Dr. Kerline Marcelin, an ophthalmologist at Hudson Valley Hospital Center in Cortlandt Manor, is on a mission. Literally.

Marcelin is one of the many physicians across Westchester County who gives back. In this case, Marcelin also went back to her roots to help another community in need. Marcelin, who teaches at the New York Eye and Ear Infirmary, traveled to Haiti from January 18-25 with the Crudem Foundation to work at the Sacred Heart Hospital (Hôpital Sacré Coeur) in Milot.

What made the trip even more special is that Marcelin’s family is from Haiti.

She specializes in treating glaucoma, which is often referred to as the “sneak thief of sight” because it has no symptoms and can end in blindness if left untreated. Marcelin says that it is a particular problem in third world countries like Haiti where treatment is not readily available.

Breakfast foods for a faster metabolism Protein Eating enough protein will ensure you both maintain and build muscle mass. Lean muscle mass is more metabolically active than fat tissue, so the more muscle you have, the more calories you'll burn-even when your body is at rest.

Breakfast idea: Aim for at least 20 grams of protein at breakfast, which you'd get from a six ounce container of nonfat Greek yogurt topped with two tablespoons chopped walnuts and 1/2 cup berries. Nonfat or low-fat Greek yogurt, eggs or egg whites, nuts and seeds, and protein powder are all helpful ways to boost A.M. protein intake.

Hot Sauce Capsaicin (the natural chemical in hot peppers that makes them hot) can give you a small metabolism boost. It increases your metabolism and body temperature by making your muscles burn energy as heat.

Breakfast idea: Have an egg and egg white on whole wheat toast with 1/4 avocado and a hearty drizzle (as much as you can handle) of hot sauce or a sprinkle of red pepper flakes.

Green Tea Both the caffeine in green tea and the natural plant chemicals will give you a small metabolism boost‌just be sure not to go overboard with milk, sugar and other additions. Aside from boosting your calorie burn, the drink is packed full of antioxidants that protect against disease and, of course, provide a natural glow to skin.

Calcium-Rich Foods Some research indicates that people who are calcium deficient may have slower metabolisms. Aim for 1000mg each day from food or food plus supplements.

Breakfast idea: Nonfat or low-fat milk, yogurt, and cheese are all good sources, as are calcium-fortified dairy alternatives (almond milk). Whip up a smoothie using 1 1/2 cups milk or fortified milk alternative and you're already almost a a quarter of the way to your daily needs.

Kids and Food: 10 tips for parents It’s no surprise that parents might need some help understanding what it means to eat healthy.

From the MyPlate food guide to the latest food fad, it can be awfully confusing.

The good news is that you don’t need a degree in nutrition to raise healthy kids.

Following some basic guidelines can help you encourage your kids to eat right and maintain a healthy weight.

4. Start them young. Food preferences are developed early in life, so offer variety.

Likes and dislikes begin forming even when kids are babies.

5. Rewrite the kids’ menu. Who says kids only want to eat hot dogs, pizza, burgers, and macaroni and cheese? When eating out, let your kids try new foods and they might surprise you with their willingness to experiment.

6. Drink calories count. Soda and other sweetened drinks add extra calories and get in the way of good nutrition. Water and milk are the best drinks for kids.

7. Put sweets in their place. Occasional sweets are fine, but don’t turn dessert into the main reason for eating dinner.

8. Food is not love. When foods are used to reward kids and show affection, they may start using food to cope with stress or other emotions.

9. Kids do as you do. Be a role model and eat healthy yourself. Choose nutritious snacks, eat at the table, don’t skip meals.

10. Limit TV and computer time. When you do, you’ll avoid mindless snacking and encourage activity.

A heart screening before competing?

Some cardiac tests aren’t needed for all young athletes

If you have a son or daught er active in sports, you’ve probably heard the marketing and promotion of electrocardiogram (EKG) and echocardiogram testing for all young athletes before they participate in school or spare-time sports.

Advertisers point to a tragic phenomenon known as sudden cardiac death (SCD), which claims the lives of a very few U.S. child and adolescent sports participants each year.

No one disputes the worthy goal of protecting kids. But not all young athletes really need pre-participation cardiac testing, says Michael Gewitz, M.D., Physician-in- Chief at Westchester Medical Center’s Maria Fareri Children’s Hospital and its Chief of Pediatric Cardiology.

For youngsters with no known risk factors, such detailed testing, which is not inexpensive, may cause more harm than it uncovers, says Dr. Gewitz. “This screening is not perfect,” he explains.

“There are a host of cardiac problems these tests do not pick up. On the other hand, the tests can be overly sensitive

and produce a lot of false-positive findings. Such inaccurate results can create unnecessary anxiety for parents and children and may lead to inappropriate restrictions.”

For almost all kids, regular vigorous exercise is one of the keys to a healthy cardiac future.

So which young athletes should be given an EKG or echocardiogram to assess their heart risk before they take the field? Specifically, those with a known risk of or predisposition toward heart problems, says Dr. Gewitz.

