A Grateful Family’s Words Dear Hospice & Community Care, to all the physicians, technicians, nurses and aides who helped our dear Judy along a dastardly, and deadly, cancer-stricken way: Please know that even though her last days made it difficult to laugh, lest put thoughts down on paper, that great big heart of hers just overflowed with appreciation and gratitude for you. Whether you guided her through treatments, gave her hope or helped her come to peace, she saw you as nothing less than a humbling team of healthcare heroes who met her tremendous aches with empathy and fought alongside her until she could fight no more. From the President and Chairman of the Board As this 2014 Annual Report goes to press, Hospice & Community Care marks its 35th year of meeting the end-of-life care needs for people in our community. Since March of 1980, Hospice & Community Care has provided quality hospice care and bereavement support to thousands of adults, teens and children in need of our care. Our mission now, as it was then, is to provide care and comfort to help patients and families live better at end of life. In 2014 we had the privilege of providing hospice and palliative care for nearly 3,000 patients and their families, most of whom received individualized care in their homes or in the senior care facilities where many of our patients live. Our Palliative Medicine Consultants physician practice supported 1,500 patients and families by providing relief from pain and symptoms that impact quality of life, offering a greater understanding of a patient’s illness and assistance with decision-making regarding treatment options. We are grateful to the thousands of families who entrust us with their loved ones’ care. From the very young to the oldest patients who have reached the end of their lives; for individuals pursuing treatment and those looking for quality, and meaningful care at end of life; and for their families there by their sides, we provided relief, compassion and trust. Our Pathways Center for Grief & Loss supported more than 8,000 adults, teens and children, and encouraged people’s inherent ability to heal and discern new paths on one’s journey through grief.

As we move into 2015 there are many new opportunities in our midst, including the privilege of caring for more patients and their families throughout our service area. To meet these needs, Hospice & Community Care is adding 17 new Hospice aide positions to our team who are specially trained in hospice and palliative care. The year ahead also challenges us to further educate the community on the benefits of palliative care and early hospice support for patients as well as their family members. As you will read in the pages of this Annual Report, Hospice & Community Care is exceedingly grateful for the kind and generous support we received during 2014 from all those who make it possible for us to continue to provide this vital care. We are eternally grateful for your continued dedication to our mission which truly impacts so many in our community.

What Makes Us Different Hospice & Community Care is dedicated to providing care and comfort to help patients and families live better at end of life. Home hospice care is—and always will be—the focus of Hospice & Community Care. Care is provided at home or at local senior care facilities, home for many patients, whenever possible. The Bob Fryer & Family Inpatient Center is available when another level of care is needed, and is the only freestanding hospice inpatient center in the area. Since 1980, Hospice & Community Care has been a leader in hospice care, based in the heart of the community.

2014 Highlights A Year of Reaching New MIlestones • Hospice & Community Care reached an average daily census of 63 patients and families served in York and Adams counties.
• Hospice & Community Care raised a record $3.1 million in philanthropy including $56,000 from the Extraordinary Give.
• The Pennsylvania Hospice Network (PHN) selected Dr. Joan K. Harrold, Vice President Medical Services, as the recipient of the Nancy Bohnet Award for Excellence, presented to an individual who exhibits integrity within the hospice community, excellence in patient care and high ethical standards in all aspects of their work.
• The organization’s Women’s Giving Circle celebrated its 10th Anniversary, having contributed $287,000 in funds to Hospice-related programs and services since 2004. • The Pathways Center for Grief & Loss created its Volcano Room through the generosity of Jan’s Circle of Friends.
• The 30th Hospice Labor Day Auction raised a record $476,000. • In coordination with Lancaster General Health, the Palliative Care Service received Advanced Certification for Palliative Care from the Joint Commission. • Palliative Medicine Consultants’ Palliative Medicine Clinic opened on Good Drive. • Hospice & Community Care participated in ‘National Take the Tour Day’, to present the Second Wind Dreams’® Virtual Dementia Tour (VDT) and mark National Alzheimer’s Disease Awareness Month.

