Dear Reader: I would like to point out how much pride I take in the fact that Poudre Valley Hospital continues to enhance its services to meet our community’s health care needs. This year we intend to begin construction on an $11-million, 30,000-square-foot cancer center on the Harmony Campus in southeastern Fort Collins. The center is designed to have all aspects of care in one location rather than multiple sites as done in most communities. For northern Colorado residents with cancer, this means less stress because they will have onedoor access to diagnosis, high-quality cancer treatment and technology, research, and support and survivorship services.

The first step in constructing the cancer center was the 2011 completion of a $7.1-million expansion of Harmony Campus’ existing radiation oncology department. This was to make room for a TrueBeam STx linear accelerator, regarded as the world’s most advanced technology for treating cancer and other potentially life-threatening illnesses. The cancer center will adjoin the radiation oncology department and a medical office of superior oncology physicians. The cancer center is a story written by thousands of community members. More than $3 million in funding came from donations from 3,500 local people. Some contributions were pennies donated by young kids. Students at 11 schools held carwashes and other fundraising events to raise $85,000 to help construct the center.

For patients, the presence of UCHealth means more opportunities for care…

In the meantime, PVH continues to remain one of two hospitals that lead Front Range hospitals in how favorably patients rate their hospital care and whether they would recommend their hospital to others. The leader is Medical Center of the Rockies in Loveland.

PVH and MCR—both part of University of Colorado Health—continue to be leaders because of the innovative care provided by our staffs and physicians. Nurses, for instance, make more frequent checks on their patients than other hospitals might, and they go the proverbial extra mile to keep patients and their families fully informed about treatment plans. One of the significant endeavors underway in 2013 at PVH, MCR and other UCHealth hospitals is to develop a top-of-the-line electronic health records system. With many features that will help in patient care and safety, the secure system will allow for seamless care. This means your medical information will be readily available regardless of whether you’re treated at PVH, MCR or other UCHealth hospitals or clinics. Throughout most of 2013 a demolition project will be underway at PVH to remove the hospital’s old wing at the corner of Lemay and Doctors Lane. Dating back to 1925, this part of the hospital has aged far beyond practical use and hasn’t been used for patient care for several decades. Our plan is to replace the structure with a two-story, 113,000-square-foot medical building. This will allow us to expand PVH’s emergency room to meet a projected increase of ER patients from the current 52,000 a year to 70,000 by 2013. The new building will also increase capacity for the operating rooms, orthopedic services, infusion services, wound care, and women and children’s services. The year 2013 is the second year for University of Colorado Health. PVH and MCR—as part of the former Poudre Valley Health System—and the University of Colorado Hospital in Aurora created UCHealth to enhance patient care while responding to changes on the horizon for health care. Throughout the nation independent hospitals and health systems are similarly consolidating and collaborating. UCHealth’s first year witnessed significant expansion. By early January of this year, the system had grown to stretch from Ivinson Memorial Hospital in Laramie, Wyo., to Memorial Health System in Colorado Springs. More than 14,000 employees work for UCHealth. Almost 160 physicians are employed by UCHealth’s Colorado Health Medical Group. For patients, the presence of UCHealth means more opportunities for care, improved access to clinical trials and, among other benefits, such new services as the cancer center. Over the past year, we awarded a $750,000 grant to the North Colorado Health Alliance to launch the Weld County Community Health Campaign; made a $90,000 donation to support Red Cross Wildfire Relief; awarded a $150,000 grant to the Colorado Meth Project to expand the organization’s public education campaign focused on reaching at-risk youth in Weld County; piloted free performances of Kaiser Permanente Educational Theatre Programs’ Amazing Food Detective at 15 Northern Colorado elementary schools; and hosted the premieres of HBO’s obesity series The Weight of the Nation in Fort Collins, Greeley and Loveland. Choosing your provider When many people look for a doctor, they don’t see all the choices before them. But they’ll find they have the option of choosing between seeing a medical doctor, a nurse practitioner or a physician’s assistant. And with each of these different options comes a unique set of styles of care and training. A medical doctor receives the most education, with about 11 years of schooling, while the nurse practitioner and physician’s assistant usually have received two to three years of graduate schooling after receiving an undergraduate degree, for a total of six to seven years of schooling. Nurse practitioners Nurse practitioners may work either under the supervision of a physician or they may, after meeting certain requirements, practice independently.

