SPOTLIGHT: Vivacious. That’s the word many of Rosemary Martino’s colleagues at Sound Shore Medical Center in New Rochelle use to describe her. Actually, you don’t know vivacious, until you meet Martino, a go-getter’s go-getter. As Sound Shore’s Senior Vice President for Clinical Systems Development, Martino is responsible for, among other things, broadening access to and coordination of care through the medical center’s outpatient department, leadership for development of an independent practice association that will help guaranteed full continued care for patients. Martino has also been front and center during the implementation of Medicaid Health Homes in the Hudson Valley. This collaborative effort, of which Sound Shore Health System (Sound Shore Medical Center and Mount Vernon Hospital) are founding members, is a dynamic approach to improving services to a population with complex healthcare needs. Martino, a Long Island native, arrived to Sound Shore in July of 2011. She has over 20 years of academic and healthcare experience. Martino came to Sound Shore with over 20 years of academic healthcare experience, including 13 at New York Medical College in other senior leadership roles.

Most enjoyable part of her position: “There are so many pieces to it. The fact that so many changes are happening in the healthcare world is exciting. I take everything I’ve learned and use in one of the most challenging times in the field. The Health Homes endeavor is just something that I’m happy to be involved with. It helps patients who don’t always have social support and are vulnerable.” If she wasn’t involved in healthcare: “I would do one of two things. I would either go back into the medical school world. But, what’s been on my mind, is that there’s a lot of aggressive bio-medical initiatives in this area. It’s interesting, especially with my financial background and exposure to clinical medicine.” How her colleagues would describe her: “A good leader. Smart. Somebody who gets it done and somebody who is fun to be around. Vivacious. I hear that a lot. Full of energy.” In her free time, she enjoys: “I don’t have much time for it, but I love to ski. That’s how I get through winters. I like to spend time with friends and family, too. I like to read and do things that are very creative.” Best ski trip: “I’m an intermediate skier, but I love Bellayre in the Catskills. Some lower Vermont places. Whistler (in British Columbia).” Food she can’t live without: “I’m Italian, I love all food. I have to have my mother’s sauce and pasta. I’m not a real sweet tooth, but I love a tiny piece of something sweet. Before I go to sleep, I have to skim the top of the rice pudding, or have a piece of candy.” Her pet peeves: “I’m really organized, so I can’t stand people who are sloppy. I also don’t like people who give up. There is always a solution to a problem. It may not be the best solution, but there is one.” In five years, she will be: “I will either be here doing something exciting or, maybe, be in one of those other two fields we discussed. No matter where I am, I want to learn and do new things.”

