Introduction Heart of England NHS Foundation Trust, which includes Heartlands Hospital, Solihull Hospital and Community Services, Good Hope Hospital in Sutton Coldfield and Birmingham Chest Clinic, is one of the largest Hospital Trusts in England. Last year for instance:

So when is a Hospital visit required?

An acute hospital Trust is a hospital that provides consultant-led health services within the National Health Service, such as Heartlands Hospital.

Acute hospital services are for people who require emergency, medical or surgical treatment within a hospital environment.

Patients should receive the right treatment at the right place and by the right

professional. Hospitals are the right place to be when you are in need of specific medical or surgical treatment.

However, we place a high priority on keeping your stay in an acute hospital bed to a minimum and once assessed and ready to leave, you will be transferred from hospital as there are more appropriate services and facilities available to you.

During your Hospital stay Dementia Support If you have dementia our hospital staff will provide support for you and your relatives and carers. You will receive a document called ‘About Me’ which asks for details about your specific needs and personal preferences. This information will help our staff to deliver specific care needs during your stay in hospital.

We also look to create an environment where we can that patients can feel safe and relaxed in during their time in hospital. On ward 30 at Heartlands Hospital for example, low level lighting has been introduced in the main reception area, the nursing station has been removed to create closer nursing relationships and the colour scheme on the ward has been chosen to create a relaxed ambiance.

We have arts and music sessions on the wards to encourage patients to take part in activities and socilaise with others and to feel comfortable in their surroundings.

Open visiting – an example of a patient first policy in action Back in April Heart of England NHS Foundation Trust introduced a policy of open visiting across all three of its hospitals, Heartlands, Good Hope and Solihull.

It was a major step for the Trust and was decided following a successful trial of extended visiting hours at Solihull Hospital and feedback from patients, relatives and staff.

It is widely recognised that support from family and friends, in the form of hospital visits, is an integral part of any patient’s recovery and open visiting allows more flexibility for relatives to visit their loved ones at a time that suits them.

Good for carers This new policy also forms part of the Trust’s aim to make its hospitals more dementia friendly by giving relatives and carers the chance to spend more time with their loved one and have an active role in their care while they are in hospital.

Campaign as being welcoming to carers.

John’s Campaign was launched in November 2014 after the death of Dr John Gerrard with the aim of giving the carers of those living with dementia the right to stay with them in hospital, in the same way that parents stay with their sick children.

Visitors Charter It was still important to emphasise that patients also need rest to help get them well as soon as possible and this could be a concern with the introduction of open visiting. Therefore the Trust has developed a Visitors Charter which it displays in each of its wards.

This is a code of conduct which visitors are asked to adhere to when they are on the wards, covering areas such as noise and hygiene.

The Trust has been seeking formal feedback from staff, patients and visitors to try and measure the success of the policy.

The Trust’s commitment to this has been recognised by John’s 5

Here to help If you or your family have any questions, concerns or wish to understand more about what is happening at any stage during your hospital stay, please speak to your nurse, doctor or other member of staff involved with your/their care. If you find that staff cannot help you, or you are not comfortable speaking with them, you can contact the patient services team.

Patient services is a confidential advice and support service for patients, relatives and carers. This team act independently when handling patient and family concerns, liaising with staff, managers, or relevant organisations, to negotiate immediate or prompt solutions. If necessary we can also refer patients and families to specific local or national

based support agencies.

Patient services will:• Help answer your questions about the Trust or your care • Offer advice and support if things go wrong • Respond to your concerns if you are unhappy about any aspect of your care • Provide information about our services • Listen to your suggestions for improving our services for patients and visitors • Pass on your compliments to staff • Provide advice on the NHS complaints procedure

Leaving Hospital When your treatment has been completed it’s important that your stay is not delayed for the following reasons: • Hospital beds are needed for people who are very unwell, and who may be waiting in the emergency department for a bed to become available.

• Patients tell us they prefer to be at home in their familiar surroundings and this is often the best place for recovery.

