Hospitals are the right place to be when you are in need of acute medical or surgical care.

However, when your treatment is completed it’s important your stay is not delayed for any reason. It is beneficial to your health to start planning your discharge from hospital at the earliest opportunity, which may be when you arrive or even before if your admission to hospital is for elective surgery.

The right place for your care United Lincolnshire Hospitals NHS Trust (ULHT) provides acute hospital care across three main hospital sites at Lincoln, Boston and Grantham. These services are for people who require emergency, medical or surgical treatment within a hospital environment. When your treatment has been completed it’s important that your stay is not delayed for the following reasons: • S ome people find it harder to return home the longer they stay in hospital.

• P atients tell us they prefer to be at home in their familiar surroundings and this is often the best place for recovery.

• T here is a risk of acquiring infections in hospital, so leaving at the earliest opportunity means this is less likely.

•H ospital beds are needed for people who are very unwell, and who may be waiting in the emergency department for a bed to become available.

• P eople awaiting surgery, both urgent and non-urgent, may have their operations cancelled if a bed is unavailable.

Our commitment to you • Patients should receive the right treatment, at the right place and by the right professional.

• We place a high priority on keeping your stay in an acute hospital bed to a minimum.

• Once your consultant led team assesses that you are medically fit for transfer, we will aim to transfer you from hospital on the same day.

• You will be transferred from a ULHT hospital when you are ready to leave hospital as there are more appropriate services and facilities available to you.

The assessment process Your assessment begins as you are admitted to the ward and is a way of working out what your ongoing needs will be.

Nurses looking after you on the ward will have discussions with you and your family or carer. We would like to know if you were receiving support from community health or social care before you were admitted, or whether there are any issues that may impact on your discharge from hospital.

Leaving hospital You will be transferred from hospital when your consultant-led team decides that you are clinically ready and that you no longer require acute hospital services.

All staff will work with you and your carer or relatives as a team to discuss your discharge with you. Being discharged home is often very straightforward but if you have ongoing health and social care needs you may require a more detailed assessment. If this is the case, you will be supported by staff working together from both health and social care. We need to discuss any concerns or needs you have at the earliest opportunity, for example the things that you want to be able to do when you go home. These may include managing your personal care, domestic tasks, carer responsibilities, returning to work or leisure, staying safe and any equipment or care you feel you may require.

You will be given an expected date for discharge based on when your treatment is due to be complete. It is important that everyone involved is aware of this date so that we can all work towards it and so the necessary arrangements can be made such as: • Transport home. Patients are normally expected to arrange their own transport, however if this is not possible please tell us as soon as possible. >>

• Suitable clothing and footwear (if you are not already using them in hospital).

• Access to a key to your property.

• Adequate basic food stocks.

• Adequate heating in your home.

• Adequate support for your carer or family members.

We aim for patients to be discharged in the morning but if this is not possible and appropriate for your needs you may be moved to the discharge lounge where this is available. The discharge lounge is a comfortable and safe place to wait and is staffed by nurses who will provide light refreshments throughout the day, as well as keeping you informed.

A supply of your current medication will be ordered from the hospital pharmacy to tide you over until you see your GP. They will be delivered to you and may take up to four hours.

If you have any questions about your medication please ask to speak to your pharmacist or a member of the nursing team.

Relatives or carers coming to collect you can meet you in the lounge or a nurse can escort you to the main reception area.

It is important to know that the majority of patients will be transferred home directly from hospital.

If you no longer require consultant led care and there is a delay in the start of your care package or preferred community based placement, you will be transferred to a bed within a non-hospital setting whilst this is arranged. We will only transfer you when the appropriate place has been identified and confirmed but this should be within 48 hours.

Your hospital team In the hospital you will find doctors, matron, ward nurses, occupational therapists, physiotherapists, pharmacists, dietitians and social workers and you may be referred to any of these as part of your care.

The relevant person will discuss with you and assess what you are able to do and what you might need more help with. They will then discuss with you what treatment, help or support is available to improve your independence. The therapists will discuss with the other members of the hospital team their findings and together with you and your family make a plan for your discharge from hospital.

Dementia support If you have dementia our hospital staff will provide support for you and your relatives and carers. You will receive a document called ‘All About Me’ which asks for details about your specific needs and personal preferences. This information will help our staff to deliver specific care needs during your stay in hospital.

