'What's in a Word' Family Game

What do you think of when you hear the word "table"? In your mind, do you see the large rectangular piece of furniture in your kitchen? Your fourth-grader might picture a multiplication chart, and your sixth-grader a list of contents at the beginning of a book. Each person's frame of reference develops from a perception of things based on the information we know and the things familiar to us. In a typical family, it's not unusual to find one person's frame of reference totally opposite from another's, which often leads to the familiar, "I don't understand where you're coming from!" Here is a fun word game to help the whole family appreciate differences in perception. Give each family member a piece of paper and a pencil, and five minutes to write down all of the pictures that pop in his or her mind when a particular word is heard. Then compare lists. For example, try the word "copy" and see what comes up. Did you write that it means to duplicate on a machine? Maybe Grandmother wrote copy

means to draw, trace or write the same thing over. If Dad is a newspaper writer, copy is the words or manuscript he writes. If Mom is a police officer, to copy means to understand. Or, think of the various meanings of the word "key." In addition to figuring out "key" meanings to this game, one person might be dreaming of the warm weather in the Florida Keys this time of year, while another who plays an instrument thinks of the musical key of a favorite song. A key to a road map helps understand its symbols, and a car key gets you on the road. Discuss factors that may have made your definitions different, such as the person's age, work or school environ-

ment. Continue with other words. The No-Cry Challenge: Tears are shed for many a reason. Unbridled joy. A particularly raucous round of laughter. Heartbreak. The ache of loss. And humans are a curious lot. There are times when we’ll go into absolute denial to avoid tears, but other times we rush right in. Case in point: tearjerkers. Some movies come at you right from the start (the entire first 10 minutes of “Up”) and some wait for the end to twist the knife (when Miguel sings with his great-grandmother “Coco”). My Girl — The death of a child is no easy thing, but in this story of a girl named Vada Sultenfuss, living with her single dad in a funeral home, it’s heartbreaking. If you can get through the scene where Vada loses it at Thomas J’s funeral because he doesn’t have his glasses, I am not sure you are human. Inside Out — A quintet of emotions interact inside the mind of 11-yearold Riley in this Pixar classic. But it’s Riley’s forgotten imaginary friend Bing Bong who steals the heart in a loving act of sacrifice that will have you weeping buckets of candy tears. Steel Magnolias — In this touching story about a group of strong Southern women, there is more than one poignant moment, but for millions of ugly criers, it’s the scene where

M’Lynn delivers her monologue in the graveyard, bemoaning daughter Shelby’s lifelong ill health. Selena — The Tejano superstar captured the hearts of a nation. Her tragic story is told with Jennifer Lopez in the lead role, and when she doesn’t catch the white rose thrown to her on stage at the end, you’ll never hear “Dreaming of You” the same way.

DEAR PAW’S CORNER: For some reason this winter, my new house has been invaded by spiders. They are building webs in every corner of the house and basement, and every day I’m knocking down another web with my broom. I’m worried about my cat, “Jameson,” being bitten by a spider. What do I do if this happens? — Janine S., via email DEAR JANIE: I’m a huge fan of spiders, because they are one of nature’s best pest control services, trapping and disposing of many other tiny critters. They probably have much more to fear from Jameson than the other way around, because housecats see spiders as interesting prey. Still, a spider invasion is not much fun to deal with. Look for something that is attracting spiders to your home. Do you have wood or leaves piled up against the foundation? Are there cracks in the foundation? Is your basement or attic damp? How clean are your HVAC vents and ductwork? Addressing the source of the problem can cut down quite a bit on the number of spiders invading your house. As for Jameson being bitten by a spider — if you are concerned, do the same thing you would do when checking for evidence of fleas or ticks. Every evening put Jameson on your lap and run your hand over his coat, backwards. This lifts the hair away so you can see his skin. A spider bite on a cat will look similar to a spider bite on a human. If you do see a spider bite on Jameson, the best thing to do is observe. If he is behaving normally, not distressed or pawing/licking the bite, he’s probably fine. If you have any concerns, contact his veterinarian. St. Patrick’s Day is an opportunity for the world to celebrate Ireland’s patron saint and Irish culture with family, friends, food, and fun. Many St. Patrick’s Day traditions have been passed down through generations. This includes various foods and recipes that are native to Ireland that have spread around the globe thanks to the vast Irish diaspora. When dishing up new traditions at home, try this recipe for “Traditional Irish Colcannon” courtesy of Kerrygold, producers of Irish butters and cheeses. Colcannon is a mashed potato recipe made even more savory with the inclusion of cabbage and other ingredients. Serve with corned beef, Irish bacon or Irish stew. Traditional Irish Colcannon Serves 8 2 pounds red potatoes, cut into large chunks 3⁄4 cup milk 3⁄4 teaspoon salt 6 tablespoons Kerrygold Salted Butter, plus additional melted butter, if desired 1 cup chopped onion 6 cups finely shredded green cabbage (or one 10-ounce package) 1 cup (about 4 ounces) shredded Kerrygold Dubliner® Cheese or Blarney Castle Dubliner® Cheese

