1. Is it possible to see my relative or friend at the hospital?

Yes, you are able to see and spend time with them on the ward or unit, directly after their death.

At the University Hospital of Wales and University Hosptial Llandough there is a room next to the mortuary where you may be able to view your relative or friend. Opening hours vary depending on the hospital site. It is possible to make an appointment for a suitable time for you both, but sometimes these arrangements cannot be guaranteed.

Unfortunately, there are no mortuary facilities at St Davids Hospital and Barry Hospital, but please be aware that you will be able to request to see your relative or friend at the funeral home.

2. Is tissue donation a possibility?

Cardiff and Vale University Health Board (UHB) fully support organ and tissue donation. Tissue donation may be a possibility after your relative or friend has died. Such a donation is a great gift and can change the lives of recipients considerably. Most people are able to donate their corneas (part of the eye). This can take place up to 24 hours after the death and can restore the sight of two people through a corneal transplant. The removal of donated tissue is carried out with great care and respect. It does not prevent you saying goodbye or affect funeral arrangements.

If you would like further information, please contact NHS Blood and Transplant National Referral Centre on 0800 432 0559. This is a 24 hour pager service. Leave your name and full telephone number and a Specialist Nurse in Tissue Donation will return your call. Alternatively you may ask a doctor, nurse or the Bereavement Office involved in the care of your relative to contact the appropriate person on your behalf.

3. What facilities are available for Multi Faith deaths?

Facilities available at;

1. The University Hospital of Wales; a multi-faith prayer room is situated on the corridor between B5 and C5. There is also a multi-faith prayer room in Noah’s Ark Children’s Hospital.

2. St David’s Hospital has a chapel that is used by all faiths.

3. University Hospital Llandough, a multi prayer room situated within the chaplaincy complex.

• What happens if there are cultural requirements over a weekend or bank holiday period?

If your beliefs require your relative or friend’s body to be buried during a weekend or bank holiday period, there are guidelines in place for staff to try and help to arrange this.

If the death needs to be reported to Coroner, staff will not be able to complete the necessary paperwork and unfortunately your relative or friend will not be able to be buried until their death has been reported, and any investigations have taken place. The Coroner’s Office is closed on the weekend and bank holidays.

Burial on weekend and bank holidays is only possible if your relative died in Cardiff, not the Vale of Glamorgan, as the Vale of Glamorgan Register Office is closed during this time.

There are certain statutory requirements that need to be fulfilled before burial.

 A Medical Certificate of Cause of Death (MCCD) needs to be completed by a Doctor; this will require scrutiny of the medical records and consultation with the Medical Examiner’s Service.

 The death needs to be registered with the Registrar of Births, Deaths and Marriages before your relative or friend leaves the hospital.

 Please make sure that Cardiff Bereavement Service are contacted before 11.45am, so that the burial can take place the following day.

4. What happens if your relative or friend dies in the Accident and Emergency Department?

Generally, when a death has occurred on the way to, or in the Accident and Emergency Department, the death will be reported to the Coroner.

An officer in the department will need to take a statement and you will be asked to make a formal identification of the body. After this, you will be able to spend time with your relative or friend.

They will tell you if;

 the GP can issue a Medical Certificate of Cause of Death or whether

 the hospital can issue a Medical Certificate of Cause of Death or that

 a post-mortem examination or inquest will be held.

If the GP is to issue a Medical Certificate of Cause of Death, contact them to find out when you can collect the certificate.

If a doctor is not allowed to issue the certificate, the Coroner’s Officer will tell you what to do next.

5. What if the death is reported to the Coroner?

Sometimes it is necessary to report a death to the Coroner before the Medical Certificate of Cause of Death can be given. Some of the circumstances include where:

 the cause of death is unknown;

 the cause of death is unnatural e.g. possible suicide, homicide, neglect, accident (including road traffic collision) or poisoning;

 death occurred within 24 hours of admission to hospital, during or after surgery or a medical procedure;

 death occurred during or immediately after detention in police custody.

The Coroner will decide either that:

 a post-mortem examination does not need to take place, in which case the responsibility of completion of the Medical Certificate of Cause of Death will rest with the doctor and Medical Examiner’s Office. Once completed this will then be sent via the Medical Examiner’s Office to the Registrars; or

 a post-mortem examination will need to take place, in which case they will instruct a Pathologist (a specialist doctor) to perform this.

After a post-mortem examination has been carried out, the Coroner will decide either that:

 no further action is necessary, in which case they will notify the registrar of this, using a form B: or

 that they need to hold an inquest; in which case, the Coroner’s Officer will tell you of what happens next.

