As much as people do not like to think about it, one of life's certainties is that everyone dies. Many understand that estate planning is something they should do, but they put it off as a task for another day. It may be difficult, but people should not underestimate the importance of estate planning. Making a careful estate plan can save people's surviving loved ones a lot of hassle, heartache and money.

Boomers & Beyond provides an in-depth look at estate planning. Keep in mind, creating an estate plan often requires the help of an attorney. An experienced estate planning lawyer can review a person's assets, discuss all of the options available and help the planner decide what works best with his or her situation and wishes.

1. What is estate planning?

Estate planning is a process. It involves people—your family, other individuals and, in many cases, charitable organizations of your choice. It also involves your assets (your property) and the various forms of ownership and title that those assets may take. And it addresses your future needs in case you ever become unable to care for yourself.

Through estate planning, you can determine: How and by whom your assets will be managed for your benefit during your lifetime if you ever become unable to manage them yourself. When and under what circumstances it makes sense to distribute your assets during your lifetime.

How and to whom your assets will be distributed after your death.

How and by whom your personal care will be managed and how health care decisions will be made during your lifetime if you become unable to care for yourself.

Many people mistakenly think that estate planning only involves the writing of a will. Estate planning, however, can also involve financial, tax, medical and business planning. A will is part of the planning process, but you will need other documents as well to fully address your estate planning needs.

The purpose of this article is to summarize the estate planning process, and illustrate how it can help you meet your goals and objectives. You will discover that estate planning is a dynamic process. Just as people and assets and laws change, it may well be necessary to adjust your estate plan every so often to reflect those changes.

2. What is involved in estate planning?

There are many issues to consider in creating an estate plan. First of all, ask yourself the following questions: What are my assets and what is their approximate value?

Whom do I want to receive those assets—and when?

Who should manage those assets if I cannot—either during my lifetime or after my death?

Who should be responsible for taking care of my minor children if I become unable to care for them myself?

Who should make decisions on my behalf concerning my care and welfare if I become unable to care for myself?

What do I want done with my remains after I die and where would I want them buried, scattered or otherwise laid to rest?

Once you have some answers to these questions, you are ready to seek the advice and services of a qualified lawyer. Such a lawyer can help you create an estate plan, and advise you on such issues as taxes, title to assets and the management of your estate. continued on next page

The value of your estate is important in determining whether your estate will be sub3. Who needs estate planning?

You do—whether your estate is large or small. Either way, you should designate someone to manage your assets and make health care and personal care decisions for you if you ever become unable to do so for yourself.

If your estate is small, you may simply focus on who will receive your assets after your death, and who should manage your estate, pay your last debts and handle the distribution of your assets. If your estate is large, your lawyer will also discuss various ways of preserving your assets for your beneficiaries and of reducing or postponing the amount of estate tax which otherwise might be payable after your death.

If you fail to plan ahead, a judge will simply appoint someone to handle your assets and personal care.

And your assets will be distributed to your heirs according to a set of rules known as intestate succession. Contrary to popular myth, everything does not automatically go to the state if you die without a will. Your relatives, no matter how remote, and, in some cases, the relatives of your spouse will have priority in inheritance ahead of the state. Still, they may not be your choice of heirs; an estate plan gives you much greater control over who will inherit your assets after your death.

4. What is included in my estate?

All of your assets. This could include assets held in your name alone or jointly with others, assets such as bank accounts, real estate, stocks and bonds, and furniture, cars and jewelry. Your assets may also include life insurance proceeds, retirement accounts and payments that are due to you (such as a tax refund, outstanding loan or inheritance).

Many people assume their spouses or children will automatically be allowed to make financial and/or medical decisions for them, but this is not necessarily true. Advance directives are written documents that tell your doctors what kind of treatment you’ll want if you become unable to make medical decisions (for example, if you’re in a coma). Forms and laws vary from state to state, so it’s a good idea to understand the laws of the state where you live as you write advance directives.

Indeed, these are tough issues, but you shouldn’t shy away from them.

5. What is a will?

A will is a traditional legal document which: Names individuals (or charitable organizations) who will receive your assets after your death, either by outright gift or in a trust.

