What is Adults and Communities? Adults and Communities is part of Birmingham City Council. We used to be called Social Care and Health and, before that, Social Services. We provide services that help adults in Birmingham to live as independently as they can and to be part of their local community. You may need help in your day-to-day life because you, or someone you care for: • is vulnerable due to old age; • has a physical disability; • has a learning disability; • has a mental health difficulty; • is affected by HIV; or • is affected by drug or substance misuse. We have a range of services to support you to live as independently as possible. These include: • advice and guidance about services available to all adults, such as leisure, transport, health and education; • advice and support to help you live as independently as possible in your own home; • equipment to keep you safe and prevent you having a fall; and • personal care – for example helping you to eat meals, wash, shower, dress and use the toilet. This guide has information about all of these services, together with detailed information
about some of the other services we provide, such as home care, meals delivery services, adult education and extra care housing. The Adults and Communities Access Point (previously known as the Older Peoples Access Service), provides the single point of access for many of our services. We are able to quickly and accurately respond to your enquiries and help you to make informed choices about what services are available and the type of care and support that you need. Our phone number and email address are featured throughout this guide. There’s an explanation of how we assess your needs on page 6, together with details on page 32-33 of other organisations that can help you, or someone you care for. Assessing your needs To find out if you, or someone you care for, is eligible for our services, please call us on 0121 303 1234 as we will need to talk to you and assess your social care needs.

If we think you could be eligible, we will send you a selfassessment questionnaire form and arrange to visit you.
• We can give you all the money for your care and you can then arrange the support you need. This is called a direct payment.

What happens if I am not eligible? If you could be eligible we will visit you to assess your social care needs. If we cannot help you with your social care needs we will tell you about other organisations that may be able to help. These could be private and voluntary organisations and community groups.
What happens if I am eligible? If you are eligible, we will work with you to meet your social care needs. If you are not able to meet your social care needs yourself, we can help you to get services to support you to be as independent as possible.

Rehabilitation and Enablement Service If you are eligible for help and you are over 65, we will provide you with an enablement service for up to six weeks. This service is to help you regain old skills or learn new
skills that you need to live as independently as you can at home. These skills could include: • preparing a meal; • washing and dressing; and • doing the laundry or learning
how to move safely around your home to prevent you from having a fall. We will review your enablement plan after four weeks to see if you still need a service from us.

Support for adults with a mental health difficulty Mental health social care services for adults are provided by Birmingham City Council. If you are worried about your mental health you can talk to your GP. Our services include individual and group support to help you deal with your mental health difficulties. We support people to continue to live at home safely and independently.
How can we help you? We provide advice, assessment and support to meet your care needs if you have a serious or long term mental health difficulty. There is more information about how we assess your needs on page 6.

Support for older people Birmingham City Council provides social care help for people over 65 in Birmingham. We help identify the care services you need to live independently and make sure these are reviewed regularly. Services are arranged from Birmingham City Council as well as from independent care providers in Birmingham.

Support for adults with a physical disability or sensory impairment Our physical disability and sensory impairment service provides help for you if you have a physical disability or sensory impairment, for example a visual or hearing impairment.
How can we help you? Our role is to provide advice, assessment and care planning for people with a physical disability or sensory impairments, who are in need of support. We support people to continue to live at home safely and independently. There’s more information about how we assess your needs on page 6.

The Substance Misuse Recovery Service supports any adult aged 18 to 65 through a residential rehabilitation and after-care programme, to become free from dependence on alcohol or drugs. We may still provide a service to you if you are under or over these ages, subject to individual need and assessment.

Birmingham Adult Education Service Birmingham Adult Education Service is one of the largest providers of adult learning opportunities in the West Midlands. You may want to: • brush up on existing skills; • learn new ones to help you get a job; • improve your English or maths; • improve your health and fitness; or • learn about a subject that interests you.
How can we help you? Each year we run a wide range of part-time courses at our
main adult education centres and in many local community venues. These include courses to improve your maths and English skills, to find out about computers, learn a language or to work with children. We also offer classes in a variety of arts and creative crafts. Most of our courses are suitable for everyone, however we also run some courses especially for families, older learners and people with learning disabilities. Home Care and Night Care Enablement Services The Home Care and Night Care Enablement Services offer a range of personal care and practical support to help you live as independently as possible in your own home.
How can we help you? The Home Care Enablement Service We specialise in short term support, to help you regain independent living skills that you may have lost through ill health or a stay in hospital, or to learn new skills. We call this service enablement. We will also find out if you have longer term support needs and what these may be. Our service usually lasts for a maximum of six weeks The home care service is available between 7.00am and 10.00pm 7 days a week. There is no charge for this service.
The Night Care Enablement Service Night Care provides a range of short term support for up to 6 weeks at night to: • Support you to return home from hospital or a care home • Support you to avoid admission to hospital or care home (perhaps where your carer has become unwell, for example). • To help find out what longer term support needs you may have at night. Night care support could be: • A carer to stay through the night with you, or • carers to call at your home up to 3 times a night.

The Night Care Service is available between 9.15pm and 7.00am, 7 days a week. There is no charge for this service.
Our standards The Home Care and Night Care Services are registered services and regulated and inspected by the Care Quality Commission (CQC). Inspection reports which say how well we meet our standards are on the CQC website. For more details see page 32.

