ACTING F.A.S.T. KNOWING WHAT TO DO AND WHERE TO GO WHEN HAVING A STROKE

A SURVIVOR’S STORY Waking up with a migraine was nothing new for Cheryl Kovalsky, known as ‘Sheri’, but last November, having gone to bed with the pain and waking up with it still there, she knew something was wrong. She told her husband “this isn’t anything like I’ve felt before,” and decided to go to the Emergency Room at Wellington Regional Medical Center. When she arrived at the ER, the doctors quickly determined that she had suffered a hemorrhagic stroke. “Sheri’s condition was a medical emergency with a high mortality rate,” said Dr. Juan Gomez, MD. The condition can be extremely life threatening if it causes bleeding in the brain, as Sheri’s did. Sheri said she didn’t realize how serious the situation was until she was being admitted to the hospital and sent to the neurointerventional radiology suite. Sheri underwent a procedure called endovascular coiling. This is where a small incision is made in the groin and a catheter is guided to the brain where tiny wires are released in to the weakened area of the artery to stop the bleeding. This prevents a free flow of blood from entering the area which could cause another internal bleed in the brain. Sheri spent 15 days in the hospital recovering and today has no disabilities or setbacks. It’s rare to come away from a stroke with no side effects but Sheri will have to be followed regularly to make sure it doesn’t happen again. Wellington Regional Medical Center was able to treat Sheri because she recognized something was wrong and got to the ER quickly. Wellington Regional Medical Center follows the American Stroke Association’s ‘Get With The Guidelines’ principles for improving stroke care by promoting consistent adherence to the latest scientific treatment guidelines. 1. Why did you want to get involved with the Palm Beach Heart Walk 2017? Chairing the Heart Walk provides me with the opportunity to engage with our community by spreading awareness of the signs and symptoms of Heart Disease and Stroke, which are our number 1 and 5 leading causes of death. 2. What is your ‘why’ or reason to live a longer, healthier life? My father passed away when I was 16 from a cardiac related event, so the mission of the American Heart Association, building healthier lives, free of cardiovascular diseases and stroke, hits home for me. My father is why. 3. Why did you want CPR training at this year’s event? Together, Palm Beach County Fire & Rescue and the American Heart Association have trained over 2,000 Palm Beach County residents in CPR, creating a community filled with additional bystander lifesavers! There are more people to train which will mean more lives saved and that is why it is important to me. Of cardiac arrests which don’t happen in the hospital, 70% happen in homes. Unfortunately, only 46% of people get the immediate help that they need before professional help arrives. 4. How do you see the American Heart Association’s Mission working in Palm Beach County? Their mission comes to life in many ways. Raising awareness for the importance of living a heart healthy lifestyle through efforts like our Heart Walk campaign is just one of them. The American Heart Association’s local Board of Directors has a strong focus on spreading awareness for the importance of knowing your numbers and empowering people to monitor and control their blood pressure through a campaign called ‘Beat the Odds’. This past April the American Heart Association, alongside the Palm Beach County School Board, passed policy that will require all high-school students throughout the Palm Beach County school district to receive hands-only CPR training at least one time prior to their high school graduation. Getting more bystander lifesavers trained in hands-only CPR has been a huge focus of the American Heart Association over the last year. You can learn this valuable skill at this year’s Palm Beach County Heart Walk.

