Over three years ago, Kaiser Permanente announced plans to expand into Northern Colorado. Now, our footprint in this vibrant community has grown and we are proud to serve more than 35,000 members living in Fort Collins, Loveland, Greeley and surrounding communities. This is rooted in our integrated care delivery system where patient care teams leverage a robust system of electronic medical records to deliver the right care at the right time. Kaiser Permanente members can also actively engage in managing their health through My Health Manager, our personal health record. My Health Manager allows users to access their health records securely online, view lab results, email their physicians, request prescription refills and make, change or cancel routine appointments. An integrated system is more efficient, cost-effective and leads to better health outcomes. For example, Kaiser Permanente members are less likely to suffer heart attacks and strokes because of our success in improving their hypertension (high blood pressure) control rates. Through a systemwide, five-step action plan, Kaiser Perma-

nente has increased Colorado member hypertension control rates from 61 percent to 82.6 percent (nationally, blood pressure control rates hover around 50 percent) since 2008. We understand that physician choice is important. That’s exactly what the Kaiser Permanente network offers in Northern Colorado. It is a rare opportunity for an individual or organization to create a generational change in a great community like Fort Collins. Yet that’s exactly what Banner Health did last year when it opened Banner Fort Collins Medical Center on Harmony Road. With more than 90 years of serving the people of northern Colorado, UCHealth is proud to be a part of this community. And with more than 5,000 employees in this area, not only are we your doctors, nurses, technicians and health educators, we also are your friends, neighbors and fellow Broncos fans. We’re committed to delivering the highest level of patient care while also focusing on building a healthier community. For several years, we’ve been expanding services to meet your needs and to continue to bring the most advanced treatments and state-of-the-art technologies close to home. That’s why I am thrilled to share with you some of the exciting projects we’ve been working on lately. We recently opened a new emergency room in southeast Fort Collins to provide convenient and accessible emergency care. The new facility features 12 private exam rooms and computed tomography, ultrasound and basic X-ray services. However, you don’t have to have a medical emergency to benefit from this new facility. It also includes an outpatient laboratory and retail pharmacy, both of which are open 24/7, the first in this area of the city. Last year, crews began work on a new wing at Poudre Valley Hospital in Fort Collins. The two-story, 113,000-square-foot

addition will house a new, expanded emergency department, an orthopedic unit and a new space for our laboratory services. Construction is expected to wrap up at the end of 2016, and we expect to be caring for patients in the new space in early 2017. Another project at PVH will have a dramatic impact for some of our youngest patients and their families. Crews are expanding the hospital’s Level III Neonatal Intensive Care Unit, which cares for newborns as young as 28 weeks rather than having them transported to Denver hospitals that offer the same specialized critical care. The first phase of the project, which includes 22 new beds and private rooms, will open this spring. The second phase, which will include an additional 10 beds, staff offices and parent amenities, is expected to open in October. We also are excited to bring a new hospital to Longmont. The 165,000-squarefoot Longs Peak Hospital will provide 48 inpatient beds and room to grow in the future, an intensive care unit, four operating rooms, emergency department, advanced cardiology services and an enhanced birth center. The hospital, which will open in early 2017, will serve the fast-

growing region that includes Longmont, Firestone and Erie. Finally, our long-standing relationships with Colorado State University and Columbine Health Systems continue to grow as we have formed a unique partnership to build a $59 million health and medical center on the CSU campus. Not only will the facility on the northwest corner of College Avenue and Prospect Road serve as a new gateway to campus, it also will house the university’s existing Health Network. There, UCHealth and Associates in Family Medicine will provide services for CSU staff and the public, including a walk-in clinic, imaging and occupational health services. The center, which opens in 2017, also features a new Columbine Health Systems Center for Healthy Aging, unlocking a new door of research and outreach program opportunities for the community. I am honored to be a part of the hospitals and health system that have been serving Fort Collins and northern Colorado since 1925, and all of us here at UCHealth look forward to providing you and your family with excellent care and service for many years to come. Since 2010, ACA has initiated and passed a Patient Bill of Rights, prioritized preventive care, and introduced a number of other reforms aimed at improving coverage and ultimately reducing costs to the American people. Despite these federal changes, navigating the system and understanding how it works will differ from state to state.

centers, and greater financial investment in public health programs. Medicare recipients have also benefited, with a reduction in out-of-pocket costs for prescription drugs, deductible- and co-pay-free preventive services, and targeted measures for reducing fraud and abuse.

Navigating Colorado’s Health Benefit Exchange Although the ACA has been around for several years now, navigating plans and enrollment can still be tricky, especially for those new to the system. Fortunately, Connect for Health Colorado (CHC), the state’s health care marketplace, opened in 2013. CHC, also known as the Colorado Health Benefit Exchange, is a non-profit established under state law. CHC provides an online “shopping” center, where consumers can peruse their health insurance options, purchase a plan and apply for federal assistance. The site is accessible to families,

individuals and small businesses (2-50 full-time employees) across the state. Using the exchange, consumers can find plans that cover minimum needs, such as prevention and wellness care, emergency services, mental health and prescription drugs. Those wanting to keep their current doctor can search for coverage by provider or facility. But CHC provides more than just an exchange for buying health care coverage. It provides a network of support services for consumers. This includes customer service centers, health coverage guides, and licensed agents and brokers who can help walk consumers through the system.