• There is a risk of acquiring infections in hospital, so leaving at the earliest opportunity means this is less likely.

• People awaiting surgery, both urgent and nonurgent, may have their operations cancelled if a bed is unavailable.

• Some people find it harder to return home the longer they stay in hospital.

All staff will work with you and your carer and/or family in order to discuss your discharge with you. We can discuss any concerns at the earliest opportunity such as your safety at home or mobility. You will be given an expected date for discharge based on when it is expected that your treatment will be completed.

It is important that everyone involved is aware of this date so that we can all work towards it and so the necessary arrangements can be made such as: • Transport home. Patients are normally expected to arrange their own transport, however if this is not possible please tell us as soon as possible.

• Suitable clothing and footwear (if you are not already using them in hospital).

• Access to a key to your property.

• Adequate basic food stocks.

• Adequate heating in your home.

• Adequate support for your carer or family members.

We aim for patients to be discharged as soon as possible on the day they are to be discharged, but if this is not possible, it is likely that you will move to our discharge lounge which enables us to prepare the bed for the next patient. The discharge lounges are located within our Hospitals and staffed by nurses, they are a comfortable and safe place to wait.

Relatives or carers coming to collect you can meet you in the discharge lounge or a nurse can escort you to the main reception area.

We understand that a hospital stay can be stressful and we want to support you and your relatives/carer during your time with us to help make your transition from hospital as smooth as possible when you no longer need our services.

For most people, being discharged from hospital is straightforward. But some people have ongoing health needs and staff will work together to plan and deliver services to support such people after discharge. See the piece about the Supported Integrated Discharge service which illustrates support in place for frail elderly patients to help them get home as quickly and safely as possible.

Supported Integrated Discharge (SID) – aiding the transition home Getting home as quickly as possible is what everyone wants as it is undoubtedly the best place to be for the patient.

However, with frail elderly patients it is vital that support is in place to ensure they are both ready and unlikely not to require a return to hospital within days of discharge.

Working together Working in partnership with its local councils, Heartlands and Solihull Hospitals have a Supported Integrated Discharge (SID) team in place which aims to help ease the transition from hospital back into the home for frail elderly patients seeking treatment for acute illnesses.

Award-winning service The service has been a resounding success, culminating with it being recognised at the Health Service Journal (HSJ) Awards 2014 in the Secondary Care Service Redesign category. Announcing the team as the winner, the judges praised the ‘significant joint working’ by the SID team and said it offered ‘outstanding integrated success’.

Better outcomes for patients Introduced in November 2012 at Heartlands Hospital, before being extended to Solihull a year later, the SID team is made up of hospital therapists, as well as social workers and carers from Birmingham City Council and the Promoting Independence Service from Solihull Council, who work together collaboratively to ensure patients regain their independence and health as quickly as possible.

Once it has been decided that a patient is well enough to return home, the team will discuss any ongoing care needs with the patient and ensure the right level of support is arranged. Physiotherapy and occupational therapy assessment and treatment are offered to promote recovery in the home, rather than on hospital wards.

As a result of introducing the programme, patients return home sooner and have a better overall experience through a more streamlined discharge and reablement process.

Staying well this winter It’s getting to that time of year again when the nights are drawing in and it’s getting cold. To help you stay warm and well during cold weather, please follow these top winter tips:

disease, you should heat your home to at least 18C and make sure you wear enough clothes to stay warm. It’s a good idea to keep your bedroom at this temperature all night.

• Draw your curtains at dusk and keep your doors closed to block out draughts.

• If you’re under 65 and healthy, you can safely have your house cooler than 18C, if you’re comfortable.

• Have regular hot drinks and eat at least one hot meal a day if possible. Eating regularly helps keep energy levels up during winter.

• The cold weather becomes a breeding ground for colds and flu and can often make people very sick.

Don’t forget that if you’re 65 and over, you are entitled to a free flu jab. Please contact your GP to find out how to get yours.