Nominates an executor who will be appointed and supervised by the probate court to manage your estate; pay your debts, expenses and taxes; and distribute your estate according to the instructions in your will.

Nominates guardians for your minor children.

Most assets in your name alone at your death will be subject to your will. Some exceptions include securities accounts and bank accounts that have designated beneficiaries, life insurance policies, IRAs and other tax-deferred retirement plans, and some annuities. Such assets would pass directly to the beneficiaries and would not be included in your will.

In addition, certain co-owned assets would pass directly to the surviving co-owner regardless of any instructions in your will. And assets that have been transferred to a revocable living trust would be distributed through the trust—not your will.

6. What is a revocable living trust?

It is a legal document that can, in some cases, partially substitute for a will. With a revocable living trust, your assets are put into the trust, administered for your benefit during your lifetime and transferred to your beneficiaries when you die— all without the need for court involvement.

Most people name themselves as the trustee in charge of managing their living trust’s assets. By naming yourself as trustee, you can remain in control of the assets during your lifetime. In addition, you can revoke or change any terms of the trust at any time as long as you are still competent. (The terms of the trust become irrevocable when you die.)

In your trust agreement, you will also name a successor trustee (a person or institution) who will take over as the trustee and manage the trust’s assets if you should ever become unable to do so. Your successor trustee would also take over the management and distribution of your assets when you die.

A living trust does not, however, remove all need for a will.

Generally, you’d still need a will—known as a pour over will— to cover any assets that havn’t been transferred to the trust.

You should consult with a qualified estate planning lawyer to assist you in the preparation of a living trust, your will and other estate planning documents. Also, keep in mind that your choice of trustees is extremely important. That trustee’s management of your living trust assets will not be automatically subject to direct court supervision.

7. What is probate?

Probate is a court-supervised process for transferring a deceased person’s assets to the beneficiaries listed in his or her will. Typically, the executor named in your will would start the process after your death by filing a petition in court and seeking appointment. Your executor would then take charge of your assets, pay your debts and, after receiving court approval, distribute the rest of your estate to your beneficiaries. If you were to die intestate (that is, without a will), a relative or other interested person could start the process.

In such an instance, the court would appoint an administrator to handle your estate. Personal representative is another term used to describe the administrator or executor appointed to handle an estate.

Simpler procedures are available for transferring property to a spouse or for handling estates in which the total assets amount to less than $100,000.

The probate process has advantages and disadvantages.

The probate court is accustomed to resolving disputes about the distribution of assets fairly quickly through a process with defined rules. In addition, the probate court

reviews the personal representative’s handling of each estate, which can help protect the beneficiaries’ interests.

One disadvantage, however, is that probates are public. Your estate plan and the value of your assets will become a public record. Also, because lawyer’s fees and executor’s commissions are based on a statutory fee schedule, a probate may cost more than the management and distribution of a comparable estate under a living trust. 8. Can I name alternative beneficiaries?

Yes. You should consider alternative beneficiaries in the event that your primary beneficiary does not survive you.

And if a beneficiary is too young or too disabled to handle an inheritance, you might consider setting up a trust for his or her benefit under your will or living trust.

Once you have decided who should receive your assets, it is very important that you correctly identify those chosen individuals and charitable organizations in your will or trust. Many organizations have similar names and, in some families, individuals have similar or even identical names. An estate planning lawyer can help you clarify and appropriately identify your beneficiaries.

9. Who should be my executor or trustee?

That is your decision. You could name your spouse or domestic partner as your executor or trustee. Or you might choose an adult child, another relative, a family friend, a business associate or a professional fiduciary such as a bank. Your executor or trustee does not need any special training. What is most important is that your chosen executor or trustee is organized, prudent, responsible and honest.

While the executor of a will is subject to direct court supervision and the trustee of a living trust is not, they serve almost identical functions. Both are responsible for ensuring that your written instructions are followed.

Renews Lives Lost to Dementia

“I’ve been in the long-term care business for 22 years, and am still moved when I witness a ‘wow’ moment with one of my elders,” shares Community Coordinator Barbara Mullins at Jewish Home Lifecare’s Sarah Neuman Center. The dedicated RN is referring to a recent exchange with a resident who she never before heard speak until this moment. A familiar tune playing in the Sarah Neuman dining room one afternoon sparked a burst of energy in the resident who began to sing along. “Moments like this are exactly why I continue to do this kind of work,” she explains.