Hospice Employees and Volunteers The Foundation… and Heart…of Our Care The organization’s 360 employees are the foundation that brings the highest quality care and dedication to meeting the needs of families at the most challenging time of life. Hospice & Community Care is also tremendously supported by nearly 1,000 volunteers who are truly the heart of our organization, and who make important contributions to the lives of our patients and their families every day. The organization’s robust Home Hospice Care program is directed from Hospice & Community Care’s facility located at 685 Good Drive in Lancaster. 75% of Hospice & Community Care’s employees work from the Good Drive facility, which is also used for staff and volunteer training and education. Inpatient care and support is provided by 70 Hospice staff members at the Bob Fryer & Family Inpatient Center’s 24bed facility, located on the Harrisburg Pike near Mount Joy, where care is provided when another level of care is needed. Twenty-three employees work from our location on St. Charles Way in York, where we are providing care and support for nearly 80 patients and families each day in York and Adams counties. Hospice & Community Care’s 124 Registered Nurses, more than half of whom are certified in hospice and palliative care nursing, provided direct patient care and support to caregivers, and worked closely with the patient’s physician to provide effective pain and symptom management.
Our 93 Licensed Practical Nurses and Hospice Aides assisted families by providing personal care under the direction of the Hospice RN case managers. Hospice & Community Care’s 25 Social Workers provided emotional support, counseling and assistance with community resources for families, helping them to cope with the stress of illness and increased care needs. The organization is privileged to have 11 Hospice Chaplains on staff who are specially trained in the spiritual needs of individuals facing life-limiting illness. Paul Grothe of York Finds His Voice Through Hospice Support Groups When Paul Grothe’s wife, Carolyn, was admitted to the hospital for complications related to cancer in February 2013, her physician said Carolyn would benefit from hospice care. A hospital social worker started naming local hospice agencies and Paul chose the first name he recognized: Hospice & Community Care. Today, he says he is glad he did. After Carolyn was discharged from the hospital to the couple’s home in York, a Hospice & Community Care nurse from the organization’s York office started making daily visits to the family’s home. When Carolyn had major problems with nausea, she spent four days in the Hospice & Community Care Inpatient Center near Mount Joy. “They were so helpful, both at the Inpatient Center and at our house,” Paul said. “At home, they would spend an hour or two at a time, and they were never rushed. They gave her whatever time was required.” But perhaps the experience with Hospice & Community Care that most caught Paul by surprise occurred after Carolyn passed away in Spring 2013. Months after her death, Paul read about a Men’s Breakfast and other support groups in Hospice’s Pathways newsletter.
After attending his first monthly Men’s Breakfast he became involved with that group, started attending weekly Hospice support groups and also special programs such as a class on meditation. He actively participated in Hospice support groups through October 2014. “What I found most helpful is that although I am a loner and a quiet person, for whatever reason, I found myself talking and sharing my grief in these support groups,” Paul said. “As I was finishing up the six-week Growing Through Grief support group last year, I started realizing, ‘I think God has a plan for me to start helping other people in their grief.’”
In Fall 2014 Paul accepted a volunteer position in the GriefShare program at his church, St. John Lutheran Church in York. “I never had a major problem with my grief, so when I started attending Hospice support groups I sort of looked at it as preventive maintenance,” he said. “I wouldn’t have believed you if you’d told me two years ago that I would end up going to support groups and sharing like I did. I really believe that that was all part of God’s plan to get me where I am today, which is helping other people with their grief.”

Palliative Care Helping Patients Live More Fully Kristina Newport, M.D., has seen the fear on people’s faces when they think, “My Kristina Newport, M.D. life or my loved one’s life must be near the end.” But Dr. Newport, a palliative care physician with Hospice & Community Care, wants people to understand that palliative care has a far greater reach. “Our goal with palliative care is to match up resources and to help the patient live as fully as possible,” Dr. Newport said. “Palliative care reaches beyond just the last weeks of life. Sometimes we are involved at a critical point in a person’s illness or at a stage
when burdensome decisions are being made. Our goal is to provide pain and care options that improve the quality of patients’ lives.” Dr. Newport is quick to mention one patient, a man in his late 60s who had multiple heart attacks and was slowing down due to heart failure. In talking with him about treatment options, Dr. Newport and his cardiologist asked about his priorities. He said two of his priorities—those things that improved his quality of life—were his grandchildren and watching Penn State football. “We literally developed his medical plan around the football schedule,” Dr. Newport said. “We worked out medical options that enabled him to go to one more Penn State football game with his family, then we worked out a medical plan that enabled him to feel well
enough to watch Penn State games with his family through the rest of the season.” After the football season, he made the decision to stop taking his IV medication and to turn off the implanted defibrillator that would have shocked his heart to bring him back to life if his heart failed. “These decisions were empowering to him, because he had the power to determine what his life would look like,” Dr. Newport said. “He chose what was important to him, and his wife and family supported him.” “We work with the patient, caregivers and entire family so everyone feels supported,” she said. “Nothing is taken away that would help an individual live better. Instead, it’s all about finding options that help patients live most fully.”
Hospice & Community Care is the only local hospice provider that has a Medical Services team of physicians and nurse practitioners, board-certified in Hospice and Palliative Medicine. In 2014, our Medical Services team made 13,000 visits to patients in their homes, local senior care facilities, hospitals, and in the Inpatient Center.