1. What is CPR and how does it work? CPR, or cardiopulmonary resuscitation, is a way of keeping oxygenated blood circulating around the body in the event of a cardiac arrest. 2. When is CPR needed? CPR is needed in the event of a cardiac arrest to keep oxygen circulating around the body to keep the vital organs alive. This is extremely important as the brain, the most sensitive of organs in the body, will start to be significantly affected by lack of oxygen in 4-5 minutes. 3. Why is it important to know Hands Only CPR? 70% of all out of hospital cardiac arrests happen in the home, so being trained in CPR can be the difference between life or death for a loved one. 4. How are your chances of surviving a cardiac event increased by receiving CPR? When a person has a cardiac arrest, survival depends on immediately getting CPR from someone nearby. Almost 90Retinopathy percent of people •who suffer out-of-hospital cardiac arrests die. DIVORCE MEDIATION – IS IT TABOO? By Dr. Eva Brown PhD, Couples and Intimacy Therapist

Why is divorce mediation Taboo for many couples? The answer to this question is multi-faceted as many variables are present when making the big decision to Divorce, especially when children are involved and of course we can’t forget, the money factor! In my experience as a couples and intimacy therapist and coach, I find that couples who decide to get their own individual lawyers are scared of the unknown and bottom the line is, they are protecting themselves. Over the years, my couples have expressed that they are scared of mediation because they may not being treated fairly, that their spouse may manipulate the situation in their favor, or that they may not be able to get to see their children as often and the list goes on. The unfortunate part of this often very painful, time consuming, and expensive battle of divorce is that the couple and their children often end up suffering financially and emotionally during this process. Since I am not a lawyer and I do not have the expertise to advise about the mediation process, I teamed up with powerhouse couple, Eric and Erica Epstein, of www. PeacefulSplit.com. Eric Epstein has been an attorney in New York and Florida for twenty-two years and is also a Florida Supreme Court Certified Family Law Mediator and Erica Epstein holds Masters

C’s of Mediation: Dr. Eva Brown (Podcast Host): “Why would a couples choose to divorce using a mediator?” Eric Epstein (Podcast Guest): “I have what I call my 5 C’s for the benefits of mediation over divorcing with individual lawyers. The 1st one is, Control over the process and what that means is that when people choose to get a divorce

and decide to go to the court to have the whole dispute resolved, the person that’s ultimately going to make the decision that’s going to affect their personal lives, their family’s life, their finances, their children is a man or women sitting in a black robe that maybe they meet once or twice for an hour. And when you go through mediation you have complete control over the process and it’s very empowering for couples to sit and know that their going to be in control of the decision-making process and not some stranger that they happen to be assigned to through a random process. The second C is Confidentiality and the great thing about mediation, similar to therapy, is that when couples are sitting in the mediation room with us, other than the legal exceptions of child or elderly abuse…everything they say in mediation room is strictly confidential. That means even if the worst happens and the mediation falls apart and they do have to go to court, whatever they have discussed or made offers on and negotiated in good faith during the mediation is going to be confidential and not used against them in court. The third C is Cost and obviously if people hire attorneys there’s going to be a very high cost … and unfortunately lawyers are there to make money and charge by the hour and so often they don’t have an incentive


to do things very quickly. They want to drag it out and make motions and depositions and the longer it goes it’s worse for the couple but often times better for the attorney. Where as in mediation it’s a limited process, a limited cost…it’s usually a fixed cost so it’s much more cost effective for people. And the last two C’s are sort of combined and they are Concern and Children. When people go through mediation they can put their families best interest first and they go in with the mindset that they want to solve the dispute…cause even though they are not going to be married anymore they are still going to be parents for the rest of their lives, they are still going to have financial relationships, even after the divorce, and in mediation they can sit and actually work on their concerns and their children’s best interest as compared to going to court.” If you’d like to listen to the entire interview, here is the link: http://bit.ly/2xjJpwW. Prenatal Care, Chiropractic and Chinese Medicine By Caroline Blanke-Pena, L.Ac, Dipl.OM, Acupuncture Physician I sat down this month with Drew Biggs, DC (Doctor of Chiropractic), to learn more about the similarities and differences between Chiropractic and Chinese Medicine, and their respective approaches to prenatal care. I wondered if it would benefit an expecting mother to receive both chiropractic treatments and acupuncture during pregnancy. At Holistic Health Palm Beach, in Wellington, Florida, my clients have asked if these two modalities should or can work together to help them best achieve their health goals. I have found that among many physicians/practitioners, we sometimes don’t know how to answer this because we don’t specialize in each others professions. Well, I’m here to create dialogue and bring these communities together, so we can learn together! Each month, I will be interviewing a different physician/ practitioner in the holistic health field or western medicine arena, with the goal of shedding light on how we can all work together to best achieve wellness for our patients. According to Dr. Biggs, chiropractic prenatal care has two main goals; one is keeping

