Bariatric Surgery

Can Be Considered to Keep Weight Off

DEAR DR. ROACH: I have had a BMI over 40 for the last 20 years, and I have not been able to lose weight or keep the weight off. My doctor prescribed Ozempic, which is helping, but as soon as I stop, all the weight creeps back on in a few weeks. I was always wary of surgery for weight loss, but recent studies seem to indicate better long-term health outcomes. What are your thoughts? — S.O.

ANSWER: Medical or surgical treatment for weight loss is not right for every person who is overweight. Many people take medications in the class called glucagon-like peptide-1 agonists, which includes semaglutide (Ozempic and Wegovy), liraglutide (Saxenda and Victoza) and tirzepatide (Mounjaro). But these medicines have the potential for harm, and an individual’s risks, especially conditions that can be affected by being severely overweight or obese, need to be carefully considered.

Surgery has far more risks than medication and requires the most careful consideration before receiving a recommendation of bariatric surgery, of which there are many types.

I am much more likely to refer a patient to one of my colleagues in bariatric surgery when a patient is very obese and when there are clear medical issues that I can expect to get better with surgery. These medical issues can include diabetes, obstructive sleep apnea, or nonalcoholic fatty liver disease. Most of the time, these conditions can be well-managed without bariatric surgery, but sometimes management is very difficult. In this case, consideration of bariatric surgery is

worthwhile.

Bariatric surgery is never the first choice in weight management. A comprehensive lifestyle intervention — with an individualized diet (ideally recommended by an expert such as a registered dietician or nutritionist), moderate exercise (with a goal of 150 minutes per week minimum), and regular meetings to help keep a person on track — is the right place to start. It is effective for many people if they maintain the behaviorial changes.

I have been prescribing some of my patients with the GLP-1 or GLP-1/GIP medications with good results, but as you mention, if you stop taking them, they stop working. Unless you make a dramatic change to your lifestyle that you can keep up, the weight will return. Bariatric surgery does have very strong long-term weight-loss data, as well as dramatic reductions in diabetes when used for the appropriate people.

DEAR DR. ROACH: My shoulder pain seems to be getting worse (lack of cartilage), and it is interfering with sleep. I haven’t taken any pain medication for it, but I read that the best relief is from either aspirin or ibuprofen. I’d like to switch them back and forth — maybe two to three days with aspirin, then one day with ibuprofen (the most I can tolerate being 200 to 300 mg a day).

How much aspirin is OK to use in this way? — J.B.

A Comfort Food Comes Back for Super Bowl

Remember French bread pizzas? If so, those memories are likely nostalgic, comforting, cozy, crunchy, soft and warm, with that aroma that made the whole house smell good.

The French bread pizza is a 1960s invention. The story goes that a Cornell University New York food truck owner named Bob Petrillose (aka “Hot Truck Bob”) invented French bread pizza. These first French bread pizzas were called PMP or “Poor Man’s Pizza” and were well-loved by the late-night student crowd. Eventually, in the 1970s, this style of pizza flourished in popularity with Stouffer’s French Bread Pizza of freezer aisle fame.

While the frozen variety was standard 1980s afterschool snack and Friday night fare, the homemade PMPs are still a hit for anyone who wants an easy and inexpensive way to get dinner on the table in under 30 minutes any day of the week.

Preheat oven to 400 F degrees. Line a baking sheet with parchment paper. Using a serrated knife, cut French bread loaf in half lengthwise through the center. This will give you two long pizza shells. Use your hands to press the center of each piece down a bit. Deflating the bread will give you plenty of room for filling while keeping the bread from getting soggy while it’s in the oven.

In a small, microwave-safe bowl, melt together the butter, garlic powder, salt, pepper and 2 teaspoons Italian seasoning. Stir to combine. Brush the melted butter evenly on top of the bread.

Put a coffee filter into a small strainer, and place over a small bowl. Pour 1 (8-ounce) can of tomato sauce in the coffee filter contraption to drain for 10 minutes to remove excess liquid. Mix the drained sauce with 1 teaspoon each garlic powder and Italian seasoning and 1 to 2 teaspoons of sugar, according to your personal taste. You can enjoy this recipe even if you’re not watching the big game. Or only watching the commercials and halftime show and feeding the masses, like me. Trompe L’oeil

The term “trompe l’oeil,” meaning “fool the eye,” is an art style that was popular in the 19th century. By then, artists and audiences had enjoyed optical illusions for thousands of years. The original trompe l’oeil style dates back much further.

An ancient Greek story tells of the legendary painter Zeuxis, whose still-life paintings were so realistic that birds would fly down to peck the grapes. Another painter, Parrhasius, challenged him to a contest and showed him a painting covered by curtains. When Zeuxis tried to pull the curtains back, he realized that they were, in fact, part of the painting! This made Parrhasius the winner of the contest.

The set of shelves shown here looks like it is filled with books and figures, but it’s a trompe l’oeil screen! The shelves and their contents are printed on wood panels. It’s a creation by Piero Fornasetti, an Italian designer who worked from 1935 to the 1980s. He brought his skill and sense of humor to furniture, ceramics and more. His work often included optical illusions or realistically detailed images in surreal compositions. The screen sold for $9,100 at Palm Beach Modern Auctions in Florida.

What is (or was)

WYVERN?

Technically, Wyverns are cool two legged dragons from European mythology, but that’s not what I am here to write about. Instead, I want to discuss Wyvern the collectible card game, which enjoyed a brief existence from 1994 to 1997, so if you missed it and have no idea what I’m talking about, read on and learn about this little known relic of the 1990s trading card (TCG) boom.

