heart (the atria and the ventricle) are impaired. MICRA AV can detect mechanical movement of a beat in one chamber of the heart — the atrium — and then deliver electricity to another chamber where MICRA AV is implanted — the ventricle — so the atrium and ventricle beat in synchrony. MICRA VR is for patients with bradycardia (slow heart rhythms), where it is unnecessary to deliver electricity to the atria (the chambers at the top of the heart). Most patients who fall into this group are those who have permanent atrial fibrillation, in whom it is impossible to deliver electricity (pace) the atria when this rhythm is present.

O
ften, we hear the boisterous phrase, “You’ll be hearing from my lawyer,” in some way, shape or form in movies or on television. However the phrase is delivered, the trope serves as what purports to be the supreme comeback for alleged wrongdoing; although, the fact that it is usually an antagonist who adopts such threatening language might say something. But over time, this cliche has created a false presumption about our litigation professionals, describing them as weapons first, perhaps even convincing us that we need such a blade at our hip at all times for whoever might wrong us. For most, this false pretense couldn’t be further from the truth. “The phrase is, indeed, a bit of hyperbole,” attorney Paul Keith says. “Most people don’t invoke their lawyer as an offensive weapon.” Keith is an attorney at law for the firm Gibson & Keith, in Monticello, and is well-inclined to speak on the subject at hand. His experience with the law spans decades and includes his serving as city attorney for Hamburg since 2002, chairman of Arkansas Bar Association’s Board of Governors and, most recently, being named the incoming president of the association. “People tend to engage a lawyer when they find themselves in a problem,” he continues. These situations run the gamut. We all know that when charged with a crime, the expertise of a lawyer is usually in our best interest to seek out; the representation of an attorney is an American right when we face charges, after all. But what about the normal comings and goings of everyday life? Do we need counsel while we are doing our taxes annually? A pantsuit to help us argue property lines with the neighbor? Well, it wouldn’t hurt. Although Keith estimates that one of the most common ways laypeople utilize the professional assistance of an attorney is for family law-related matters. By definition, family law is general, broad and well-encompassing. In practice, the scope is usually narrowed to domestic disputes of most kinds — specifically, divorce and child custody. According to the American Psychological Association (APA), more than 90 percent of Americans are married by the age of 50. It’s a social construct that, for better or worse, has been ingrained into our

I had to leave one foot on the carcass so they would know they were getting a raccoon and not a small dog or big cat.

FIRST TASTE OF SUCCESS: When I was hired at The BridgeWay. I was given a lot of creative freedom in my programs to really shape them to the needs of our patients. To see patients benefit from that really validated that I was doing what was right.
WHAT IS A “GOOD MAN?” To listen to your heart and gut. It can be hard to really do what you feel is right, but it’s extremely important to do that.
MOST ADMIRABLE QUALITY IN OTHER MEN: The ability to go against the grain to do what’s right; to follow your gut feelings. I admire those who can make tough decisions to really be able to do the right thing.
PERSON YOU ADMIRE MOST: 100 percent, without question, my wife. To see her walk through life humble, energetic and vulnerable with such grace is an extremely rewarding experience and to be able to share that with her is even better.

SURPRISING FACT: I am very down to earth. People sometimes make assumptions about you as a neurosurgeon, and someone who is well achieved, so they come to you with some caution or preconceived ideas about you not being approachable. It is completely the opposite.
ADVICE TO OTHERS: I usually tell my fellows and younger staff not to try and be someone else. If they want to come learn from me, I tell them not to try and be me. Always try to be the best version of yourself because your best may be much better than me. If you try to be like someone else, you limit yourself. If you try to be the best version of you, there is no limit to that.