St. Patrick’s Day occurs during the Christian season of Lent; however, Lenten restrictions on the consumption of meat were lifted on this Feast Day, tying the celebration to a tradition of eating and drinking. The consumption of alcohol was traditionally frowned upon on St. Patrick’s Day and has become associated with the holiday over time largely due to the marketing efforts of beer producers.

Freshly ground pepper to taste Cook potatoes in boiling water about 20 minutes or until very tender; drain well and mash with skins on, adding milk and salt. While potatoes are cooking, melt butter in a large skillet. Add onion; cook 10 minutes, stirring occasionally, until very soft. Add cabbage; cook and stir for 5 minutes more or until very soft. Stir cabbage mixture and Kerrygold cheese into hot potatoes and season with pepper. Mound onto serving plates and make a well in the center of each. Pour a little melted butter into each well, if desired. Garnish to taste.

St. Patrick was most likely born in Scotland as a Roman citizen, but was captured and enslaved by Irish pirates and brought to Ireland at the age of 16. His original name was Maewyn Succat, which he changed to Patricius, stemming from the Latin term for “father figure,” after becoming a priest.

Tips to master a video job interview The adage “there’s a first time for everything” has certainly rang true throughout the COVID-19 pandemic. Life as the world knew it changed during the pandemic, and as a result many people found themselves in unfamiliar positions and settings. One unfamiliar position that job seekers had to grow accustomed to during the pandemic was interviewing via video conferencing apps like Zoom. Interviewing for a new job has long been considered a nerve-wracking experience, even for seasoned professionals. That anxiety did not necessarily disappear during the pandemic, but it may have taken on a new form as applicants were forced to present the best, albeit virtual, version of themselves when interviewing. Video interviews may become integral parts of the interviewing process in the future, as companies recognize how efficient video interviews can be. As a result, it can benefit professionals to polish up on their video interviewing skills. • Dress the part. Dressing for success still matters. Though in person interviews may not be in job seekers’ immediate futures, appropriate attire still sends the right message. Dress the part from head to toe, even if you expect to only be seen from the waist up. This saves you the embarrassment of being seen in sweat pants or other inappropriate attire should you unexpectedly need to stand up during the interview. • Beware of the background. Make sure the background behind you during the interview is clean and appropriate. Zoom offers a curated list of virtual backgrounds that can help job seekers make a strong first impression. If an existing space is fine serving as a background, clean the area prior to the interview. This creates the impression that you are organized and attentive to detail. • Use a laptop or computer. Smartphones have video conferencing capabilities, but it’s best to use a laptop or desktop computer when interviewing via video. Laptops and desktops are heavier and less likely to move during the interview and their screens are larger, giving job seekers a better view of the person they’re speaking with. If you must use a smartphone, keep the phone still throughout the interview. • Sit in a brightly lit, quiet room. A dark room may make it hard for interviewers to see applicants, and that can create a bad first impression. Find a well-lit, quiet room, ideally one that is away from the hustle and bustle of the household. • Use the mute button. One of the more common issues to arise from the Zoom boom has been some users’ failure to realize their microphones are not muted. CT Scan Unlikely to Miss Advanced Lung Cancer DEAR DR. ROACH: Is it possible for X-rays and a CT scan to miss advanced lung cancer? My brother worked two days before he died. He went to the hospital, was admitted to hospice and died the next day. I wonder if he knew at least a year ago, but didn’t tell anyone. He said his scans showed pneumonia. His son was called to the hospital and spent the day with him until he died. During his last day, my brother told him that he had lung cancer and that it was not diagnosed earlier. — N.W. ANSWER: I am very sorry about your brother. Lung cancer, especially early lung cancer, is often missed on a regular chest X-ray. Pneumonia can accompany early lung cancer, and the pneumonia can hide the cancerous mass, but experienced radiologists should be able to see something. Also, experienced clinicians should check an X-ray weeks after a pneumonia to be sure there is no cancer in a person at risk, such as a current or former smoker. Advanced lung cancer is only very rarely missed on X-ray, and should essentially never be missed on a CT scan. While I can’t say it’s impossible, it’s much more likely that a year ago he was told that he had lung cancer and did not say anything, perhaps to spare his family’s feelings, or perhaps he didn’t want the sympathy and attention that accompany a diagnosis such as advanced lung cancer.

