Healthcare at new heights
Flagstaff’s medical and health field and its fascinating elements | By Seth Muller As the editor of Northern Arizona’s Mountain Living for the past eleven years, I have crossed paths with several amazing medical stories. I also have helped shepherd thematic features around the unusual aspects of medicine at 7,000 feet in elevation and in a mostly rural landscape. There are stories of how heart surgery is performed at altitude, how local care providers have needed to adapt to the great distances of patients with “telemedicine,” and how to respectfully care for the Native American populations given their strong cultural beliefs and practices. Our magazine has covered dozens of facets of regional health issues under our department Mind & Body, as well as an annual health issue we do that has examined such topics as elite athlete training, better eating through local foods, stress management and holistic and alternative care. Flagstaff has some very specific medical challenges beyond the aforementioned realities of life at altitude. The UV factor in the desert Southwest at altitude makes many of us more susceptible to skin cancer. The light here also has an impact on our vision, which makes the use of sunglasses in many outdoor situations essential. Even the lack of relative humidity can cause challenges for our eyes and skin. Our love for the outdoors further creates situations that lead to the need for wilderness medicine. We’ve been on the scene for Wilderness First Responder training, where fictive disasters in the wilds have helped prepare many people over the years to assist in these
remote medical situations. Coconino County Search & Rescue, with one of the area’s most impressive volunteer units, is also an extension of the life-saving community. Our love for high adventure, such as rock climbing, mountain biking, skiing and river-running, can also bring medical emergencies that need swift response and extensive follow-up care. What I’ve learned over the years in these wide arrays of local coverage is that we are blessed to have a deep and extraordinary well of medical and healthcare providers. For all of the features we’ve written and all of the sources we’ve contacted, it’s always incredible to meet people with such incredible dedication in saving lives and improving the overall quality of life of their patients. We have multiple facilities and multiple offices and providers delivering the kind of care that has won national awards. It’s impressive to meet some of these people and to learn of how passionate they are to make a difference. With that in mind, I want to introduce the new Arizona Daily Sun Healthcare Directory. I hope you’ll take the time to get to know some of our providers and some of the services available in our mountain town. Despite being a small city, we have extensive offerings in many categories of health provision and services and a level of care that we might expect would be reserved for a major metropolitan area. Preventing Sports Injuries
H
igh school athletes are naturally at risk for sportsrelated injuries. According to the Centers for Disease Control and Prevention, these athletes account for an estimated 2 million injuries, 500,000 doctor visits and 30,000 hospitalizations annually. So what kind of steps can you take to make sure your high school studentathlete stays safe on the diamond, football field or track? As with most injury-related issues, it all comes down to preparation and preventive education.
Most at Risk Sports From baseball, basketball and soccer to football, wrestling and volleyball, high school athletics spread across a broad spectrum of activities. Some have higher degrees of contact and difficulty, putting kids more at risk for injuries. Of the nine different sports studied, the CDC confirmed that football, girls basketball and wrestling had the greatest proportions and severity of 4

injuries While these findings point to some of the most dangerous activities, they aren’t conclusive. You may think soccer is less dangerous than football, but studies have found increased occurrences of concussions in the sport. Golf may not seem like a very dangerous athletic endeavor to those outside the sport, but those who have participated know that injuries can take a toll.
Types of Injuries & Safety Just as there are many different sports for boys and girls to choose from at the high school level, there are many types of injuries that can occur. These can range from cuts and bruises
to mobility-limiting sprains and fractures. Concussions have grabbed the national spotlight with Hollywood movies and highprofile athletes calling attention to the issue. The high-school level can be just as dangerous, so precautions must be taken when it comes to protecting the heads of student-athletes. Safety is always the goal so that high school athletes can play, compete and thrive in their chosen sports. According to Kids Health (The Nemours Foundation), student-athletes should follow four guiding principles: • The use of compliant and properly fitting equipment; • The appropriate maintenance and integrity of playing surfaces; • Adult supervision and commitment to safety; and • Proper preparation. If any of your child’s athletic experiences fail to meet these standards, it’s time to bring it up to your school board, coach or community leaders. We must work together to ensure the consistent safety of our children.