20 NOVEMBER 2017

the mother comfortable and functional throughout her pregnancy, and the other is to create optimal birth outcomes for both mother and baby. Dr. Biggs stated, “Most women come in because they have heard that chiropractic can help them have a better birth experience. Chiropractors look for pelvic distortion and any rotation or torsion of the pelvis, which is a common scenario for any adult. Most adults and kids have it in varying degrees, although never ideal, it’s particularly problematic for a woman at the time of labor or during delivery of the baby. The pelvis needs to expand so the baby can come out as easily as possible.” In Chinese Medicine, which includes acupuncture, herbal medicine, Asian massage, and more, the goal is quite similar. Acupuncture creates space, moves what is potentially “stuck” and harmonizes and tonifies the body. In addition to helping with proper placement of the baby and comfort for the mother, acupuncture is ideal for stress, hormonal changes, anxiety, and the many emotional changes that woman experience during their pregnan-

cies. Instead of looking at structural alignment, as in Chiropractic, Chinese Medicine looks for pathological imbalances which are manifested through the eyes, tongue, pulse, and what the mother is subjectively feeling. Dr. Biggs goes on to explain that the pelvic bones expand to create an opening for the baby to come out. If these are distorted, the bones don’t expand properly and this keeps the aperture (or opening) smaller. If the aperture is larger, this means the baby can come out with relative ease. Chiropractic keeps the pelvis aligned so it can expand and open up, minimizing the necessity for birth interventions. It may be less likely that the baby gets “stuck,” resulting in the necessity for a c-section, or requiring the use of forceps or vacuum suction delivery, or Pitocin injected for stimulation of uterine contractions. What about fetal positioning during pregnancy? How can Chiropractic and Chinese Medicine/Acupuncture help? Acupuncture has long been indicated to help induce labor or stimulate the repositioning of the baby while in utero. Clients


come into my Chinese Medicine practice for special labor induction sessions that are focused on needling and stimulating points that are indicated for encouraging contractions to begin for the mother. This is usually done around 38 to 42 weeks. There are special acupuncture points that are also indicated for turning or repositioning a baby. According to Dr. Biggs, “Improper fetal positioning, also known as dystocia, can be positively affected with chiropractic care, although it isn't a primary focus. Chiropractic does not aim to turn babies, rather it balances the pelvis, and in turn, balances tension on the four ligaments that attach between the pelvis and the uterus.” If there is torsion, there is an uneven tension on those ligaments that causes the uterus to distort its position, thus leading to intrauterine constraint. If this becomes the case, there is likely less free movable space for the baby, creating a challenge for the baby to get into an optimal position, i.e., breach, face forward, etc. Through balancing of the pelvis, this constraint is eliminated and the baby may find its way into proper positioning. Dr. Biggs stated, “Ultimately, all I am focused on is addressing misalignment patterns and pelvic ligaments. Many woman come in with breach presentations, and its common for them to get that resolved.”

