Welcome to SFH News The Trust has made many improvements over the last year, and it continues to build on the good work that is going on to make sure all services are delivering consistent high quality services everywhere.

However, there is still much more to do to meet the challenges the Trust faces both in terms of financial turnaround and some of the quality issues we need to get better at managing – such as sepsis, C diff rates and mortality which as you know we have a steady focus on.

In this edition of SFH News you can read about some of our improvements and innovations, all of which is testament to the incredible dedication of our staff to provide the very best care to our patients. You will read about new services at Newark Hospital with the introduction of breast surgery and state of the art diagnostic imaging of the eyes. These services provide essential diagnosis and treatment for people who would otherwise need to travel to another hospital.

The Trust has once again topped the list for the quality of its patient environments. Results from the Patient-Led Assessments of the Care Environment (PLACE), published by the Health and Social Care Information Centre, show the Trust had impressive scores for cleanliness, nutrition and hydration, privacy, dignity and wellbeing, condition, appearance and maintenance. We have also scored top marks for our children’s diabetes service – the quality of care we provide has been rated at an all-time high according to the Royal College of Paediatrics and Child Health.

These are just a few examples of the many developments that teams across our hospitals are putting into practice every day in order to improve quality. All this hard work has been reflected in the number of nominations we have received for this year’s Staff Excellence Awards from patients and staff – a fantastic 202 entries!

Our Staff Excellence Awards ceremony, taking place in October this year, is a heart-warming occasion full of celebration and appreciation for the great care delivered by our staff, and is an event we all really look forward to every year.

We promised to keep you updated on any news of our CQC report following the inspection in June. We are expecting the CQC to publish its findings in a report during the autumn. We will respond to the findings once the report has been published and share with you any key messages and actions.

I know we will continue to face many challenges over the coming months, I am confident that, by working together, we will continue to bring effective and safe delivery of healthcare to our local communities.

Financial turnaround The Trust’s July 2015 financial position is a deficit of £15.91m, against a year to date plan deficit of £14.45m. This represents a deterioration of £1.52m against plan in July.

The increase in the rate of deterioration means that the Trust is now at significant risk of exceeding its planned loss of £44.5m for 2015/16.

The Trust continues to develop its Turnaround and cost improvement plans, requiring all staff to be fully engaged and working on these plans at pace.

At the end of 2014 the Board of Directors committed to making the Trust truly ‘smoke free’ and encouraging all staff, patients and visitors to give up smoking to help create a better, healthier environment for everyone.

The Trust is supporting the national Stoptober campaign in October, educating about the risks of smoking, highlighting the benefits of not smoking and working with partners to provide support for people who wish to quit - enabling all of the Trust’s hospitals to be smoke free.

The Nursing Welcome Day has been developed as part of the Trust’s drive to recruit more qualified staff and will now be held annually.

The nurses were invited to a welcome day during the summer, pictured on the front cover. They were given the opportunity to tour their future ward and the hospital, meet with their line manager, ask any Human Resource questions.

Teresa Shaw, Trust Preceptorship Development Nurse, said: “We are working really hard to recruit nurses and are battling against a national shortage. Our welcome day was a great opportunity to meet nurses due to start with us in late September and get them prepared for their first day.

Louise Curtis, one of the Trust’s new starters, said: “It was great to be able to come along to King’s Mill Hospital and meet the staff there, as well as the other new starters. I am very excited about joining the Trust and am looking forward

Victoria Bagshaw, Deputy Director of Nursing and Quality

New Deputy Director of Nursing and Quality The Trust is delighted to announce the appointment of Victoria Bagshaw as the new Deputy Director of Nursing and Quality.

Victoria, who began her role in June, joined the Trust from Central Manchester University Hospitals, where she worked for over two years as Deputy Director of Nursing. Previous to this she worked in a variety of senior nursing, clinical, and academic roles.

Victoria, who has special interests in professional practice and patient experience and safety, said: “It is a privilege to have the opportunity to work with the unparalleled staff at the Trust and be part of a team which is committed to sustaining high standards of care to people who depend on our services.”

Victoria lives in Harthill with her partner and two sons. She replaces Lisa Dinsdale, who served as Deputy Director of Nursing and Quality since 2014.

Successful overseas nurse recruitment The Trust is also celebrating a successful overseas recruitment drive after offering 64 highly skilled nurses posts within the Trust next year.

Recruited from the Philippines, the new staff will now undergo the international recruitment processes including English tests and visa applications, and will arrive in groups of 20 in March, April and May 2016. This is to ensure they are integrated successfully.