Treatment for varicose veins V
aricose veins are swollen, twisted veins that you can see just under the skin. They usually occur in the legs, but also can form in other parts of the body. Hemorrhoids are a type of varicose vein. Why do they occur? Your veins have oneway valves that help keep blood flowing toward your heart. But, if those valves are weak or damaged, blood can back up and pool in your veins. This causes the veins to swell. So what are some treatment options, both at-home and professional?
Self-care The Mayo Clinic notes that self-care — such as exercising, losing weight, not wearing tight clothes, elevating your legs, and avoiding long periods of standing or sitting — can go a long way toward preventing varicose veins from getting worse.

If interested, you can buy compression stockings at most pharmacies and medical supply stores. Prices vary. Prescriptionstrength stockings also are available.
Laser surgeries Doctors are now using new technology in laser treatments to close off smaller varicose veins and spider veins, according to the clinic. Laser surgery works by sending strong bursts of light onto the vein, which makes the vein slowly fade and disappear. No incisions or needles are used.
Ambulatory Phlebectomy The American Society of Dermatologic Surgery notes ambulatory phlebectomy is an outpatient procedure developed by dermatologic surgeons that removes superficial veins through small, slit6

s the American Heart Association notes, high cholesterol is one of the major controllable risk factors for coronary heart disease, heart attack and stroke. The basics Cholesterol is a waxy substance that comes from two sources: your body and food, per the AHA. Your body, and especially your liver, makes all the cholesterol you need and circulates it through the blood. Cholesterol is also found in foods from animal sources, such as meat, poultry and full-fat dairy products. Your liver produces more cholesterol when you eat a diet high in saturated and trans fats. Excess cholesterol can form plaque between layers of artery walls, making it harder for your heart to circulate blood. Plaque can break open and cause blood clots. If a clot blocks an artery that feeds the brain, it causes a stroke. If it blocks an artery that feeds the heart, it causes a heart attack.
The two types There are two types of cholesterol: “good” and “bad.” Too much of one type — or not enough of another — can put you at risk for coronary heart disease, heart attack or stroke. It’s important to know the levels of cholesterol in your blood so that you and your doctor can determine the best strategy to lower your risk. Making healthy eating choices and increasing exercise are important first steps in improving your cholesterol. For some people,

Connection to heart disease As your blood cholesterol rises, so does your risk of coronary heart disease. If you have other risk factors, such as smoking, high blood pressure or diabetes, this risk increases even further. The greater the level of each risk factor, the more that factor affects your overall risk. Your cholesterol level can be affected by your age, gender, family health history and diet. When too much LDL (bad) cholesterol circulates in the blood, it can slowly build up in the inner walls of the arteries that feed the heart and brain, according to the AHA. Together with other substances, cholesterol can form a thick, hard deposit called plaque that can narrow the arteries and make them less flexible. This condition is known as atherosclerosis. If a clot forms and blocks a narrowed artery, a heart attack or stroke can result.

Genetic testing is a wonderful tool for physicians and patients. 5-10%, and in some cases up to 25% of certain cancers are due to an inherited genetic cause, or mutation, which is identifiable with genetic testing. Not only can this provide information that could impact treatment decisions if a person has cancer, but it allows physicians to screen appropriately for any other cancers a patient may be at increased risk for. It is also useful for relatives who can then be tested for the familial mutation. Genetic testing provides the ability to not just help one person, but all of their blood relatives across the whole family tree. Genetic testing also has some practical value for treating, not just preventing, cancers. First, patients who have a BRCA mutation actually have a relatively better chance of survival for breast and ovarian cancer. Second, new drugs like PARP inhibitors specifically target the BRCA mutation to exploit a weak-

