RoyalCollege of Emergency Medicine/ Irish Committee for Emergency Medicine Training Core Ultrasound Criteria Prior to Sign-off In order to be eligible to be accredited through a Finishing School candidates must have completed all theoretical AND hands on training as evidenced by the appropriate logbook requirements PRIOR to attendance at any Finishing School. Specifically they must complete in ADVANCE and provide appropriate evidence of: 1. Completion of all 6 EnlightenMe sessions (Indications, Physics, Image acquisition,FAST, Aortic assessment and Vascular access) 2. Completion of a Core (Level 1) CEM approved course, OR approved local modular training 3. A reflective Logbook of 10 pictorial cases, which must include at least 1 case involving each of all the 4 Core competencies (FAST/AAA/Vascular Access/ELS); these must also include some pathological findings in patients and NOT include training scans on normal volunteers. 4. A general logbook evidencing on-going experience and the use of ultrasound in appropriate clinical cases and settings. Whilst Core accreditation is aimed to be competency based and not numerically defined (i.e. solely based on numbers of scans undertaken) it is advised that the General logbook should include a MINIMUM of: 1. 10 FAST/FAFF scans 2. 10 AAA scans 3. 5 Vascular accesses using USS of which AT LEAST 1 must be central access 4. 10 ELS scans of which AT LEAST 5 must be in full cardiac arrest There should be a MINIMUM of 50% of these performed with supervision from an accredited Core supervisor. There should be a MINIMUM of 2 cases in FAST/AAA and 1 case in ELS where there were positive findings.
Acceptance for accreditation through a Finishing School is only appropriate where ALL the above evidence is either; 1. Submitted in advance and has been reviewed and confirmed as acceptable by either the CEM approved Finishing School Course Director OR where such has been reviewed and confirmed as complete by an appropriately accredited Regional Lead for the Region where the candidate is based. If the candidate has this proof of theoretical training and logged experience signed as completed by their Regional Lead, where this is an appropriately accredited individual, then they can submit this evidence alone. The CEM approved Course Director should be appropriately accredited for Core level assessment and have liaised with the CEM Regional Lead or member of the CEM Ultrasound Subcommittee. OR 2. Full evidence if provided at the commencement of the Finishing School. Candidates will NOT be able to accredit through a Finishing School if ALL the requirements are not completed in advance. Completion/compilation of logbooks and required scanning experience CANNOT be undertaken during the Finishing School. It is essential that candidates wanting to trigger competency assessments have the appropriate experience in scanning ACTUAL patients; not just experience scanning in volunteers or simulators. It is also essential that they have experience in scanning in the presence of POSITIVE pathological findings. Candidates who register for Finishing School and subsequently fail to meet these requirements may at the discretion of the course director be included in the Finishing School for additional training but CANNOT be accredited at such time. By prior agreement from the course director, a candidate may be may be given a place on a Finishing School with the sole aim at receiving additional training and NOT assessment; this is likely to be beneficial for candidates from regions where there may be a paucity of training opportunities or supervision. However, the USSC would prefer that course directors prioritise places to trainees requiring assessments and sign-off. Â