Kate Lorenti, Deputy Director of Human Resources at the Trust said: “This has been a real success. We were very impressed by the high quality of the caring nurses we interviewed in Manila and to offer roles to 64

qualified nurses is fantastic news.”

The recruitment drive continues at a pace in the UK and overseas. Over the coming months the Trust has plans to visit Italy and Romania, Spain, Greece and Croatia. On 26 September an event will be held at King’s Mill Hospital to recruit qualified UK nurses. At a similar event in April the Trust recruited over 20 new staff members, and towards the end of September 35 newly qualified nurses are set to start working in the Trust’s hospitals.

Improving for excellence Innovative operating theatres

Staff at Sherwood Forest Hospitals are leading the way and bringing new and innovative techniques into their operating theatres.

Three Operating Department Practitioners, based at King’s Mill Hospital, have embarked on an intensive training course and are now accredited Radiation Protection Supervisors.

This qualification means that they can support surgeons and take the pressure off our radiographers by operating a Mini-C Arm Image Intensifier Machine to take X-rays whilst an operation is carried out.

The course, hosted by the University of Nottingham, focused on managing the hazards of radiation and looked at quality and control and the legal side of providing X-rays during surgery. Students then completed a test to confirm they had the knowledge needed to act as Radiation Protection Supervisors.

Brent Gillicker, Team Leader Operating Theatres, attended the course and said: “Our accreditation is fantastic news for theatres as it means that we can keep X-rays happening during operations, releasing our radiographer

Caring for children with diabetes The quality of care for children and young people with diabetes provided is at an all-time high at the Trust according to the results of an audit published by the Royal College of Paediatrics and Child Health.

The National Diabetes Paediatric Audit 2013/14 obtained results of 26,598 children and young people receiving care within England and Wales. The Trust’s results were rated well above the national average.

This is great news for the health of 150 children and young people in the local area who are currently receiving care from the Trust’s Children and Young People’s Paediatric Diabetes Team.

Excellent control of diabetes is indicated by achieving an HbA1c (a measure of diabetes control) target of less than 58mmol/mol (unit measurements).

An HbA1c above 80 mmol/mol would indicate poor control, and if allowed to persist would put the individual at a significant risk of long term damage to their health.

Diabetes Specialist Nurse Helen Marsh, said: “The Trust’s results are a third higher than the national average - 25.5% of children and young people achieved a target HbA1c of less than 58mmol/mol compared to the national average of 18.4%. This indicates that we have more children and young people at the desired target level, which is an achievement we are very proud of.”

The recommended completion of seven key care processes that children and young people should receive within their care package also compared favourably to the national average – the Trust completed almost one quarter more care processes than the national average.

Helen added: “We are committed to continuing and progressing our service for all children and young people within our care. We are passionate about improving outcomes to help them live with diabetes without complications.”

This new advance is part of the Trust’s plan to streamline the work of its operating theatres and increase the number of procedures carried out each day.

Patients SCORE Trust’s hospitals top again Sherwood Forest Hospitals has once again topped the list for the quality of its patient environments.

Results from the Patient-led Assessments of the Care Environment (PLACE) published by the Health and Social Care Information Centre show the Trust scored an impressive 99.87% for cleanliness, 94.47% for nutrition and hydration, 89.36% for privacy, dignity and wellbeing, 94.36% for condition, appearance and maintenance, The dementia-friendly assessment was new for this year and we scored 71.77% for These results, which are combined for King’s Mill, Mansfield Community and Newark hospitals, are well above the national averages of 97.57% for cleanliness, 88.49% for nutrition and hydration, 86.03% for privacy, dignity and wellbeing, 90.11 % for condition, appearance and maintenance. The national average for the dementia assessment was 74.51%.

Comments from patient representatives involved in the audit included: “Every patient that I spoke to had nothing but praise for the staff - how kind and respectful they were. The food was good with a varied menu and the food service was excellent. The hospitals were spotless in every area I visited - I am so impressed.”

Frailty Intervention Team

Dedicated care for frail, older people People in the UK are living longer, but many are living with one or more long-term medical conditions and, for a significant number, advancing age brings frailty.

Committed to improving care for frail patients, the Trust has expanded its Frailty Intervention Team – professionals who are dedicated to providing high quality care to frail people.

Frailty has many different causes, but in essence it means that a person is particularly vulnerable to adverse events or illnesses (for example a simple urine infection or a fall), and as a consequence may suffer a dramatic change in their physical or mental health. Frailty affects nearly 50% of the over 85s population and 15% of over 65s.

