ROYAL VISIT TO OPEN

As the temperature drops and the dark nights approach, we are getting ready at The Countess to face the seasonal winter pressures and our busiest time of the year.

So it is a good opportunity to look back on the summer and what I can only describe as the brightness of the last few months, including a visit from HRH The Duchess of Cornwall, Countess of Chester.

It is a proud and rare moment to be a Chief Executive of a hospital and have the privilege to introduce royalty to our patients and so many colleagues from both our Chester and Ellesmere Port hospitals. What clearly came across during the visit was an overwhelming sense of collective staff pride and celebration. You could feel it… you could see it… and it created a powerful and lasting impression. One that I personally will remember for a lifetime. For those of you that couldn’t be with us on the day, you can read about it and much more in this latest edition of Countess Matters. Enjoy.

HE Countess of Chester NHS Foundation Trust welcomed a special visit from HRH The Duchess of Cornwall, Countess of Chester to officially open and tour its new Haygarth Building.

The £15m wing at the rear of the hospital estate houses a flagship 21-bed intensive care unit – all single rooms, overnight relative facilities, a modern endoscopy facility with world class decontamination services and additional outpatient clinic rooms.

The building has been named after John Haygarth an important 18th century physician who spent the majority of his career working in Chester. He was known as one of the best doctors of his time, after discovering new ways to prevent the spread of fever among patients and reduce the mortality rate of smallpox. Following his death in 1827, a Haygarth medal was established for the best nurse in the Chester Royal Infirmary Hospital. In the 30th anniversary year of The Countess of Chester Hospital this tradition of awarding the Haygarth Medal has been re-established to coincide with the official opening of the Haygarth Building.

The opening of the Haygarth Building marks an exciting and new chapter for the future of this hospital, as we can now provide some of the most modern intensive care and endoscopy facilities anywhere in the country. We are thrilled to have the opportunity to showcase what we have achieved and meet with the Duchess of Cornwall, Countess of Chester.”

‘Infinite Care’ commemorative sculpture now in place A COMMEMORATIVE sculpture called ‘Infinite Care’ has been unveiled to mark the Royal visit and the Trust anniversary year. The sculpture has been made possible thanks to the generosity and contributions of local innovation company USP Creative and IHP building contractors for the new wing.

This modern artwork will remain in the courtyard adjacent to the Haygarth Building. It is lit at night as a source of comfort to those family members who may be caring for patients on the intensive care unit that part overlooks the courtyard. The design is based on the concept of the circle of life, and features the words patients have most commonly used throughout the 2014

John Haygarth was an important 18th-century British doctor who discovered new ways to prevent the spread of fever among patients and reduce the mortality rate of smallpox.

Haygarth spent 30 years at Chester and became known as one of the best physicians of his time. He was particularly interested in the treatment of fever patients and the prevention of smallpox. In 1778 Haygarth helped found the Smallpox Society of Chester; the group advocated inoculation, an unpopular position at the time, and tried to educate so as to avoid casual contraction of the disease. Only four years after this effort began, Chester’s smallpox mortality rate had been reduced by almost 50%. Soon other towns, such as Leeds and Liverpool, adopted the Society’s methods. For his important work on smallpox, Haygarth was elected as a Fellow of the Royal Society in 1781. When Haygarth died in 1827 award at the Chester Infirmary was named after him and a Haygarth medal was established for the best nurse in the hospital.

The intention is that by having the words scribed on the inside of the circle, it creates an intrigue that encourages people to explore the feature.

It has become a talking point among colleagues at the Trust, with opinions varying from it looking like a polo mint to requests for a version in pin badges to support staff recognition.

ian Williams has worked in a variety of roles within the NHS since 1978. She is passionate about patient care, particularly care of the older person and those with dementia. She loves nursing and feels very honoured to be part of peoples’ lives at their most vulnerable of times. Sian is an advocate in ensuring that the public perception of the profession is enhanced and restored.

Prior to taking up her current post as the

Deputy Director of Nursing and Quality Sian has held a variety of roles at the Countess of Chester Hospital over the last 20 years, including Head of Nursing, Senior Diabetes Specialist Nurse, Ward Manager and Senior Sister. She has also served a three year term of office as a staff governor on the Trust Council of Governors.

Living in Wirral she is married to husband Chris, and they will be celebrating their 30th wedding anniversary next year. Both Sian’s grown up children have followed in her footsteps to also have careers within

the NHS. Her family think she is ‘married to the job’ but believe she will never tire of trying to make improvements to patient care.