Intermediate care It may be that you are medically well enough to leave hospital but are not yet at a level where you can manage at home without support. A period of further rehabilitation may be beneficial to you, this is called ‘intermediate care’.

This can be provided within your own home, community hospital or care home dependent on the assessment outcome. The aim is to support patients as they are discharged to reduce the length of time in hospital, to allow a more accurate assessment of ongoing care needs and to help re-learn daily living skills.

There is a multi-disciplinary team approach which comprises of social worker, community matron, therapists, GP, access coordinator (social care) and appropriate attendees related to your care programme.

We offer permanent care in addition to respite. Our small team of highly trained staff are dedicated in providing person centred care with their passionate and empathetic approach.

All food is freshly prepared and special diets can be catered for, we also encourage residents to take part in growing herbs and vegetables in our raised beds.

We provide daily activities in a safe and homely environment and also have organised day trips which family members and friends are welcome to join.

We also encourage residents to maintain their hobbies and interests enabling individuals to feel part of their community.

Scotts View has five single rooms three of which are en-suite with two ground floor rooms with French doors opening up onto the garden. Personal items can be accommodated to ensure individuals feel at home for permanent residents.

We also offer respite care where we work closely with informal carers, to ensure we promote independence and support for carers and the cared for. Families and friends are welcome to visit at any time of the day and encouraged to join in on family day events. Family and friends can also be accommodated if they live away.

We also enable families to holiday together yet have support for their relatives or friends where needed. Scotts View is connected to South Farm caravan park where we have disabled facilities and our care team can provide support with personal care and offer respite on a daily basis. NHS continuing healthcare NHS continuing healthcare is a free package of care for people who have significant ongoing healthcare needs. It is arranged and funded by the NHS. You can receive NHS continuing healthcare in any setting outside hospital, including in your own home or in a care home.

If you receive care in your own home, the NHS will cover the cost of the support you need from health professionals and the cost of personal care which can include help with washing and getting dressed. If you receive NHS continuing healthcare in a care home, the NHS will also pay your care home fees.

Eligibility for NHS continuing healthcare When assessing your eligibility for NHS continuing healthcare, staff must follow certain processes.

You must be assessed by a team of healthcare professionals as having a ‘primary health need’ for care.

This means that you need care primarily because of your health needs.

This is assessed by looking at: • The type and particular characteristics of your needs.

• How intense and severe they are.

• The complexity of your needs.

• How unpredictable your needs are; including any risks to you and others if adequate and timely care is not provided.

Eligibility is always based on these needs, rather than any particular diagnosis or condition.

When should I be considered for NHS continuing healthcare?

If you have ongoing health needs, there are times when staff should consider whether you may be eligible.

These include: •W hen you are ready to be discharged from hospital and your long-term needs are clear.

• Once a period of intermediate care or rehabilitation following a hospital stay have finished and it’s agreed your condition is unlikely to improve.

• Whenever your health or social care needs are being reviewed as part of a community care assessment.

• If your physical or mental health deteriorates significantly and your current level of care seems inadequate.

• When your nursing needs are being reviewed. This should happen annually if you live in a nursing home.

• If you have a rapidly deteriorating condition and may be approaching the end of your life.

In these circumstances, your discharge staff, staff co-ordinating your intermediate care, GP or a member of the social work team should raise the issue of NHS continuing healthcare and assess your eligibility. If they don’t, make sure you ask for an assessment.

Assessment for NHS continuing healthcare You should be fully involved in the assessment and have your views taken into account. You can ask a relative or carer to help and support you throughout the process.

If you’re not eligible

Most people will start by completing a checklist.

This assesses your needs and indicates whether or not you should have a full assessment.

However, you may be eligible for NHS-funded nursing care.

If you’re referred for a full assessment, evidence will be collected from all relevant health and social care professionals to give a full picture of your physical, mental health and social care needs.

If you’re turned down, you should be sent a letter telling you why. It should also tell you what steps to take and who to contact if you’re unhappy with the decision.

Then a team of health and social care professionals involved in your care will look at this evidence and recommend whether or not you’re eligible.

You and / or your representative can attend and participate at this meeting.

If you are eligible The team will forward their recommendation to the Clinical Commissioning Group (CCG) that is responsible for funding your care. The CCG will then discuss and agree a care and support package with you.

