Rothercare

community alarm service enables you to live safely and independently in your own home.

Your community alarm can be used from your home to call for help and advice 24 hours a day, 365 days a year and is available to any adult living in Rotherham.

Rothercare also work closely with Assistive Technology to provide a dynamic service to support your independence.

To make things easier for you we have set up a first point of contact for all our services called single point of access team. If you think that you may need help from the Council, you can ask us for advice, a social care assessment or any safeguarding advice.

Welcome and

Introduction For most patients, being discharged from hospital to their home is straightforward. Most are unlikely to need the support of Social Services, though some may need some information or advice about social care.

However some people have new or ongoing health and social care needs which require a detailed assessment.

Staff from Health and Social Services will usually work together to plan and deliver services to support such people after discharge.

This guide explains some of these processes, and explains the role of Adult Social Services in the hospital discharge process.

Our hospital team Rotherham Council has a Supported Discharge Pathway Team based in Rotherham Hospital. This team is made up of a team manager, team leader, social workers , well being advisors, Independent living officers and support planners.

Do I need to be referred to the Supported discharge team?

Planning for discharge normally begins at the point, or even before, you are admitted to hospital. The discharge process will be coordinated by a member of the nursing team on the ward. Following an initial assessment by nursing and medical staff, you will be referred, with your consent, to the hospital social work team if you appear to have social care needs that require assessment prior

to your discharge. This is done by the ward sending a request for assessment notification (care act 2014) previously this was called a section 2. You can also give your consent to your carer who can ask for you to be referred to the supported discharged pathway team.

The assessment process If you agree to an assessment a member of supported discharge pathway team allocated to your particular ward will contact you. The assessment is not a test, but a way of working out what your needs are and which ones might be putting you at risk.

The assessment process will start with discussions with the ward staff, with you and your family/ carers. This process of information gathering and clarification of the situation will enable the member of the support discharge pathway team to work alongside you to: • identify your needs • establish your eligibility • give you information about services that could immediately support you to be discharged from hospital or support you in the longer term.

Depending on how complex your care needs are, other specialist assessments may also be required.

The referral process and time scales for assessments are outlined in the (care act 2014).

The length of time required to complete the process can vary depending on how complex your needs are.

In Adult and Housing Services, we aim to do what we can to help people to live independently and safely. We work within resources available to us, and therefore ensure that we prioritise the people who are in greatest need of help.

This booklet tells you more about assessments and eligibility criteria. It also tells you about what will happen after you have an assessment. This may be relevant to you if you are an adult, have a physical or sensory disability, have mental health problems, or have a learning disability.

Getting the most out of your assessment You might want to prepare for your assessment by jotting down anything you want to talk through with us.

What can you do for yourself?

Do you have any family or friends who are able to offer you support and with what?

What is difficult for you?

What kind of support are you having at the moment?

Are some days much easier than others?

If so, why do you think this is?

We know that there are some things which can be difficult or stressful to talk about, but please don't be embarrassed. We will understand your feelings and respect them.

We can provide access to an advocate to assist you with the process.

If your first language is not English, or if you use sign language, we can arrange for an interpreter.

Please let us know what you need.

More about the assessment Once the ward has asked for an assessment, one of our staff will come and visit you on the ward.

You can have a friend or relative with you at the assessment, if you wish.

The assessment is free and will involve discussions about; • What you are able to do for yourself • What you would like to achieve • What type of help you feel is needed • What difficulties you are having at the moment • How stressful and urgent you feel your situation is.

On the assessment form we write down everything we discussed and agreed with you during the assessment, and also anything we might have disagreed about.

You will get a copy of the completed assessment form to keep this will contain any contact numbers and agreed contingency plans, you may need following your discharge. If you have additional communication needs ie large print we can ensure you get any information from us in the correct format.

Following the assessment, we will give you information about support that may be available, and any charges involved.

Some of the questions we may also ask you 1) How much control you have over your own life, and how easy or difficult you find it to make decisions about the way you live 2) Your health and safety, including any risks to your mental health or wellbeing 3) Your daily routines, such as personal care and domestic chores, and how well you are able to manage them 4) How well you are able to involve yourself in family responsibilities, community life, and work or study.

