After helping hundreds of Westchester County herniated disc sufferers eliminated their pain, Local Doctor explains how…
“Back Pain Technology Cheats Mother Nature Helps Herniated Discs Heal Naturally… In As Little As 25 Minutes!” At Last! Westchester County Doctor Offers Technology That Doesn’t Just End SEVERE Pain From Herniated Discs, But Helps Your Body To Start Rebuilding Them… As Fast As Humanly Possible! Best Part: You can check it all out for FREE if you like! There is no doubt… technology has made all of our lives better… and easier. Just about every miraculous medical cure and comfort of modern living can be attributed to amazing breakthroughs in technology. Heck, laser eye surgery can give people who are almost as blind as a bat perfect 20/20 vision. How amazing is that? That’s why it’s no surprise that technology might be able to solve your back and neck pain from herniated/bulging discs or sciatica…for good. Here is why: Back and neck pain can be devastating. And nothing is worse than going to doctor after doctor… trying treatment after treatment… spending money you can’t afford… and still suffering in pain. That’s why… I bet you would love to instantly end your pain… without any risk what-soever… and have it never come back for the rest of your life? Well, that might NOT be so easy. But, what if there was a wonderful advancement in technology that, with a handful of simple, non-invasive treatments, could possibly have you out of pain and on your way to healing naturally? Better yet, what if this technology could possibly heal your herniated/bulging discs (instead of cutting it out like surgery) so you have the best chance to live pain-free and do all the things you want and love to do? And what if this technology was: 3 FDA cleared! 3 Proven safe and effective! 3 Allows your herniated discs to heal naturally without the risky and dangerous side effects of surgery! 3 Relieves pain fast… in some cases in as little as 25 minutes! Well, guess what? There is such technology and there is a very good chance it can help you… like it has already helps thousands of herniated/bulging disc and sciatica sufferers all around the country… No Matter How Bad Your Pain Is, How Long You’ve Been Suffering Or How Many Other Treatments And Doctors Have Failed You! Don’t laugh. It’s really true. Here’s proof…
When you have a herniated disc, compressive forces cause your spinal bones to come together… basically squashing your disc. The space in between your bones decreases and the soft, jello-like disc material leaks out. This disc material that leaks out is called a herniated disc and can cause severe pain, numbness and tingling in your neck, back, arms or legs when the herniation pushes into and “pinches” a nerve. So it makes sense, if you could slightly pull the spinal bones apart… that would open up the space, take the pressure off the disc, the disc off the nerve and solve the problem. Right? But the only problem is… how do you do that? For years and years, doctors have been trying to do it with traction. But traction has been proven ineffective and often very painful. Why? Traction doesn’t work because when you have a herniated or bulging disc, the area is injured and when you try to move, your muscles go into spasm. Spasm is your body’s way of trying to protect you from causing further injury. That’s why traction hurts so much. As soon as the traction starts… your muscles overprotect the area and spasm. But the good news is: Now there is technology that out-smarts your body’s natural spasm reaction. In other words: It fools mother nature! This technology is called non-surgical spinal decompression (NSSD) and here’s how it works… NSSD is very different from plain traction because it has advanced computer technology that actually senses your spinal muscles. When it starts to gently pull, it immediately knows when your muscles start to contract and it stops pulling. As soon as your muscles relax, it starts pulling again. The amazing thing is: the computer senses your muscles contraction so early… you don’t even feel what is going on. Which means you never feel any pain! In fact, the only thing most patients feel is… RELIEF!!! But the best part is: NSSD is able to gently separate your spinal bones which, in many cases, actually sucks the herniated disc material back into the disc where it belongs! This takes away your pain and allows your herniated and bulging discs to heal naturally!
And check this out: Treatments are pleasant and simple. All you have to do is lie on your back, listen to your favorite music, read a magazine… or… take a nap! The wonderful technology does everything while you simply RELAX the pain away! To sum up NSSD: • For most patients it is 100% painless… in fact many patients actually fall asleep during treatment! • Works fast! Most patients get relief after a handful of treatments – some after the very first! • Is non-invasive so it does not have the dangerous risks of surgery! • Gets to the root cause of the problem and helps it heal naturally! Isn’t modern technology wonderful? And wouldn’t you like to… Give This Great Technology A Try… For Free? If you would, there’s great news. There is a Doctor located at 1241 Mamaroneck Ave. right here in White Plains, NY who has invested hundreds of thousands of dollars in this wonderful technology and has been treating herniated/bulging disc and sciatica patients with incredible success. His name is Dr. Jonathan Donath DC, MS and he is so excited about the results he’s been getting, he would like to share it with as many pain sufferers as possible. But There Is A Catch… There’s always a catch... isn’t there? Even though this technology has helped a majority of the patients Dr. Donath has treated… it is not a wonder cure and it does not help everyone.
On the right: healthy disc. Center: Herniated disc with soft disc material leaking out. On the left: Herniated disc material compressing or “pinching” nerve.
For that reason, Dr. Donath does a complete examination on every potential patient and only accepts you if he feels you are most likely to get the pain relief and outcome you are looking for. Dr. Donath’s Special Deal For You! Because the economy is so bad and Dr. Donath wants to help you if possible, he is offering the first 17 people who respond to this article a free consultation and NSSD qualification examination. And if you qualify for NSSD treatments, Dr. Donath will also give you one treatment for FREE! And if you do not qualify, Dr. Donath will help you find the best option for you. That’s why, if you suffer with pain from a herniated/bulging disc or sciatica you should call 914-259-8104 right now. When Barbara answers the phone, simply tell her you would like your free non-surgical spinal decompression consultation and examination. If you are one of the first 17, she will schedule you within the next few days. If you are in a lot of pain or already scheduled for surgery, tell Barbara and she will be sure to get you in immediately. Maybe even today.
On right: Herniated disc with spinal bones “squished” together. Center: NSSD slightly pulls the spinal bones apart, opens up the disc space and allows herniated material to move back into the disc. Left: Herniated material completely back in center of disc allows the disc to heal naturally. Your consultation and examination should take about 60 minutes. During this time you can get all of your questiones answered in a warm and friendly environment. You will know exactly what’s causing your pain and the best possible way to solve it. Dr. Donath will explain to you all your possible options along with his recomendations so you can make the best choice for YOU.
Why Limited To 17? Dr. Donath has limited the number he will accept right now to 17 because he does not believe patients should wait and they should also get the quality personal attention they deserve. When you go for your free visit, I’m sure you will see how unique and pleasant the experience is. It’s nice to be treated like a person and not a number or based on insurance coverage. It’s also nice to know exactly what’s causing your pain and how to relieve it as fast as possible. Dr. Donath’s favorite cases are the one’s that were able to cancel back surgery after only a handful of treatments. Dr. Donath also loves when a herniated disc patient that is in excruciating pain and thinks they’ve tried everything, lays down on the NSSD table, takes a 25 minute nap… and… wakes up with all their pain GONE! Just imagine how it would feel to have all your pain finally gone. Imagine going to bed and being able to sleep the entire night through… … and waking up refreshed and energized… ready to take on the brand new day – without the pain and stiffness that has been terrorizing you. And imagine finally knowing you have the solution to your herniated disc pain. Well, you may not have to just “imagine” anymore… because… NSSD might start relieving your pain… in just 25 minutes! So, if you’d like to see if you qualify and give this amazing technology a try for free… give Barbara a call at 914-259-8104 right now. Why wait one more day in pain when the solution may be one quick phone call away? Call now before someone else gets your free spot. One More Thing It’s Important… Ever since getting this wonderful technology, Dr. Donath’s office has been flooded with calls. For that reason, if when you call, the lines are busy or you go to voice mail… just keep calling back. The possibility of living pain-free is well worth the time and effort it may take you to get through and schedule your appointment. 914-259-8104