“Any family history of sudden cardiac arrest is one red flag,” says Dr. Gewitz.

And Christa Miliaresis, M.D., a pediatric cardiology exercise specialist at Maria Fareri Children’s Hospital, agrees. “In a physical, a doctor must ask if the child ever passed out or fainted during sports,” she says.

If either of these questions draws a yes, EKG and/or echocardiogram testing can be an important next step to evaluate the danger and guide appropriate parental decisions.

Childhood obesity Turning the Tide

Being overweight carries a host of health problems, including higher risks for diabetes, hypertension and heart disease. There are many ways you can help your child live a healthy lifestyle and avoid childhood obesity. Here are some guidelines:

Your child should engage in some physical activity for one to two hours every day. It would be ideal for her to get outside for sports, tag or jump rope.

Limit screen time to one hour a day — and no screen time at all for children younger than 2 years of age.

Don’t give your child soda or other sugary beverages such as sports drinks and juices. Water and milk are the best beverages for children.

Make your meals as healthy as possible.

Serve whole-grain foods, such as w hole-grain bread, cereal, rice and pasta; protein-rich foods, such as lean meats, seafood, beans, nuts and seeds; calciumrich foods, such as low-fat milk, fat-free or low-fat yogurt and cheese, and don’t forget fruits and vegetables, and stay away from fried foods. REMOVING LIFE INSURANCE RATINGS

By Andrew J. Cavaliere, CLTC Most people believe that after you buy a life insurance policy everything about that policy is set in stone. This is simply not the case at all. If you can prove via paramedical testing that you stopped smoking or you lost 50 pounds, you are certainly eligible to re-underwrite your policy and attain a less expensive annual premium. Yours truly has done this dozens of times. In fact, non-smoking status can decrease your policy premiums by as much as 50% depending upon your age. It is also a great incentive to get your loved one to quit.

In order to make a profit, a life insurance company needs to calculate policy premiums that fund future claims and cover administrative costs. The cost of insurance is determined using mortality tables calculated by its own actuaries. Actuaries are professionals who determine future probabilities based upon mathematics, primarily historical statistics. Mortality tables are statistically based tables showing expected annual mortality rates.

It is possible to derive life expectancy estimates from these mortality assumptions.

The three main variables in a mortality table are commonly age, gender, and use of tobacco. The mortality tables provide a baseline for the cost of insurance, but in practice these mortality tables are used in conjunction with the health and family history of the individual applying for a policy to determine premiums and insurability.

Most life insurance companies separate applicants into four general categories. These categories are preferred best, preferred non-smoker, standard non-smoker, and standard smoker. Preferred best is reserved only for the healthiest individuals in the general population. This may mean, that the proposed insured has no adverse medical history, is not under medication for any condition, and his/her family (immediate and extended) have no history of early-onset cancer, diabetes, or other chronic health conditions.

Most people fall in the standard category. Professional occupation, travel history, and lifestyle factor into whether the proposed insured will be granted a policy and which category the insured falls into. For example, a person who would otherwise be classified as preferred best may be denied a policy if he or she travels to a high risk country. Underwriting practices vary from insurer to insurer and they are not all the same.

More than 100 people packed the hospital lobby for a presentation of the Hospital’s “Acts of Kindness Awards” presented by HVHC President John C. Federspiel. Recognized at the event were: Nancy Montgomery and her husband the late Jim Lovell of Cold Spring. Lovell, who died in the Metro North train derailment in December, was recognized posthumously as part of the Hospital 125th Anniversary’s theme – 125 Acts of Kindness. Also honored were Ellen Buccatello of Peekskill, an advocate for the homeless and hungry, chaplain and member of the Hospital’s Ethics Committee, and a team of employees who head the Hospital’s annual Santa’s Helpers holiday gift drive.

Hudson Valley Hospital Center was opened in 1889 in the City of Peekskill after a group of community minded women raised $1,800 to purchase property on lower South Street. The hospital was known as the Helping Hand Hospital until 1911 when it was incorporated as Peekskill Hospital. In 1966, the hospital moved to its current location in Cortlandt Manor. It was renamed Hudson Valley Hospital Center in 1992 to reflect its new status as a regional facility.

WHUD’s Kacey Morabito Grean served as Mistress of Ceremonies at the event and there was a performance by violinist Daisy Jopling. Public officials including Westchester County Health Commissioner Sherlita Amler and mayors and supervisors of Peekskill, Cold Spring, Cortlandt Manor and Yorktown attended.

How much is too much?

Our bodies need nutrients to stay healthy. The body makes a certain amount itself; the rest comes from our diet.

As we age, food intake and metabolism changes increase our risk for nutritional deficiency. Vitamin and mineral supplements may become necessary. Consult your doctor to determine your specific needs; supplements may have an adverse effect on medications.

Supplements are not a substitute for a nutritious diet!

