Blaze guts house, bringing in multiple departments An afternoon blaze that seemed to start in the rear of a private home left the house uninhabitable. Firefighters from Dan Leghorn Fire Department were alerted to a fire at approximately 3:30 p.m. on Monday, June 17th. They arrived at 7 Princess Lane in the Meadow Hill section of the Town of Newburgh to find flames and smoke coming from the rear of the bi-level style private dwelling. A second alarm was quickly called, bringing in mutual aid from

area departments as well as Mobile Life Support Services and Town of Newburgh Emergency Medical Services. Firefighters stretched several lines to the interior and exterior area of the home working quickly to bring the fire under control. A rehab area was set up as temperatures in the area were close to

90 degrees. Fortunately, not injuries were reported. An hour into the fire, command still reported an active fire situation, but firefighters worked diligently to soon bring it under control. Overhaul commenced and all hot spots were doused. The dwelling was deemed uninhabitable and the cause of the fire is under investigation by local officials. - HEATHER PILLSWORTH

Ogden, NY. The Spencerport Fire Department responded to a motor vehicle accident on Tuesday afternoon, June 11th, on NYS Route 531 at the overpass of Union Street in the Town of Ogden. Arriving apparatus and chiefs found a single car accident with entrapment and the car hanging over the abutment of about 20 feet. Many aspects of technical rescue were deployed including securing the vehicle to the ground, using ropes to secure personnel, and vehicle extrication. Limited access also presented a problem due to the divided highway overpass. Spencerport paramedics special requested a RSI paramedic from the Barnard Fire District and later, the Monroe County Medical Direc-

tor to the scene. Additional resources included a heavy wrecker from Jim’s Service in Churchville. Coincidentally, T&R Rescue Solutions had just conducted a three day training seminar on the use of towing and recovery equipment at incidents that many on scene personnel had attended. The highway was closed in both directions for about two hours. One woman was transported with serious injuries to Strong Memorial Hospital in Rochester. - JOHN SPAULDING

Fire in Rouses Point causes extensive damage On June 3, 3013 an alarm at 7:08 a.m. alerted firefighters to a fire in Rouses Point. The fire with an unknown exact cause, started in the vicinity of a plastic outlet box in garage. The fire’s spread was assisted by broken propane gas line and actuation of garage door opener. The garage was heavily involved with fire spreading to the attic and interior portions of the house, upon arrival of first units. There was heavy heat, smoke and fire damage to first floor

JUMP TO FILE #061213125

and minimal water damage to the finished basement. Initial action was an interior search from the rear as it was not known if occupants were in town. Intense heat prevented the search of more than one bedroom. A rapid knock down was made with CAFS. A chipboard floor in the attic made overhaul difficult as there

was a lot of personal effects stored there and minimal head room. A 2012 Camaro in driveway was heavily damaged. Radiant heat prevented removal before the fire department’s arrival. All firefighting units went available by about 9:30 a.m. On scene mutual aid arrived from from Champlain, NY, Alburgh, VT, Lacolle, QC and 35 Air from Mooers, NY. Mooers also stood by in RP station.

Garage fire in Maybrook Maybrook, NY - Several departments in Orange County were alerted to structure fires by the 911 Center on Monday, June 17th. One of those departments was the Maybrook Fire Department. Firefighters received an alert for a garage fire at 109 Bole Circle at a little after 10 a.m. They arrived to find smoke showing from the garage area of a bi-level style home. A FAST team from Washingtonville was quickly dispatched. Maybrook had a quick stop, but not before some extension occurred to the residence. Overhaul was performed and the power company was called in to disconnect utilities. The cause and origin of the fire is currently under investigation.

Firefighters rescue over a dozen felines from fire Greenport, NY - Greenport firefighters responded to a residential structure fire at 14 Charles Street in the Columbia County town of Greenport with a report of numerous pets still inside. First responding fire chiefs reported heavy smoke and fire in a bedroom. Two neighbors were using a garden hose to knock down the fire. The first arriving engine finished extinguishing the blaze, containing the fire to the sole occupant's bedroom. Heat and smoke damage was sustained throughout the dwelling. At the same time, reports from the scene indicate firefighters rescued 17 cats from the blaze, six of which were given oxygen. The house was ventilated and an investigation determined the fire to be accidental and caused by unattended candles.