Kirsty Woodward, Geriatric Advanced Nurse Practitioner at the Trust, said: “Working together, the team - which consists of doctors, nurses, physiotherapists,

occupational therapists, Social Services and carers - manages patients through a holistic assessment known as a Comprehensive Geriatric Assessment. A personalised care plan is developed to address a patient’s needs and appropriate treatment and services to enhance their care both within hospital and in their home.

The team’s leading edge performance and drive to provide exceptional care to frail, older patients has warranted an entry into the East Midlands Academic Health Science Network Awards, which recognise and celebrate the work of NHS, health and social care organisations to develop new - and ensure better delivery of existing healthcare services for local people.

Leading edge technology for Newark

Ophthalmic Imaging of the first patient – Mr Michael Woodward

As part of the commitment to increase services at Newark Hospital, state of the art diagnostic imaging of the eyes has now been introduced.

The SPECTRALIS OCT is a unique ophthalmic imaging device which uses laser light instead of a bright flash of white light to to scan the eye.

Funded by the Trust’s equipment budget, commissioners, and an incredible £35,000 donation from the Friends of Newark Hospital,

the new scanner with fundus photography will allow patients to have specialised eye imaging done closer to home, rather than travelling to Lincoln or King’s Mill hospitals.

“Using this leading edge technology, doctors can monitor glaucoma, diabetic retinopathy, macular degeneration and many other eye diseases less evasively and even more effectively than before.”

Mrs Prema Maharajan, Associate Specialist in Ophthalmology said: “This exciting new imaging technology gives doctors the ability to see inside the eye like never before - the 3D images produced allow for improved detection of structural changes within the eye.

There are currently four imaging clinics running over two days at the hospital. Plans are underway to expand this service by introducing a dedicated medical retina clinic with treatment facilities. This is due to be in place by the autumn.

New CT scanner for Newark A new £400,000 CT scanner is in the process of being installed at Newark Hospital, which will provide detailed CT cross-section images.

Hospital is equipped with the latest technology to deliver the most advanced service to the local community. It highlights the Trust’s commitment to the future of Newark Hospital as a diagnostic and ambulatory centre outlined in the Vision For Newark Hospitals.”

Wheelchair donation to thank nurses Former Newark Hospital patient, Mrs Vera Broadley, has kindly donated a self-propelling wheelchair to the Fernwood Unit, costing £150, to say thank you to the nurses who cared for her while she recovered from septicaemia and pneumonia.

Vera spent several months earlier this year in the hospital’s Fernwood Unit, which cares for patients who require a short period of recuperation prior to returning home after treatment.

Mrs Broadley said: “The unit was one of

the best places to be in in the hospital. It was like a three star hotel. The staff were interested in you and the food was always hot. It was nice that we ate dinner in the dining room rather than in our beds. I made some friends there too.”

“The wheelchair is my way of saying thank you to the staff who looked after me.”

Acting Ward Leader, Hazel Siddington, said: “This was a lovely gesture and Vera’s kind donation is much appreciated. It will be very useful for our wheelchair dependent patients.”

Breast surgery at Newark Hospital Local women are now benefitng from having breast surgery closer to home thanks to the introduction of operations at Newark Hospital in July.

There is now a surgical list every two weeks for local women who require benign (non-cancerous) breast procedures.

It is anticipated that most women undergoing these surgical procedures will be day case patients, and will be able to return home after a short period of recuperation following the operation.

Mr Mohamed Jahan, Consultant Breast Surgeon at the Trust said: “We are delighted to introduce benign breast surgery to Newark Hospital. This allows us make the most of our skills and resources, and provide essential surgery to local women who would otherwise need to travel.”

Braving the shave Newark Hospital Housekeeping Assistant, Tracy Smith, got rid of all her hair on 31 August as part of the Macmillan Brave the Shave campaign.

Tracey decided to brave it after hearing the idea advertised on the radio, and her colleague, Healthcare Assistant Kiyth Fotitt, volunteered to do the deed. Almost £2m has been raised nationally for Macmillan Cancer Support through the campaign.

“I know people who have died of cancer and so wanted to do my bit” Tracey said.

“I’ve always had my hair short so I thought this was something I could do.” It is the first time she has done charity fundraising and so set herself a target of £100. So far she has managed to raise £222.

It took about an hour to complete the shave, which was watched by off-duty colleagues and family. Once it was finished Tracey said she was glad she had decided to do the challenge and was pleased with the finished result. Her granddaughter, eight year old Madison, said she felt her grandmother had been brave to lose her hair. “I think she looks really pretty,” she said.

Members of the Trust Board and Council of Governors convey their deepest sympathy to the family of John Swanwick, Public Governor for Mansfield, who passed away peacefully on 16 July. Fellow governors attended his funeral on 5 August, together with the Trust’s Chairman and Chief Executive. Donations for the Haematology Ward were requested by John’s family in lieu of flowers, which totalled £700 at the time of going to print. Thank you to all those people who kindly donated.