• Wear several light layers of warm clothes (rather than one chunky layer).

• Keep as active in your home as possible.

• If you have reduced mobility, are 65 or over, or have a health condition such as heart or lung

• If you are feeling poorly there’s a variety of health care services that offer advice about non-urgent medical problems such as your local pharmacist, GP or NHS walk-in centre.

Travelling in cold weather If you need to travel in icy weather this winter, remember that pavements and roads can be very slippery. Take extra care if you go out, and wear boots or shoes with good grip on the soles.

The Met Office advises putting grit or cat litter on paths and driveways to lessen the risk of slipping. If you’re travelling by car, you should also wait until the roads have been gritted.

Bear in mind that black ice on pavements or roads might not be clearly visible, and compacted snow may turn to ice and become slippery.

Are you entitled to help with your bills?

You may be entitled to help to pay your bills, so check if you are eligible for benefits such as the Winter Fuel Payment or Cold Weather Payment.

The Winter Fuel Payment could entitle you to between £100 and £300 tax-free to help pay your heating bills if you were born on or before 5 January 1953. Most payments are made automatically between November and December and you should get your money by Christmas. You usually get a Winter Fuel Payment automatically if you get the State Pension or another social security benefit (not Housing Benefit, Council Tax Reduction, Child Benefit or Universal Credit). If you qualify but don’t get paid automatically, you’ll need to make a claim.

To check if you are eligible for the Winter Fuel Payment, call 0345 915 1515.

You may also be entitled to a Cold Weather Payment if you’re getting certain benefits. Cold Weather Payments are made when your local temperature is either recorded as, or forecast to be, an average of zero degrees Celsius or below over seven consecutive days. If you’re eligible – this will be paid directly to you. You can find out more information about Cold Weather Payments by visiting www.gov.uk.

Birmingham Community Healthcare’s ‘outstanding’ end-of-life care Last year, the Care Quality Commission (CQC) rated BCHC’s end-of-life care both for adults and children as ‘outstanding’. As part of a continuing commitment to developing communitybased care of the highest quality, BCHC plans to recruit an end-of-life care clinical champion with a specific remit to focus on ways of improving the care and support offered in the final months of a patient’s life.

Medical director Dr Andy Wakeman said: “There is always more we can do to improve the quality of care we deliver to patients during their final days.

“The Trust recognises the considerable and increasing pressure that the divisions are under from the rising demand for these services in the community.”

Better Care: Healthier Communities 12

Birmingham Community Healthcare provides community nursing services from 8am to 10pm, seven days a week.

Thirty-six integrated teams of district nurses, occupational therapists, physiotherapists, clinical case managers and healthcare assistants provide a full range of domiciliary care citywide including general health and wellbeing support, management of long-term conditions, wound care, continence advice and end-of-life support for patients and their families.

If you, or someone you know, would benefit from community nursing and therapies support, speak to your GP or other healthcare professional caring for you for advice on services to which you may be entitled.

Compassion at the heart of community healthcare There were many hard days and nights for Linda Bridgewater and her family as her late husband Don’s health failed. But they took comfort from the quality and compassion of BCHC’s ‘outstanding’ end-of-life care.

The final days of a beloved relative are among the most testing that any family will experience.

As regular and reassuring visitors to the homes of families coming to terms with imminent bereavement, community nurses know that compassion is as important to their role as clinical expertise – particularly at times of loss.

When Don Bridgewater’s health began to fail, Birmingham Community Healthcare’s district nurses became a source of valued practical and emotional support, attending to his medical needs and giving his wife Linda whatever assistance she needed.

Above all, the number one priority was to try and provide the care Don needed for long-term complex health conditions in his and Linda’s own home in New Oscott for as long as possible.

District nurse Paula Bennett was just one of the nurses whose dedication to caring made such a difference to the Bridgewater family.

“Don, Linda and their family were a very solid unit, making sure he was well cared for at home,” said Paula.