This care and support package will be reviewed after 3 months and then at least every year. If your care needs change, your funding arrangements may also change.

Personal Health Budgets If you’re eligible for NHS continuing healthcare, you have the right to a Personal Health Budget.

Personal Health Budgets allow more choice and control over the services and care you receive and can be used to pay for a wider range of items and services, including therapies, personal care and equipment.

If you choose this, you will develop a care plan with your NHS team that will meet your personal health and wellbeing needs.

If you’re not eligible for NHS continuing healthcare, you’ll be referred to your local authority to decide whether you’re eligible for help to arrange or pay for care.

If you’re not happy with the decision

• If you weren’t given a full assessment, you can ask the CCG to reconsider your case. If you are still dissatisfied you can use the NHS complaints system to pursue your case.

• If you’re not happy after a full assessment, you can ask for the CCG to reconsider its decision.

If you are still dissatisfied, you can ask for an independent review of your case.

• If you’re not happy with any help you receive from the NHS, you have a right to complain.

Details of the complaints procedure are available from the relevant organisation.

Fast Track If you are approaching the end of your life, you may be eligible for ‘fast tracking’ which allows you to bypass the full assessment process mentioned above. You must have a rapidly deteriorating condition that may be entering a terminal phase.

If you’re eligible, a care package should be put in place as soon as possible, preferably within 48 hours.

What is NHS-funded nursing care?

NHS-funded nursing care is care provided by a registered nurse for people who live in a care home. The NHS will pay a flat rate contribution directly to the care home towards the cost of this registered nursing care.

What is the rate of payment for NHSfunded nursing care?

Who is eligible for NHS-funded nursing care?

Before October 1 2007, there were three different levels or bands of payment for NHS-funded nursing care – low, medium and high.

You may be eligible for NHS-funded nursing care if: • You are not eligible for NHS continuing healthcare but have been assessed as needing care from a registered nurse.

• You live in a care home registered to provide nursing care.

How will my needs be assessed?

You should be assessed for NHS continuing healthcare before a decision is made about whether you are eligible for NHS-funded nursing care.

Most people don’t need a separate assessment for NHS-funded nursing care. However, if you do need an assessment or you haven’t already had one, your clinical commissioning group (CCG) can arrange an assessment for you.

Outcome of the assessment If you’re eligible for NHS-funded nursing care, the NHS will arrange and fund nursing care provided by registered nurses employed by the care home.

Services provided by a registered nurse can include planning, supervising and monitoring nursing and healthcare tasks, as well as direct nursing care.

If you’re not eligible for NHS-funded nursing care and you don’t agree with the decision about your eligibility, you can ask your CCG to review the decision.

NHS-funded nursing care is paid at the same rate across England. For 2015/16, the rate is £112.00 a week (standard rate).

If you moved into a care home before October 1 2007, and you were on the low or medium bands, you would have been transferred to the standard rate from that date.

If you moved into a care home before October 1 2007, and you were on the high band, NHS-funded nursing care is paid at a higher rate. For 2015/16, the higher rate is £154.14 a week. You’re entitled to continue on this rate unless: • You no longer have nursing needs.

• You no longer live in a care home that provides nursing.

• Your nursing needs have reduced and you’re no longer eligible for the high band, when you would change to the standard rate of £112.00 a week, or • You become entitled to NHS continuing healthcare instead.

Friends and family test When you receive care at any of the services provided by us, you will be given the opportunity to give your feedback by answering a simple question about your experience.

The results will provide a way for you to easily compare NHS hospitals so that you know where you and your family can get the best possible care.

The information will also give the NHS invaluable information on what patients think of services, which can be used to help make improvements if required.

How will it work?

When you are discharged you will either receive an automated phone call or text message survey and you will be asked to answer the following question: “How likely are you to recommend our services to friends and family if they needed similar care or treatment?”

You will be invited to respond to the question by choosing one of six options, ranging from ‘extremely likely’ to ‘extremely unlikely’.

It is really important to us that you tell us why you gave your answer, so please answer any follow-up questions.

Your answer will not be traced back to you, and your details will not be passed on to anyone, so please tell us exactly what you think. A member of your family or a friend is welcome to help you give your feedback to the question if you are unable to.

Responses to the surveys are free.

Do you have to respond to the question?

Your feedback is extremely important and will help to continually improve our services.