Our survey said…

A big thank you to all members who completed our recent membership survey - a total of 299 responses were received in total.

Your top preferences for articles in SFH News were items about new and existing services, patient stories and behind the scenes. In view of this feedback we have included articles about new innovations which are coming to theatres and a new ophthalmic imaging device which is coming to Newark Hospital in this edition.

Results from the survey indicated that you wished for health events and behind the scenes tours to be arranged for members, with topics such as healthy living and eating, dementia and cardiology being the most popular. We have therefore scheduled healthy living events in Alfreton and Newark and at King’s Mill in October and January. A dementia event will take place at this year’s Annual General Meeting in September and a cardiology event is in the pipeline for February 2016.

The July 2015 edition of SFH News included a follow up article which advised that in the future the magazine would no longer be posted out, unless members contacted us. So far a total of 278 members have responded to this question. Therefore, with effect from this edition, copies will only be posted to those members who have contacted us in this regard.

Volunteers’ cash boost for Dementia Care Appeal Volunteers from the Daffodil café at Kings Mill Hospital have donated a staggering £30,000 to the Trust’s Dementia Care Appeal. The funds were raised from profits of the Daffodil Café, thanks to its many hardworking and dedicated volunteers who wholeheartedly support the Dementia Care Appeal.

Currently undergoing refurbishment, the Daffodil Café will re-open its doors to customers on 28 September.

Lorraine Brooks, Dementia Specialist Nurse, said: “We are very grateful to the Daffodil

Café volunteers for their very generous donation to our appeal. We hope the whole community will support our campaign to provide high quality ‘dementia friendly’ care to all our patients.”

Summer fun for Newark Hospital’s dementia patients Patients at Newark Hospital celebrated the summer with an innovative new approach to dementia care adopted on Sconce Ward.

The ward which looks after older patients, many with dementia, has been holding a new monthly café style get-together. Sessions are designed to encourage interaction and focus on reminiscence, with patients given the opportunity to have a cup of tea, a piece of cake and a chat.

At their August meeting, the group focussed their fun on a summer theme and celebrated the season with ice-creams, candy floss, music and even some sand.

Dionne Archer, Occupational Therapist at the Trust, is involved in arranging the sessions, and said: “Hospital can be a daunting place for everyone, but especially for those with dementia, so it’s wonderful to get our patients out of the ward and interacting with each other.

“The café has gone down brilliantly, with 10 SFHnews | Autumn 2015

patients singing, dancing and chatting about the ‘good old days’. Patients were particularly

excited about our summer café and were very excited to take part.”

The Trust is celebrating another record year for nominations! Congratulations to the many dedicated members of staff across the hospital sites who have been nominated for this year’s Staff Excellence Awards.

Vision Studio School students As one of the key employers working in partnership with Vision West Notts College, the Trust is helping to develop its future workforce at The Vision Studio School. The school caters for students aged 14 to 19 (Years 10 to 13) of all abilities who follow a specially-designed curriculum combining academic and vocational subjects.

Students in Year 10 study core subjects of English, maths and science alongside specialist training in health and social care or engineering, and weekly work experience.

Kerisha Brocklehurst a Year 10 health and social care student said: “coming to the

Studio School is the best decision I’ve ever made. I go home every night to tell my mum I’ve learnt something new.”

Students at the school benefit from small class sizes of no more than 20, which give them quality time with teachers, and the opportunity to build more supportive relationships with them. This ensures any difficulties students may have with their learning can be quickly addressed, and they feel confident that the teachers will help them progress with their studies and future career.

All students are allocated a personal coach whom they meet each week to help them track their progress both on specific criteria

such as attendance, progress towards grades and on life skills such as time keeping, and interview skills. Each coaching session also gives students the opportunity to discuss their goals, aspirations and areas to improve.

As they progress into Year 12, students continue to focus on their specialised learning pathway in either health and social care or engineering. Their learning is enhanced through twice weekly work experience with partner organisations such as the Trust – enabling them to develop valuable skills and knowledge that will help them secure a job.

A day in the life of a porter Porters play a vital role throughout NHS hospitals. Their role is varied - transporting patients, equipment, medication, samples and test results from one place to another, ensuring that services run smoothly.

However, there are some important things to remember, and certain information you should never share online to prevent everything from identity theft to your physical safety. In particular:

• Limit your work history details on sites such as LinkedIn • Always verify that a page claiming to be from a friend actually belongs to that person before sharing too much information or clicking on links • Take time to review your privacy options for limiting who can see various aspects of your personal information, and search your name on Google to view your profile as others see it on social networking sites.