Community meals How can we help you? You can choose to have a lunchtime meal from a number of providers including voluntary and faith-based groups. The meals are normally provided at day centres or other similar services and offer you the opportunity to have a meal in social situation. Care homes and care homes with nursing Care homes are for people who have social care needs that can no longer be met at home. Care homes with nursing are for people who have health and social care needs that can no longer be met at home.
How can we help you? You can stay in a care home for a short-time
(known as respite care), over a longer period or permanently. Care homes usually provide accommodation, meals, and personal care. Personal care can include help with getting up or going to bed, eating, washing, dressing, and using the toilet. In some homes staff can help you to gain skills that help you to be independent. Care homes with nursing offer 24 hour nursing care by trained nursing staff. The person in charge of the nursing home must be a qualified doctor or nurse, and there will always be nursing staff on duty to care for you. Extra Care Housing Extra Care Housing provides older people with a range of options to help them live independently. It is flexible enough to meet changing needs and provide a home for life.
How can we help you? You will live in a good quality, modern, individual flat or bungalow with your own front door. There will be a range of community facilities such as a lounge, library, hairdresser, laundry, garden and assisted bathroom. Professional staff will offer you flexible and dedicated support, including onsite day and night care for people who are assessed as needing it. The design of the schemes and the services on offer mean that most people who choose Extra Care will not need to move into new accommodation if they become frail.
A wider choice The council is currently working with two private extra care housing developers to provide older people in the city with a wider choice of accommodation for rent, sale and shared ownership. In partnership with the Extracare Charitable Trust, the council is developing five retirement villages for Birmingham. In these schemes, accommodation is based around a village centre and a large range of health and leisure facilities are available for both residents and the wider community.
The first retirement village – at New Oscott – opened in 2010. A further village, at Pannel Croft, Newtown is due to open in 2012, with a third village, Hagley Road, Edgbaston opening in 2013, and a fourth village at Shaftmoor Lane, Acocks Green, set to open in 2015. Two more Extra Care developments – in partnership with Housing 21 – have opened over the past year: at Westhall Court, Yardley and Saxon Court, Turves Green.

Careline Careline is a 24-hour emergency contact service run by Birmingham City Council. It provides peace of mind and reassurance if you are an older person living in Birmingham and have a disability or feel vulnerable. In the event of an emergency at home, help is just a telephone call away.
How can we help you? You will be provided with an alarm unit that uses your existing telephone line to create a direct link to Careline. You can contact the Careline service at any time by simply pressing a pendant worn around your neck or a large red button on the alarm unit. The pendant can be clipped to clothing, worn on a cord around the neck or attached to a wrist strap. Pressing your alarm automatically identifies you to a Careline operator who can then call the emergency services or one of your ‘responders’ (people you have told us can help you in an emergency).
What does it cost? The charge for the service is ÂŁ1.76 per week. The charge includes all installation, maintenance and monitoring costs (as at June 2012).

Birmingham Telecare Service The Birmingham Telecare Service is a FREE 24-hour service that provides and monitors a range of equipment and sensors that you would either wear or keep in your home to help you stay independent.
What types of sensors and equipment are available? The service provides a range of sensors and equipment that suit your needs and help you stay in your own home. Examples include equipment that monitors: • your movement in and out of a room, a chair or your bed; • if you fall; • if temperatures fall or rise quickly in your home; • if smoke is detected; • if carbon monoxide is detected; • if there is a flood in your bathroom; and • if you have an epileptic seizure in bed. Equipment also available includes: • personal alarms that you can wear so you can call for help from anywhere in your home; • light-switch sensors to turn a light on if you get out of bed; • a bogus-caller button so you can call for help if you’re frightened by a stranger at your door; and • a medication dispenser that lights up and makes a noise to remind you to take your tablets.
Am I eligible for this service? You may be eligible if you are 18 or over, live at home and need help, or have a physical disability, a learning disability or a mental-health difficulty.
How can I get referred to the Birmingham Telecare Service? You can’t refer yourself to the service. To find out if you are eligible, you will need to have a Birmingham Telecare Service referral form filled in for you. You can do this by: • asking your social care worker, if you have one, to fill in a referral form; or • phoning the Adults and Communities Access Point on 0121 303 1234. If you phone the Adults and Communities Access Point you will be asked some questions to see if you are eligible to apply for this free service. If you are eligible, an operator will take your details and fill in the referral form for you over the phone.

Equipment Loans Service How can we help you? In partnership with the NHS, we provide a free loan equipment service to people over 18 years old and who are eligible for our support. We may be able to lend you equipment to make life easier, or carry out simple adaptations to your house, like fitting hand rails.

Safeguarding Adults Some adults are less able to protect themselves than others, as they are vulnerable because of age, illness, a disability or a mental health need. Safeguarding is about making people aware of their rights, protecting them, and preventing abuse. Abuse may be physical, verbal, sexual, financial or emotional. It may include not looking after someone properly, bullying or humiliating, or not allowing contact with friends and family. Anyone can be an abuser – relatives, partners, people paid to provide care and services, volunteers, neighbours, friends and strangers. Abuse can happen anywhere at anytime. People can be abused at home, in care or nursing homes, hospitals, day centres, or any place where an adult should be safe.
How can we help you? We, the Birmingham Safeguarding Adults Board, include service users and organisations such as the police, health trusts and the Department for Work and Pensions. We work together to share information to try to safeguard vulnerable adults from abuse.