However, you do not have to respond to the question. You can opt out at any time by simply replying STOP when you receive the text message or by pressing # if you receive an automated call. You will be opted out from receiving any future survey unless you change your contact details.

How will the results be used?

We will gather the results and analyse them rapidly to see if any action is required. The NHS will publish the results and you will also be able to see results for other hospitals to see how they compare.

Where will the results be published?

The results will be published on the NHS Choices website (www.nhs.uk). We will also publish our results on our website (www.ulh.nhs.uk), displayed within the trust and also in our annual reports and quality accounts.

Does this replace the existing compliments or complaints procedure, or other forms of feedback used by the hospital?

No, you can still pass your compliments or complaints to your hospital in the normal way.

Hospitals can continue to use existing ways of gathering feedback, in addition to the Friends and Family Test.

Paying for permanent residential care Lots of people worry about paying for care for themselves or for a loved one. Most people will be expected to pay something towards the costs.

If your care is being organised by the local authority, then the main steps of the process are: 1. Your local authority will firstly do a care needs assessment to identify what help you need.

2. They will make recommendations about your needs and whether or not you need residential care – this is called a Care Plan.

3. They will then work out a budget to ensure you get what you need – this is called a Personal Budget.

4. Then, they will do a means test, also called a financial assessment, to work out how much you should pay towards your care home fees and how much they will cover.

If your care is arranged by the NHS or social services, you may not have to pay for some or all of the care. For more information, see our page on NHS continuing healthcare.

After the means test the local authority should give you a written record of their decision of what you will have to pay and what they will pay, and how they calculated it.

What does the means test look at?

Generally the local authority will pay the full amount to the home and collect from you the amount that you need to pay. This may be different if you are paying a ‘top-up’ fee.

The means test will look at your capital and income, such as your savings and your property. Certain types of income, such as money from certain benefits and pensions, may not be counted.

If you and someone else jointly hold capital, such as a savings account, it’ll be treated as divided equally between the two of you.

In some circumstances your home won’t be included in the means test, for example, if your partner still lives there.

What if I give away some of my money?

You may be thinking about giving away some of your savings, income or property to avoid higher care costs, and to give something to your relatives or charity, for example. If the council thinks that you have done this to avoid paying care fees they may still assess you as if you still had the money or property that you have given away. This is referred to as deprivation of assets.

What amount of capital will make me eligible to pay for my care?

What if I prefer a more expensive care home than the local authority will pay for?

If you’d prefer to live in a care home that costs more than the local authority would usually expect to pay, it can arrange this, provided that someone else is willing to meet the difference in cost. This is usually known as a top-up or third-party payment.

You could also choose a care home in another area to be closer to family or friends. Find out more about having choice of accommodation.

How do I pay my part of the fees?

What if I run out of money?

If you are paying fees yourself (called selffunding) and your capital goes down to under £23,250, the local authority may assist with funding.

You should request an assessment a few months before that happens. They should arrange one as soon as possible so you don’t have to use up your capital below that amount.

Paying for temporary care in a care home Local authority charging rules differ when a stay in a care home is temporary, rather than permanent.

Some people go into a care home on a temporary basis to give themselves or their carers a break‚ or while they are recuperating from an illness.

Others enter a home temporarily for a ‘trial’ period to decide whether they want to live permanently in that home.

Paying for care if you have a partner Being part of a couple can affect a care home resident’s eligibility for local authority assistance with care home fees and both partners’ eligibility for social security benefits during the period in care.

We have produced a factsheet that addresses this issue and includes sections on the valuation of jointly owned capital, temporary admissions and where both members of a couple go into a care home.

Personal budgets and direct payments - more control over your care Direct payments When your personal budget is paid directly to you this is called a Direct Payment. Direct Payments allow you to arrange your care as you wish.

There are rules about what you can and can’t use direct payments for. The money must be used to meet the eligible needs identified in your assessment.

You can use direct payments for anything that meets those needs, for example: • Employ your own care workers.

• Buy services from a voluntary or private agency.

• Buy equipment or pay for home adaptations.

• Purchase other types of support to meet your assessed needs.

• Get to a café to meet your friends, a place of worship, or other places that are important to you.

You can be personal and creative with your budget.

For example, if one of your assessed needs is help with shopping, you could pay for someone to help you go shopping yourself rather than just pay someone to do it for you.

