Services: Adult/ teen medical care, health screenings and testing, chronic disease management, prescription assistance, laboratory services, wellness education and disease prevention, help accessing healthcare resources, social services and case management.
Services: individual education, case management, referrals to drug treatment and detoxification, and one-for-one syringe exchange.