Healthy Holiday eating You have to love the holidays, filled with parties, family gatherings, and lots and lots of food and drink! As they say, 'tis the season. It can be hard to stick to healthy eating habits you’ve worked so hard to follow all year. With so many food options to choose from, it can be difficult to avoid temptation. Here are some tips to help you avoid the dreaded weight gain during the holidays.
Never go to a party hungry. This will only cause you to eat more throughout the night. Try to have a small snack before attending so you aren’t ravenous when you arrive. While standing around a buffet table, most people tend to take a little bit of everything. This may sound like a good plan, but it can quickly add up to more calories than you need. Your best bet is to stick to the dishes that look the most appetizing to you. This way you can enjoy the foods you like, and still leave room for dessert. Remember to watch your portions. Meat should be about the size of the palm of your hand. Starches, including pasta, rice, and potatoes, should be no larger than the size of your closed fist. Try to fill your plate with fresh, healthy vegetables, which will help fill you up quicker, without adding too many calories. Watch out for appealing sauces, gravies and dips. These items tend to contain a lot of fat and calories that will quickly add up. Take caution with alcohol at parties. Many holiday drinks contain a lot of fat and calories that will quickly add unwanted pounds to your waistline. Stick with a 12 oz. beer, 5 oz. glass of wine, or a small cocktail made with fresh juice. Try to get in some type of exercise the day of a holiday party. Even if you can only find 10 free minutes, it will be beneficial to burn some extra calories before you consume a lot of them. During the holiday season, socialize and celebrate good times with friends and family. Remember to stick to your healthy eating plan so you don’t overindulge on things that will steer you off your path.
We wish you and your family safe, healthy, and happy holidays!

Helping patients sleep better, feel younger and live longer, pain free
The Gelb Center is a practice known worldwide for pioneering an integrative approach to treating TMJ, snoring and sleep disorders. In looking at our patients with a holistic eye to well being we offer multiple services including: applied kinesiology, physical therapy, massage, orthodontics, prosthodontics, as well as providing comprehensive dentistry. Our doctors are widely recognized as leaders in their respective fields. They include best selling authors and speakers whose articles have appeared in virtually every recognized dental journal. With our vast range of expertise, our practice aims to help patients to feel younger, live longer and be pain free; thus making a positive change in people's lives. A team approach with Dentists, ENT’s, Pulmonologists, Orthodontists, and Oral Myologists can effectively treat your sleep disorder. INTRODUCTION Snoring and Sleep Apnea are common conditions in our society today. They are caused by a narrowing or collapse of the airway during sleep. Snoring or sleep apnea appliances are devices that are worn in the mouth during sleep that are designed to reposition the lower jaw so that the airway is supported in such a way that air can flow more freely. This reduces or eliminates snoring and can treat your sleep apnea, which can lead to a more quiet, restful night’s sleep. CAUSES OF SNORING 70-90 million people are affected by snoring. Snoring can occur when the airway becomes partially blocked during sleep. This may occur more frequently when: ❖ You sleep on your back ❖ You are overweight ❖ You have a large neck size ❖ The tongue falls back into the throat ❖ The tonsils and/or adenoids are large ❖ You mouth breathe during sleep ❖ You are overly tired ❖ You eat or drink alcohol too late at night ❖ You have a cold or are congested Snoring becomes more common: ❖ As we get older ❖ If you are a male ❖ After menopause ❖ If you have a small airway (may be genetic)

WHAT IS SLEEP APNEA? Sleep Apnea is estimated to affect 20 million people. Sleep Apnea occurs when the airway becomes completely blocked and air is not able to move through the airway. This is commonly called Obstructive Sleep Apnea because the airway is totally obstructed. This obstruction may occur anywhere from the area of the soft palate down to below the tongue. When the airway is obstructed, the patient will awaken many times during the night with a cough or gasping sound as the body attempts to restore normal breathing. SYMPTOMS AND SIGNS The most common symptoms and signs of Sleep Apnea: ❖ Snoring ❖ Non-refreshing sleep ❖ Daytime fatigue or tiredness ❖ History of falling asleep driving, watching TV, or reading ❖ Awaken during sleep coughing, choking, or gasping ❖ Morning headaches ❖ Elevated blood pressure ❖ Cardiovascular disease Other commonly occurring findings associated with Snoring and Sleep Apnea: ❖ Gastric reflux or acid reflux with indigestion ❖ Grinding of the teeth during sleep (Bruxism) ❖ Symptoms of depression, anxiety, dementia ❖ Diabetes ❖ Mood swings
It's a time to celebrate, decorate, shop for gifts, decorate the Christmas tree or light the menorah.
Ah, the holidays. What a wonderful time of the year.
What a stressful time! The Holidays are one of the most stressful events of the year. The expense of buying gifts, the pressure of last minute shopping, and the heightened expectations of family togetherness can all combine to undermine our best intentions. Here are some suggestions can help you reduce your 'Christmas stress'. Budgeting for the Holidays For many of us, the Holiday aftermath includes massive credit card bills that can take months to clear. Christmas or Hanukkah doesn't have to be a financial headache if you plan ahead. Stress reduction strategies include: Work out a rough budget of expected costs as early as possible. This includes ‘hidden’ expenses such as food bills and overseas telephone charges. Calculate how much disposable income you have between now and the holidays. A certain percentage of this can be dedicated each week (or fortnight or month) to covering your expected costs. Don't be discouraged if the amount seems small. If you save $5, $10, or $20 per week over a year, it can provide you with a hefty nest egg. If your nest egg isn't enough to cover your estimated expenses, consider recalculating your budget to a more realistic amount. If you have trouble keeping your hands off your nest egg, consider opening a 'Christmas Club' account with a bank or credit union.
Presents If you have a large circle of extended family or friends to buy gifts for, it can be very costly. You might be able to reduce the stress and cost of Christmas for everyone if you suggest a change in the way your family and friends give presents. For example, you could suggest that your group: Buy presents only for the children. Have a Secret Santa, where everyone draws a name out of a hat and buys a present only for that person. Set a limit on the cost of presents for each person continued on next page