“We would normally come in about once a fortnight to check they had all the support they needed and change Don’s dressings. And we also worked closely with our colleagues in the specialist heart failure team and the case management team to support the family’.

“The key to providing excellent care for people nearing the end of their life is to make sure their wishes, and those of their loved ones, are put ahead of everything else.

“So, the whole focus was on caring for Don at home as much as possible, in familiar surroundings with his family around him.“ A couple of stays in Good Hope Hospital and a week in John Taylor Hospice became necessary when Don’s health needed stabilising. And when the father of three and grandfather of eight finally passed away, Linda says the whole family took some comfort from the ring of care that

had been built around them.

Linda and Don met in 1966 when he was home on leave from military service with the Coldstream Guards.

During an armed forces career spanning two decades, Don completed three tours of duty in Northern Ireland at the height of the Troubles, was a keen rugby player and spent dozens of days and nights on sentry duty at Royal palaces, castle and residences.

“We met when he came home from leave with a friend who was going to set him up with his sister, a friend of mine,” remembers Linda, aged 66.

“I thought he was very good-looking as soon as I met him and so what’s a girl to do? I stepped straight in!

I lost a friend that day but it was worth it - I gained a wonderful husband.”

“It was so hard to see his health failing and so I was more than grateful for the support of all the nurses and the efforts they made to keep him at home with his family, which is all he wanted. He really looked forward to their visits, they really cheered him up.

“He was always so active and full of life and loved to spend time with the grandchildren, telling them his stories of when he looked after the Royal Family.

“Like the nights when Princess Margaret, his favourite Royal, would bring the guards at Windsor Castle a cup of hot chocolate late at night and chat to them. Or when the young Prince Edward would deliberately go in and out of the doors at Buckingham Palace so they had to keep presenting arms. He was a real patriot and very proud of his time in the army.”

“We all miss him terribly but it’s some comfort that he was so well cared for at the end of his life and that everything that happened with his care was focussed on what he wanted.”

NHS continuing healthcare NHS continuing healthcare is a free package of care for people who have significant ongoing healthcare needs. It is arranged and funded by the NHS. You can receive NHS continuing healthcare in any setting outside hospital, including in your own home or in a care home.

If you receive care in your own home, the NHS will cover the cost of the support you need from health professionals and the cost of personal care which can include help with washing and getting dressed. If you receive NHS continuing healthcare in a care home, the NHS will also pay your care home fees.

Eligibility for NHS continuing healthcare When assessing your eligibility for NHS continuing healthcare, staff must follow certain processes.

You must be assessed by a team of healthcare professionals as having a ‘primary health need’ for care. This means that you need care primarily because of your health needs.

This is assessed by looking at: • the type and particular characteristics of your needs • how intense and and severe they are • the complexity of your needs • how unpredictabe your needs are; including any risks to you and others if adequate and timely care is not provided Eligibility is always based on these needs, rather than any particular diagnosis or condition.

When should I be considered for NHS continuing healthcare?

If you have ongoing health needs, there are times when staff should consider whether you may be eligible.

These include: • when you are ready to be discharged from hospital and your long-term needs are clear • once a period of intermediate care or rehabilitation following a hospital stay have finished and it’s agreed your condition is unlikely to improve • whenever your health or social care needs are being reviewed as part of a community care assessment • if your physical or mental health deteriorates significantly and your current level of care seems inadequate • when your nursing needs are being reviewed. This should happen annually if you live in a nursing home • if you have a rapidly deteriorating condition and may be approaching the end of your life In these circumstances, your discharge staff, staff coordinating your intermediate care, GP or a member of the social work team should raise the issue of NHS continuing healthcare and assess your eligibility.

If they don’t, make sure you ask for an assessment.

Assessment for NHS continuing healthcare You should be fully involved in the assessment and have your views taken into account. You can ask a relative or carer to help and support you throughout the process.

