Seniors Outreach Beaumont FCSS Seniors Outreach 780-929-1006 • Provides information regarding programs and services for seniors and families caring for seniors. • Assists with the completion of forms for various government programs. Please phone for an appointment. • One-on-one support is available to connect seniors and adults with disabilities to available resources in their communities. • Subsidized counselling available to eligible residents. 2

Outreach Home Support is a year-round light housekeeping service that clients may access on a weekly or monthly basis. Call 780-929-1006 for more information.


Federal Government – Benefits The website for federal benefits is canada.ca. Call 1-800-206-7218 or visit Service Canada, located in Millbourne Market Mall. Address: 148 38 Ave NW & Millwoods Road, Edmonton, AB T6K 3L6. Canada Pension Plan (CPP) • If you have contributed to the CPP, you can apply as early as age 60 for benefits. • You need to apply at least six months before you want to receive your CPP retirement benefits. Old Age Security (OAS) • If you are 65 or older and have lived in Canada for a minimum of 10 years after your 18th birthday, you will be sent a form to apply for OAS six months before you turn 65. • You do not need to provide proof of birth with your application. • If you were born in Canada and have lived in Canada your whole life, you do not need to prove your legal residence status.

Guaranteed Income Supplement (GIS) • The GIS is an additional monthly benefit for seniors who receive OAS pension and have a low income. • You must apply for this benefit. • If you are married or in a common-law relationship, you will be asked to send documentation as proof of your relationship. • You do not need to reapply yearly for GIS as this will occur by filing your tax return. • If you marry or separate, it may affect your benefits. Your GIS is automatically added to your OAS cheque. Veteran Affairs 1-866-522-2122

Spousal Allowance • If you are between 60 and 64 and your spouse or common-law partner is receiving OAS, you may qualify for the allowance to help bridge your income until you turn 65. • You must apply for this allowance.

Seniors Drop-in Drop-in and meet other club members, find out what’s new in the community and enjoy a coffee. Second Glance Clothes Ltd. 5607 B - 50 St., Leduc, AB T9E 6Z7 780-986-1450 Hours: Monday to Saturday, 9 am to 6 pm Second Glance accepts donations of clothing and household items, utilizing these items for resale in the store. Proceeds generated from the sale of donated items provide the basis for the donation program. Second Glance’s donation program assists many local community groups. Applications for the donation program are available at the store and may be picked up during regular store hours. Volunteers are crucial to the operation of Second Glance.

Beaumont Sports and Recreation Centre 5001 Rue Eaglemont, 780-929-7946 Seniors (60+) are eligible for free access to the pool at the BSRC (Aqua-Fit) between 12:00 pm-4:00 pm every day. This includes Aqua-Fit swim classes on Tuesdays, Thursdays, and Saturdays. Beaumont Public Library The Beaumont Public Library provides free membership to residents of the City. Health and Support Health Link Alberta 780-408-LINK (5465) Registered nurses are available to provide health advice and information 24 hours a day, seven days a week. They provide comprehensive information on services and programs offered by Capital Health. Primary Care Network (PCN) 780-986-6624 The PCN is a community-focused initiative developed by Capital Health, Alberta Health and Wellness, and physicians from the communities of Leduc, Devon, Beaumont, and Calmar so patients can have improved access to health services. The physicians identified gaps in services and came up with the following services: • The After Hours Clinic • A Seniors Clinic (referral only) • A Chronic Disease Management Program, which includes a pharmacy consultant, a social worker, and a registered nurse. •

The PCN provides ongoing bereavement support groups and is looking at developing more groups to support people dealing with chronic disease.

Leduc Community Mental Health Clinic 780-986-2660 Therapists provide professional help for people experiencing severe or persistent mental disorders or moderate to severe emotional distress. Distress Line 780-482-4357 Offers confidential, non-judgmental supportive listening service for anyone in distress. Help is also provided for suicide and violent situations. Devon General Hospital 780-342-7000 101 Erie St. South, Devon, AB T9G 1A6 The Devon General Hospital is an acute care and continuing care facility. Services include a laboratory and radiology services and a 24 hour emergency service.

Alzheimer Support, Edmonton 780-488-2266 The Alzheimer Society of Alberta and the Northwest Territories is a great source of information for caregivers who are dealing with a family member with Alzheimer’s disease or a related dementia.