Mansfield and Ashfield Respiratory Service County Health Partnership’s Respiratory Service, based at Ashfield Health Village, provides a range of services within the community for patients from Mansfield and Ashfield.

Home Oxygen Assessment ServiceAssessment and Review (HOS-AR) Assessment, support and education is provided for patients requiring oxygen therapy at home. In additional to Long Term Oxygen Therapy (LTOT), the team can assess patients for the benefit of Ambulatory Oxygen (AOT) and give advice regarding Palliative Oxygen Therapy (POT) Pulmonary rehabilitation Patients can be assessed for a six week programme running twice weekly at locations in Mansfield and Ashfield. To gain a greater insight into how porters influence the patient experience and to understand the challenges they face, Ben Widdowson, Head of Estates & Facilities, spent a day shadowing the portering team at King’s Mill Hospital.

Ben spent his time with five different porters carrying out patient moves, medical gas moves and working in the post room. He found it really rewarding to interact with the patients and was delighted to hear them comment favourably, without exception, on the care they were receiving. Ben finished his shift physically worn out! He said: “The Trust’s dedicated porters are an essential part of the hospital team.

They go above and beyond their daily duties to provide great care for patients and support to staff, and provide the scaffolding that keeps our hospitals running smoothly.”

Providing support to the consultant-led clinic at Mansfield Community Hospital, the service also has strong links with the secondary and community based (PRISM) Specialist Respiratory and Early Discharge Nurses.

Celebrating staff success

Showcasing best practice Sherwood Forest Hospitals is showcasing examples of its best practice and leading edge performance with six entries in this year’s Health Service Journal (HSJ) Awards. The shortlist will be announced on 17 September.

Category: Compassionate Patient Care Project: Chlamydia Screening for teenagers by non-clinical staff The most commonly diagnosed sexually transmitted infection in England is chlamydia, with 206,774 diagnoses being reported to the Public Health England in 2014. Young people aged between 15-24 years of age have the highest prevalence of the infection and the National Chlamydia Screening Programme recommends that sexually active men and women under 25 years old should be screened for chlamydia every year, and on change of sexual partner.

The aim of screening for, and treating, chlamydia infection is to reduce the community pool of infection and to lower the prevalence of chlamydia infection.

This will then reduce the long term consequences of infection such as pelvic inflammatory disease, ectopic pregnancy, tubal infertility and epididymo-orchitis.

Category: Improving Care with Technology Project: Successful introduction of VitalPAC The innovative use of VitalPAC electronic observations does much more than replace a paper chart. It also drives compliance with policies and procedures, reduces human error, provides clear audit trails and allows rapid evaluation of data.

This improves patient outcomes and also organisational efficiency. Time is saved with paperwork processes – releasing more time for nurses and doctors to provide direct patient care. Decisions can be made at the bedside instead of around a computer as test results and other key clinical data can be reviewed on mobile devices.

Category: Improving Care with Technology Project: Sexual Health – introduction of an automated results service High numbers of patients have STI tests by the Trust’s Sexual Health Service includes Genitourinary Medicine (GUM) clinics on two sites and Contraception and Sexual Health (CaSH) clinics on nine sites.

In order to deal with the volume of results, the Trust previously used a system of ‘no news is good news’ and only informed patients of positive results.

This, however, resulted in patients with negative results having no notification or ringing the clinics to request results. While those patients with positive results were contacted, there could be delays if they did not respond to telephone calls or letters.

Category: Compassionate Patient Care Project: Extending specialist women’s health physiotherapy service to include treatment of male incontinence Historically the Trust offered treatment to women who suffered incontinence and an adhoc service to men. However, following some changes within the Women’s Health Team, the need was identified for sustainable changes to the provision of care provided for the treatment of male incontinence.

Sherwood Forest Hospitals staff worked with other acute NHS trusts to determine best practice and found that male incontinence was routinely treated by band 6 staff and above. Engaging the band 6 physiotherapists who would be treating men, a meeting was held with HR in attendance to enable all staff to raise their concerns privately and it was identified that the overwhelming request was for thorough training.

Category: Patient Safety Project: Reducing missed and delayed doses of medicines The Nursing and Midwifery Council states that medicines administration “is not solely a mechanistic task to be

performed in strict compliance with the written prescription…it requires thought and… professional judgement.” An area of major concern is where a dose is omitted or delayed, which was the second largest category of medication incidents reported to the NPSA in 2007.

Improvements have been made within the Trust’s Medicines Management to ensure that patients are given medicines according to their prescription.