You can’t use direct payments to: • Pay a spouse, civil-partner, live-in partner, or a close relative who lives in the same household as you to care for you (although there may be exceptions to this).

• Buy services from your local council.

• Pay for permanent care in a care home. You can, however, pay for a short stay of up to four consecutive weeks within any 12-month period if that meets an assessed need.

Not everyone will feel comfortable or capable of managing their direct payments themselves, and you don’t have to if you don’t want to. You can always choose the option for the council to arrange care for you, and you can still have a say in what will best meet your needs.

Patient advice and liaison service (PALS) Welcome to the Patient Advice and Liaison Service We know that it can be a worrying and confusing time when you or someone you know is unwell. If you have concerns or questions about any aspect of your care, or the service you receive, it is best to first speak with a member of staff involved with your/their care. If you find that staff cannot help you, or you are not comfortable speaking with them, you can contact PALS. PALS is a confidential, on-the-spot, advice and support service for patients, relatives and carers.

You can ask a friend or relative to contact us on your behalf but we must have your permission before we can discuss your personal circumstances with anyone else.

PALS act independently when handling patient and family concerns, liaising with staff, managers, or relevant organisations, to negotiate immediate or prompt solutions. If necessary we can also refer patients and families to specific

local or national based support agencies.

PALS will: • Help answer your questions about the Trust or your care.

• Offer advice and support if things go wrong.

• Respond to your concerns if you are unhappy about any aspect of your care.

• Provide information about our services.

• Listen to your suggestions for improving our services for patients and visitors.

• Pass on your compliments to staff.

• Provide advice on the NHS complaints procedure.

Our formal complaints process We aim to resolve all complaints to the satisfaction of the person making the complaint. This may be the patient or anyone else/advocacy organisation making a complaint on behalf of the patient. If the latter, the patient will be required to give consent.

Complaints should ideally be in writing, sent by either email or post (see below for all of our contact details). However, we are able to support those wishing to complain but are unable to put their complaint in writing for one reason or the other.

Upon receipt of your complaint, we will send you an acknowledgement within three working days.

You will also receive a phone call from one of our Deputy Directors/Senior Managers, to confirm the key issues you would like resolved, your preferred method of response (e.g. letter, meeting) and the timeframe (typically 25 working days. This may be longer or not as long, depending on the complexity of the case). You will also be given a contact name, direct telephone number and e-mail address, to make it easy for you to find out how the processing of your complaint is progressing or if you need to give us additional information.

Chaplaincy Welcome to the chaplaincy service The chaplaincy department has the responsibility of providing for the pastoral, religious and spiritual care of all patients, their relatives and carers, as well as the staff on all hospital sites. We are there for all people of any faith or none.

Pastoral and spiritual care is an integral part of the holistic care provided within our hospitals. If you would like a visit from one of the chaplains whilst you are in hospital, then either make a referral in advance of your admission to hospital or ask a member of your healthcare team on the ward to contact a chaplain on your behalf.

In addition, chapel and prayer room facilities are available for quiet prayer and reflection on each of the hospital sites. We are pleased to welcome people of all faiths and none, who wish to use these areas for personal times of quiet, prayer and reflection. These areas are open to all, day and night.

The Orders of St John Care Trust is the single biggest care provider for older people in Lincolnshire.

Independent inspections by the Care Quality Commission show the homes to be delivering high standards of care.

As a charitable Trust, we focus on people not profits. Any surplus goes straight into improving the health and wellbeing of our residents, including a wide range of activities within and outside the homes.

How to use your health services There are a range of healthcare services available to help you find the right expert care to meet your needs. Choosing the service most appropriate to your symptoms means you get the right treatment in the right place.

Care services in your home If you need help around the home, a good option is to have a care worker come in to your home to help you.

Types of care and support Care and support comes in many forms and has many names used to describe it, including home help, care attendants and “carers” (not to be confused with unpaid family or friends who care for you).

Care and support can suit you if you need: • Personal care, such as washing or dressing.

• Housekeeping or domestic work, such as vacuuming.

• Cooking or preparing meals.

If you have chosen direct payments or a personal budget, or you aren’t eligible for local authority help and want to get care privately, you can arrange it in several different ways.

• Companionship.

Independent care and support agencies

Care and support can be very flexible, in order to meet your needs, and the same person or agency may be able to provide some or all of these options for the duration of your care:

If you use an independent care and support agency, you or the person you’re looking after has to find the care agency and pay them.