In keeping with the therapeutic benefits derived from music, a new project is being launched at the Sarah Neuman Center in Mamaroneck and is spearheaded by Deirdre Downes, Director of Social Work at Jewish Home Lifecare. The Music and Memory program, developed

and founded by Dan Cohen, MSW, Executive Director, aims to make this form of personalized therapeutic music a standard of care throughout the health care industry.

Using iPod technology, Music and Memory is being implemented on all of Jewish Home campuses, including Manhattan and the Bronx.

With the help of Cohen’s non-profit organization, Sarah Neuman staff and volunteers are being trained to create and provide personalized playlists of songs for the nursing home elders. Through music-triggered memories, those struggling with Alzheimer’s and dementia are able to reconnect and improve their quality of life. Music and Memory training also provides Sarah Neuman staff with ways to assess and measure positive outcomes.

Grounded in extensive neuroscience research about how the brain responds to music, care professionals are

seeing life-changing experiences for everyone involved.

Music is a source of pleasure and comfort for many individuals with dementia; studies have shown that it increases their cooperation and attention. Staff morale is also significantly enhanced since the residents are less resistant to care. Music memories improve an elder’s engagement and socialization, which makes for a calmer environment.

Dan Cohen created special questionnaires for the Sarah Neuman staff to use, helping them with overall care plans for their residents, with a focus on personalized music selections. In most cases, the forms were distributed to the residents’ family members for responses. Discovering each elder’s favorite musical genre, recording artists and specific songs is an exciting part of the process – one that engages family, staff and residents. continued on next page

“Through new or used iPod donations, our immediate goal is to have one for each of our 88 residents in Sarah Neuman’s special dementia unit,” says Ms. Mullins. “It would be a dream come true to be able to provide all 300 residents in the facility with their own iPods.”

Family members, staff and the local community have responded favorably to Sarah Neuman’s grass roots donation requests, yet there is a need for many more iPods. “We find that the iPod Shuffle is the easiest for elders to handle, but we’ll take whatever we can get!” exclaims Ms. Mullins.

In addition to receiving used iPods, there were several donations of new iPods. Other donors gifted iTune gift cards and credits that helped to defray the costs of uploading songs. Using a special computer program

that links the iTunes account between the Sarah Neuman campus and the other Jewish Home Lifecare locations, songs can be downloaded and legally shared.

Sarah Neuman has a team of volunteers who are ready to help with the process of sanitizing the donated items, deleting its content and uploading the music that will renew the lives of the dementia residents.

Mullins states, “It’s really remarkable that such a compact electronic device can bring so much joy and engagement each day to our residents.”

2. Teeth have angles and curves which are more difficult to clean than flat surfaces 3. Always focus on brushing at the gums.

4. Be absolutely certain the bristle tips are in the sulcus between the tooth and the gums. Effective brushing is done by feeling the bristles in that location.

This is VIP 5. This is just like brushing under your finger nails. Do the same thing you would do to get debris from between the nail bed and the finger.

6. Short wiggle strokes are the most effective: back and forth, circular or up-down pivot; they all work.

7. Choose the instruments that will best accomplish the task. Do not be fooled by marketing claims. The right instruments are not what you think.

8. If an electric brush cannot stay under the gums when working…it does not work.

11. Think of brushing this way: imaging a structure shaped exactly like the teeth in your mouth that is 20 times larger. There are many more details to be learned to obtain total oral health in the wide variety of conditions that can exist in patients’ mouths. The good news is that they can and are learned by patients each day.

When are you totally healthy?

When all the appropriate dental health tools are used on a regular basis, which does not necessarily mean daily, and there is zero sensitivity in the gums and zero bleeding, that is healthy. Even advanced gum disease cases can achieve this in as little as a few weeks.

The Road to Recovery Navigating the world of rehabilitation can be tough. Usually the beginning of this journey occurs after an admission to the hospital, or following a doctor’s visit when it is discovered that there has been a change in your typical health status. Salute to Seniors Businesses and non-profit organizations can now register for booths to be part of the Business Expo at the 45th annual Salute to Seniors on May 1 at the Westchester County Center in White Plains.