WHAT IS SLEEP APNEA? Sleep Apnea is estimated to affect 20 million people. Sleep Apnea occurs when the airway becomes completely blocked and air is not able to move through the airway. This is commonly called Obstructive Sleep Apnea because the airway is totally obstructed. This obstruction may occur anywhere from the area of the soft palate down to below the tongue. When the airway is obstructed, the patient will awaken many times during the night with a cough or gasping sound as the body attempts to restore normal breathing. Come on. You can do it. People have great hopes of changing what they do not like about themselves, yet within a couple of weeks after the New Year, they have broken or given up on their New Year’s Resolutions. One of the main reasons is because the goals that they set are too vague or unrealistic. Goals like wanting to quickly lose 50 pounds, paying off credit card debt in a month, exercising two hours a day or suddenly start eating healthy foods all of the time may be difficult for people to achieve. Another key in making a successful resolution is the person’s confidence that he or she is able to commit to making the change. Tips to Help You to Keep Your New Year’s Resolutions – Don’t make too many resolutions. Pick a realistic, attainable goal with a reasonable time frame. – Choose your own resolution. Make sure your goal is something that you want to accomplish for yourself and not just for friends or family. – Make a plan and write it down. Plan what you’d like to accomplish in a certain period of time, like three months. Achieving small goals over time gives you a sense of accomplishment and motivation to keep going. Writing your goals down is a good way to keep track of your progress. – Involve friends and family. They can support your efforts, and can motivate you to keep going. – If you get off track, forgive yourself. Review your plan and make adjustments. – Congratulate yourself. Reward yourself when your intermediate goals or resolutions are met. Make Intentions or Set Goals Rather than Making Resolutions Some people find the language they use to make their New Year’s Resolutions may help them stay on track. Instead of referring to them as “resolutions” think of them as “goals” or even better “intentions” that you make to commit to change. ASK AN EXPERT Save your teeth: An Ounce of Prevention is Worth a Pound of Cure By Edward Feinberg DMD Mrs. T. has great dentistry! Her dentistry--which was made at our office in 1973 has lasted nearly 40 years. (Figure 1) She made a great investment in her mouth by treating her dental problem with a comprehensive approach. Most practitioners only offer patients a piecemeal approach to dentistry that only addresses filling a hole or a space. Without an overall approach to correcting problems, Mrs. T. would most likely be wearing dentures or implant restorations that are far more expensive and surgically invasive than the bridgework she now has. Mrs. T.'s investment ultimately saved her a great deal of money, pain and hours in the dental chair. The Longevity of Dentistry Why did Mrs. T.'s dentistry last 40 years? It was not because of luck, or even genetics. First, her teeth were prepared in a three-dimensional way-that is, they were prepared to relate to the gum and underlying bone with ideal architecture. Second, her dentistry was designed according to sound principles of engineering. Individual copper band impressions were taken on each tooth to create restorations that do not fall out and seal the teeth like Mason Jar covers to protect against decay. This technique is no longer taught in the dental schools. That is why it is commonplace to hear stories of how original bridgework lasted decades, but the replacement bridgework quickly fell out. In our office, all the teeth have been prepared and handled in exactly the same way since 1950 with an amazing track record of longevity and success. Because sound techniques for crown and bridgework are no longer commonplace, it is no wonder that many practitioners feel more comfortable with placing implants. There seems to be an epidemic of implant placement since

and exposed roots. (Figure 2A) When roots are exposed, they are more difficult to clean, as the crevices between the roots pack food, as well as tartar and plaque. This accumulation irritates the gums and tends to accelerate the periodontal bone loss.

Figure 1 the modern osseo-integrated implant came to this country from Sweden in the early 1980's. Our office was one of the first in the county to offer implant restorations to patients. As wonderful as implants are, they are not panaceas. Implants--with a few notable exceptions--should be a last resort. No dentistry is lifetime--including implants. A successful case preserves the teeth and implants so that they are healthy enough to make a new case when the time for replacement is at hand. The human body ages--as we all know too well--and some of those changes inevitably occur in the mouth. To compensate for changes, the dentistry may require replacement in order to prevent future trouble. Most of the time changes occur gradually, so it can be very difficult to pinpoint precisely when dentistry should be remade. When should dentistry be replaced? Although wear of the materials can be an important factor in the decision to replace dentistry, periodontal bone loss and gum recession are the most important considerations. Dentistry has not discovered a way to regenerate periodontal bone, so once it is lost, it is gone forever. Periodontal bone loss is not only visible in the X-Rays, it can be seen in the mouth directly in the form of gum recession, exposed crown margins

Many patients also become susceptible to decay when the roots are exposed--especially when there are changes in saliva content or flow. Dry mouth, or xerostomia, is a potential side effect of such commonly prescribed medications as Albuterol, Alprazolam, Ambien, Elavil, Klonipin, Flexeril, Valium, Allegra, Neurontin, Ativan, Lunesta, Nexium, Trazodone, Wellbutrin, and Zyrtec. Dry mouth is so uncomfortable that patients often resort to lozenges in desperation for relief. However, if these lozenges contain sugar, decay can run rampant and quickly destroy a mouth. All too often patients do not receive this information until it is too late. Mrs. T. was lucky in that only two of her teeth became affected by recurrent decay. The decay showed up in a routine set of X-Rays. (Figure 2). By the time it was detected, it had already spread to the nerve tissue, so root canal treatment was required. After the old crowns and underlying decay were removed, very little tooth structure remained above the gum. (Figure 2B).