The absorption of vitamin B-12 from meat, salmon, cod, diary and fortified cereals is affected by stomach acid. Twenty percent of people over fifty do not have enough stomach acid to absorb sufficient amounts of vitamin B-12. A deficiency can lead to anemia, memory loss and gait problems. The recommended daily dosage of vitamin B-12 is 2mg or less. An excess of any vitamin may cause harm.

Folic acid (folate) , found in dark green, yellow and orange fruits and vegetables, beans, lentils, nuts, seeds and fortified grains (pasta and flour), can protect against cardiovascular disease and stroke, help the liver eliminate toxins, promote growth and reproduction of body cells and aid in the formation of red blood cells and bone marrow. Taking 1mg (1000 micrograms) or less of folic acid supplements daily is unlikely to cause harm.

A major antioxidant, Vitamin E, found in fatty foods such as nuts and oils supplies oxygen to the blood, provides nourishment to cells, prevents blood clots and slows cellular aging. The recommended daily amount of vitamin E for men is 4mg and 3mg for women.

Vitamin C, a major antioxidant found in fruits and vegetables, is essential for healthy teeth, gums and bones. It also helps heal wounds, fractures and scar tissue; builds resistance to infections; and assists in the prevention and treatment of colds. Vitamin C is also known to block damage caused by free radicals which are largely responsible for the aging process, and may play a role in cancer, heart disease and conditions like arthritis.

Calcium and vitamin D from dairy products, green vegetables and nuts is needed to maintain strong bone health. Vitamin D, which improves the absorption of calcium and phosphorous, diminishes as we age because of our inability to digest dairy foods. Our skin produces vitamin D from the sun, however as the skin ages, its ability to produce vitamin D significantly decreases.

The recommended daily dose for adults is 700mg of calcium and ten micrograms (0.01mg) of vitamin D.

Five foods you think are healthy but aren’t Many people don’t realize they’re sabotaging their attempts at healthy living with food and drinks that masquerade as nutritious but actually aren’t.

Eating better is often a simple matter of reading labels and nutrition information to figure out exactly what’s in whatever it is you’re ingesting.

Many products purport to be healthy when they’re actually packed with sugar, preservatives and other chemicals. Instead, aim for the fewest ingredients possible, and eat the real stuff instead of faux health food.

Here are five primary offenders: 1. Pre-made salads Good for you! You went out to Chili’s and ordered a salad because you’re trying to be healthy. Sorry, but you should have ordered nachos, southwest eggrolls or loaded potato skins, because any of those is healthier than some of Chili’s calorically blessed salads. Same is true of most fast-casual eateries and fast food restaurants. Once you add cheese, meat, croutons and creamy dressings to a salad, the fat piles up fast. Sure, taco salads are great, but if you must indulge, try one with black beans, grilled chicken, a little cheese and salsa instead of salad dressing.

2. Diet sodas Please tell me you know by now that diet soda is just as bad for you as regular soda and part of the reason that Americans struggle so much with obesity. Diet soda can increase your risk of metabolic syndrome, making you an ideal candidate for heart disease. The artificial sweeteners in diet soda disrupt your body’s ability to regulate calorie intake, causing you to eat more and gain more weight.

3. Dried fruit In some fruits, dehydration causes the nutrients to become more concentrated, which is great. However, many dried fruits, such as cranberries and cherries, often have added sugar, along with nasty preservatives called nitrites. Also, when you eat an apple, you’re consuming all the water in the fruit, which helps hydrate and fill you up. When you eat 20 dried apple rings like they’re chips, you’ll probably still be hungry, and you’ve just eaten the equivalent of several apples. Instead, just eat fruit. It’s cheaper than the dried stuff, anyway.

4. Beef jerky Pure protein. What could be bad about that, right? Wrong.

Though the protein in jerky is great, in order to dehydrate the meat and keep it safe for consumers, nitrites and other preservatives are added. Nitrites produce carcinogens in the body, which could contribute to colon cancer. Beef jerky is also usually loaded with salt. Because it’s too delicious to give up on entirely (and can’t not eat beef jerky on road trips), look for organic jerky with no preservatives and as little sodium as possible.

5. energy energy bars If you must eat an energy bar, think of it like a candy bar.

It’s an indulgence or a sweet snack, not a meal replacement.

Energy bars are loaded with sugar, and most of them are also packed with enriched white flour, saturated fats and corn syrup, in addition to the few vitamins the wrapper touts on the front.

Additionally, a small energy bar isn’t likely to fill you up for long. Lara Bars and Kind Bars are made with mostly nuts, and the sugar is kept to a minimum. When you need a burst of energy in a hurry, try those instead.

What do doctors do to treat their own pain?

As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result.

Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’s outstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead.

My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well.

My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable.

I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare ups of low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for a few days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeutic massage, and even painful steroid injections. Nothing worked.

Since that last back episode, Dr. Donath has helped me with my tendonitis in my right hand and with my chronic knee problems. I recommend Dr. Donath to everyone with back, muscle or joint pain. Even if you have not had relief with other treatments and other Chiropractors, give Dr. Donath a chance. I did and he changed my life.
