Although your comfy old sneakers may make your feet happy at first, the amount of support and stability may not be sufficient. Choosing a shoe that fits your activity is essential. Blisters aside, joint pain, ankle sprains and stress fractures may be in your future. These days, shoes are engineered to help you achieve peak performance. Not sure where to begin? Let’s first consider the basic functions of the foot: • Provides a base of support for stability and balance • Absorbs shock and impact • Provides you with propulsion and force to move forward Personal foot structure, flexibility, and conditions also need to be considered. Celebrate National Men’s Health Week, June 9-15, with your loved ones In June, you will celebrate that special man in your life on Father’s Day. Move More Adults need at least 2½ hours of moderate-intensity aerobic activity (such as brisk walking) every week, and muscle strengthening activities that work all major muscle groups (legs, hips, back, abdomen, chest, shoulders, and arms) on two or more days a week. You don’t have to do it all at once. Spread your activity out during the week, and break it into smaller chunks of time during the day. Tame Stress Sometimes stress can be good. However, it can be harmful when it is severe enough to make you feel overwhelmed and out of control. Take care of yourself. Avoid drugs and alcohol. Find support. Connect socially. A sleep mask will keep the light out and help turn any situation into a more restful one.
For his mind: Recent studies show that adults with hobbies that regularly exercise their brains are 2.5 times less likely to develop Alzheimer’s disease, so get Dad something to keep his mind in shape, like puzzles, models, or even certain video games.
For his thirst: It’s safe to say that most of us don’t get the full recommended eight glasses of water a day. Help your dad get there by buying him a reusable water bottle for work and play. There’s a water bottle for everyone, from outdoorsman to tennis player.

Estate Planning 101: Avoiding Family Conflict By Andrew J. Cavaliere, CLTC How many times do you hear about family or adult children fighting over inheritance after mom and dad are gone? It happens more often than not. For whatever reasons, mom and dad fail to take into account the complexities of the situation revolving around their remaining children and the legitimate claims by others. What’s worse is that most of these family squabbles could have been easily avoided with some basic estate planning. Let’s look at a few scenarios: 1- Mom & Dad owned a tool and die company for 50 years and have three children. The oldest has worked diligently at the family business for 25 years, while the other two younger children pursued successful careers outside the business in law and medicine. After mom and dad passed away, they leave the tool & die business to the oldest son and unfortunately there are very few other family assets. Regardless of the fact that the oldest son worked for 25 years at the shop, the younger siblings resent him receiving 100% ownership of the family business. Conversely, the oldest son feels no obligation to give an equal share to his sister and brother in an operation they have never participated in. Looking back, one solution here would have been to have mom and dad purchase a permanent life insurance policy on themselves. By naming the two younger children as beneficiaries, they would immediately have provided a cash inheritance equal in amount to the value of the business. This way the oldest son gets what he feels he deserves without any financial conflicts with his siblings. 2- John is a 55 years old widower with two adult children whom he loves very much. John decides to marry a much younger woman named Barbara. Unfortunately, John’s two children are not particularly fond of Barbara or her children from her prior marriage. After 15 years of being married to Barbara, John’s health is terminal and the family is beside themselves. Upon his death, his estate plan calls for most of his assets to be placed in a trust for the benefit of his children from his first marriage. The majority of these assets are in real estate properties that have been in the family for decades. The children do not want to sell these buildings and summer homes any time soon as the rental properties provide tremendous income and the vacation homes have sentimental value beyond any sales price. But they know Barbara, as his legal wife for the past 15 years, will want her fair share of everything once dad is gone. And rightfully so, she is legally entitled to it, but the children cannot afford to buy her out. Looking back, one solution here would have been to have John purchase a permanent life insurance policy on himself while he was healthy and name Barbara as the primary beneficiary. This way his children could retain control of the family properties and she could receive her fair share of his estate with no malice attached. 