Many practitioners would recommend extracting these teeth to place implants. But Mrs. T. was frail and not a good candidate for implant surgery. It was better for her to save her own teeth. Her teeth were, in fact, easily restored without posts! She wore temporary crowns that did not fall out at any time. She wore her permanent crowns on a trial basis with rubber before they were permanently cemented and they did not fall out. How is this possible? The copper band technique--which comes from the very "roots" of dentistry and has the longest track record--takes full advantage of tooth structure below the gum for the retention of crowns. Mrs. T.'s replacement restorations restored her teeth with an ideal architecture that prevents root surface decay and debris accumulation. (Figure 2C). X-Rays show how closely the crowns relate to the underlying bone to optimally distribute the forces close to the foundation. This architecture halts the acceleration of periodontal bone loss. Looking at Mrs. T.'s X-Rays, it is easy to see that her new crowns are much closer to the underlying bone than the adjacent crowns that were made 40 years ago. Mrs. T. should really replace the rest of her dentistry to compensate for the periodontal changes that have occurred. Nearing 90, she is does not have the stamina for this endeavor. She understands that because she is not going to replace the dentistry, she must compensate for existing recession and bone loss with more diligent home care and extra hygiene visits at the office. Prevention is especially important for Mrs. T. to maintain a high quality of life as she ages. Benjamin Franklin's famous adage still rings true: an ounce of prevention is worth a pound of cure.

THERMOGRAM (CHEMO PREVENTION) This is an attempt to clarify some of the misunderstandings about the value of a Thermogram Breast and body imaging. Thermography is a test of cellular physiology (function) at the origin of disease, ten years before it is visible in an x-ray. In the inflammation stage, suspect disease can be seen in a Thermogram before the cells develop their own blood supply to support the growth of a tumor. Functional breakdown occurs long before there is a structural defect, meaning the cells of concern are highlighted in a Thermogram representing an early condition in progress. Mutated cells develop their own blood supply to support the accelerated growth. As the metabolic rate, production of these cells, increases the production of heat also increases, diseased cells are warmer than a healthy cell.

Breast cancer cells double in number in approximately 90 days. It is necessary to have a follow up Thermogram three months after your first scan to allow the Thermologist (interpreting doctor) to compare the results to look for evidence of cell doubling through increased heat. Thermography will indicate the cellular changes that precede or accompany an active tumor by exposing the increased heat. By the time a mammogram is able to detect breast cancer there is already a tumor that has been growing for several years undetectable in an x-ray. This treatment is non-invasive meaning there is no exposure to radiation and no compression of the breast at all. The client will simply sit about 3 feet away from of the camera and a digital image will appear on a screen. Thermal images can show abnormalities

anywhere in the body including some stress fractures that are undetectable in an x-ray. Most areas take less than 15 minutes to scan and the report is available in 24 hours. The reports include all color images and a full written report by certified doctors in Thermology. The cost of a breast Thermogram is $200 and the three month follow up scan is $150. Your insurance company may pay for this treatment, you will have to call your insurance provider to inquire. The cost is very affordable and the early findings are nothing you can put a price on. You can get off the road to breast cancer...At this early stage, if cellular activity is present, a follow up sonogram will be prescribed. The area of concern can be surgically removed without any additional treatments and can possibility be reversed by a diet

Stress Long-term stress can help cause a variety of health conditions for every women, from the doting stay-at-home mom, to the working woman. Put simply, stress usually occurs when bad things happen. Think three sick kids, not getting that job promotion and trying to manage the bills. High blood pressure, heart attacks and stroke may be stress related cardiovascular conditions. Some women experience changes in their sexuality and encounter various sexual dysfunctions such as loss of desire as a result of stress.