From Loss to a New Life One Volunteer’s Inspiring Story What Marie could not have known, as she participated in the Center’s Coping with the Loss of a Child program, was that another tragic loss was just two years away. In 2005, Marie’s husband, Charles, died in a work-related accident and the bottom dropped out of her world. “When you lose a spouse, your whole life changes,” says Marie, now 69. “You lose a lot of your self identity and the life that you had before.”

When Marie Roach’s 16-year-old grandson Heath, died in 2003 of leukemia she struggled with the terrible grief of losing a cherished grandchild. She sought help through Hospice’s Pathways Center for Grief & Loss, and found the comfort and support she needed.
Once again, Marie turned to the Pathways Center, and again discovered that the way forward lay in support of others. She learned that processing her grief did not require letting go of her life with Charles, but rather embracing the joys of her past and incorporating them into a new life, with new activities and fulfillment. “I became determined to find something that would fill up my

life after I lost Charles,” says Marie. And that something became a richly rewarding life as one of Hospice’s most active and loyal volunteers. She participated in volunteer training in 2006, then began processing electronic medical records for Hospice patients. A year later, she trained to become a Volunteer Bereavement CoFacilitator. Today, she co-facilitates the Growing Through Grief group and assists with various support groups at the Pathways Center and Hospice events. “Sometimes people ask me if volunteering at Hospice is depressing, and I say, ‘No!’ It’s uplifting, because I feel this is what my life has prepared me to do,” Marie says. “As a volunteer, I have met and been inspired by so many of my fellow volunteers .” “My husband inspires me to do what I do today,” she says. “I will never forget my relationship with him.”
Supporting the Community on the Journey Through Grief The Pathways Center for Grief & Loss is a jewel, the resources of which our community is priviledged to have available and which is a free resource to the entire community, regardless of whether your family utilized hospice services. Eight professionally-trained bereavement counselors specialized in grief and loss are available to support and encourage the inherent ability to heal and discern new paths on one’s journey through grief.

Jan’s Circle of Friends Supports New ‘Volcano Room’ Providing a Unique Place for Grieving Kids On a very personal level, Susan Van Zant understands the importance of helping children grieve the loss of a parent. Van Zant has cared for her two nieces and two nephews since her sister died at the hand of domestic violence in 2008. Committed to honoring her sister, Jan Roseboro, Van Zant founded a non-profit organization called Jan’s Circle of Friends which provides funding to help children who have experienced parental loss or who are survivors of domestic violence. Jan’s Circle of Friends gave a generous donation to Hospice & Community Care to create a special “Volcano Room,” a safe space where grieving children can express their intense feelings in a healthy way. The Volcano Room opened in Fall 2014.