Category: Rising Star - Samantha Musson Physiotherapist Samantha Musson is a band 7 Team Leader in the Women’s Health and Continence Physiotherapy service and has been employed at the Trust since November 2007.

When asked to describe her position and the changes that she has made since taking on her role Samantha said: “I felt it was essential was to ensure equity in service delivery across our sites. We previously covered both obstetrics and gynaecology at King’s Mill but only gynaecology at Newark Hospital, which meant that pregnant and postnatal women had to travel for a physiotherapy appointment with our specialist team. A Newark obstetric clinic is well established and our patients are getting equitable care.

“Running alongside this development was the urgent development of a male incontinence service. We now have well trained staff, improved patient care and an increase in the profile of the team in the general women’s health and continence community.

“We are the first team to use social media - to advertise courses and keep up to date with developments in women’s health and continence, and are currently engaging patients to determine if they would appreciate access to live weekly chat physiotherapist sessions. This would lead to improved patient access to clinicians, and we would be the first team to do something like this both within the hospital and within the East Midlands area.

Surviving Sepsis Sepsis kills 37,000 people every year in the UK. It is the third highest cause of mortality in the hospital setting and the number of patients with sepsis has more than doubled over the last ten years. It is a time-sensitive condition that requires prompt identification and treatment, which means that healthcare professionals need the knowledge and skills to care for and manage the septic patient.

Teams of healthcare professionals at Sherwood Forest Hospitals have been working hard towards reducing sepsis associated mortality, overseen by the Sepsis Working Group, supported by a Sepsis Nurse and a Clinical Lead. The newly formed Sepsis Task Force has been established to build on the excellent work already underway, while supporting

and driving the pace of change and development.

Key to the management of the septic patient is recognition of urgency. Vigilance and surveillance are key components of the identification process and we acknowledge that all members of the team play a relevant part in this. Many of our nurses, doctors, allied healthcare professionals and healthcare assistants have attended nationally recognised courses that contain a component of sepsis training. Sepsis updates are also included in our mandatory training, induction programmes and, more recently, sepsis awareness sessions have been cascaded across all wards and departments to update all healthcare professionals across the Trust with key messages about sepsis.

These messages are consistent across

all our training programmes. Screening patients for sepsis is paramount in order to identify those with the early signs of sepsis, therefore any patient with a National Early Warning Score of 3 or more must be screened for sepsis using a universal tool, provided across the Trust. Once identified as such, treatment is time-critical and the team must ensure that the patient receives six key elements of care within the hour – the Sepsis Six (see image): 1. Oxygen therapy 2. Intravenous fluids 3. IV antibiotics 4. M easurement of lactate and haemoglobin levels 5. Blood cultures 6. F luid balance monitoring to be commenced with accurate urine output measurement When given within 60 minutes, IV antibiotics have been proven to improve mortality rates. For every additional hour that the patient waits for these important drugs, the mortality rate is increased by 7.6%, so compliance is being monitored in all aspects of sepsis care across the Trust. Recent weekly audits have shown that compliance with screening has frequently reached 100% in the Trust’s admission areas, and compliance with the Sepsis Six within the hour is also increasing.

A number of initiatives are currently underway to increase the speed with which treatment is administered and to improve the service provided for patients with sepsis. Sepsis Champions are now available on every ward within the Trust.

Tackling C diff The Trust has continued to see a higher number of post-72 hours Clostridium difficile (C diff) infections during this year. C diff is a type of bacterial infection that can affect the digestive system. One of the main contributing causes is the use of antibiotics. The symptoms of C diff range from mild to severe and include diarrhoea, high temperature and painful abdominal cramps. What can you do to help?

•R eport any recent courses of antibiotics you have had, when admitted to hospital • I nform staff of your usual bowel habits at home and let them know if there are any changes to these whilst in hospital • I nform staff if you have had C diff infection in the past • I f visiting a relative/friend please wash your hands on arrival to the ward and before leaving. A hand wash sink is available in all side rooms and bay areas • Please do not visit hospital if you are experiencing symptoms of diarrhoea that are not usual for you. You should refrain from

visiting until you have been symptom free for at least 72 hours • Always take courses of antibiotics as prescribed by your doctor and finish the course, even if you are feeling better. Never share antibiotics with friends or other family members.

Discussions have been taking place regarding future management of C diff across the whole health economy, identifying triggers and practice issues. Delegates from the Infection Prevention and Control Committee visited Royal United Bath Hospitals in June and brought back ideas to put into place at Sherwood Forest Hospitals.