How do we decide what help you receive?

National Eligibility Criteria Social care is not free for everyone. We have to balance the needs of adults in Rotherham with the limited amount of money we have.

We do this by giving help and support to the people who are most at risk.

We use guidance to decide who needs the most help, and what help and support we can give. The guidelines are called ‘Eligibility Criteria’, which were made by the Government.

In the ‘Eligibility Criteria’ guidelines the areas of risk are:(a) managing and maintaining nutrition; (b) maintaining personal hygiene; (c) managing toilet needs; (d) being appropriately clothed;

(e) being able to make use of the adult’s home safely; (f) maintaining a habitable home environment; (g) developing and maintaining family or other personal relationships; (h) accessing and engaging in work, training, education or volunteering; (i) making use of necessary facilities or services in the local community including public transport, and recreational facilities or services; and (j) carrying out any caring responsibilities the adult has for a child.

Once we have completed you’re assessment together we would look at how we can utilise support from friends and family and local services prior to looking at commissioning council services.

Assessments and carers If you have a carer - a relative or friend who provides you with unpaid support - we may want to talk to them about what support they are able and willing to give.

The care Act means carers may also be entitled to support with their caring role. If you have an identified carer, this would also be an appropriate time for them to receive a carer's assessment. Please ask us for more information on this.

Emergency carers scheme This service is free and is for all Adults.

If you are faced with an emergency of your own we can help you make the arrangements, so that the person you look after still receives the right level of care. we will discuss with you and record your emergency arrangements. this will include the names of 2 people if you have them (friends relatives, known to the cared for person) who we can contact If there is an emergency. If there is no one who can assist then we will meet, with as little disruption as possible, the cared for persons needs.

We will hold the information about you and the person you care for. This information is completely confidential. you will then be issued with a key ring, phone sticker and a credit card sized card containing our telephone number. These should be carried with you at all times.

In the event of an accident or emergency anyone finding the card will be able to contact us. staff will look up your carers emergency plan and put it into action.

Assistive Technology – What is it ?

Assistive technology is equipment or sensors which are specifically designed to help reduce the risks associated with independent living for vulnerable people. Some of these sensors are placed around the home and can detect environmental issues e.g. fires, floods others are used to detect a fall or when someone has left the property at an in-appropriate time of the day e.g. during the night.

Some of the equipment works with the Rothercare community alarm system. Rothercare is the Council's community alarm service which provides an emergency call handling service and mobile response service 24 hours a day 365 days per year.

Environmental package These four sensors generate an alert to Rothercare when the following are detected: • Heat detector • High temperature or low temperatures • Smoke • Water (floods) • Carbon Monoxide • Gas detector

Falls Package This package consists of a fall detector which is worn on the waist, this generates an alert when the person falls anywhere in the home or garden during the daytime.

A bed sensor can be provided, which is used during the night time. If the person gets out of bed during the night and does not return within a predetermined time it generates an alert which may indicate the person has had a fall.

Purposeful walking package A voice announcer plays a message to try to deter the person from leaving the property at inappropriate times of the day e.g. during the night time. If the person does exit the property an alert is generated to either Rothercare or a carer’s pager. This does require a family/carer responder. It is very useful for people with Dementia.

Carers package This package consists of a portable pager and possibly a vibrating pillow alerter, it informs an on site carer when a telecare alert is generated e.g. fall detector. Can also be used to alert a person with a hearing impairment of a telecare alert e.g. smoke alarm.

We can offer some items without the need for a social care assessment some of these include

A Magi plug – a simple devise to prevent bath/sink from overflowing.

Automated medication dispenser Equipment that automatically dispenses medication at the correct time of day, it provides an audible and visual alert to encourage the user to take their medication.

Orientation clock The orientation clock assists the user to orientate the day of the week and whether it is day or night.

Big Picture telephone This telephone has compartments where users can insert photographs of key people e.g. family members; once the telephone is programmed, the user would then press the picture of the person they wish to contact.

How can I get this equipment?

If you have a social work assessment as part of your discharge the allocated worker will be happy to discuss and consider any appropriate assistive technology as part of the assessment.

