Dear Physicians: Here are your monthly 'Tips' for March from the Clinical Documentation Improvement team. ICD-10 implementation is fast approaching so we need to step up our education by having multiple diagnoses in our monthly ‘TIP’. The topics are “Skin Infections and Reactions & Connective Tissue Disorders.’ These 'TIPS' are provided in effort to assist you as we move toward the new documentation requirements. They will be stored on the Physician Portal as reference tool. Please incorporate these “TIPS’ in your documentation starting now! Skin Infections and Reactions -Abscesses, furuncles, lymphangitis, cellulitis --Use 'acute' when appropriate --Laterality (right vs. left) --Location --Erythema multiform --Identify, when appropriate, Stevens-Johnson, toxic epidermal necrolysis, and overlap between these two --Identify percent of body surface involved Connective Tissue Disorders -Name the disorder (e.g., SLE, panarteritis, scleroderma) -Name the organs involved, as appropriate --Endocarditis or pericarditis --Respiratory --Esophagus --Muscles --Kidneys ---Name pathology ---Stage CKD Get ready for ICD-10 starting now! Baptist opens the first medically integrated
Y Healthy Living Center We are partnering with the Y in this exciting new communitybased initiative designed to increase personal engagement and accountability for health within the local community. Community members don’t have to belong to the Y in order to access the health-related programs and services offered through our new Healthy Living Center in Mandarin, which will be open every weekday. Offerings include a regular calendar of health screenings, classes and health education opportunities. We believe this increased access to health and wellness resources will be a win-win for the community, for the Y and for the growth of many Baptist Health services. For example, a person who comes in for a screening will have ready access to our care navigation for follow-up care, if needed. If someone is interested in a primary care relationship, they can be offered the opportunity to connect with interested providers in our network. Patients can also be referred to the Healthy Living Center by themselves or by a provider (such as patients newly diagnosed with diabetes), where they may take advantage of health coaching and nutrition education. Some may end up joining the Y to help them sustain a healthier lifestyle. We will be expanding our Healthy Living Centers initiative in the coming months and look forward to sharing more details soon.
Meet Our Y Healthy Living Center Coordinator Julie Schafer has joined Baptist Health as our Y Healthy Living Center coordinator. She previously developed and managed YMCA Healthy Living Programs for Brooks Health System. She has a double-major bachelor’s degree in Exercise Science and Health Studies from Miami University in Ohio, and a Master of Public Health degree from the University of North Florida. Julie is onsite at the Y Healthy Living Center in Mandarin each weekday between 8:30 am - 12:30 pm and 4 - 7:30 pm to answer health care related questions and assist with class registrations and physician appointments.
Our medical director Medical leadership of the Y Healthy Living Centers is provided by Aristides Sastre, MD, a family medicine physician with Baptist Primary Care. Dr. Sastre believes that empowering patients with information that they can apply to their everyday lives is at the foundation of good health.

For immediate release March 20, 2014 Media contact: Mitzi Saul, Senior Marketing Coordinator 904.407.6165 office, 904.803.6238 cell, msaul@communityhospice.com Dr. Elizabeth McCullough Joins Community Palliative Consultants To Practice at Baptist Medical Center Jacksonville JACKSONVILLE—Patients at Baptist Medical Center Jacksonville have a new palliative care specialist to support them during any stage of a serious illness with the hiring of Elizabeth McCullough, M.D. The board-certified internist and hospice and palliative fellow has joined Community Palliative Consultants as an associate medical director. At Baptist, Dr. McCullough and a team of healthcare professionals work with patients to help relieve symptoms, pain and suffering from any serious, complex or chronic condition. She also guides intensive discussions with patients and their families to help them establish goals of care and prepare them to make important decisions about their care. Dr. McCullough is the fifth physician to join Community Palliative Consultants, a program established by Community Hospice of Northeast Florida in 2010 to provide palliative consultative services. Community Palliative Consultants serves patients at four area hospitals, including Baptist Medical Center Jacksonville, UF Health Jacksonville and St. Vincent’s HealthCare Riverside and Southside campuses, with plans to expand into other care facilities. Before joining Community Palliative Consultants, Dr. McCullough worked for the Program of Allinclusive Care for the Elderly (PACE) in Portland, Ore. The PACE model of care serves the geriatric population in primary, hospice and palliative care. Her research experience includes Mayo Clinic Florida, where she was a special project associate. Dr. McCullough earned a Doctor of Medicine from the University of Miami School of Medicine in 2007, completed her internal medicine residency at Oregon Health and Sciences University (OHSU)/Portland Veteran’s Hospital in 2010, and a fellowship in hospice and palliative medicine from the same hospital in 2011. She received OHSU’s Humanism Award in 2009 and 2010. “We’re overjoyed to have Dr. McCullough return to Jacksonville and bring home her extensive training and experience in palliative medicine,” said Andrew P.