State Senator, Irene Aguilar told NPR that, while the ACA was a good start, it still isn’t enough. Last year, the organization ColoradoCareYES gathered over 100,000 signatures and in November 2016, voters will see single payer style health plan on the ballot: Everybody will pay in, and everybody will be covered. But, people will still be able to choose their primary care providers.

What would ColoradoCare look like? The program would guarantee coverage for all Coloradans and offer more comprehensive health benefit packages than ACA plans and equivalent dental care. But the real distinctions are in the continuity of coverage. ColoradoCare would ensure the same coverage even when life circumstances change. Factors such as age, health, marriage, employment status, or employer insurance plan would no longer disrupt or alter individuals’

access to their health care providers. And, according to ColoradoCareYES, providers will benefit from simplified billing, timely payment, and support for creativity and responsiveness. Benefits that advocates predict will be passed along to patients in the form of improved accessibility and consistent quality of care. People already using centralized care plans — such as seniors on Medicare or those in the military’s health system, Tricare — would keep their coverage. Those preferring to purchase private coverage would be able to do so, but this wouldn’t exempt them from paying into the statewide program.

How would we fund it? A series of tax increases would provide $25 billion in revenue that would support the program. Payroll taxes would increase by 7% for employers, employees would pay at least 3% of their gross pay, and self-employed people would contribute

the equivalent — 10% of their net income. There’s no doubt the tax hikes and the program’s projected cost, which is close to the budget for the whole state, may turn voters off. After all, Coloradans haven’t passed a tax increase other than those on tobacco or marijuana in over twenty years. Opponents worry about insufficient regulation of the governing board, a dearth of transparency, and the potential for ballooning costs over time. Others worry about the jobs that will be lost without health care insurance administration. Advocates, however, argue that the plan will save Colorado’s businesses and individual residents $4.5 billion in out-of-pocket costs. And while unemployment, or “job churn” will likely increase, the ColoradoCareYES economic analysis predicts this will be a temporary condition. By 2019, the organization predicts that the program’s economic stimulus and redistribution of spending from out-of-state to in-state would actually lead to 32,000 new jobs.

Varicose veins are an unfortunate reality for many adults. Approximately 25% of the adult population deals with some type of vein issue in the legs. Even though vein issues can affect people across all demographics, they most often affect adult women, commonly after childbirth. Fortunately an easy medical solution, endovenous laser therapy (EVLT) is available to fix varicose vein problems.

Usually a couple of over the counter medications just after the procedure are all that most patients need.”

A laser is inserted into the damaged vein, sealing it closed and forcing the blood to

For the smaller veins, a phlebectomy is performed. These veins are removed through incisions that are about the size of a grain of rice. These smaller veins branch off of the main vein but require this separate treatment. Removing the entire affected complex of veins prevents further blood backup, and relieves the symptoms over time. “Patients continue to feel better and better over the months following their procedure,” Dr. Quaid said.

After the procedure, most people go right back to work. Minimal pain and bruising is common. Wearing compression stockings and lots of walking are the main regimen for recovery. “The minimally invasive and highly effective treatment is not something that anyone should be worried about. Varicose veins can cause serious health problems if they aren’t treated, so putting to rest any fear of the treatment is important,” Dr. Quaid said.

HEALTH PRIORITIES BY DECADE

The late 1800s and early 1900s saw numerous and significant changes in medicine and the field of public health. Advances in microbiology and widespread acceptance of ‘germ theory’ ushered the U.S. into what Lloyd Novick and Cynthia Morrow call the “second or bacteriologic phase of the public health movement” in Chapter 1 of the edited volume, Public Health Administration: Principles for Population-Based Management. Throughout the 20th century and into the 21st, health priorities have expanded from infectious and environmental diseases to those of a chronic nature.

100 YEARS OF PUBLIC HEALTH Chronic diseases become the leading cause of death. However, then Surgeon General Thomas Parran, names pneumonia, still the top infectious killer, a serious health emergency. Faced with the Depression, focus turns to cost, as people ask: How will we pay for medical care?

1920s Antitoxins and vaccination programs drastically reduce the public’s risk for contracting many communicable diseases, including scarlet fever, diphtheria, and pulmonary tuberculosis. However, compared to 50 years prior, we see a leap in chronic diseases such as cancer and heart disease.

Penicillin, discovered two decades prior, comes under widespread use. Following the conversation about paying for medical care, this decade marks the beginning of the employerbased health insurance system we see in the U.S. today.

Attention returns once again to the cost of health care as costs rose and physicians increasingly turn to specialties. Over 700 companies sell health insurance. A shortage of care providers engenders federal efforts to expand educational opportunities for health care providers.

This decade witnesses a successful push for federal responsibility for the health of the poor as well as growing availability of medications and vaccinations. Key developments include medications to treat glaucoma and arthritis, the widespread introduction of the polio vaccine, and the first successful organ transplant.