For some NPs, one emphasis is on being an educator, and that can draw patients. “People will come to our clinic just to see a nurse practitioner. Because we take the time to explain things,” said Julie Deters, a nurse practitioner at Miramont Family Medicine. “I will take an hour with the patient to explain every piece of their blood work because that’s what they feel like they need to know.” Most nurse practitioners will complete a master of science in nursing after completing a fouryear bachelor’s program before becoming licensed, according to the American Academy of Family Physicians. Physician’s assistant The physician’s assistant operates with a bit less autonomy and is more similar to a medical doctor. The physician’s assistant allows a practice to see more patients than it could with

just a physician. And with this structure comes a team approach in treating patients. “I work very closely with my supervising physician. We are more like a team than anything, and that team approach helps to give them better care, because two minds are better than one,” said Kara Bradley, a physician’s assistant at Miramont Family Medicine.

“If you have a good and capable physician’s assistant, [a patient] should expect a very similar experience to seeing a medical doctor or nurse practitioner.“ A physician’s assistant also differs from a nurse practitioner in the style of training received.

27 months, according to the American Academy of Physicians Assistants. Medical doctors A medical doctor differs from the physician’s assistant and nurse practitioner, at least initially, in the amount of training received. On average,

doctors get 11 years of training, according to the American Academy of Family Physicians. A medical doctor will typically have completed a bachelor’s degree, a four-year doctorate program and three years of residency. However, according to Dr. Jan Hester of Caring for the Family in Fort Collins, it can come down to experience, where the length of time on the ground can make the difference. “I’m not convinced that [care] is remarkably different within the standards of care,” she said. Though she does believe that the depth of training can make a difference when selecting a care provider. “If you’re a complicated patient, I think a medical doctor would be better; the depth of training is greater.”

Navigating the labyrinth Story By Sam Noblett

HSA, PPO, HDHP, coinsurance, copay, co-op, COBRA, OOP, deductible. All these terms can be a bit intimidating to the person shopping for insurance. According to the Kaiser Family Foundation, about 14 million (nonelderly) Americans bought individual insurance in 2008 and an additional 157 million received insurance through their employers. For many of those 14 million, there is very little to guide their decisions. It can come down to a process of self education and buying whatever seems best from the Internet. And while there are those out there who make a living trying to guide those people through the process, such as insurance brokers, many people don’t use those resources. While insurance might not be something that everyone keeps at the forefront of their minds, it is something that can be essential to getting proper health care. “There are barriers [to receiving care] when you don’t have

insurance,” said Kathy McCreary, vice president of payer relations and network development at University of Colorado Health. While there are many changes coming to the health care industry going into 2014, there are still those who need health care until then. For more information about the upcoming , read the story “Health-care changes abound in 2014” on page 17. Individual insurance Some of those who have tough decisions to make are looking for insurance on their own. And the choices can

have a profound impact on the amount of money they may have to spend on health care each year. Before looking at various insurance plans, there are a few questions to think about: The first question a shopper should ask is how much financial risk he or she can handle, according to Robin Mitchell, owner of Mitchell-Zuber Insurance in Fort Collins. In other words, a person should decide how much he or she could spend in a worst-case scenario. The next questions include how healthy a person is, if that person currently has care providers and whether he or she wants to stay with them. After answering those questions, a buyer will next need to take a look at the different plan options. The most common type of insurance for an individual is the PPO, or preferred provider organization. And within that group there are some different types.

A PPO can be defined as a network of pre-approved providers who work for a nonnegotiable amount, said Mitch West, director of MW Family Services and a Denver-area licensed insurance broker. That means an insurance company will have a list of providers with which they have an existing agreement and with whom they prefer to work. Most traditional PPO policies provide coverage in a few ways. First the policyholder has a deductible for which they will pay most expenses up to the specified amount, with a couple exceptions. The two major exceptions are copayments, also known as copays, and coinsurance. Copays usually apply to doctor visits and some prescription drugs and mean that the insurance company covers some of the cost, while the consumer covers a predetermined amount. With coinsurance, the policyholder will pay a percentage of all bills up to an

out-of-pocket expense cap after reaching his or her deductible. To figure the potential outof-pocket cost for an individual policy, an individual should add the deductible to the outof-pocket expense cap on the coinsurance. An HSA, which stands for Health Savings Account, is one of the variations on the typical PPO plan. It’s also one of the more popular types in Northern Colorado, said Tim Hebert, owner of Sage Benefit Advisers in Fort Collins. The HSA features a higher deductible but also offers a tax benefit for the policyholder. Many times, however, the policy lacks copayments but includes zero percent coinsurance, meaning that there will be no out-of-pocket expenses above the deductible. The minimum deductible for an HSA in 2013 is $1,250, according to irs.gov, and the