Reacting to the accolade Sian said: “It is a huge recognition not just for me but for all of the nursing team and their achievements at The Countess. I have so much pride and allegiance to my profession and I want to leave nursing as I found it – a fantastic and honourable role that invites you into peoples’ lives to care for them at the most vulnerable of times.”

NHANCING care and comfort tasks delivered by nursing staff has inspired the introduction of new care and comfort assistant roles at the Trust.

This is a non-nursing role purely to add value to the patient experience by being available to spend time talking with patients and helping them, for example

walking alongside patients who have been told by therapists to mobilise, but who are reluctant to do so unless someone is with them.

The key aims of the role are to release clinical time for registered and nonregistered nursing staff to focus on clinical patient need, enhance patient comfort and care and improve patient experience and satisfaction with care overall.

OUNTESS patients are being asked to volunteer to take part in a diary project to record first hand their experiences of care in the hospital. Alternatively if you are interested in finding out how you can be a patient blogger contact countess.feedback@nhs.net My appointment was what I hoped was going to be the last in a “forced” relationship with The Countess of Chester Hospital since June of this year.

When I say “forced”, I meant that my association with hospital wards, doctor’s clinics and surgical rooms has never happened in my 42 years before this June. Attending a hospital for any reason isn’t something I have ever wanted to do, or hope not to do again for a long time.

The Countess of Chester or CoCH as it is known when you become a patient isn’t overwhelming, isn’t frightening, but can be a bit intimidating and confusing if you are a hospital newbie.

The appointment I had was for the “DR ELDERLY MEDICINE CONSULTANTS”, which I later found was the elderly clinic. Not really what I wanted to hear as a middle aged man and perhaps a computer error.

Outpatients can be intimidating. It was full of people at 4pm and very hot and there were a number of non-audible tannoy announcements asking patients to go in different directions, interjected by nurses appearing from rooms and also calling patients. You have to have your wits about you to listen up and as the directional signage is totally hidden from the waiting area.

What I liked was explanation and this is something I wanted more of during my recent visits. Nurses and Doctors assume that the patient knows a lot about why they are in hospital, where they are and what is happening or not happening. This isn’t often the case. Communication is key and if this had been as abundant as it had been during my most recent appointment my other touch points with the hospital would have felt more comfortable.

On my most recent visit the nurse explained why I was having my weight, height, BP taken and the doctor who I saw was calm, clear and engaging as he ran through my history and gave advice on what to do and why. I came away with no further questions and feeling content.

The downsides to my most recent trip include the general outpatient experience in the waiting area, including the reception desk and then the wait again for the doctor in a very narrow corridor culde-sac with three other people. It made you feel as if you were on top of each other.

I would always recommend going with someone else if you can as an outpatient and have a friend or relative close by when the doctor is giving advice.

I have been in and out of the hospital since June and applaud the hard work that the nursing staff and doctors do on a daily basis. I have spent a few nights on AMU (which I later found was Acute Medical Unit) and one night on the stroke ward. Getting Information from staff is vitally important when you are new to all of this. The Trust, don’t assume the patient knows what you know and try and put yourself in their shoes.

I would never criticise the hard work the NHS staff of today put into their profession but I sometimes think, and am guilty of this myself, that you can easily become numb to what the patient feels and needs and doesn’t know whilst you go about your job.

I would always recommend going with someone else if you can as an outpatient and have a friend or relative close by when the doctor is giving advice. Quite often there is a lot to take in and you can’t digest all the information. There have been a few times when I have forgotten to ask the right questions or thought of something after the event.

■ Was I looked after? Yes. ■ Was I kept informed? Sometimes No. ■ Would I go back? Well I wouldn’t have much of a choice, but would never feel worried about being a patient at the hospital.

■ Could the experience have been better? Yes, but hopefully these blogs will help!

THERE’S NEVER A DULL MOMENT in hospital fundraising…

T

HE HOSPITAL fundraising team has had a busy three months with zip wire challenges, duck races, coffee mornings, sponsored walks and Tough Mudder events, and as winter approaches the office is being taken over by Santa suits for the annual city centre Santa Dash.

Staff and patients regularly take part in events and fundraising activities for the charity – and not just for the Babygrow Appeal, despite this being the main focus at the moment.

The charity has recently raised more than £10,000 to create a Centenary Garden at Ellesmere Port Hospital.

The Bothered and Bewildered play at The Forum in October was a great success, raising funds to purchase equipment and support the wellbeing of patients with dementia at both The Countess and Ellesmere Port hospitals.