The Salute takes place from 9 a.m. to 2 p.m., and usually attracts some 2,000 seniors. An even bigger turnout is expected at this year’s Salute, which also includes a health and wellness fair, an arts and crafts fair, live entertainment and special programs.

The Expo offers a marketing opportunity for companies and non-profit groups to target their products and services to Westchester’s vital and growing senior population. Last year there were 82 Business Expo participants, and they included assisted living centers, hospitals, recreational services, insurance firms, financial service companies and social service agencies.

The corporate rate for a half booth is $400, a single booth is $750, and a double booth is $1,400. The rate for non-profits is $200 for a half booth, $350 for a single and $650 for a double. To be eligible for these rates, deposits must be received by March 29.

After that date, the fee increases $50 for the half and single booths and $100 for the double. Full payment is required by April 6.

When a stroke hits, the clock starts ticking: If patients get the right treatment within the next few hours, it can make all the difference in their chances for recovery, according to Dr. Akira Todo, Director of the Stroke Program at Northern Westchester Hospital, Mt. Kisco.

Many people don¹t realize that we have really effective treatments for stroke.

Time is of the essence and knowing the symptoms of stroke is crucial. The National Stroke Association has come up with the easy-to-remember acronym FAST:

f is for face, or facial drooping. Ask the person to smile: Does one side of the smile droop? a is for arms. Can the person lift both arms overhead? Is there any numbness or weakness? s is for speech. Ask the person to repeat a simple phrase and listen for slurred or strange sounding pronunciation. t is for time, which is a reminder that if you see any of these signs, you should call 911 immediately.

Understanding who is at risk can also help you be prepared. People who are overweight, have a history of high blood pressure or cholesterol, heart disease, diabetes, or smoking have a greater risk of stroke. Risk can increase as we age, though doctors see stroke patients under 50 all the time. Heavy drinking and drug use can also trigger a stroke.

The good news is that ER doctors have medications that can stop and even help undo stroke damage. Northern Westchester Hospital is a New York State-designated Stroke Center which means we¹re able to diagnose and quickly treat patients. Once a patient has been diagnosed and the medical history has been checked, doctors can administer medications to bust up the clot in the brain that is triggering the stroke symptoms and loss of function.

More and more patients are able to make a strong recovery due to early treatment and good rehabilitation services. Everyone needs to keep FAST in mind and think stroke if they see symptoms—that message can¹t be emphasized enough. While some 800,000 people suffer a stroke each year in the United States, only about 5 percent get drug treatment in time.

Sound Shore Medical Center (SSMC) recently launched an innovative program, Rapid Response. In partnership with the Visiting Nurse Association of Hudson Valley (VNA), the Program – one of the first of its kind in the area – targets patients utilizing the Emergency Department Services (ED), who are stabilized and do not require admission to the Hospital. Rapid Response, which is available 24/7, focuses on individuals with chronic diseases or the elderly, who can benefit from support services when they return home after an ED visit. Its goal is to quickly deliver services to discharged patients so repeat emergency department visits are not necessary.

When an ER patient is deemed medically stabilized by an Emergency Department physician and hospitalization is not indicated, ED staff makes a referral to the VNA for home care services. A Visiting Nurse then visits the patient’s home the day of discharge, and if needed, within hours of discharge. During this visit, the Visiting Nurse assesses the patient’s ability to follow the discharge instructions, reinforces instruction on proper use of medications and teaches the patient to be alert to changes in their condition.

Shut your Mouth!

How the mouth holds clues to your current and future health potential

By Dr. Michael Wald, Blood Detective Throughout our lives our nutritional needs vary greatly. Newborns require very different nutrition than adults, males require different nutrition than females and the sick require different nutrition than athletes. Even a given individual’s nutritional needs are subtly or sharply different from day-to-day depending upon stress levels, sleep habits, medications, exercise and other factors. Genetics is always influencing our core nutritional needs. How does one figure out how to manage optimal nutritional needs throughout our lives?