maximum out-of-pocket expenses, excluding premiums, is $6,250 for an individual. It’s double that for a family. In most cases, the overall out-of-pocket expenses for a HSA will usually be less for an individual than a traditional PPO policy, West said. Carriers can currently screen potential customers for preexisting conditions, though this will change in 2014, due to implementation of the Patient Protection and Affordable Care Act. The carriers can then either exclude or increase rates depending on the results of the screenings. However, families insured under the plans are guaranteed coverage. The screenings will typically look at a person’s body mass index, tobacco use, current or past prescriptions, diagnoses, surgeries and hospitalizations, Mitchell said. As people hunt for insurance,

there are brokers available to help navigate the process at no cost to the consumer; however, brokers can receive a commission from the insurance companies. Another option available to consumers is the HMO, or health maintenance organization. An example of a company offering this type of plan in Northern Colorado is Kaiser Permanente. Due to the structure of an HMO, costs can traditionally be driven lower, but it does so by offering less out-of-network coverage, Hebert said. This model places a greater emphasis on the primary care

physician, who acts as the gatekeeper to specialists. Group Insurance for the individual Those people currently covered under their employer’s health insurance benefits fall into the largest category of the currently insured. But many of the choices they face when enrolling are similar to those decisions individuals make when buying insurance. Companies can choose to offer either just one or multiple plans to their employees, said Hebert, whose firm sells plans to businesses. If the employer does offer multiple plans to the employee,

the employee may have many of the same options as the individual. These options include whether an HSA is offered or what the deductibles are. Employees should look at what financial risk they can handle in a worst-case scenario, whether they can stay with current providers under the new plan and if they want to include family members in their plans. When it comes to including families on group plans, however, things become more muddled. When a company offers insurance to its employees, it must also contribute to the cost of premiums. However, that requirement does not extend to the families of the employees. For example, Hebert said, many times in a household in which two people have the option of enrolling in employer-provided insurance, it is

best not to enroll as a family because one person would lose the contribution of their employer. Most companies do not contribute to insurance beyond the employee themselves. Group insurance for the small business While most small-business owners go through a broker to get health insurance, they should be prepared to consider various factors. Employers considering offering group insurance should ask whether they want to provide multiple plans, define who will receive coverage and whether the business can prove it with payroll records. Also, because employers must contribute to the premiums, they must decide if it makes sense to provide insurance coverage or to give employees a raise so they can provide their own insurance.

Health-care changes abound in 2014 Story By Sam Noblett

The coming year, 2014, is going to be a year of moving mountains in terms of health care. Much of the major provisions of the Patient Protection and Affordable Care Act, also known as Obamacare, will come into effect.

While the act itself is a massive 906 pages long and contains many provisions designed to change the health care industry, there are a few major components that will have a direct effect on a large part of the population. Starting next year, every state will have an independent insurance exchange and insurers will no longer be able to deny coverage for pre-existing conditions. While the implementation of these policies will vary from state to state, Colorado has been moving forward with the creation of an exchange since 2010, when it passed Senate Bill 200. The bill sparked creation of a state health insurance exchange that will allow people to compare potential coverage options.

Medicaid

Medicaid is a state-run program that is designed to provide health coverage to lower income people, according to healthcare. gov. The program has been in place since 1965 after it was created through amendments to the Social Security Act. The people it aims to cover can include low-income individuals, families, children, the elderly and people with disabilities. Medicaid provides benefits to those who are eligible for the program, including doctor visits, emergency care, prescription drugs and preventive care for children, among other benefits. Furthermore, in Colorado, Medicaid patients have the option to enroll in a managed care program. These programs provide a care organization centered around a primary care physician who manages the care for a patient by either treating a patient or referring them to a specialist.

Additionally, insurance companies will no longer be allowed to deny coverage or adjust rates for customers based on pre-existing conditions or factors, with the exception of tobacco use. Another major factor will be the expansion of Medicare, which will raise the income qualification to 133 percent of the federal poverty level. The coming year will also bring a fee for anyone who chooses not to enroll in an insurance plan. That fee will increase year by year until 2016. Along with the expansion of Medicaid and the creation of the exchange, a system that will operate through the exchange will work to provide subsidies to individuals and families with an income between 133 and 400 percent of the federal poverty level. The effects of these changes will mean different things for a few different people. What it means for those who are uninsured The changes coming in 2014 will make the biggest differ-

ence for those who are uninsured. “The goal is to get people covered,” said Dede de Percin, executive director of the Colorado Consumer Health Initiative. De Percin and the Colorado Consumer Health Initiative have openly supported the Patient Protection and Affordable Care Act. The act aims to make health insurance more accessible for those who do not have it because they cannot afford it. It approaches this issue in a few ways. One of those ways is by expanding Medicaid. [See sidebar about Medicaid] Individuals who make no more than $15,282, or up to 133 percent of the 2013-defined federal poverty line, will be eligible for Medicaid when they weren’t before. Designations for 2014 aren’t yet available, though the income cap will likely be higher. For those who still struggle in 2014 to afford insurance but don’t qualify for Medicaid, there are further subsidies that have been put in place. The subsidies are designed to bridge the gap, based on income, on a sliding scale for those people who fall between 133 and 400 percent of the federal poverty line. That would qualify those individuals who make between