DEAR DR. ROACH: What does a person do when symptoms are vague and hard to explain? I am an 82-year-old woman. I started feeling discomfort in one breast about three weeks ago. It felt like irritation from a bra. I did all the things I thought I should, and three weeks later the symptoms are the same or slightly worse. Nothing looks or feels different when I touch it. Do I see my primary doctor (he’s never seen me naked), or find a gynecologist? How do I present this issue? I feel like a fool. — V.B. ANSWER: Both clinicians should be able to appropriately evaluate this concern. You should go to whoever you are more comfortable seeing. What you should not do is ignore it. In your particular case, it’s unlikely that your symptoms are because of something serious. Breast discomfort is common and only rarely due to breast cancer, which is what you must surely be concerned about. You should NEVER stop yourself from seeing your doctor for fear of not being taken seriously. Primary doctors like me and gynecologists are very familiar with women coming in with breast symptoms, and we take them seriously. Clinicians know how justifiably concerned women are about breast cancer. Most often, a woman can be reassured with a careful history and physical exam; other times, a mammogram or ultrasound may be ordered. You don’t want to regret missing that visit to the doctor. It is often said that the only foolish question is the one that is not asked. This situation is analogous: It would be foolish for you not to go. While the overwhelming likelihood is that you will be reassured, in the unlikely event there is something wrong it’s far better to know about it earlier than later. Table, or Art? When is a pile of rocks considered art? An unusual table made of glass and “stone” was offered in an Andrew Jones auction in Los Angeles, with an estimated price of $2,500 to $3,500. It didn’t sell, but other related sculptures by the artist, Woods Davy (1949), have sold from $1,000 to $7,000. He first collected natural stones and turned them into sculptures without altering the shapes. Then he started making “stones” that look like they came from a riverbed using concrete, metal and glass. He positions them in impossible, strangely balanced shapes, which are held together by concealed steel rods and adhesives. This table is 22 inches high with a round glass top 42 inches in diameter. Poking through the glass is a rock with its top 28 inches from the floor. He used old natural forms in a new unbalanced way, which is known as postmodernism. That is the name of an unusual period of art developed in the 1950s that fuses past styles with the look of modern magazines, films and other unexpected sources. But is this just a great table? Or is it art? *** I have a wooden table that is marked “Larkin Soap Company.” Does that mean it was used in the company office or was it made by a soap company? How old is it? John D. Larkin (1845-1926) worked for a soap factory in Buffalo in the 1860s. He became a partner and moved with the company to Chicago. He met and married a girl from Buffalo, and they moved back to their hometown in 1875. There he started his own company, John D. Larkin, and sold “Sweet Home” soap. By 1881, he had a full line of related products and gave a free colored picture card with each bar of soap. Cards were not enough, so he started giving better premiums including, hand-