We have a solution! Look and feel your best!
Shopping strategies Make a list of all the gifts you wish to buy before you go shopping. If you wait for inspiration to strike, you could be wandering aimlessly around the shopping centre for hours. Cross people off the list as you buy to avoid duplication Buy a few extras, such as chocolates, just in case you forget somebody or you have unexpected guests bearing gifts. If possible, do your holiday shopping early – in the first week of December or even in November. Some well-organized people do their Christmas shopping gradually over the course of the year, starting with the post-Christmas sales. Buy your gifts by mail catalogue or over the Internet. Some companies will also gift-wrap and post your presents for a small additional fee.
Tips to reduce the stress of cooking If you are cooking lunch at home, delegate tasks. You don't need to do everything yourself. Consider keeping it simple – for instance, you could always arrange for a 'buffet' lunch, where everybody brings a platter. Make a list of food and ingredients needed. Buy as many nonperishable food items as you can in advance – supermarkets on Christmas Eve are generally extremely busy. Write a Christmas Day timetable. For example, 11.30am – put turkey in the oven. You may need to order particular food items (such as turkeys) from your supermarket by a certain date. Check to avoid disappointment. Book well in advance if you plan to have lunch at a restaurant. Some restaurants may be fully booked for months before Christmas, so don't wait till the last minute.
Relationships Stress, anxiety, and depression are common during the festive season. If nothing else, reassure yourself that these feelings are normal. Stress reduction strategies include: Don't expect miracles. If you and certain family members bicker all year long, you can be sure there'll be tension at gatherings. Avoid known triggers. For example, if politics is a touchy subject in your family, don't talk about it. If someone brings up the topic, use distraction and quickly move on to something else to talk about. Use relaxation techniques, such as deep breathing or focusing on your breath to cope with anxiety or tension. Family members involved in after-lunch activities (such as cricket on the back lawn) are less likely to get into arguments. Plan for something to do as a group after lunch if necessary.

ASK AN EXPERT You Can Save Hopeless Teeth without Implants By Edward Feinberg DMD Although implants were available in 1993, Ms. B. did not want them. She was not a particularly good candidate for implants because the low-hanging sinus cavity would not allow the placement of implants long enough to support dentistry without bone grafts to raise the level of the sinus. She had an odd pattern of tooth loss, with missing teeth only on one side. Implants should not be placed when there is a poor foundation or if a patient does not want to go through the expense, surgery and healing time. Ms. B. chose to restore her mouth with the precision attachment case. Most practitioners are not able to offer this option for restorative treatment--they are more adept with implants and they are biased in their beliefs that fixed bridgework is always a better option than removable bridgework. Not true! The Precision Attachment Case When there are not enough teeth for fixed bridgework, the precision attachment case is a great restorative option--equal to or better than fixed bridgework with implants. The precision attachment case has a much longer track record than implants. Very few practitioners know much about precision attachment partial dentures as demonstrated in this article-even though the concept was invented in 1906. Most patients are only offered the clasp type of removable partial denture with visible hooks that grip or lock onto the remaining teeth. Clasps and locking mechanisms apply destructive forces to the teeth that support the removable and usually result in the loss of these teeth. As a result, this type of removable is really transitory to a full denture.
This is simply amazing! Most partial dentures that utilize attachments contain components that require constant replacement. Saving Hopeless Teeth
Figure 1
In a precision attachment case, the fixed bridgework houses “female receptacles” (locks) and the partial denture contains a male “key” that is machined to fit precisely into these receptacles. The attachment is so precise that any male out of the box will fit any female with perfection. The precision attachment partial denture is not locked in place. It cannot come out during normal function because the path of insertion of the partial denture is different from pull of the muscles and gravity. If a stress is applied to the partial denture, it can move slightly to dissipate the stress, but it cannot come out. The result is health and longevity of the teeth that support the partial denture and preservation of the tissues underneath. The precision partial denture is worn 24 hours a day and is only removed for hygiene purposes. No one can tell that it is removable bridgework, as the teeth are custom made to hide all evidence of the attachment. The precision attachment partial denture has been made at our since 1950 and has a track record of lasting longer than any other type of case (implants included)—even on the weakest teeth imaginable. It is rare for the males on the partial denture to be replaced—even after decades of continuous wear.
In 2000, Ms. B.'s upper left bridge supporting the removable partial denture snapped off at the gumline. Teeth that have had root canal or become non-vital also become brittle and prone to fracture. Ms. B. did not want to remake her bridgework, since making new bridgework to fit an existing precision attachment case--a difficult challenge when that demands precision to tolerances of tenths of millimeters to be successful. Most practitioners cannot even save teeth that are fractured at the gumline without building the teeth up in some way. Every dental school teaches techniques that require tooth structure above the gum to retain bridgework. A great deal of time and effort is expended on building the teeth up or making posts for this purpose. This approach often fails. When bridgework fails, it is a sure bet that the post will come out with it. However, there is a precision technique with copper bands that has been used in crown and bridge-
work for over seventy years that only requires tooth structure below the gum for success. Crown and bridgework made with this technique fits with perfection. Since 1950, our office has made thousands of restorations with this technique. All of them are worn for a trial period with Vaseline or rubber prior to permanent cementation and they do not fall out! They also have margins that are far below the gum and are not visible. Because they are designed like the Mason Jar cover (the best device ever invented for food preservation), decay is a rarity—even in patients who are highly susceptible to decay! We were able to make two crowns for Ms. B. that we attached to her existing bridge with acrylic. This bridge has the housing for her precision attachment case, so adding the crowns requires a high degree of accuracy. Ms. B.'s bridge now fit like brand-new! It has served her well for the past twelve years--supporting her partial denture under continuous forces, under water and in an environment loaded with bacteria. There have been no changes in her X-Rays during all this time--an absolute miracle! Precision Dentistry is Least Expensive in the Long Run
Figure 2
Ms. B. knows that precision dentistry is the very best investment she ever made for her oral health, her smile, her ability to eat anything and her peace of mind. Her dentistry has given her far more pleasure and quality of life than any vacation, piece of jewelry or fancy car.