Additional health services include public health, home care, adult day program, diabetes education, speech language, rehabilitation services, community nutrition, and mental health.

A home visit can be arranged through the First Link program to provide information and set up support services.

Community Care Access, Home Care 780-496-1300 Community Care Services provide an integrated system of health and personal support services to clients based on assessed need and refers to the nearest home care office. Services are provided in clients’ homes, supportive living environments, and continuing care facilities. Requests for equipment only are forwarded to Aids to Daily Living. Emergency Response Support Services Lifeline 780-471-2175 Lifeline is an easy-to-use personal response service that ensures older adults living at home get quick assistance when needed 24 hours a day, 365 days a year. Telecare 780-431-3630 Help is a push-of-a-button away. Highly trained operators immediately contact the appropriate help 24 hours a day.

Alzheimer Support, Leduc 780-980-7115 The Leduc Alzheimer support group is designed for caregivers who want to gather information, share experiences, and receive support to cope more effectively in caring for someone with Alzheimer’s disease or a related dementia. Meetings are held on the second Monday of each month at 10 am at the Peace Lutheran Church (4608 48A St. Leduc). Dates are changed if Monday is a holiday. Caregivers can become isolated, lonely, and burned out when they are constantly monitoring a person with Alzheimer’s disease. It is important for caregivers to contact Home Care to periodically arrange respite so they can have a break.


Leduc Adult Day Support Program at Leduc Alliance 780-980-3064 5503 Blackgold Drive, Leduc, AB Call for information or to make a referral. The Telford Day Program provides structured recreation, socialization, exercises, and a nutritious meal to dependent adults who live in Leduc and surrounding communities. The program runs Mondays and Fridays from 9:30 am to 2:30 pm Northern Alberta Brain Injury Society (NABIS) 780-477-7575 The NABIS is a non-profit organization that supports northern Albertans and their families who have been affected by brain injuries. The society’s mission is to promote an optimal lifestyle for individuals affected by a brain injury.

Canadian National Institute for the Blind (CNIB) 780-453-8310 12010 - Jasper Ave., Edmonton, AB Low vision rehabilitation services are available at the CNIB. Call the Edmonton coordinator of the Seniors Vision Loss Program for more information. Leduc Sightseekers Low Vision Group 780-980-8152 The Leduc Sightseekers support group meets at Telford House the first Tuesday of the month from 10 am to 11:30 am This group provides information, peer support, and connection for those coping with vision loss.

Does one arm drop down? • SPEECH: Say a short phrase and check for slurred or strange speech. • TIME: If the answer to any of these is yes, call 911 right away and write down the time when symptoms started.

Self-contained apartments or lodge accommodation is available. Apartments are located in Leduc, Devon, Calmar, Beaumont, Thorsby, and New Sarepta. Lodges are located in Leduc and Warburg. Tenants/residents pay 30 percent of their gross monthly income for rent. Additional services or supports are supplied at an extra cost.

Seniors waiting to take advantage of housing options must complete an application form, have a medical form completed by their doctor, submit their previous year’s tax information, and go for an interview. Applications can be obtained from the office or the website leducfoundation.ca. Applicants who are approved are put on a waiting list by priority. The WeCan Food Co-op operates monthly to provide families and individuals with an opportunity to access nutritious food at an affordable price. A membership fee entitles people to purchase food from the Co-op. Safety Protect Yourself from Identity Theft Don’t disclose personal information about your finances, bank accounts, credit cards, social insurance, birth certificate, drivers licence, and passport numbers to anyone who can’t provide a legitimate reason for needing the information. Criminals can use your personal information to obtain credit cards, identification cards, mortgages, and loans in your name. Shred any unwanted personal information such as bank statements, credit card bills, unwanted receipts, cheques, pre-approved credit applications, and old tax returns. Check your credit report yearly. Some Indicators of Suspicious Behaviour • Promise of a valuable prize in return for a low cost purchase. • Calls, emails, or mailings offering international bonds or lottery tickets, a portion of a foreign dignitary’s bank account, free vacations, credit repair, or schemes with unlimited income potential. • Large sums of money taken from your bank account. • Problems with your credit card statement or credit rating. • If you suspect you are being followed, cross the street, go to the nearest home/service station/ business, and call the police. • Be cautious of any stranger approaching your vehicle. If you feel you are in danger, press the horn repeatedly in short blasts as this will attract attention.