Most people will start by completing a checklist. This assesses your needs and indicates whether or not you should have a full assessment.

If you’re referred for a full assessment, evidence will be collected from all relevant health and social care professionals to give a full picture of your physical, mental health and social care needs.

Then a team of health and social care professionals involved in your care will look at this evidence and recommend whether or not you’re eligible. You and / or your representative can attend and participate at this meeting.

If you are eligible The team will forward their recommendation to the Clinical Commissioning Group (CCG) that is responsible for funding your care. The CCG will then discuss and agree a care and support package with you.

This care and support package will be reviewed after 3 months and then at least every year. If your care needs change, your funding arrangements may also change.

Personal Health Budgets If you’re eligible for NHS continuing healthcare, you have the right to a Personal Health Budget. Personal Health Budgets allow more choice and control over the services and care you receive and can be used to pay for a wider range of items and services, including therapies, personal care and equipment.

If you choose this, you will develop a care plan with your NHS team that will meet your personal health and wellbeing needs.

If you’re not eligible If you’re not eligible for NHS continuing healthcare, you’ll be referred to your local authority to decide whether you’re eligible for help to arrange or pay for care.

However, you may be eligible for NHS-funded nursing care.

If you’re not happy with the decision If you’re turned down, you should be sent a letter telling you why. It should also tell you what steps to take and who to contact if you’re unhappy with the decision.

• If you weren’t given a full assessment, you can ask the CCG to reconsider your case. If you are still dissatisfied you can use the NHS complaints system to pursue your case.

• If you’re not happy after a full assessment, you can ask for the CCG to reconsider its decision. If you are still dissatisfied, you can ask for an independent review of your case.

• If you’re not happy with any help you receive from the NHS, you have a right to complain. Details of the complaints procedure are available from the relevant organisation.

Fast Track If you are approaching the end of your life, you may be eligible for ‘fast tracking’ which allows you to bypass the full assessment process mentioned above. You must have a rapidly deteriorating condition that may be entering a terminal phase.

If you’re eligible, a care package should be put in place as soon as possible, preferably within 48 hours.

What is NHS-funded nursing care?

NHS-funded nursing care is care provided by a registered nurse for people who live in a care home. The NHS will pay a flat rate contribution directly to the care home towards the cost of this registered nursing care.

Who is eligible for NHS-funded nursing care?

You may be eligible for NHS-funded nursing care if: • you are not eligible for NHS continuing healthcare but have been assessed as needing care from a registered nurse • you live in a care home registered to provide nursing care

Outcome of the assessment If you’re eligible for NHS-funded nursing care, the NHS will arrange and fund nursing care provided by registered nurses employed by the care home.

Services provided by a registered nurse can include planning, supervising and monitoring nursing and healthcare tasks, as well as direct nursing care.

If you’re not eligible for NHS-funded nursing care and you don’t agree with the decision about your eligibility, you can ask your CCG to review the decision.

What is the rate of payment for NHS-funded nursing care?

How will my needs be assessed?

NHS-funded nursing care is paid at the same rate across England. For 2015/16, the rate is £112.00 a week (standard rate).

You should be assessed for NHS continuing healthcare before a decision is made about whether you are eligible for NHS-funded nursing care.

Before October 1 2007, there were three different levels or bands of payment for NHS-funded nursing care – low, medium and high.

Most people don’t need a separate assessment for NHS-funded nursing care. However, if you do need an assessment or you haven’t already had one, your clinical commissioning group (CCG) can arrange an assessment for you. Find your local CCG.

If you moved into a care home before October 1 2007, and you were on the low or medium bands, you would have been transferred to the standard rate from that date.

If you moved into a care home before October 1 2007, and you were on the high band, NHS-funded nursing care is paid at a higher rate. For 2015/16, the higher rate is £154.14 a week. How to use your health services There are a range of healthcare services available to help you find the right expert care to meet your needs. Choosing the service most appropriate to your symptoms means you get the right treatment in the right place.