ATTORNEYS AT LAW 15 YEARS OF ELDER LAW EXPERIENCE
Why should I spend money hiring attorneys to do asset protection planning for me and my loved ones? Currently, the monthly cost for nursing home care in New York ranges between $10,000-$15,000, and properly certified home health attendants (nurse aides) can cost as much or more for round- the-clock home care. Statistically, it is likely that most people will need either or both of these care options as they grow older. It is critical that you plan appropriately to best minimize the strain that elder care costs may have on your estate, and to help put you in the best position possible to qualify for benefits that are largely covered by the government, under Medicare or Medicaid. Our firm has attorneys with extensive experience in developing tax-effective action plans for our clients that help protect their assets from government imposition. We have also been very successful in negotiating the complex bureaucratic minefield in which Medicaid is now mired. We have achieved remarkable results for our clients on a host of Medicaid related issues, from obtaining Medicaid approvals for coverage of home care and institutional care benefits, to very accommodative resolutions of Medicaid spousal recovery claims, where the office of Medicaid aggressively pursues the non-institutionalized (i.e., or “community”) spouse for Medicaid benefits provided.
Why should I have a lawyer prepare my will? If you die without a Will, the law, not you, determines how your assets are divided and to whom they are given. A poorly drafted Will may result in a sizeable chunk of your assets not ending up where you want them to go. We are an experienced law firm that can expertly craft your will to address your long-term care, tax, estate planning, and asset protection planning issues. It is in your best interest that planning be done properly so as to minimize the tax burden on your estate. Our experience and skill in the fields of Trusts, Wills, and Estates enables our firm to uniquely tailor estate planning to your individual needs. We will review your entire estate and draft a personalized plan that will best direct the disposition of your assets after death, whether through your Will, or by means of testamentary substitutes (trusts, joint-asset ownership with right of survivorship, or by beneficiary designation in life insurance policies, annuities, or individual retirement accounts).
My father is having difficulty caring for himself. I am afraid he will no longer be able to live on his own. I am worried about finding the money to pay for his care, whether he stays at home or moves to an assisted living facility or nursing home. What do I do?

What makes a good doctor? A degree from a top medical school? A good bedside manner? We all choose our physicians based on different criteria. But what makes a top doctor? One of the many perks of living in Westchester County, is the abundance of great doctors. They are innovators in their field, cutting-edge, renowned. This edition of Healthcare News presents Westchester’s Top Doctors. Of course, there were so many to choose from: We selected 10 and told their stories. Just because one of your doctors didn’t make the list, doesn’t mean you should search the internet for a new one. The doctors included in Healthcare News listings were selected carefully after peer nomination, extensive research and careful review and screening by our editorial staff. We also consulted, online, the directory of top doctors provided by Castle Connolly Medical Ltd. The mission of Castle Connolly Medical Ltd. is to help consumers find the best healthcare. They publish a variety of books including the “Top Doctors” series, the most popular of which is America’s Top Doctors®. These Top Doctors’ medical educations, training, hospital appointments, disciplinary histories - and much more - are screened by the Castle Connolly physician-led research team. Doctors do not and can not pay to be listed as a Healthcare News Top Doctor.
Dr. Michael Tobias, M.D., Co-Chief of Pediatric Neurosurgery, Maria Fareri Children’s Hospital at Westchester Medical Center The prospect of brain surgery is daunting, but it’s especially frightening when the patient is a child. Fortunately, the pediatric neurosurgeons of Maria Fareri Children’s Hospital at Westchester Medical Center are equipped to successfully treat the youngest and smallest patients using the latest technology—including minimally invasive techniques that can help with surgery, and sometimes eliminate the need for open surgery. Leading the way for a truly talented group is Tobias. Complex neurological disorders in children such as epilepsy, brain tumors, spina bifida and hydrocephalus often require a multi-disciplinary team of specialists to provide the most comprehensive and up-todate treatment.
For Tobias, there are two elements to providing the best patient care: surgical skill and attention to a child’s emotional well being. As Chief Resident in Neurosurgery at Albert Einstein College, Dr. Tobias sought to improve both sides of the equation by redesigning a shunt system used to treat children with hydrocephalus (excess fluid on the brain) and establishing a new curriculum for neurosurgery residents to promote better communication. He feels strongly that caring for children poses unique challenges, but that’s what makes the job so rewarding. When treating children, he says, medical specialists really have multiple patients to consider -- the child and his or her parents. Specialists also need to collaborate closely with the child’s pediatrician and be sensitive to the child’s emotional well being. Tobias received both his undergraduate degree with honors and his medical degree with honors in neurosurgery and pediatric neurosurgery from Brown University. He completed his internship and residency in neurosurgery at Albert Einstein College of Medicine, Bronx, NY, where he participated in the establishment of a new educational curriculum for resident training in pediatric neurosurgery. He then trained as a Pediatric Neurosurgery Fellow at LeBonheur Children’s Medical Center and St. Jude Children’s Research Hospital, in the Department of Neurosurgery, University of Tennessee, Memphis, TN. Tobias has also co-authored multiple academic publications.

Dr. Roy Ashikari, M.D., FACS, Hudson Valley Hospital Center (HVHC) Ashikari started a new journey-opening the Ashikari Breast Center at Hudson Valley Hospital’s new cancer center in Cortlandt Manor last November. HVHC basically welcomed a legend. Chief of The Breast Service at Sloan Kettering during the 1970’s and 1980’s, Ashikari has been the premier senior breast surgeon in Westchester County for the past two decades. He has written the seminal research papers on Paget’s disease, Ductal (DCIS) and Lobular (LCIS) Carcinoma In-Situ, Male breast cancer, and breast cancer in pregnancy. Ashikari has opened his new Cortlandt Manor location to better reach patients in northern Westchester, Rockland, Putnam and Orange counties. The Ashikari Breast Center is located beside the Emergency Department and consists of Ashikari, his son Andrew Ashikari, and Dr. Pond Kelemen. With Roy Ashikari, semi-retired, his son and Keleman continue to provide some of the best care in the field. The Ashikari team has pioneered a breast reconstruction procedure that has attracted women from all over the world, most notably a nipple-sparing procedure that allows them to remove cancerous or potentially cancerous breast tissue, minimizing the trauma of a mastectomy. Many women, including those who are cancer-free, but have high-genetic risk and are seeking prophylactic surgery, favor this approach. What sets the Ashikari team apart from others is their determination to use the latest and more effective techniques to eradicate cancer while fully supporting and treating the patient as a whole person.