How To Report a Crime • If it is an emergency or the crime is currently in progress, CALL 911. •

If you wish to remain anonymous, call CRIMESTOPPERS at 1-800-222-TIPS (8477). Crimestoppers is open 24 hours a day, seven days a week. Call when you are alone to maintain your anonymity. Crimestoppers does not subscribe to call display.

Call your local RCMP detachment at 780-929-7410. Ask to speak to an officer about a crime and an RCMP member will talk to as soon as possible.

The Beaumont RCMP detachment is located at 5501 Magasin Ave. If you cannot make it to the detachment, ask for the officer to come to you. Every effort will be made to help you. The RCMP will not disclose the identity of people who report a crime without their consent. Tips When Reporting a Suspected Crime • Identify the crime. • Provide the date, time, and location of the crime if possible. • If known, supply the suspect’s name, address, place of work, and phone number or any portion of this information. •

Give a physical description of the suspect (sex, age, race, accent, facial features, height, weight, hair colour and length, glasses, scars/marks, tattoos, facial hair).

• If a weapon is involved, describe it and its location.


• If a vehicle is involved, describe it (the make, model, colour, and license plate information).

• Call the Kid’s Help Phone at 1-800-668-6868 or go to kidshelpphone.ca.

• If something was stolen, describe the item(s).

Your Personal Boundaries Have Been Crossed When: • Someone has asked you to do something that is uncomfortable to you. • You feel like you are too much trouble and need to get away from family or friends. • Someone close to you is intimidating you and you feel like you are walking on egg shells.

• You give in when someone forces you to do some thing you don’t want to do. • You’ve been hit, threatened, had your property damaged, forced to sign documents you don’t agree with or are being followed or watched. Estate Planning This information was obtained from the Legal Resource Centre, a non-profit society that works to provide Albertans with reliable information regarding their legal rights and responsibilities. The information contained in this section is not legal advice. Personal Directive A Personal Directive appoints a person or persons to make non-financial decisions for you while you are alive but when you do not have the mental ability to make decisions for yourself. The person you appoint as your “Agent� will be permitted to make decisions for you regarding health care, housing, and treatment. Alberta law does not allow anyone to automatically make decisions for you, not even your spouse or your family. By making a Personal Directive, you are appointing someone to make decisions for you based upon your expressed wishes. You should discuss your wishes with your Agent so your Agent will know what is important to you. A Personal Directive may only be used once you

have been declared mentally incapacitated and is only valid until you die or you are determined to have regained your mental ability. You may include in your Personal Directive whom will be responsible for deciding your capacity. You may contact the Office of the Public Guardian at 780-422-1868 to receive a form for creating your own Personal Directive. You can also print a Personal Directive form from the following website: alberta.ca/personaldirective You can also register your Personal Directive with the Office of the Public Guardian. Health Care Professionals can access the registry to find out if you have a Personal Directive and contact your Agent. Personal Directives must adhere to the Personal Directives Act in order to be valid.


Enduring Power of Attorney An Enduring Power of Attorney helps you look after your financial affairs while you are still alive and gives the person that you choose the right to act on your behalf regarding financial matters. You may create a document that takes effect immediately and continues on once you lose mental capacity, or takes effect once you lose mental capacity. “Mental capacity” refers to your ability to understand information and make informed choices. Generally, assessments by two medical practitioners are required to determine mental capacity. It is important to seek legal advice prior to creating an Enduring Power of Attorney, especially if your financial affairs are complex. You may also create your own Enduring Power of Attorney. In Alberta, this document must be in writing. It must be signed by you and a witness in the presence of each other. This witness must be an independent third party. An Enduring Power of Attorney must adhere to the Powers of Attorney Act in order to be valid.

Wills The purpose of a Will is to pass your belongings to the people that you want to have them after you die. Making a Will is voluntary and optional. Your wishes need to be in writing and must adhere to the specific conditions of the Wills Act (Alberta). You can have a lawyer draw up your Will. This can be costly, but is definitely more advantageous. Lawyers are skilled and have experience with complicated estates. You can also make your own Will by purchasing a kit or downloading forms from the internet. It is very important to follow all directions carefully in order for your Will to be valid. The registry agent will need to see the vehicle registration, insurance policy, and death certificate. The deceased person’s driver’s license needs to be given to the registry agent. Ensure the name of the deceased is removed from any car insurance policies by contacting the insurance company.