A Coroner’s post-mortem is a legal obligation and not subject to the permission of the deceased’s family. If there is time to organise it in advance, relatives may be represented at the examination by their own doctor.

6. Is it possible to have a hospital post-mortem examination?

Although the Coroner may not require a post-mortem examination, the doctor may wish to carry out a hospital post- mortem examination, as this may help in the treatment of other patients in the future. A post-mortem can also give more detailed information about the cause of death. If the next of kin is sure that the deceased did not have any objections to the examination when they were alive; they can sign the consent form. It is not always necessary to examine all of the body and a limited post-mortem can be completed, however this could restrict the findings.

You do not have to agree to a hospital post-mortem examination and it is up to you if this is to take place or not.

The post-mortem examination is carried out sympathetically and should generally not delay funeral arrangements. Please tell the Funeral Director that this examination is taking place. The report is then sent to the Consultant who looked after your relative or friend; it can take many months for this report to be completed.

7. What if my relative or friend wishes to donate their body to medical research?

If they wished to donate their body to medical research you should contact the Department of Anatomy at Cardiff University as soon as possible on Tel: (029) 2087 4370 and they will tell you what you need to do next.

It is essential that they have signed the necessary paperwork during life, explaining that this is their wish and that their signature has been witnessed.

You can contact a Funeral Director to make provisional plans before you register the death.

The Following Days

8. What is the Medical Examiners Service?

This service provides an independent scrutiny of a person’s death and is a legal requirement, unless the Coroner is investigating the circumstances of death. On some occasions both the Coroner and Medical Examiner will require involvement.

The service allows the cause of death to be more accurately identified, and the circumstances surrounding the death to be more objectively assessed in order to identify any queries about the treatment or care provided that may require further investigation.

When referred to the Medical Examiners Service, bereaved families will have the opportunity to speak to someone who wasn’t involved in the care of their loved one but who is able to listen to their view of the care provided. They can provide more detail regarding the cause of death and the circumstances that led up to it.

9. How do I obtain the ‘Medical Certificate of Cause of Death’?

Providing there is no Coroner’s investigation, a ‘Medical Certificate of Cause of Death’ will be issued by the hospital. This certificate is required to register the death.

You will need to speak with the hospital where your relative or friend died for information regarding the certificate and what happens next. This is to make sure the certificate is ready. You do not need to attend the bereavement office to collect the Medical Certificate of Cause of Death; this will be sent electronically to the Register Office.

Your relative or friend’s property will be kept on the ward or unit. Cash and valuable items may be kept elsewhere, such as the Cashiers Office. The Bereavement Office will be able to advise you.

Please do not attend the hospital, the Bereavement Office staff can offer further advice and support regarding registering a death:

It is possible to register your relative or friend’s death with a different Register Office, if you have any questions, you can contact the Register Office to discuss this with them.

We will endeavour to complete the necessary paperwork to enable you to register the death of your loved one within the legal time frame, however this may not always be possible as individual circumstances will vary. Please be assured that the Registrars will extend the registration period if required.

Who can register the death?

The death should be registered by a relative of the deceased. If there are no relatives then it is possible for other people to register the death, these include;

1. Someone who was present at the death.

2. A representative of the establishment/hospital in which the death occurred.

3. The person instructing the Funeral Director.

What Information is required by the Registrar?

 Date and place of death.

 Full name of the deceased and any previous names they have been legally known by, including maiden name if appropriate.

 Date and place of birth of the deceased.

 Deceased’s occupation and the full names and occupation of their spouse/civil partner.

 Deceased’s usual address.

 If the deceased was married or in a civil partnership, the date of birth of the spouse/civil partner.

Tell Us Once

At the point of registration the Register Office also offers the “Tell Us Once” service; this allows them to notify various government departments of the death on your behalf. If you would like to utilise this during your appointment we kindly ask you to bring the following where appropriate;

 National Insurance number for the deceased and any surviving spouse/civil partner.

 Deceased’s British Passport (valid or expired).

 Deceased’s Driving Licence.

 Deceased’s Blue Badge.

What happens next?

The Registrar will issue a certificate (known as the green form). This will be required by the Funeral Director to give them authority to continue with arrangements for the funeral.

There is a fee for copies of the death certificate; the Registrar will advise how many copies you may need. These will be required for banks, building societies, insurance companies, solicitors or for pension claims etc.