Mitchell Cairo, M.D., Pediatric Oncology – Maria Fareri Children’s Hospital at Westchester Medical Center Fresh out of medical school, Cairo took an interest in researching treatments and cures for pediatric cancer. The year was 1979, the cure rate for children with different types of cancer was about 20 percent: It’s now 80 percent.
Devine is a wonderful doctor, she knows medicine. Combine that with her personable bedside manner and you have one of the best. Devine prides herself on taking time with her patients. Whatever it takes: Devine has spent as many as three hours with a patient on a visit.
Cairo has played an integral role over the last three-plus decades in developing life-saving treatments for pediatric hematology and oncology patients. He is a pioneer in the field, a premier figure in the pediatric cancer and blood disorder subspecialty, especially research.
But Devine’s reach at SSMC reaches even further. Devine was recruited to develop an Antepartum Testing Laboratory (ATL) for SSMC. Her successful development and launch of the program positioned the hospital to be the first institution-based facility in Westchester County to achieve accreditation by The American Institute of Ultrasound in Medicine. It helped secure SSMC’s New York State Department of Health designation as a Perinatal Hospital.
Last year, Maria Fareri Children’s Hospital hired Cairo as its new Chief of Pediatric Hematology, Oncology and Stem Cell Transplantation. He runs a new Stem and Cellular Therapy and Engineering Laboratory on the grounds of Westchester Medical Center that will give patients across the East Coast access to cuttingedge treatments for forms of blood cancer and other diseases.
Through her hard work and dedication, Devine has nurtured the ATL’s elevation to a Center of Excellence for First Trimester Screening. Devine evaluates treats more than 1,400 women who experience high-risk pregnancies each year.
Cairo came to Maria Fareri Children’s Hospital from New York-Presbyterian’s Morgan Stanley Children’s Hospital, where he was Director of Pediatric Blood and Marrow Transplantation. He has been the principal or co-principal investigator of a dozen clinical research studies and a contributor to nearly 300 peer-reviewed publications.

Karen Pechman, M.D., director of physical medicine and rehabilitation – White Plains Hospital Center and Burke Rehabilitation Hospital What would this list be without a local girl made good? Pechman graduated from the Bronx High School of Science. From there, it was on to even greater things. In 1980, at age 22, Pechman completed an accelerated medical program at Boston University. Pechman returned home to New York, where she received a training grant to conduct research in immunology at NYU. There, she worked near a rehabilitation center, watching doctors working with a variety of patients. She saw physicians help people rehab serious injuries, she watched in awe as they helped patients get their lives back – and Pechman was deeply moved. All these years later, Pechman helping patients, including many who need prosthetics. Pechman uses cutting-edge techniques and products, including the “myoelectric” arm. This breakthrough hand’s open and close in response to electrical signals from muscles. She uses other innovative techniques such as electromyogram (EMG), which identifies nerve damage in patients with spinal-cord injuries. It was invented by one of Pechman’s mentors, Dr. Edward Delagi, of Albert Einstein College of Medicine

BONNIE S. SILVERMAN, M.D. Voted Best Ophthalmologist in Westchester the last 7 years in a row Dr. Youmin Wu, M.D. Chief of Intra-abdominal Transplant and Hepatobiliary Surgery, Westchester Medical Center Westchester Medical Center landed itself another star when it announced the hiring of Wu in June. Wu has performed over 800 liver and kidney transplants over the course of his career and was a member of the four man surgical team that performed the world’s first multi-visceral organ transplant. Wu has made many valuable contributions to the field of transplantation and cancer research and may be best known for pioneering cavaplasty, one of the most widely used surgical techniques in liver transplantation. Developed during his tenure at the University of Iowa Hospitals and Clinics, the cavaplasty technique shortens operative time and minimizes the opportunity for postsurgical complications. With this technique Wu is credited with performing the youngest living related pediatric liver transplant (19 days old) as well as transplanting the oldest recipient with the longest survival and the oldest donor with the longest survival. Wu has also pioneered new concepts involving early diagnosis and combined radiation therapy in specific types of liver transplantation—treatment regimens which have become the standard of care, prompting UNOS to change its liver transplant policies. Wu has developed and led kidney and pancreas programs in deceased and living donor adult and pediatric liver transplant at the University of Iowa and the University of Arkansas where in 2004 he launched the State’s first and only liver transplant program which was ranked number one in the nation for best outcomes by Healthgrades Born and raised in Mainland China, he has devoted significant resources and time to promoting transplantation in his native country. Wu travels to China regularly to teach, lecture and serve as a special advisor to the country’s Minister of Health.

Erik Larsen, M.D., Assistant Director of Emergency Room – White Plains Hospital
Monique Regard, M.D., pediatric and adolescent gynecology specialist – Westchester Medical Center, Valhalla
Larsen has the stuff you see on television shows. Think ER or Grey’s Anatomy. But there is a big difference between Larsen and the TV dramas. He is the real thing.
In years past, a young girl experiencing a gynecological issue such as an ovarian mass could not receive care from a gynecologist, because she was too young. Today, there is a new subspecialty: pediatric and adolescent gynecology. Regard is one of the few pediatricians throughout the nation whose entire practice is devoted to this subspecialty.
What Larsen did to help save a patient three years ago is still the stuff of legends. With the 49-year-old mother of two clinging to life, Larsen made a quick decision that you only see on Grey’s Anatomy. He used cooling blankets recently purchased by the hospital but not tried, with no protocol established, and induce therapeutic hypothermia, to stave off brain damage that so often accompanies this kind of without-warning massive heart attack. And it worked. Thanks to the enterprising Larsen, the work of his staff and EMT’s, the patient survived. Larsen has an extensive history of progressive leadership and administrative experience with Emergency Medicine. He has experience in flight, disaster and wilderness medicine. Most notably, Dr. Larsen was the Medical Director at the New Orleans International Airport during Hurricane Katrina as well as the Medical Director of the NDMS Rapid Needs Assessment Team during Hurricane Wilma. In addition, he traveled with the NYC Medics Response Team during the 2006 Pakistan Earthquake. In 2006 Dr. Larsen received the NY State Department of Health/Emergency Medical System Council Award for Physician of Excellence.
Regard’s specialty integrates several areas of medicine such as surgery, endocrinology and dermatology. She treats patients who range in age from infancy through adolescence. Parents bring their babies or young daughters to see Regard for very specific reasons. As a gynecological surgeon with two complementary sub-specializations Regard stresses the importance of treating both young girls and adolescents. After attending Stanford University, she completed medical school at Baylor College of Medicine and her Obstetrics and Gynecology residency at the University of Minnesota. Following her training, she practiced for eight years in St. Paul, Minnesota were she became the city’s principal Pediatric and Adolescent Gynecology specialist. On staff at the St. Paul Children’s Hospital, she regularly taught in the residency program and treated girls with complex gynecological disorders.
Pete Richel, M.D., Pediatrician – Northern Westchester Hospital, Mount Kisco He’s affectionately known as Dr. Pete. Richel has developed his own remedy for children with doctor’s office stress: He sings and performs funny routines to help ease the anxiety and pain of his patients. Dr. Pete, chief of pediatrics at Northern Westchester Hospital, has been singing to his patients in his office for 22 years. Dr. Pete has even released children’s CD’s – with all proceeds going to special needs children – and performs at local bookstores. Dr. Pete worked in commercials on his way to becoming a pediatrician. Some of his favorites artists are The Eagles, James Taylor and Michael McDonald. However, when he’s working a room in his office, you can catch Dr. Pete singing one of his trademark songs such as “Hot Shots”: A typical Dr. Pete visit combines his affection for music with his desire to heal. The songs and rhymes aren’t just for entertainment: They teach patients about their bodies, and walk them through check-ups and procedures.