Paying for care What help is available?

Our factsheets explain the rules about local authority funding of care and explains what happens if your partner needs residential care in a care home.

Paying for permanent residential care Most people will be expected to pay something towards the costs of their accommodation and personal care from their income and capital.

If the local authority is involved in arranging your placement, the amount you will have to pay will be worked out via a means test, which is based on nationally set guidelines.

Your local authority will firstly do a care needs assessment to identify what help you need. Then they will carry out a means test to work out how much you have to pay towards your care home fees.

Currently, if you have assets of more than £23,250, you will need to pay the full cost of your care. Your home won’t be included in this if a partner or close relative still lives there. Your income is also taken into account.

From April 2015, your local authority must provide a personal budget showing how much they’ve calculated they should pay towards your 18

eligible needs (the ones identified in the care needs assessment).

The Care Act The Care Act is the biggest change in social care in more than 60 years. It is being introduced in two parts in April 2015 and April 2016. It will change the way the social care system works.

There will be a care cap from April 2016. This means no-one will spend more than £72,000 of their own money on their care needs. The amount that you and the council spend on your needs will be added up in your care account. Once this reaches £72,000, the council will pay for all your eligible needs.

From April 2016, the upper capital limit will also increase from £23,250 to £118,000.

Paying for temporary care in a care home Local authority charging rules differ when a stay in a care home is temporary, rather than permanent.

Some people go into a care home on a temporary basis to give themselves or their carers a break‚ or while they are recuperating from an illness.

Others enter a home temporarily for a ‘trial’ period to decide whether they want to live permanently in that home.

Temporary care may also be part of an ‘intermediate care’ package‚ which should be free of charge.

Paying for care if you have a partner Being part of a couple can affect a care home resident’s eligibility for local authority assistance with care home fees and both partners’ eligibility for social security benefits during the period in care.

At Age UK, we have produced a factsheet that addresses this issue and includes sections on the valuation of jointly owned capital,

temporary admissions and where both members of a couple go into a care home.

Government guidance on the rights of a spouse in a care home to pass 50% of their private or occupational pension to their partner living at home are also included.

Direct payments When your personal budget is paid directly to you this is called a Direct Payment. Direct Payments allow you to arrange your care as you wish.

There are rules about what you can and can’t use direct payments for. The money must be used to meet the eligible needs identified in your assessment.

You can use direct payments for anything that meets those needs, for example: • employ your own care workers • buy services from a voluntary or private agency • buy equipment or pay for home adaptations • purchase other types of support to meet your assessed needs • get to a café to meet your friends, a place of worship, or other places that are important to you.

You can be personal and creative with your budget. For example, if one of your assessed needs is help with shopping, you could pay for someone

to help you go shopping yourself rather than just pay someone to do it for you.

You can’t use direct payments to: • pay a spouse, civil-partner, live-in partner, or a close relative who lives in the same household as you to care for you (although there may be exceptions to this) • buy services from your local council • pay for permanent care in a care home. You can, however, pay for a short stay of up to four consecutive weeks within any 12-month period if that meets an assessed need.

Not everyone will feel comfortable or capable of managing their direct payments themselves, and you don’t have to if you don’t want to. You can always choose the option for the council to arrange care for you, and you can still have a say in what will best meet your needs.

Care services in your home If you need help around the home, a good option is to have a care worker come in to your home to help you.

Types of care and support Care and support comes in many forms and has many names used to describe it, including home help, care attendants and “carers” (not to be confused with unpaid family or friends who care for you).