11. What if my relative or friend is to be transferred outside England or Wales?

If they are to be taken outside England or Wales someone must ‘apply to the Coroner for an ‘Out of England/Wales’ form. This can be arranged through your Embassy or your Funeral Director. You will also require a ‘free from infection’ certificate that can be issued by the hospital.

12. How do I arrange the funeral?

It is important that if your relative or friend has made a Will it is read as soon as possible as it may contain information regarding their wishes.

It is a good idea to contact a Funeral Director of your choice as soon as possible, details can be found via our Bereavement Office or on the internet. They will give you an estimate cost; it is possible to discuss ways of reducing this cost with them.

If you receive benefits and financial support from the government, you may be entitled to help with costs and funeral expenses. More information can be found on the government website or via their Bereavement Service Helpline:

If the person who has died lived in the Cardiff area, they might be entitled to a ‘Cardiff Funeral’; the staff at the Bereavement Office can advise which Funeral Director you would need to contact to discuss this. Tel: (029) 2184 2789.

You do not have to have a funeral ceremony, use a religious minister or funeral director.

13. What about the Estate and the Will?

If you are an executor of a Will, you are responsible for making sure that what is stated in the Will is carried out.

You may need to apply for probate. Probate is a certificate granted by a court that a Will is valid and the executors may administer the estate.

The Citizens Advice Bureau or a solicitor will tell you what needs to be done. The Probate Registry of Wales can be contacted on Tel: (029) 2047 4373.

14. What if they die without a Will?

If a person dies without leaving a valid Will it is known as dying intestate. It is necessary to obtain a Grant of Letters of Administration from the Probate Registry in order to administer the estate. The persons entitled to apply for the Grant and the persons entitled to benefit from the estate are the nearest relatives in a fixed order. The Citizens Advice Bureau or a solicitor will advise you on what needs to be done. The Probate Registry of Wales can be contacted on Tel: (029) 2047 4373.

Grief is unique to everyone. There is no right or wrong way to feel. Although there are recognised stages of grief, you will likely not progress through these in a clear and straightforward manner. Grief is complicated and unpredictable, and the bereavement service are here to support you.

Though family and friends can be of great support at this time, sometimes talking with someone outside of this group can provide much comfort.

18. Reactions to grief – Adult / Child

Adult’s Grief

There are certain common reactions to grief, as mentioned previously these do not always follow a particular pattern and can resurface many months or even years after the death.

Shock/numbness – This is usually one of the first reactions to grief and can make people feel confused and unable to think clearly. There

may be an inability to believe that the person has died or difficulty in remembering what they looked or sounded like. The confusion and feeling of fogginess in the mind can continue for some time, but it will pass.

Overwhelming sadness – This can be accompanied by many tears or an inability to cry at all. Grief can change your perception of life and many things may seem pointless, planning for the future may seem fruitless.

Tiredness, exhaustion and other physical symptoms – The body can feel overwhelmed just like the mind, muscles can feel tense and ache, and there may even be symptoms such as nausea, stomach pains and heart palpitations.

Anger – There maybe anger at the person who died or perhaps anger at those who let it happen. It is common to become upset at relatively minor things, perhaps things that you wouldn’t have been upset at before.

Guilt – Feeling guilty that we didn’t do more, or that a different decision/ action may have changed the outcome. There may also be guilt at still being alive, and feeling that you wish others had died instead may also instil feelings of guilt.

Fear – The fear of dying or thoughts of something bad happening to family and friends may increase. New fears may also develop such as leaving the house or seeing people you know when you are out.

Repeated visions of the death and surrounding circumstances –Although these can be distressing and upsetting, they can help the mind to process all that has happened. These visions may happen when awake or asleep and are not uncommon when grieving.

Grief can place enormous pressure on the bereaved and the wider circle of family and friends, sometimes it can feel as though people do not understand the depth of grief being experienced. Try to maintain relationships with loved-ones and explain the range of emotions being experienced, no two people will have the same experience of grief.

Please reach out for support if it is felt that these reactions to grief are overwhelming and continuing for an extended period of time.

Be kind to yourself. Allow yourself to do things that make you smile, we can be both happy and sad at the same time.

Children’s grief

Although a child may exhibit many of these same reactions and emotions, the way adults approach a child’s grief is important too.

It is our instinct to protect children from being hurt and upset, however it is important to talk to children about the person that has died and allow feelings to be shared honestly and openly. Show children that it is normal to feel sad when someone has died, give plenty of reassurance. Using clear language is important, saying ‘dead’ or ‘died’ is much more helpful for children than ‘passed away’ or ‘gone to sleep’. Children should feel able to ask questions about the death and often they will guide the adult with how much they want to know. This can be painful as children can be quite direct in their questioning.

