1. What this book will tell you. The information enclosed will be of help if you: • are thinking of moving into long-term nursing or residential care. • need a short break (sometimes called respite care) in either a residential or a nursing home. • are seeking advice regarding support in your own home.

The information contained within the book will: • give advice on choosing a home best suited to you. • explain the difference between residential and nursing homes. • detail homes registered with the Care Quality Commission and which have contractual arrangements with Nottinghamshire Social Services. • explain how to go about getting care at home.

2. What is a Registered Care Home? Before any home can appear in this book, it must have been inspected and registered. Registrations are carried out by the Care Quality Commission. This body will register and inspect all homes that provide four or more places for residents. They ensure that all facilities and staffing meet National Standards. You have a right to see what inspectors have said about a home. Homes should be able to show you a copy of their report or you may obtain one from the relevant address below. Homes may be run by Social Care Departments, private profit making companies or charitable organisations. There may be different levels of fees according to the care offered.

3. Nursing and Residential Care Homes What is the difference? Nursing Homes These have to employ a qualified nurse to offer supervision of residents and to advise unqualified care staff caring for residents. The cover must be maintained 24 hours a day. These homes care for people who are extremely frail either in mental or physical health. For someone entering a nursing home, and being funded by Adult Social Care and Health Department, a designated nurse employed by the Health Service will determine the level of health care needed, and a financial contribution will be provided by the Health Service. The resident will, however, be required to contribute towards their board and personal care. For further information see the section on: Financial Support for Residential and Nursing Care.

Residential Homes These homes provide full personal care and board. They need to have staff appropriately qualified or experienced to look after residents. Sometimes, people in residential homes may deteriorate to the extent that they need supervision by a qualified nurse. They would then need to move to a Nursing Home.

Dual-Registered Homes Some care homes are registered to care for both residential and nursing residents within the same building. /6


4. Home Care and Extra-Care Housing Home Care is for people who are very frail and incapacitated but still wish to remain at home and maintain their independence for as long as possible. The Social Care Services Department may be able to offer a range of support including help with personal care, such as: • washing and dressing. • meal preparation or the delivery of a meal. • short breaks in care to give the person or their carer a rest. • a sitting service to keep someone company whilst a carer is out of the house. Advice on claiming benefits to pay for care can also be offered.

Extra-care Housing Bassetlaw Adult Social Care and Health, in partnership with Bassetlaw District Council, is able to offer 24-hour care within two war-den supervised complexes sited in Worksop and Harworth/Bircotes. Tenants of council flats in these complexes can be provided with help with personal care over a 24 hours a day, seven days a week period. For many people this is a chance to maintain independence in a safe environment without having to enter a residential care home.

5. Financial Support for Residential and Nursing Care To access help with funding your care through Adult Social Care and Health Department, you will need to have a self directed support assessment which will determine the level of care that Adult Social Care and Health Department think you need. A Worker will complete this assessment with you, your carers and family. They will explain the financial implications of entering care and ensure you have the right benefits to help you pay for care. As financial arrangements are complex and can change, it is difficult to tell you what the cost of your care will be. At the present time, if you have savings and property that are likely to be worth more than ÂŁ23,000 (at 24/07/09), you may have to pay the full cost of your care. It is always worthwhile checking with Adult Social Care and Health Department what the likely cost might be and information, in written form, can be provided to give you a clearer idea of costs. Unless you can afford to meet the full cost of your care, you should not arrange to give up your home and enter a residential or nursing home before getting authorisation from Adult Social Care and Health Department. 6. Deciding to go into Care You may be deciding to go into a home permanently or just for a short break. When you are deciding which home to choose, you may wish to consider the following points: • Is the home near to your family and friends so they are able to visit you? Or you may want a home closer to facilities such as local shops or a church. • C an the home in which you are interested provide the sort of care you want, and in the way you want it to be provided? • Think of all the facilities you will need for your personal care and make sure the home can provide them if you are going to live there permanently.