Sophisticated laboratory tests and medical technologies are available that, when considered together with medical and health history questionnaires and consultation, can very accurately estimate one’s nutritional needs. In the interest of teaching my patients how to take care of them selves to a certain degree, I have written this article providing specific physical signs of current or potential illness that are related to diet and overall nutrition as well as lifestyle in general. If you pay attention to these “signs of health status” you should go a long way towards maintaining health for longer and avoiding premature chronic disease.

Your mouth tells all The structures and lining of the mouth, or oral cavity, offer valuable, and easily accessible, information on your nutritional status. Lesions in the mouth, often overlooked by even well-trained health professionals, may indicate a nutrient deficiency or may be manifestations of gastrointestinal or other disease. Because of the very rapid cell turnover of the oral mucosa, or cells that line the mouth, these lesions often may precede other manifestations of nutrient deficiency or systemic disease by weeks, months or even years. Some typical lesions are mucosal ulceration, cheilosis, gingivitis, and glossitis. Between 5% and 10% of the people in the United States are deficient in one or more nutrients, so signs of nutritional deficiency are common, but often go unrecognized producing other symptoms.

Anatomy of the mouth Before one can start to recognize signs of nutritional problems in the mouth a little anatomy of the mouth is necessary. But don’t be discouraged! Once you learn a small amount of mouth anatomy you will start recognizing signs of nutritional deficiency and disease very quickly. The top of the healthy tongue is covered with smooth, pink mucous membrane and lymphoid follicles. On the bottom surface, the various specialized cells called papillae (which contain the organs of taste) produce a rough, grayish red appearance. The thick epithelial tufts of the filiform papillae give the tongue its characteristic grayish white coating, whereas the globular, pale red fungiform papillae give it a speckled pink appearance. Furrows are not a characteristic of the healthy tongue. The buccal mucosa (the sides of the in-

side of the mouth) has a grayish red color and may be crossed by fine grayish ridges where it touches the closed teeth. The healthy gums have a light reddish appearance and cover the roots of the teeth completely.

The assessment of oral health, obviously, must assess the health and status of the teeth. Patients should be asked about their dental history and encouraged to have regular dental checkups. Here are some key nutritional considerations for examination of the teeth. However, do not expect your dentist to pick up any or all of the oral signs of nutritional deficiency; so it is up to you to take a careful look and bring what you suspect is a problem in the mouth to your dentist or nutritional health care provider.

Also, having fewer than 20 teeth as an adult is associated with significantly reduced capacity to eat nutri-

tious foods, such as salads, raw fruit and vegetables, nuts, and whole grain products. The same applies even to denture wearers. Other causes of poor nutritional status showing up as decay in the mouth include: • Bulimia may manifest as erosive tooth wear.

• Osteoporosis may manifest initially as changes in jaw structure and/or loose teeth.

• Presence of mercury amalgams may be associated with systemic disease, especially neurologic conditions.

Membranes can be a “pain”!

Below I have summarized many common typical oral lesions associated with a particular nutrient deficiency, are summarized below.

In general, ulceration should be considered a nonspecific expression

of a disease state. A search for the cause will usually result in a specific therapy. Aphthous stomatitis is a common example of a mucosal ulceration and I see many patients with this particular finding. I visited with a 15-year old girl with ongoing intestinal problems undiabnosed by other doctors. I noticed this finding and then tested appropriately and discovered the presence of gluten intolerance. Removal of gluten caused an immediate reduction of this 15 year old intestinal bloating, cramps, weight gain and gas.

Similarly, cheilosis is a common expression for acquired nutrient deficiency. Gingivitis is associated with the classic signs of scurvy, but other nutrients have now been shown to play a role in gingival health.

Glossitis is associated with numerous vitamin deficiency states, each with a characteristic appearance.