Depending on their age and personalities, children will grieve differently and they can move very quickly from one emotion to another. If possible a child should be given a choice on whether they wish to attend the funeral or not. The service should be explained in an age-appropriate way and things they will see/hear explained. It can be beneficial to involve them in some way such as choosing a photo, song or poem. There are of course other ways that the person can be remembered, such as making a memory box or jar together, creating a collage or drawing pictures.

Children do not grieve in the same way as adults and as they grow and realise the enormity of what has happened, they may need a lot of support.

Gallwch weld a threulio amser gyda’ch plentyn ar y ward neu’r uned, yn syth ar ôl iddynt farw.

Yn Ysbyty Athrofaol Cymru ac Ysbyty Athrofaol Llandochau mae ystafell wrth ymyl y marwdy lle gallech chi weld eich perthynas neu ffrind. Mae oriau agor yn amrywio yn dibynnu ar safle’r ysbyty. Mae’n bosibl gwneud apwyntiad ar gyfer amser addas i’r ddau ohonoch, ond weithiau ni ellir gwarantu’r trefniadau hyn.

Yn anffodus, nid oes unrhyw gyfleusterau marwdy yn Ysbyty Dewi Sant ac Ysbyty’r Barri, ond byddwch yn ymwybodol y byddwch yn gallu gwneud cais i weld eich perthynas neu ffrind yn y cartref angladd.

2. A yw’n bosibl rhoi meinwe?

Ar ôl i’ch perthynas neu ffrind farw efallai y bydd yn bosibl iddynt roi organau neu feinwe os mai dyna oedd eu dymuniad. Mae rhodd o’r fath yn werthfawr tu hwnt. Gall y rhan fwyaf o bobl roi cornbilennau (rhan o’r llygad) a meinweoedd (falfiau’r galon) ar ôl iddynt farw. Gellir rhoi cornbilennau hyd at 24 awr ar ôl marw, a falfiau’r galon hyd at 48 awr ar ôl marw. Gall un rhoddwr adfer golwg dau berson drwy drawsblaniad cornbilennau, a gall rhoi falfiau’r galon achub bywydau llawer mwy. Caiff meinweoedd a roddir eu tynnu â gofal a pharch. Nid yw’n eich atal rhag ffarwelio nac yn effeithio ar drefniadau’r angladd.

Os hoffech ragor o wybodaeth, cysylltwch â Chanolfan Atgyfeirio Genedlaethol Gwaed a Thrawsblaniad y GIG ar 0800 432 0559. Mae hwn yn wasanaeth galw 24 awr. Gadewch eich enw a rhif ffôn llawn a bydd Nyrs Arbenigol mewn rhoi meinwe yn galw chi yn ôl. Fel arall, gallwch ofyn i feddyg, nyrs neu’r Swyddfa Profedigaeth sy’n ymwneud â gofalu am eich perthynas i gysylltu â’r unigolyn priodol ar eich rhan.

3. Pa gyfleusterau sydd ar gael ar gyfer marwolaethau amlffydd?

 Mae cyfleusterau ar gael yn;

 Ysbyty Athrofaol Cymru; mae ystafell weddïo aml-ffydd wedi’i lleoli ar y coridor rhwng B5 a C5. Mae ystafell weddïo amlffydd yn Ysbyty Arch Noa i Blant Cymru hefyd.

 Mae gan Ysbyty Dewi Sant gapel a ddefnyddir gan bob ffydd.

 Ysbyty Athrofaol Llandochau; ystafell weddi aml-ffydd yn yr adeilad caplaniaeth.

 Beth sy’n digwydd os oes gofynion diwylliannol dros y penwythnos neu ŵyl y banc?

Os yw eich credoau yn ei gwneud yn ofynnol i gorff eich perthynas neu ffrind gael ei gladdu yn ystod penwythnos neu gyfnod gŵyl y banc mae canllawiau yn eu lle i staff geisio helpu i drefnu hyn.

Os bydd yn rhaid rhoi gwybod i’r Crwner am y farwolaeth, ni fydd staff yn gallu cwblhau’r gwaith papur angenrheidiol ac yn anffodus ni fydd eich perthynas neu ffrind yn gallu cael eu claddu hyd nes y bydd eu marwolaeth wedi’i adrodd, a bod unrhyw ymchwiliadau wedi’u cynnal. Mae Swyddfa’r Crwner ar gau dros y penwythnos a gwyliau banc.

