Data Protection Bath and North East Somerset Council will store information given by yourself and your GP. This information will then be passed on to your chosen leisure provider or an officer within the Council’s Sport & Active Lifestyles Team. Your information will only be shared for the purposes of safe exercise prescription and eligibility for the service. More information will be collected from you for the purposes of project and exercise prescription evaluation. If you do not wish to have your information collected this may affect your participation in the programme. Following your exit from the programme (after 24 weeks), your personal information will be destroyed. If you should decide to take out a membership with a leisure provider you will be asked to complete new forms and this will be stored separately. This project is commissioned by NHS Bath & North East Somerset. As a requirement of the funding, anonymous data is sent to the NHS on a quarterly basis for monitoring purposes only.

Referral Pathway

Scheme Details Passport to Health is a well established referral scheme based in Bath & North East Somerset. It aims to help you take ownership of your health by encouraging you to participate in regular physical activity as a prescribed treatment alongside, or as an alternative to, any traditional medical approaches. The programme will inform you about what makes a healthy lifestyle and support you in making your lifestyle changes.

Referral to the scheme enables you to meet with our friendly referral consultants to discuss your health goals and develop an activity plan.

Motivational Lifestyle Programme

A lifestyle referral will be completed by your GP and sent to Sirona Care & Health’s Healthy Lifestyles Hub and forwarded on to the Sport & Active Lifestyles Team

A project officer from the team will call you to check your eligibility and advise you of your activity options

An initial consultation appointment will be arranged where your baseline measures will be assessed and your health goals agreed

As an integral part of the Passport to Health you will be invited to attend the Motivational Lifestyle Programme. The Motivational Lifestyle Programme (MLP) aims to help you: ■ identify behaviours that could be harmful to your health ■ understand why they could be harmful ■ address these behaviours to improve your health

You will commence a programme of activity and receive a mid-point, completion and 3 month post-programme review

From here you will be entitled access to:

Goal - Setting 12 weeks of subsidised Physical Activity • Free assessments and progress reviews • Either facility-based activities at a local leisure centre, group-based courses in a range of activities including walking, cycling, aqua aerobics and circuits, or up to 12 weeks support from a Community Activator.

12 week Activity Programme

2 x 6 week courses in either: Facility-based OR Community Activator

Emotional Health Body Image Healthy Eating Physical Activity Choices