Preventative screenings Visit a health care professional to receive regular checkups and preventive screenings. As recommended by the United States Department of Health and Human Services, every two years, all women should receive a blood pressure test. Get tested once a year if you have blood pressure between 120/80 and 139/89. Discuss treatment with your doctor or nurse if you have blood pressure 140/90 or higher. Starting at age 50, women should get screened for breast cancer every two years. Other important screenings for women include screenings for Cervical cancer (Pap test), Chlamydia, cholesterol, Colorectal cancer, diabetes, Gonorrhea, HIV, and Syphilis.

The National Cancer Institute recommends a mammogram every one to two years after age 40. If you’re at higher risk, talk to your doctor about being screened earlier, and do monthly breast self-examinations. To reduce your breast cancer risk, be sure to exercise regularly and maintain a healthy weight.

Breast health Thanks to improvements in treatment and early detection, millions of women are surviving breast cancer today. The chance of a woman having invasive breast cancer some time during her life is a little less 1 in 8. The chance of dying from breast cancer is about 1 in 36. Eat Healthy Following a healthy eating plan doesn’t mean that you can’t indulge every now and then. If what you eat is generally low in fat (especially saturated and trans fat) and sugars and you are getting enough vitamins and minerals, you may indulge in a rich dessert or serving of fried food every once in a while. If, on the other hand, you eat a lot of highcalorie foods, you are likely to get all the calories you need quickly without getting enough vital nutrients. Over 60 percent of U.S. adult women are overweight, according to estimates from the National Center for Health Statistics of the Center for Disease Control and Prevention. Just over onethird of overweight adult women are obese. You can become overweight or obese when you eat more calories than you use. A calorie is a unit of energy in the food you eat. Your body needs this energy to function and to be active.

But if you take in more energy than your body uses, you will gain weight. Many factors can play a role in becoming overweight or obese. These factors include: • Behaviors, such as eating too many calories or not getting enough physical activity • Environment and culture • Genes

PMS and Menopause As most women know, PMS is a very common syndrome. It is believed that changing hormone levels and brain chemistry play a role in PMS. Long-Term Care Insurance… it may be now or never. People say they do not want to purchase long-term care insurance for a variety of reasons. I have literally heard them all. Some say they feel they will never need it, while others claim that their family will take care of them. Most say it is simply too expensive, let alone having to worry about future premium increases. If you know one of those people who keeps saying they want to ‘talk about it later’, you may want to get them to reconsider ‘talking about it now’. Why you may ask? Long-term care insurance has traditionally been underwritten using unisex rates. That is, policies have been issued based upon your age, health status and benefits purchased regardless of gender. Unfortunately, beginning early next year, Genworth Financial, the country's largest long-term-care insurer, plans to start charging women applying for coverage as much as 40% more than men1. And that means that policies that are already expensive are going to become even more costly very soon. So interested parties should meet with their advisor as soon as possible in order to buy a unisex policy today. According to the American Association for Long-Term Care Insurance, the move is designed to better reflect the risks involved in covering women, who are paid two out of every three benefit dollars from long-term-care insurance, in part because they live longer and often have no caregivers at home. In fact, married women represent 28% of all claims, while single women represent 39% of all claims2. It is this author’s opinion, that once this sex distinct underwriting change occurs, other insurers are sure to follow down this path. For the last ten years insurers have taken a beating due to this extended low interest rate environment. They have no place to invest current premiums today in order to cover future claims down the road. Combine this with rather liberal underwriting practices, lower than expected lapse ratios and higher than expected claims and… ‘Houston, we have a problem’. In the past few years, carriers like MetLife and Prudential have exited the individual long-term care market. Others carriers like Transamerica, Genworth, John Hancock and Unum have all raised rates substantially on existing policyholders. Genworth has filed with insurance regulators to raise prices for single women in all states but the two that require unisex rates (Montana and Colorado), and it already has won approval in more than half. The rate increases are expected to be significant—more than 10% higher than Genworth charges single women now, says Steve Zabel, Genworth's senior vice president for long-term-care insurance3. Adding insult to injury, underwriting guidelines are also planned to be tighter. Before blood & urine screens were not part of the LTCI underwriting process, but not anymore. Genworth plans to start conducting blood tests on all applicants, mainly to confirm what they report about nicotine and drug use, Mr. Zabel says. Genworth also wants to be able to look at some indicators of cardiovascular disease and stroke. This means that traditional individual long-term care insurance is going to be more difficult to qualify for and much more expensive if you are female. Even if you are married and are eligible for the hefty spousal discount, it still behooves you not to take action now. The landscape is about to change dramatically and many more people are going to be categorically declined. So tell your friends and family before it is too late. Prepared seasonings can have high salt content and increase your risk for high blood pressure. Replace salt with herbs and spices or some of the salt-free seasoning mixes. Use lemon juice, citrus zest or hot chilies to add flavor. Canned, processed and preserved vegetables often have very high sodium content. Look for “low-sodium” veggies or try the frozen varieties. Compare the sodium content on the Nutrition Facts label of similar products (for example, different brands of tomato sauce) and choose the products with less sodium. Prepare muffins and quick breads with less saturated fat and fewer calories. Use three ripe, very well-mashed bananas, instead of 1/2 cup butter, lard, shortening or oil or substitute one cup of applesauce per one cup of these fats.