Dim ond os bu farw eich perthynas yng Nghaerdydd ac nid ym Mro Morgannwg y gellir claddu ar benwythnosau a gwyliau banc gan fod Swyddfa Gofrestru Bro Morgannwg ar gau yn ystod y cyfnod hwn.

Mae rhai gofynion statudol y mae angen eu cyflawni cyn claddu.

 Mae angen i Dystysgrif Feddygol o Achos Marwolaeth (MCCD) gael ei chwblhau gan Feddyg; bydd hyn yn gofyn am graffu ar y cofnodion meddygol ac ymgynghori â Gwasanaeth yr Archwilwyr Meddygol

 Mae angen cofrestru’r farwolaeth gyda’r Cofrestrydd Genedigaethau, Marwolaethau a Phriodasau cyn i’ch perthynas neu ffrind adael yr ysbyty. -Gwnewch yn siŵr eich bod yn cysylltu â Gwasanaeth Profedigaeth Caerdydd cyn 11.45am, fel y gellir cynnal y gladdedigaeth y diwrnod canlynol.

4. Beth sy’n digwydd os bydd eich perthynas neu’ch ffrind yn marw yn yr adran Damweiniau ac Achosion Brys?

Yn gyffredinol, pan fydd rhywun wedi marw ar y ffordd i’r Adran Damweiniau ac Achosion Brys, neu yn yr Adran ei hun, caiff y farwolaeth ei hysbysu i’r Crwner.

Bydd angen i swyddog yn yr adran gymryd datganiad a gofynnir i chi adnabod y corff yn ffurfiol. Yn dilyn hyn, byddwch yn gallu treulio amser gyda’ch perthynas neu ffrind.

Bydd yn rhoi gwybod i chi;

 a gaiff archwiliad post-mortem neu gwest ei gynnal.

Os bydd y meddyg teulu yn cyflwyno tystysgrif feddygol o achos marwolaeth, cysylltwch â nhw i holi pryd y gallwch nôl y dystysgrif.

Os na chaiff meddyg gyflwyno tystysgrif, bydd swyddog y crwner yn rhoi gwybod i chi beth i’w wneud nesaf.

5. Beth sy’n digwydd os caiff y Crwner ei hysbysu am y farwolaeth?

Weithiau mae angen hysbysu’r Crwner o farwolaeth cyn y gellir cyflwyno’r dystysgrif feddygol o achos marwolaeth. Ymhlith rhai o’r amgylchiadau hyn mae:

 achosion lle nad yw achos y farwolaeth yn hysbys;

 achosion lle nad yw achos y farwolaeth yn naturiol

 e.e. hunanladdiad posibl, llofruddiaeth, esgeulustod, damwain (gan gynnwys damwain cerbyd) neu wenwyno;

 achosion o farwolaeth o fewn 24 awr i’r person gael ei dderbyn i’r ysbyty, yn ystod llawdriniaeth neu driniaeth feddygol neu’n fuan ar ôl hynny;

 marwolaeth yn y ddalfa neu’n fuan ar ôl bod yno.

Bydd y Crwner yn penderfynu naill ai:

 nid oes angen cynnal archwiliad post-mortem, ac os felly, y meddyg a Swyddfa’r Archwiliwr Meddygol fydd yn gyfrifol am gwblhau’r Dystysgrif Feddygol o Achos y Farwolaeth. Unwaith y bydd wedi’i gwblhau caiff ei anfon drwy Swyddfa’r Archwiliwr Meddygol at y Cofrestryddion; neu

 bod angen cynnal archwiliad post-mortem, ac os felly bydd yn cyfarwyddo Patholegydd (meddyg sydd wedi cael hyfforddiant helaeth) i wneud hyn

Ar ôl i archwiliad post-mortem gael ei gynnal, bydd y Crwner yn penderfynu naill ai:

 nad oes angen cymryd camau pellach. Os felly bydd yn hysbysu’r cofrestrydd o hyn, gan ddefnyddio ffurflen B:

 bod angen iddynt gynnal cwest. Os felly, bydd swyddfa’r crwner yn rhoi gwybod i chi beth sy’n digwydd nesaf.

Mae post-mortem crwner yn rhwymedigaeth gyfreithiol ac nid oes angen caniatâd teulu’r person a fu farw. Os oes amser i drefnu ymlaen llaw, gall perthnasau gael eu cynrychioli yn yr archwiliad gan eu meddyg eu hunain.