March is Endometriosis Awareness Month

What is endometriosis? Endometriosis affects 1 in 10 reproductive-aged women across all races. Endometriosis occurs when the endometrium, or lining of the uterus, grows outside of the uterus in the pelvis and abdomen. Endometriosis can grow on the ovaries, fallopian tubes, intestines, bladder and lining of the pelvis, and has been identified in almost every organ of the body. Endometriotic implants grow in response to estrogen and can trigger an inflammatory response in the pelvis. This inflammation is what causes painful symptoms. What are the symptoms of endometriosis? The classic presentation of endometriosis is painful periods. Pain typically begins before your period and continues until after the period is over. Patients with endometriosis often describe pain in their lower abdomen that travels to their back and down their legs. Other common symptoms include pain with sex, pain with bowel movements, painful urination and pelvic pain outside of periods. Endometriosis can also cause infertility. Up to 7 in 10 women with pelvic pain and infertility will have endometriosis. Finally, some

patients with endometriosis are asymptomatic or have very mild symptoms. The severity of symptoms does not correlate with how much endometriosis is present at the time of surgery.

Staying Healthy

How is endometriosis diagnosed? Endometriosis is underdiagnosed because the only way to definitively confirm endometriosis is with a biopsy performed during surgery. On average, patients see seven doctors over seven years between symptom onset and diagnosis. In the primary care setting, endometriosis is a clinical diagnosis based primarily on history and physical exam. There are no accurate blood tests for endometriosis however medical providers can use imaging (such as an ultrasound) to narrow their diagnosis for endometriosis. Medical professionals can presume endometriosis in patients with classic symptoms and begin treatment without surgery. The more symptoms a patient has, the more likely they will have endometriosis at the time of surgery. How is endometriosis treated? Patients with painful periods should start by taking non-

by Christine Foley, MD steroidal anti-inflammatory drugs (NSAIDS) such as ibuprofen, naproxen or celecoxib. We recommend starting two to three days before your period begins and continuing through the entire cycle. If your pain does not improve, your doctor may recommend a hormonal medication to help relieve your symptoms. Hormonal options include the combined (estrogen and progesterone) birth control pill, progesterone only pill, depo provera shot, the progesterone intrauterine devices (IUDs), and gonadotropin-releasing hormone (GnRH) agonists and antagonists. Partner with your provider to review the risks and benefits of each method and to understand which options are best for you.

Who should consider surgery for endometriosis? If your symptoms do not improve with medications, surgery may be the next treatment option for you. The goal of surgery is to both diagnose and treat endometriosis. During surgery endometriosis implants are removed and/ or destroyed. Depending on your life stage, your surgeon may recommend fertility sparing surgery or removal of your uterus (hysterectomy) with or without removal of the ovaries. Research shows long-term symptom relief following surgery for endometriosis. However, endometriosis can recur after surgery, and recurrence is more common with fertility sparing procedures. Typically, this surgery is performed through a minimally invasive approach (either laparoscopic or robotic) and does not require an overnight hospital stay. Surgery for endometriosis can be very difficult due to the scarring endometriosis causes. Consult with your provider to ensure your surgeon is experienced with endometriosis surgery. The goal of an expert endometriosis surgeon is to educate our patients using evidencebased research and patient reported outcomes in order to give our patients the best qual-

Top 5 healthcare trends and predictions for 2021 By Varsha Rao, Nurx CEO

The challenges and tragedies of the past year are well-known, but amidst the hardships of 2020 some hopeful signals have emerged in healthcare. Patients and the people and systems who care for them have been forced to do things differently this year, and many of these experiments will be with us to stay. These are some trends that will strengthen and take shape in 2021.

#1 Stakeholders embrace asynchronous Payors, providers and other industry stakeholders who may have been reluctant to engage with async models in the past have been won over in 2020. The pandemic accelerated the understanding that async can safely and efficiently care for patients at scale. Providers who waded into async out of necessity during Covid have found that it allows for less rushed, more direct communication with patients that in many cases results in better care, while increasing provider flexibility and quality of life. Payors are realizing telehealth offers smart savings compared to legacy systems. State laws are coming along too — in May Maryland changed legislation allowing for asynchronous telemedicine to be accepted, and we expect more states to modernize in this way.