3- Nick & Linda are young doctors and are married for 10 years when Linda suddenly gets sick and dies. They have three children and one million dollars in life insurance each. Fortunately, Nick moves on in life and five years later marries his second wife Carol. Carol insists that Nick name her as his primary beneficiary on both his life insurance policy and brokerage accounts. Nick agrees because what are the chances of him dying, right? Tragically, a few years later, Nick is killed in an automobile accident working late one night. Not only do his children suffer the loss of both parents, but because Nick failed to plan properly, they are effectively disinherited of almost all of their assets to Carol. Whether Carol is capable of doing the right thing is immaterial, all of Nick’s money is hers legally and a fight is imminent between her and the children’s next of kin. Looking back, one solution here would have been to have Nick purchase additional life insurance on himself while he was healthy and name a Trust as the primary beneficiary for the benefit of his dependent minor children. Updating his will and/or his revocable living trust would have been essential as well. As always, any estate planning of this kind should be conducted with the full knowledge and guidance of a qualified and trusted estate attorney and/or CPA. Understanding Your Cholesterol Numbers Robert Pilchik, MD, Chief of Cardiology at Northern Westchester Hospital, doesn’t want you to just know your allimportant cholesterol numbers. He wants you to understand them in order to remain heart healthy. First, a short review of cholesterol basics: The two types of cholesterol in your body – lowdensity lipoprotein (LDL) and high-density lipoprotein (HDL) – behave totally differently. Over time, excess LDL in your body, largely from fatty foods you eat, builds up in your artery walls as hard plaque, narrowing these vessels and restricting blood flow to your heart and brain. This condition, atherosclerosis, is THE leading cause of heart attack and stroke. By contrast, HDL acts as a vacuum, ridding the arterial walls of cholesterol and flushing it from the body. Because it’s very common to experience no symptoms while your arteries are already badly blocked, it is a potentially lifesaving measure to get regularly tested for cholesterol levels. Over time, your test results give you and your physician clues as to the health of these essential arteries. Sound familiar? You get blood work done, and are given a number for “total” cholesterol.
You’re told that a total over 200 is worrisome. However, the total cholesterol figure commonly given patients is actually a very deceptive number. It can mean different things in different situations. That’s why I’m primarily concerned with your numbers for LDL and HDL cholesterol. Now you know why you should always ask your physician for the breakdown of test results for your LDL and HDL cholesterol levels. Your total cholesterol number is actually a formula that includes HDL cholesterol, LDL cholesterol, and a certain fraction of the level of your triglycerides. Taken together, these three numbers form your “lipid (fat) profile.” Triglycerides are part of what remains when your body
breaks down the fats you eat. The link between high triglycerides and artery-clogging plaque is much weaker than with cholesterol. However, people with high triglycerides often have low levels of the cholesterol-busting HDL cholesterol. If your triglycerides are high, your HDL is low, and you have fat around the belly, it’s likely you have metabolic syndrome, a condition that starts with insulin resistance and can lead to diabetes. This condition also puts you at increased risk for developing heart disease. How often should you get tested for cholesterol? Every three years, if you’re between 20 and 30 years old, and have not had cholesterol issues. Yearly, if you’re 40 and over, and haven’t had cholesterol