Charity bike ride for children with kidney disease Sherwood Forest Hospitals’ Paediatricians, Dr Michael Yanney and Dr Simon Rhodes, joined Nottingham City Consultant Paediatric Nephrologist Dr Andrew Lunn on part of a mammoth charity bike ride for EMEESY, the children’s kidney network in the summer.

EMEESY cares for children with a variety of kidneys and bladder problems, including complex ones and those requiring dialysis and renal transplants. They provide care to children from the East Midlands, East of England and South Yorkshire (hence, the EMEESY children’s kidney network). The main specialist service is at Nottingham Children’s Hospital, but they have links with consultant paediatricians and shared care clinics in local hospitals throughout the region, including King’s Mill Hospital.

Reducing mortality The quality of care provided to patients is of paramount importance to the Trust. Reducing unexpected deaths, or mortality, is the key priority and staff are working hard to address this. There are numerous reasons for these, often caused by seasonal pressures.

Mortality is higher in the winter months, which is related to winter virus epidemics - particularly the severity of the flu strains, which affect elderly patients or those with chest infections more, and the severity of cold weather.

The Trust is addressing its mortality figures and a significant amount of effort has gone into understanding why they have been raised. There are some clinical issues which could be improved and an action plan has been agreed with the Trust’s partners in the Mid Notts Clinical Commissioning Group (CCG) to address these. Specific clinical areas are being targeted, including the diagnosis and treatment of sepsis, pneumonia, GI (stomach and bowel) bleeding and acute kidney injury.

King’s Mill employee dons his dancing shoes for Neonates Unit A staff member, his wife and family have all donned their 1940s dancing shoes and have raised £900 for the Trust’s Neonatal Unit.

Steve Davies, who works as a Materials Management and Goods Receipt Services Coordinator at King’s Mill, and his wife Angela, decided to celebrate their 25th wedding anniversary in style this year by hosting a 1940s themed party.

Instead of presents the generous couple asked for donations to the King’s Mill Neonatal Unit, a decision made to mark the care their nephew

Benjamin received there six years ago.

Steve said: “Benjamin was looked after brilliantly when he was in the unit and is now a very healthy little boy. When we were planning our anniversary party we thought that it would be good to give something back to the hospital and say a proper thank you.”

Karen Fisher, Acting Chief Executive, said: “It’s wonderful that people think of us when celebrating such a great anniversary. My thanks go out to Steve, Angela and their friends and family who have made a real difference to patient care.”

Local carers were given the opportunity to access valuable free support in July, thanks to a roadshow hosted at King’s Mill Hospital.

There comes a time when the job is just so hard

Trevor Clower, an unpaid voluntary carer, planned the event from his home in Nottingham and brought together seven local community groups who support those caring for their loved ones. Organising many other events across the county and city of Nottingham, Trevor is working with a total of 60 local organisations who offer free support about rights and benefits to help carers to also have a life themselves and to cope with the isolation they are often dealing with.

Trevor said: “There are 26,000 unpaid carers in the county, many of them unaware of the additional free support available that the Carers’ Roadshows can help them to access. I am pleased to have brought the roadshow back to King’s Mill Hospital for the second year and delighted that so many people came along to access some additional help and support. This is a subject incredibly close to my heart and I plan to continue the events in the city and county for as long as I possibly can.”

Volunteers, Cardiorespiratory Department, Newark Hospital Nominated by Lynne Knowles, Cardiorespiratory & Vascular Manager “I would like to nominate the volunteers in the Cardiorespiratory Department at Newark Hospital. They have been there on a trial basis and have been a tremendous success with staff and patients. The department uses the Eastwood Unit reception but this is not always manned. Having a volunteer around has offered the reassurance to patients that they are in the right area and they can re-direct if patients come to the department when they should be in the outpatients department.

Cardiorespiratory staff have witnessed at first hand the positive effect the volunteers have – answering queries from patients, reassuring them they are in the right place, keeping them occupied, notifying staff of patients arrivals, dealing with late arrivals and helping out with various aspects of paperwork etc. All staff are of the opinion that the volunteer service should definitely be provided on a permanent basis.”

I came to the NHS as a temp, along with five others. The office gets very busy, but Amanda always makes time to make sure we all know what we’re doing and never fails to help us out when we need it. She’s been so stressed training all of us, even though she’s only been permanent since December she does a really good job of it, and we, the temps, really appreciate it. Most of the week Amanda is the most experienced team member in our office and she has to train us and make sure we don’t make mistakes, on top of her massive workload, but she’s always really happy to do it.

This nomination is not just from myself but also on behalf of the rest of the team – Linda, Anne, Nicky, Andrew, Will, Kay and Claire and we hope you will seriously consider her for Star of the Month as she really deserves it.”