#2 Decrease of PCP as gatekeeper Today’s young adults were already less likely than those of previous generations to have a primary care provider, and this trend will grow as PCPs close practices and people grow accustomed to a la carte care. Circumstances of 2020 have led people to get care formerly channeled through a primary provider directly, in a diverse array of settings. Covid swabs at drive-through clinics, flu shots at supermarket pharmacies, and prescription medications through telehealth, combined with increased utilization of home mon-

itoring devices and wearables, have transformed patients (for better or worse) into their own care coordinators.

#3 Patients moving away, literally, from brick-and-mortar care Simultaneous with a decrease in PCPs as the first line of care, patients are on the move. With remote work allowing many professionals to live anywhere, some have chosen to move closer to family or to try out a new location. With urban centers less of a magnet during Covid closures, patients are moving to more affordable rural areas or less crowded resort communities. All of this means a break with their existing brick and mortar healthcare systems, and makes telehealth an attractive alternative as it allows them to get care from anywhere and also maintain continuity of care when they’re unsure where they’ll move next. Great for people like me who frequently move and don’t have a regular obgyn.

#4 Mental and behavioral health go virtual Mental healthcare has been an area of medicine with some of the biggest financial and logistical barriers to patient access — therapist appointments were often hard to get, hard to get to, and expensive, with insurance coverage generally limited or nonexistent. But the pandemic swiftly pulled mental and behavioral healthcare into the future, as more people needed this support at the same time as providers were forced to adopt remote care technology. While in some cases it may be preferred by a patient or provider, it’s often not necessary for mental healthcare to be delivered in person, so we may see that remote care will be the default rather than the exception going forward.

#5 Patients turning to telemedicine first Until recently many patients looked to telemedicine as a stop-gap measure — a way to get a prescription when you

need it quickly and your doctor doesn’t have an appointment available, but only as a one-off and not a replacement for that in-person provider relationship. But we see this order reversing, with patients looking to telemedicine first and only seeking out in-person care for conditions that absolutely require it, and this is especially true for younger adults. A recent survey of 2000+ people conducted by HIMSS found that members of the Millenial and Gen Z generations “overwhelmingly” prefer, and would actually pay more for, online care. Rather than seeing telemedicine as transactional, they’re increasingly experiencing it as relational, with the expectation that they can receive not only medication, but answers and ongoing care from responsive real providers on the other end of their devices. In many ways it’s a hard time to be a human, but it’s a truly exciting era to work in healthcare. I’m optimistic that the trends defining next year will result in more empowered patients and modes of care that are more nimble, effective, and satisfying for those who care for them. There have been several incarnations of DC Comics’ Superman since the Christopher Reeve era, the most recent having starred Henry Cavill in 2013’s “Man of Steel,” followed by “Batman v Superman” and “Justice League.” Cavill, sporting long white hair, is the actor who plays the monster hunter in the hit fantasy series “The Witcher” on Netflix. “The Rookie’s” Brandon Routh was cast as Clark Kent/Superman in the 2006 film “Superman Returns,” with Kevin Spacey, but it didn’t deliver at the box office, so he faded into obscurity for a while. Fortunately, the DC Universe must like him because he went on to play either Superman or The Atom in several CW series like “The Flash” and “Arrow.” In December, Routh, now 41, was cast on the third season of the ABC drama “The Rookie” as an officer with polarizing views and actions. The role is recurring, but it gives him a chance to play against type, the opposite of a superhero. *** Are Ryan Reynolds and Blake Lively still married? “The Rifleman” is a classic Western that ran for five seasons from 1958 to 1963. It starred Chuck Connors as a rancher and Johnny Crawford as his young son, Mark. The number of shots that Connors fires during the opening credits appears to my ears to be 13.

Brandon Routh in “Superman Returns” Crawford is still acting, but not so much these days. He’s about to turn 75, and his most recent credit is another Western, a 2019 movie titled “Bill Tilghman and the Outlaws” (aka “The Marshal”), which can be rented through Amazon Prime. Crawford was one of the original Musketeers with the Walt Disney Company before he appeared on “The Rifleman.” He subsequently guest-starred on such notable series as “Hawaii Five-O,” “Little House on the Prairie” and “Murder, She Wrote.” He and his on-screen father Connors remained close friends, and Crawford delivered the eulogy when Connors passed away in 1992.