$15,282 and $45,960, according to 2013 definitions. De Percin, citing information from a separate study, claims this expansion of Medicaid could help save the lives of up to 629 more Coloradans a year. Those people who are unsure of where they are in the spectrum will see benefit from the next major piece put in place to help insure the population through the Colorado Health Benefit Exchange. This will provide an initial screening for Medicare and will be the only avenue for individuals to receive subsidies for their insurance. The Colorado Health Benefit Exchange, mandated by the Colorado Senate, plans to offer an avenue for people to shop for and compare insurance plans. “The goal of the Colorado Health Benefit Exchange is to make the process of looking for health insurance easier and to

provide high-quality support to help customers find the plan they need,“ said Myung Kim, director of communications and community outreach for the Colorado Health Benefit Exchange. The Colorado Health Benefit Exchange hasn’t yet been fully implemented; however, it will launch on Oct. 1. Customers can expect an experience similar to TurboTax — a popular tax preparation software by Intuit — when the exchange launches, Kim said. Customers will still have the option to buy insurance from a broker. One aim of the exchange is to provide a truly independent place to shop for insurance, Kim said. Starting in 2014, insurance companies will be barred from either denying coverage or raising rates due to the results of pre-screenings, according to healthcare.gov, the website dedicated to providing information about the upcoming legislation changes. Once in effect, Kim said, the only health question in Colorado that may factor into cost of coverage is the use of tobacco. Those who are uninsured and wish to stay that way will face a yearly fee. The initial fee in 2014 will be $95, or 1 percent of an individual’s income, whichever is larger, according to tax

documents surrounding the Patient Protection and Affordable Care Act. In 2015, it will rise to $325, or 2 percent, and finally in 2016, it will peak at $625, or 2.5 percent. However, a person who has chosen not to have insurance because he or she cannot afford it might be able to opt out of the penalty. What it means for the currently insured Moving forward, there will be very few obvious changes to coverage received by people who already have insurance. That does not, however, mean there will not be any changes at all. Among the changes, according to healthcare.gov: the annual spending limits by insurance companies, removing the option to change rates or drop coverage due to health conditions during renewal, and not allowing insurance companies to revoke insurance on those customers who have participated in a clinical trial for a life-threatening disease. The part of the Patient Protection and Affordable Care Act that bars insurance companies from asking about prior health conditions for those shopping for insurance also applies to those who are looking to renew their current coverage. Furthermore, all insurance plans will no longer be allowed to have any kind of benefit cap, meaning there may no longer be annual or lifetime limits

on the amount of coverage an individual may receive. A crucial change that might occur for someone who is currently insured: Those who are individually insured might qualify for a subsidy through the exchange system. Those who may qualify will be able to check their eligibility through the system when it comes online in October. There also might be a shift in the emphasis in health care that could trickle down to the customer from the provider due to increased attention being paid to the role of the primary care physician, said Jeff Thompson, director of government and corporate relations for University of Colorado Health. However, the different parts of the legislation that will help to provide the increased emphasis on primary care have been in effect already before this year, making them part of a gradual shift. What it means for health care providers While health care providers already have and will continue to face changes in their industry due to the enactment of the Patient Protection and Affordable Care Act, there are a few areas that have become a large focus and will continue to be going forward. From the perspective of a large hospital system, such as University of Colorado Health, there has been a shift to focus on providing more primary care access. With that has

come coordination of care for patients, with a focus on the patients themselves. Part of that shift has included creating a communication network to facilitate the care coordination. One of the biggest recent expenses for University of Colorado Health has been the creation of a centralized system to access patient records across its different locations, Thompson said. Due to the expansion in Medicaid, there may be as many as 160,000 people newly insured by Medicaid, said Dr. John Bender, the president-elect of the Colorado Medical Society.

“The private sector, I feel, has an opportunity, if they are able, to adapt and see Medicaid patients profitably,” said Bender, also CEO of Miramont Family Medicine. “There would be few industries that would be upset about 160,000 new customers.” Bender echoed Thompson’s belief that coordination of care will become more important, referring to it as an increase in team-based care. And furthermore, patients should expect to be more engaged with their health care providers.