Care and support can suit you if you need: • personal care, such as washing or dressing • housekeeping or domestic work, such as vacuuming • cooking or preparing meals • nursing and health care • companionship Care and support can be very flexible, in order to meet your needs, and the same person or agency may

be able to provide some or all of these options for the duration of your care: • long-term 24-hour care • short breaks for an unpaid family carer • emergency care • day care • sessions ranging from 15-minute visits to 24-hour assistance and everything in between If you already know what you want, you can search NHS Choices directories for: • local care and support services and agencies • a list of national care and support organisations • services that can help you stay safe and well in your home on a long-term basis; these services, often known as “supported living services”, can include financial, help with medication, advocacy, social and practical support • a place to live in a family who will care for you, known as “shared lives services” or adult placement services If you believe that you might benefit from some help at home, the first thing to do is to contact your social care department to ask for an assessment of your care and support needs. To contact social care, go to GOV.UK: find your local authority.

If you are eligible for care and support services, the local authority may provide or arrange the help themselves. Alternatively, you can arrange your own care, funded by the local authority, through direct payments or a personal budget.

If you have chosen direct payments or a personal budget, or you aren’t eligible for local authority help and want to get care privately, you can arrange it in several different ways.

Independent care and support agencies If you use an independent care and support agency, you or the person you’re looking after has to find the care agency and pay them.

The agency will provide a service through a trained team of care workers, which means you may not always have the same person visiting your 20

home, although the agency will do its best to take your choices into account. Independent care and support providers are regulated by the Care Quality Commission (CQC). Care and support agencies must meet CQC’s national minimum standards and regulations in areas such as training and recordkeeping. The CQC has the power to inspect agencies and enforce standards.

Care and support agencies must vet care and support workers before engaging them by taking up references and carrying out Disclosure and Barring Service (DBS) checks on potential employees. Care and support agencies can also: • take over the burden of being an employer – for example, payroll, training, disciplinary issues and insurance • train their care and support workers through national qualifications and service-specific training • replace workers when they are ill, on holiday

You can hire a “personal assistant” to act as a care and support worker for you.

An agency will want to see you and the person you’re looking after so that they can assess your needs. This also means that a joint decision can be made about the most appropriate type of care and support.

Hiring a personal assistant Personal assistants can offer you all that you’ll get from an agency worker, but you’ll also get the continuity, familiarity and ongoing relationship with your assistant.

However, if you employ a personal assistant, you will then have the legal responsibility of an employer. This will include arranging cover for their illness and holidays.

Employing a care worker on a private basis If you employ a care worker privately, you will not be obliged to use the DBS scheme, but you can use it if you choose to. You need to ask social care or the police to make the checks on your behalf. The care worker must have already applied to be vetted, and must consent to the check.

If you have concerns about the suitability of someone you employ privately as a personal assistant, you can ask Adult Social Care to look into the matter.

The Motability Scheme The Motability scheme allows some disabled people getting DLA, PIP or War Pensioners’ Mobility Supplement to obtain a car, powered wheelchair or scooter.

The scheme uses some or all of your mobility benefit payments to cover the cost of “contract hire” or “hire purchase” of an appropriate vehicle. You can also pay an extra amount of money if you want a more expensive vehicle.

With hire purchase, the price of the car will be agreed directly with the Motability dealer, and you will own the vehicle outright at the end of the agreement.

will get a new car every three years, full insurance for the driver and passengers, servicing, maintenance and repairs, vehicle excise duty, replacement tyres and breakdown cover.

Adaptations to the car can be made if needed, although there may be an extra charge for some adaptations.

Under the Motability scheme, cars can also be adapted for people in wheelchairs.

If you don’t need or want a car, you can transfer your allowance to lease a scooter or powered wheelchair.

The Care Quality Commission (CQC) registers all care homes and home care agencies. You can find out which ones support specific groups of people, such as people with a learning disability or those living with dementia.

CQC’s Chief Inspector for Adult Social Care, Andrea Sutcliffe always uses ‘The Mum Test’: is a care home safe, caring, effective, responsive to people’s needs and well-led? In other words, is it good enough for my Mum (or anyone else I love and care for)?

Look for care homes and home care agencies where the staff involve people who use services and their families and carers, and treat individuals with compassion, kindness, dignity and respect.

Whether you are being cared for in your own home or in a residential setting, the staff looking after you need to be skilled, kind and supportive.