To mark October being Breast Cancer Awareness month the fundraising team has championed an inspirational cancer survivorship story, fronted by a former patient Marion – who lives in Little Sutton is almost 85 years old and had a mastectomy 24 years ago. She is the face of a charity campaign to raise £65,000 to purchase some state of the art vacuum assisted biopsy equipment for the Breast Care Unit.

preparing for a CARE QUALITY COMMISSION inspection

ARLY next year, the Countess of Chester Hospital NHS Foundation Trust will be visited by Care Quality Commission inspectors as part of a routine national inspection regime. Director of Nursing Alison Kelly sets out her views on what the Trust can expect from the visit and what we need to know…

“This is a new approach and won’t be like inspections we have had in the past. The inspectors come in large teams. They won’t just be clinicians – they will bring patients, third sector representatives and analysts. They build up a profile of us before they arrive from external sources such as our website, data that is published nationally, views on NHS Choices and other staff and patient feedback websites, local media features and published minutes from meetings.

They also run focus groups for patients in public venues to encourage local people to share their views about The Countess. They want to understand that we are clear and transparent.

“When the inspectors arrive on site they will want to get stories from our staff and patients. It is important that our workforce is open and honest and go about their day jobs as they would normally do, it is also an opportunity for staff to celebrate what they are proud of. As well as observing care the inspectors will be looking at other areas such as risk management, estates, record keeping and awareness of policies and procedures and how

to find them. The inspectors have a detailed document with prompts to assist them in their enquiries and will constantly be assessing how effective we are, how caring we are, how responsive we are, how safe we are and whether or not our organisation is a well-led.

Following the inspection we will receive a report with both an overall rating and ratings for a range of areas - emergency care, care of the elderly, surgery, critical care, maternity, outpatients, end of life and children’s services.

This will help us understand the areas that are good but also the areas that require improvement, it will support our action planning going forward.

“We have started workshops with staff and patient involvement to plan for the visit which could be anytime in the next 12 months. We are doing are own clinical rounds to assess how safe it is on wards. We are setting up teams to test the methodology of what would be expected on a mini review as well as reviewing reports from other organisation’s’ inspections to help us learn and prepare.

We want to aim for the best from these inspections, but we also acknowledge that we are not perfect and have more to do and this is a valuable learning opportunity at the same time.”

More than 115 additional spaces have been created across a range of locations at the hospital to support both visitors and staff. NATIONAL AWARDS SUCCESS THE Countess of Chester has been shortlisted for its best practice and leadership across a range of national Health Service Journal (HSJ) awards. The Trust has been recognised for its work in value and improvement in procurement and in workforce efficiency, following the introduction of the care and comfort worker role. At the same time the leadership team has been shortlisted for Board of the Year and Tony Chambers for Chief Executive of the Year.

OPEN AND HONEST INITIATIVE sees new web reporting

T

he Countess of Chester NHS Foundation Trust has recently begun to publish information on their staffing levels, patient feedback and harms as part of a new Open and Honest initiative, pioneered by NHS England.

It is hoped that the initiative will help ensure every patient gets high-quality care and help to build improved services for the future.

As part of the Trust’s ongoing pledge to be open and transparent, the nursing team jumped on board and began collating

the necessary statistics which paint the picture of what care looks like within the Chester and Ellesmere Port hospitals. All stats are published mid-month on the Trust’s website http://www.coch.nhs.uk/ openandhonestcare/ The page presents staffing data for the previous month providing an insight into each ward’s staffing capacity and informs the public of whether a particular ward is under or over staffed.

Publishing such data is a first in the history of Countess but Sian Williams, Deputy Director of Nursing and Quality

believes that the data should be accessible to the general public.

Deputy director of nursing Sian Williams said: “Staffing data helps the nursing team effectively adjust ward staffing levels to suit demand. We pride ourselves on our reactive nature and monitoring these levels daily helps us to ensure we are as close to 100% Trust staffing level as possible.”

This data is supported by patient experience feedback scores via the Friends and Family Test and a summary of harms which have occurred monthly.

Cancer survey results PATIENTS have rated The Countess in an independent survey as among the top performing trusts in the country for the quality of its cancer care. The annual survey involves patients being asked a series of questions covering a range of stages in their journey, from diagnostics to hospital treatment and after care.

Those receiving care at The Countess of

Chester rated 32 of the 70 indicators with scores placing local services as among the top 20 per cent in the country, with particular good practice recognised for information made available to patients about their cancer. The results were based

on feedback from 205 adult patients at the close of last year. The full survey is available to view on line at the Quality Health website www.quality-health.co.uk

parliamentary review

The leadership challenge facing this hospital is how we swiftly move from delivering good patient care to consistently delivering great patient care. To do this we have to call entirely on our own resources. We need our clinicians in partnership with managers to ignite a sense of collective ownership and individual responsibility. We want to showcase the district general hospital as the jewel in the crown of the NHS, and not the ‘stone in the shoe’ as it has come to be perceived.