Used mostly by children under age 13, the colorful 11-foot-by-13-foot room has padded walls; a padded floor; equipment such as punching bags and boxing gloves; and a volcano painted on one of the walls. There is a second, smaller room with a punching bag and boxing gloves for teens.
Hospice’s Coping Kids & Teens Program is designed to help participants release intense feelings instead of internalizing them. Children are encouraged to talk about their feelings, and they also are taught other options for releasing them. The Volcano Room provides one such outlet.
“Children who are grieving need a place to go to release their frustrations and anger,” Van Zant said. “When Jan’s Circle of Friends decided it wanted to partner with an agency that helped children deal with grief and loss, we thought, ‘Who better than Hospice & Community Care?’ ”
“In their grief work, children learn how to identify their feelings and express them in healthy ways so they can move beyond the anger,” Anewalt said. “That way, later in life, they can cope with other transitions because they’ve learned these skills along the way.”
Since its completion, the Volcano Room has been used by individual children as well as by group participants in Hospice’s Coping Kids & Teens Program. The children are supervised by a counselor or trained adult in the room.
Van Zant and Jan’s Circle of Friends are delighted to have provided funding for the Volcano Room.
“We are excited about this unique addition to our Pathways Center for Grief & Loss,” said Patti Anewalt, the center’s director. “Kids who are experiencing loss have a lot of pentup emotions, and this room provides a helpful, easy, safe way for children to vent and express all that energy.”
“This gives us the opportunity to do something positive for children who are hurting in our community,” Van Zant said. “It is meaningful to know that children are benefiting from the Volcano Room. ‘Stars of Hope’ Songwriter Shares Heart and Talent with Hospice Stars of hope, stars of hope–to guide you as you find your way back home… Indeed, it has. Lutz has played the piece at every Light Up a Life celebration since 2010. The first year he was joined by his daughter, Brielle, on flute and his son, Taylor, on cello, in the trio Starlight Serenade. In fact, the piece was videotaped with Lutz on guitar and his daughter on flute at the 2013 Light Up a Life celebration. The performance can be viewed on YouTube. Lutz believes a song can change the world. His song, ‘Stars of Hope,”’ certainly has been a meaningful gift to Hospice and the people who attend Light Up a Life.
When Hospice & Community Care asked local musician Deeter Lutz to select a song that would be fitting for him to perform at its Light Up A Life celebration in December 2010, neither Hospice nor Lutz realized just how perfectly the stars were aligning. Hospice asked Lutz to find a song that would capture the love and meaning of Light Up a Life, a holiday event that places thousands of delicate white lights on evergreen trees. Each light represents a gift to Hospice in memory of a loved one. Hospice knew that Lutz was a talented musician because he had performed at other Hospice events including its children’s camp, Camp Chimaqua. But what Hospice may not have

known was that Lutz is also a songwriter, and at that time of this request he was grieving the loss of a special loved one. Lutz pulled together a list of existing songs to share with Hospice, but nothing seemed quite right. That’s when he decided to write his own song. The result, ‘Stars of Hope,’ fit the bill perfectly. “I wanted to write a song that points toward hope in one another and in the stars that shine; a song that speaks of our journey, our loss and the treasures that await us just above the clouds,” said Lutz, of Marietta. “I hoped this song would catch the heart of people feeling the same things I was feeling.”
“Stars of Hope is about how in our day-to-day lives with each other, we can be a bridge of hope,” he said. “We can reach out a hand and smile at our neighbor, and see a twinkle in their eye. The song is about our hope in the journey forward. That is indeed what our loved ones would want us to do — to carry on, to take what they have infused in us and carry it forward as neighbors, as brothers and sisters, and as a community.” “I think the world of Hospice, and I am thankful to support them,” he said. “When Hospice desired a signature song for its annual treelighting ceremony, it simply couldn’t have known the degree of empathy I would have—as a songwriter, and as someone who had recently suffered a loss. Hospice has given me a gift as well.”

Our Community’s Tremendous Response It’s all because of you… 2014 marked the 30th Anniversary of Hospice & Community Care’s signature fundraising event, the Labor Day Auction. Thanks to the overwhelming support of the community the Auction raised a record $476,000 to support hospice care and bereavement support for the community. The generosity of our corporate sponsors, the many donors who contributed thousands of unique auction items, our friends from the Amish community who gave hours of time, the Auction Club members, our amazing auctioneers, our dedicated volunteers and staff who worked tirelessly, and all those who enjoyed the Labor Day Auction—thank you for making the difference! Special thanks to the Hospice Circle of Friends for helping to raise nearly $48,000 at Spring Fling held in May at the Lancaster Country Club. Our guests enjoyed a wonderful array of gourmet fare, jazz by The Reese Project and live and silent auctions including fine jewelry, private golf lessons, catered dinners, and unique gift certificates. 288 new bricks honoring the memory of Hospice patients and loved ones were added to the Path of
Remembrance on the grounds of the Bob Fryer & Family Inpatient Center, and 324 individuals became Hospice Partners through their generous support.
Throughout the year, individuals, organizations and businesses in the community raised more than $80,000 from 25 Special Events to benefit Hospice & Community Care.
Membership in the Evergreen Circle of Friends increased to 100 individuals who have included Hospice & Community Care in their estate plan.
All funds raised from local fund raising activities directly benefit patients and families in the communities that we serve. Thank you.
More than 300 days of hospice care, or $57,000, were provided through the generosity of our donors during the One Gift, One Day program. Our York County supporters contributed more than $25,000 at the 2nd Annual Wine, Dine & Unwind event in October at Heritage Hills Resort. 4,400 lights glistened on the evergreen trees at our location on Good Drive through the holiday DINE season, and nearly 800 people joined us for the annual Light Up A Life Tree-Lighting Ceremony.