Examples of nutritional deficiencies seen in the mouth Vitamin deficiencies of various kinds can cause these and other oral lesions: Biotin Geographic tongue, atrophy of lingual papillae Folic acid Gingivitis, glossitis with atrophy or hypertrophy of filiform papillae, cheilosis Niacin Intraoral burning, canker sores, halitosis, glossitis, tongue swollen with red tip and sides, swollen red fungiform papillae, filiform papillae becoming inflamed and losing their epithelial tufts (giving the characteristic slick red appearance) Pyridoxine Intraoral burning, glossitis, mucosal ulcerations and erosions, cheilosis Vitamin B12 Intraoral burning, mucosal ulcerations and erosions, painful glossitis with a beefy red or fiery appearance eventually resulting in an atrophic (smooth and shiny) tongue Riboflavin Soreness and intraoral burning, cheilosis, angular stomatitis, glossitis with a magenta tongue Vitamin C Sore and bleeding gums, deep blue-red color to gums, loose teeth, follicular hyperkeratosis Vitamin D Intraoral burning Vitamin E Glossitis Minerals Calcium Periodontal disease, tooth decay Iron Cheilosis, atrophic glossitis, gingivitis, candidiasis, intraoral burning or pain, mucosal ulcerations and erosions, pallor Zinc Marked halitosis, cheilosis, stomatitis, discrete red, scaly plaques Specific oral manifestation of disease Cheilosis Crohn’s disease, acrodermatitis enteropathica, alcoholism, celiac disease, malabsorption syndrome Gingivitis Crohn’s disease, anorexia nervosa, celiac disease, scurvy Erythroplakia Dysplasia or carcinoma Glossitis Crohn’s disease, diabetes, alcoholism, celiac disease, malabsorption syndrome, pernicious anemia, iron-deficiency anemia, amyloidosis, carcinoid syndrome, cigarette smoking, anemia Intraoral burning Menopause, diabetes mellitus, esophageal reflux, Sjögren syndrome Leukoplakia Chronic irritation, dysplasia, early invasive squamous cell carcinoma Ulcerations Crohn’s disease, ulcerative colitis, celiac erosions disease, corticosteroid use, acrodermatitis enteropathica, anorexia nervosa, pernicious anemia, iron-deficient anemia, mercury poisoning, nicotine withdrawal If you should discover any suspected oral lesions please report them to your qualified health care provider.

Visual clues of disease can prove to be life-savers. They have for many of my patients.

DSPS Commissioner Mae Carpenter said the survey feedback will be a blueprint for its projects in future years. “I encourage every senior to make it a priority to take this survey,” Carpenter said. “We might not be able to shift the wind, but we may be able to tilt the sails. Of course, we first have to know which way the wind is blowing.”

About the Elder Economic Security Standard Index The Index is calibrated to reflect the needs of specific living situations, for adults who want to remain in their homes as they age through benchmark figures. In the housing portion of the survey, these figures indicate how that amount will vary, depending on the life circumstances of a senior or couple. For example, the survey asks whether the senior lives alone or with a spouse and if they own or rent their home. If they own their home, the survey asks if the mortgage is fully paid or if they continue to pay it off.

Westchester County’s Department of Senior Programs and Services (DSPS) has kicked off its signature 2012 initiative in conjunction with its Livable Communities program: a survey of county residents age 60 and older to learn if a senior's income covers his or her basic expenses without public or private subsidies.

Using the nationally recognized Elder Economic Security Standard Index, the survey will measure the economic security of older adults by determining whether or not they have sufficient income to pay for their basic needs. For the purpose of this survey, basic needs are food, transportation, health care insurance and housing.

Westchester County Executive Robert P. Astorino praised the survey and said he eagerly looks forward to its findings. “Like all Westchester residents, seniors are up against the highest property taxes in the country,” Astorino said. “But armed with the data from this survey, we will have a better picture of where to put our tax dollars to benefit seniors and have the most impact.”

Take the survey The survey is being distributed at DSPS coalition meetings, nutrition sites and Livable Communities Villages, among other locations. Seniors with Internet access can complete the survey online. The results will be tallied and compiled by zip code in 2013 to create a portrait of the economic security of Westchester’s seniors.

The county will share its findings with the public as well as with service providers, senior advocates and policymakers.

The anonymous survey is sponsored by the county’s Department of Senior Programs and Services (DSPS) and the Westchester Public/Private Partnership for Aging Services. The project is vitally important for the county’s 155 Livable Communities Villages, which are places where seniors can continue to live in their homes as they age with dignity and independence.