Generate Retirement Income More Efficiently Retiring at last. You have earned it by every measure. But now, you face an important transition in your personal financial plan. You have finished accumulating retirement assets and are now beginning to distribute them as income to support your retirement lifestyle. The choices you make now can certainly impact your long-term financial situation. Preserving Your Assets Building a systematic withdrawal plan around a sustainable withdrawal rate is a smart effort for making your money last, but it may not be enough. Assume you have a retirement portfolio worth $1 million. Initially, annual withdrawals of 5% ($50,000) seem conservative. You may be surprised to learn however, that at that rate there is only a 90% chance your money will last just 20 years.1 In other words, one in ten people following this seemingly conservative withdrawal strategy could potentially out live their retirement nest egg later in life. In addition to careful consideration of withdrawal rates, retirees must be aware of the significant risk market volatility poses. Erratic swings in equity-based investments can decimate portfolio values without warning - seriously threatening potential future income since percentage-based withdrawals shrink as total assets are eroded. Holding onto what you have - asset preservation – is a top priority for retirees. Though you may not know how long you’ll live, you do know that running out of money is not a scenario you hope to experience. If only there were guarantees… The good news is: there are. Get Guaranteed Life-long Income A Guaranteed Lifetime Income Annuity2 is a financial product that does, in fact, guarantee life-long income. To illustrate, imagine that same hypothetical million-dollar portfolio. From it, you could use $842,3343 to purchase a Guaranteed Lifetime Income Annuity, which will provide the $50,000 per year of income you had planned to live on. But, this $50k is now guaranteed for as long as you live, regardless of market performance. No more worrying about outliving your savings. Plus, you would still have $157,663 left in your portfolio to access as you wish, making the Guaranteed Lifetime Income Annuity significantly more efficient than a simple withdrawal strategy. In case of premature death, the Guaranteed Lifetime Income Annuity has a Cash Refund option, which returns to beneficiaries the difference between the initial premium and whatever has been already paid out. If inflation is a concern, an Annual Increase Option4 can be added to the annuity contract at purchase -- initial annual payments would start off smaller, but increase each year between 1 – 10% according to the owner’s preference. Over time, the total annually adjusted payouts would potentially provide more income than their unadjusted counterparts. Systematic withdrawal plan rates, and associated portfolio duration based on hypothetical allocation between equities and bonds; 125 basis point annual fund management expense. This illustration is based on a Monte Carlo model simulation of 1,000 scenarios each of correlated equity and bond returns using an economic scenario generator. Correlations estimated using historical monthly S & P 500 Index and Barclay's U.S. Aggregate Index returns over 20 years (1989-2009). Equity and bond returns are assumed to be normally distributed. Each withdrawal rate is adjusted annually for 3% inflation. This model is intended to be an indicator of potential returns at various confidence levels and is not designed to be a forecast of future investment performance. A Monte Carlo simulation analyzes probabilities and seeks to identify the number of years your profile will last, given your financial profile. The simulation is hypothetical, does not reflect past performance, and is not a forecast of future results. Simulation results may vary with each use and over time. Each client’s results depend on individual factors which a simulation does not account for, such as how investment decisions are implemented in reality and the costs of investing. A simulation may not capture how asset classes fall in and out of favor over time. These factors can influence a client’s results materially. 2 New York Life’s Guaranteed Lifetime Income products are issued by New York Life Insurance and Annuity Corporation, (A Delaware Corporation), a wholly owned subsidiary of New York Life Insurance Company. Guarantee is based on the claims paying ability of the issuer. Product available in jurisdiction where approved. 3 Payout based on female, 75 years-old, Single Life with Cash Refund payout option, with 3% Annual Increase Option as of 10/31/2011. Payout amounts vary depending upon age, gender, premium amount, payout option selected and interest rates at time of policy issue. In general, income annuities have less liquidity than investment portfolios. Guarantee is based on the claims paying ability of the issuer. Payments consist of interest and return of premium. 4 Must be selected at time of purchase, policy owner must be at least age 59 ½ at the time of the first payment. Other restrictions may apply.

Healthcare News - Westchester December, 2012
Like so many other areas in the northeast, Westchester County was hit hard by Hurricane Sandy. In some parts of Westchester, the damage was devastating, and it will take several weeks to recover. Even though Sandy has left, it's still important to remain safe during clean-up and restoration efforts. Here are important post-Hurricane safety tips. Stay safe! Carbon Monoxide Gasoline-powered generators release carbon monoxide (CO) which can be deadly. If your power goes out and you are using a gasoline-powered generator for energy, be sure that the generator is installed safely and far from your home. Never use your generator, charcoal or gas grills, gas lanterns or camping stoves indoors or in an enclosed area, like a garage. Gas stoves can produce carbon monoxide. Do not use your gas oven to heat your home. Chain Saw Safety Before moving or cutting tree limbs, make sure there are no wires tangled in the limbs. If you have little or no experience using a chain saw, hire a licensed professional to remove tree limbs and downed trees safely from your property. Wear eye/face protection, foot protection, hard hat, ear protection, and chainsaw chaps. Drinking Water After a storm, water from public water supplies and private wells may not be safe to drink. Water that is dark, has an odor, or has floating pieces should not be used. Listen to the news or your water company to know if your water’s safe. Private wells that are flooded or may be contaminated should be disinfected before use. Electrical Wires DO NOT TOUCH downed electrical wires. Report it to your local utility company, police or fire department. Do not turn power back on until a qualified electrician has inspected the property and says it is safe to do so. Ask him/her to check wiring, appliances, and mechanical system components like, the furnace, water heater, and gas valves, to see if they can be salvaged or will need to be replaced.
Fires Prevent fires by using flashlights or battery powered lanterns, if possible. If you must use candles, place them in safe holders, away from anything that can burn and never leave unattended. Flooding Flooding can contaminate your well, making your water unsafe. Information on what to do if your well is contaminated. Medications If you have to evacuate or leave your home, be sure to bring your medications and medical supplies with you. You should bring enough medication and medical supplies to last you for a few days. If you have a question on how to store your medication, including medication that requires refrigeration, you should contact your pharmacist. Persons who depend on medical supply deliveries and plan to temporarily stay at another address – with a neighbor, family member or at a shelter – should notify their medical supply providers to have their supplies delivered to the temporary location with no lapse in their care. Sewage safety Severe rainstorms, floods, and even spring thaws can put a strain on sanitary sewers and septic systems. Large volumes of storm water and ground water entering sewage systems can flood them, causing back-ups into basements and on to private property. Blockages in sewer systems can also cause back-ups. Your health may be impacted if a sewage back-up occurs in your home. Sewage back-ups can contaminate your private drinking well water. It can also pollute surface water (lakes, ponds, rivers, and streams, and reservoirs used for drinking water).