Dywedwch wrth Swyddfa’r Crwner os hoffech i’ch meddyg fynychu.

6. A yw’n bosibl cael archwiliad post-mortem yn yr ysbyty?

Er ei fod yn bosibl na fydd y Crwner yn gofyn am archwiliad postmortem, efallai y bydd y meddyg am gynnal archwiliad post- mortem yn yr ysbyty, oherwydd y gall hyn helpu i drin cleifion eraill yn y dyfodol. Gall post-mortem hefyd roi gwybodaeth fanylach am achos y farwolaeth. Os yw’r perthynas agosaf yn siŵr nad oedd yr ymadawedig yn gwrthwynebu’r archwiliad pan oedd yn fyw, gallant lofnodi’r ffurflen gydsynio. Nid oes angen archwilio’r corff cyfan bob amser, a gellir cynnal post-mortem cyfyngedig, fodd bynnag gallai hyn gyfyngu ar y canlyniadau.

Nid oes yn rhaid i chi gytuno i archwiliad post-mortem yn yr ysbyty a’ch penderfyniad chi yw p’un a ddylid ei gynnal ai peidio.

Cynhelir archwiliad post-mortem mewn ffordd sympathetig ac fel arfer ni ddylai achosi oedi i drefniadau’r angladd. Rhowch wybod i’r Trefnydd Angladdau fod yr archwiliad hwn yn mynd rhagddo. Yna caiff yr adroddiad ei anfon at yr Ymgynghorydd a oedd yn gofalu am eich perthynas neu ffrind; gall gymryd misoedd lawer i’r adroddiad hwn gael ei gwblhau.

7. Beth os yw fy mherthynas neu ffrind yn dymuno gadael ei gorff i ymchwil feddygol?

Os oeddent am adael eu corff i ymchwil feddygol dylech gysylltu â’r Adran Anatomi ym Mhrifysgol Caerdydd cyn gynted â phosibl ar (029) 2087 4370 a byddant yn rhoi gwybod i chi beth sydd angen i chi ei wneud nesaf.

Mae’n hanfodol eu bod wedi llofnodi’r gwaith papur angenrheidiol pan oeddent yn fyw, gan egluro mai dyma eu dymuniad, a’u bod wedi ei lofnodi gyda thyst.

Gallwch gysylltu â threfnydd angladdau i wneud cynlluniau amodol cyn i chi gofrestru’r farwolaeth.

8. Beth yw’r Gwasanaeth Archwilwyr Meddygol?

Mae’r gwasanaeth hwn yn darparu archwiliad annibynnol o farwolaeth person ac mae’n ofyniad cyfreithiol, oni bai bod y Crwner yn ymchwilio i amgylchiadau marwolaeth. Ar rai adegau bydd angen cyfranogiad y Crwner a’r Archwiliwr Meddygol.

Mae’r gwasanaeth yn ei gwneud yn bosibl nodi achos y farwolaeth yn fwy cywir, a darparu asesiad mwy gwrthrychol o’r amgylchiadau sy’n ymwneud â’r farwolaeth, er mwyn nodi unrhyw ymholiadau ynghylch y driniaeth neu’r gofal a ddarperir y gallai fod angen ymchwilio iddynt ymhellach.

Os cânt eu hatgyfeirio at y Gwasanaeth Archwilwyr Meddygol, bydd teuluoedd mewn profedigaeth yn cael y cyfle i siarad â rhywun nad oedd yn ymwneud â gofalu am eu hanwylyd, ond sy’n gallu gwrando ar eu barn hwy ynghylch y gofal a ddarparwyd. Gallant roi mwy o fanylion ynghylch achos y farwolaeth a’r amgylchiadau a arweiniodd ati.

9. Sut ydw i’n cael ‘Tystysgrif Feddygol o Achos Marwolaeth’ (MCCD)?

Ar yr amod nad oedd ymchwiliad gan Grwner, caiff ‘Tystysgrif Feddygol o Achos Marwolaeth’ ei rhoi gan yr ysbyty. Mae angen y dystysgrif hon er mwyn cofrestru’r farwolaeth.

Bydd angen i chi siarad â’r ysbyty lle bu farw eich perthynas neu ffrind i gael gwybodaeth ynghylch y dystysgrif a beth sy’n digwydd nesaf. Mae hyn er mwyn sicrhau bod y dystysgrif yn barod. Nid oes angen i chi fynd i’r swyddfa brofedigaeth i gasglu’r dystysgrif feddygol o achos y farwolaeth; Bydd hon yn cael ei hanfon yn electronig i’r Swyddfa Gofrestru.