ness in the cancer. One of them has recently been FDA-approved in the treatment of ovarian cancer. The genetic testing is complex, as there are now dozens of other genes that can be tested, beyond the BRCA genes. It is not just a simple matter of positive or negative results. There may be unusual “variants of uncertain clinical significance” that require the expertise of a genetic counselor to decipher. Genetic testing should be considered when there is a strong family history of cancer or with rare cancers such as ovarian cancer or male breast cancer. There are many facets to a genetic risk assessment that are useful to the physician, the patient fighting cancer, and family members who might potentially benefit from this life-saving information. leep is the foundation for overall good health. Without sleep we’re grouchy and testy. With proper sleep, we’re back to our old peachy selves.
Getting plenty of sleep will improve your quality of life both mentally and physically, so finding ways to increase your ratio of shut-eye to being awake should be an important part of your overall health. Some of the benefits of sleep are quite obvious, because we can all relate to that feeling of rest when we sleep well. That gives us significant

advantage over sleep-deprived people in many areas of life, including job performance and overall relationships with others. But just how many of us are getting a full night’s sleep? Not enough. About 70 million Americans of all ages suffer from chronic sleep problems, according to the National Institutes of Health.
Lack of Sleep Because Americans are very busy, sleep is sacrificed too often in order to keep up the pace. However, sleep deprivation actually decreases an individual’s quality of life. According to the NIH, although personal needs vary, on average, adults need seven to eight hours of sleep per night. Babies typically sleep about 16 hours a day. Young children need at least 10 hours of sleep, while teenagers need at least nine hours. If we consistently fall below these averages, there are many negative consequences, including increased irritability, dulled senses and less energy, according to the NIH.

Sleep Tips Getting enough sleep is easier said than done, and sometimes it is just hard to wind down after a long day at the office. Sleep can be disrupted by many things, including stimulants such as caffeine or electronic distractions. If you’re having trouble settling into a consistent sleep routine, try these tips from the NIH: • Go to bed at the same time each night and get up at the same time each morning; • Sleep in a dark, quiet, comfortable environment; • Exercise daily (but not right before bedtime); • Limit the use of electronics before bed; and • Avoid alcohol and stimulants such as caffeine late in the day. If these tips do not help, talk to your doctor. There could be physical reasons why you cannot sleep, including insomnia and sleep apnea. These conditions can be dangerous, but they are treatable, so it’s important to visit your physician.

llergies can be caused by the environment, food or reactions to medications. And without a doubt, they can all cause misery. Itchy eyes and scratchy throats are just the beginning for many allergy sufferers.
More serious issues can occur if allergies are not kept in check. If you suspect that you may be suffering from an allergic reaction — even if it is mild — you should consider making an appointment with your physician to investigate.
What are Allergies? According to the Mayo Clinic, allergies occur when your immune system reacts to a foreign substance by producing antibodies that identify a particular allergen as harmful, even though it isn’t. The National Institutes of Health identifies the following as common allergy triggers: • Airborne allergens, such as pollen, animal dander, dust mites and mold; • Certain foods, particularly peanuts, tree nuts, wheat, soy, fish, shellfish, eggs and milk; • Insect stings, such as bee or wasp stings; • Medications, particularly penicillin or penicillin-based antibiotics; and • Latex or other substances you touch, which can cause allergic skin reactions.

Dealing With Allergens
If your family has a history of allergy problems, it’s worth a checkup with your doctor to check your sensitivities. Broad Symptoms When an allergy sufferer comes into contact with an allergen, his immune system reacts. Skin can become inflamed, as can sinuses, airways and digestive systems. This results in a runny nose, sneezing, itching, rashes, swelling or asthma. Allergies can range from minor to severe, and a life-threatening reaction called anaphylaxis also can occur.
Treatments & Prevention Sadly, there are many allergies that have no cure. Treatment and management are the only ways to relieve the suffering in many cases. Nasal saline and 24-hour allergy medications or antihistamines can alleviate symptoms for many sufferers, but it’s important to check with your doctor before starting any new medication. If dust or pet mites are causing your reactions, frequently washing your bedding in hot water can help tamp down the effect. Ventilation fans and dehumidifiers can reduce moisture in damp areas where mold grows.