• Long-term 24-hour care.

The agency will provide a service through a trained team of care workers, which means you may not always have the same person visiting your home, although the agency will do its best to take your choices into account.

Independent care and support providers are regulated by the Care Quality Commission (CQC).

Care and support agencies must meet CQC’s national minimum standards and regulations in areas such as training and record-keeping.

The CQC has the power to inspect agencies and enforce standards.

• Nursing and health care.

• Short breaks for an unpaid family carer.

• Emergency care.

• Day care.

• Sessions ranging from 15-minute visits to 24-hour assistance and everything in between.

• A list of national care and support organisations.

• Services that can help you stay safe and well in your home on a long-term basis; these services, often known as “supported living services”, can include financial, help with medication, advocacy, social and practical support.

• A place to live in a family who will care for you, known as “shared lives services” or adult placement services.

If you believe that you might benefit from some help at home, the first thing to do is to contact your social care department to ask for an assessment of your care and support needs. To contact social care, go to GOV.UK: find your local authority.

If you are eligible for care and support services, the local authority may provide or arrange the help themselves.

Alternatively, you can arrange your own care, funded by the local authority, through direct payments or a personal budget.

Care and support agencies must vet care and support workers before engaging them by taking up references and carrying out Disclosure and Barring Service (DBS) checks on potential employees. Care and support agencies can also: • Take over the burden of being an employer – for example, payroll, training, disciplinary issues and insurance.

• Train their care and support workers through national qualifications and service-specific training.

• Replace workers when they are ill, on holiday or resign.

• Put things right when they go wrong.

An agency will want to see you and the person you’re looking after so that they can assess your needs. This also means that a joint decision can be made about the most appropriate type of care and support.

Hiring a personal assistant You can hire a “personal assistant” to act as a care and support worker for you.

Personal assistants can offer you all that you’ll get from an agency worker, but you’ll also get the continuity, familiarity and ongoing relationship with your assistant.

However, if you employ a personal assistant, you will then have the legal responsibility of an employer. This will include arranging cover for their illness and holidays.

Employing a care worker on a private basis If you employ a care worker privately, you will not be obliged to use the DBS scheme, but you can use it if you choose to. You need to ask social care or the police to make the checks on your behalf. The care worker must have already applied to be vetted, and must consent to the check.

If you have concerns about the suitability of someone you employ privately as a personal assistant, you can ask Adult Social Care to look into the matter.

Helping and supporting you at difficult times Park Street Activity Centre A bespoke activity centre for older people, providing a range of facilities, activities, events, holidays and day trips. The centre helps older people to lead a full and active life and provides a relaxing and enjoyable social environment.

Ruckland Court and Eccleshare Court Day Centres The Social Outreach Centres now provide support for older people with low or moderate needs from within the North and South of the City and surrounding villages.

Weekly activities include baking, bowls, kurling, arts and crafts, bingo and miniature golf. Facilities include a shop, hair salon, coffee shop, foot health care practitioner, bathing and a restaurant providing three-course meals.

Adapted vehicles with a passenger lift provide a door-to-door transport service for older people with disabilities.

Dementia in Lincolnshire Marie Betts, Alzheimer’s Society Services Manager for Lincolnshire There are now around 11,000 people living with dementia in Lincolnshire and that number is continuing to rise. With that in mind, it’s increasingly important that people get access to good quality information, education, support and care to help them live their lives to the full.

Misunderstandings Like many medical conditions, there are a number of commonly held misunderstandings around dementia. For example, many people don’t understand the link between Alzheimer’s disease and dementia. Dementia is a term used to describe various brain disorders that have in common a loss of brain function such as loss of memory, mood changes, and problems with communication and reasoning.

The condition is most commonly caused by Alzheimer's disease, vascular dementia and dementia with Lewy bodies although there are dozens of causes. Dementia is progressive, which means the symptoms will gradually get worse but the rate at which it progresses varies greatly.

Many people also think of dementia as being something that just affects older people. Whilst it is true that your risk of developing dementia increases significantly with age, there are more than 40,000 people in the UK under the age of 65 that are living with the condition. In some instances, it can even affect people in their 30s and 40s, although this is relatively rare.

Whilst there is not yet a cure for dementia, there are treatments that improve life for many people and Alzheimer’s Society funds research to find new ways to make life more manageable and enjoyable for people living with dementia.