In the aftermath of Hurricane Sandy, County Executive Robert P. Astorino has advised homeowners and businesses needing repairs to use caution when selecting a contractor to avoid scams and being gouged. "Common sense is a consumer's greatest asset," said Astorino. "Use it. People who have had their homes damaged are upset and very vulnerable. Opportunistic individuals can and will take advantage of this." John Gaccione, the acting director of the county's Consumer Department, said that the county will enforce any laws against gouging. "Taking advantage of consumers in need and raising prices without any justification will be investigated by the department," he said. Home improvement contractors working in Westchester must be licensed. A licensed contractor will have the necessary contractor insurance. To find out if a contractor is licensed, check online at www.westchestergov.com/consumer. Here are other tips from the county's Consumer Department: If you haven't done so, call your insurance company as soon as you can. Many insurance companies have existing relationships with home repair contractors. Watch out for contractors who are suddenly in the area. Fly-by-night contractors will take advantage of homeowners in need. Don't get rushed into signing a contract or agreeing to repairs. Avoid contractors who demand payment in cash or want more than one-third of the total payment before work has begun. Do not agree to an oral contract. Get everything you can in writing. Insist on a written contract that sets forth all the important details of the job. That includes a detailed scope of work, a start date, a completion date, cancellation clause, warranty information and a schedule of payments. Get at least three estimates. In reviewing the estimates, don't always go with the lowest bidder. A higher bid may be worth it if the contractor is the most experienced, uses better materials and keeps to a timeline. Consumers who need to purchase essential goods and services need to take notice of what they are paying for these items and should contact the department if prices seem to be much higher than normal. The Department of Consumer Protection can be reached by e-mail at conpro@westchestergov.com, or by phone at (914) 995-2155. You may also visit www.westchestergov.com/consumer.

The Westchester Lung Nodule Center at Phelps Memorial Hospital Center Sleepy Hollow, NY • (914) 366-2333 • www.lungnodulecenter.org
The early diagnosis of lung cancer, before symptoms occur, provides the best chance for cure according to Avraham Merav, MD, co-director of the Westchester Lung Nodule Center at Phelps. Early lung cancer is most often detected as a nodule on a CT scan. Since the increased use of diagnostic CT scanning and the recent introduction of CT screening for lung cancer in high-risk individuals, nodules are being discovered ever more frequently. The majority of such nodules are benign, and it requires a great deal of expertise to identify the malignant ones.
Dr. Merav and Dr. Rocco Lafaro, with more than 60 years of combined thoracic surgical experience, joined to establish the Westchester Lung Nodule Center at Phelps, providing comprehensive evaluation for patients with recently diagnosed lung nodules and offering low-dose CT scan screenings for individuals who are at high risk for developing lung cancer, particularly those with a history of smoking.
Each case is thoroughly reviewed by the Center’s select team of specialists who are board certified in Radiology, Pulmonology, Pathology, Oncology and Thoracic Surgery. Every attempt is made to determine if the nodule is benign or malignant with the least invasive methods available. Quite often, only observation is recommended. For those patients who do require a surgical intervention, the Center offers the benefit of an experienced team who perform the majority of their procedures using minimally invasive thoracoscopic (VATS) techniques. Patients are seen within 48 hours of their call for an appointment. Ongoing communication is maintained with each patient’s primary care physician or other referring physicians. Phelps distinguished itself in 2009 by becoming the first hospital in Westchester to report its thoracic surgery cases to the National Database of The Society of Thoracic Surgeons. Continued reporting assures our patients that Phelps’ results are in line with the national experience.

PAIN IN THE NECK She’d been living with Bob’s chronic neck pain for years. They were told to just live with it. But it was ruining their lives. So they sought a solution. One doctor prescribed physical therapy, but the pain persisted. Another doctor proposed neck fusion surgery, but Bob didn’t want to limit his mobility. So they went to Phelps Memorial Hospital Center. There, an experienced orthopedic surgeon accurately diagnosed Bob’s condition and performed the most advanced surgical solution that could preserve his mobility and treat his pain – an artificial disc replacement. And then they lived happily ever after.

Hudson Valley Hospital Center receives $150,000 grant to improve breast cancer care Women treated for breast cancer at Hudson Valley Hospital Center will have more choices thanks to a $150,000 grant the Hospital is to receive from New York State with the help of Senator Greg Ball. The money will help to pay for equipment that will allow the Ashikari Breast Center at Hudson Valley Hospital Center to perform intra-operative radiation on women undergoing breast cancer surgery. The procedure will be available to patients starting the second week in November. “The Ashikari Breast Center at Hudson Valley Hospital Center offers women the best cancer care close to home,’’ said breast cancer surgeon Dr. Andrew Ashikari recently at a press conference at the Hospital. “Now women who could benefit from intra-operative radiation don’t have to travel elsewhere to get it. This is a great addition to the Hospital and a victory for women with breast cancer.’’
Sen. Greg Ball said he was happy to advocate for his community in Albany. “The stresses associated with battling breast cancer are overwhelming and providing world class services locally, without the additional stress of travel and hardship of leaving the familiarity of their community is exactly why I couldn’t be happier to deliver this $150,000 grant to the Hudson Valley Hospital Center. I am proud to assist in making the Cheryl R. Lindenbaum Cancer Center the destination of choice for cancer patients in the Hudson Valley,” said Senator Greg Ball. "It is a great honor to be able to deliver this money for the hard working doctors, nurses, staff and the entire Hudson Valley Hospital Center community, especially as we embrace the struggles of our mothers, sisters, aunts and other beloved ones during breast cancer awareness month."
Dee Curtis, who was diagnosed with breast cancer in June 2011, said she had no idea what Intra-operative radiation was or how the treatment could help to save her breast before speaking with Dr. Andrew Ashikari. Back then, Hudson Valley Hospital Center’s Cancer Center had not yet opened so Curtis had to travel to another Hospital to have her surgery. “I was going to have a mastectomy, but Dr. Ashikari said with this treatment, he could save my breast,’’ said Curtis who lives in Yorktown with her husband. “ I also had 1 less week of radiation, which was a good thing because if I had to go one more week, I wouldn’t have been able to stay in work. I was just that tired.’’ continued on next page