WORKOUT SUPPLEMENTS

MUSCLES IN A BOTTLE? A quick flip through today’s fitness, health and even lifestyle magazines reveals an abundance of ads for workout supplements. Plugs for everything from protein powders to energy enhancers feature vibrant, ageless men and women at the prime of life and in optimal physical condition. The pitch is clear: Young people, especially men, especially athletes, need help meeting their fitness goals. And thousands of products on the market promise countless solutions. But do they, really? What are workout supplements and are they safe?

Common types of workout supplements A daunting variety of workout supplements grace the shelves of pharmacies, grocery stores, and vitamin shops. Wading through all the labels and claims can overwhelm even the most knowledgeable among us. And beyond the ads, publications — from Mens Fitness to Muscle & Fitness to bodybuilding.com — teem with articles recommending the best supplements for various activities. Definitions of a fitness supplement vary widely, including vitamins, chemicals, and macronutrient solutions. Some of the current favorites are packed with carbohydrates, caffeine, vitamin C, glutamine, creatine, tart cherry, B-complex vitamins, and branched-chain amino acid (BCAA) blends.

Who actually uses supplements? According to the National Health and Nutrition Examination Survey, more than half of Americans take some sort of dietary supplement on a regular basis. Meanwhile, it seems that more teens currently take sports-related supplements than previously thought. In a study published in the December 2012 issue of Pediatrics, a study of nearly 3,000 boys found that one in four routinely used muscle-enhancing substances. Almost half of the adolescent boys had tried such substances at least once.

Do they work? Vitamins and dietary supplements constitute a $1.5-billion annual market in the U.S. Thus, understanding the efficacy and safety of these products is essential to being an informed consumer. But untangling the data can be sticky and confusing. A 2014 study published in SpringerPlus examined effects of protein-carbohydrate blend supplements used pre- and post-

workout on CrossFit performance and body composition. This randomized study that included multiple products revealed mixed results. While some athletes benefited over the 6-week study period, others saw no improvements. In another project, researchers investigated a pre-workout supplement blend containing creatine, betaine and a dendrobium extract over a six-week period. They reported in a 2014 issue of International Journal of Medical Sciences that the product was well-tolerated and trial participants reported increased energy and focus. However, objective measures of performance and body composition showed no improvements. A 2011 study published in Nutrition & Metabolism found that the specific supplements examined, taken in conjunction with heavy weight-lifting, contributed to significant improvements in body composition, muscle mass, and performance. When taken as recommended, before and after exercise, the researchers deemed these particular products safe. In a 2014 article in the Journal of the International Society of Sports Nutrition, researchers evaluated a pre-workout blend including amino acid chains, creatine and caffeine. They found that, combined with eight days of training, no significant improvements resulted in body composition or performance. Supplement takers did, however, experience some gains (though not statistically significant) in lean body mass and lower-body strength. A 2012 Nutrition & Metabolism article showed that individuals taking a particular pre-workout supplement experienced significant improvements on objective and subjective measures. Ingesting the product before workouts led to better agility performance and lower-body muscular endurance. Study participants also reported better energy and reduced fatigue.

So, should I take workout supplements?

The studies highlighted above represent just a small sampling of the research on workout supplements, but they illustrate why making any strict claims about these products can be quite the challenge. Study designs are inconsistent, study populations tend to be small, and much research only evaluates a single product. All of this makes findings difficult to generalize.

The thing with workout supplements is, even though some do offer results, most of us don’t really need them. Remember, they’re intended to supplement, not replace, nutrition! The Mayo Clinic explains that only those engaged in high-intensity training are likely to require such extras when it comes to calories, electrolytes, or macronutrients. Which is why they suggest alternative solutions to your workout woes. Instead of taking the leap with workout supplements, the Mayo Clinic recommends athletes • Eat plenty of nutrient-rich whole foods • Stay hydrated with old-fashioned H20 • Adjust their training and meal schedules to optimize energy and re-fueling needs. If you do all that and you’re still not getting desired results, it may be time to consider supplements. But remember, talk to your doctor to ensure your exercise regimen and workout supplements are safe and right for you!

Risks of workout supplements There’s a lot of confusion about whether or not dietary, and thus workout, supplements are actually regulated in the U.S. They are, but neither under the same guidelines as other foods nor as pharmaceuticals. According to the Food & Drug Administration, “the manufacturer is responsible for ensuring that its dietary supplement products are safe before they are marketed.” Under the 1994 Dietary Supplement Health and Education Act, products only require approval if they introduce a new dietary ingredient. In these cases, the FDA will require proof of safety. The responsibility of honesty regarding the product’s effects and ingredients is largely left to the manufacturers. The FDA warns against the following with any dietary supplements:

• Mixing them with prescription and/or over-the-counter medications • Substituting supplements for prescript ion medication • Taking too much of certain supplements. In addition, supplements may aggravate existing medical issues or directly cause others. For example, many supplements should be avoided before or after surgery while others have even been linked to grave health outcomes such as acute hepatitis and liver failure. All of this means that, while many supplements can be taken safely, it’s best to keep your doctor and pharmacist in the loop. They’ll be able to advise you on the latest studies and will know which risks pertain to you specifically.