Beth sy’n digwydd nesaf?

Bydd y Cofrestrydd yn rhoi tystysgrif (a elwir yn ffurflen werdd). Bydd hon yn ofynnol gan y Trefnydd Angladdau i roi awdurdod iddo barhau â’r trefniadau ar gyfer yr angladd.

Codir ffi am gopïau o’r dystysgrif marwolaeth; bydd y Cofrestrydd yn rhoi gwybod i chi faint o gopïau y gallai fod eu hangen arnoch. Bydd angen y rhain ar fanciau, cymdeithasau tai, cwmnïau yswiriant, cyfreithwyr neu ar gyfer hawlio pensiynau ac ati.

11. Beth sy’n digwydd os bydd fy mherthynas neu ffrind yn cael ei drosglwyddo y tu allan i Gymru neu Loegr?

Os ydynt yn cael eu trosglwyddo y tu allan i Gymru neu Loegr rhaid i rywun wneud cais i’r Crwner am ffurflen ‘Tu allan i Gymru/ Lloegr’. Gellir trefnu hyn drwy eich Llysgenhadaeth neu’ch Trefnydd Angladdau. Bydd angen tystysgrif ‘rhydd rhag haint’ arnoch hefyd, y gall yr ysbyty ei chyflwyno.

12. Sut ydw i’n trefnu’r angladd?

Os yw eich perthynas neu ffrind wedi ysgrifennu Ewyllys, mae’n bwysig ei fod yn cael ei ddarllen cyn gynted â phosibl oherwydd gall gynnwys gwybodaeth am eu dymuniadau.

Mae’n syniad da cysylltu â Chyfarwyddwr Angladdau o’ch dewis cyn gynted â phosibl, gellir dod o hyd i fanylion ar y rhyngrwyd neu drwy’r

13. Beth am yr ystâd a’r Ewyllys?

Os ydych yn ysgutor ewyllys, chi sy’n gyfrifol am sicrhau bod yr hyn a nodir yn yr ewyllys yn cael ei gyflawni.

Efallai y bydd angen i chi wneud cais am brofiant. Tystysgrif a gyflwynir gan lys yw profiant sy’n datgan bod Ewyllys yn ddilys ac y gall yr ysgutorion weinyddu’r ystâd.

Bydd y Ganolfan Cyngor ar Bopeth neu gyfreithiwr yn rhoi gwybod i chi beth sydd angen ei wneud. Gellir cysylltu â Chofrestrfa Brofiant Cymru drwy ffonio (029) 2047 4373.

14. Beth os byddant yn marw heb Ewyllys?

Os bydd rhywun yn marw heb adael Ewyllys dilys, caiff ei alw’n etifeddiant diewyllys. Mae angen cael Grant Llythyrau Gweinyddu gan y Gofrestrfa Brofiant er mwyn gweinyddu’r ystâd. Y bobl sydd â’r hawl i wneud cais am y Grant, a’r bobl sydd â hawl i gael budd o’r ystâd, yw’r perthnasau agosaf yn ôl pa mor agos yr oeddent at yr ymadawedig. 18. Ymateb i alar – Oedolyn / Plentyn

Galar yr Oedolyn

Mae rhai ffyrdd cyffredin iawn o ymateb i alar, fel y soniwyd eisoes, ond nid yw’r rhain bob amser yn dilyn patrwm penodol a gallant godi eto fisoedd neu flynyddoedd lawer ar ôl y farwolaeth.

Sioc/diffyg teimlad – Fel arfer dyma un o’r ymatebion cyntaf o safbwynt galar, a gall wneud i bobl deimlo’n ddryslyd ac yn methu meddwl yn glir. Efallai na fyddwch yn gallu credu bod y person wedi marw, neu gallech ei chael hi’n anodd cofio sut yr oedd yn edrych neu’n swnio. Gall y dryswch a’r teimlad o niwl yn y meddwl barhau am amser hir, ond bydd yn dod i ben.

Tristwch llethol – Gall llawer o ddagrau neu anallu i grio o gwbl gydfynd â hyn. Gall galar newid eich ffordd o edrych ar fywyd a gall llawer o bethau ymddangos yn ddibwrpas, gall cynllunio ar gyfer y dyfodol ymddangos yn ofer.