Some questions you may wish to ask about a home • Is it warm and welcoming on my first visit? • Do I like it? • Is there a lift to upper floors? • Is the available room too big for me? Or even too small? • Is it clean? • Can I have a single room or could I share it if I wished to? • Can I bring personal possessions with me, such as furniture, mementoes and personal, treasured items? • Can I stay in my room all day if I wish to? • Can I have a television in my room? • Can I have a personal telephone? /10


Other questions which might help you decide if its the place you'll feel comfortable In for a long time • Does the person in charge make me feel at home? • Do staff seem friendly and talk to me? • Do staff spend time talking to residents? • How do staff address me? Are they respectful and polite? • Will I be allowed to do things for myself if I wish to? • Can I keep my own doctor if he/she is able to visit me? • Can the home help me sort out hospital appointments? • Do the residents seem happy?

You can find out a lot about the home if you ask • Does the home have residents' meetings? • What is the food like? Will I have a choice? • Can I get drinks or snacks for myself? • Can I get up and go to bed when I like? • Can friends and family visit me whenever they wish to? • Can visitors stay overnight? • Can I go out when I like with friends or family? Or could staff take me out?

What restrictions does the home set? • Are there smoking/non-smoking areas? • Can I take charge of my own money if I wish? • Can I bring my pet dog or cat (or other) with me?

As far as activities are concerned • Can I follow hobbies or interests? • Are outings and holidays arranged by the home? • Can clergy visit me in the home? • Is there a library service? • Can I sit out in a garden?

7. Making a Decision Having thought about all these questions, you might decide a home is the one for you. You need to be clear whether you or going for a short stay or if this will become your permanent home.

Points to think about next are • Will I have a 'trial stay' in order for me to change my mind if I am not comfortable? • Could I have an overnight stay before moving in? • Are there additional charges for things such as hairdressing, laundry, chiropody and continence aids, or are they included in the overall price I pay as my contribution to care? • What happens if I become frailer or have difficulty standing and walking? Will the home be able to cope with my care? • What happens if the home cannot cope with my needs? • If I am away for a short while, such as in hospital, is my room kept for me? • Will anyone talk to me if I have a complaint or if I want to discuss my care arrangements?

8. Other Aspects of Care Third Party Contributions The cost of residential care can vary from home to home depending on the facilities it provides, its location and so on. If a home charges a fee higher than the Adult Social Care and Health Department would normally pay, then a “friend, relative or other person” may need to pay the extra amount. It cannot be paid by you, directly from your income. If this is going to present a difficulty, you need to get advice from your Social Worker.

Choice and Need The Adult Social Care and Health Department is required to meet your needs as indicated by the assessment completed about you by your Social Worker. If you choose to go into a home which charges above the standard rate acceptable to Adult Social Care and Health Department, you must find a way of meeting the difference if you are seeking funding help from the Adult Social Care and Health Department. This is when a Third Party Contribution will need to be organised.

Out of Authority Care If you wish to consider a care home not within the geographical boundaries of your Adult Social Care and Health Department, you need to ensure the home is registered with the Care Quality Commission. It must also have a contractual arrangement with the local Adult Social Care and Health Department in which it is situated.

Meals at Home (County Enterprise Foods) We have been producing and delivering high quality meals to the people of Nottinghamshire for over 24 years. Anyone who has reached retirement age or is a vulnerable adult can have the meals delivered to their door by our professional drivers. /12


The meals are delivered either hot to eat immediately or frozen to reheat whenever you require them. We offer over sixty meals covering many dietary needs. Frozen meals, selected by you are delivered on an agreed day. A minimum order of three and delivery is FREE. The amount and frequency of ordering is your choice. The meals come with a sweet course are reheated in a microwave, steamer or conventional oven. Hot meals, (main course and sweet course) selected to suit your dietary needs and likes, are delivered on the days of your choice. The service is available Monday to Friday with emergency service at weekends. In addition we deliver breakfast packs to reheat the following morning and tea-time snack packs. Breakfast packs, delivered frozen, consists of bacon, sausage, tomatoes, scrambled eggs and sautĂŠed potatoes, fruit juice and a box of cereal. Tea-time packs contain a salad, bread roll, butter and a cake. Alternatively a sandwich, cake or crisps and fruit or fruit juice. Our drivers will assist with plating and cutting up the meal , fetching in milk, assisting you to the table or pouring a cold drink. Whether hot or frozen, a main meal of the day or the additional breakfast and tea time packs ALL are delivered to you for the price of ÂŁ3.95 per meal.