problems. If you’re on medication for high cholesterol, every six to 12 months. There is no evidence that women or men are at greater risk for elevated levels of LDL cholesterol. OK, you’ve been tested. What are desirable numbers? For HDL cholesterol, if you’re male, your goal is to have greater than 40 (mg/dl or milligrams per deciliter of blood). If you’re female, your goal is greater than 50. For LDL, if you have no history of heart disease, your LDL should be less than 130. If you have a history, you want to be below 70. Now let’s say you have great numbers. The doctor and nurse congratulate you. Are you off the hook? No! You need to maintain those good numbers.

It was the best feel-good moment of the week when approximately 6,000 residents joined together in support of the smallest citizens, our Westchester babies — by participating in the 2014 March for Babies to benefit the March of Dimes. Saxon Woods Pool was packed with strollers, families and corporate teams who raised more than $800,000 to help babies be born healthy.

New Sports Medicine program at HVHC Each year 1 in 100 female high school athletes will suffer a serious knee injury. The Center for Rehabilitation at Hudson Valley Hospital Center is doing something about it with a new Sports Medicine that specializes in injury prevention, performance enhancement and rehabilitation of sports injuries. “Athletes, especially young female athletes, are prone to injury of their ACL and it can be devastating to the athlete and the team,” said Kyle Mack, PT, DPT, Clinical Manager, Department of Rehabilitation. "These injuries have become an epidemic and can sideline athletes for 6-12 months, but are preventable with the correct training. The center is now also offering a SportsMetrics™ ACL Injury Prevention program to schools and individual athletes. The center also offers Return to Sports Programs to help athletes recover from injuries more quickly and return to sports safely. Created under the direction of internationally-renowned orthopaedic sports medicine surgeon, Frank R. Noyes, M.D., Sportsmetrics™ is the first and only training program scientifically proven to decrease knee injuries in female athletes. It is a six-week jump training program that meets three times per week on alternating days and is comprised of a dynamic warm-up, jump/plyometric drills, strength training and flexibility exercises. Knee ligament injury rates are 2-10 times higher in female athletes than in male athletes. The majority of these injuries are non-contact, occurring in sports that involve pivoting, cutting or jumping. The ACL controls the pivoting and forward motion of the knee joint, along with the hamstrings muscles. If these muscles are not adequately prepared, jumping/twisting motions may create shear forces, which can tear the ACL. Hudson Valley Hospital Center’s Center for Sports Medicine is now offering this program. The centers’ team of highlytrained therapists work with the area’s top orthopedic specialists. When smartphone is near, parenting may falter Mealtime is supposed to be family time, but a new study suggests that ever-present smartphones are impeding parent-child communication at the table. Researchers who observed more than 50 family-type groups eating out found a significant number of adults were preoccupied with their smartphones. “We know from decades of research that face-to-face interactions are important for cognitive, language and emotional development. Before mobile devices existed, mealtime would’ve been a time where we would’ve seen those interactions,” said study author Dr. Jenny Radesky, a fellow in developmental-behavioral pediatrics at Boston Medical Center. It’s unclear how much of an impact parents’ smartphone use will ultimately have on a child’s development, and whether that effect will have a negative, positive or neutral impact, said the researchers. One basic thing that may be affected is child vocabulary, said Dr. Rahil Briggs, director of pediatric behavioral health services at Montefiore Medical Center in New York. “The single most powerful predictor of a child’s vocabulary is conversations with the child. Dinnertime is an important time for those conversations, and if you’re absorbed with your phone, that’s a lost opportunity.” Briggs added that social, emotional and conversational skills might also be affected if parents spend too much time looking at their phones and other media. “What really concerned me was those children who appeared to accept this lack of engagement. It seemed like they’d given up,” noted Briggs, who wasn’t involved with the research. To capture a snapshot of how parents use their phones around their children at mealtime, Radesky and her colleagues visited Boston-area fast food restaurants and observed groups that included at least one adult and
one or more children who looked younger than 10. They took detailed notes on how caregivers -which likely included parents, grandparents and babysitters -used their smartphones and how children responded. They conducted 55 observations last summer. Forty caregivers took out their phones at some point during the meal. A few kept it on the table, but didn’t appear to use it. Another small group kept their phones in their hands while doing other things. The largest group -- 16 caregivers -- seemed totally absorbed by their phones, using them continuously, even eating and talking while looking at the phone. In most cases, it appeared the caregivers were using the phones’ keyboards or making swiping motions on the phones rather than making phone calls. Another nine caregivers used their devices intermittently, and then put the phone away. The researchers said these caregivers appeared to balance use of the device and paying attention to the child or children. While the adults used their phones, some school-aged children were busy eating, talking to another child or playing with the toy that came with their meal, and didn’t seem concerned that the caregiver was on a device, especially if it was for a short period of time. When caregivers were completely absorbed with their phones, some children just seemed to accept it. However, many other children started acting up in an attempt to garner the caregivers’ attention. Some of these caregivers appeared to ignore the child’s behavior for a bit and then scolded them, sometimes without even looking up from the phone. Radesky and Briggs agreed that smartphone use isn’t all bad. Sharing apps and games with kids can be a way to connect. And smartphones certainly won’t be going away anytime soon.

What do doctors do to treat their own pain? As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result. Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’s outstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead. My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well. My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable. I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare ups of low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for a few days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeutic massage, and even painful steroid injections. Nothing worked. When I moved to Westchester, I met Dr. Jonathan Donath. In just a couple of sessions with Dr. Donath, the pain completely disappeared. If it hadn't happened to me, I wouldn't have believed it. During my last flare up, I was expecting to be on my back for 2-3 days and in pain for 2 weeks. Instead, after one session with Dr. Donath, I was able to walk; two more sessions and I was fine. I could walk, bend down and I had almost no pain. Since that last back episode, Dr. Donath has helped me with my tendonitis in my right hand and with my chronic knee problems. I recommend Dr. Donath to everyone with back, muscle or joint pain. Even if you have not had relief with other treatments and other Chiropractors, give Dr. Donath a chance.