They should also be capable and confident in dealing with your particular needs. You should always feel that their support is helping you to live the life you want to.

A care home will be a home for you or your loved one. Residents should be treated as individuals with their likes and dislikes taken into account.

Think about whether a home is close enough to family, friends, and community facilities.

Look at how well-led and managed a home is. What does it have in place to ensure that it delivers high quality care? Does it promote meaningful activity and connect the home with the community?

If you or a loved one needs help with day-today care, you can contact your local council’s social services department. If you would like to organise your care yourself, you can find a care worker or personal assistant through an agency. Your local social services department should be able to provide details of approved agencies.

Our team of advisors are there to provide expert advice and information including welfare benefit calculations, and help to complete forms. Handyfix We offer a citywide Handyfix service that covers the whole city. With one phone call to 0121 437 0033 people over 50 or their relatives, friends and carers can book a Handyperson to do those jobs around the house that need fixing.

Age UK Birmingham staff will agree whether “Handyfix” can help, what needs doing, how long it’s likely to take and then book a day and time that best suits the customer.

All the work will be carried out only by Age UK Birmingham Handyfix’s highly trained, reliable and trustworthy, in-house team of staff.

Support from Dementia Support Workers Dementia Support Workers can offer advice on accessing professional help. We are in contact with a wide range of services and other advice centres, so we can put you in touch with specialists who can help you if your concerns are outside of our expertise. We can also refer you, or the person you care for, to services which provide for people with dementia – particularly if you are having trouble accessing this yourself.

Home visits We can arrange for a Dementia Support Worker to visit you in your own home and provide information, advice and support.

Information We can provide a wide range of advice sheets, which include specific information on different types of dementia, care issues and understanding the needs of the person with dementia.

Dementia Cafés Are places for people with dementia, their families and friends to meet other people in a similar situation for support, information and social interaction.

Singing for the Brain Is a service provided by Alzheimer’s Society which uses singing to bring people together in a friendly and stimulating social environment.

Singing is not only an enjoyable activity – it can also provide a way for people with dementia, along with their carers, to express themselves and socialise with others in a fun and supportive group.

Dementia Friends Programme Dementia Friends is a national initiative run by the Alzheimer’s Society. Funded by the government, it aims to improve people’s understanding of dementia and its effects.

Through the Dementia Friends programme, we aim to change the way the nation thinks, talks and acts to create ‘dementia friendly’ communities.

How does it work?

A group of volunteers will be trained to spread the word about what dementia is and what action individuals can take in their community, and recruit Dementia Friends.

What is a Dementia Friend?

A Dementia Friend learns about what it is like to live with dementia and then turns that understanding into action.

What is a Dementia Friends Champion?

Dementia Friends Champions are volunteers who, after attending a training course, run information sessions where they talk to people about being a Dementia Friend in their communities.

Welcome to an exciting care home, designed to accommodate a variety of care needs in a pleasant and homely environment. This purpose built home is divided into independent units that can accommodate service users in either 10, 17 or 27 bed units. Each Unit has light and airy communal dining and sitting rooms providing ample choice for quiet and stimulating areas.

Berwood Court Care Home, located on Cadbury Drive, Castle Vale has established itself in the heart of the community and provides accommodation for 74 service users requiring care for both nursing and residential care needs. All bedrooms are single with en suite facilities and the home boasts 6 spacious lounge and dining rooms. All service users receive thorough assessments both prior to joining us and throughout their stay with us. The home ensures that all individual care plans are thoroughly Person Centered and that family and loved ones are as involved as they wish to be with care planning. Staff enjoy a pro active approach to their development and are supported with their learning needs, enabling all staff to achieve a minimum of NVQ level 2.

The home is equipped to enhance the quality of life of people with care needs due to dementia, whatever their age, however severe their symptoms.

Specially selected and trained staff ensures care is delivered through care planning involving the service user, relatives and the multi disciplinary team.