Brent Gillicker, ODP, Theatres, and Lee Scothern, Theatre Support Worker Nominated by Dale Travis, Divisional General Manager, and Liz Williamson, Divisional Nurse “Brent is an Operating Department Practitioner in theatres and Lee is a Theatre Support Worker, but they work as a collective team on improving the patient experience through theatres. They have put themselves forward voluntarily to focus on the patient journey through theatre and to make improvements to ensure the changes are embedded. Their work portfolio focuses on scheduling improvements and ensuring all the elements that make a productive theatre are ready to go for the first patient.

Our particular nomination is to reflect the impact they have had on the reduction of same day cancelled operations. They developed a new cancellation policy and algorithm in March and have worked tirelessly to implement it. In April we saw a 50% reduction, compared to the same month the previous year, in the number of cancelled operations (same day) for non-clinical reasons. Again in May this continued and we see the same in June. For May the overall Trust compliance had reduced to 0.3% against a target of 0.8%. This is a fantastic achievement in just two months and is a perfect example of how staff on the shop floor can really make the difference by a simple change.

Thank you I was very apprehensive about my procedure yesterday at the Endoscopy ward. I had no need to be though as all the staff in the department were so professional and considerate which I thank you all for. A truly wonderful group of people.

I am writing to give you some feedback following my visits to Newark Hospital to have hearing test/hearing aid fitted. I found the person who dealt with me on the three appointments I had to be extremely helpful. I found it difficult to come to terms with having hearing aids and although I really didn’t persevere with them as I should, she was very patient and encouraging.

Thank you so much for the kindness and care given to …………. on the many occasions she attended Newark Hospital for scans. It was an ordeal for her attending your department as she hated being scanned but it helped so much having such nice, professional, caring people to look after her.

Special thanks to …….. who always went that “extra mile”. Thank you all.

I attended Newark Hospital early on Sunday morning with an infected wound which would not wait until my GP was available. I was home within one hour of leaving with antibiotics which have solved the problem.

Today I have taken an older friend with a knee injury and again received exemplary treatment, including an x-ray. On both occasions everyone was kind and helpful.

I recently spent the day on Minster Ward and had a small surgical procedure carried out on my feet. I was delighted with the care I received from the podiatry staff, the ward staff and the theatre staff. I was especially pleased with the care I had from the theatre nurse. The hospital is spotless, the staff considerate and caring. I would not hesitate to recommend Newark Hospital. Thank you.

Patient Experience Team – here to listen The Patient Experience Team (PET) incorporates Complaints and the Patient Advice and Liaison Service.

PET provides a confidential advice and information service. Dedicated to listening to those who use our services, their carers and relatives. Your views are important and will enable the Trust to improve services.

The Patient Experience Team can help to: • Make a complaint, compliment or resolve issues you may be concerned about • Signpost to other services both within and outside of the Trust • Collate your comments and suggestions through questionnaires, face to face discussions, comment cards, Trust Web site, email or telephone • Find independent help and support • Involve and engage you in Trust work through membership, volunteering and attending specific groups.

All NHS Staff are experienced in helping resolve concerns, but if they are not able to resolve the problem or you don’t know who to ask, the Patient Experience Team is here to assist you. Their aim is to resolve any

issues quickly, working with you to gain a satisfactory outcome.

Confidentiality The PET service is confidential. The only exception to this is if you indicate that you or someone else is at risk, in which case the staff member may act with or without your consent in the interests of safety.

You will be asked to give permission for the staff member to share any information you give. If you are not the patient, consent of the patient must be determined. No action on behalf of the patient will be taken without consent.

The staff member may discuss your difficulties with a colleague or supervisor in confidence to seek advice. This is to ensure that the team provides the best possible advice and support.

New bedside folders help keep things clear Back in June you may have noticed the new patient bedside folders being delivered to all wards across the Trust. The folders were designed to encourage nurses, healthcare assistants, physiotherapists and occupational therapists to complete all of their documentation and store it safely in one place – the patient’s bedside.

The folders contain all of the documentation that needs to be completed for patients and by using them Trust staff can ensure that all of the relevant paperwork is complete. They also act as a helpful reminder for temporary and student staff as they contain vital information on infection control, medicines management, nutrition, tissue

viability, fluid management and glucose management.

Denise Clay, Trust Practice Development Matron and lead for the new folders, said: “The folders have been a great success as transferring all documentation to the bedside aids communication between nursing staff and patients, ensuring that the patient remains at the heart of everything the Trust does.

Care is planned and written with patient involvement and there is clear evidence of plans made throughout each patient’s journey from admission to discharge.”

The folders benefit both patients and staff by ensuring excellent record keeping on all the Trust’s wards.