Enabling service Enabling is about helping you become more independent and improve your quality of life,it is different to traditional ‘homecare’ because it focusses on helping you regain your independence doing everyday tasks such as making a meal, getting out of bed, personal care, moving around. We will help you to do these things for yourself rather than someone else doing the tasks for you.

Intermediate care Intermediate Care is the name for support to help you build back your skills and enable you to live at home as independently as possible. In some cases hospital staff may recommend a period of rehabilitation in order to maximise your independence. A member of the Therapy Team

would assess you for this. An intermediate care service may be provided by community health and social care services to support you during this period. This service will not require input from the supported discharge pathway team, the ward will advise you about this service.

24 hour Care homes If it is felt that you can no longer manage at home even with support, the decision may be for you to move to a care home. You and your family will be involved in this decision. The assessment will identify your care needs. This will help you and your family to select a registered care home which will be able to meet your needs.

There are 3 categories of care homes.

• Residential care – provides accommodation meals care and support throughout the day and night.

• Nursing care – offers the same as residential care with the addition of 24 hour care by a qualified nurse. This will require a CHC assessment (see page 14 -16)

• Dementia care or EMI (Elderly Mentally Ill) care – which provide specialist extra care for people often due to dementia or other mental health diagnosis.

You and your family have the right to choose the care home you will move to.

During the care planning process you should be supported to identify a suitable care home which is registered to meet your needs and has a vacancy. Your social worker/social services officer can help you with this process. However if the home of your choice has no vacancy, you may have to accept a place in an alternative care home until a place in your home of choice is available.

Safeguarding adults at risk The Care Act places a legal requirement on all agencies to make sure that all adults (aged 18 and over) should be able to live without being harmed or at risk of abuse/harm.

AND

Adults who have care and support needs (visual impairment, hearing impairment, physical disability, physical ill health, learning disability, mental health issue etc) may be less able to protect themselves from harm and may struggle to share their worries.

Harm is defined in the Care Act as

Where a local authority has reasonable cause to suspect that an adult in its area (whether or not ordinarily resident there) – • has needs for care and support (whether or not the authority is meeting any of those needs) AND • is experiencing, or is at risk of, abuse or neglect,

• as a result of those needs is unable to protect himself or herself against the abuse or neglect or the risk of it.

Physical – for example hitting etc Sexual – for example - forcing adults to do sexual acts they don’t want to or can’t consent to (including rape,sexual assaults etc) Financial or Material – for example taking money or anything of value from adults etc Neglect and Acts of Omission – any action that restricts choice, causes ham or isolates people, for example not supporting them to get washed/ dressed etc

Discriminatory – It is a criminal offence to bully someone who has a disability or is “different”, or other associated hate crimes.

Psychological or Emotional – for example.

Threating to leave them alone or intimidating them etc.

Self Neglect - is any failure of an adult to take care of himself or herself that causes, or is reasonably likely to cause within a short period of time, serious physical, mental or emotional harm or substantial damage to or loss of assets.

Domestic Abuse – Any incident of threatening behaviour, violence or abuse (psychological, physical, sexual, financial or emotional) between adults who are or have been intimate partners or family members, regardless of gender or sexuality. Modern Slavery/Human Trafficking – The movement: recruitment, transportation, transfer, harbouring or receipt of people.

Organisational Abuse – Organisational abuse can typically occur in a care home, nursing home, acute hospital or in patients home setting. It can

be caused by the regimentation of residents or of the individuals that are supported and the rules and regimes can be oppressive and abusive.

Organisational abuse can also be in the forms of lack of choice, lack of privacy, lack of respect or lack of dignity.

What should I do if I think something is wrong?

• If you feel a crime has been committed, contact the police on 101, or in an emergency situation, call 999 immediately.

Paying for my care and support Social care, unlike health care, is not free for everyone. The financial assessment process helps us work out how much the council can pay towards your care and support needs, and what costs you will be responsible for.

If you are an adult with eligible needs and have assets above the capital threshold of £23,250, you will be responsible for meeting the full cost of your care and support. However, if you need help arranging services, we can do this with you and offer you advice and information.

If you do not have assets above the capital limit, you may still need to pay towards your care and support depending on your financial circumstances.