Breaking the Chronic Pain Cycle of

FIBROMYALGIA Chronic pain takes many shapes and forms, but we know that it has a huge social and economic burden. The Institute of Medicine reports that roughly 116 million, or 30% of the population suffer from pain lasting a year or longer. Although chronic pain isn’t a life threatening issue, it’s certainly a problem that has a dramatic effect on the quality of someone’s life. After taking care of hundreds of people with conditions like fibromyalgia, I know that these people experience quite a toll both physically and emotionally. Perhaps the worst part about fibromyalgia is that there is no known cause or cure. It’s basically invisible to any blood tests or MRI studies, and most of the drugs available are for pain control. People with fibromyalgia may often be depressed, have anxiety, and have a sense of isolation because they don’t feel that their friends and family understand their struggle. This often compounded by the fact that their doctors have told them that the pain is psychosomatic, or just in their minds because no tests can reveal an underlying disease process. In recent years, fibromyalgia has become one of the most published topics in medical journals. As a result, we are starting to develop an understanding of the disease on a deeper level to know that it’s certainly NOT just in the patient’s mind. Research is pointing to problems within the central nervous system and tiny blood vessels in the skin as likely causes of the deep bodywide pain. But what does this mean in terms of treatment? For most people, drugs that target the nervous system like Lyrica or Gabapentin are the first line of treatment and many have had success. However, some patients can be sensitive to drug reactions, or their bodies may develop a high tolerance for the dosages. In cases like these, many patients

start to seek out alternative therapies like acupuncture, reiki, and homeopathy. Traditional chiropractic has also been a source of relief for many people seeking help with chronic pain. However, many people with the chronic pain of fibromyalgia can feel apprehensive about seeing a Doctor of Chiropractic because manipulations can seem too rough for someone who can barely stand to be touched. Not All Adjustments are Built the Same In chiropractic, there are dozens of techniques and approaches to correcting the spine. Some are heavy handed and require a lot of force, but others are very gentle and rely more on precision. It’s not to say that one is better than the other, but some are just designed for specific patient populations in mind. In my office, I attract many patients with fibromyalgia, trigeminal neuralgia, and TMJ problems that are Secondary to a Structrual problem in the neck called Atlas Displacement Complex. Most of these people are afraid to be touched, and gravitate to a lighter approach. This type of condition requires a precision type of adjustment called an Atlas Correction. This type of adjustment is so light, that most people often wonder if I even touched them, and then they start to feel their body change. Currently, only about 1000 doctors in the country are trained in this type of care. This approach is effective because of the spine’s influence on the central nervous system. When the top of the neck shifts abnormally and moves improperly, the nerves firing into the brainstem become distorted. In this way, the brain is like a computer that relies on spinal positioning to operate normally. patients creates a condition called central sensitization. This is the reason why people with fibromyalgia can feel excruciating pain from a light touch. It’s not that there’s a physical injury, but the nervous system is getting scrambled signals and is primed to experience pain. This is NOT designed to cure you. The cure lies in the fact that the body is capable of healing itself. When you remove interference to the body’s self-healing ability, I find that people can return to a level where life is livable again. By breaking the pain cycle through the nervous system, you can impact the way stress affects the body and the immune system. By no means is this a cure all, but it can be a great catalyst to being steered back on the path towards a normal life. I’ve seen numerous patients who’ve been able to go from disability to working or resuming exercise again in a matter of months. One of the greatest joys of doing this type of work is giving people a glimmer of hope. So many people are convinced that they have to live in pain, or have been told that the problem they’re having is all in their head. AMERICAN CANCER SOCIETY’S MAKING STRIDES IN BREAST CANCER Whenever I have an opportunity to join my South Florida community in supporting important causes, I always come away richer in spirit and warmer in heart. On Saturday, October 21, 2017, our Flourish Masterminds team walked to raise money and awareness for breast cancer along with many other teams

to hand paint our Flourish Mastermind shirts to put love and dedication into the whole process of being part of this walk. The word flourish means to thrive, and it is my hope and desire that everyone who is touched by this disease thrives in the face of it. Breast cancer statistics are mind blowing. One out of every eight