Grand Gorge EMS receives cutting edge training Bill had a pulse, he certainly was breathing at one point, and with a stethoscope you could listen to his lung sounds. After getting a blood pressure, noting that his oxygen levels had dropped and listening to him speak, Bill suddenly went into cardiac arrest. The work of the Grand Gorge Rescue Squad EMTs began to go into overdrive. Not a real emergency, but on Wednesday, May 8, 2013, a state-ofthe-art manikin named Bill visited the Grand Gorge Fire Department. The squad hosted the drill and invited all area squads to attend. Bill,

JUMP TO FILE #060713104

an interactive training manikin, simulated a heart attack and provider’s skills were put to the test as their performance was recorded on a video camera for later analysis and critique. This training provided an opportunity for its squad members to work on what actually appears to be real emergency medical situations, but are actually simulations. Two cardiac scenarios were presented for the squad members to respond.

Greg Vis of Hudson Simulation and Jon Behrens of SUNY Cobleskill provided the attendees with excellent training. It is a part of a research project funded by the U.S. Department of Agriculture aimed at improving EMS education and in turn EMS response and treatment. Its goal is also to help small rural squads with retention and recruitment issues by providing exceptional training experiences - as well as boost emergency care in rural communities. - KAREN HINKLEY

Youngstown volunteers report line of duty death It is with heavy hearts that the Youngstown Volunteer Fire Company reports the death of one of our own, a member of The Youngstown Volunteer Fire Company and The Youngstown Volunteer Fire Company's Explorer Post 911. Corporal Thomas Burley, age 20, has been in our youth explorer post for over four years and had recently become a probationary firefighter with The Youngstown Volunteer Fire Company. Tom was always around to give a helping hand in the community and put his service above himself. Tom was also part of the Air Force Military Police and an active member of the Lewiston Police Department’s Explorer Post. He was planning on putting

himself through the academy in hopes of becoming an officer with Lewiston Police Department or the Youngstown Police Department. Tom no doubt, would have made an excellent firefighter and police officer someday. There will always be a seat in our apparatus for Tom. On his way home from Firefighter 1 training on Church Road in the Town of Cambria, Tom’s motorcycle collided with a van headon. He was in critical condition roadside and succumbed to his injuries at Mt. St. Mary’s Hospital. The NCSO is investigating the accident. The Youngstown Volunteer Fire Company extends our deepest condolences to the family and friends of this wonderful young man. - YOUNGSTOWN  FIRE COMPANY

Flooding in Rush puts department to work On June 13th, the rain began at 8 a.m. coming down in buckets. The small streams that feed the Honeoye Creek and Genesee River through the center of town were flowing like no one had ever seen before. At 12:30 p.m., the calls started coming in. Rush had 15 water in basement calls (several were eight feet of water), two ambulance calls and one gas leak in a six hour pe-

Trauma Junkie, Memoirs of an Emergency Flight Nurse ON THE BOOK SHELF On The Book Shelf by John Malecky

Trauma Junkie Memoirs of an Emergency Flight NurseUpdated and Expanded By Janice Hudson Published by Firefly Books, Ltd, Richmond Hill, Ontario Available from your local book seller or online book sellers Price $19.95 (paperback) This book measures 6” x 9”, has 272 pages with 24 pages of photos, maps and glossary. You can probably imagine that if you are a flight nurse assigned to an emergency medical helicopter ambulance, it is likely that the calls you respond to are for individuals who are seriously injured or sick. Many wind up dead from their injuries or complications. So it is with this in mind that I advise the reader that if you have a sensitive comprehension to reading about graphic material, this book may not be for you! I have found by talking with many people who are readers, that some cannot stomach the content of some books even though in most cases there are no accompanying photos. The author became a flight nurse in 1987 after leaving a full time job as an emergency room nurse. After many years as a flight nurse she later became a full time, certified registered nurse anesthetist. She accidently found out one day after stopping to visit her old quarters, that her book was required reading for all new flight nurses at CALSTAR (California Shock/Trauma Air Rescue), which is an air ambulance in the San Francisco Bay area. You will notice that the current book is updated and expanded so there was a previous printing but it was sold out. There are 16 action packed chapters which hold multiple incidents including the earthquake in 1989 (during the baseball world series coverage) and a major, fatal Oakland brush fire. She writes with a technique that causes the reader to empathize with what is going on and imagery which tends to place you on the scene. Having had a background in fire and EMS, I envisioned myself at the emergency departments of a hospital and scene locations, not because I had actually been to any of these places, but because of her expert writing technique. The book is also written in an honest way, explaining that things do not always go as planned or ex-

pected. Many times they had to work amidst terrible negative odds caused by the results of the conditions and circumstances leading up to the acci-

dents. The book also has some station life segments and brief home life pieces. The afterwards chapter lays out her life after CALSTAT and

touches on what many of her former coworkers are currently doing since they left the job. Again, I believe this is an action packed book that you may have diffi-

culty putting down, especially if you still have some time to put aside for reading.