From April 2016 to April 2017, carers will not be charged for services meeting their eligible support needs. If the person being cared for has agreed to receive care services to meet the needs of their carer, they may need to complete a financial

assessment to determine how much, if anything, they will contribute towards those services.

How we work out what you must pay for your care and support Unless you are a carer, you will need to complete a financial assessment form. This will tell us about your financial situation so that we can work out if you will need to pay and if so, how much you pay towards your care and support services.

If it is clear that you have significant financial resources to use to pay for your care and support, and you do not wish to undergo a full financial assessment, we may offer a ‘light touch’ financial assessment.

If you do not agree with the light touch assessment, you can ask for a full financial assessment.

How much you will pay will depend on whether you are receiving care in a care home, your own home, or another setting.

In the following circumstances, the value of your main or only home will not be taken into account in the financial assessment:

be taken into account as income. However, any money you receive from employment is not taken into account.

• if you are receiving care in a setting that is not a care home

If you live in a care home, you will contribute most of your income towards the cost of your care home fees. However, you will always have at least £24.90 per week for personal expenses.

• if your stay in a care home is temporary • where you no longer occupy the property (e.g. you live in a care home) but: - your partner continues to live there - or a family member who is over 60 continues to live there - or a family member who is under 60 and incapacitated continues to live there - your child under the age of 18 continues to live there If you have savings between £14,250 and under the self-funding threshold of £23,250 you will be deemed to have an income of £1 per week for every £250 (or part thereof) you have above £14,250. This is called Tariff Income.

These capital thresholds, personal expenses allowances and minimum income levels are subject to change based on annual review and Government policy.

We will give you a copy of your financial assessment once it has been completed.

Direct payments Every local council must now offer people who need help to stay in their own home, money instead of arranging services for them. This is called direct payments.

These direct payments enable you to choose how you organise the help you need. Giving you money instead of services will give you greater control over your life, because you make the decisions about how your care is provided.

The vast majority of people getting care from the council have a right to receive a direct payment. The member of the support discharge pathway team who undertakes your assessment will discuss direct payments with you when they assess your care needs.

Discharge from Hospital Once all assessments are completed the social work staff will discuss with you what help may be available and decide with you how your care needs can best be met.

Your discharge should take place in a timely fashion. We will endeavour to give you a copy

of your community care assessment and support plan before leaving hospital. If this is not possible we will give you a discharge letter giving you an overview of your planned care and your community care assessment will follow in the post.

Problems or queries If you have queries or are experiencing difficulties relating to your discharge you can either contact the supported discharge pathway worker working with you or speak to the nurse caring for you on the ward.

Carers Centre – 01709 254809 Caring for someone is not always easy which is why the Council and its partners have all come together at 'Carers Corner' to provide you with all the support you need under one roof.

At Carers Corner you can get face to face advice - such as how to access benefits or legal advice, along with how to access the Carers Emergency Scheme and Carers Assessments.

Community Occupational Therapist Service – 01709 822330 The Community Occupational Therapy Team helps anyone who is elderly or disabled and is having problems with activities of daily living. You can refer yourself for help, or someone else can contact us on your behalf.

All contacts to the Community Occupational Therapy Service are taken by Rothercare Direct. They may be able to respond to your problem at the point of contact or they may pass your request to another agency, such as the Home Improvement Agency. If they are unable to do this, your request will be passed to the Community Occupational Therapy Team.

Rehabilitation into the community Rotherham Intermediate Care Centre (RICC) – 01709 514003 The Rotherham Intermediate Care Centre, located on Badsley Moor Lane, is a joint partnership between Rotherham Council and Rotherham Foundation Trust. The centre is set to provide new rehabilitation facilities within a day setting for the people of Rotherham.

The new centre will work as an intermediate care hub, bringing together therapists, specialist mental health workers and support workers who are providing services to promote and maximise independence following a patient's recent episode in hospital, change in functional abilities or a worsening of their long-term condition. The new service will provide activities which are tailor-made to a patient's own interests and abilities.

Paying for care and support What the Council will pay towards care home costs Following your assessment if a decision is made for you to move into residential care you will always have to contribute something towards the cost of your care.

Each year, the Council sets a maximum figure that we will pay towards your weekly care home costs.