Wyvern was published by U.S. Games Systems (USGS) out of Stamford, Connecticut in 1994. The company has been around for over 50 years and makes everything from tarot cards to motivational cards. This was just a year after Magic: The Gathering hit the market and interest in collectible card games was on the rise. Mike Fitzgerald, who later became known for his work with Wizards of the Coast, designed Wyvern and all the artwork was done by international fantasy artist Peter Pracownik.

The game was (or maybe still is?) played with two decks: one holds dragon and terrain cards and the other treasure and action cards. Billed as “The Game of Dragons, Dragon Slayers, and Treasure,” it could be played quickly by two people within a half hour. Like other popular TCG games of the era, Wyvern was heavy on strategy and fantasy; one interesting aspect of this game is all characters and terrains

were based on actual elements from ancient mythology. Unfortunately, the downside of coming into a market saturated with competition is not every game is going to make it, and Wyvern is an example of this. Magic: The Gathering and later Pokémon took the world by storm and after three short years Wyvern was pulled from production. Between 1995 and 1997, USGS put out only five editions: Premiere Limited, Limited, Phoenix, Chameleon, and Kingdom. Due to their narrow run, Wyvern trading cards are difficult to find today.

Believe it or not, some of the rarest Wyvern cards are errors; a misprint at the factory caused a few card backs to print with fronts from Magic: The Gathering - We actually sold one of these error cards at Bruneau and Co in February of 2022 for $800! However, for the most part Wyvern’s

collectible value is what most would consider low - But hey, that’s great news for anyone interested in playing the game or beginning a collection!

Saving strategies for young adults

When a person is young, saving money may be the furthest thing from his or her mind. After all, this may be a time to enroll in college or trade school, make a first big purchase, such as a car, or even get married. Thinking about establishing a solid financial footing for the future can take a back seat when life is filled with so many significant events.

But it’s never too early to start saving — even when saving seems to be an impossible task. Young adults should keep saving in mind and look to various strategies that can set them up for long-term financial security.

Set long-term goals

It’s easier to save when saving is attached to specific goals. While some may aspire to retire early, establish an emergency fund or to purchase a home, others may want to save for an overseas vacation. Motivation to save can make it that much easier to do so.

Determine where you spend the most

Saving money on smaller purchases will add up over time, but to really build a robust savings, figure out your biggest expenditures and how you can cut back to pad your savings. The Logic of Money reports that the average American spends more than 60 percent of their income on housing and transportation. Figuring out how to cut costs in these categories can be a great way to save.

Use cashback apps

Young adults are tied to their digital devices. Why not make them work for you? Free cash back apps give you money back for various purchases. Ibotta and Dosh are just two cashback apps available. Some can be linked directly to a credit or debit card to have passive income deposited directly. With others, you can cash out as a direct deposit or via a payment app like PayPal.

Set aside one-third of your income

Make it a point to put away $1 for every $3 earned into a savings account, advises U.S. News & World Report. That is a good measure for establishing a rainy day fund. If you don’t trust yourself to transfer the money, have a set amount automatically deposited from your paycheck into a designated savings account.

Treat credit cards like using cash

The “buy now, pay later” option is an attractive trap to fall into. Using credit cards often is a safer way to pay merchants, because you’re risking others’ money rather than your own with a debit card. However, using credit can make it challenging to visualize what you’re actually spending. Do not purchase more than you can pay off within each billing cycle. Set account alerts on your phone to let you know when you’ve hit your budgeted credit card spending limit. Resist the urge to open and use too many cards. Young adults can begin saving early with some conventional and highly effective strategies.

Much like the cost of a loaf of bread or a carton of eggs now costs consumers considerably more than it did a couple of years ago, the price to heat and cool a home has risen considerably. Various factors, from climate-related events to supply chain issues to the Russian invasion of Ukraine, have been cited as contributors to the rise in utility costs, which is not just a North American problem. Following pandemic-induced lows in 2020, natural gas prices have risen consistently, even during off-peak months, over the last year-plus. The cost of natural gas that’s delivered through pipes was up 24 percent in February from the year prior. Electricity has gone up as well. According to Choose Energy, an energy reporting resource, electricity rates have risen across the 50 states in 2022 by anywhere from 1.7 percent over 2021 (Alaska) to 46.1 percent (Maine). The national average increase is 11.3 percent. CBS News reported in 2019 that Americans are paying up to 30 percent more on water and wastewater bills in less than a decade. Water and sewer bills are rising faster than inflation rates, having increased for an eighth consecutive year in a study of the country’s 50 largest metropolitan regions. People concerned with the rising costs of utilities may have to be creative. Running appliances during off-peak hours; turning off lights and unplugging devices when not in use; investigating solar power; and investing in water-saving faucets, shower heads and toilets can help individuals curtail their energy consumption.

2. The DeFranco Family. Their best known hit was “Heartbeat, It’s a Lovebeat,” in 1973. They were on Dick Clark’s “American Bandstand” nine times.

3. The J. Geils Band, in 1981, on the album of the same name. The song charted around the world.

4. Donny Osmond, in 1976. His cover of “C’mon Marianne” was the one song from the album released as a single, and it only went to No. 38. The Four Seasons’ original 1967 release had climbed to No. 9.

• On Feb. 22, 1879, Frank Winfield Woolworth opened the Great 5 Cents Store (later Woolworth’s) in Utica, New York.