Whole-wheat flour can be substituted for up to half of all-purpose flour. most important things to boost your preconception health

Not only women, but men should prepare for pregnancy before becoming sexually active — or at least three months before getting pregnant. Some actions, such as quitting smoking, reaching a healthy weight, or adjusting medicines you are using, should start even earlier. The five most important things you can do for preconception health are:

1. Take 400 to 800 micrograms (400 to 800 mcg or 0.4 to 0.8 mg) of folic acid every day if you are planning or capable of pregnancy to lower your risk of some birth defects of the brain and spine, including spina bifida. All women need folic acid every day. Talk to your doctor about your folic acid needs. Some doctors prescribe prenatal vitamins that contain higher amounts of folic acid.

2. Stop smoking and drinking alcohol. 3. If you have a medical condition, be sure it is under control. Some conditions that can affect pregnancy or be affected by it include asthma, diabetes, oral health, obesity, or epilepsy.

4. Talk to your doctor about any over-the-counter and prescription medicines you are using. These include dietary or herbal supplements. Be sure your vaccinations are up to date.

5. Avoid contact with toxic substances or materials that could cause infection at work and at home. Stay away from chemicals and cat or rodent feces.

Harvest of Hope Charity Ball proceeds despite Sandy Carla Ramos was an obvious choice to receive a 2012 Hearts for ARCS Award. She created the Hairdressers’ Disco Ball and Fantasy Hair Show in 2011 to unite her passions for music, the beauty industry and assisting those living with HIV/AIDS. The event grew in 2012 with over 600 attendees at the sold-out hair runway show at Shadows in Poughkeepsie on September 9th. ARCS honored Pleasantville resident Alan Shapiro and Hyde Park resident Carla Ramos for their consistent support for local residents with HIV/AIDS and community spirit at its “Harvest of Hope” gala on November 3, 2012. Organizers held the event despite worries over gas shortages and continued power outages. By the end of the night, ARCS collected just as many donations as they did at last year’s event. Alan Shapiro was selected as one of this year’s honorees for his long-term commitment to ARCS’ food and nutrition programs. Shapiro recently conducted a food drive at the Woodlands Community Temple which netted over 160 boxes of nonperishable food for ARCS’ Westchester pantry. He has been a partner at Philips Nizer LLC in Manhattan for over twenty years and practices corporate and business law when not volunteering for his community.

What do doctors do to treat their own pain? As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result. Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’s outstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead. My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well. My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable. I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare ups of low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for a few days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeutic massage, and even painful steroid injections. Nothing worked. I could walk, bend down and I had almost no pain. Since that last back episode, Dr. Donath has helped me with my tendonitis in my right hand and with my chronic knee problems. I recommend Dr. Donath to everyone with back, muscle or joint pain. Even if you have not had relief with other treatments and other Chiropractors, give Dr. Donath a chance.