In the future Accident and Emergency will no longer act as the front door to the hospital, but become the back door of community services.

Our acute care hub

will be the centre of clinical expertise, fast diagnostics, multidisciplinary assessment, treatment planning and rapid transition to other health and social care environments. If we get this right 80% of people will be seen, assessed and discharged within 48 hours.

The principle is to focus on the needs of the patient and to have a more tailored health service response. Patients with complex needs will have skilled and experienced nursing staff to care for them.

Patients with more generic needs will be managed through high flow facilities with staff trained to manage their different care needs. Specialist nursing input to the ambulatory and short stay areas will be within easy reach from teams based in complex ward areas with staffing levels adjusted to reflect this.

As patients arrive in our accident and emergency service, those who can walk in through the front door are directed to an enhanced ambulatory care unit, already operating six days a week and reducing pressures in accident and emergency.

Hospital at home services have expanded significantly and are providing a rapid discharge option for more than 100 patients at any one time. This is at least three times more cost effective than treatment being provided in a bed and we have reduced the hospital acute bed pool. By front-loading investment in state of the art diagnostics including an additional MRI scanner, a brand new

endoscopy suite, and expanding CT scanner capability we can move to a position of treatment planning more rapidly.

We believe in the direction of the Royal College of Physicians as set out in The Future Hospital Commission and will work with our generalist and specialist physicians as well as our surgeons to understand how we adopt it across the local health system. Meanwhile our medical director is leading conversations with the University of Chester and local health and social care partners to pioneer new models of education that address the generalist medical skills shortages. A formal collaborative agreement has been signed by both organisations establishing an academic institute for integrated health and social care research hosted by the Trust.

If we end up in five years with more patients cared for at home, or near where they live; if we achieve a service where the NHS, social services, and the voluntary sector are consistently working closely with families and patients to help them remain within their communities; then we will have made lives better for thousands of people. If we provide emergency, urgent and specialist care quickly, efficiently and more safely in a smaller better organised hospital; if patient outcomes are the best possible; then we will have improved on what we are doing now.

The representatives selected are used as an example of best practice to leaders in the public and private sector. NEW LINKS WITH UNIVERSITY

T

HE MEDICAL director at The Countess of Chester is leading discussions with the University of Chester to set up a new joint venture, The Centre for Integrated Healthcare Science.

The facility is expected to be based at Bache Hall on The Countess of Chester Health Park site. Principle activities for the Centre will be education and training to support integration through post graduate medical teaching, research and expansion of participation in clinical trials, and the creation

of an innovation space for our health and social care colleagues to come together. In addition the joint venture to explore the feasibility of developing an undergraduate medical school which will pioneer new models of medical training to meet the needs of our populations across Cheshire, Shropshire and North Wales.

A formal collaborative agreement has been signed by both organisations to get the project off the ground.

Ian Harvey explained: “The local health system is facing major challenges in dealing with an increasingly aging population at the same time as developing integrated healthcare, managing workforce issues and addressing financial pressures.

“Our relationship with Chester University has always been good, but more recently we have shifted our thinking and vision towards one of the biggest opportunities we have had since opening the hospital 30 years ago, in terms of reputation and sustainability for the future. The whole aim is to integrate three areas: research, innovation and education, and encourage cross fertilisation of ideas.

“There is more work to be done, but this project is starting to move forward quickly and governance structures are being set up. Watch this space! We want our patients, staff and healthcare partners to be involved and make a contribution to this development.”

West Cheshire Care Record A COLLABORATIVE project is underway to support improved and secure sharing of information between health and social care professionals, with patient approval.

The development of the local West Cheshire Care Record is a partnership between NHS West Cheshire Clinical Commissioning Group, Countess of Chester Hospital NHS Foundation Trust, Cheshire and Wirral Partnership NHS Foundation Trust, Clatterbridge Cancer Centre

NHS Foundation Trust and Cheshire West and Chester Council.

Medical Director at The Countess of Chester Ian Harvey explained: “We hope to start piloting use of the record over the coming months, initially in Accident and Emergency. Information will only be accessed by health and social care professionals if a patient has given consent.

Timely access to a person’s health and social care records will ensure that GPs, hospital doctors, nurses, social workers and other health and social care professionals have an overview of the patient’s care in order to make the best decisions about diagnosis, treatment and care plans.”