Blinder a symptomau corfforol eraill – Gall y corff deimlo ei fod wedi’i lethu yn union fel y meddwl, gall cyhyrau deimlo’n dynn ac yn boenus, a gall fod symptomau fel cyfog, poenau yn y stumog a chrychguriadau’r galon hyd yn oed.

Dicter – Efallai y byddwch yn teimlo dicter at y person a fu farw, neu efallai ddicter at y rhai wnaeth adael iddo ddigwydd. Mae’n gyffredin deimlo’n drist ynghylch pethau cymharol fach, efallai pethau na fyddech wedi teimlo’n drist yn eu cylch o’r blaen.

Euogrwydd – Teimlo’n euog na wnaethom fwy, neu y gallai penderfyniad/gweithred wahanol fod wedi newid y canlyniad. Efallai y bydd euogrwydd hefyd o fod yn fyw o hyd, a gall teimlo eich bod yn dymuno y byddai eraill wedi marw yn lle hefyd arwain at deimladau o euogrwydd.

Ofn – Gall yr ofn o farw neu feddyliau am rywbeth drwg yn digwydd i deulu a ffrindiau gynyddu. Gall ofnau newydd hefyd ddatblygu, fel gadael y tŷ neu weld pobl rydych chi’n eu hadnabod pan fyddwch chi allan.

Gweld y farwolaeth a’r amgylchiadau cysylltiedig yn eich meddwl dro ar ôl tro. Er y gall hyn fod yn ofidus, gall helpu’r meddwl i brosesu popeth sydd wedi digwydd. Gallech feddwl amdano pan fyddwch yn effro neu’n cysgu ac nid yw’n anghyffredin wrth alaru.

Gall galar roi pwysau enfawr ar y rhai mewn profedigaeth a’r cylch ehangach o deulu a ffrindiau, weithiau gall deimlo fel pe na bai pobl yn deall dyfnder y galar sy’n cael ei brofi. Ceisiwch gynnal perthynas ag anwyliaid ac egluro’r ystod o emosiynau rydych yn eu profi, ni fydd gan unrhyw ddau berson yr un profiad o alar.

Gofynnwch am gymorth os nad ydych yn gallu ymdopi â’ch ymateb i alar a’i fod yn parhau am gyfnod estynedig.

Byddwch yn garedig wrthoch chi eich hun. Ceisiwch wneud pethau sy’n gwneud i chi wenu, gallwn fod yn hapus ac yn drist ar yr un pryd.

Galar plant

Er y gall plentyn arddangos llawer o’r un ymatebion ac emosiynau, mae’r ffordd y mae oedolion yn delio â galar plentyn yn bwysig hefyd.

Ein greddf yw amddiffyn plant rhag cael eu brifo a theimlo’n drist, ond mae’n bwysig siarad â phlant am y person sydd wedi marw a chaniatáu i deimladau gael eu rhannu’n onest ac yn agored. Dangoswch i’r plant ei bod hi’n gyffredin iawn i deimlo’n drist pan fydd rhywun wedi marw, rhowch ddigon o sicrwydd. Mae defnyddio iaith glir yn bwysig, mae dweud ‘wedi marw’ yn llawer mwy defnyddiol i blant na dweud bod y person wedi ‘mynd i gysgu’. Dylai plant deimlo eu bod yn gallu gofyn cwestiynau am y farwolaeth, ac yn aml byddant yn rhoi gwybod i’r oedolyn faint y maent am ei wybod. Gall hyn fod yn boenus gan y gall plant fod yn eithaf uniongyrchol wrth holi.

Yn dibynnu ar eu hoedran a’u personoliaethau, bydd plant yn galaru’n wahanol a gallant symud yn gyflym iawn o un emosiwn i’r llall. Os yw’n bosibl, dylid rhoi dewis i blentyn a yw’n dymuno mynychu’r angladd ai peidio. Dylid esbonio’r gwasanaeth mewn ffordd sy’n briodol i’w hoedran a dylid esbonio’r pethau y byddant yn eu gweld/clywed. Gall fod yn fuddiol eu cynnwys mewn rhyw ffordd fel dewis llun, cân neu gerdd. Wrth gwrs, mae yna ffyrdd eraill y gellir cofio’r person, fel gwneud blwch neu jar atgofion gyda’ch gilydd, creu collage neu dynnu lluniau.

Nid yw plant yn galaru yn yr un ffordd ag oedolion ac wrth iddynt dyfu a sylweddoli difrifoldeb yr hyn sydd wedi digwydd, efallai y bydd angen llawer o gymorth arnynt.