For example, the indexed benchmark figures indicate that a single older person who owns a home but has paid off the mortgage needs a monthly income of $2,279 ($27,248 a year) to meet their basic needs. A couple in the same situation would need $3,186 a month ($38,232). A single senior who rents a one-bedroom apartment, needs an income of $2,607 a month ($31,284) whereas a couple renting a one-bedroom needs a monthly income of $3,514 ($42,168).

The survey was developed by Wider Opportunities for Women (WOW) in collaboration with the Gerontology Institute of the University of Massachusetts, Boston.

WOW and the Gerontology Institute developed the benchmark figures for every county in 17 states, including New York. To compile the regionspecific figures, they used data from sources such as the U.S. Department of Housing and Urban Renewal, the U.S. Food and Drug Administration, the Census Bureau and the Consumer Price Index.

How big is the problem?

• One out of three adults age 65 and older falls each year,1 but less than half talk to their healthcare providers about it.

• Among older adults (those 65 or older), falls are the leading cause of injury death. They are also the most common cause of nonfatal injuries and hospital admissions for trauma.

• In 2010, 2.3 million nonfatal fall injuries among older adults were treated in emergency departments and more than 662,000 of these patients were hospitalized.

• In 2010, the direct medical costs of falls, adjusted for inflation, was $30.0 billion.

How can older adults prevent falls?

• Exercise regularly. It is important that the exercises focus on increasing leg strength and improving balance, and that they get more challenging over time.

• Ask their doctor or pharmacist to review their medicines—both prescription and over-the counter to identify medicines that may cause side effects or interactions such as dizziness or drowsiness.

• Have their eyes checked by an eye doctor at least once a year and update their eyeglasses to maximize their vision. Consider getting a pair with single vision distance lenses for activities such as walking outside.

• Make their homes safer by reducing tripping hazards, adding grab bars inside and outside the tub or shower and next to the toilet, adding railings on both sides of stairways and improving the lighting in their homes.

To lower their hip fracture risk, older adults can: • Do weight bearing exercise.

• Get screened/treated for osteoporosis.

• Get adequate calcium and vitamin D—from food and/or from supplements.

Who is at risk?

Fall Injuries • People age 75 and older who fall are four to five times more likely than those age 65 to 74 to be admitted to a long-term care facility for a year or longer.

• Rates of fall-related fractures among older women are more than twice those for men.

• Over 95% of hip fractures are caused by falls.14 In 2009, there were 271,000 hip fractures and the rate for women was almost three times the rate for men.

• White women have significantly higher hip fracture rates than black women.

Gilda’s Club Westchester forms new partnership with Hudson Valley Hospital Center When Patricia Witters of Cortlandt Manor was first diagnosed with breast cancer in April 2012 she said one of the most difficult things she faced was telling her 8 and 11-year-old sons.

“One of the biggest hardships was how would I tell my sons,’’ said Witters, a patient at HVHC’s Cheryl R. Lindenbaum Cancer Center. “The C word is very scary. I had to think very carefully about it.’’

That’s why Patricia said she is so excited about a new program for children of cancer patients now offered at Hudson Valley Hospital Center through Gilda’s Club Westchester. “Kid 2 Kid” gives children ages 5-12 a safe, supportive environment to discuss how cancer has affected their lives. It’s one of two new support groups being offered by Gilda’s Club as part of a new partnership with Hudson Valley Hospital Center in Cortlandt Manor.

Hudson Valley Hospital Center President John C. Federspiel said the new partnership with Gilda’s Club Westchester was an example of how the Hospital continues to expand services at its cancer center.

The cancer center celebrates its one-year anniversary this month and, Federspiel said, it continues to grow, offering new treatments, adding new physicians and support services like Gilda’s Club.

Gilda’s Club Westchester, a leader in support for cancer patients and their families, has been located in White Plains since 2001, but recently chose Hudson Valley Hospital Center as a second location so it can make its services more accessible to upper Westchester and Putnam residents.

Neil Castaldo, a former prostate cancer patient at HVHC, said the Gilda’s Club groups would make a big difference to patients at the center.

“It’s something really essential to any good cancer program,’’ he said.

“Gilda’s Club Westchester is thrilled to be partnering with Hudson Valley Hospital Center and their distinguished team of clinical professionals at their Comprehensive Cancer Center,’’ said Gilda’s Club Westchester Chief Executive Officer Melissa Lang.