Local support Crucially, support is only available to those people who have been given a diagnosis of dementia and so can be pointed in the right direction. We believe there are thousands of people who are living with dementia without a diagnosis.

As well as missing out on the support we provide as a charity, it also means they are not

getting the financial help that comes with a diagnosis (such as a council tax discount) and the possibility of medication which makes a real difference to thousands of people.

Around Lincolnshire, Alzheimer’s Society runs services to support carers and people who are living with the condition. We have Dementia Cafes which meet in Grantham, Sleaford, Lincoln, Market Rasen, Gainsborough, Skegness and Boston. The cafés provide people with dementia and carers a platform to try out activities and talk openly about living with the condition within a relaxed and informal environment.

Our Side by Side service runs throughout the county and offers the perfect opportunity for volunteers to support people with dementia to continue doing the things they enjoy.

www.alzheimers.org.uk We have Singing for the Brain groups which meet in Grantham, Sleaford, Lincoln and Gainsborough.

These offer a social activity for people with dementia and their carers enabling them to express themselves and sing with others in a similar situation. We also have activity groups in Sleaford and Lincoln.

We also deliver the county wide Dementia Family Support Service. If you have a dementia diagnosis, or you care for a family member or friend with dementia, our specialist family support workers can provide advice on how to manage the condition and stay independent for as long as possible.

Symptoms The key to getting this valuable support begins with speaking to your GP if you are worried

about your memory, or that of a loved one.

But how can you tell if something which we all experience - like losing your car keys - is a sign of something serious?

Everyone is a little bit forgetful now and again, but when memory loss starts to interfere with your daily life it is important to get it checked out as soon as possible. Symptoms might include frequently forgetting the names of people, places, appointments and recent events. It could also involve mood swings and feeling sad, angry or frustrated at memory loss.

Other symptoms could include becoming more withdrawn, either due to a loss of confidence or due to difficulty in communicating. Prevention There are steps that we can all take to reduce the risk of developing dementia and part of Alzheimer’s Society work involves encouraging people to be ‘head strong’ by living a healthier lifestyle. Research suggests that smoking doubles your risk of dementia, whilst high blood pressure and high cholesterol increases your risk by between 1.5 and 3 times. Obesity is also a risk factor, whilst regular exercise and only consuming moderate levels of alcohol reduce the risk slightly.

Simple steps like eating less salt and more fruit and vegetables are shown to make a difference.

The Care Quality Commission (CQC) registers all care homes and home care agencies. You can find out which ones support specific groups of people, such as people with a learning disability or those living with dementia.

CQC’s Chief Inspector for Adult Social Care, Andrea Sutcliffe always uses ‘The Mum Test’: is a care home safe, caring, effective, responsive to people’s needs and well-led? In other words, is it good enough for my Mum (or anyone else I love and care for)?

Look for care homes and home care agencies where the staff involve people who use services and their families and carers, and treat individuals with compassion, kindness, dignity and respect.

Whether you are being cared for in your own home or in a residential setting, the staff looking after you need to be skilled, kind and supportive.

They should also be capable and confident in dealing with your particular needs. You should always feel that their support is helping you to live the life you want to.

A care home will be a home for you or your loved one. Residents should be treated as individuals with their likes and dislikes taken into account.

Think about whether a home is close enough to family, friends, and community facilities.

Look at how well-led and managed a home is. What does it have in place to ensure that it delivers high quality care? Does it promote meaningful activity and connect the home with the community?

If you or a loved one needs help with day-today care, you can contact your local council’s social services department. Our home is privately owned by a family with over 30 years experience in caring for the elderly.

We only have one home and an owner, with a unique approach to care. We believe each resident should be able to enjoy every day with good company and a smile on their face.

Entertainment is varied and planned by our staff who take pleasure participating and organising trips in our minibus.

Purposely designed to cater for the lifestyles and entertainment of our residents. Our location offers the benefit of easy trips into town. While family and staff have the convenience of a large car park and good public transport links.

We have abundant facilities for social recreation, comfortable lounges and large bedrooms, each with their own en-suite shower room.

Our staff are extraordinary people delivering outstanding care. Dedicated and focused on individual needs, with the residents at the heart of everything they do. After an extensive recruitment process, each staff member is managed, trained and supported to be professional, enthusiastic, jovial and courteous.