The amounts vary depending on the type of care you receive. The current maximum weekly Council contribution is:

Private care home £401 for residential care £451 for nursing care £419 for residential dementia care £518 for nursing dementia care

Rotherham Council care home £550 for residential care The amount you pay is worked out using a national set of rules. The Council will pay the difference between your contribution and the fee for your place in the home, so long as the home doesn't charge more than we expect to pay.

The value of your former home will be included as

part of the financial assessment, however it should be disregarded if your spouse or partner lives there, or another relative lives there who is either over 60 years of age or receiving a disability benefit. Your home is also ignored if you enter a care home for a temporary stay. If your property is taken into account, you will usually have more than the limit for getting local authority assistance.

The local authority may assist you under the ‘deferred payments’ scheme agreement.

You will always be left with a weekly amount for your own use called a Personal Allowance. This amounts to £24.90 per week.

If you need nursing care the NHS will pay a contribution towards your nursing care, which is called Funded Nursing Care or Continuing Healthcare. The NHS will pay this money directly to the care home to cover the costs of providing nursing cover on a 24 hour, 7 days a week basis.

If you choose a care home that is more expensive than what the Council will pay, you will need to make arrangements for someone else to pay the difference. This is often called a third party contribution or a top up fee.

NHS continuing healthcare NHS continuing healthcare is a free package of care for people who have significant ongoing healthcare needs. It is arranged and funded by the NHS. You can receive NHS continuing healthcare in any setting outside hospital, including in your own home or in a care home.

If you receive care in your own home, the NHS will cover the cost of the support you need from health professionals and the cost of personal care which can include help with washing and getting dressed. If you receive NHS continuing healthcare in a care home, the NHS will also pay your care home fees.

Eligibility for NHS continuing healthcare When assessing your eligibility for NHS continuing healthcare, staff must follow certain processes.

You must be assessed by a team of healthcare professionals as having a ‘primary health need’ for care. This means that you need care primarily because of your health needs.

This is assessed by looking at: • the type and particular characteristics of your needs • how intense and severe they are • the complexity of your needs • how unpredictable your needs are; including any risks to you and others if adequate and timely care is not provided Eligibility is always based on these needs, rather than any particular diagnosis or condition.

When should I be considered for NHS continuing healthcare?

If you have ongoing health needs, there are times when staff should consider whether you may be eligible.

These include: • when you are ready to be discharged from hospital and your long-term needs are clear • once a period of intermediate care or rehabilitation following a hospital stay have finished and it’s agreed your condition is unlikely to improve • whenever your health or social care needs are being reviewed as part of a community care assessment • if your physical or mental health deteriorates significantly and your current level of care seems inadequate • when your nursing needs are being reviewed. This should happen annually if you live in a nursing home • if you have a rapidly deteriorating condition and may be approaching the end of your life In these circumstances, your discharge staff, staff co-ordinating your intermediate care, GP or a member of the social work team should raise the issue of NHS continuing healthcare and assess your eligibility. If they don’t, make sure you ask for an assessment.

Assessment for NHS continuing healthcare You should be fully involved in the assessment and have your views taken into account. You can ask a relative or carer to help and support you throughout the process.

Most people will start by completing a checklist. This assesses your needs and indicates whether or not you should have a full assessment.

If you’re not eligible for NHS continuing healthcare, you’ll be referred to your local authority to decide whether you’re eligible for help to arrange or pay for care.

If you’re referred for a full assessment, evidence will be collected from all relevant health and social care professionals to give a full picture of your physical, mental health and social care needs.

Then a team of health and social care professionals involved in your care will look at this evidence and recommend whether or not you’re eligible. You and / or your representative can attend and participate at this meeting.

If you are eligible The team will forward their recommendation to the Clinical Commissioning Group (CCG) that is responsible for funding your care. The CCG will then discuss and agree a care and support package with you.

This care and support package will be reviewed after 3 months and then at least every year. If your care needs change, your funding arrangements may also change.

Personal Health Budgets If you’re eligible for NHS continuing healthcare, you have the right to a Personal Health Budget. Personal Health Budgets allow more choice and control over the services and care you receive and can be used to pay for a wider range of items and services, including therapies, personal care and equipment.