The Forgotten Epidemic EMS ISSUE CHELLE CORDERO

It sounds like a story straight from a TV movie… the old lady is ignored, her meds aren’t refilled, the small pension she receives is grabbed up by the one distant relative who supposedly cares for her, her meals are inadequate, her hair is unkempt and there’s a sinister son just waiting for her to pass on so he can sell the house and fund his next vacation. Unfortunately, the TV movies you’ve seen with this sad plot are too often inspired by real life stories. We get called to a house and we know in our gut that the sweet elderly man who called 911 needs help. He craves companionship because he’s been ignored by his family, whose prescription medication ran out days before, whose bedsores haven’t been treated. His caregiver rushes in and chastises the man for wasting the time of the emergency responders and tells you that the man is “just getting senile”. When you note welts and bruising on the patient you’re told the man falls all the time. When you try to ask a question about the patient’s medical history, the caregiver gets defensive and stories change. It’s hard to walk away and not care. It’s also hard to know what’s right when others are telling you that with age comes dementia, lack of balance, forgetfulness, lethargy and more. Common signs of possible elder abuse or neglect may include: burns; skin bruising of different colors; repeated visits by EMS for injuries; delay in seeking medical care for injuries; dehydration; request for transport to other than usual ER; the extremes of caregiver attention-overbearing to neglectful; evidence of alcohol in the household; and/or the household in disarray. Psychological or

financial abuse may not manifest itself with physical signs. It is also not uncommon for the suspected victim to deny abuse, perhaps they don’t want to cause trouble for their child or spouse, or they may worry that their situation could get worse. As a first responder, your view of the patient includes the home setting and could be a crucial key. Do a thorough assessment of the patient and don’t focus solely on the injury you’ve been called about. Note the patient’s overall condition, note past bruising or scarring, be sure to register the general condition of the house and the actions of caregivers or family that may be present. Address the patient when asking questions even if a relative offers the responses. If the family member monopolizes all the responses, it might be a good idea to ask them to retrieve an article of clothing for the patient, prescription bottles, or something that would get them to leave the room and give the victim time to talk freely. It says in the current NYS DOH BLS Treatment Protocols: Suspected Child/Spouse/Elder/ Abuse: Visually assess the immediate scene for evidence of possible abuse, recording all appropriate information on the Prehospital Care Report. Make a verbal report summarizing the above to the responsible medical personnel upon arrival to the emergency department. Report suspected child abuse or neglect according to NYS Bureau of EMS Policy Statement #02-01. As with the child abuse cases, the law grants both civil and criminal immunity to reporters in proceedings that might arise from the report, except for liability for perjury, unless the reporter acted in bad faith or with malice. (CGS § 17b-451). Be aware of procedures put in place by your agency regarding the reporting procedure. Or without set guidelines, to report adult abuse, call (within New York State only): 1-800-342-3009 (Press Option 6) or contact the local county Department of Social Services Adult Protective Services.

Rockland EMS Providers Receive Regional Honors for Service to Community New City, NY. On Wednesday, June 19, 2013, Rockland County Executive C. Scott Vanderhoef presented plaques from the Hudson Valley Regional EMS Council to members of the Rockland Emergency Medical Services community, along with a trophy to a Youth Corps that won Best Overall at a drill competition earlier this year. Rockland County EMS Coordinator Kim Lippes was on hand to help present the plaques. The following honors were handed out at today’s ceremony: Katherine Zoda of the Nanuet Community Volunteer Ambulance Corps as Youth Member of the Year; Piermont Fire Department 13-EM as EMS Agency of the Year; South Orangetown Volunteer Ambulance Corps for Excellence in EMS Quality and Safety; and Stony Point Ambulance Corps Youth Team #1 as Winner of the Overall EMS Youth