Held annually every October, the Making Strides in Breast Cancer Walk, which is associated with The American Cancer Society, begins in quaint Mizner Park and winds down beautiful Palmetto Park Road, over the bridge, toward the beach, and then back again. Leading up to and during the walk, I had many insights. First, I felt inspired

women will be affected. That sucks, to say it simply. In the U.S., 252,710 women will be diagnosed with breast cancer, and 40,610 of them will die in 2017. In my own family, I lost both my grandmother and my cousin to this disease, so it hits very close to home. With the front of the shirts complete, I wanted them also to reflect the healing power of love, so I painted a graffiti-

style heart on the back of each one, representing all women who have received this diagnosis. I also thought about our daughters and the future of this disease—I brought thoughts of healing, cures, and eradicating it completely to mind The morning of the walk, I woke up super early—ready and prepared. A sense of honor washed over me when I arrived. So many others had come to support this important cause, all waking up early too to arrive for the 8:30 start time. My dear friend, Lori Samuels, who had been diagnosed more than 10 years ago had just arrived home from Canada at midnight and drove 45 minutes to join us—arriving on time and ready to go. What touched me deeply was how many young girls were walking and how many men came out to support the cause. When I noticed two twenty-something young men wearing t-shirts honoring their grandmother, I felt a deep feeling of warmth and compassion. What a beautiful gift of their time and honor they were giving to their grandmother. Boca Raton Mayor Susan Haynie and Boynton Beach Mayor Steven Grant were there to officially open the walk at the 7 a.m. City Council members, Robert Weinroth, Andrea Levine O’Rourke were there as well. Truly, this was an awesome and heartwarming sight to see and experience. There were teams from neighborhoods, well-known companies, and local businesses all out for this cause. Even the firefighters were there, giving out complimentary pink fire hats. Some


people went way out and dressed in pink feathers, rhinestones, and even wore pink wigs! We even saw a pink motorcycle, pink car, and pink tow truck. Creativity for a good cause at its finest.

I could feel the loving energy of the walk, too, as if it were a physical thing. People were holding hands, group members had their arms around one another, and real heart-and-soul connections were being made all around. I had the most beautiful conversation with one of my students, which I wouldn’t have had had it not been for the walk. And to add an even greater dimension to it all, Shelley Plumb, Plumbtalk Productions, joined us and expertly captured the feeling in photography and video, and for that I couldn’t be more grateful. I felt so blessed to have been part of the day and fortunate that I was part of a bigger picture toward a brighter future where everyone can someday flourish better health. The Flourish Mastermind team ended the experience at Starbucks, where we recapped the day and shared our thoughts. We talked about what we felt, and we all wholeheartedly agreed that the day reflected immense love, compassion, connection, community, bonding, healing, wellness, and lots of HOPE.

Parent-Coach Communication Realizing the importance of the interplay between player, parent, and coach is why tennisconsulting.net was formed. Since it’s easier for others on the outside to create responsibility protocols for the entire team regardless of whose mainly at fault for, a consultant like me helps bridge the gaps. The aim is to make the team more successful providing parties are willing and open to improve and change any limitations. Below is an example of how an outside consultant or the coach in the case below with myself, attempts to intervene with a student’s underlying issues that stem from themselves or possibly their parent or coach not doing more. I am sorry that we have not spoken since Troy walked off the court the other day. My instinct is to always resolve issues right away. In this case, I thought things would simmer down. I overrode my desire to call you because I needed time to gather my thoughts, of which I have been pondering on for a while. I was glad you called me because it pushed me to dig deep and express myself. Firstly, I wanted to say that it has been such a pleasure working with Troy and you guys. Subsequently, if we have to depart our coaching relationship temporarily or permanently for whatever reason, I do not want it to affect our overall relationship. As a coach, my objective is to improve the student. If I am not, then there is little reason to continue our coaching relationship unless the student and family are ok with stagnant progress. The problem with coaches who push very little is that students barely improve and

28 NOVEMBER 2017

at the end of the day it can lead to more frustration and self-esteem problems later on. Hence, as a coach it is my job to do whatever I can within proper boundaries to push the student to the next level. Because peoples’ boundaries differ for different reasons, communication and reality checks must come into play. Realistically for Troy to get better in my opinion, he has to allow the coach to push him more than he does. When push comes to shove, I believe that Troy becomes too soft to make effective headway and this softness affects every aspect of his tennis game. Similar to me, Troy is not the strongest, most athletic person. Hence, we need to be relatively coachable in a sport that requires simultaneous technical, mental, and physical improvement. I know you have asked me to lighten up at times especially being that Troy is only 10 years old and the end goal is not to be professional or play in college for now.