If you choose this, you will develop a care plan with your NHS team that will meet your personal health and wellbeing needs.

If you’re not eligible

However, you may be eligible for NHS-funded nursing care.

If you’re not happy with the decision If you’re turned down, you should be sent a letter telling you why. It should also tell you what steps to take and who to contact if you’re unhappy with the decision.

• If you weren’t given a full assessment, you can ask the CCG to reconsider your case. If you are still dissatisfied you can use the NHS complaints system to pursue your case.

• If you’re not happy after a full assessment, you can ask for the CCG to reconsider its decision.

If you are still dissatisfied, you can ask for an independent review of your case.

• If you’re not happy with any help you receive from the NHS, you have a right to complain.

Fast Track If you are approaching the end of your life, you may be eligible for ‘fast tracking’ which allows you to bypass the full assessment process mentioned above. You must have a rapidly deteriorating condition that may be entering a terminal phase.

If you’re eligible, a care package should be put in place as soon as possible, preferably within 48 hours.

What is NHS-funded nursing care?

NHS-funded nursing care is care provided by a registered nurse for people who live in a care home.

The NHS will pay a flat rate contribution directly to the care home towards the cost of this registered nursing care.

Who is eligible for NHS-funded nursing care?

You may be eligible for NHS-funded nursing care if: • you are not eligible for NHS continuing healthcare but have been assessed as needing care from a registered nurse • you live in a care home registered to provide nursing care

How will my needs be assessed?

You should be assessed for NHS continuing healthcare before a decision is made about whether you are eligible for NHS-funded nursing care.

Most people don’t need a separate assessment for NHS-funded nursing care. However, if you do need an assessment or you haven’t already had one, your clinical commissioning group (CCG) can arrange an assessment for you.

Outcome of the assessment If you’re eligible for NHS-funded nursing care, the NHS will arrange and fund nursing care provided by registered nurses employed by the care home.

Services provided by a registered nurse can include planning, supervising and monitoring nursing and healthcare tasks, as well as direct nursing care.

If you’re not eligible for NHS-funded nursing care and you don’t agree with the decision about your eligibility, you can ask your CCG to review the decision.

What is the rate of payment for NHSfunded nursing care?

NHS-funded nursing care is paid at the same rate across England. For 2015/16, the rate is £112.00 a week (standard rate).

Before October 1 2007, there were three different levels or bands of payment for NHS-funded nursing care – low, medium and high.

If you moved into a care home before October 1 2007, and you were on the low or medium bands, you would have been transferred to the standard rate from that date.

If you moved into a care home before October 1 2007, and you were on the high band, NHS-funded nursing care is paid at a higher rate. For 2015/16, the higher rate is £154.14 a week. You’re entitled to continue on this rate unless: • you no longer have nursing needs • you no longer live in a care home that provides nursing • your nursing needs have reduced and you’re no longer eligible for the high band, when you would change to the standard rate of £112.00 a week, or • you become entitled to NHS continuing healthcare instead Content supplied by:

The Motability Scheme The Motability scheme allows some disabled people getting DLA, PIP or War Pensioners’ Mobility Supplement to obtain a car, powered wheelchair or scooter.

The scheme uses some or all of your mobility benefit payments to cover the cost of “contract hire” or “hire purchase” of an appropriate vehicle. You can also pay an extra amount of money if you want a more expensive vehicle.

With hire purchase, the price of the car will be agreed directly with the Motability dealer, and you will own the vehicle outright at the end of the agreement.

With contract hire, you won’t own a vehicle, but you will get a new car every three years, full insurance for

the driver and passengers, servicing, maintenance and repairs, vehicle excise duty, replacement tyres and breakdown cover.

Adaptations to the car can be made if needed, although there may be an extra charge for some adaptations. Under the Motability scheme, cars can also be adapted for people in wheelchairs.

If you don’t need or want a car, you can transfer your allowance to lease a scooter or powered wheelchair.

Care services in your home If you need help around the home, a good option is to have a care worker come in to your home to help you.

Types of care and support Care and support comes in many forms and has many names used to describe it, including home help, care attendants and “carers” (not to be confused with unpaid family or friends who care for you).