Hudson Valley Hospital President John Federspiel said the Hospital chose to use the grant money and invest an additional $413,000 to buy the intra-operative radiation equipment because it is committed to making the Hospital’s Cheryl R. Lindenbaum Cancer Center the destination of choice for cancer patients in the Hudson Valley. “Cancer patients should not have to add extra travel to their already hectic lives during such a stressful time’’ said Federspiel. “The philosophy of our Cancer Center is to provide quality, comprehensive cancer care close to home. It should be no different when it comes to women who choose to have intraoperative radiation.’’ Intra-operative radiation (IORT) allows selected breast cancer patients undergoing breast preservation surgery to receive one dose of radiation while asleep during surgery, compared with 6-1/2 weeks of radiation after surgery. In higher risk patients, it is used to boost the surgical cavity with radiation to reduce the post-op radiation to only 5 weeks. Women who undergo IORT only and develop a recurrence can then undergo a repeat lumpectomy and whole breast radiation, while those who get standard radiation and develop a recurrence are recommended to have a mastectomy. The Ashikari Breast Center
has participated in the largest international trial of IORT and will be active in the American trial, which will begin soon. The IORT procedure at HVHC will be overseen by the Cancer Center’s Department of Radiation Oncology under its Medical Director, Dr. Chika Madu. “Not everyone is a candidate for IORT, but in many women undergoing a lumpectomy it provides a less stressful option to traditional whole breast radiation treatments,â€? said Dr. Pond Kelemen of the Ashikari Breast Center. The Ashikari Breast Center came to Hudson Valley Hospital Center in July 2011 and their offices are located in the Hospital’s Cheryl R. Lindenbaum Comprehensive Cancer Center, which opened in November 2011. Leaders in their field, The Ashikari team has pioneered a breast reconstruction procedure that has attracted women from all over the world, most notably a nipple-sparing procedure that allows them to remove cancerous or potentially cancerous breast tissue, minimizing the trauma of a mastectomy. A Valuable Medicaid Planning Solution Today many people are involved in “Medicaid Planning” with a certified elder law attorney. What does this kind of planning mean and why do they do it? In order to become eligible for Extended Medicaid coverage, an individual must not exceed their state’s minimum income & asset requirements1. This usually requires that a person transfer assets out of their estate in order to protect those assets from expensive long-term care bills. Moreover, any gifts or transfers have a 60 month look back period regarding eligibilty2. Most people are familiar with the “Medicaid Spend-Down” scenario and how devastating this can be to most families financially. Paying for the cost of long-term care of a loved one dollar-for-dollar is never a good option. Many families are financially ruined because of the cost of extended care. It is essential that every person carry out their advanced planning regarding long-term care services. Otherwise, these expenses can easily run into the hundreds of thousands of dollars. So what medical bills can be counted toward a spend-down? • • • • • • • •
Your own medical bills. Your spouse’s medical bills. Parent’s bills for their children’s spend down. Bills of a child living with you. Bills of a child who does not live with you, but whose medical bills you help pay for. Past unpaid medical bills (sometimes up to 6 years old) for yourself or any of the people named above. The part of any medical bill not covered by Medicare or private insurance. Medical expenses (including insurance premiums) paid for you by certain public programs, for example, the Elderly Pharmaceutical Insurance Program (EPIC) and the Child Health Insurance Program (CHIP).
Despite knowing about this potentially enormous liability, many folks still do not purchase long-term care insurance because they see it as too expensive. They just do not want to pay the premiums every year nor do they want to have to worry about if they can afford the policy if those premiums increase sometime in the future. So instead, they decide to transfer the majority of their assets out of their estate to facilitate eligibility for Medicaid benefits in five years. Having said that, there still remains the danger of a claim during the transfer period. Often, attorneys recommend that the Medicaid Planning involve the purchase of a temporary long-term care policy with a five year benefit period in order to “bridge” the liability of the 60 month look back period as per the Deficit Reduction Omnibus Reconciliation Act 2005. Once the look back period has expired, the client can cancel their coverage and avoid paying any future premiums. This strategy however still costs money, typically $30,000 to $50,0003. The following planning takes this concept to the next level… What if the client has the means to purchase a Single Premium Universal Life Insurance policy with a Long-Term Care Rider instead of the traditional LTCI policy in order to “bridge” the gap? These contracts are often referred to as “Linked” policies because during the 60 month look back period the client not only has adequate long-term care insurance, but also a guaranteed life insurance death benefit that is higher than the original deposit. And what if the policy can be cancelled for a 100% return of premium after the 60 month look back period with no surrender charges? Sounds too good to be true? Well, two carriers currently offer linked products with guaranteed return or premium riders4. Thus, the client can buy the insurance and keep it in force until such time as they no longer need it and their assets are fully protected. This strategy completes the entire leveraged transaction at no cost to the insured. EXAMPLE: John is a 68 year old male who purchases a single premium $100,000 Linked Universal Life Insurance policy with a Long-Term Care Rider to insure his liability while he waits for his assets to become Medicaid ineligible in 60 months. The $100,000 single premium buys him $190,000 of life insurance and $398,000 of long-term care coverage from day one. Assuming there are no claims, at the end of five years, he can surrender the contract and receive 100% of his premium back with no surrender charges. The transaction is essentially a 60 month FREE loan with some very significant tax-free insurance benefits5. Call us today if you would like additional information about this cutting edge estate planning concept. What do doctors do to treat their own pain? As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result. Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’s outstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead. My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well. My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable. Dr. Robert Sidlow MD, Associate Professor of Medicine at Albert Einstein College of Medicine
I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare ups of low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for a few days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeutic massage, and even painful steroid injections. Nothing worked. When I moved to Westchester, I met Dr. Jonathan Donath. In just a couple of sessions with Dr. Donath, the pain completely disappeared. If it hadn't happened to me, I wouldn't have believed it. During my last flare up, I was expecting to be on my back for 2-3 days and in pain for 2 weeks. Instead, after one session with Dr. Donath, I was able to walk; two more sessions and I was fine. I could walk, bend down and I had almost no pain. Since that last back episode, Dr. Donath has helped me with my tendonitis in my right hand and with my chronic knee problems. I recommend Dr. Donath to everyone with back, muscle or joint pain. Even if you have not had relief with other treatments and other Chiropractors, give Dr. Donath a chance. I did and he changed my life. Eugene Dinkevich MD, Mount Vernon, NY It’s my great pleasure to write about Westchester Spinal Decompression Center. I really don’t have words to express my feelings about the care I got in last 3 months. I was almost handicapped, living with terrible pain all the time, crippling my day to day life. I am a medical doctor and work in a hospital with all sorts of amazing physicians, but after seeing a physical therapist, getting epidural steroids injections, many orthopedists and neurosurgeons, finally my destination lead me to see Dr Jonathan Donath. With great skepticism, I lied down for spinal decompression therapy. It was unbelievable that after only two initial sessions, my radiating pain down my leg had almost disappeared. Gradually, I started getting better, started doing exercises that they gave me midway through the treatment. Now, I am doing all my daily activities without any pain. I highly recommend Dr Jonathan and his colleague Dr Kavita for any problems related to the musculoskeletal system, you will definitely feel at home and will get better.