Competition. Ms. Zoda and South Orangetown Ambulance Corps will also be presented with NYS EMS Council awards this October. “We are very proud of the honorees and their continued dedication in making this a safer county for all who live here,” said Vanderhoef. “On behalf of all Rocklanders, we thank them for their tremendous efforts.” The county executive especially noted the dedication and promise of the county’s youth in EMS and called them “The Future”. Rockland County's highest award for bravery, the DeFlumere Award, was presented to the Pearl River Ambulance Corps, the South Orangetown Ambulance Corps, Rockland Paramedics and the Rockland County Technical Rescue Team. This annual award, named for Albert “Alfoxie” DeFlumere who perished in a house fire while trying to rescue his son, was

awarded this year for heroic efforts in rescuing a family during Hurricane Sandy. County Executive Vanderhoef spoke about the devastation suffered throughout the county during Hurricane Sandy and mentioned that in addition to the high water/waves and winds, approximately 95% of the county lost power during the storm. He remarked that each of the county’s emergency services agencies, many of them volunteer, worked endless hours to ensure the safety and well-being of the residents. Piermont Fire Department Chief William Cavanaugh said, “When you’re caught in the battle, you aren’t thinking about yourself, just about the people who need your help, and it’s a whole team effort.”

Car versus tractor trailer aftermath

Rediscover Amherst Car struck by tractor trailer in Charlton The Amherst Business Association had their Rediscover Amherst St. on Saturday June 15, 2013. Old Ladder Co. 12's house, which was closed for budget cuts, is now a private residence. It was the backdrop for many displays. Three old firehouses, two of which are on Amherst St., are all private residences now. Also a turnout gear competition was held for the children.

On June 13th at about 11:58 a.m., Saratoga County 911 Center received reports that a passenger car was truck by a tractor trailer on State Route 67 at the intersection of Jockey Street in the Town of Charlton. Motorists that day experienced heavy rains throughout the area causing limited visibility while driving. Saratoga County Sheriff's deputies concluded their investigation reporting that a Galway resident was traveling northbound on Jockey Street at the intersection with State Route 67, entered into the intersection and into the path of a westbound tractor trailer was pushed off the roadway and came to rest in a culvert area on the north

side of S.R. 67. The car suffered extensive damage. The tractor trailer also left the highway, striking and severing a utility pole prior to coming to the tractor coming to rest on its drivers side. The trailer, carrying magazines, remained upright. The driver of the tractor trailer was able to self extricate himself and walk to the highway where he was assisted by passing motorists, provided shelter, until Galway EMS arrived to render care. He was later transported to a Schenectady Hospital.

The Galway resident operating the passenger car succumbed to her injuries at the scene and was later extricated by Harmony Corners Fire Department. Harmony Corners Fire crews were required to place booms and absorbent pads down as diesel fuel leaked into the rain saturated ground. DEC's spill response was called to assist. Also assisting HCFD with detouring traffic were West Charlton and Charlton Fire Departments and Eagle-Matt Lee Fire Co (Ballston Spa) relocated a Rescue Truck to Harmony's Station. Instructor “JJ” Johnson of Fully Involved Training points to safety features of Electric/Hybrid Vehicle.

Rombout hosts new vehicle technology training Fishkill, NY. Rombout Fire Company recently held training with local area departments in new vehicle technology. For the past decade, the automotive industry has developed electric hybrid/hybrid electric vehicles to increase fuel mileage, but this advanced technology presents different challenges to fire personal when compared to conventional vehicles. Some of the information covered was the various automotive manufactures technologies and myths and the hidden dangers involving hybrid electric/electric hybrid vehicles.

After the classroom session, firefighters viewed cars to see where the cutpoints are located, emergency shut offs, and other safety features for fire personnel. Members from Glenham, New Hamburg, New Hackensack and Rombout attended the two classes, which were offered for the first time in Dutchess County. With more hybrid cars being sold and it is estimated that approximately 75,000 vehicles daily use the I-84

and Route 9 corridors and fire personal in the Southern Dutchess area we will see an increase of this technology at accident scenes. Rombout members received immediate benefit from this course. When preparing for the second day class, Rombout Fire Company was dispatched to a two car motor vehicle accident involving an electric/hybrid car. With instructor Johnson at the accident scene, he was able to review and offer advice to his previous day’s students who passed with flying colors. - KEVIN BARRY

City of Newburgh firefighters were quickly on the scene stretching one line to the second floor rear to extinguish the fire. At the same time, numerous mutual aid companies were alerted to either respond to the scene, cover at the public safety building in the city or stand by at mutual aid stations. Firefighters found a heavy smoke condition upon entering the building and the fire was located in a second floor classroom. The fire was quickly brought under control and overhaul was completed. Horizontal ventilation was then initiated. On the scene were the City of Newburgh, Castle Point, Winona Lake, Coldenham, West Point and Good-Will Fire Departments as well as Mobile Life Support Services. Numerous other fire departments were standing by. Students were evacuated across from and next to the school and the district quickly began issuing an early dismissal policy.