Care and support can suit you if you need: • personal care, such as washing or dressing • housekeeping or domestic work, such as vacuuming • cooking or preparing meals • nursing and health care • companionship Care and support can be very flexible, in order to meet your needs, and the same person or agency may be able to provide some or all of these options for the duration of your care: • long-term 24-hour care • short breaks for an unpaid family carer • emergency care • day care • sessions ranging from 15-minute visits to 24-hour assistance and everything in between If you already know what you want, you can search NHS Choices directories for: • local care and support services and agencies • a list of national care and support organisations • services that can help you stay safe and well in your home on a long-term basis; these services, often known as “supported living services”, can include financial, help with medication,

advocacy, social and practical support •a place to live in a family who will care for you, known as “shared lives services” or adult placement services

If you believe that you might benefit from some help at home, the first thing to do is to contact your social care department to ask for an assessment of your care and support needs. To contact social care, go to GOV.UK: find your local authority.

If you are eligible for care and support services, the local authority may provide or arrange the help themselves. Alternatively, you can arrange your own care, funded by the local authority, through direct payments or a personal budget.

If you have chosen direct payments or a personal budget, or you aren’t eligible for local authority help and want to get care privately, you can arrange it in several different ways.

• train their care and support workers through national qualifications and servicespecific training • replace workers when they are ill, on holiday or resign • put things right when they go wrong An agency will want to see you and the person you’re looking after so that they can assess your needs. This also means that a joint decision can be made about the most appropriate type of care and support.

Hiring a personal assistant

Independent care and support agencies

You can hire a “personal assistant” to act as a care and support worker for you.

If you use an independent care and support agency, you or the person you’re looking after has to find the care agency and pay them.

Personal assistants can offer you all that you’ll get from an agency worker, but you’ll also get the continuity, familiarity and ongoing relationship with your assistant.

The agency will provide a service through a trained team of care workers, which means you may not always have the same person visiting your home, although the agency will do its best to take your choices into account.

Independent care and support providers are regulated by the Care Quality Commission (CQC). Care and support agencies must meet CQC’s national minimum standards and regulations in areas such as training and record-keeping. The CQC has the power to inspect agencies and enforce standards.

Care and support agencies must vet care and support workers before engaging them by taking up references and carrying out Disclosure and Barring Service (DBS) checks on potential employees. Care and support agencies can also: • take over the burden of being an employer – for example, payroll, training, disciplinary issues and insurance

However, if you employ a personal assistant, you will then have the legal responsibility of an employer. This will include arranging cover for their illness and holidays.

Employing a care worker on a private basis If you employ a care worker privately, you will not be obliged to use the DBS scheme, but you can use it if you choose to. You need to ask social care or the police to make the checks on your behalf. The care worker must have already applied to be vetted, and must consent to the check.

If you have concerns about the suitability of someone you employ privately as a personal assistant, you can ask Adult Social Care to look into the matter.

A care home will be a home for you or your loved one. Residents should be treated as individuals with their likes and dislikes taken into account.

Think about whether a home is close enough to family, friends, and community facilities.

Singing for the Brain An innovative project providing structured musical stimulation for people with dementia and their carers.

Singing is an enjoyable activity and provides a way for people with dementia to express themselves and socialise with others in a fun, supportive group. Groups run once a month.

Memory Café Anyone who has dementia or is caring for a person with dementia is welcome to attend the monthly Cafés.

Each Café is staffed by Alzheimer’s Society workers and volunteers. Staff from partnership agencies including Social Workers, Community Psychiatric Nurses and

Carers Resilience Service Carers Resilience is a new integrated service: Crossroads Care and Alzheimer’s Society will work together to provide Link Workers for all GP Practices across Rotherham. It works to build confidence and resilience by offering: a full assessment of the needs of the carers by a Dementia Adviser; fast track to immediate short term in-home respite care where necessary; support for carers for up to one month; signposting on to other services as required.

How will it work?

All members of Alzheimer’s Society receive a monthly national magazine. The more members we have, the greater our lobbying power. Membership is free to carers and people with dementia.

We will train and develop a group of volunteers to spread the word about what dementia is and what action individuals can take in their community. We will then ask them to recruit Dementia Friends who will then receive a basic information session on dementia and be encouraged to take action.