I definitely take your needs into account because it is my job to establish the proper boundaries between what a family wants and what I feel a student is capable of accomplishing. In my opinion, Troy’s softness was starting to add up of late based upon the other day and the week before. I was tough on him, but not overly tough in my opinion. As always, during and after pushing Troy, I expressed how much I cared about him and my belief in him. I told him my reasons for being tougher on him versus my toughness would be for a recreational player with less desire for instance. I also suggested to Troy that we do not have to train all the time. One of my suggestions was possibly taking time off or working with me less time. I am passionate about the subject of tennis and proper push. As much as I aim to improve students, my number one goal is for them to be happy. If I did not care as much as I do about the


students I coach, I would just adapt to how the student is feeling in the moment. However, I would not be doing my job for their tennis or happiness. Even though Troy is very talented and smart, I can’t ultimately do what I have to do to improve his tennis if I do not have yours and your husband’s support when push comes to shove. Personally I was a stubborn, challenging student so I can relate to Troy. My understanding from both a personal and coaching perspective of being coachable and non-coachable has led me to believe that unless the coach and parent(s) are on the same wavelength with a student who needs to be pushed, the student will control things too much. When this happens, softness rather than consistent, sufficient effort takes over and improvement ultimately is minimized or ceases all together. I have done lots of mediation and written lots of letters to parents over this same issue. I have also brought in third parties to help resolve things or been part of a family’s third party efforts including therapists and sports psychologists who were already on the family’s team. Hence, this issue of softness and Troy’s frustration with me for being tough on him is not a Troy problem. It’s a common problem that is best when nipped early on with all students. If not, the patterns become habits and breaking habits become very tough to overcome. I think the world of Troy. I also want nothing but the best for him. However, if Troy wants to keep improving (and this does not mean having to be a professional or even a college player), then I believe he must allow me to fully coach him with your support or its best to make some changes. What I am asking for with your support is allow me to push Troy to follow through with his tennis specific needs for the day, rather than Troy giving in to what I call ‘beneficial discomfort’. The more Troy consistently follows

through with his daily needs, rather than avoids what he needs to do when things get tough, the more he will improve and reach his tennis specific goals. I appreciate listening to my feelings on these important matters. Please communicate with me before Friday so we know how to best proceed. Sincerely Coach Jonathan

Best Chiropractor I’ve Ever Had My name is Allan Schaefer. I am a 63 year old male, who has lived in 4 different cities over the years, many of them in New Haven, Connecticut where I was born. I started going to chiropractors when I was 17 years old and have been to approximately 16 different chiropractors on a regular basis over the years. I also have had 3 back operations on a total of 5 different discs. Many of the chiropractor's I have been to were very good, but none of them quite match up to Dr. Randy. His office has very

modern equipment and technology and the staff is very friendly and welcoming. The first thing you get to do when you go into Dr. Randy's office, is you go on a hydro bed which is like a heated water bed with a gentle massage. When it's your turn you are loose and ready to be adjusted. Dr.Randy is very strong, but only use's enough force necessary for each patient. My 93 year old father and both my 9 year old and 12 year old have seen Dr.Randy and were thrilled with the results. The

He has significantly increased the quality of my life as I'm still very active athletically and have young children. I don't know if Dr. Randy is the best chiropractor in the country, but I do know that he's the best chiropractor I've ever been to in my 46 years of going. I hope Dr. Randy is happy here, because if he ever moves, I'm going with him. I'm not giving him up.
